BACKGROUND Internal hernia(IH)is a rare culprit of small bowel obstruction(SBO)with an incidence of<1%.It poses a considerable diagnostic challenge requiring a high index of suspicion to prevent misdiagnosis,improp...BACKGROUND Internal hernia(IH)is a rare culprit of small bowel obstruction(SBO)with an incidence of<1%.It poses a considerable diagnostic challenge requiring a high index of suspicion to prevent misdiagnosis,improper treatment,and subsequent morbidity and mortality.AIM To determine the clinico-demographic profile,radiological and operative findings,and postoperative course of patients with IH and its association with SBO.METHODS Medical records of 586 patients with features of SBO presenting at a tertiary care centre at Lucknow,India between September 2010 and August 2023 were reviewed.RESULTS Out of 586 patients,7(1.2%)were diagnosed with IH.Among these,4 had congenital IH and 3 had acquired IH.The male-to-female ratio was 4:3.The median age at presentation was 32 years.Contrast-enhanced computed tomography(CECT)was the most reliable investigation for preoperative identification,demonstrating mesenteric whirling and clumped-up bowel loops.Left paraduodenal hernia and transmesenteric hernia occurred with an equal frequency(approximately 43%each).Intraoperatively,one patient was found to have bowel ischemia and one had associated malrotation of gut.During follow-up,no recurrences were reported.CONCLUSION IH,being a rare cause,must be considered as a differential diagnosis for SBO,especially in young patients in their 30s or with unexplained abdominal pain or discomfort post-surgery.A rapid imaging evaluation,preferably with CECT,is necessary to aid in an early diagnosis and prompt intervention,thereby reducing financial burden related to unnecessary investigations and preventing the morbidity and mortality associated with closed-loop obstruction and strangulation of the bowel.展开更多

Internal learning-based video inpainting methods have shown promising results by exploiting the intrinsic properties of the video to fill in the missing region without external dataset supervision.However,existing int...Internal learning-based video inpainting methods have shown promising results by exploiting the intrinsic properties of the video to fill in the missing region without external dataset supervision.However,existing internal learning-based video inpainting methods would produce inconsistent structures or blurry textures due to the insufficient utilisation of motion priors within the video sequence.In this paper,the authors propose a new internal learning-based video inpainting model called appearance consistency and motion coherence network(ACMC-Net),which can not only learn the recurrence of appearance prior but can also capture motion coherence prior to improve the quality of the inpainting results.In ACMC-Net,a transformer-based appearance network is developed to capture global context information within the video frame for representing appearance consistency accurately.Additionally,a novel motion coherence learning scheme is proposed to learn the motion prior in a video sequence effectively.Finally,the learnt internal appearance consistency and motion coherence are implicitly propagated to the missing regions to achieve inpainting well.Extensive experiments conducted on the DAVIS dataset show that the proposed model obtains the superior performance in terms of quantitative measurements and produces more visually plausible results compared with the state-of-the-art methods.展开更多

Deep-time Earth research plays a pivotal role in deciphering the rates,patterns,and mechanisms of Earth's evolutionary processes throughout geological history,providing essential scientific foundations for climate...Deep-time Earth research plays a pivotal role in deciphering the rates,patterns,and mechanisms of Earth's evolutionary processes throughout geological history,providing essential scientific foundations for climate prediction,natural resource exploration,and sustainable planetary stewardship.To advance Deep-time Earth research in the era of big data and artificial intelligence,the International Union of Geological Sciences initiated the“Deeptime Digital Earth International Big Science Program”(DDE)in 2019.At the core of this ambitious program lies the development of geoscience knowledge graphs,serving as a transformative knowledge infrastructure that enables the integration,sharing,mining,and analysis of heterogeneous geoscience big data.The DDE knowledge graph initiative has made significant strides in three critical dimensions:(1)establishing a unified knowledge structure across geoscience disciplines that ensures consistent representation of geological entities and their interrelationships through standardized ontologies and semantic frameworks;(2)developing a robust and scalable software infrastructure capable of supporting both expert-driven and machine-assisted knowledge engineering for large-scale graph construction and management;(3)implementing a comprehensive three-tiered architecture encompassing basic,discipline-specific,and application-oriented knowledge graphs,spanning approximately 20 geoscience disciplines.Through its open knowledge framework and international collaborative network,this initiative has fostered multinational research collaborations,establishing a robust foundation for next-generation geoscience research while propelling the discipline toward FAIR(Findable,Accessible,Interoperable,Reusable)data practices in deep-time Earth systems research.展开更多
BACKGROUND Monkeypox(Mpox),is a disease of global public health concern,as it does not affect only countries in western and central Africa.AIM To assess Burundi healthcare workers(HCWs)s’level of knowledge and confid...BACKGROUND Monkeypox(Mpox),is a disease of global public health concern,as it does not affect only countries in western and central Africa.AIM To assess Burundi healthcare workers(HCWs)s’level of knowledge and confidence in the diagnosis and management of Mpox.METHODS We conducted a cross-sectional study via an online survey designed mainly from the World Health Organization course distributed among Burundi HCWs from June-July 2023.The questionnaire comprises 8 socioprofessional-related questions,22 questions about Mpox disease knowledge,and 3 questions to assess confidence in Mpox diagnosis and management.The data were analyzed via SPSS software version 25.0.A P value<0.05 was considered to indicate statistical significance.RESULTS The study sample comprised 471 HCWs who were mainly medical doctors(63.9%)and nurses(30.1%).None of the 22 questions concerning Mpox knowledge had at least 50%correct responses.A very low number of HCWs(17.4%)knew that Mpox has a vaccine.The confidence level to diagnose(21.20%),treat(18.00%)or prevent(23.30%)Mpox was low among HCWs.The confidence level in the diagnosis of Mpox was associated with the HCWs’age(P value=0.009),sex(P value<0.001),work experience(P value=0.002),and residence(P value<0.001).The confidence level to treat Mpox was significantly associated with the HCWs’age(P value=0.050),sex(P value<0.001),education(P value=0.033)and occupation(P value=0.005).The confidence level to prevent Mpox was associated with the HCWs’education(P value<0.001),work experience(P value=0.002),residence(P value<0.001)and type of work institution(P value=0.003).CONCLUSION This study revealed that HCWs have the lowest level of knowledge regarding Mpox and a lack of confidence in the ability to diagnose,treat or prevent it.There is an urgent need to organize continuing medical education programs on Mpox epidemiology and preparedness for Burundi HCWs.We encourage future researchers to assess potential hesitancy toward Mpox vaccination and its associated factors.展开更多
The algebraic reconstruction technique(ART),multiplicative algebraic reconstruction technique(MART),and simultaneous iterative reconstruction technique(SIRT)are computational methodologies extensively utilized within ...The algebraic reconstruction technique(ART),multiplicative algebraic reconstruction technique(MART),and simultaneous iterative reconstruction technique(SIRT)are computational methodologies extensively utilized within the field of computerized ionospheric tomography(CIT)to facilitate three-dimensional reconstruction of the ionospheric morphology.However,reconstruction accuracy elicits recurrent disputes over its practical application,and people usually attribute this issue to incomplete and uneven coverage of the measurements.The Thermosphere Ionosphere Electrodynamics General Circulation Model(TIEGCM)offers a reasonable physics-based ionospheric background and is widely utilized in ionospheric research.We use the TIEGCM simulations as the targeted ionosphere because the current measurements are far from able to realistically reproduce the ionosphere in detail.Optimized designations of satellite measurements are conducted to investigate the limiting performance of CIT methods in ionospheric reconstruction.Similar to common practice,electron density distributions from outputs of the International Reference Ionosphere(IRI)model are used as the iterative initial value in CIT applications.The outcomes suggest that despite data coverage,iterative initial conditions also play an essential role in ionospheric reconstruction.In particular,in the longitudinal sectors where the iterative initial height of the F2-layer peak electron density(hmF2)differs substantially from the background densities,none of the three CIT methods can reproduce the exact background profile.When hmF2 is close but the ionospheric F2-layer peak density(NmF2)is different between the targeted background and initial conditions,the MART performs better than the ART and SIRT,as evidenced by the correlation coefficients of MART being above 0.97 and those of ART and SIRT being below 0.85.In summary,this investigation reveals the potential uncertainties in traditional CIT reconstruction,particularly when realistic hmF2 or NmF2 values differ substantially from the initial CIT conditions.展开更多

[Objective]The work is devoted to the study of irreversible deformation of artificial samples subjected to a set of standard experiments,with an aim to study their mechanical properties.The principal idea of the study...[Objective]The work is devoted to the study of irreversible deformation of artificial samples subjected to a set of standard experiments,with an aim to study their mechanical properties.The principal idea of the study is related to the preparation of an artificial material with an established constitutive behavior model.The existence of such a well-described material provides future opportunities to conduct controllable experiments on various mechanical processes in rock-like material for further development and validation of theoretical models used in rock mechanics.[Methods]A set of artificial samples was prepared for careful assessment through a number of loading tests.Experimental work was carried out to determine the rheological properties under conditions of triaxial compression tests and uniaxial tension.Triaxial loading tests are completed for 9 samples with varying radial stress levels(0-5 MPa).The samples are loaded up to the yield point with control of radial and volumetric strain.The experimental results,which contain the obtained interrelationships between axial and radial stresses and strains,are analyzed using the Drucker-Prager yield surface.Material hardening is taken into account through the non-associated plastic flow law with the cap model.Numerical modeling of sample loading is performed through the finite difference method.Mathematical model parameters are adjusted to minimize the discrepancy between numerical modeling results and experimental data.The design of a series of experimental studies necessary to determine all the parameters of the model has been studied.[Results]It is shown that the formulated mathematical model allows to reliably reproduce the inelastic behavior of the studied material,and it can be used to solve a set of applied problems in continuum mechanics,the problem of numerical simulation of hydraulic fracture growth in an elastoplastic medium in particular.It was found that for the entire range of applied lateral loads(0-5 MPa),the elastic limit varied from 2 to 4 MPa,after which the material began to behave plastically.It was also determined that at lateral loads≥3 MPa,compaction began to appear in the material beyond the yield point.Judging by the dependence of volumetric strains under a lateral load equal to 1.4 MPa,compaction should begin to appear even at lateral loads lower than 3 MPa.[Conclusion]Taking the plastic behavior of the material into account is necessary when moving on to modeling the hydraulic fracturing process in such a material,and the resultant plasticity parameters for the model material can be used for numerical modeling of elastoplastic deformation of the rock under consideration,including processes such as hydraulic fracture growth in a poroelastoplastic medium.[Significance]The suggested procedure to interpret results of experimental studies can be used for further numerical modeling of mechanical processes in rock masses with inelastic strain accumulation.This opportunity can increase the reliability of geomechanical models used for the optimization of hydrocarbon fields development.展开更多

BACKGROUND Pain in the back or pelvis or fear of back pain may affect the timing or cocontraction of the core muscles.In both static and dynamic movements,the Sahrmann core stability test provides an assessment of cor...BACKGROUND Pain in the back or pelvis or fear of back pain may affect the timing or cocontraction of the core muscles.In both static and dynamic movements,the Sahrmann core stability test provides an assessment of core muscle activation and a person's ability to stabilize the lumbopelvic complex.Preparatory cues and images can be used to increase the activation of these muscles.To attain optimal movement patterns,it will be necessary to determine what cueing will give the most effective results for core stability.AIM To investigate the effects of external and internal cues on core muscle activation during the Sahrmann five-level core stability test.METHODS Total 68 participants(21.83±3.47 years)were randomly allocated to an external(n=35)or internal cue group(n=33).Participants performed the Sahrmann fivelevel core stability test without a cue as baseline and the five-level stability exercises with an internal or external cue.External cue group received a pressure biofeedback unit(PBU),and the internal cue group received an audio cue.A Delsys Trigno^(TM)surface electromyography unit was used for muscle activation from the rectus abdominis,external oblique,and transverse abdominis/internal oblique muscles.RESULTS Linear mixed effects model analysis showed that cueing had a significant effect on core muscle activation(P=0.001);however,there was no significant difference between cue types(internal or external)(P=0.130).CONCLUSION Both external and internal cueing have significant effects on core muscle activation during the Sahrmann five-level core stability test and the PBU does not create higher muscle activation than internal cueing.展开更多

This study explores ethical violations and audit failures in various large-scale corporate fraud cases.Using a qualitative descriptive method and based on secondary data from documented fraud cases and published audit...This study explores ethical violations and audit failures in various large-scale corporate fraud cases.Using a qualitative descriptive method and based on secondary data from documented fraud cases and published audit reports,the study applies the Fraud Triangle framework,focusing on how weak integrity,objectivity,and professional competence have undermined public trust in the auditing profession.Using a qualitative descriptive method and the Fraud Triangle framework,which includes pressure,opportunity,and rationalization,the study analyzes cases from Indonesia(SNP Finance,Jiwasraya),China(Evergrande),and Germany(Wirecard).The analysis reveals that many audit failures observed in this study appear to stem more from ethical challenges than from technical incapability,driven by client pressure,weak internal controls,and compromised auditor independence.These cases demonstrate a recurring global pattern in which auditors neglect their responsibility to act in the public interest.展开更多

文摘Internal learning-based video inpainting methods have shown promising results by exploiting the intrinsic properties of the video to fill in the missing region without external dataset supervision.However,existing internal learning-based video inpainting methods would produce inconsistent structures or blurry textures due to the insufficient utilisation of motion priors within the video sequence.In this paper,the authors propose a new internal learning-based video inpainting model called appearance consistency and motion coherence network(ACMC-Net),which can not only learn the recurrence of appearance prior but can also capture motion coherence prior to improve the quality of the inpainting results.In ACMC-Net,a transformer-based appearance network is developed to capture global context information within the video frame for representing appearance consistency accurately.Additionally,a novel motion coherence learning scheme is proposed to learn the motion prior in a video sequence effectively.Finally,the learnt internal appearance consistency and motion coherence are implicitly propagated to the missing regions to achieve inpainting well.Extensive experiments conducted on the DAVIS dataset show that the proposed model obtains the superior performance in terms of quantitative measurements and produces more visually plausible results compared with the state-of-the-art methods.

文摘CLIL,which stands for Content and Language Integrated Learning,is an instructional approach that gives ample curricular and pedagogical attention to content and language outcomes in multilingual educational settings.Increasingly,it is heralded as a way to responsibly enact top-down English-Medium-of-Instruction(EMI)policies at the university level,where teachers and students are tasked with developing their English proficiency while remaining competitive in the international job market.However,teachers and teacher educators hoping to implement this approach in their science,technology,engineering and mathematics(STEM)content courses face significant challenges.This article serves as an introduction to a vip-edited special issue that reports on several aspects related to a project of international collaboration called Project SCILLA,an acronym for“STEM Content Integrated with Language-Learning Activities”.We first provide a brief overview of the project,which was developed and carried out in collaboration between Michigan State University and a consortium of 10 rural universities in Kazakhstan as a way to support STEM educators who wish to adapt their teaching practices to Kazakhstan’s Ministry of Education.We then offer an overview of the six articles that comprise the special issue,and call for deliberate and dialogic international collaboration as a way to support teachers responding to language policy demands.

文摘Deep-time Earth research plays a pivotal role in deciphering the rates,patterns,and mechanisms of Earth's evolutionary processes throughout geological history,providing essential scientific foundations for climate prediction,natural resource exploration,and sustainable planetary stewardship.To advance Deep-time Earth research in the era of big data and artificial intelligence,the International Union of Geological Sciences initiated the“Deeptime Digital Earth International Big Science Program”(DDE)in 2019.At the core of this ambitious program lies the development of geoscience knowledge graphs,serving as a transformative knowledge infrastructure that enables the integration,sharing,mining,and analysis of heterogeneous geoscience big data.The DDE knowledge graph initiative has made significant strides in three critical dimensions:(1)establishing a unified knowledge structure across geoscience disciplines that ensures consistent representation of geological entities and their interrelationships through standardized ontologies and semantic frameworks;(2)developing a robust and scalable software infrastructure capable of supporting both expert-driven and machine-assisted knowledge engineering for large-scale graph construction and management;(3)implementing a comprehensive three-tiered architecture encompassing basic,discipline-specific,and application-oriented knowledge graphs,spanning approximately 20 geoscience disciplines.Through its open knowledge framework and international collaborative network,this initiative has fostered multinational research collaborations,establishing a robust foundation for next-generation geoscience research while propelling the discipline toward FAIR(Findable,Accessible,Interoperable,Reusable)data practices in deep-time Earth systems research.
文摘BACKGROUND Monkeypox(Mpox),is a disease of global public health concern,as it does not affect only countries in western and central Africa.AIM To assess Burundi healthcare workers(HCWs)s’level of knowledge and confidence in the diagnosis and management of Mpox.METHODS We conducted a cross-sectional study via an online survey designed mainly from the World Health Organization course distributed among Burundi HCWs from June-July 2023.The questionnaire comprises 8 socioprofessional-related questions,22 questions about Mpox disease knowledge,and 3 questions to assess confidence in Mpox diagnosis and management.The data were analyzed via SPSS software version 25.0.A P value<0.05 was considered to indicate statistical significance.RESULTS The study sample comprised 471 HCWs who were mainly medical doctors(63.9%)and nurses(30.1%).None of the 22 questions concerning Mpox knowledge had at least 50%correct responses.A very low number of HCWs(17.4%)knew that Mpox has a vaccine.The confidence level to diagnose(21.20%),treat(18.00%)or prevent(23.30%)Mpox was low among HCWs.The confidence level in the diagnosis of Mpox was associated with the HCWs’age(P value=0.009),sex(P value<0.001),work experience(P value=0.002),and residence(P value<0.001).The confidence level to treat Mpox was significantly associated with the HCWs’age(P value=0.050),sex(P value<0.001),education(P value=0.033)and occupation(P value=0.005).The confidence level to prevent Mpox was associated with the HCWs’education(P value<0.001),work experience(P value=0.002),residence(P value<0.001)and type of work institution(P value=0.003).CONCLUSION This study revealed that HCWs have the lowest level of knowledge regarding Mpox and a lack of confidence in the ability to diagnose,treat or prevent it.There is an urgent need to organize continuing medical education programs on Mpox epidemiology and preparedness for Burundi HCWs.We encourage future researchers to assess potential hesitancy toward Mpox vaccination and its associated factors.

文摘The algebraic reconstruction technique(ART),multiplicative algebraic reconstruction technique(MART),and simultaneous iterative reconstruction technique(SIRT)are computational methodologies extensively utilized within the field of computerized ionospheric tomography(CIT)to facilitate three-dimensional reconstruction of the ionospheric morphology.However,reconstruction accuracy elicits recurrent disputes over its practical application,and people usually attribute this issue to incomplete and uneven coverage of the measurements.The Thermosphere Ionosphere Electrodynamics General Circulation Model(TIEGCM)offers a reasonable physics-based ionospheric background and is widely utilized in ionospheric research.We use the TIEGCM simulations as the targeted ionosphere because the current measurements are far from able to realistically reproduce the ionosphere in detail.Optimized designations of satellite measurements are conducted to investigate the limiting performance of CIT methods in ionospheric reconstruction.Similar to common practice,electron density distributions from outputs of the International Reference Ionosphere(IRI)model are used as the iterative initial value in CIT applications.The outcomes suggest that despite data coverage,iterative initial conditions also play an essential role in ionospheric reconstruction.In particular,in the longitudinal sectors where the iterative initial height of the F2-layer peak electron density(hmF2)differs substantially from the background densities,none of the three CIT methods can reproduce the exact background profile.When hmF2 is close but the ionospheric F2-layer peak density(NmF2)is different between the targeted background and initial conditions,the MART performs better than the ART and SIRT,as evidenced by the correlation coefficients of MART being above 0.97 and those of ART and SIRT being below 0.85.In summary,this investigation reveals the potential uncertainties in traditional CIT reconstruction,particularly when realistic hmF2 or NmF2 values differ substantially from the initial CIT conditions.

文摘[Objective]The work is devoted to the study of irreversible deformation of artificial samples subjected to a set of standard experiments,with an aim to study their mechanical properties.The principal idea of the study is related to the preparation of an artificial material with an established constitutive behavior model.The existence of such a well-described material provides future opportunities to conduct controllable experiments on various mechanical processes in rock-like material for further development and validation of theoretical models used in rock mechanics.[Methods]A set of artificial samples was prepared for careful assessment through a number of loading tests.Experimental work was carried out to determine the rheological properties under conditions of triaxial compression tests and uniaxial tension.Triaxial loading tests are completed for 9 samples with varying radial stress levels(0-5 MPa).The samples are loaded up to the yield point with control of radial and volumetric strain.The experimental results,which contain the obtained interrelationships between axial and radial stresses and strains,are analyzed using the Drucker-Prager yield surface.Material hardening is taken into account through the non-associated plastic flow law with the cap model.Numerical modeling of sample loading is performed through the finite difference method.Mathematical model parameters are adjusted to minimize the discrepancy between numerical modeling results and experimental data.The design of a series of experimental studies necessary to determine all the parameters of the model has been studied.[Results]It is shown that the formulated mathematical model allows to reliably reproduce the inelastic behavior of the studied material,and it can be used to solve a set of applied problems in continuum mechanics,the problem of numerical simulation of hydraulic fracture growth in an elastoplastic medium in particular.It was found that for the entire range of applied lateral loads(0-5 MPa),the elastic limit varied from 2 to 4 MPa,after which the material began to behave plastically.It was also determined that at lateral loads≥3 MPa,compaction began to appear in the material beyond the yield point.Judging by the dependence of volumetric strains under a lateral load equal to 1.4 MPa,compaction should begin to appear even at lateral loads lower than 3 MPa.[Conclusion]Taking the plastic behavior of the material into account is necessary when moving on to modeling the hydraulic fracturing process in such a material,and the resultant plasticity parameters for the model material can be used for numerical modeling of elastoplastic deformation of the rock under consideration,including processes such as hydraulic fracture growth in a poroelastoplastic medium.[Significance]The suggested procedure to interpret results of experimental studies can be used for further numerical modeling of mechanical processes in rock masses with inelastic strain accumulation.This opportunity can increase the reliability of geomechanical models used for the optimization of hydrocarbon fields development.

文摘BACKGROUND Pain in the back or pelvis or fear of back pain may affect the timing or cocontraction of the core muscles.In both static and dynamic movements,the Sahrmann core stability test provides an assessment of core muscle activation and a person's ability to stabilize the lumbopelvic complex.Preparatory cues and images can be used to increase the activation of these muscles.To attain optimal movement patterns,it will be necessary to determine what cueing will give the most effective results for core stability.AIM To investigate the effects of external and internal cues on core muscle activation during the Sahrmann five-level core stability test.METHODS Total 68 participants(21.83±3.47 years)were randomly allocated to an external(n=35)or internal cue group(n=33).Participants performed the Sahrmann fivelevel core stability test without a cue as baseline and the five-level stability exercises with an internal or external cue.External cue group received a pressure biofeedback unit(PBU),and the internal cue group received an audio cue.A Delsys Trigno^(TM)surface electromyography unit was used for muscle activation from the rectus abdominis,external oblique,and transverse abdominis/internal oblique muscles.RESULTS Linear mixed effects model analysis showed that cueing had a significant effect on core muscle activation(P=0.001);however,there was no significant difference between cue types(internal or external)(P=0.130).CONCLUSION Both external and internal cueing have significant effects on core muscle activation during the Sahrmann five-level core stability test and the PBU does not create higher muscle activation than internal cueing.

文摘This study explores ethical violations and audit failures in various large-scale corporate fraud cases.Using a qualitative descriptive method and based on secondary data from documented fraud cases and published audit reports,the study applies the Fraud Triangle framework,focusing on how weak integrity,objectivity,and professional competence have undermined public trust in the auditing profession.Using a qualitative descriptive method and the Fraud Triangle framework,which includes pressure,opportunity,and rationalization,the study analyzes cases from Indonesia(SNP Finance,Jiwasraya),China(Evergrande),and Germany(Wirecard).The analysis reveals that many audit failures observed in this study appear to stem more from ethical challenges than from technical incapability,driven by client pressure,weak internal controls,and compromised auditor independence.These cases demonstrate a recurring global pattern in which auditors neglect their responsibility to act in the public interest.
