English-language literature cited in MEDLINE from January,1980 to October 30,2014 was searched by using terms of antipsychotic,generic and brand names of atypical antipsychotics, ＂bipolar depression/bipolar disorder...English-language literature cited in MEDLINE from January,1980 to October 30,2014 was searched by using terms of antipsychotic,generic and brand names of atypical antipsychotics, ＂bipolar depression/bipolar disorder＂, ＂placebo＂,and ＂trial＂.The parameters of response（≥50%improvement on MADRS,Montgomery-Asberg Depression Rating Scale total score）,remission（either ≤12 or 8 on MADRS total score at endpoint）,discontinuation due to adverse events（DAEs）,somnolence,≥7%weight gain,overall extrapyramidal side-effects（EPSs）,and akathisia,were extracted from originally published primary outcome papers.The number needed to treat to benefit（NNT） for response and remission or harm（NNH） for DAEs or other side effects relative to placebo were estimated and presented with the estimate and 95%confidence interval.Olanzapine monotherapy,olanzapine-fluoxetine combination（OFC）,quetiapine-IR monotherapy,quetiapine-XR monotherapy,lurasidone monotherapy,and lurasidone adjunctive therapy were superior to placebo with NNTs for responses of 11-12,4,7-8,4,4-5,and 7,and NNTs for remission of 11-12,4,5-11,7,6-7,and 6,respectively.There was no significant difference between OFC and lamotrigine,and between aripiprazole or ziprasidone and placebo in response and remission.Olanzapine monotherapy,quetiapine-IR,quetiapine-XR,aripiprazole,and ziprasidone 120-160 mg/day had significantly increased risk for DAEs with NNHs of 24,8-14,9,12,and 10,respectively.For somnolence,quetiapine-XR had the smallest NNH of 4.For ≥7%weight gain,olanzapine monotherapy and OFC had the smallest NNHs with both of 5.For akathisia,aripiprazole had the smallest NNH of 5.These findings suggest that among the FDA-approved agents including OFC,quetiapine-IR and-XR,lurasidone monotherapy and adjunctive therapy to a mood stabilizer,the differences in the NNTs for response and remission are small,but the differences in NNHs for DAEs and common side-effects are large.Therefore,the selection of an FDA-approved atypical antipsychotic for bipolar depression should be based upon safety and tolerability.展开更多

Methods: From August 2022 to April 20...Objective: To examine the daily life care, spiritual comfort, and fall safety care needs of the elderly in urban and rural areas of Tangshan, along with their influencing factors. Methods: From August 2022 to April 2023, an investigation was conducted among urban and rural elderly individuals aged over 75 years in Tangshan City using the Activities of Daily Living Scale, the Revised Community Elderly Fall Risk Assessment Tool, and the Loneliness Scale. Results: The study included 750 urban and 740 rural elderly individuals aged over 75 years. Matrix analysis revealed a significant proportion of fall safety care needs across various daily life and spiritual care requirements. Multiple factor analysis indicated that advanced age, lower education levels, a greater number of chronic diseases, and lower levels of family and social support were associated with higher care demands among the elderly in both urban and rural areas. These differences were statistically significant (P < 0.05). Conclusion: The elderly in urban and rural areas demonstrate a high demand for fall safety care. Particular attention should be given to individuals with lower education levels, those who are widowed, those with multiple chronic diseases, and those with low levels of family and social support to better meet the diverse care needs of this population.展开更多
Objective:To explore the self-management experiences and outpatient nursing needs of intestinal stoma patients during the post-discharge transition period,in order to provide a basis for scientific decision-making in ...Objective:To explore the self-management experiences and outpatient nursing needs of intestinal stoma patients during the post-discharge transition period,in order to provide a basis for scientific decision-making in patient self-care and improvements in specialized stoma outpatient services.Methods:Using purposive sampling,13 colorectal cancer patients in the post-discharge transition period who had undergone intestinal stoma surgery were selected from a tertiary hospital in Shandong Province between November 2024 and March 2025.Semi-structured interviews were conducted,and data were analyzed using Colaizzi’s seven-step method to extract themes.Results:Three core themes were identified:challenges in self-management during the transition period,outpatient nursing needs during the transition period,and gaps in doctor-patient information continuity.Conclusion:Future efforts should focus on high-risk groups of stoma patients during the transition period by building intelligent and systematic outpatient guidance and support systems to improve their quality of life.展开更多
Background:Despite self-determination theory’s prominence,few studies examine BPN in China’s ethnic frontier regions.Therefore,this study investigates and analyzes the current situation of community residents with d...Background:Despite self-determination theory’s prominence,few studies examine BPN in China’s ethnic frontier regions.Therefore,this study investigates and analyzes the current situation of community residents with different demographic characteristics in Yanbian in the three basic psychological needs of competency needs,autonomy needs and attribution needs,so as to provide reference for the construction of community service in Yanbian area.Methods:A survey was conducted among 234 residents in four communities in Yanbian from September to October 2024.The equal probability random sampling method was used to collect data by using a general information questionnaire and basic psychological demand scale,and SPSS 28.0 was used for analysis.Through single factor analysis,Pearson correlation analysis and multiple linear regression analysis,the influencing factors and correlation of basic psychological needs of community residents in Yanbian area were discussed.Results:The basic psychological needs of residents were above the middle level;there is a significant positive correlation between economic status and the three dimensions of basic psychological needs(P<0.001).There is a significant positive correlation between the three dimensions of competence demand(β=2.028,P<0.001),autonomy demand(β=2.022,P<0.001),and attribution demand(β=2.693,P<0.001).Conclusion:The satisfaction degree of basic psychological needs of community residents in Yanbian is significantly affected by economic situation,which urges nurses to advocate for economic empowerment programs in community health and provides an empirical basis for the construction of a psychological service system in this area.At the same time,it is proposed that the region can respond by improving social support and improving employment results.展开更多
BACKGROUND China has recently encountered severe challenges associated with population aging.Parents of first-generation only children face significant challenges regarding elderly care needs and the associated negati...BACKGROUND China has recently encountered severe challenges associated with population aging.Parents of first-generation only children face significant challenges regarding elderly care needs and the associated negative emotions.AIM To analyze the elderly care needs of first-generation only child parents in China and identify factors that influence negative emotions.METHODS This study used a cross-sectional design.Convenience sampling was used to select 1580 elderly individuals who met the inclusion criteria in a Chinese city between June and September 2022.A questionnaire was administered to collect general information about participants.Depression and anxiety were assessed using the patient health questionnaire-9 and generalized anxiety disorder-7 scale,respectively.A logistic regression analysis was performed to evaluate the relevant correlations.RESULTS Among 1580 first-generation only child parents,1120(70.89%)preferred family based care,324(20.51%)opted for community care,and 136(8.61%)chose institutional care,with 460(29.11%)reporting negative emotions.Significant differences in the distribution of negative emotions among only child parents were observed based on age,marital status,living conditions,disability,type of chronic disease,frailty status,and family support(P<0.05).The regression analysis indicated that disability,type of chronic disease,living environment,frailty status,and level of family support were independent risk factors for negative emotions among parents with only children(P<0.05).CONCLUSION Elderly care for parents of only children is primarily family-based.Independent risk factors for negative emotions in this group include disability,chronic disease type,and living environment.展开更多
