Scoring the Clinical Application of a Novel Scale in a Hybrid Operating Room for Neurosurgery

Objective The lack of clarity regarding the application performance of a hybrid operating room(HOR)and the uncertainty of surgical scheduling often lead to its inefficient application.This study aimed to review the clinical application of our neurosurgical HOR and propose a scale to score cases clearly.Methods We reviewed the operating procedures and duration of stay in 1865 HOR cases.The actual procedures of each case were summarized into 5 application types,and numerical assignment was used to distinguish the dependence of each type on our HOR:surgical procedures combined with interventional procedures(4 points,the highest dependence),surgical procedures combined with imaging procedures(3 points),interventional procedures(2 points),imaging procedures(1 point),and surgical procedures(0 points,the lowest dependence).Results A novel scale that could score 1865 cases into those 5 grades was developed.The percentages by grade were as follows:4 points,4.24%;3 points,4.88%;2 points,20.75%;1 point,69.38%;and 0 points,0.75%.The cumulative usage time was 4241.9 h,the duration of which was as follows:4 points,16.17%;3 points,15.50%;2 points,31.32%;1 point,35.62%;and 0 points,1.39%.Conclusions The HOR serves as a multifunctional room to treat neurosurgical diseases.The scale helps to quickly prioritize cases that rely more on HOR,providing guidelines for surgical scheduling.Although our HOR is unsuitable for emergency cases,it clearly shows the application performance of our HOR to provide a reference for promoting its efficient application. 展开更多

Research on the Path to Improving Humanistic Literacy of Operating Room Nursing Students–Application and Practice of the ORTCC Model

Objective:To explore the application effect of the path for improving humanistic literacy of operating room nursing students based on the ORTCC model,in order to make up for the deficiency of systematic humanistic education in the traditional teaching model.Methods:120 nursing students who practiced in the operating room from June 2023 to June 2024 were randomly divided into the experimental group and the control group,with 60 students in each group.The control group received routine teaching,while the experimental group applied the ORTCC model on the basis of routine and intervened through setting humanistic literacy goals,formulating behavioral norms,conducting diversified training,implementing two-way assessment,and creating departmental culture.The effect of the intervention was evaluated using the Nurse Humanistic Quality Scale,the Clinical Communication Ability Assessment Scale for Operating Room Nursing Students,and a self-made nursing satisfaction questionnaire.Results:After the intervention,the total score of humanistic literacy in the experimental group(4 weeks of intervention:161.43±10.06 points,12 weeks of intervention:165.71±9.45 points)was significantly higher than that in the control group(139.38±10.95 points,143.31±10.52 points)(all p<0.001);The total score of clinical communication ability in the experimental group(109.69±8.05 points for 4 weeks of intervention,116.90±7.21 points for 12 weeks of intervention)was also significantly better than that in the control group(99.14±9.23 points,103.03±8.78 points)(all p<0.001);The total score of patient care satisfaction in the experimental group(94.2±4.3)was significantly higher than that in the control group(82.6±5.7)(p<0.001).Conclusion:The ORTCC model can systematically enhance the humanistic literacy,clinical communication skills and patient satisfaction of operating room nursing students,providing an effective approach for optimizing clinical teaching in the operating room. 展开更多

Objective:To explore the application effectiveness of the“Six-Step”Scenario-Based Teaching Method in operating room nursing education.Methods:Seventy nursing students undergoing clinical training in the operating room of a certain hospital from January 2024 to June 2025 were selected.They were randomly divided into an observation group(n=35)and a control group(n=35)using a random number table.The control group received traditional“mentor-apprentice”on-the-job training,while the observation group underwent the“six-step”scenario-based teaching method.The two groups were compared on final assessment scores,comprehensive competency,surgical nursing emergency response ability,and teaching satisfaction indicators.Results:The observation group achieved significantly higher final assessment scores(85.54±5.05)than the control group(78.63±4.75);After instruction,the observation group scored significantly higher than the control group in:mastery of basic duties and procedures(4.22±0.30 vs.3.98±0.30),understanding of surgical nursing essentials(4.39±0.19 vs.3.98±0.30),proficiency in surgical assistance(4.11±0.33 vs.3.98±0.30),aseptic awareness(4.32±0.24 vs.3.98±0.30),risk awareness(4.22±0.17 vs.3.98±0.30),and occupational safety awareness(4.01±0.23 vs.3.98±0.30).(4.01±0.23),which were significantly higher than the control group’s scores(3.36±0.28),(3.14±0.27),(3.29±0.24),(3.53±0.36),(3.17±0.25),and(3.51±0.18),respectively.Students in the observation group scored significantly higher than the control group in emergency hands-on skills(24.53±1.85 points),surgical coordination skills(27.65±1.87 points),emergency coordination skills(25.34±1.83 points),and patient condition observation skills(24.34±1.79 points)were significantly higher than those of the control group(20.78±1.74 points,26.31±1.95 points,22.92±1.69 points,and 21.58±1.77 points,respectively).The satisfaction rate with operating room nursing education among students in the observation group(97.00%)was significantly higher than that in the control group(77.00%).All differences were statistically significant(p<0.05).Conclusion:The“Six-Step”Scenario-Based Teaching Method effectively enhances operating room students’mastery of theoretical knowledge,practical skills,and core comprehensive abilities,while significantly improving their teaching satisfaction.It warrants promotion and application in operating room nursing education. 展开更多

Objective:To explore the application of moderate and hypothermia control in patients undergoing cerebral hemangioma surgery.By adjusting the precise body temperature of the patients and controlling the changes,the intraoperative and postoperative risks can be reduced,thereby better promoting postoperative recovery.Methods:Thirty patients undergoing cerebral hemangioma surgery were randomly selected as the subjects.All the patients were inpatients in the neurology department between May 2023 and May 2024.The control group received traditional routine nursing management.For the patients in the experimental group,in addition to traditional nursing management,there was also moderate and hypothermia intervention.The body temperature was monitored at three points:the tympanic membrane,esophagus,and rectum.The body temperature was effectively monitored to make it fluctuate within a relatively ideal range.The postoperative recovery was paid attention to and monitored.Key monitoring will be conducted on the intraoperative body temperature changes,hemodynamic indicators of the patients,as well as the postoperative awakening time and neurological function recovery indicators of the patients.Results:Compared with the control group,the patients in the experimental group with moderate to low body temperature had relatively gentle body temperature curves and little fluctuation in body temperature.In terms of hemodynamics,patients in the experimental group were more stable than those in the control group.Meanwhile,they recovered faster after the operation,the time required for patients to regain consciousness was shortened,and the incidence of postoperative complications was lower,especially infection,cerebral edema,and electrolyte abnormalities were more prominent.The comfort level and satisfaction of patients were relatively higher compared with those in the control group.Therefore,it can play a very good promoting role for the patient in the postoperative recovery.Conclusion:The application of moderate and low body temperature in cerebral hemangioma surgery can better stabilize the patient’s condition,reduce the occurrence of complications,and is conducive to the recovery of the patient’s neurological function.This method has changed the deficiency of body temperature management in traditional nursing and created a more ideal surgical environment for patients. 展开更多

The contamination diffusion to the operating room when the door is open was simulated with a computational fluid dynamic(CFD) method,to give the extent of the contamination diffusion.The influence of the door-opening procedure was ignored since the door of the operating room is normally a sliding one.The flow field in the case of the 16 s course of opening the door was simulated.The simulated and the experimental results demonstrate that the extent of the contamination diffusion is around 1.5 m when there is no temperature difference between indoor and outdoor,and there is hardly any contamination diffusion when the temperature difference is 1 ℃.It can be concluded that the positive pressure difference in the operating room lost its function in preventing the contamination when the door is open.That the temperature of corridor is lower than that of operating room contributes to contamination control.Keeping 1 ℃ temperature difference between corridor and operating room and increasing positive pressure and air flow are suggested.It is more secure to set up an anteroom if persons come in or out of the operation room at the course of surgery. 展开更多

Objective:According to the World Federation of Medical Education,critical thinking should be part of the training of medical and paramedical students.Professionals can improve the quality of care of patients after surgery by having or acquiring this skill in health care.Also,Emotional intelligence is introduced as an impor tant and effective factor on the professional performance and mental health of healthcare professionals.Thus,the present study was designed and implemented to determine the relationship between emotional intelligence and critical thinking among operating room nursing students of medical sciences universities in Iran.Methods:This cross-sectional study was done on 420 operating room students in 10 top medical sciences universities of Iran in 2022.The sampling method in this research was multistage sampling.The data collection instruments included demographic characteristics,Rickett's critical thinking,and Bradberry-Greaves'emotional intelligence questionnaires.After receiving the ethics code,data collection was done for 2 months.For data analysis,descriptive and inferential analyses including independent t-tests,analysis of variance,and Pearson correlation were used.The collected data were analyzed by SPSS 18(IBM Corporation,Armonk,New York,United States).P-value<0.05 was considered significant.Results:The mean age of the students participating in this study was 23.02±3.70 years,with women constituting 67.4%of them.The results of data analysis indicated that the mean total score of critical thinking and emotional intelligence was 124.10±37.52 and 114.12±43.63,respectively.A direct significant correlation between critical thinking and emotional intelligence(r=0.459,P-value<0.001)and a significant relationship between gender and emotional intelligence(P-value=0.028)were found.Conclusions:Based on the present study results,educational managers in the Ministry of Health are suggested to consider suitable educational programs for improving critical thinking and emotional intelligence to enhance the quality of care provided by students in operating rooms. 展开更多

Objective: to comprehensively enhance the nursing effect of strengthening the application of detail management in operating room management by operating room nurses. Methods: 80 patients who came to the operation from January 2020 to December 2020 were selected and divided into the control group with routine nursing and the experimental group with detailed nursing management in the operating room. The patients' satisfaction, operation time, anxiety and body temperature were compared between the two groups. Results: the overall operating room management and detailed management of the optimized nursing measures were effectively improved. The experimental study found that the satisfaction degree of the experimental group was significantly higher than that of the control group, and the whole operation time was shorter than that of the control group. The degree of anxiety of the observation group was lower than that of the control group, and the body temperature was also higher than that of the control group. The relevant data of the two groups of patients had clinical statistical significance, P < 0.05. Conclusion: in the process of comprehensive improvement of surgical treatment, for the whole implementation of relevant nursing, the real effect of routine nursing and corresponding implementation of detail management is adopted, so as to reduce the anxiety of patients in the operating room, ensure the patient's temperature reaches a stable level, effectively shorten the operation time, and thus comprehensively improve the whole satisfaction of the whole detail operating room. 展开更多

BACKGROUND Patients’lack of correct understanding of cardiovascular disease and interventional therapy is often accompanied by varying degrees of fear,depression and anxiety.Negative emotion will affect the hemodynamic fluctuation of patients undergoing interventional surgery,which is not conducive to the smooth and safe operation of interventional surgery.Therefore,it is very important to implement effective nursing intervention in the operating room.AIM To explore the intervention effect of motivational psychological nursing combined with programmed nursing on compliance and bad mood of patients in interventional operating room.METHODS A total of 98 patients in the interventional operating room of our hospital from October 2019 to March 2021 were randomly divided into study group(n=49)and control group(n=49).The control group took routine nursing.However,the study group took motivational psychological nursing combined with procedural nursing on the basis of the control group.Statistics were made on rehabilitation compliance,Positive and Negative Affect Schedule of bad mood,Simplified Coping Styles Questionnaire score of coping style and satisfaction of intervention between the two groups before and after intervention.RESULTS The rehabilitation compliance of the study group(95.92%)was higher than that of the control group(81.63%)(P<0.05).After intervention,the scores of upset,fear,irritability,tension and fear in the study group were respectively,which were lower than those in the control group(P<0.05).After intervention,the score of positive coping in the study group was higher than that in the control group.However,the score of negative coping in the study group was lower than that in the control group(P<0.05).The intervention satisfaction of the study group(93.88%)was higher than that of the control group(79.59%)(P<0.05).CONCLUSION The intervention of motivational psychological nursing combined with procedural nursing can improve the rehabilitation compliance,and alleviate the bad mood.In addition,it can change their coping style to the disease,and the patients are more satisfied with the nursing work. 展开更多

BACKGROUND The nursing working environment is an important subsystem in the hospital environment.A good working environment could have a positive impact on nurses.However,the work-family conflict and unsatisfactory working environment could significantly reduce their working enthusiasm,efficacy as well as the overall quality of the nursing,increase their fatigue,and thereby compromise their career status.AIM To explore the possible status quo and to analyze the correlation between work environment perception and the work-family conflict among nurses in the operating room.METHODS A total of 312 operating room nurses from two first-class hospitals at Grade 2 and two first-class hospitals at Grade 3 in China from May to September 2017 were included in this research using the cluster sampling method.The data,including the general information questionnaire,the practice environment scale of the nursing work index(PES-NWI),and the work-family conflict scale,were systematically collected.Pearson correlation analysis was applied to analyze the correlation between the two scores,with influencing factors analyzed by hierarchical regression analysis.RESULTS A total of 312 questionnaires were issued,and the response rate and effective questionnaire rate were both 96.15%(300/312).The total scores of the PES-NWI scale and the work-family conflict scale were 3.07±0.43(vs maximum up to 4 points)and 52.32±8.79(vs maximum up to 90 points),respectively.The scores of the PES-NWI scale were negatively correlated with that of work-family conflict scale(all P<0.05).The perception of the nursing work environment and the number of night shifts per month were the major factors contributing to the workfamily conflict(all P<0.05).CONCLUSION The nursing work environment and the work-family conflict among nurses in the operating room were both found at a medium level with a negative correlation between the two. 展开更多

Background and objective Airway stent placement is the effective regimen for central airway obstruction(CAO),while its application scenarios varied.This study aimed to make clinical comparison of airway stent placement in the intervention room and operating room.Methods Patients underwent airway stent placement between 2014 and 2018 were included in this retrospective case-control study.Clinical performance of airway stent placement in intervention room and operating room were compared.Results 82 patients were included in this study,including 39 in the intervention room and 43 in the operating room.Patients treated in the intervention room had lower Charlson comorbidity index(CCI)(P=0.018)and received less Y-shaped stents(P<0.001).Better clinical response(P=0.026),more stents placed(P<0.001)and longer length of stent(P<0.001)were observed in operating room,while there was no significantly statistical difference of stent-related complications and post-stent survival rate between the two groups.Extracorporeal membrane oxygenation(ECMO)supported airway stent placement procedures were performed in the operating room,which provided definitive safety support for high-risk intervention.Conclusion Patients with CAO could benefit from the operating room scenario,and airway stent placement in the operating room is more suitable for patients with higher CCI scores and receiving more complicated procedures. 展开更多

BACKGROUND A hybrid operating room(Hybrid-OR)is a surgical theatre that combines a conventional operating room with advanced medical imaging devices.There are still plenty of limitations when endovascular treatment or microsurgical treatment is used individually to treat large or giant carotid-ophthalmic aneurysms.AIM To explore and summarize the technical features and effectiveness of the application of a Hybrid-OR in managing major intracranial carotid-ophthalmic aneurysms.METHODS The Department of Neurosurgery treated 12 cases of large or giant intracranial carotid-ophthalmic aneurysms between March 2013 and December 2019 in a Hybrid-OR.All cases were treated with clipping and parent vessel reconstruction.RESULTS With the assistance of the Hybrid-OR,the rate of incomplete intraoperative aneurysm clipping decreased from 25%(3/12)to 0%,while the rate of vessel stenosis decreased from 16.7%(2/12)to 8.35%(1/12).In terms of thromboembolic events,ischemic infarction complication occurred in only one patient,and none of the patients experienced embolic infarction complications.All 12 patients were followed for an average of 3 years,and no aneurysms recurred.The postoperative recovery was evaluated with the modified Rankin Scale(mRS):11 patients showed no symptoms(mRS=0),1 patient showed slight disability(mRS 1-2),and none of the patients had severe disability(mRS=5)or died(mRS=6).CONCLUSION The Hybrid-OR provides new ideas for the surgical clipping of large or giant intracranial carotid-ophthalmic aneurysms and decreases the rate of intraoperative vessel stenosis and unsuccessful clipping. 展开更多

Objective:Surgical procedures manifest immense risks to patients.One of the adverse events that healthcare professionals see as a threat to the patient’s health is the development of complications known as surgical site infection(SSI).Although several effor ts are being under taken to determine the proper means to reduce such complications,there is still a high incidence of SSI worldwide.Surgery requires knowledge in infection control and high precision in maintaining a clean surgical site.This study tested the effectiveness of an operating room(OR)bundle of care in preventing SSI in patients undergoing elective exploratory laparotomy cholecystectomy surgery.Methods:A quasi-experimental pretest and posttest design was utilized to determine its effectiveness.The study was composed of 60 par ticipants divided into two groups:30 subjects were selected to receive the OR bundle of care,while the other 30 subjects received the usual care.The groupings were determined through a systematic random sampling technique.The OR bundle of care had three interventions,namely:(1)maintaining perioperative normothermia,(2)no pre-operative surgical site hair removal,and(3)changing gloves before abdominal wall closure.These patients were evaluated using the standard instrument,Bates–Jensen Wound Assessment Tool(BWAT)in the post-intervention phases of the wound healing process,which are as follows:hemostasis,inflammatory,and proliferative phases.To describe the difference in the patients’wound status after implementation of the OR bundle of care in each post-intervention phase,Friedman’s test was used.To describe the difference in the patients’wound status in both groups after implementation of the OR bundle of care,the Mann–Whitney U test was used.Results:The patient’s wound status was lower,indicating a more healing process.Differences between the wound status of the control and the experimental group were observed on the third postoperative day.This indicates that the experimental group’s wound status healed much faster and more effectively than the control group based on the BWAT scoring severity scoring.A significant difference in the patient’s wound status from the hemostasis phase compared with the proliferative phase was observed.Conclusions:The OR bundle of care has been shown to be effective in preventing SSI in patients who had undergone exploratory laparotomy cholecystectomy surgery in the selected hospital,if there is uniform and consistent implementation of the said intervention. 展开更多

Objective:Operating room nurses,as essential members of health care teams,often face ethical challenges in the operating room.By using the ethical experiences of operating room nurses,a better understanding of ethics in the operating room can be achieved,which can lead to better nursing decisions in the face of these challenges.Therefore,this study was conducted to investigate operating room nurses’lived experiences of ethical codes.Methods:A hermeneutic phenomenological study was performed in Hamadan(Iran)from February 2019 to November 2020.Ten operating room nurses were selected as participants by purposive sampling.Data were collected through in-depth and semi-structured interviews.Data analysis was performed based on Van Manen methodology.Results:Data analysis revealed three main themes and 11 sub-themes representing the operating room nurses experience of the ethical code.The main themes were;adherence to professional commitments,preserving patient dignity,and respect to colleagues.Conclusion:The results underlined ethics and ethical values in the operating room.Due to the intense interactions between operating room nurses with the patient and surgical team,commitment to ethics by nurses can lead to improving quality of care and interactions among members of the surgical team.It is suggested that using these codes as a guideline and a framework could be developed to improve the ethical and professional performance of operating room nurses. 展开更多

Objective:To evaluate and compare the effect of semi-rigid ureterorenoscopy on post-procedural sexual function with three different operating room settings.Methods:In this prospective randomized study,consecutive sexually active patients with normal pre-operative sexual function undergoing uncomplicated semi-rigid ureterorenoscopy for distal ureteric stones were randomized into three groups,with three different operating room settings.Procedure-related anxiety and sexual function were assessed pre-operatively using Amsterdam Preoperative Anxiety and Information Scale and Brief Sexual Function Inventory(in males)and Female Sexual Function Index-6(in females),respectively.All the participants were stented following the procedure,and the stent was removed after 3 weeks.Post-procedural sexual function and general discomfort were assessed and compared between three groups at 1 week,3 weeks,and 12 weeks.The effect of surgery-related anxiety,preoperative sexual function,age,and general discomfort(including stent-related discomfort)on post-procedural sexual function were analyzed using multiple regression analysis.A p-value of less than 0.05 was considered statistically significant.Results:Totally,327 eligible patients were randomized into three groups.The group of patients who underwent the procedural with a screen separating the operating area from the patient vision,while the patient could watch the endoscopy through a separate monitor,had better post-procedural sexual function compared to those who had total vision of the operating area as well as to those whose eyes were blocked.This difference was statistically significant.This post-procedural reduction in sexual function could not be attributed to in situ stent alone.Conclusions:Our study showed that semi-rigid ureterorenoscopy can have significant negative effect on sexual function,which can be reduced with proper preoperative counseling and an ideal operating room settings. 展开更多

Methods: From June 2021 to December 2021, 78 patients who underwent gastric cancer surgery in our hospital were selected for research. Combined with the random number table method, they were divided into the control group (providing routine nursing care in operating room) and the observation group (providing nursing intervention in operating room) with 39 patients in each group respectively. The body temperature of the two groups during operation, during abdominal closure and after operation, the time of leaving anesthesia room, extubation, postoperative wakefulness and hospitalization, degree of satisfaction with nursing work was compared. Results: Compared with the control group, the body temperature in the observation group tended to be more normal during operation, during abdominal closure and after operation (P 0.05). The time of leaving anesthesia room, extubation, postoperative wakefulness and hospitalization in the observation group were shorter than those in the control group (P 0.05). The satisfaction degree of the observation group with nursing work was higher than that of the control group (P 0.05). Conclusion: Nursing intervention in operating room is effective for gastric cancer patients in anesthesia recovery period, which can maintain their perioperative temperature stability, promote their postoperative recovery and enhance their satisfaction with nursing work.
