Influence of cognitive behavioral therapy-based psychological interventions on psychological well-being and quality of life among laryngeal carcinoma patients

BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence,implementing customized interventions to ease adverse emotional states is imperative.AIM To evaluate the efficacy of cognitive behavioral therapy(CBT)-based psychological interventions in improving the psychological well-being and quality of life(QoL)of patients with laryngeal carcinoma.METHODS This study enrolled 120 patients admitted from February 2022 to February 2024.The control group,comprising 50 participants,received standard supportive psychological care,while the research group,consisting 70 participants,underwent CBT-based interventions.Several clinical outcomes were systematically assessed that included postoperative recovery metrics(duration of tracheostomy and nasogastric tube dependence and length of hospitalization),psychological status(Self-Rating Anxiety Scale and Self-Rating Depression Scale),nutritional markers(serum albumin and hemoglobin levels),sleep quality(Self-Rating Scale of Sleep and Athens Insomnia Scale),and QoL(Functional Assessment of Cancer Therapy-Head and Neck).RESULTS The results demonstrated that the research group experienced superior outcomes,with significantly reduced durations of tracheostomy and nasogastric tube dependence,as well as shorter hospital stays,compared with the control group.Additionally,the research group exhibited markedly lower post-intervention Self-Rating Anxiety Scale,Self-Rating Depression Scale,Self-Rating Scale of Sleep,and Athens Insomnia Scale scores,along with minimal but higher change in serum albumin and hemoglobin levels compared with the control group.All five domains of Functional Assessment of Cancer Therapy-Head and Neck showed notable improvements in the research group,exceeding those observed in the control group.CONCLUSION CBT-based psychological support positively affects the mental well-being and QoL of patients with laryngeal carcinoma,highlighting its potential for broader clinical application. 展开更多

Objective:To explore the effect of cognitive intervention combined with aerobic limb rehabilitation exercise on neurological and limb functions in hemiplegic patients with hypertensive intracerebral hemorrhage.Methods:A prospective study was conducted with eighty-six hemiplegic patients with hypertensive intracerebral hemorrhage admitted to Taikang Xianlin Drum Tower Hospital from October 2021 to October 2023.The patients were randomly divided into a study group(43 cases)and a control group(43 cases).After 6 months of intervention,neurological function,motor and limb function,psychological state,and quality of life scores were compared between the two groups to assess the effects of the intervention.Results:After the intervention,neurological function indexes in the study group,including S100B protein,myelin basic protein and National Institutes of Health Stroke Scale score,were significantly lower than those in the control group(all P<0.05).In contrast,brain-derived neurotrophic factor and Mini-Mental State Examination scores were significantly higher in the study group(all P<0.05).Additionally,motor function,Barthel score,Fugl-Meyer score,and muscle strength(grades V and IV)were significantly higher in the study group compared to the control group(all P<0.05).Psychological resilience,including scores for toughness,optimism,and strength,as well as the total resilience score,were significantly higher in the study group(all P<0.05).Moreover,the Generic Quality of Life Inventory score was also significantly higher in the study group(P<0.05).Conclusion:Cognitive intervention combined with aerobic limb rehabilitation exercises can alleviate neurological damage in hemiplegic patients with hypertensive intracerebral hemorrhage,enhance muscle strength,promote limb and motor function recovery,and improve psychological state and quality of life. 展开更多

In recent years, cognitive difficulties associated with normal aging and dementia have been receiving increased attention from both public and scientific communities. With an increase in overall lifespan, promoting healthy cognition has become a priority and a necessity for minimizing and preventing individual and societal burdens associated with cognitive dysfunctions in the elderly. The general awareness concerning the efficacy of preventive(e.g., lifestyles) and palliative treatment strategies of cognitive impairments, related to either healthy or unhealthy trajectories in cognitive aging, is continuously rising. There are several therapeutic strategies which can be broadly classified as either pharmacological or non-pharmacological/psychosocial. In face of the modest evidence for success of pharmacological treatments, especially for dementia related impairments, psychosocial interventions are progressively considered as a complementary treatment. Despite the relative spread of psychosocial interventions in clinical settings, research in this area is rather scarce with evidence for success of these therapies remaining controversial. In this work we provide an evidence based perspective on cognitive intervention(s) for healthy aging, pre-dementia(mild cognitive impairment), and dementia populations. Current evidence and future directions for improving cognitive functions in the elderly are discussed as well. 展开更多

Background Cognitive impairment (CI) increases cardiac mortality among very elderly patients. Percutaneous coronary intervention (PCI) for ischemic heart disease (IHD) patients is considered a favorable strategy for decreasing cardiac mortality. Here, we investigated the influence of CI on cardiac mortality after PCI in very elderly patients. Methods We performed a retrospective observational analysis of patients who received PCI between 2012 and 2014 at the South Miyagi Medical Center, Japan. IHD patients over 80 years old who underwent the Mini-Mental State Examination for CI screening during hospitalization and/or who had been diagnosed with CI were included. Participants were divided into CI and non-CI groups, and cardiac mortality and incidence of adverse cardiac events in a 3-year follow-up period were compared between groups. Statistical analyses were performed using the t-test,χ^2 test, and multivariable Cox regression analysis, with major comorbid illness and conventional cardiac risk factors as confounders. Results Of 565 patients, 95 were included (41 CI, 54 non-CI). Cardiac mortality during the follow-up period was significantly higher in the CI group (36%) compared with the non-CI group (13%)(OR = 4.3, 95% CI: 1.56–11.82, P < 0.05). CI was an independent cardiac prognostic factor after PCI and, for CI patients, living only with a CI partner was an independent predictor of cardiac death within three years. Conclusions CI significantly affected cardiac prognosis after PCI in very elderly patients, particularly those living with a CI partner. To improve patients’ prognoses, social background should be considered alongside conventional medical measures. 展开更多

BACKGROUND Surgery is an effective method for treating certain diseases.Factors such as disease,preoperative fear and tension,surgical stress,postoperative pain,and related complications directly affect the smooth progression and outcome of surgery.Patients may experience a series of psychological and physiological changes during the perioperative period,resulting in anxiety and depression,which may reduce the pain threshold and worsen their prognosis.METHODS We enrolled 200 patients who underwent surgical care at The First People’s Hospital of Lin’an District,Hangzhou between January and December 2023.They were categorized into a routine intervention group(n=103)and a psychological intervention group(n=97),based on the intervention strategies used.Various assessment tools,including the self-rating anxiety scale(SAS),the self-rating depression scale(SDS),and the Connor–Davidson Resilience scale,were used to measure patients’negative states and emotions.The pre-and post-intervention scores for these metrics in the two groups were then analyzed.RESULTS In the psychological intervention group,the SAS and SDS scores(31.56±5.18 and 31.46±4.57,respectively)were significantly reduced compared to the routine intervention group(P<0.05).The visual analog scale pain scores at 12 and 24 hours after intervention(6.85±1.21,4.24±0.72)were notably higher than those in the routine intervention group(P<0.05).The psychological intervention group also demonstrated superior scores in perseverance(36.08±3.29),self-reliance(22.63±2.91),optimism(11.42±1.98),and resilience(70.13±5.37),compared to the routine intervention group(P<0.05).Additionally,the psychological intervention group’s confrontation score(23.16±4.29)was higher(P<0.05).This group also reported lower scores in avoidance(9.28±1.94)and yielding(6.19±1.92)(P<0.05).Lastly,the Short Form 36 Health Survey scores were significantly higher in the psychological intervention group,indicating a better quality of life(P<0.05).CONCLUSION Psychological intervention measures based on SCT can effectively alleviate pain,anxiety,and depression in periop-erative patients. 展开更多

BACKGROUND The intervention value of a drug-psycho-social-skill model on medication compliance and cognitive ability in patients with chronic schizophrenia was unknown.AIM To explore the intervention value of a drug-psycho-social-skill model on medication compliance and cognitive ability in patients with chronic schizophrenia.METHODS Overall,98 out-patients and in-patients with chronic schizophrenia treated in our hospital from February 2022 to January 2023 were included and randomly divided into the study(50 patients)and control groups(48 patients).For 3 months,the control group was treated with conventional drugs,and the research group with a comprehensive intervention model of drug-psychology-society-skills.Data were obtained for the mini mental assessment scale(MMSE),Montreal cognitive assessment scale(MoCA),positive and negative symptom scale,insight and treatment attitude scale,cognitive ability scale and social skills[personal and social function scale(PSP)].The adverse reactions were observed.RESULTS After the intervention,the MMSE and MoCA scores improved in both groups.MoCA scores in the study group(26.58±3.21)were significantly(P<0.05)higher than those in the control group(24.68±3.02),MMSE scores were not significantly higher.Post-intervention,positive and negative symptom scores improved in both groups,and the positive and negative symptom scores in the study group[(12.01±2.58)and(32.51±2.11)]were significantly(P<0.05)different than those in the control group[(14.54±2.33)and(33.74±2.55)].Post-intervention,insight and treatment attitudes questionnaire scores of both groups were improved and compared with the control group(7.97±3.02),the study group(13.56±6.35)had significantly(P<0.05)higher scores.Post-intervention,the MATRICS consensus cognitive battery score of both groups was improved and compared with the control group(38.44±6.23),the score of the study group was significantly(P<0.05)increased(43.51±6.01).Post-intervention,the PSP score of the study group(78.38±6.63)was significantly(P<0.05)higher than that of the control group(74.52±7.01).During the intervention period,the incidence of adverse reactions in the study group was 6.25%,not significantly different from that in the control group(8.33%).During the intervention,both groups experienced adverse reactions,with no significant difference between groups(P>0.05).CONCLUSION The comprehensive intervention model based on drug-psychology-society-skills has obvious intervention effects on patients with chronic schizophrenia,which improves their cognitive ability and reduces their positive and negative symptoms.Simultaneously,it improves the self-knowledge of patients,improves their attitude toward treatment,effectively promotes the recovery of patients'social functions,and is safe.Therefore,it is worthy of being vigorously promoted and widely used in clinics. 展开更多

BACKGROUND As lifestyles continue to change worldwide,the incidence of digestive tract carcinoma has gradually increased.Digestive endoscopy is an important tool that can assist in the diagnosis,treatment,and surgical intervention for this disease.However,the examination process is affected by many factors,and patient cooperation is often poor,which can increase the risk of complications.AIM To explore the effects of integrated management and cognitive intervention on cooperation and complications in patients undergoing endoscopy for early gastrointestinal neoplasms.METHODS A total of 354 patients with early stage gastrointestinal cancer who underwent digestive endoscopy procedures between January and December 2023 at our hospital were divided into observation and control groups(177 patients in each group)in a randomized controlled blind trial.The control group received routine interventions,while the observation group received comprehensive integrated management combined with cognitive interventions.We compared the changes in adverse mood,discomfort,examination time,cooperation with the examination,and complications before and after the intervention between the two groups.RESULTS The self-rated anxiety and depression scale scores were lower in the observation group than in the control group(P<0.05).The visual analog scale scores for discomfort during intubation and examination were also lower in the observation group than in the control group(P<0.05).Furthermore,the examination time was shorter in the observation group than in the control group(P<0.05),and the degree of cooperation(94.35%)was higher in the observation group than in the control group(84.75%;P<0.05).Lastly,the incidence rates of gastrointestinal adverse reactions(10.17%vs 20.34%),choking agitation(14.69%vs 24.86%),abdominal pain(8.47%vs 18.08%),and muscle tension(5.08%vs 14.12%)were all lower in the observation group than in the control group(P<0.05).CONCLUSION Integrated management and cognitive intervention in early gastrointestinal neoplasm endoscopy alleviate mood,reduce discomfort,shorten examinations,improve cooperation,and reduce complications. 展开更多

Web-based cognitive interventions on subjective cognitive impairment in cancer survivors:A systemic review

Objective:Cancer survivors have experienced subjective cognitive impairment(SCI)when they received cancer diagnoses or treatments.Their psychosocial and emotional statuses were also impacted.With the advancement of web technologies,web-based cognitive interventions have been implemented in the management and the alleviation of the SCI,the psychosocial distress,and the emotional distress in cancer survivors.This review aimed to summarize the intervention contents of web-based cognitive interventions for SCI,and to explore the effects of the interventions on SCI,psychosocial status,and emotional health.Methods:Six databases(CINAHL Plus,Cochrane Library,Embase,APA PsycInfo,PubMed and CNKI)were searched from the establishment of databases up to December 2023.Literature references were also manually searched for related articles.Results:This review contained 21 studies that covered the contents of web-based cognitive interventions,such as computer-assisted cognitive training,online cognitive rehabilitation,cognitive behavior therapy with the Internet,telehealth physical exercise,and web-based mindfulness interventions.The effects of web-based cognitive interventions positively impacted SCI for cancer survivors.Also,these interventions showed varying degrees of effectiveness in alleviating psychosocial and emotional distresses.Conclusion:By summarizingfive types of cognitive intervention contents delivered via web technology,this review demonstrated that web-based cognitive interventions optimized SCI and overall psychosocial and emotional statuses for the cancer survivors.It is recommended that future research focus on the development of customized web-based cognitive interventions for individuals with SCI,along with their psychosocial and emotional statuses. 展开更多

Objective: To summarize and evaluate the evidence of guidelines and systematic reviews(SRs) of nonpharmacological interventions for mild cognitive impairment(MCI) to support the development of future guidelines and clinical decisions for MCI patients.Methods: Scottish Intercollegiate Guideline Network(SIGN), National Institute for Health and Clinical Excellence(NICE), American Academy of Neurology(AAN), Registered Nurses Association of Ontario(RNAO), Web of Science, PubMed, Cochrane Library, CNAHL, VIP, China National Knowledge Infrastructure(CNKI), and Wanfang Database were searched for relevant publications, including guidelines and SRs, from January 2014 to March 2019. Two authors independently screened articles, extracted data, and assessed the publications for adherence to the inclusion criteria. Appraisal of Guidelines for Research and Evaluation(AGREE II) was used to assess the quality of the guidelines, and Assessment of Multiple Systematic Reviews(AMSTAR 2) was used to assess the quality of SRs. In addition, the Grading of Recommendations Assessment, Development and Evaluation(GRADE) approach was used to evaluate the quality of outcomes.Results: Thirty-two articles were retrieved, including 1 guideline and 31 SRs. Fourteen SRs of physical exercise for MCI, six articles describing cognitive interventions, four articles describing acupuncture, and seven articles assessing dietary interventions(including four articles employing a Mediterranean diet, one article using vitamin B supplementation, and two articles assessing the effects of tea, coffee, and caffeine) were included. The quality of the articles was very low for 4(13%), low for 10(32%), and moderate for 17(55%).Conclusions: Based on the evidence available to date, nonpharmacological interventions may improve the current cognitive function of persons with MCI. In particular, physical exercise, cognitive interventions, and acupuncture exerted promising effects. However, due to the limited number and quality of the included publications, additional high-quality reviews are needed to further confirm. 展开更多

Two motor-cognitive interventions were implemented to evaluate their efficacy in cognitive improvement in Mexican older adults 65 years and more. The intervention group received a dance video games plus a cognitive task (dual-task);the comparison group received only the dance video game, in sessions of 45 minutes, 3 times a week, for 12 weeks. The Barthel Index and Lawton and Brody Index, the Center for Epidemiologic Studies Depression Scale-Revised were applied the Digit Span Test, the Stroop Test, and Color Trail Making. Gait parameters were assessed by GAITRite? electronic walkway (CIR Systems). The design was quasi-experimental. Sites were randomly assigned to intervention groups. The intervention group started with 32 participants and finished with 15;the comparison group started with 20 participants and finished with 18. Participants in the intervention group had to mimic the dance movement of a video game and, after the second week to name progressively three objects without stopping dancing. A multivariate repeated measures model (MANO-VA) was fit with four variables. A time-by-group interaction was observed in the Stroop test, and Digits Span Backwards was in favor of the dual task group. The Stroop test, Digit Span Backwards, gait speed, and step length showed effect time. Both groups improved in gait speed and step length by the end of the intervention. Results show it is feasible for Mexican older adults with little schooling to perform dual tasks and improve cognitive tasks and gait speed. Limitations were high attrition due to unforeseen situations and small sample size. 展开更多

Objectives: This study aimed to understand the experience and impact of a physical activity and sleep wrist-worn tracker (Fitbit)-based healthy lifestyle intervention for older patients attending a memory assessment service, who are experiencing cognitive impairment but do not receive a dementia diagnosis. Methods: A qualitative design was employed. Semi-structured interviews were conducted with a purposeful sample of thirteen participants recruited from a memory assessment service. Thematic analysis, that was data driven and inductive, was undertaken to analyse the data. Results: Two global themes were developed. “Understanding exercise and sleep as part of my lifestyle” was made up of themes representing how participants viewed exercise and sleep as part of their lifestyles in terms of acknowledging the positive impacts and the barriers to exercise and sleep. The second global theme “Understanding my experience of the healthy lifestyle intervention” was made up of themes that identified the positive impact of the intervention regarding improving health and wellbeing, enabling validation of proactive behaviours and motivation to engage in healthy lifestyle behaviours, so promoting positive behaviour change. Conclusion: Patients experiencing age-related cognitive impairment, applied and benefited from a healthy lifestyle Fitbit-based intervention to facilitate and promote physical activity, better sleep hygiene and healthy lifestyles. 展开更多

Methods: SCL-90, EPQ-N and cognitive behavior intervention were performed on 30 patients with PTSD, 30 patients without PTSD and 30 normal subjects, and their fasting blood was collected to analyze cortisol in the morning. Results: The scores of SCL-90 and EPQ-N in patients with breast cancer PTSD were significantly higher than those in non-PTSD patients and normal subjects (P < 0.01). After cognitive behavioral intervention, the plasma cortisol level of breast cancer PTSD patients was significantly decreased (P Conclusion: Compared with non-PTSD breast cancer patients and normal control group, breast cancer PTSD patients had more serious psychological problems, worse emotional stability and higher plasma cortisol level. This paper examines the bidirectional relationship between heart failure (HF) and cognitive impairment, underscoring the need for integrated intervention strategies to address these interconnected conditions effectively. Cognitive deficits often hinder the effective management of HF, leading to poorer treatment adherence and health outcomes, while the physiological stress of HF can further impair cognitive function, creating a complex interplay that complicates patient care. This study highlights the effectiveness of angiotensin-converting enzyme (ACE) inhibitors and cardiac resynchronization therapy (CRT) in improving both cardiac and cognitive functions. By targeting the underlying physiological and neurobiological mechanisms of HF, these therapies enhance patient outcomes, leading to better adherence to treatment regimens and overall quality of life. Furthermore, the findings suggest that regular cognitive assessments should be integrated into HF management protocols, enabling early identification of cognitive impairment and timely intervention. Incorporating ACE inhibitors and CRT into standard care practices not only addresses the complexities of managing HF and cognitive decline but also fosters a holistic approach to patient health. Reporting quality evaluation of meta-analyses for cognitive interventions among Alzheimer's dementia

Background:To appraise the reporting quality of systematic reviews as well as meta-analyses of cognitive interventions among Alzheimer's disease using the PRISMA statement,and to investigate the related factors influencing the reporting quality.Method:Web of Science,Pubmed,Embase,Medline,as well as Cochrane Library were searched to collect the systematic reviews as well as meta-analyses of cognitive interventions among Alzheimer's disease from inception to September 17th 2019.The PRISMA statement was used to appraise their reporting quality and investigate the compliance with each PRISMA item.Furthermore,the potential factors affecting the reporting quality were analyzed based on the univariate analysis.Results:Seventeen articles were included.The mean compliance with the PRISMA statement was 74.86% and the standard deviation was 0.13.The univariate analysis indicated that the number of authors(P=0.019)might be a factor affecting the reporting quality.However,the journal impact factor(P=0.133),the number of studies included(P=0.170),the number of databases retrieved(P=0.201),funding report(P=0.228),and original studies(randomized controlled trials only)(P=0.322)might not affect the reporting quality.Conclusion:The overall reporting quality was high,but the compliance with the 11 PRISMA items,such as"structured summary"and"protocol and registration"was low.Future research should focus on adequate reporting of these items in the hope that readers will be better able to discern the accuracy and reliability of relevant information and make correct decisions. 展开更多

Influence of Intensive Cognitive Intervention on Compliance, Self-management and Lifestyle in Hospitalized Elderly Hypertensive Patients

Objective: to observe the effect of intensive cognitive intervention on treatment compliance, self-management and lifestyle in hospitalized elderly hypertensive patients. Methods: from January 2021 to January 2022, members of this research group collected 120 elderly hypertensive patients in hospital in the fourth department of general internal medicine, First Affiliated Hospital of Xinjiang Medical University, numbered all patients, and then divided them into controlgroup and observation group, and each group included 60 cases. The control group received routine intervention, and the observation group received intensive cognitive intervention. Comparison of treatment adheres to self-management scores, and lifestyle. Results: after nursing intervention of all patients, the comparative analysis of various observation indicators showed that the treatment compliance of the observation group was higher than that of the control group (95.5% vs 81.7%), and the salt intake score in the self-management score (8.27±0.91 vs 7.51± 0.88 points), doctor-patient communication score (8.02±0.95 vs 7.01±0.87 points), cognitive symptom management score (9.03±0.67 vs 8.14±0.91 points), endurance exercise score (8.98±0.82 vs 7.84±0.83 points) and the scores of physical exercise (8.83±0.66 vs 8.02±0.71) were higher than those of the control group, and the proportion of reasonable diet in lifestyle (90.0% vs 73.3%), the proportion of smoking cessation and alcohol restriction (95.0% vs 81.7%) and the proportion of reasonable exercise ( 86.7% vs 70.0%) were higher than those of the control group in hospitalized elderly hypertensive patients, with statistical difference (Pall <0.05). Conclusion: intensive cognitive intervention can improve the treatment compliance and self-management level of hospitalized elderly hypertensive patients, and promote the patients to establish a healthy lifestyle. 展开更多
