BACKGROUND The posterior cruciate ligament(PCL)is vital for regulating posterior tibial translation in relation to the femur,which is critical for knee stability.PCL tears are infrequently isolated in knee injuries;ho...BACKGROUND The posterior cruciate ligament(PCL)is vital for regulating posterior tibial translation in relation to the femur,which is critical for knee stability.PCL tears are infrequently isolated in knee injuries;however,the absence of the PCL results in abnormal knee kinematics,which may cause injuries to other ligaments.The ideal tendon source for PCL reconstruction is still a subject of debate.AIM To evaluate the results of employing the peroneus longus tendon(PLT)in PCL reconstruction.METHODS A comprehensive search was conducted to identify relevant randomized controlled trials and retrospective observational studies discussing the outcomes of using the PLT for PCL reconstruction.Studies published up to August 2024 were searched across multiple databases,including PubMed,EMBASE,Scopus,Web of Science,Cochrane Library,and Google Scholar.Full texts of the selected articles were retrieved,reviewed,and independently assessed by the investigators.Discrepancies were resolved by consensus,with any remaining disagreements being arbitrated by a third author.RESULTS This meta-analysis included five studies on PLT use for PCL reconstruction:(1)Four prospective studies with 104 patients;and(2)One retrospective study with 18 patients.Most studies followed up participants for 24 months,while one had a shorter follow-up of 18 months.Lysholm and modified cincinnati scores improved by pooled means of 32.2(95%CI:29.3-35.1,I2=0%)and 31.1(95%CI:27.98-34.22,I2=0%),respectively.Postoperative American Orthopaedic Foot and Ankle Society and Foot and Ankle Disability Index scores were 94.5(I2=61.5%)and 94.5(I2=80.09%),respectively.Single-hop and triple-hop test scores averaged 95.5(95%CI:94.5-96.5)and 92.4(95%CI:91.9-92.9)respectively.No significant differences were observed in thigh circumference at 10 cm and 20 cm between the injured and healthy sides.CONCLUSION Evidence supports PLT autografts for PCL reconstruction,improving knee function and patient outcomes.Larger randomized trials are needed to confirm efficacy and compare graft options.展开更多

BACKGROUND Although surgery remains the primary treatment for proximal gastric cancer(PGC),ongoing refinements in surgical strategies are essential to improving clinical outcomes.AIM To investigate the effect of doubl...BACKGROUND Although surgery remains the primary treatment for proximal gastric cancer(PGC),ongoing refinements in surgical strategies are essential to improving clinical outcomes.AIM To investigate the effect of double-tract reconstruction(DTR)on immune function and stress response in patients undergoing laparoscopic proximal gastrectomy(LPG).METHODS In total,78 patients with PGC admitted between August 2020 and August 2024 were enrolled.The research group consisted of 39 patients who underwent DTR+LPG,whereas the control group comprised 39 patients who underwent laparoscopic total gastrectomy with Roux-en-Y esophagojejunostomy.Perioperative indices(intraoperative blood loss,digestive tract anastomosis time,and time to first postoperative flatus),postoperative complications(intestinal obstruction,anastomotic ulcer,diarrhea,dumping syndrome,and gastroesophageal reflux),nutritional parameters(serum albumin,hemoglobin,and body mass index),immune function immunoglobulin(IgG,IgA,and IgM),and stress response indicators(C-reactive protein,interleukin-6,and tumor necrosis factor-α)were collected and analyzed for both groups.RESULTS The intraoperative blood loss was lower(P<0.05),and the time to first postoperative flatus time was shorter(P<0.001)in the research group than in the control group.The two groups had comparable digestive tract anastomosis time(P>0.05).The overall complication rate was significantly lower in the research group than in the control group(P=0.042).Compared with the control group,the research group exhibited notably higher albumin,hemoglobin,and body mass index levels at 2 and 3 months postoperatively,as well as considerably high immunoglobulin(Ig)G,IgA,and IgM levels on postoperative day 1(P<0.05).The postoperative levels of C-reactive protein,interleukin-6,and tumor necrosis factor-αwere also lower in the research group than in the control group(P<0.001).CONCLUSION The combination of DTR and LPG in the treatment of patients with PGC is more effective in enhancing immune function and suppressing stress responses,showing more advantages over laparoscopic total gastrectomy.展开更多
BACKGROUND Congenital intestinal atresia(CIA)is a common intestinal malformation in the neonatal period,and surgery is currently the main treatment method.The choice of postoperative feeding is crucial for the recover...BACKGROUND Congenital intestinal atresia(CIA)is a common intestinal malformation in the neonatal period,and surgery is currently the main treatment method.The choice of postoperative feeding is crucial for the recovery of gastrointestinal function in children.AIM To compare and analyze the effects of different postoperative feeding methods on gastrointestinal function reconstruction in newborns with CIA.METHODS Twenty-six children diagnosed with neonatal CIA,treated with minimally invasive surgery at Shijiazhuang Maternal and Child Health Hospital between January 2021 and May 2024,were selected for this single-center prospective randomized controlled study.They were divided into two groups using envelope randomization:Enteral nutrition(EN)group(n=13)and parenteral nutrition(PN)group(n=13).Baseline and clinical characteristics were collected,and recovery time of bowel sounds and time to first defecation were used as evaluation indices for gastrointestinal functional reconstruction.Differences between the groups were analyzed using t-test,χ2 test,and Fisher’s exact test.Spearman’s correlation tests and linear regression models were employed to analyze factors influencing time to first defecation.RESULTS The time to bowel sound recovery(51.54 vs 65.85,P=0.013)and first defecation(58.15 vs 76.62,P<0.001)was shorter in the EN group compared to the PN group.Clinical improvements in the EN group,including discharge weight(P=0.044),hospital stay(P=0.027),white blood cell count(P=0.023),albumin content(P=0.013),and direct bilirubin content(P=0.018),were also better than those in the PN group.No substantial differences in postoperative complications were found between the groups.Correlation analysis indicated that abdominal infection and operation time may relate to time to first defecation.Linear regression analysis demonstrated a considerable association between EN feeding and shorter time to first defecation.Abdominal infection and an operation time>2 hours may be risk factors for prolonged time to first defecation.CONCLUSION EN substantially promotes the recovery of gastrointestinal function after CIA in neonates and can improve clinical outcomes in children.Future research should explore optimal EN practices to enhance clinical application and child health.展开更多
This paper demonstrates that the spatial distribution of the ionospheric TEC over the Indian region can be reconstructed with appreciable accuracy using minimal numbers of empirical orthogonal functions as a basis.The...This paper demonstrates that the spatial distribution of the ionospheric TEC over the Indian region can be reconstructed with appreciable accuracy using minimal numbers of empirical orthogonal functions as a basis.These basis functions were derived using the Singular Value Decomposition of a matrix composed of pragmatic vertical Total Electron Content(VTEC)values collected across varied ionospheric conditions and measured over the region of interest.The reconstruction was achieved by linearly combining the appropriately chosen significant bases with corresponding weight factors.The reconstruction accuracy of the algorithm was found to be better than 4 TECU(TECU=1016electrons/m2)for more than 99.9%of the time when tested over the complete year of 2016 with only eight basis vectors.The containment factor,defined here,indicates the goodness of the chosen bases in representing the arbitrary VTEC distributions and is found to remain typically high,aiding in improved algorithm performance.The performance,however,was found to be sensitive to the seasons and geomagnetic conditions.Deteriorated performance was observed when tested for the St.Patrick's Day storm data.The deterioration was attributed to the structural alteration of the ionospheric plasma density and the presence of atypical modes during the storm.The results ascertain the prospect of a faithful representation of the spatial distribution of the ionospheric VTEC using limited parametric variables,which may find utility in navigation,radar,and various other applications.展开更多
BACKGROUND For patients with advanced gastric cancer,surgical resection remains the main treatment option.Total gastrectomy combined with radical resection of gastric cancer lesions and sentinel lymph nodes can signif...BACKGROUND For patients with advanced gastric cancer,surgical resection remains the main treatment option.Total gastrectomy combined with radical resection of gastric cancer lesions and sentinel lymph nodes can significantly prolong the survival of patients.Digestive tract reconstruction after total gastrectomy is essential to maintain gastrointestinal function and optimize postoperative recovery.Therefore,it is very important to choose a suitable reconstruction method to improve the quality of life of total gastrectomy patients.AIM To evaluate the effects of different digestive tract reconstruction methods in gastric cancer patients undergoing total gastrectomy.METHODS This retrospective study included 172 patients who underwent total gastrectomy for gastric cancer at The First Hospital of Hebei Medical University for analysis.The patients were categorized into two groups:Group A,consisting of 90 patients who underwent modified Roux-en-Y gastrojejunostomy,and group B,consisting of 82 patients who underwent uncut Roux-en-Y gastrojejunostomy.The general patient characteristics,perioperative indicators,postoperative gastrointestinal mucosal barrier function,nutritional status,immunological markers,and occurrence of complications were compared between the two groups.RESULTS Group A showed shorter digestive tract reconstruction time than group B(P<0.05).On the first postoperative day,group A showed lower serum levels of D-lactate,diamine oxidase,and endotoxin than group B(P<0.05).One month postoperatively,group A showed higher prognostic nutritional index,serum albumin,total protein,and body weight than group B(P<0.05).One month postoperatively,the levels of cluster of differentiation(CD)3+,CD4+,and CD8+cells were not significantly different between two groups(P>0.05).The complication rates were 10.00%in group A and 24.39%in group B;group A had a significantly lower complication rate than group B(P<0.05).CONCLUSION Using modified Roux-en-Y gastrojejunostomy during total gastrectomy shortens the time required for gastrointestinal anastomosis,reduces surgery-induced gastrointestinal mucosal damage,and mitigates postoperative declines in nutritional status.展开更多

PF) computerized adaptive test(CAT), hip disability and osteoarthritis outcome score for joint reconstruction(HOOS JR), and the knee injury and osteoarthritis outcome score for joint reconstruction(KOOS JR) from February 2014 to April 2017. MCIDs were calculated using anchorbased and distribution-based methods. Patient reports of meaningful change in function since their first clinic encounter were used as an anchor.RESULTS There were 2226 patients who participated with a mean age of 61.16(SD = 12.84) years, 41.6% male, and 89.7% Caucasian. Mean change ranged from 7.29 to 8.41 for the PROMIS~? PF CAT, from 14.81 to 19.68 for the HOOS JR, and from 14.51 to 18.85 for the KOOS JR. ROC cut-offs ranged from 1.97-8.18 for the PF CAT, 6.33-43.36 for the HOOS JR, and 2.21-8.16 for the KOOS JR. Distribution-based methods estimated MCID values ranging from 2.45 to 21.55 for the PROMIS~? PF CAT; from 3.90 to 43.61 for the HOOS JR, and from 3.98 to 40.67 for the KOOS JR. The median MCID value in the range was similar to the mean change score for each measure and was 7.9 for the PF CAT, 18.0 for the HOOS JR, and 15.1 for the KOOS JR.CONCLUSION This is the first comprehensive study providing a wide range of MCIDs for the PROMIS? Understanding and manipulating the structural evolution of water oxidation electrocatalysts lays the foundation to finetune their catalytic activity.Herein,we present a synthesis of NiSe_(2)-Ce_(2)(CO_(3))_(2)O hetero...Understanding and manipulating the structural evolution of water oxidation electrocatalysts lays the foundation to finetune their catalytic activity.Herein,we present a synthesis of NiSe_(2)-Ce_(2)(CO_(3))_(2)O heterostructure and demonstrate the efficacy of interfacial Ce_(2)(CO_(3))2O in promoting the formation of catalytically active centers to improve oxygen evolution activity.In-situ Raman spectroscopy shows that incorporation of Ce_(2)(CO_(3))2O into NiSe_(2) causes a cathodic shift of the Ni^(2+)→Ni~(3+) transition potential.Operando electrochemical impedance spectroscopy reveals that strong electronic coupling at heterogeneous interface accelerates charge transfer process.Furthermore,density functional theory calculations suggest that actual catalytic active species of NiOOH transformed from NiSe_(2),which is coupled with Ce_(2)(CO_(3))_(2)O,can optimize electronic structure and decrease the free energy barriers toward fast oxygen evolution reaction(OER) kinetics.Consequently,the resultant NiSe_(2)-Ce_(2)(CO_(3))_(2)O electrode exhibits remarkable electrocatalytic performance with low overpotentials(268/304 mV@50/100 mA cm^(-2)) and excellent stability(50 mA cm^(-2) for 120 h) in the alkaline electrolyte.This work emphasizes the significance of modulating the dynamic changes in developing efficient electrocatalyst.展开更多
BACKGROUND Suspension training(SET)is a method of neuromuscular training that enables the body to carry out active training under unstable support through a suspension therapy system.However,there have been few report...BACKGROUND Suspension training(SET)is a method of neuromuscular training that enables the body to carry out active training under unstable support through a suspension therapy system.However,there have been few reports in the literature on the application of SET to anterior cruciate ligament reconstruction(ACLR)patients.It is not clear what aspects of the patient's function are improved after SET.AIM To investigate the effect of SET on the neuromuscular function,postural control,and knee kinematics of patients after ACLR surgery.METHODS Forty participants were randomized to an SET group or a control group.The SET group subjects participated in a SET protocol over 6 wk.The control group subjects participated in a traditional training protocol over 6 wk.Isokinetic muscle strength of the quadriceps and hamstrings,static and dynamic posture stability test,and relative translation of the injured knee were assessed before and after training.RESULTS The relative peak torque of the quadriceps and hamstrings in both groups increased significantly(P<0.001),and the SET group increased by a higher percentage than those in the control group(quadriceps:P=0.004;hamstrings:P=0.011).After training,both groups showed significant improvements in static and dynamic posture stability(P<0.01),and the SET group had a greater change than the control group(P<0.05).No significant improvement on the relative translation of the injured knee was observed after training in either group(P>0.05).CONCLUSION Our findings show that SET promotes great responses in quadriceps and hamstring muscle strength and balance function in ACLR patients.展开更多
Objective:To evaluate the safety profile and short-term functional outcome of sustainable functional urethral reconstruction(SFUR)in robotic-assisted radical prostatectomy(RARP).Methods:One hundred and sixty-two conse...Objective:To evaluate the safety profile and short-term functional outcome of sustainable functional urethral reconstruction(SFUR)in robotic-assisted radical prostatectomy(RARP).Methods:One hundred and sixty-two consecutive prostate cancer patients who underwent RARP were retrospectively analyzed,in which 53 had undergone SFUR while the other 109 had undergone conventional RARP procedures.Immediate,2-week,1-month and 3-month continence recovery and other perioperative data were compared to evaluate short-term surgical and functional outcome.Results:The median age was 68 and 67 years in the experimental group and control group,respectively(pZ0.206),with a median prostate-specific antigen(PSA)of 13.6 ng/mL(interquartile range[IQR],8.46e27.32 ng/mL)in the experimental group and 13.84 ng/mL(IQR,9.12e26.80 ng/mL)in control group(pZ0.846).Immediate,2-week,1-month and 3-month continence recovery rates between the groups were 34.0%vs.3.7%,50.9%vs.14.7%,62.3%vs.27.5%,and 79.2%vs.63.3%(all p<0.05).The morphological changes made by the new reconstruction technique were maintained on magnetic resonance imaging(MRI)3 months postoperatively.Nerve-sparing procedures and adoption of the new reconstruction technique were significantly relevant to continence recovery on logistics regression model(p<0.001).Conclusions:SFUR is a safe and easy-to-handle modification that may contribute to early continence return for RARP.Long-term follow-up and prospective studies are required to further evaluate its value in postoperative quality-of-life improvement.展开更多

A sparse approximation set of scattered data is constructed by reducing the number of interpolating points on the surface. We present an adaptive method for finding the off-surface normal points. The order of the equation decreases greatly as the number of the off-surface constraints reduces gradually. Experimental results are provided to illustrate that the proposed method is robust and may draw beautiful graphics.展开更多
Grain production space reconstruction referred to the changes in the quantity,quality and pattern of grain produc-tion space caused by functional tradeoffs and conflicts between grain production space,urban-rural deve...Grain production space reconstruction referred to the changes in the quantity,quality and pattern of grain produc-tion space caused by functional tradeoffs and conflicts between grain production space,urban-rural development space,and ecological service space.Exploring tradeoffs between land system functions caused by grain produc-tion space reconstruction was particularly important for ensuring food security,promoting the construction of ecological civilization,and achieving sustainable development.Therefore,this study identified four relationships of land system functions during the process of grain production space reconstruction(1980-2018)in China by using Set Pair Analysis.Research results showed that the reconstruction of grain production space was achieved mainly through three pathways:Grain for Green,deforestation and reclamation,and urban expansion.Generally,ecological service had spatial negative correlation with grain production,economic development and population carrying capacity(P<0.01),but grain production,economic development and population carrying capacity were positively correlated with each other(P<0.01).In the process of grain production space reconstruction,eco-logical services and economic development,ecological services and population carrying capacity had all shown inverse synergies;there was a tradeoffbetween grain production and ecological services,a codirectional tradeoffbetween grain production and economic development,but a strong synergy between economic development and population carrying capacity.However,the functions of land systems only appeared as synergies or tradeoffs,and there were no inverse synergies and codirectional tradeoffs in the separate processes of Grain for Green,deforestation and reclamation,and urban expansion.It can be concluded that the relationships between land system functions were relatively simple in a single process,but it became complex and diverse when multiple processes were integrated for system analysis.展开更多
Seismic data typically contain random missing traces because of obstacles and economic restrictions,influencing subsequent processing and interpretation.Seismic data recovery can be expressed as a low-rank matrix appr...Seismic data typically contain random missing traces because of obstacles and economic restrictions,influencing subsequent processing and interpretation.Seismic data recovery can be expressed as a low-rank matrix approximation problem by assuming a low-rank structure for the complete seismic data in the frequency–space(f–x)domain.The nuclear norm minimization(NNM)(sum of singular values)approach treats singular values equally,yielding a solution deviating from the optimal.Further,the log-sum majorization–minimization(LSMM)approach uses the nonconvex log-sum function as a rank substitution for seismic data interpolation,which is highly accurate but time-consuming.Therefore,this study proposes an efficient nonconvex reconstruction model based on the nonconvex Geman function(the nonconvex Geman low-rank(NCGL)model),involving a tighter approximation of the original rank function.Without introducing additional parameters,the nonconvex problem is solved using the Karush–Kuhn–Tucker condition theory.Experiments using synthetic and field data demonstrate that the proposed NCGL approach achieves a higher signal-to-noise ratio than the singular value thresholding method based on NNM and the projection onto convex sets method based on the data-driven threshold model.The proposed approach achieves higher reconstruction efficiency than the singular value thresholding and LSMM methods.展开更多

This model overcomes the difficult problem of OTF estimation that the previous IFP faces. The effectiveness of this algorithm is demonstrated by numerical simulations, and the superior reconstruction is presented. We believe that the reported algorithm can extend the original IFP for more complex conditions and may provide a solution by using structured light for characterization of optical systems' aberrations.展开更多

In this paper,we present an approach for smooth surface reconstructions interpolating triangular meshes with ar- bitrary topology and geometry.The approach is based on the well-known radial basis functions (RBFs) and ...In this paper,we present an approach for smooth surface reconstructions interpolating triangular meshes with ar- bitrary topology and geometry.The approach is based on the well-known radial basis functions (RBFs) and the constructed surfaces are generalized thin-plate spline surfaces.Our algorithm first defines a pair of offset points for each vertex of a given mesh to en- hance the controUability of local geometry and to assure stability of the construction.A linear system is then solved by LU decomposi- tion and the implicit governing equation of interpolating surface is obtained.The constructed surfaces finally are visualized by a Marching Cubes based polygonizer.The approach provides a robust and efficient solution for smooth surface reconstruction from various 3 D meshes.展开更多

DATA SOURCES: A search of Medline was performed to select functional reconstruction and rehabilitation following brachial plexus injury-related English articles published between January 1990 and July 2006, with key words of "brachial plexus injury, reconstruction and rehabilitation". Meanwhile, a computer-based search of CBM was carried out to select the similar Chinese articles published between January 1998 and July 2006, with key words of "brachial plexus injury, reconstruction and rehabilitation". STUDY SELECTION: The materials were checked primarily, and the literatures of functional reconstruction and rehabilitation of brachial plexus injury were selected and the full texts were retrieved. Inclusive criteria: ①Functional reconstruction following brachial plexus injury. ②Rehabilitation method of brachial plexus injury. Exclusive criteria: Reviews, repetitive study, and Meta analytical papers. DATA EXTRACTION: Forty-six literatures about functional reconstruction following brachial plexus injury were collected, and 36 of them met the inclusive criteria. DATA SYNTHESIS: Brachial plexus injury causes the complete or incomplete palsy of muscle of upper extremity. The treatment of brachial plexus is to displace not very important nerves to the distal end of very important nerve, called nerve transfer, which is an important method to treat brachial plexus injury. Postoperative rehabilitations consist of sensory training and motor functional training. It is very important to keep the initiativeness of exercise. Besides recovering peripheral nerve continuity by operation, combined treatment and accelerating neural regeneration, active motors of cerebral cortex is also the important factor to reconstruct peripheral nerve function. CONCLUSION: Consciously and actively strengthening functional exercise after operation is helpful to form cerebral plasticity and produce voluntary movements, can re-educate re-dominated muscle, obviously improves postoperative therapeutic effect and promote functional reconstruction.展开更多
文摘BACKGROUND The posterior cruciate ligament(PCL)is vital for regulating posterior tibial translation in relation to the femur,which is critical for knee stability.PCL tears are infrequently isolated in knee injuries;however,the absence of the PCL results in abnormal knee kinematics,which may cause injuries to other ligaments.The ideal tendon source for PCL reconstruction is still a subject of debate.AIM To evaluate the results of employing the peroneus longus tendon(PLT)in PCL reconstruction.METHODS A comprehensive search was conducted to identify relevant randomized controlled trials and retrospective observational studies discussing the outcomes of using the PLT for PCL reconstruction.Studies published up to August 2024 were searched across multiple databases,including PubMed,EMBASE,Scopus,Web of Science,Cochrane Library,and Google Scholar.Full texts of the selected articles were retrieved,reviewed,and independently assessed by the investigators.Discrepancies were resolved by consensus,with any remaining disagreements being arbitrated by a third author.RESULTS This meta-analysis included five studies on PLT use for PCL reconstruction:(1)Four prospective studies with 104 patients;and(2)One retrospective study with 18 patients.Most studies followed up participants for 24 months,while one had a shorter follow-up of 18 months.Lysholm and modified cincinnati scores improved by pooled means of 32.2(95%CI:29.3-35.1,I2=0%)and 31.1(95%CI:27.98-34.22,I2=0%),respectively.Postoperative American Orthopaedic Foot and Ankle Society and Foot and Ankle Disability Index scores were 94.5(I2=61.5%)and 94.5(I2=80.09%),respectively.Single-hop and triple-hop test scores averaged 95.5(95%CI:94.5-96.5)and 92.4(95%CI:91.9-92.9)respectively.No significant differences were observed in thigh circumference at 10 cm and 20 cm between the injured and healthy sides.CONCLUSION Evidence supports PLT autografts for PCL reconstruction,improving knee function and patient outcomes.Larger randomized trials are needed to confirm efficacy and compare graft options.

文摘Realistic human reconstruction embraces an extensive range of applications as depth sensors advance.However,current stateof-the-art methods with RGB-D input still suffer from artefacts,such as noisy surfaces,non-human shapes,and depth ambiguity,especially for the invisible parts.The authors observe the main issue is the lack of geometric semantics without using depth input priors fully.This paper focuses on improving the representation ability of implicit function,exploring an effective method to utilise depth-related semantics effectively and efficiently.The proposed geometry-enhanced implicit function enhances the geometric semantics with the extra voxel-aligned features from point clouds,promoting the completion of missing parts for unseen regions while preserving the local details on the input.For incorporating multi-scale pixel-aligned and voxelaligned features,the authors use the Squeeze-and-Excitation attention to capture and fully use channel interdependencies.For the multi-view reconstruction,the proposed depth-enhanced attention explicitly excites the network to“sense”the geometric structure for a more reasonable feature aggregation.Experiments and results show that our method outperforms current RGB and depth-based SOTA methods on the challenging data from Twindom and Thuman3.0,and achieves a detailed and completed human reconstruction,balancing performance and efficiency well.
文摘BACKGROUND Although surgery remains the primary treatment for proximal gastric cancer(PGC),ongoing refinements in surgical strategies are essential to improving clinical outcomes.AIM To investigate the effect of double-tract reconstruction(DTR)on immune function and stress response in patients undergoing laparoscopic proximal gastrectomy(LPG).METHODS In total,78 patients with PGC admitted between August 2020 and August 2024 were enrolled.The research group consisted of 39 patients who underwent DTR+LPG,whereas the control group comprised 39 patients who underwent laparoscopic total gastrectomy with Roux-en-Y esophagojejunostomy.Perioperative indices(intraoperative blood loss,digestive tract anastomosis time,and time to first postoperative flatus),postoperative complications(intestinal obstruction,anastomotic ulcer,diarrhea,dumping syndrome,and gastroesophageal reflux),nutritional parameters(serum albumin,hemoglobin,and body mass index),immune function immunoglobulin(IgG,IgA,and IgM),and stress response indicators(C-reactive protein,interleukin-6,and tumor necrosis factor-α)were collected and analyzed for both groups.RESULTS The intraoperative blood loss was lower(P<0.05),and the time to first postoperative flatus time was shorter(P<0.001)in the research group than in the control group.The two groups had comparable digestive tract anastomosis time(P>0.05).The overall complication rate was significantly lower in the research group than in the control group(P=0.042).Compared with the control group,the research group exhibited notably higher albumin,hemoglobin,and body mass index levels at 2 and 3 months postoperatively,as well as considerably high immunoglobulin(Ig)G,IgA,and IgM levels on postoperative day 1(P<0.05).The postoperative levels of C-reactive protein,interleukin-6,and tumor necrosis factor-αwere also lower in the research group than in the control group(P<0.001).CONCLUSION The combination of DTR and LPG in the treatment of patients with PGC is more effective in enhancing immune function and suppressing stress responses,showing more advantages over laparoscopic total gastrectomy.
文摘BACKGROUND Congenital intestinal atresia(CIA)is a common intestinal malformation in the neonatal period,and surgery is currently the main treatment method.The choice of postoperative feeding is crucial for the recovery of gastrointestinal function in children.AIM To compare and analyze the effects of different postoperative feeding methods on gastrointestinal function reconstruction in newborns with CIA.METHODS Twenty-six children diagnosed with neonatal CIA,treated with minimally invasive surgery at Shijiazhuang Maternal and Child Health Hospital between January 2021 and May 2024,were selected for this single-center prospective randomized controlled study.They were divided into two groups using envelope randomization:Enteral nutrition(EN)group(n=13)and parenteral nutrition(PN)group(n=13).Baseline and clinical characteristics were collected,and recovery time of bowel sounds and time to first defecation were used as evaluation indices for gastrointestinal functional reconstruction.Differences between the groups were analyzed using t-test,χ2 test,and Fisher’s exact test.Spearman’s correlation tests and linear regression models were employed to analyze factors influencing time to first defecation.RESULTS The time to bowel sound recovery(51.54 vs 65.85,P=0.013)and first defecation(58.15 vs 76.62,P<0.001)was shorter in the EN group compared to the PN group.Clinical improvements in the EN group,including discharge weight(P=0.044),hospital stay(P=0.027),white blood cell count(P=0.023),albumin content(P=0.013),and direct bilirubin content(P=0.018),were also better than those in the PN group.No substantial differences in postoperative complications were found between the groups.Correlation analysis indicated that abdominal infection and operation time may relate to time to first defecation.Linear regression analysis demonstrated a considerable association between EN feeding and shorter time to first defecation.Abdominal infection and an operation time>2 hours may be risk factors for prolonged time to first defecation.CONCLUSION EN substantially promotes the recovery of gastrointestinal function after CIA in neonates and can improve clinical outcomes in children.Future research should explore optimal EN practices to enhance clinical application and child health.
文摘This paper demonstrates that the spatial distribution of the ionospheric TEC over the Indian region can be reconstructed with appreciable accuracy using minimal numbers of empirical orthogonal functions as a basis.These basis functions were derived using the Singular Value Decomposition of a matrix composed of pragmatic vertical Total Electron Content(VTEC)values collected across varied ionospheric conditions and measured over the region of interest.The reconstruction was achieved by linearly combining the appropriately chosen significant bases with corresponding weight factors.The reconstruction accuracy of the algorithm was found to be better than 4 TECU(TECU=1016electrons/m2)for more than 99.9%of the time when tested over the complete year of 2016 with only eight basis vectors.The containment factor,defined here,indicates the goodness of the chosen bases in representing the arbitrary VTEC distributions and is found to remain typically high,aiding in improved algorithm performance.The performance,however,was found to be sensitive to the seasons and geomagnetic conditions.Deteriorated performance was observed when tested for the St.Patrick's Day storm data.The deterioration was attributed to the structural alteration of the ionospheric plasma density and the presence of atypical modes during the storm.The results ascertain the prospect of a faithful representation of the spatial distribution of the ionospheric VTEC using limited parametric variables,which may find utility in navigation,radar,and various other applications.
文摘BACKGROUND For patients with advanced gastric cancer,surgical resection remains the main treatment option.Total gastrectomy combined with radical resection of gastric cancer lesions and sentinel lymph nodes can significantly prolong the survival of patients.Digestive tract reconstruction after total gastrectomy is essential to maintain gastrointestinal function and optimize postoperative recovery.Therefore,it is very important to choose a suitable reconstruction method to improve the quality of life of total gastrectomy patients.AIM To evaluate the effects of different digestive tract reconstruction methods in gastric cancer patients undergoing total gastrectomy.METHODS This retrospective study included 172 patients who underwent total gastrectomy for gastric cancer at The First Hospital of Hebei Medical University for analysis.The patients were categorized into two groups:Group A,consisting of 90 patients who underwent modified Roux-en-Y gastrojejunostomy,and group B,consisting of 82 patients who underwent uncut Roux-en-Y gastrojejunostomy.The general patient characteristics,perioperative indicators,postoperative gastrointestinal mucosal barrier function,nutritional status,immunological markers,and occurrence of complications were compared between the two groups.RESULTS Group A showed shorter digestive tract reconstruction time than group B(P<0.05).On the first postoperative day,group A showed lower serum levels of D-lactate,diamine oxidase,and endotoxin than group B(P<0.05).One month postoperatively,group A showed higher prognostic nutritional index,serum albumin,total protein,and body weight than group B(P<0.05).One month postoperatively,the levels of cluster of differentiation(CD)3+,CD4+,and CD8+cells were not significantly different between two groups(P>0.05).The complication rates were 10.00%in group A and 24.39%in group B;group A had a significantly lower complication rate than group B(P<0.05).CONCLUSION Using modified Roux-en-Y gastrojejunostomy during total gastrectomy shortens the time required for gastrointestinal anastomosis,reduces surgery-induced gastrointestinal mucosal damage,and mitigates postoperative declines in nutritional status.

文摘In order to reconstruct and render the weak and repetitive texture of the damaged functional surface of aviation,an improved neural radiance field,named TranSR-NeRF,is proposed.In this paper,a data acquisition system was designed and built.The acquired images generated initial point clouds through TransMVSNet.Meanwhile,after extracting features from the images through the improved SE-ConvNeXt network,the extracted features were aligned and fused with the initial point cloud to generate high-quality neural point cloud.After ray-tracing and sampling of the neural point cloud,the ResMLP neural network designed in this paper was used to regress the volume density and radiance under a given viewing angle,which introduced spatial coordinate and relative positional encoding.The reconstruction and rendering of arbitrary-scale super-resolution of damaged functional surface is realized.In this paper,the influence of illumination conditions and background environment on the model performance is also studied through experiments,and the comparison and ablation experiments for the improved methods proposed in this paper is conducted.The experimental results show that the improved model has good effect.Finally,the application experiment of object detection task is carried out,and the experimental results show that the model has good practicability.

Questions remain, however, regarding the long-term implications on sexual function after major genital reconstructive surgery. In this article, we review the pathologic features of urethral stricture disease and urologic trauma that may affect erectile function (EF) and assess the impact of various specific contemporary urethroplasty surgical techniques on male sexual function.

文摘AIM To establish minimum clinically important difference(MCID) for measurements in an orthopaedic patient population with joint disorders.METHODS Adult patients aged 18 years and older seeking care for joint conditions at an orthopaedic clinic took the Patient-Reported Outcomes Measurement Information System Physical Function(PROMIS~? PF) computerized adaptive test(CAT), hip disability and osteoarthritis outcome score for joint reconstruction(HOOS JR), and the knee injury and osteoarthritis outcome score for joint reconstruction(KOOS JR) from February 2014 to April 2017. MCIDs were calculated using anchorbased and distribution-based methods. Patient reports of meaningful change in function since their first clinic encounter were used as an anchor.RESULTS There were 2226 patients who participated with a mean age of 61.16(SD = 12.84) years, 41.6% male, and 89.7% Caucasian. Mean change ranged from 7.29 to 8.41 for the PROMIS~? PF CAT, from 14.81 to 19.68 for the HOOS JR, and from 14.51 to 18.85 for the KOOS JR. ROC cut-offs ranged from 1.97-8.18 for the PF CAT, 6.33-43.36 for the HOOS JR, and 2.21-8.16 for the KOOS JR. Distribution-based methods estimated MCID values ranging from 2.45 to 21.55 for the PROMIS~? PF CAT; from 3.90 to 43.61 for the HOOS JR, and from 3.98 to 40.67 for the KOOS JR. The median MCID value in the range was similar to the mean change score for each measure and was 7.9 for the PF CAT, 18.0 for the HOOS JR, and 15.1 for the KOOS JR.CONCLUSION This is the first comprehensive study providing a wide range of MCIDs for the PROMIS? PF, HOOS JR, and KOOS JR in orthopaedic patients with joint ailments.

文摘Understanding and manipulating the structural evolution of water oxidation electrocatalysts lays the foundation to finetune their catalytic activity.Herein,we present a synthesis of NiSe_(2)-Ce_(2)(CO_(3))_(2)O heterostructure and demonstrate the efficacy of interfacial Ce_(2)(CO_(3))2O in promoting the formation of catalytically active centers to improve oxygen evolution activity.In-situ Raman spectroscopy shows that incorporation of Ce_(2)(CO_(3))2O into NiSe_(2) causes a cathodic shift of the Ni^(2+)→Ni~(3+) transition potential.Operando electrochemical impedance spectroscopy reveals that strong electronic coupling at heterogeneous interface accelerates charge transfer process.Furthermore,density functional theory calculations suggest that actual catalytic active species of NiOOH transformed from NiSe_(2),which is coupled with Ce_(2)(CO_(3))_(2)O,can optimize electronic structure and decrease the free energy barriers toward fast oxygen evolution reaction(OER) kinetics.Consequently,the resultant NiSe_(2)-Ce_(2)(CO_(3))_(2)O electrode exhibits remarkable electrocatalytic performance with low overpotentials(268/304 mV@50/100 mA cm^(-2)) and excellent stability(50 mA cm^(-2) for 120 h) in the alkaline electrolyte.This work emphasizes the significance of modulating the dynamic changes in developing efficient electrocatalyst.
文摘BACKGROUND Suspension training(SET)is a method of neuromuscular training that enables the body to carry out active training under unstable support through a suspension therapy system.However,there have been few reports in the literature on the application of SET to anterior cruciate ligament reconstruction(ACLR)patients.It is not clear what aspects of the patient's function are improved after SET.AIM To investigate the effect of SET on the neuromuscular function,postural control,and knee kinematics of patients after ACLR surgery.METHODS Forty participants were randomized to an SET group or a control group.The SET group subjects participated in a SET protocol over 6 wk.The control group subjects participated in a traditional training protocol over 6 wk.Isokinetic muscle strength of the quadriceps and hamstrings,static and dynamic posture stability test,and relative translation of the injured knee were assessed before and after training.RESULTS The relative peak torque of the quadriceps and hamstrings in both groups increased significantly(P<0.001),and the SET group increased by a higher percentage than those in the control group(quadriceps:P=0.004;hamstrings:P=0.011).After training,both groups showed significant improvements in static and dynamic posture stability(P<0.01),and the SET group had a greater change than the control group(P<0.05).No significant improvement on the relative translation of the injured knee was observed after training in either group(P>0.05).CONCLUSION Our findings show that SET promotes great responses in quadriceps and hamstring muscle strength and balance function in ACLR patients.
文摘Objective:To evaluate the safety profile and short-term functional outcome of sustainable functional urethral reconstruction(SFUR)in robotic-assisted radical prostatectomy(RARP).Methods:One hundred and sixty-two consecutive prostate cancer patients who underwent RARP were retrospectively analyzed,in which 53 had undergone SFUR while the other 109 had undergone conventional RARP procedures.Immediate,2-week,1-month and 3-month continence recovery and other perioperative data were compared to evaluate short-term surgical and functional outcome.Results:The median age was 68 and 67 years in the experimental group and control group,respectively(pZ0.206),with a median prostate-specific antigen(PSA)of 13.6 ng/mL(interquartile range[IQR],8.46e27.32 ng/mL)in the experimental group and 13.84 ng/mL(IQR,9.12e26.80 ng/mL)in control group(pZ0.846).Immediate,2-week,1-month and 3-month continence recovery rates between the groups were 34.0%vs.3.7%,50.9%vs.14.7%,62.3%vs.27.5%,and 79.2%vs.63.3%(all p<0.05).The morphological changes made by the new reconstruction technique were maintained on magnetic resonance imaging(MRI)3 months postoperatively.Nerve-sparing procedures and adoption of the new reconstruction technique were significantly relevant to continence recovery on logistics regression model(p<0.001).Conclusions:SFUR is a safe and easy-to-handle modification that may contribute to early continence return for RARP.Long-term follow-up and prospective studies are required to further evaluate its value in postoperative quality-of-life improvement.
文摘We use Radial Basis Functions (RBFs) to reconstruct smooth surfaces from 3D scattered data. An object's surface is defined implicitly as the zero set of an RBF fitted to the given surface data. We propose improvements on the methods of surface reconstruction with radial basis functions. A sparse approximation set of scattered data is constructed by reducing the number of interpolating points on the surface. We present an adaptive method for finding the off-surface normal points. The order of the equation decreases greatly as the number of the off-surface constraints reduces gradually. Experimental results are provided to illustrate that the proposed method is robust and may draw beautiful graphics.

文摘Grain production space reconstruction referred to the changes in the quantity,quality and pattern of grain produc-tion space caused by functional tradeoffs and conflicts between grain production space,urban-rural development space,and ecological service space.Exploring tradeoffs between land system functions caused by grain produc-tion space reconstruction was particularly important for ensuring food security,promoting the construction of ecological civilization,and achieving sustainable development.Therefore,this study identified four relationships of land system functions during the process of grain production space reconstruction(1980-2018)in China by using Set Pair Analysis.Research results showed that the reconstruction of grain production space was achieved mainly through three pathways:Grain for Green,deforestation and reclamation,and urban expansion.Generally,ecological service had spatial negative correlation with grain production,economic development and population carrying capacity(P<0.01),but grain production,economic development and population carrying capacity were positively correlated with each other(P<0.01).In the process of grain production space reconstruction,eco-logical services and economic development,ecological services and population carrying capacity had all shown inverse synergies;there was a tradeoffbetween grain production and ecological services,a codirectional tradeoffbetween grain production and economic development,but a strong synergy between economic development and population carrying capacity.However,the functions of land systems only appeared as synergies or tradeoffs,and there were no inverse synergies and codirectional tradeoffs in the separate processes of Grain for Green,deforestation and reclamation,and urban expansion.It can be concluded that the relationships between land system functions were relatively simple in a single process,but it became complex and diverse when multiple processes were integrated for system analysis.

文摘Seismic data typically contain random missing traces because of obstacles and economic restrictions,influencing subsequent processing and interpretation.Seismic data recovery can be expressed as a low-rank matrix approximation problem by assuming a low-rank structure for the complete seismic data in the frequency–space(f–x)domain.The nuclear norm minimization(NNM)(sum of singular values)approach treats singular values equally,yielding a solution deviating from the optimal.Further,the log-sum majorization–minimization(LSMM)approach uses the nonconvex log-sum function as a rank substitution for seismic data interpolation,which is highly accurate but time-consuming.Therefore,this study proposes an efficient nonconvex reconstruction model based on the nonconvex Geman function(the nonconvex Geman low-rank(NCGL)model),involving a tighter approximation of the original rank function.Without introducing additional parameters,the nonconvex problem is solved using the Karush–Kuhn–Tucker condition theory.Experiments using synthetic and field data demonstrate that the proposed NCGL approach achieves a higher signal-to-noise ratio than the singular value thresholding method based on NNM and the projection onto convex sets method based on the data-driven threshold model.The proposed approach achieves higher reconstruction efficiency than the singular value thresholding and LSMM methods.

The leading result is a proposed algorithm that can recover both the super-resolution image and the OTF of an imaging system with unknown aberrations simultaneously. This model overcomes the difficult problem of OTF estimation that the previous IFP faces. The effectiveness of this algorithm is demonstrated by numerical simulations, and the superior reconstruction is presented. We believe that the reported algorithm can extend the original IFP for more complex conditions and may provide a solution by using structured light for characterization of optical systems' aberrations.

文摘Facial fracture repair is time-dependent.Early reduction and fixation after trauma help with later repair.Abnormal healing caused by delayed repair increases not only the difficulty of reconstruction,but also the risk of aesthetic or functional defects.Digital technology was used to model local trauma in three dimensions.The fracture fragments were reset,and the facial shape was reconstructed on the reset model.After resampling,3D printing was used to construct a personalised external fixation helmet.Combined with the posterior nasal passage lift reduction technique,early reduction of the mid-face fractures was performed.Through the early application of a 3D-printed personalised external fixation helmet to a patient with a comprehensive fracture,the helmet manufacturing process and application methods were introduced,and the effect of this application was investigated.In the treatment of facial fractures,the early application of a 3D-printed personalised external fixation helmet is conducive to fracture reduction and fixation and reduces the difficulty of later reconstruction.
文摘In this paper,we present an approach for smooth surface reconstructions interpolating triangular meshes with ar- bitrary topology and geometry.The approach is based on the well-known radial basis functions (RBFs) and the constructed surfaces are generalized thin-plate spline surfaces.Our algorithm first defines a pair of offset points for each vertex of a given mesh to en- hance the controUability of local geometry and to assure stability of the construction.A linear system is then solved by LU decomposi- tion and the implicit governing equation of interpolating surface is obtained.The constructed surfaces finally are visualized by a Marching Cubes based polygonizer.The approach provides a robust and efficient solution for smooth surface reconstruction from various 3 D meshes.

文摘A multi-group pin power reconstruction method that fully exploits nodal information obtained from global coarse mesh solution has been developed.It expands the intra-nodal flux distributions into nonseparable semi-analytic basis functions,and a colorset based form function generating method is proposed,which can accurately model the spectral interaction occurring at assembly interface.To demonstrate its accuracy and applicability to realistic problems, the new method is tested against two benchmark problems,including a mixed-oxide fuel problem.The results show that the new method is comparable in accuracy to fine-mesh methods.
文摘OBJECTIVE: To sum up the treatment of brachial plexus root avulsion and the progress in functional reconstruction and rehabilitation following brachial plexus root avulsion. DATA SOURCES: A search of Medline was performed to select functional reconstruction and rehabilitation following brachial plexus injury-related English articles published between January 1990 and July 2006, with key words of "brachial plexus injury, reconstruction and rehabilitation". Meanwhile, a computer-based search of CBM was carried out to select the similar Chinese articles published between January 1998 and July 2006, with key words of "brachial plexus injury, reconstruction and rehabilitation". STUDY SELECTION: The materials were checked primarily, and the literatures of functional reconstruction and rehabilitation of brachial plexus injury were selected and the full texts were retrieved. Inclusive criteria: ①Functional reconstruction following brachial plexus injury. ②Rehabilitation method of brachial plexus injury. Exclusive criteria: Reviews, repetitive study, and Meta analytical papers. DATA EXTRACTION: Forty-six literatures about functional reconstruction following brachial plexus injury were collected, and 36 of them met the inclusive criteria. DATA SYNTHESIS: Brachial plexus injury causes the complete or incomplete palsy of muscle of upper extremity. The treatment of brachial plexus is to displace not very important nerves to the distal end of very important nerve, called nerve transfer, which is an important method to treat brachial plexus injury. Postoperative rehabilitations consist of sensory training and motor functional training. It is very important to keep the initiativeness of exercise. Besides recovering peripheral nerve continuity by operation, combined treatment and accelerating neural regeneration, active motors of cerebral cortex is also the important factor to reconstruct peripheral nerve function. CONCLUSION: Consciously and actively strengthening functional exercise after operation is helpful to form cerebral plasticity and produce voluntary movements, can re-educate re-dominated muscle, obviously improves postoperative therapeutic effect and promote functional reconstruction.
