Effectiveness Evaluation of Need-Based Nursing for Hemodialysis Patients with Uremia

Objective:To investigate the clinical effectiveness of need-based nursing interventions for hemodialysis patients with uremia.Methods:A double-blind,randomized controlled trial was conducted from January 2020 to June 2024,including a sample of 100 hemodialysis patients with uremia.The patients were randomly assigned into a test group(n=50)and a control group(n=50)using a random number table based on their medical record numbers.The control group received standard basic nursing care,while the test group received need-based nursing intervention.The two groups were compared based on self-care ability scores,resilience scores using the CD-RISC-10 scale,complication incidence rates,and nursing satisfaction.Results:After the nursing intervention,the test group showed significantly higher scores in self-care ability(P<0.05)and CD-RISC-10 resilience(P<0.05)than the control group.The incidence of complications was significantly lower(P<0.05),and nursing satisfaction was notably higher(P<0.05)in the test group compared to the control group.Conclusion:Need-based nursing for hemodialysis patients with uremia can enhance self-care abilities,improve psychological resilience,reduce complication rates,and increase nursing satisfaction,making it worthy of broader implementation. 展开更多

Psoriatic arthritis(PsA)is a complicated psoriasis comorbidity with manifestations of psoriatic skin and arthritic joints,and tailoring specific treatment strategies for simultaneously delivering different drugs to different action sites in PsA remains challenging.We developed a need-based layered dissolving microneedle(MN)system loading immunosuppressant tacrolimus(TAC)and antiinflammatory diclofenac(DIC)in different layers of MNs,i.e.,TD-MN,which aims to specifically deliver TAC and DIC to skin and articular cavity,achieving simultaneous alleviation of psoriatic skin and arthritic joint lesions in PsA.In vitro and in vivo skin permeation demonstrated that the inter-layer retained TAC within the skin of~100μm,while the tip-layer delivered DIC up to~300μm into the articular cavity.TD-MN not only efficiently decreased the psoriasis area and severity index scores and recovered the thickened epidermis of imiquimod-induced psoriasis but also alleviated carrageenan/kaolin-induced arthritis even better than DIC injection through reducing joint swelling,muscle atrophy,and cartilage destruction.Importantly,TD-MN significantly inhibited the serum TNF-αand IL-17 A in psoriatic and arthritic rats.The results support that this approach represents a promising alternative to multi-administration of different drugs for comorbidity,providing a convenient and effective strategy for meeting the requirements of PsA treatment. 展开更多

A number of optical sensing tools are now available and can potentially be used for refining need-based fertilizer nitrogen(N)topdressing decisions.Algorithms for estimating field-specific fertilizer N needs are based on predictions of yield made while the crops are still growing in the field.The present study was conducted to establish and validate yield prediction models using spectral indices measured with proximal sensing using GreenSeeker canopy reflectance sensor,soil and plant analyzer development(SPAD)chlorophyll meter,and two different types of leaf color charts(LCCs)for five basmati rice genotypes across different growth stages.Regression analysis was performed using normalized difference vegetation index(NDVI)recorded with GreenSeeker sensor and leaf greenness indices measured with SPAD meter and LCCs developed by Punjab Agricultural University,Ludhiana(India)(PAU-LCC)and the International Rice Research Institute,Philippines(IRRI-LCC).The exponential relationship between NDVI and grain yield exhibited the highest coefficient of determination(R^(2))and minimum normalized root mean square error(NRMSE)at panicle initiation stage and explained 38.3%–76.4%variation in yield using genotype-specific models.Spectral indices pooled for different genotypes were closely related to grain yield at all growth stages and explained53.4%–57.2%variation in grain yield.Normalizing different spectral indices with cumulative growing degree days(CGDD)decreased the accuracy of yield prediction.Normalization with days after transplanting(DAT),however,did not reduce or improve the predictability of yield.The ability of each model to predict grain yield was validated with an independent dataset collected from two experiments conducted at different sites with a range of fertilizer N doses.The NDVI-based genotype-specific models exhibited a robust linear correlation(R^(2)=0.77,NRMSE=7.37%,n=180)between observed and predicted grain yields only at 35 DAT(i.e.,panicle initiation stage),while the SPAD,PAU-LCC,and IRRI-LCC consistently provided reliable predictions(with respective R^(2)of 0.63,0.60,and 0.53 and NRMSE of 10%,10%,and 13.6%)even with genotype invariant models with 900 data points obtained at different growth stages.The study revealed that unnormalized values of spectral indices,namely NDVI,SPAD,PAU-LCC,and IRRI-LCC,can be satisfactorily used for in-season estimation of grain yield for basmati rice.As LCCs are very economical compared with chlorophyll meters and canopy reflectance sensors,they can be preferably used by small farmers in developing countries. 展开更多

In the present scenario, environmental writing becomes a social discourse. This is the result of a new, vibrant and growing environmental constituency in societies such as Brazil, India, Thailand and other Third World Countries. The features of environmental writing include a visible concern for social justice, defence of livelihood and the determining part played by women. This new discourse has emerged with a new focus on redefinition of terms like human rights, common good, democracy and CPRs (common property resources). Environmental Writing becomes a powerful discourse when it protests against the increasingly intrusive and interventionist developmental projects, implemented by the government, which in turn assert dominion over indigenous people of the land. There is also a strong emphasis on prudence among the poor as contrasted to the ecological profligacy of the rich. This paper is an attempt to examine the voices of dissent and the accountability of postmodern writers and environmental activists in India regarding environmental issues in India. 展开更多
