These records were compared with those of patients who received standardized care without CPAP in the MICU from January 2004 to December 2004. Categorical variables were summarized as frequencies and compared between groups using Fisher's exact test or the Chi-square test. Continuous variables were summarized as medians(interquartile range), and comparison between the groups was made using Wilcoxon's rank-sum test. The relationship between CPAP and intubation rate was determined using multivariable logistic regression analysis of propensity scores. The results were presented as odds ratio(OR), 95% confidence interval(CI), and P value for test effect. The adequacy of the model was calibrated using Hosmer and Lemeshow's goodness-of-fit test. P<0.05 was considered statistically significant.RESULTS: The records of 785 patients were reviewed. Of the 215 patients treated with CPAP in the MICU, 13% were intubated after admission. In contrast, of the 570 patients who did not receive CPAP, 28% were intubated after ED admission. Unadjusted logistic regression analysis showed that patients who had been treated with CPAP were less likely to be intubated than those without CPAP treatment(OR=0.37, 95% CI, 0.24–0.57, P<0.0001). With propensity scores adjusted, multivariate logistic regression analysis showed that CPAP treatment was associated with a 62% reduction of intubation(OR=0.384, 95%CI, 0.25–0.60, P≤0.0001).CONCLUSIONS: In patients with acute respiratory disorder, there was a relationship between CPAP therapy and the decreased intubation rate. CPAP therapy was feasible in prehospital management of patients with respiratory distress.展开更多

文摘To investigate the efficacy of a structured continuous nursing intervention program on clinical outcomes,self-efficacy,and sleep quality in patients diagnosed with aplastic anemia(AA).Methods:This study was a single-center,parallel-group,randomized controlled trial.A total of 64 patients with AA,treated at our hospital’s hematology department from May 2022 to May 2025,were enrolled.Participants were randomly allocated in a 1:1 ratio to either the control group(n=32),receiving routine nursing care,or the intervention group(n=32),receiving a six-month continuous nursing intervention program in addition to routine care. The intervention program consisted of structured health education, individualized psychological support, regular telephone follow-ups,and a 24/7 online communication platform. The primary outcome was the change in the Pittsburgh Sleep Quality Index(PSQI)score from baseline to six months.Secondary outcomes included hematological parameters(hemoglobin[Hb],absolute neutrophil count[ANC],platelet count[PLT]),incidence of adverse events(infections, bleeding episodes),quality of life(assessed by the Functional Assessment of Cancer Therapy–Anemia[FACT-An]),and self-efficacy(assessed by the General Self-Efficacy Scale[GSE]).Statistical analyses were performed using independent t-tests,and chi-square tests or Fisher’s exact tests as appropriate. Results:A total of 55 patients(28 in the intervention group and 27 in the control group)completed the study. At baseline, there were no statistically significant differences in demographic data, clinical characteristics,or outcome measures between the two groups(p>0.05).After six months, the mean PSQI score in the intervention group was significantly lower than that in the control group(7.12±2.05 vs.13.49±2.88;t=−13.450;p<0.001).The intervention group also demonstrated a significantly lower incidence of infections(4/28,14.29%vs.10/27,37.04%;p=0.048)and bleeding episodes requiring intervention(3/28,10.71%vs.9/27,33.33%;p=0.042).Furthermore,patients in the intervention group reported significantly higher scores on the FACT-An(125.70±10.31 vs.109.44±12.10;t=7.934;p<0.001)and the GSE scale(29.82±4.11 vs.23.51±4.80;t=7.311;p<0.001).While hematological parameters showed a trend toward improvement in the intervention group, the differences were not statistically significant compared to the control group(p>0.05).Conclusion:The implementation of a structured continuous nursing intervention program can significantly improve sleep quality, reduce the incidence of complications, and enhance quality of life and self-efficacy in patients with aplastic anemia. This evidence-based model of care should be considered for integration into standard clinical practice for the long-term management of this patient population.
文摘BACKGROUND: To determine whether the prehospital use of continuous positive airway pressure(CPAP) therapy is associated with a reduced rate of endotracheal intubation in patients with an acute respiratory disorder brought to the emergency department(ED).METHODS: We reviewed medical records of patients with acute respiratory distress who had been treated with CPAP in the Mobile Intensive Care Unit(MICU) from January 2010 to December 2011. CPAP therapy was feasible in prehospital management of patients with respiratory distress.

文摘In this paper we study multi-dimensional mean-field backward doubly stochastic differential equations(BDSDEs),that is,BDSDEs whose coefficients depend not only on the solution processes but also on their law.The first part of the paper is devoted to the comparison theorem for multi-dimensional mean-field BDSDEs with Lipschitz conditions. With the help of the comparison result for the Lipschitz case we prove the existence of a solution for multi-dimensional mean-field BDSDEs with an only continuous drift coefficient of linear growth, and we also extend the comparison theorem to such BDSDEs with a continuous coefficient.
文摘Objective: to analyze and study the influence of continuous midwifery service nursing on the rate of non drug analgesia delivery and natural delivery of primipara. Methods: 90 primiparas were randomly selected as the research object, and randomly divided into control group and observation group, with 45 cases in each group. The control group was given routine nursing, and the observation group was given continuous midwifery nursing. The effects of the two nursing methods on the choice of delivery mode, delivery process, postpartum bleeding and satisfaction of primiparas were compared. Results: the natural delivery rate in the observation group was significantly higher than that in the control group (P < 0.05). The delivery time and bleeding volume in the observation group were significantly lower than those in the control group (P < 0.05). The maternal satisfaction in the observation group was significantly higher than that in the control group (P < 0.05). Conclusion: after the intervention of continuous midwifery nursing, more primiparas choose natural delivery and complete it smoothly, which is conducive to improve the nursing satisfaction of primiparas.
