Methodology: This was a retrospective cohort study. HIV-infected children on ART aged less than 15 years were divided into two groups Group 1 (G1) comprising children who started ART prior 2 years and Group (G2) those put on treatment thereafter. Main Measures: Percentage of children with growth retardation measured by Height for Age (H/A) and Weight for Age (W/A) Results: In total, we included 90 subjects. The median age was 10 years with a slight female predominance (51.2%). Most children were asymptomatic at the time of the study (96.6%), compliant with treatment (81%), 54.8% of children were on cotrimoxazole, 33% knew their status. At initiation, underweight was predominant in group 1 compared to group 2 (52% versus 29.5%;p = 0.015). Conversely, stunted growth predominated in G2 compared to G1 but without significant difference (38% versus 50%;p = 0.147). At the time of our study (median age of 10 years), catch-up height and weight predominated in G1 compared to G2;only a small proportion remained below −2SD (4% versus 18.2%;p = 0.015 and 9% versus 29.5%;p = 0.006 respectively for underweight and stunted growth). Conclusion and Global Health Implications: Growth retardation was common at ART initiation. Catch-up in height was more evident in the early treatment group. Initiation of ART before the age of 2 years rather influences children’s height than weight. The result of this study further emphasizes the need of early ART and closed clinical monitoring to better assess the impact of ART in areas with high rates of undernutrition.展开更多

Compared toYAD:YAR, OAD pairs had the lowest likelihoods of improved functional status 5 years post-LT (OAD:YAR oddsratio 0.53, 95% confidence interval 0.42-0.67, P < 0.001;OAD:OAR odds ratio 0.67, 95% confidence interval 0.51-0.89, P = 0.006). Donor:recipient age pairs with older donors had higher rates of graft- and infection-relatedmortality compared to those with younger donors (P < 0.001). Meanwhile, donor:recipient age pairs with olderrecipients had higher cardioneurovascular- or malignancy-related deaths compared to those with youngerrecipients (P < 0.001).CONCLUSIONDonor:recipient age mismatch was associated with differences in cause-specific mortality and functional status. These insights could potentially inform age-matched organ allocation strategies, though future work is warranted.展开更多

文摘BACKGROUND Old donor allografts in liver transplantation(LT)account for 25%of all allografts, and their utilization is projected to increase with the aging general population.Older allografts are associated with higher rates of all-cause mortality and graft failure;however, there is limited literature exploring the specific phenotypic changes(e.g.,functional status,cause-specific mortality)observed in different donor:recipient age pairs.AIM To investigate differences in functional impairment and cause-specific mortality between different donor:recipient age pairs.METHODS This was a retrospective analysis of LT patients from the Scientific Registry of Transplant Recipients from 2002 to 2022.Donors were categorized into younger age donors,≤45-years(YAD),middle-aged donors,46-69-years(MAD),and older age donors,≥70-years(OAD).Recipients were categorized into younger age recipients,≤55-years(YAR)and older age recipients,>55-years(OAR)age recipients.Multivariate Fine-Gray competing risk and logistic regression analyses identified independent risk factors for cause-specific mortality and improvements in functional status, respectively.RESULTS Overall,126185 patients were included in the analysis:YAD:YAR(32.7%), YAD:OAR (25.2%), MAD:YAR (17.5%), MAD:OAR (20.7%), OAD:YAR (1.3%), and OAD:OAR (2.7%). Compared toYAD:YAR, OAD pairs had the lowest likelihoods of improved functional status 5 years post-LT (OAD:YAR oddsratio 0.53, 95% confidence interval 0.42-0.67, P < 0.001;OAD:OAR odds ratio 0.67, 95% confidence interval 0.51-0.89, P = 0.006). Donor:recipient age pairs with older donors had higher rates of graft- and infection-relatedmortality compared to those with younger donors (P < 0.001). Meanwhile, donor:recipient age pairs with olderrecipients had higher cardioneurovascular- or malignancy-related deaths compared to those with youngerrecipients (P < 0.001).CONCLUSIONDonor:recipient age mismatch was associated with differences in cause-specific mortality and functional status. These insights could potentially inform age-matched organ allocation strategies, though future work is warranted.

文摘BACKGROUND Gastric cancer is among the most prevalent malignancies worldwide.Despite significant advancements in chemoradiotherapy, targeted therapy, and neoadjuvant therapy, conventional surgical intervention remains the cornerstone of gastric cancer management.Improvements in surgical techniques, coupled with the use of staplers and other advanced instruments, have substantially reduced the incidence of complications and mortality following gastric cancer surgery. However, dysphagia remains a common postoperative complication.AIM To retrospectively investigate the potential factors contributing to dysphagia in patients who have undergone laparoscopic radical gastrectomy for gastric cancer and to explore effective strategies for its postoperative management.METHODS In this retrospective study,we analyzed data from patients who underwent elective laparoscopic total gastrectomy at Lihuili Hospital,Ningbo University, between January 2018 and May 2022.A total of 115 eligible postoperative patients were included.Postoperatively, patients completed questionnaires and were categorized into two groups based on their responses:The dysphagia group(Eating Assessment Tool-10 score≥3)and the non-dysphagia group(Eating Assessment Tool-10 score<3).Risk factors associated with dysphagia following total gastrectomy were assessed usingχ2 tests,Fisher’s exact tests,t-tests,Pearson correlation coefficients, and univariate and multivariate regression analyses.RESULTS Multivariate analysis further identified anastomotic style, prolonged intubation time, advanced age,and low albumin(ALB)levels as independent risk factors for postoperative dysphagia.Implementing targeted preventive measures for high-risk groups may significantly enhance postoperative quality of life.CONCLUSION Univariate analysis revealed that anastomotic style, low serum ALB levels, advanced age,and prolonged intubation time were significantly associated with postoperative dysphagia in gastric cancer patients.Multivariate analysis further identified anastomotic style, prolonged intubation time, advanced age,and low ALB levels as independent risk factors for postoperative dysphagia.Implementing targeted preventive measures for high-risk groups may significantly enhance postoperative quality of life.

文摘In order to study the effects of the contents of used mortar recycled aggregate(OMRA)and brick recycled aggregate(BRA)on the deformation properties of recycled aggregate concrete(RAC),under uniaxial compression conditions,The RAC of OMRA(0%,5%,10%,and 15%)and BRA(0%,3%,6%,9%,12%,and 15%)were studied. The experimental results show that, under uniaxial compression, the interfacial relationships of RAC containing OMRA and BRA between different materials are more complex, and the failure mechanism is also more complex. The content of OMRA and BRA had significant influence on the deformation behavior of RAC.When the content of OMRA and BRA is high,it is difficult for existing formulas and models to accurately represent the actual value. In this study, the influence of OMRA and BRA content is taken into account, and the existing formulas for calculating concrete deformation are modified,so that these formulas can more accurately calculate the elastic modulus, peak strain and ultimate strain of recycled concrete. The stress-strain formula of Guo concrete fits the stress-strain curve of concrete very well.We modified the formula on the basis of Guo formula to make the formula more suitable for the stress-strain curve of recycled concrete containing old mortar and brick, and the theoretical model proposed has better fitting accuracy. The study provides a valuable reference for nonlinear analysis of recycled aggregate concrete structures under different proportions of OMRA and BRA.

文摘This study analyzed case and prescription data of children aged 12 to 18 years diagnosed with influenza A from the outpatient and emergency departments at the Women and Children’s Hospital of Ningbo University during the influenza epidemic period between February and April 2023.Patients were categorized into two groups based on their medication:the Baloxavir Marboxil group(196 cases)and the Oseltamivir group(126 cases).SPSS Statistics 24.0 software was utilized to compare variables between the two groups, including age, sex, weight, first visit department, number of visits, average prescription cost per patient, drug varieties used, hospitalization rate, and the combined medication usage,to evaluate the relative advantages and disadvantages of the two treatments. The results indicated that the number of visits in the Baloxavir Marboxil group was significantly lower than in the Oseltamivir group, whereas the average prescription cost per patient was higher(P<0.05).There were no significant differences between the two groups regarding age, sex, weight, first visit department, number of drug types used, hospitalization rate,or combined medication usage(P>0.05).When comparing the use of single-class combined drugs, only the quantity of antiviral drugs differed significantly(P<0.05).The Oseltamivir group required more adjuvant treatment with both Chinese and Western antiviral medications. The distribution of the number of combined drug types was concentrated around three and two types in both groups(Baloxavir Marboxil group:41.34%and 39.66%;Oseltamivir group:44.54%and 36.97%).This study highlighted that the new drug,Baloxavir Marboxil, offered certain therapeutic benefits over Oseltamivir in treating influenza among children aged 12 to 18 years, including advantages in single-dose administration, oral compliance, and specific treatment outcomes. The higher unit cost associated with Baloxavir Marboxil might be a factor for further consideration. These findings provided a new reference point for the clinical selection of anti-influenza medications.
文摘BACKGROUND Total hip arthroplasty(THA)is an increasingly common treatment for older patients with hip osteoarthritis.Psychological stress is common before THA,although its clinical effects on selected parameters such as joint function, quality of life, and postoperative complications remain unclear.AIM To investigate the effects of preoperative psychological stress on selected parameters in older patients who underwent THA.METHODS Ninety older patients who underwent THA between January 2023 and August 2024 were divided into two groups by their preoperative self-rated anxiety scale and self-rated depression scale scores, including high-stress(n=42)and lowstress(n=48).The postoperative joint function, short form-36 health survey(SF36)score,incidence of postoperative complications, and other indicators were compared between the two groups.Pearson’s correlation coefficient analysis of the relationship among preoperative psychological stress, quality of life, and postoperative complications was performed.RESULTS Postoperative joint function and quality of life were lower in the high-stress group than they were in the low-stress group(P<0.05).The incidence of postoperative complications was higher in the high-stress group(29.27%)than it was in the low-stress group(9.30%)(P<0.05).Cor-relation analysis revealed that psychological stress was correlated with the Harris hip and SF-36 scores.Total scores on the scale, including physical function, physical pain, general health, mental health, social function, vitality, and emotional function, were negatively correlated(P<0.05).CONCLUSION Preoperative psychological stress results in adverse effects on quality of life and complications in older patients undergoing THA.Therefore,pre-operative psychological interventions should be strengthened to improve postoperative outcomes.
