Objective:To identify the root causes of typical adverse drug events through the lens of patient experiences proposing novel strategies to mitigate preventable harm.Methods:A qualitative case study leveraging in-depth...Objective:To identify the root causes of typical adverse drug events through the lens of patient experiences proposing novel strategies to mitigate preventable harm.Methods:A qualitative case study leveraging in-depth interviews with patients and families,anchored by Interactive Patient Par ticipation Theory,to analyze 4 high-severity adverse drug events(ADE)cases.Cases were purposively sampled from 8 communities in China's National Adverse Event Monitor Center(2018-2023).Semi-structured interviews explored patient perspectives,with data analyzed via thematic coding and triangulation against clinical records.Results:Five interconnected themes emerged:(1)erosion of trust,(2)communication breakdowns,(3)information asymmetry,(4)environmental inadequacies,and(5)technological alienation.Notably,75% of participants had≤high school education,and 50% used≥7 medications daily,compounding ADE risks.Conclusions:We considered elements mentioned by theory,exploring trust,communication,information,and suppor t as the root causes.In addition,we added“adaptability to new technology”as an impor tant and necessary component.It is impor tant and necessary to analyze typical adverse drug events from the perspectives of patients.展开更多
AIM:To evaluate potential risk factors in the development of ulcerative colitis(UC) in China.METHODS:A total of 1308 patients with UC and 1308 age-matched and sex-matched controls were prospectively studied in China.T...AIM:To evaluate potential risk factors in the development of ulcerative colitis(UC) in China.METHODS:A total of 1308 patients with UC and 1308 age-matched and sex-matched controls were prospectively studied in China.The UC cases were collected from 17 hospitals in China from April 2007 to April 2010.Uniform questionnaires were designed to investigate risk factors including smoking,appendectomy,stress,socio-economic conditions,nonsteroidal antiinflammatory drugs(NSAIDs),oral contraceptives,diet,breastfeeding,infections and family sanitary conditions.Group comparisons by each factor were done using simple logistic regression analysis.Conditional logistic regression was used for multivariate analysis.RESULTS:By univariate analysis,the variables predictive of UC included feeling stress,light and heavy alcoholic drinking,spicy food,sugar consumption and infectious diarrhea,while heavy tea intake and tap water consumption were protective against UC.On multivariate analysis,the protective factor for UC was tap water consumption [odds ratios(OR) = 0.424,95%CI:0.302-0.594,P < 0.001];while the potential risk factors for UC were heavy sugar consumption(OR = 1.632,95%CI:1.156-2.305,P < 0.001),spicy food(light intake:OR = 3.329,95%CI:2.282-4.857,P < 0.001;heavy intake:OR = 3.979,95%CI:2.700-5.863,P < 0.001),and often feeling stress(OR = 1.981,95%CI:1.447-2.711,P < 0.001).Other factors,such as smoking habit,appendectomy,breastfeeding,a history of measles,rural or urban residence,education,oral contraceptives,and NSAID use have not been found to have a significant association with the development of UC in the present study.CONCLUSION:Our study showed tap water consumption was a protective factor for UC,while spicy food,heavy sugar consumption and often feeling stress were risk factors for UC in this Chinese population.展开更多

Coal bursts pose significant safety and operational challenges in deep mining environments,necessitating effective mitigation strategies to address high-stress concentrations and dynamic failure risks.This study evalu...Coal bursts pose significant safety and operational challenges in deep mining environments,necessitating effective mitigation strategies to address high-stress concentrations and dynamic failure risks.This study evaluated the efficacy of hydraulic fracturing as a preconditioning tool at a longwall face of the Mengcun coal mine with strong coal bursts,Shaanxi Province.The program involved the systematic creation of a fracture network through high-pressure fluid injection,monitored via microseismic arrays,stress measurements,and hydrological sensors.Results demonstrated that hydraulic fracturing effectively redistributed in-situ stresses,reducing high-stress concentrations by up to 30%,lowering the frequency of high-energy microseismic events,and enhancing the stability of fractured zones.However,the presence of unfractured blind spots and interactions with pre-existing faults highlighted the need for optimized well placement and adaptive fracturing designs.These findings underscore the potential of hydraulic fracturing as a critical preconditioning tool in high-stress mining operations,which could provide a framework for improving safety and efficiency in similar geological and operational settings.展开更多

Brighton pediatric early warning score(PEWS) is a disease risk assessment system that is simple and easy to operate, which has good sensitivity and specificity in disease recognition among children. Because detection indicators vary widely in children, a single indicator is difficult to assess the posttreatment status of children with sepsis.AIM To investigate the relationship between serological markers, Brighton PEWS, and death in children with sepsis after treatment.METHODS A total of 205 children diagnosed with sepsis at our hospital were enrolled. The baseline data, serum scores, and PEWS scores were recorded. In the nested casecontrol study, children who died during the study period were included in an observation group. According to the matching principle, the children who were not dead in the same cohort were included in a control group. The influencing factors of death in children with sepsis after treatment and the value of each evaluation index in predicting the prognosis of children were analyzed.RESULTS A total of 96 children were enrolled in the study, including 48 each in the observation group and the control group. Multivariate logistic regression analysis indicated that antibacterial treatments within 1 h(P = 0.017), shock(P = 0.044),multiple organ dysfunction syndrome(MODS)(P = 0.027), serum procalcitonin(PCT)(P = 0.047), serum albumin(ALB)(P = 0.024), and PEWS(P = 0.012) were independent risk factors for the death of children with sepsis. The area under the curve of the combination of ALB, PCT, and PEWS to predict the death in children with sepsis was the highest(0.908).CONCLUSION Antibacterial treatments within 1 h, shock, MODS, PCT, ALB, and PEWS are independent risk factors for the death of children with sepsis. The predictive accuracy of the combination of PCT, ALB, and PEWS for the prognosis of children with sepsis is the best.展开更多

Objective This case-control study aimed to investigate whether diabetes mellitus(DM),hypertension,hyperlipidemia,and chronic viral hepatitis are risk factors for multiple myeloma(MM).Moreover,the clinical characterist...Objective This case-control study aimed to investigate whether diabetes mellitus(DM),hypertension,hyperlipidemia,and chronic viral hepatitis are risk factors for multiple myeloma(MM).Moreover,the clinical characteristics of MM patients with or without the abovementioned exposure factors were analyzed.Methods In total,340 MM patients and 680 patients with benign diseases who were hospitalized from January 2012 to December 2017 were classified under the case group and control group,respectively.Data about medical history of DM,hypertension,hyperlipidemia and chronic viral hepatitis were collected by reviewing medical records.Univariate and multivariate analyses were conducted to compare the history of DM,hypertension,hyperlipidemia,and viral hepatitis between the two groups.Considering DM,hypertension,hyperlipidemia,and chronic viral hepatitis as exposure factors,clinical characteristics,such as renal function and presence of fungal and other types of infections,between the exposed and nonexposed groups were analyzed.Results No significant difference was observed in the prevalence of DM,hypertension,and hyperlipidemia between the case and control groups.MM patients had a higher prevalence of chronic viral hepatitis than those with benign diseases.No significant difference was observed in the prevalence of renal dysfunction,fungal infection,and non-fungal infections in MM patients with or without DM,hypertension,and hyperlipidemia.MM patients with chronic viral hepatitis had a significantly higher prevalence of nonfungal infections during hospitalization than those without.Conclusion No significant association was noted between MM and DM,hypertension,and hyperlipidemia.Chronic viral hepatitis is correlated to a significantly higher risk of MM,and MM patients with chronic viral hepatitis were more susceptible to non-fungal infections during hospitalization.Although a non-significant trend was observed in this study,we believe that DM and hypertension might be associated with a higher risk of MM.Thus,large-scale studies must be conducted to validate the results of the current study.展开更多

Initially invented to replace the need for physical data cables, Bluetooth offers users a quick and easy way to share data files over a wireless network. Traffic engineers and transportation engineering researchers have utilized the potential opportunities that exist with Bluetooth and have implemented this technology into traffic monitoring techniques. To gain a better understanding of Bluetooth sensors and how they work, a comprehensive literature search was conducted. Twenty-five articles were studied regarding case studies of Bluetooth sensor implementation for travel time measurement. Besides reviewing the literature and previous case studies, three new case studies in the State of Delaware, USA, were also conducted and carefully analyzed. The benefits and drawbacks associated with Bluetooth technology for travel time measurements have been identified in this paper. The overall conclusion of the authors is Bluetooth alone and by itself is not a proper technology for travel time measurements. More studies need to be conducted on the accuracy and overall application, before one can confidently utilize the Bluetooth technology for travel time measurements.展开更多

Managing the acute problem of environmental deterioration due to the industrialization of modern China is necessary.Using three cases from a theoretical perspective of procedure-structure,this study explored how envir...Managing the acute problem of environmental deterioration due to the industrialization of modern China is necessary.Using three cases from a theoretical perspective of procedure-structure,this study explored how environmental deterioration is governed in three circumstances.Research findings revealed that the pattern to treat the worsening environment is associated closely with individual-specific circumstances,and thus three patterns of governance are developed:bureaucratic,contractual,and network-based cooperatives.The cases and the results are described the advantages and disadvantages of these models are discussed.Suggestions on how to more effectively manage these urgent problems are as follows:first,the governance must fit the circumstance,namely,its physical,biological,and social attributes;second,the regulations and conditions regarding the government-dominated pattern are came up from the perspective of mega-governance,mutual trust contracts,benefit sharing,and social capital;and third,the government should abandon the pattern based on the single subject and adopt a plural subject governance model to integrate all the relevant forces to facilitate the attainment of benign environmental governance.展开更多

The area under the curve of the combination of ALB, PCT, and PEWS to predict the death in children with sepsis was the highest(0.908).CONCLUSION Antibacterial treatments within 1 h, shock, MODS, PCT, ALB, and PEWS are independent risk factors for the death of children with sepsis. The predictive accuracy of the combination of PCT, ALB, and PEWS for the prognosis of children with sepsis is the best.

文摘This paper deals with reduction of losses in electric power distribution system through a dynamic reconfiguration case study of a grid in the city of Mostar,Bosnia and Herzegovina.The proposed solution is based on a nonlinear model predictive control algorithm which determines the optimal switching operations of the distribution system.The goal of the control algorithm is to find the optimal radial network topology which minimizes cumulative active power losses and maximizes voltages across the network while simultaneously satisfying all system constraints.The optimization results are validated through multiple simulations(using real power demand data collected for a few characteristic days during winter and summer)which demonstrate the efficiency and usefulness of the developed control algorithm in reducing the grid losses by up to 14%.

文摘As an important branch of traditional medicine,Zhuang Medicine is renowned for its unique prescriptions and techniques in clinical treatment,with significant therapeutic effects and widespread popularity.This article introduces Professor Fang Gang’s experience in treating obesity-type polycystic ovary syndrome(PCOS)with modified Zhuang Medicine Mao Xia Yin.Professor Fang Gang proposes that the etiology and pathogenesis of obesity-type polycystic ovary syndrome(PCOS)primarily stem from spleen deficiency,which impairs the transport of water and dampness.This dysfunction leads to the accumulation of dampness that transforms into phlegm.Over time,the stagnation of phlegm and dampness generates internal heat,resulting in the formation of“dampness toxin,”“phlegm toxin,”and“heat toxin.”These pathological toxins obstruct the circulation of Qi and blood,causing stasis and the emergence of“stasis toxin.”The buildup of these toxins ultimately blocks the three channels and two pathways,disrupting the harmony among the three Qi of heaven,earth,and man.This disharmony culminates in impaired uterine gland function.Therefore,the treatment mainly focuses on tonifying deficiency,detoxifying,and regulating Qi.
文摘Objective This case-control study aimed to investigate whether diabetes mellitus(DM),hypertension,hyperlipidemia,and chronic viral hepatitis are risk factors for multiple myeloma(MM).Moreover,the clinical characteristics of MM patients with or without the abovementioned exposure factors were analyzed.Methods In total,340 MM patients and 680 patients with benign diseases who were hospitalized from January 2012 to December 2017 were classified under the case group and control group,respectively.Data about medical history of DM,hypertension,hyperlipidemia and chronic viral hepatitis were collected by reviewing medical records.Univariate and multivariate analyses were conducted to compare the history of DM,hypertension,hyperlipidemia,and viral hepatitis between the two groups.Considering DM,hypertension,hyperlipidemia,and chronic viral hepatitis as exposure factors,clinical characteristics,such as renal function and presence of fungal and other types of infections,between the exposed and nonexposed groups were analyzed.Results No significant difference was observed in the prevalence of DM,hypertension,and hyperlipidemia between the case and control groups.MM patients had a higher prevalence of chronic viral hepatitis than those with benign diseases.No significant difference was observed in the prevalence of renal dysfunction,fungal infection,and non-fungal infections in MM patients with or without DM,hypertension,and hyperlipidemia.MM patients with chronic viral hepatitis had a significantly higher prevalence of nonfungal infections during hospitalization than those without.Conclusion No significant association was noted between MM and DM,hypertension,and hyperlipidemia.Chronic viral hepatitis is correlated to a significantly higher risk of MM,and MM patients with chronic viral hepatitis were more susceptible to non-fungal infections during hospitalization.Although a non-significant trend was observed in this study,we believe that DM and hypertension might be associated with a higher risk of MM.Thus,large-scale studies must be conducted to validate the results of the current study.
文摘Bluetooth technology emerged over twenty years ago and has continuously improved throughout the years to meet diverse and complex applications. Initially invented to replace the need for physical data cables, Bluetooth offers users a quick and easy way to share data files over a wireless network. Traffic engineers and transportation engineering researchers have utilized the potential opportunities that exist with Bluetooth and have implemented this technology into traffic monitoring techniques. To gain a better understanding of Bluetooth sensors and how they work, a comprehensive literature search was conducted. Twenty-five articles were studied regarding case studies of Bluetooth sensor implementation for travel time measurement. Besides reviewing the literature and previous case studies, three new case studies in the State of Delaware, USA, were also conducted and carefully analyzed. The benefits and drawbacks associated with Bluetooth technology for travel time measurements have been identified in this paper. The overall conclusion of the authors is Bluetooth alone and by itself is not a proper technology for travel time measurements. More studies need to be conducted on the accuracy and overall application, before one can confidently utilize the Bluetooth technology for travel time measurements.

文摘Managing the acute problem of environmental deterioration due to the industrialization of modern China is necessary.Using three cases from a theoretical perspective of procedure-structure,this study explored how environmental deterioration is governed in three circumstances.Research findings revealed that the pattern to treat the worsening environment is associated closely with individual-specific circumstances,and thus three patterns of governance are developed:bureaucratic,contractual,and network-based cooperatives.The cases and the results are described the advantages and disadvantages of these models are discussed.Suggestions on how to more effectively manage these urgent problems are as follows:first,the governance must fit the circumstance,namely,its physical,biological,and social attributes;second,the regulations and conditions regarding the government-dominated pattern are came up from the perspective of mega-governance,mutual trust contracts,benefit sharing,and social capital;and third,the government should abandon the pattern based on the single subject and adopt a plural subject governance model to integrate all the relevant forces to facilitate the attainment of benign environmental governance.
文摘To uncover how different backgrounds influence teacher beliefs,a multiple case study was done to three pre-service and three in-service EFL teachers through semi-structured interviews.The results show that pre-service and in-service teachers hold a general belief that while English exposure is important,the use of Chinese as the L1 is complementary.Nonetheless,teachers’rationale reflects influence from prior learning experience,teacher education and classroom practice.The findings provide implications for career development of EFL teachers.

文摘Randomized controlled trials(RCTs) are the gold standard in terms of study design, however, in the surgical setting conducting RCTs can often be unethical or logistically impossible. Case-control studies should become the major study design used in surgical research when RCTs are unable to be conducted and definitely replacing case series which offer little insight into surgical outcomes and disease processes.

Based on the analysis, the research proposed countermeasures and suggestions. It is necessary to define the range and scale on family farm before giving policy supports. It is important to establish access system of industry and commerce capital to rural areas to guarantee the operators in need be benefited from national preferential policies. Meanwhile, new system on land transfer should be established to guarantee security system for family farm, improve socialized service system and promote government-built production facilities operated by farmers.
