Aim: The aim of this study was to investigate the impact of war on children and comorbidity of post traumatic stress disorder, attention deficit with hyperactivity, conduct, and oppositional defiant disorder in Palest...Aim: The aim of this study was to investigate the impact of war on children and comorbidity of post traumatic stress disorder, attention deficit with hyperactivity, conduct, and oppositional defiant disorder in Palestinian children. Methods: The study was conducted in the entire Gaza Strip 6 months after the end of the war on Gaza that lasted for 23 days. The study sample included 410 children aged 6 to 17 years. Children completed measures of experience of traumatic events (Gaza Traumatic Checklist-War on Gaza), post traumatic stress disorder index, attention deficit with hyperactivity scale for parents and self, conduct disorder scale, and oppositional defiant disorder. Results: Palestinians children investigated 6 months after the war on Gaza still reported traumatic experiences: 94.6% heard the sonic sounds of the jet fighters, 91.7% heard shelling of the area by artillery, 92% watched mutilated bodies on television, 80% were deprived from water or electricity during the war, 50.7% said they left home for safer place. Using Diagnostic Manual of Mental Disorders-IV (DSM-IV) criteria for post traumatic stress disorder, 25.1% of children reported no psychological reactions, 25.9% of children reported one criteria, 39.3% of children reported partial post traumatic stress disorder and 9.8% of children reported full criteria for post traumatic stress disorder. According to the parents’ report, the results showed 31.3% of children met the criteria for inattentive type, 36.3% of children were impulsive and 29% met criteria for combined type. According to the children’s report, the results showed 28.8% of children met the criteria for inattentive type, 37.3% of children were impulsive and 28.3% met criteria for combined type. Using DSM-IV diagnostic criteria of conduct disorder and oppositional defiant disorder, the study showed that 38.1% of parents reported conduct disorder in their children and 46.3% reported oppositional defiant disorder. While 39.3% of children themselves reported conduct disorder and 44% of them reported oppositional defiant disorder. The study showed that 5.1% of children had comorbidity of post traumatic stress disorder and attention deficit disorder, 4.4% had comorbidity of post traumatic stress disorder and impulsivity-hyperactivity disorder, and 4.4% had comorbidity of post traumatic stress disorder and attention deficit with hyperactivity combined type. Also, 4.6% of children had comorbidity of conduct and post traumatic stress disorder and 6.1% had comorbidity of oppositional defiant disorder and post traumatic stress disorder. Conclusions: This study revealed that children living in areas of conflict and war are the main group at risk of developing post traumatic stress disorder and other behavioral problems including attention deficit with hyperactivity, conduct, and oppositional defiant disorder, and the internationallaws must protect the civilians during the conflict, establishing safe havens for children and their families to decrease the effect of war on children.展开更多

Advancements in Natural Language Processing and Deep Learning techniques have significantly pro-pelled the automation of Legal Judgment Prediction,achieving remarkable progress in legal research.Most of the existing r...Advancements in Natural Language Processing and Deep Learning techniques have significantly pro-pelled the automation of Legal Judgment Prediction,achieving remarkable progress in legal research.Most of the existing research works on Legal Judgment Prediction(LJP)use traditional optimization algorithms in deep learning techniques falling into local optimization.This research article focuses on using the modified Pelican Optimization method which mimics the collective behavior of Pelicans in the exploration and exploitation phase during cooperative food searching.Typically,the selection of search agents within a boundary is done randomly,which increases the time required to achieve global optimization.To address this,the proposed Chaotic Opposition Learning-based Pelican Optimization(COLPO)method incorporates the concept of Opposition-Based Learning combined with a chaotic cubic function,enabling deterministic selection of random numbers and reducing the number of iterations needed to reach global optimization.Also,the LJP approach in this work uses improved semantic similarity and entropy features to train a hybrid classifier combining Bi-GRU and Deep Maxout.The output scores are fused using improved score level fusion to boost prediction accuracy.The proposed COLPO method experiments with real-time Madras High Court criminal cases(Dataset 1)and the Supreme Court of India database(Dataset 2),and its performance is compared with nature-inspired algorithms such as Sparrow Search Algorithm(SSA),COOT,Spider Monkey Optimization(SMO),Pelican Optimization Algorithm(POA),as well as baseline classifier models and transformer neural networks.The results show that the proposed hybrid classifier with COLPO outperforms other cutting-edge LJP algorithms achieving 93.4%and 94.24%accuracy,respectively.展开更多

Cryptography is deemed to be the optimum strategy to secure the data privacy in which the data is encoded ahead of time before sharing it.Visual Secret Sharing(VSS)is an encryption method in which the secret message i...Cryptography is deemed to be the optimum strategy to secure the data privacy in which the data is encoded ahead of time before sharing it.Visual Secret Sharing(VSS)is an encryption method in which the secret message is split into at least two trivial images called’shares’to cover it.However,such message are always targeted by hackers or dishonest members who attempt to decrypt the message.This can be avoided by not uncovering the secret message without the universal share when it is presented and is typically taken care of,by the trusted party.Hence,in this paper,an optimal and secure double-layered secret image sharing scheme is proposed.The proposed share creation process contains two layers such as threshold-based secret sharing in the first layer and universal share based secret sharing in the second layer.In first layer,Genetic Algorithm(GA)is applied to find the optimal threshold value based on the randomness of the created shares.Then,in the second layer,a novel design of universal share-based secret share creation method is proposed.Finally,Opposition Whale Optimization Algorithm(OWOA)-based optimal key was generated for rectange block cipher to secure each share.This helped in producing high quality reconstruction images.The researcher achieved average experimental outcomes in terms of PSNR and MSE values equal to 55.154225 and 0.79365625 respectively.The average PSNRwas less(49.134475)and average MSE was high(1)in case of existing methods.展开更多
It is challenging for aqueous Zn-ion batteries(ZIBs)to achieve comparable low-temperature(low-T)performance due to the easy-frozen electrolyte and severe Zn dendrites.Herein,an aqueous electrolyte with a low freezing ...It is challenging for aqueous Zn-ion batteries(ZIBs)to achieve comparable low-temperature(low-T)performance due to the easy-frozen electrolyte and severe Zn dendrites.Herein,an aqueous electrolyte with a low freezing point and high ionic conductivity is proposed.Combined with molecular dynamics simulation and multi-scale interface analysis(time of flight secondary ion mass spectrometry threedimensional mapping and in-situ electrochemical impedance spectroscopy method),the temperature independence of the V_(2)O_(5)cathode and Zn anode is observed to be opposite.Surprisingly,dominated by the solvent structure of the designed electrolyte at low temperatures,vanadium dissolution/shuttle is significantly inhibited,and the zinc dendrites caused by this electrochemical crosstalk are greatly relieved,thus showing an abnormal temperature inversion effect.Through the disclosure and improvement of the above phenomena,the designed Zn||V_(2)O_(5)full cell delivers superior low-T performance,maintaining almost 99%capacity retention after 9500 cycles(working more than 2500 h)at-20°C.This work proposes a kind of electrolyte suitable for low-T ZIBs and reveals the inverse temperature dependence of the Zn anode,which might offer a novel perspective for the investigation of low-T aqueous battery systems.展开更多

[Objective] This study was to explore the effect of purple yam on the nutritional and physiological functions of rats.[Method] Forty SD rats were randomly divided into four groups with ten rats in each group which wer...[Objective] This study was to explore the effect of purple yam on the nutritional and physiological functions of rats.[Method] Forty SD rats were randomly divided into four groups with ten rats in each group which were reared with artificial semisynthetic feed added with 10% steamed yam powder.The four groups,the casein（CK） group which was used as the control,the purple yam（PY） group,the Tiegun yam（TY） group and common Huai yam（HY） group,were supplied with feed of equal content of energy,protein and fat.The rats were pair-fed for 56 days to observe changes of relevant nutritional and physiological indices.[Result] There were no significant differences of body weight,food conversion rate and organ indices between rats in PY group and CK group.Mean corpuscular hemoglobin in PY group was significantly lower than that in CK group;hemoglobin showed no significant difference with that in CK group;eosnophils was lower than that in CK group.Low density lipoprotein-cholesterol,total cholesterol and atherogenic index in PY group were lower than that in the other three groups;anti-atherogenic index was the highest among the four groups,while showing no significant difference;triglyceride content was lower than that in CK group.Among the antioxidant indices,the activity of catalase and superoxide dismutase were higher than that in CK group,while malondialdehyde content was lower than that in CK group.[Conclusion] Purple yam showed the trend of reducing the content of blood glucose,total cholesterol,low density lipoprotein-cholesterol,triglyceride,malondialdehyde,and raising the activity of glutathione peroxidase,superoxide dismutase and catalase,thus having certain antioxidative function.展开更多
AIM: To investigate child and adolescent psychiatrists'(CAPs) attention deficit hyperactivity disorder(ADHD) and oppositional defiant disorder(ODD) diagnoses and treatments in real-world clinical practice. METHODS...AIM: To investigate child and adolescent psychiatrists'(CAPs) attention deficit hyperactivity disorder(ADHD) and oppositional defiant disorder(ODD) diagnoses and treatments in real-world clinical practice. METHODS: The medical records of 69 ADHD children(mean age = 9.5 years), newly referred to the ADHD clinic, were reviewed for their scores of parent- and teacher-reported Vanderbilt ADHD Diagnostic Rating Scales(VADRSs), CAPs' diagnoses of ADHD and ODD, and CAPs' treatment recommendations. Among 63 ADHD subjects who completed both parent and teacher VADRSs, we examined the agreement of the parent and teacher VADRSs. We also examined the concurrent validity of CAPs' ODD diagnoses against the results from the VADRSs. In addition, we compared CAPs' treatment recommendations against established ADHD and ODD guidelines.RESULTS: Among 63 ADHD subjects, the majority of the subjects(92%) met full ADHD diagnostic criteria at least in one setting(parent or teacher) on the VADRSs. Nearly half of the patients met full ADHD diagnostic criteria in two settings(parent and teacher). Relatively low agreement between the parent and teacher VADRSs were found(95%CI:-0.33 to 0.14). For 29 children who scored positive for ODD on the rating scales, CAPs confirmed the ODD diagnosis in only 12 of these casepositives, which is considered as a fair agreement between CAPs and VADRSs(95%CI: 0.10-0.53). For 27 children with no ODD diagnosis made by either CAP or VADRS, more than half of them were recommended for medication only. In contrast, where CAPs made the diagnosis of ODD, or where the parent or teacher VADRS was positive for ODD, almost all of the patients received recommendations for medication and behavior therapy.CONCLUSION: CAPs' ADHD diagnoses have strong concurrent validity against valid rating scales, but ADHD's most common comorbid condition- ODD- may be underrecognized.展开更多

[ Method ] Using HV medium, actinomycete strains were isolated from the rhizesphere soil of frangipani （Plumeria rubra L. ）. The active antagonistic actinomycetes were selected by using confrontation culture method and antifimgal test of fermentation products. Ac- cording to morphology, combined with 16S rRNA gene analysis, the taxonomic status of active actinomycetes was identified. [ Result] Twelve strains of actinomyce- tes were isolated from the rhizosphere soil of medicinal plant frangipani. Strain 51173 showed significant antifungal activity against Colletotrichum gloeosporioides, which was identified to be a member of Streptomyces violaceusniger 16S rRNA gene clade. [ Conclusion] Strain 51173 showed a good application potential for bio- logical control against yam anthracnose.展开更多

BACKGROUND Treatment efficacy for attention-deficit/hyperactivity disorder(ADHD)is reported to be poor,possibly due to heterogeneity of ADHD symptoms.Little is known about poor treatment efficacy owing to ADHD heterog...BACKGROUND Treatment efficacy for attention-deficit/hyperactivity disorder(ADHD)is reported to be poor,possibly due to heterogeneity of ADHD symptoms.Little is known about poor treatment efficacy owing to ADHD heterogeneity.AIM To use generalized structural equation modeling(GSEM)to show how the heterogeneous nature of hyperactivity/impulsivity(H/I)symptoms in ADHD,irritable oppositional defiant disorder(ODD),and the presentation of aggression in children interferes with treatment responses in ADHD.METHODS A total of 231 children and adolescents completed ADHD inattention and H/I tests.ODD scores from the Swanson,Nolan,and Pelham,version IV scale were obtained.The child behavior checklist(CBCL)and parent’s satisfaction questionnaire were completed.The relationships were analyzed by GSEM.RESULTS GSEM revealed that the chance of ADHD remission was lower in children with a combination of H/I symptoms of ADHD,ODD symptoms,and childhood aggressive behavior.ODD directly mediated ADHD symptom severity.The chance of reaching remission based on H/I symptoms of ADHD was reduced by 13.494%[=exp(2.602)]in children with comorbid ADHD and ODD[odds ratio(OR)=2.602,95%confidence interval(CI):1.832-3.373,P=0.000]after adjusting for the effects of other factors.Childhood aggression mediated ODD symptom severity.The chance of reaching remission based on ODD symptoms was lowered by 11.000%[=1-exp(-0.117)]in children with more severe baseline symptoms of aggression based on the CBCL score at study entry[OR=-0.117,95%CI:(-0.190)-(-0.044),P=0.002].CONCLUSION Mediation through ODD symptoms and aggression may influence treatment effects in ADHD after adjusting for the effects of baseline ADHD symptom severity.More attention could be directed to the early recognition of risks leading to ineffective ADHD treatment,e.g.,symptoms of ODD and the presentation of aggressive or delinquent behaviors and thought problems in children with ADHD.展开更多

Using DSM-IV diagnostic criteria of conduct disorder and oppositional defiant disorder, the study showed that 38.1% of parents reported conduct disorder in their children and 46.3% reported oppositional defiant disorder. While 39.3% of children themselves reported conduct disorder and 44% of them reported oppositional defiant disorder. The study showed that 5.1% of children had comorbidity of post traumatic stress disorder and attention deficit disorder, 4.4% had comorbidity of post traumatic stress disorder and impulsivity-hyperactivity disorder, and 4.4% had comorbidity of post traumatic stress disorder and attention deficit with hyperactivity combined type. Also, 4.6% of children had comorbidity of conduct and post traumatic stress disorder and 6.1% had comorbidity of oppositional defiant disorder and post traumatic stress disorder. Conclusions: This study revealed that children living in areas of conflict and war are the main group at risk of developing post traumatic stress disorder and other behavioral problems including attention deficit with hyperactivity, conduct, and oppositional defiant disorder, and the internationallaws must protect the civilians during the conflict, establishing safe havens for children and their families to decrease the effect of war on children.

文摘Oppositional collocations,such as dead nice in English,or shuai dai le(handsome dumb le,‘extremely handsome’)in Chinese,are expressions that use words with emotionally contradictory senses to achieve a single positive meaning.They are not just linguistic expressions or rhetorical devices,but signs that reflect the addresser’s identity and social position.Though between English and Chinese there are distinct differences in terms of formal features,distributive properties,and emotional impacts,the use of such collocations in both languages is often associated in the speaker’s mind with contexts characterized by a desire for informality and resistance to orthodoxy.Though the base of an oppositional collocation is the structurally controlling element for the whole expression,the collocate,represented as a degree adverb in English and degree complement in Chinese,is the emotionally controlling element,which not only reinforces the positive degree on the part of the base but,with some trace of negativity,takes the addresser away from formal or serious language.This essay investigates the phenomenon through a social-semiotic perspective derived from Halliday’s(1976)antilanguage and anti-society theories and Halliday&Hasan’s(1976)register theory.

文摘Advancements in Natural Language Processing and Deep Learning techniques have significantly pro-pelled the automation of Legal Judgment Prediction,achieving remarkable progress in legal research.Most of the existing research works on Legal Judgment Prediction(LJP)use traditional optimization algorithms in deep learning techniques falling into local optimization.This research article focuses on using the modified Pelican Optimization method which mimics the collective behavior of Pelicans in the exploration and exploitation phase during cooperative food searching.Typically,the selection of search agents within a boundary is done randomly,which increases the time required to achieve global optimization.To address this,the proposed Chaotic Opposition Learning-based Pelican Optimization(COLPO)method incorporates the concept of Opposition-Based Learning combined with a chaotic cubic function,enabling deterministic selection of random numbers and reducing the number of iterations needed to reach global optimization.Also,the LJP approach in this work uses improved semantic similarity and entropy features to train a hybrid classifier combining Bi-GRU and Deep Maxout.The output scores are fused using improved score level fusion to boost prediction accuracy.The proposed COLPO method experiments with real-time Madras High Court criminal cases(Dataset 1)and the Supreme Court of India database(Dataset 2),and its performance is compared with nature-inspired algorithms such as Sparrow Search Algorithm(SSA),COOT,Spider Monkey Optimization(SMO),Pelican Optimization Algorithm(POA),as well as baseline classifier models and transformer neural networks.The results show that the proposed hybrid classifier with COLPO outperforms other cutting-edge LJP algorithms achieving 93.4%and 94.24%accuracy,respectively.

文摘In the space of new media, all sorts of new media events which imply oppositional moods are written, presented repeatedly, and therefore form oppositional discourse. Strangers are able to come together as netizens due to their identification by antithesis, and construct an imagined community which allows them to "unite closely" and "speak in one voice". Based on their imagined community, netizens are able to co-construct discourse presentation. This article argues that the critical step in reproduction of oppositional discourse is the providing of scripts. In order to enhance the tension of oppositional discourse, the events representation needs to be processed with overstating, such as making a scary story. The scripts of new media events are usually written according to a preset imaginative chain of "good people-bad people" under the influence of identification by antithesis.

文摘Cryptography is deemed to be the optimum strategy to secure the data privacy in which the data is encoded ahead of time before sharing it.Visual Secret Sharing(VSS)is an encryption method in which the secret message is split into at least two trivial images called’shares’to cover it.However,such message are always targeted by hackers or dishonest members who attempt to decrypt the message.This can be avoided by not uncovering the secret message without the universal share when it is presented and is typically taken care of,by the trusted party.Hence,in this paper,an optimal and secure double-layered secret image sharing scheme is proposed.The proposed share creation process contains two layers such as threshold-based secret sharing in the first layer and universal share based secret sharing in the second layer.In first layer,Genetic Algorithm(GA)is applied to find the optimal threshold value based on the randomness of the created shares.Then,in the second layer,a novel design of universal share-based secret share creation method is proposed.Finally,Opposition Whale Optimization Algorithm(OWOA)-based optimal key was generated for rectange block cipher to secure each share.This helped in producing high quality reconstruction images.The researcher achieved average experimental outcomes in terms of PSNR and MSE values equal to 55.154225 and 0.79365625 respectively.The average PSNRwas less(49.134475)and average MSE was high(1)in case of existing methods.

文摘It is challenging for aqueous Zn-ion batteries(ZIBs)to achieve comparable low-temperature(low-T)performance due to the easy-frozen electrolyte and severe Zn dendrites.Herein,an aqueous electrolyte with a low freezing point and high ionic conductivity is proposed.Combined with molecular dynamics simulation and multi-scale interface analysis(time of flight secondary ion mass spectrometry threedimensional mapping and in-situ electrochemical impedance spectroscopy method),the temperature independence of the V_(2)O_(5)cathode and Zn anode is observed to be opposite.Surprisingly,dominated by the solvent structure of the designed electrolyte at low temperatures,vanadium dissolution/shuttle is significantly inhibited,and the zinc dendrites caused by this electrochemical crosstalk are greatly relieved,thus showing an abnormal temperature inversion effect.Through the disclosure and improvement of the above phenomena,the designed Zn||V_(2)O_(5)full cell delivers superior low-T performance,maintaining almost 99%capacity retention after 9500 cycles(working more than 2500 h)at-20°C.This work proposes a kind of electrolyte suitable for low-T ZIBs and reveals the inverse temperature dependence of the Zn anode,which might offer a novel perspective for the investigation of low-T aqueous battery systems.

文摘[Objective] This study was to explore the effect of purple yam on the nutritional and physiological functions of rats.[Method] Forty SD rats were randomly divided into four groups with ten rats in each group which were reared with artificial semisynthetic feed added with 10% steamed yam powder.The four groups,the casein（CK） group which was used as the control,the purple yam（PY） group,the Tiegun yam（TY） group and common Huai yam（HY） group,were supplied with feed of equal content of energy,protein and fat.The rats were pair-fed for 56 days to observe changes of relevant nutritional and physiological indices.[Result] There were no significant differences of body weight,food conversion rate and organ indices between rats in PY group and CK group.Mean corpuscular hemoglobin in PY group was significantly lower than that in CK group;hemoglobin showed no significant difference with that in CK group;eosnophils was lower than that in CK group.Low density lipoprotein-cholesterol,total cholesterol and atherogenic index in PY group were lower than that in the other three groups;anti-atherogenic index was the highest among the four groups,while showing no significant difference;triglyceride content was lower than that in CK group.Among the antioxidant indices,the activity of catalase and superoxide dismutase were higher than that in CK group,while malondialdehyde content was lower than that in CK group.[Conclusion] Purple yam showed the trend of reducing the content of blood glucose,total cholesterol,low density lipoprotein-cholesterol,triglyceride,malondialdehyde,and raising the activity of glutathione peroxidase,superoxide dismutase and catalase,thus having certain antioxidative function.

文摘AIM: To investigate child and adolescent psychiatrists'(CAPs) attention deficit hyperactivity disorder(ADHD) and oppositional defiant disorder(ODD) diagnoses and treatments in real-world clinical practice. METHODS: The medical records of 69 ADHD children(mean age = 9.5 years), newly referred to the ADHD clinic, were reviewed for their scores of parent- and teacher-reported Vanderbilt ADHD Diagnostic Rating Scales(VADRSs), CAPs' diagnoses of ADHD and ODD, and CAPs' treatment recommendations. Among 63 ADHD subjects who completed both parent and teacher VADRSs, we examined the agreement of the parent and teacher VADRSs. We also examined the concurrent validity of CAPs' ODD diagnoses against the results from the VADRSs. In addition, we compared CAPs' treatment recommendations against established ADHD and ODD guidelines.RESULTS: Among 63 ADHD subjects, the majority of the subjects(92%) met full ADHD diagnostic criteria at least in one setting(parent or teacher) on the VADRSs. Nearly half of the patients met full ADHD diagnostic criteria in two settings(parent and teacher). Relatively low agreement between the parent and teacher VADRSs were found(95%CI:-0.33 to 0.14). For 29 children who scored positive for ODD on the rating scales, CAPs confirmed the ODD diagnosis in only 12 of these casepositives, which is considered as a fair agreement between CAPs and VADRSs(95%CI: 0.10-0.53). For 27 children with no ODD diagnosis made by either CAP or VADRS, more than half of them were recommended for medication only. In contrast, where CAPs made the diagnosis of ODD, or where the parent or teacher VADRS was positive for ODD, almost all of the patients received recommendations for medication and behavior therapy.CONCLUSION: CAPs' ADHD diagnoses have strong concurrent validity against valid rating scales, but ADHD's most common comorbid condition- ODD- may be underrecognized.

文摘[ Objective ] The paper was to isolate actinomycete strains with antagonistic activity against yam anthracnose from the rhizosphere soil of medicinal plants in Hainan Province, and identify its taxonomic status. [ Method ] Using HV medium, actinomycete strains were isolated from the rhizesphere soil of frangipani （Plumeria rubra L. ）. The active antagonistic actinomycetes were selected by using confrontation culture method and antifimgal test of fermentation products. Ac- cording to morphology, combined with 16S rRNA gene analysis, the taxonomic status of active actinomycetes was identified. [ Result] Twelve strains of actinomyce- tes were isolated from the rhizosphere soil of medicinal plant frangipani. Strain 51173 showed significant antifungal activity against Colletotrichum gloeosporioides, which was identified to be a member of Streptomyces violaceusniger 16S rRNA gene clade. [ Conclusion] Strain 51173 showed a good application potential for bio- logical control against yam anthracnose.

文摘Intelligent Transportation Systems(ITS)have become a vital part in improving human lives and modern economy.It aims at enhancing road safety and environmental quality.There is a tremendous increase observed in the number of vehicles in recent years,owing to increasing population.Each vehicle has its own individual emission rate;however,the issue arises when the emission rate crosses a standard value.Owing to the technological advances made in Artificial Intelligence(AI)techniques,it is easy to leverage it to develop prediction approaches so as to monitor and control air pollution.The current research paper presents Oppositional Shark Shell Optimization with Hybrid Deep Learning Model for Air Pollution Monitoring(OSSOHDLAPM)in ITS environment.The proposed OSSO-HDLAPM technique includes a set of sensors embedded in vehicles to measure the level of pollutants.In addition,hybridized Convolution Neural Network with Long Short-Term Memory(HCNN-LSTM)model is used to predict pollutant level based on the data attained earlier by the sensors.In HCNN-LSTM model,the hyperparameters are selected and optimized using OSSO algorithm.In order to validate the performance of the proposed OSSO-HDLAPM technique,a series of experiments was conducted and the obtained results showcase the superior performance of OSSO-HDLAPM technique under different evaluation parameters.
文摘BACKGROUND Treatment efficacy for attention-deficit/hyperactivity disorder(ADHD)is reported to be poor,possibly due to heterogeneity of ADHD symptoms.Little is known about poor treatment efficacy owing to ADHD heterogeneity.AIM To use generalized structural equation modeling(GSEM)to show how the heterogeneous nature of hyperactivity/impulsivity(H/I)symptoms in ADHD,irritable oppositional defiant disorder(ODD),and the presentation of aggression in children interferes with treatment responses in ADHD.METHODS A total of 231 children and adolescents completed ADHD inattention and H/I tests.ODD scores from the Swanson,Nolan,and Pelham,version IV scale were obtained.The child behavior checklist(CBCL)and parent’s satisfaction questionnaire were completed.The relationships were analyzed by GSEM.RESULTS GSEM revealed that the chance of ADHD remission was lower in children with a combination of H/I symptoms of ADHD,ODD symptoms,and childhood aggressive behavior.ODD directly mediated ADHD symptom severity.The chance of reaching remission based on H/I symptoms of ADHD was reduced by 13.494%[=exp(2.602)]in children with comorbid ADHD and ODD[odds ratio(OR)=2.602,95%confidence interval(CI):1.832-3.373,P=0.000]after adjusting for the effects of other factors.Childhood aggression mediated ODD symptom severity.The chance of reaching remission based on ODD symptoms was lowered by 11.000%[=1-exp(-0.117)]in children with more severe baseline symptoms of aggression based on the CBCL score at study entry[OR=-0.117,95%CI:(-0.190)-(-0.044),P=0.002].CONCLUSION Mediation through ODD symptoms and aggression may influence treatment effects in ADHD after adjusting for the effects of baseline ADHD symptom severity.More attention could be directed to the early recognition of risks leading to ineffective ADHD treatment,e.g.,symptoms of ODD and the presentation of aggressive or delinquent behaviors and thought problems in children with ADHD.
