Efficacy and safety of warming Yang and dredging intestines method in metastatic colon cancer maintenance

BACKGROUND There is a lack of integrated Chinese and Western medicine treatment regimens supported by high-level evidence-based medicine in the maintenance therapy phase of metastatic colorectal cancer(mCRC).Based on the traditional Chinese medicine theory of“Yin tumor”,we believe that“Yang does not transform Yin,and it is blocked in the intestines”is the core pathogenesis of mCRC.Based on the basic treatment principle of“warming Yang and dredging intestines”,we developed the Quxie Capsule.Previous randomized controlled clinical trials demonstrated that the Quxie Capsule can significantly prolong the overall survival of patients with mCRC,but it remains to be verified whether the combination of the“warming Yang and dredging intestines method”prescription with Western medicine standard regimen can prolong the efficacy and safety of the mCRC during the period of maintenance therapy.AIM To confirm and clinically validate that the combination of“warming Yang and dredging intestines method”prescription with Western medicine standard regimen can prolong progression-free survival(PFS)during maintenance treatment of mCRC.The safety of“warming Yang and dredging intestines method”prescription is also assessed.METHODS The study has a prospective,open-label,randomized,controlled study design.Patients have been recruited beginning November 2023 from Xiyuan Hospital of China Academy of Chinese Medical Sciences,Guang’anmen Hospital of China Academy of Chinese Medical Sciences,Dongfang Hospital of Beijing University of Chinese Medicine,and Xinjiang Uygur Autonomous Region Hospital of Traditional Chinese Medicine.The study period is from March 2024 to March 2026.After screening in outpatient clinics or wards,subjects who met the inclusion criteria are randomized into the treatment or control group in a 2:1 ratio.The treatment group receives the“warming Yang and dredging intestines method”formula combined with Western standard maintenance therapy.The control group receives Western standard maintenance therapy formulated by the investigators based on the Chinese Society of Clinical Oncology guidelines for colorectal cancer.All participants receive treatment until the occurrence of disease progression,death,or unmanageable adverse effects,with post-treatment monitoring continued until mortality.An independent panel of chief physicians with extensive clinical experience evaluates the progression of the disease.RESULTS This study aims to clarify whether the combination of warming Yang and dredging intestines method formula with standard Western medicine regimens can prolong PFS during maintenance therapy for mCRC and whether the treatment has a favorable safety profile.The goal is to provide a combined Chinese and Western medicine treatment option for clinical physicians and mCRC patients.Notably,with the actual sample size,this study has an 80%probability of detecting a significant difference if a true difference exists.Small sample sizes may lead to increased instability of the results of subgroup analyses,and may also result in findings that are only applicable to patients with characteristics highly similar to those of the present study population(e.g.,specific genotypes,therapeutic backgrounds,etc.),making it difficult to generalize to the broader mCRC population.In the future,it may be possible to expand the sample size based on this study to further validate the efficacy and safety of combining Chinese and Western medicine in the treatment of mCRC.Basic research on the therapeutic combination of warming Yang and dredging intestines method formula and standard Western regimen will be performed in parallel.CONCLUSION This study aims to clarify whether the combination of warming Yang and dredging intestines method formula with standard Western medicine regimens can prolong PFS during maintenance therapy for mCRC and whether the treatment has a favorable safety profile. 展开更多

The number of hydropower stations in China is large, but the service time of hydropower stations is relatively short, and there are still some problems in many aspects. Governor is an indispensable device for hydropower generation, which plays an irreplaceable role in hydropower generation. Once there is a problem with the governor of hydropower station, it will affect the whole power station. In order to reduce the impact of the governor failure of hydropower station, we must analyze the common faults of the governor of hydropower station, and prepare the corresponding maintenance plan for these common faults, which can be repaired quickly in case of failure, so as to minimize the impact on the hydropower station as far as possible. The implementation of artificial intelligence(AI)in a smart society,in which the analysis of human habits is mandatory,requires automated data scheduling and analysis using smart applications,a smart infrastructure,smart systems,and a smart network.In this context,which is characterized by a large gap between training and operative processes,a dedicated method is required to manage and extract the massive amount of data and the related information mining.The method presented in this work aims to reduce this gap with near-zero-failure advanced diagnostics(AD)for smart management,which is exploitable in any context of Society 5.0,thus reducing the risk factors at all management levels and ensuring quality and sustainability.We have also developed innovative applications for a humancentered management system to support scheduling in the maintenance of operative processes,for reducing training costs,for improving production yield,and for creating a human–machine cyberspace for smart infrastructure design.The results obtained in 12 international companies demonstrate a possible global standardization of operative processes,leading to the design of a near-zero-failure intelligent system that is able to learn and upgrade itself.Our new method provides guidance for selecting the new generation of intelligent manufacturing and smart systems in order to optimize human–machine interactions,with the related smart maintenance and education. 展开更多

Currently, the main decision method of maintenance mode is reliability centered maintenance( RCM) logic decision-making process, but the process is a qualitative analysis process. Based on a comprehensive analysis of factors affecting equipment reliability and maintenance work, it adopts a fuzzy synthesis decision method to establish a maintenance decision model,which uses the maximum subordination principle and expert assessment method to determine the maintenance mode of complex equipment. Combined with a concrete example of generators in nuclear power plant,a description of maintenance decision method was proposed in the application of complex equipment. The purpose of this paper is to implement and validate a decision making framework for building maintenance projects. The growing importance of maintenance sector not only in Malaysia but al- so increasingly globalized, the difficulty in selecting an appropriate procurement method and lack of research in this area provide an impetus for this research. There were 19 procurement selection criteria identified. In addition, there were 4 types of procurements methods which include outsourcing by specialist term contract, outsourcing by tendered schedule term contract, out- sourcing by repair and maintenance contract and outsourcing by measured term contract identified to be the most popular procurement methods adapted by public universities currently and at the same time were considered most important. The decision making framework was developed based on AHP technique and principles. Expert Choice Software was employed as development tool where the criteria and alternatives were integrated into the framework. Finally, structured interview was conducted to validate the framework developed. The validation process was carried out through structured interview with selected public university. 展开更多

Maintenance decision problems generally involve multiple criteria which apparently are best addressed using Multi-Criteria Decision Making (MCDM) tools. This paper describes the use of a hybrid MCDM technique in prioritizing maintenance strategy for ship systems. The Hybrid MCDM technique combines Delphi method, AHP and TOPSIS methods. While the Delphi method and AHP are applied in screening of decision criteria and decision criteria weights determination respectively, the TOPSIS method is used in the ranking of alternative maintenance strategies. Five alternative maintenance strategies which include corrected maintenance, scheduled overhaul, scheduled replacement, continuous on-condition task and scheduled on-condition task are considered and the optimum maintenance strategy is selected based on twelve critical maintenance decision criteria. To demonstrate the suitability of the approach a case study of sea water pump of the central cooling system of a marine diesel engine is used. 展开更多

Here, the author conducted a detailed investigation and study on the road of a highway section in Jiangsu province;according to the technical status and disease causes of bridges, tunnels, facilities along the line, testing and maintenance equipment, etc., six issues were discussed including technical durability, structural safety, engineering coordination, maintenance convenience, operation safety and efficient management. According to the characteristics of highway engineering and according to the construction needs of different professions, an integrated countermeasure library of construction, management, maintenance and transportation has been established with six functional requirements of technical durability, structural safety, engineering coordination, maintenance convenience, operation safety and efficient management. According to the multi-level evaluation method, according to the needs of engineering construction and the characteristics of the region, the comprehensive analysis of the scheme was carried out and a comprehensive plan was given. 展开更多

The vast potential of system health monitoring and condition based maintenance on modern commercial aircraft is being realized through the innovative use of Airplane Condition Monitoring System(ACMS) data.However there are few methods addressing the issues of failure prognostics and predictive maintenance for commercial aircraft Air Conditioning System(ACS).This study developed a Bayesian failure prognostics approach using ACMS data for predictive maintenance of ACS.First, a health index characterizing the ACS health state is inferred from a multiple sensor signals using a data driven method.Then a dynamic linear model is proposed to describe the degradation process for failure prognostics.Bayesian inference formulas are carried out for degradation estimation and prediction.The developed approach is applied on a passenger aircraft fleet with ACMS data recorded for one year.The analysis of the case study shows that the developed method can produce satisfactory prognostics results, where all the ACS failure precursors are identified in advance, and the relative errors for the failure time prediction made when just entering the degradation warning stage are less than 8%.This would allow operators to proactively plan future maintenance. 展开更多

A new multi-level analysis method of introducing the super-element modeling method, derived from the multi-level analysis method first proposed by O. F. Hughes, has been proposed in this paper to solve the problem of high time cost in adopting a rational-based optimal design method for ship structural design. Furthermore,the method was verified by its effective application in optimization of the mid-ship section of a container ship. A full 3-D FEM model of a ship,suffering static and quasi-static loads, was used as the analyzing object for evaluating the structural performance of the mid-ship module, including static strength and buckling performance. Research results reveal that this new method could substantially reduce the computational cost of the rational-based optimization problem without decreasing its accuracy, which increases the feasibility and economic efficiency of using a rational-based optimal design method in ship structural design. 展开更多

Airframe maintenance is traditionally performed at scheduled maintenance stops.The decision to repair a fuselage panel is based on a fixed crack size threshold,which allows to ensure the aircraft safety until the next scheduled maintenance stop.With progress in sensor technology and data processing techniques,structural health monitoring(SHM) systems are increasingly being considered in the aviation industry.SHM systems track the aircraft health state continuously,leading to the possibility of planning maintenance based on an actual state of aircraft rather than on a fixed schedule.This paper builds upon a model-based prognostics framework that the authors developed in their previous work,which couples the Extended Kalman filter(EKF) with a firstorder perturbation(FOP) method.By using the information given by this prognostics method,a novel cost driven predictive maintenance(CDPM) policy is proposed,which ensures the aircraft safety while minimizing the maintenance cost.The proposed policy is formally derived based on the trade-off between probabilities of occurrence of scheduled and unscheduled maintenance.A numerical case study simulating the maintenance process of an entire fleet of aircrafts is implemented.Under the condition of assuring the same safety level,the CDPM is compared in terms of cost with two other maintenance policies:scheduled maintenance and threshold based SHM maintenance.The comparison results show CDPM could lead to significant cost savings. 展开更多

This paper is on the suggestion of maintenance items for electric railway facility systems.With the recent increase in the use of electric locomotives,the utilization and importance of railroad electrical facility systems are also increasing,but the railroad electrical facility system in Korea is rapidly aging.To solve this problem,various methodologies are applied to ensure operational reliability and stability for railroad electrical facility systems,but there is a lack of detailed evaluation criteria for railroad electrical facility system maintenance.Also,maintenance items must be selected in a scientific and systematic method.Therefore,railroad electrical facility systems are selected for study.Design StructureMatrix(DSM)is utilized to establish considerations tailored to the maintenance characteristics,and the Fuzzy-TOPSIS methodology is utilized for determining the maintenance detail evaluation item baseline weights,a multi-criteria decision-making problem.Studies show that degradation,insulation items have the highest weight of 14.63%,and capacity items have the lowest weight of 5.34%.The results of this may be contributed to the underlying research in carrying out maintenance activities to ensure the reliability and safety of railroad electrical facility systems. 展开更多

Objective: to study and analyze the value of humanistic care model in patients with maintenance hemodialysis.Method: into our hospital from October 2019 to May 2021 to maintenance hemodialysis treatment of 60 patients, random digital table is divided into research group, control group, each 30 cases, implement routine care, research group to carry out humanistic care care mode intervention, the two groups of patients coping way, hope level, nutritional status, nursing satisfaction evaluation comparison.Results: the MCMQ scale and M C M Q scores were lower than the control group, the Herth Hope scale was higher than the control group, albumin, total protein and proalbumin levels, nursing satisfaction was higher than the controls, and the difference was statistically significant (P <0.05).Conclusion: the application of humanistic care model in maintenance hemodialysis patients can improve patient response, improve nutritional status and nursing satisfaction with high application value. 展开更多
