BACKGROUND Colonic anastomotic leakage(AL)remains a feared complication of colorectal surgery.Usually,a defunctioning stoma or a proximal colostomy is performed to reduce the AL rate but cannot completely prevent AL.M...BACKGROUND Colonic anastomotic leakage(AL)remains a feared complication of colorectal surgery.Usually,a defunctioning stoma or a proximal colostomy is performed to reduce the AL rate but cannot completely prevent AL.Moreover,defunctioning colostomy is associated with high morbidity.This study assessed the feasibility of completely preventing colonic AL using total enteric flow diversion without a defunctioning stoma in a pig model of colonic AL.AIM To determine the feasibility of preventing colonic AL via total enteric flow diversion in pigs.METHODS A total of 14 pigs underwent surgery to create colon anaesthesia with severe defects for establishing the AL model.The pigs were then randomized into the control group(n=7),which received no further therapy,and a diversion group(n=7),which underwent placement of a modified ileostomy tube to divert the enteric contents from the colon externally.The general condition,serum Creactive protein level,white blood cell count,5-day incidence of colon AL,and development of abdominal abscesses were evaluated.RESULTS A modified ileostomy tube with a balloon was placed and pressurized to 20 kPa at a distance of 10-20 cm proximal to the ileocecal valve,effectively obstructing the intestine without causing injury and efficiently diverting the enteric contents.In the diversion group,no cases of peritonitis or abscess were observed.In contrast,all pigs in the control group developed either abdominal abscesses or peritonitis.CONCLUSION Instead of ileostomy or colostomy,the total enteric flow diversion technique with the placement of a modified ileostomy tube and balloon in the ileum can effectively or completely prevent colon AL.展开更多

BACKGROUND Progressive familial intrahepatic cholestasis type 1(PFIC-1)is a genetic cholestatic disease causing end-stage liver disease,which needs liver transplantation(LT).Simultaneous biliary diversion(BD)was recom...BACKGROUND Progressive familial intrahepatic cholestasis type 1(PFIC-1)is a genetic cholestatic disease causing end-stage liver disease,which needs liver transplantation(LT).Simultaneous biliary diversion(BD)was recommended to prevent allograft steatosis after transplantation,while increasing the risk of infection.Here,an attempt was made to perform BD using appendix to prevent bacterial translocation after LT.CASE SUMMARY An 11-month-old boy diagnosed with PFIC-1 received ABO compatible living donor LT due to refractory jaundice and pruritus.His mother donated her left lateral segment with a graft-to-recipient weight ratio of 2.9%.Internal BD was constructed during LT using the appendix by connecting its proximal end with the intrahepatic biliary duct and the distal end with colon.Biliary leakage was suspected on the 5th day after transplantation and exploratory laparotomy indicated biliary leakage at the cutting surface of liver.The liver function returned to normal on the 9th day post-operation and maintained normal during the 15-month follow-up.Cholangiography at 10 months after transplantation confirmed the direct secretion of bile into colon.Computerized tomography scan(4 months and 10 months)and liver biopsy(10 months)indicated no steatosis in the allograft.No complaint of recurrent diarrhea,infection or growth retardation was reported during follow-up.CONCLUSION Internal BD using appendix during LT is effective in preventing allograft steatosis and post-transplant infection in PFIC-1 recipients.展开更多
The South-to-North Water Diversion Project in China aims to address the imbalanced distribution of water resources between the northern and southern regions,a critical safeguard for sustainable socio-economic and ecol...The South-to-North Water Diversion Project in China aims to address the imbalanced distribution of water resources between the northern and southern regions,a critical safeguard for sustainable socio-economic and ecological development.In this study,we investigated the hydroclimatic evolution of the water source area and its driving mechanisms using the inflow runoff data at Danjiangkou Reservoir from 1954 to 2013,along with multiple gridded hydroclimatic datasets.Based on the correlations between instrumental runoff data and gridded hydroclimatic variables,we used linear regression to extend the long-term runoff record to the period of 1902-2019.Our results indicate that climate changes,dominated by regional wet-dry cycles,have significant impacts on runoff variations,while the influence of human activities remains comparatively limited.Danjiangkou Reservoir can maintain a balanced base flow,even during the operation of the water diversion project.Preliminary synoptic climatology analyses reveal that runoff variations are mainly driven by the El Niño-Southern Oscillation(ENSO)and the Pacific Walker Circulation(PWC),which affect runoff by altering large-scale ocean-to-continent water vapor processes.This study advances the field by integrating multi-source data with analytical techniques,which enhances understanding of long-term runoff changes in the Danjiangkou Reservoir and their climatic drivers,ultimately supporting sustainable water resource management.展开更多
Multi-stage and multi-cluster fracturing(MMF)is a crucial technology in unconventional oil and gas development,aiming to enhance production by creating extensive fracture networks.However,achieving uniform expansion o...Multi-stage and multi-cluster fracturing(MMF)is a crucial technology in unconventional oil and gas development,aiming to enhance production by creating extensive fracture networks.However,achieving uniform expansion of multi-cluster hydraulic fractures(HFs)in MMF remains a significant challenge.Field practice has shown that the use of temporary plugging and diversion fracturing(TPDF)can promote the balanced expansion of multi-cluster HFs.This study conducted TPDF experiments using a true triaxial fracturing simulation system setting a horizontal well completion with multi-cluster jetting perforations to investigate the equilibrium initiation and extension of multi-cluster fractures.The influence of key parameters,including cluster spacing,fracturing fluid viscosity,differential stress,and fracturing fluid injection rate,on fracture initiation and propagation was systematically examined.The results indicate that while close-spaced multi-cluster fracturing significantly increases the number of HFs,it also leads to uneven extension of HFs in their propagation.In contrast,TPDF demonstrates effectiveness in mitigating uneven HF extension,increasing the number of HFs,and creating a larger stimulated reservoir volume,ultimately leading to improved oil and gas well productivity.Moreover,under conditions of high differential stress,the differential stress within the formation exerts a stronger guiding effect in HFs,which are more closely aligned with the minimum principal stress.Low-viscosity fluids facilitate rapid and extensive fracture propagation within the rock formation.High-volume fluid injection,on the other hand,more comprehensively fills the formation.Therefore,employing lowviscosity and high-volume fracturing is advantageous for the initiation and extension of multi-cluster HFs.展开更多

Tajikistan represents a core region of the biodiversity hotspot in Central Asian mountains and has exceptional vascular plant diversity.However,the species diversity of the country faces urgent conservation challenges...Tajikistan represents a core region of the biodiversity hotspot in Central Asian mountains and has exceptional vascular plant diversity.However,the species diversity of the country faces urgent conservation challenges.There has been a lack of a comprehensive and multidimensional assessment to inform strategic conservation planning.Therefore,this study integrated 4 key biodiversity indices including species richness(SR),phylogenetic diversity(PD),threatened species richness(TSR),and endemic species richness(ESR)to map species diversity distribution patterns,identify conservation gaps,and elucidate their effects of climatic factors.This study revealed that species diversity shows a clear trend of decreasing from the western region to the eastern region of Tajikistan.The central–western mountains(specifically the Gissar-Darvasian and Zeravshanian regions)emerge as irreplaceable biodiversity hotspots.However,we found a severe spatial mismatch between these priority areas and the existing protected areas(PAs).Protection coverage for all hotspots was alarmingly low,ranging from 31.00%to 38.00%.Consequently,a critical 64.80%of integrated priority areas fall outside of the current PAs,representing a major conservation gap.This study identified precipitation seasonality and isothermality as the principal drivers,collectively explaining over 50.00%of the diversity variation and suggesting high vulnerability to hydrological shifts.Furthermore,we detected significant geographic sampling bias in the public biodiversity databases,with the most critical hotspot being systematically under-sampled.This study provides a robust scientific basis for conservation action,highlighting the urgent need to strategically expand PAs in the under-protected southwestern region and to mitigate critical sampling gaps through targeted data digitization and field surveys.These measures are indispensable for securing Tajikistan’s unique biodiversity and achieving the Kunming-Montreal Global Biodiversity Framework Target 3(“30×30 Protection”).展开更多
Haemosporidian parasites are widespread among birds globally,with impacts ranging from severe disease to negligible effects,particularly in host species that have coevolved with their parasites.Despite their ecologica...Haemosporidian parasites are widespread among birds globally,with impacts ranging from severe disease to negligible effects,particularly in host species that have coevolved with their parasites.Despite their ecological importance,the divereity and prevalence of these parasites in the Afrotropical region remain poorly understood,especially in urban environments.Our study investigated the prevalence and diversity of haemosporidian parasites in Afrotropical avian hosts,focusing on differences between urban and non-urban habitats.We screened 95birds from various species in Nigeria and investigated whether urbanization is associated with chan ges in the prevalence and richness of lineages of three haemosporidian parasites(Haemoproteus,Plasmodium,and Leucocytozoon).We found a haemosporidian prevalence of 36.8%with genus-specific differences between urban and non-urban habitats.The probability of Haemoproteus infection was higher in urban than non-urban habitats,but Plasmodium and Leucocytozoon did not differ between these habitats.Moreover,Haemoproteus lineages were exclusively found in urban habitats,while most Plasmodium lineages were restricted to non-urban habitats.Notably,we expanded the knowledge on diversity of haemosporidian lineages and avian hosts in the Afrotropics,with the first-ever record of hPYNJOC1 and pLUME2 lineages for the region,and the addition of new hosts for four Haemoproteus and two Plasmodium lineages.Our findings highlight the complexity of host-parasite relationships and the need for further research into the dynamics of haemosporidian parasites in Afrotropical avian hosts inhabiting diverse habitats.Overall,our study contributes to a better understanding of the prevalence,diversity,and distribution of haemosporidian parasites in the Afrotropics,emphasizing the importance of continued surveillance and monitoring to inform strategies for avian conservation and management.展开更多
Habitat loss driven by land-use change is a major factor shaping the dynamics of urban bird community structures.However,the potential mechanisms by which the spatial configuration and composition of blue-green infras...Habitat loss driven by land-use change is a major factor shaping the dynamics of urban bird community structures.However,the potential mechanisms by which the spatial configuration and composition of blue-green infrastructure,recognized as biodiversity hotspots in urban landscapes,influence urban bird beta diversity remain insufficiently understood.This study was conducted in the built-up area of Yinchuan,an internationally recognized wetland city in Northwest China.From December 2023 to June 2024,we systematically surveyed bird communities during both the breeding and wintering periods across 29 blue-green space mosaics.We quantified taxonomic,functional,and phylogenetic beta diversity,along with their turnover component and nestedness-resultant component,based on both pairwise beta diversity and multiple-site beta diversity.We further assessed the relative importance of landscape variables and spatial geographic distance in shaping beta diversity patterns and used hierarchical modeling of species communities(HMSC)to explore the responses of bird occurrence and functional traits to landscape variables.Our results revealed that species turnover was the dominant driver of taxonomic,functional,and phylogenetic beta diversity.Seasonal differences were observed in the effects of spatial geographic distance and landscape structure on beta diversity and its components,with landscape variables showing higher explanatory power than geographic isolation.In the breeding period,landscape diversity and waterbody area had positive effects on bird occurrence,whereas in the wintering period,most landscape features—except for landscape diversity—exerted neutral or negative influences.Regarding functional traits,we found that reproductive traits,flight ability,and foraging characteristics responded significantly to landscape structure,and that some small-bodied species active in aerial and canopy layers were more adaptable to habitat fragmentation.This study provides novel insights into the assembly processes and driving mechanisms of urban bird communities and offers scientific support for the notion that designing and maintaining blue-green infrastructure can contribute to urban biodiversity conservation.展开更多

The mai...The Fujian oyster(Crassostrea angulata) is an economically significant shellfish species distributed mainly along the Fujian coast, Southeast China. However, its genetic diversity and structure remain unclear. The main distribution area of the C. angulata is located in Fujian, South China. In total, 420 C. angulata were collected from 14 natural habitats(populations) along the Fujian coast, and their genetic diversity and structure were analyzed in the mitochondrial COI and nuclear gene ITS2 sequences. Results reveal that all the 14 populations of C. angulata exhibited high levels of genetic diversity, with a total of 57(haplotype diversity: 0.811±0.016) and 124(haplotype diversity: 0.912±0.007) haplotypes revealed by COI and ITS2, respectively. Notably, significant intermediate level of genetic differentiations between the Ningde Zhujiang(ZJ) population(FS T by COI: 0.035–0.142, P<0.05;FS T by ITS2: 0.078–0.123, P<0.05) with other populations were observed for the first time, which is also supported by the results of molecular variance analysis(FC T by COI: 0.105, P<0.05;FC T by ITS2: 0.086, P<0.05) and the clustering of the ZJ population into distinct branches in the interpopulation genetic differentiation tree. Furthermore, the evolutionary tree and haplotype network analyses do not support the formation of a clear geographical genealogical structure among these 14 populations. In addition, the population dynamics analysis suggests that the C. angulata may have undergone expansion during the third ice age of the Pleistocene. These results provide a reference for the preservation and further genetic improvement of C. angulata.展开更多
The latitudinal diversity gradient(LDG)is one of the most notable biodiversity patterns in biogeography.The metabolic theory of ecology(MTE)explains ecological patterns,including the LDG.However,little is known about ...The latitudinal diversity gradient(LDG)is one of the most notable biodiversity patterns in biogeography.The metabolic theory of ecology(MTE)explains ecological patterns,including the LDG.However,little is known about whether the LDG remains stable over time as climate warming progresses and whether MTE remains applicable to clarify this pattern.In this study,forest data spanning temperate,subtropical,and tropical zones across China were used to analyze long-term changes in the LDG of tree species over 2005-2020.Based on the MTE framework,spatial scales were considered to assess temperature dependence of typical forest trees species.Our results show that species richness decreased with increasing latitude,and that temperature was the primary driver of this change.Although temperature in China has significantly increased over the past two decades,the LDG of tree species has remained stable.However,there was a decrease in species richness in tropical regions over time.With predictions of the MTE,the logarithm of typical forest tree species richness exhibited negative linear relationships with the inverse of ambient temperature,indicating temperature dependence of species richness.However,the relationship remained stable and was strongly influenced by spatial scale,intensifying as spatial scale increased.The findings emphasize the important role of temperature in shaping the LDG.The effects of spatial scale,in particular,should be considered when biodiversity management plans are developed for future climate change.展开更多

Similar to diversion colitis, diversion pouchitis is an inflammatory disorder occ...Diversion colitis is characterized by inflammation of the mucosa in the defunctioned segment of the colon after colostomy or ileostomy. Similar to diversion colitis, diversion pouchitis is an inflammatory disorder occurring in the ileal pouch, resulting from the exclusion of the fecal stream and a subsequent lack of nutrients from luminal bacteria. Although the vast majority of patients with surgically-diverted gastrointestinal tracts remain asymptomatic, it has been reported that diversion colitis and pouchitis might occur in almost all patients with diversion. Surgical closure of the stoma, with reestablishment of gut continuity, is the only curative intervention available for patients with diversion disease. Pharmacologic treatments using short-chain fatty acids, mesalamine, or corticosteroids are reportedly effective for those who are not candidates for surgical reestablishment; however, there are no established assessment criteria for determining the severity of diversion colitis, and no management strategies to date. Therefore, in this mini-review, we summarize and review various recently-reported treatments for diversion disease. We are hopeful that the information summarized here will assist physicians who treat patients with diversion colitis and pouchitis, leading to better case management.展开更多
AIM:To investigated the incidence of diversion colitis(DC) and impact of DC symptoms on quality of life(QoL) after ileostomy reversal in rectal cancer.METHODS:We performed a prospective study with 30 patients who unde...AIM:To investigated the incidence of diversion colitis(DC) and impact of DC symptoms on quality of life(QoL) after ileostomy reversal in rectal cancer.METHODS:We performed a prospective study with 30 patients who underwent low anterior resection and the creation of a temporary ileostomy for the rectal cancer between January 2008 and July 2009 at the Department of Surgery,Korea University Anam Hospital.The participants totally underwent two rounds of the examinations.At first examination,endoscopies,tissue biopsies,and questionnaire survey about the symptom were performed 3-4 mo after the ileostomy creations.At second examination,endoscopies,tissue biopsies,and questionnaire survey about the symptom and QoL were performed 5-6 mo after the ileostomy reversals.Clinicopathological data were based on the histopathological reports and clinical records of the patients.RESULTS:At the first examination,all of the patients presented with inflammation,which was mild in 15(50%) patients,moderate in 11(36.7%) and severe in 4(13.3%) by endoscopy and mild in 14(46.7%) and moderate in 16(53.3%) by histology.At the second examination,only 11(36.7%) and 17(56.7%) patients had mild inflammation by endoscopy and histology,respectively.There was no significant difference in DC grade between the endoscopic and the histological findings at first or second examination.The symptoms detected on the first and second questionnaires were mucous discharge in 12(40%) and 5(17%) patients,bloody discharge in 5(17%) and 3(10%) patients,abdominal pain in 4(13%) and 2(7%) patients and tenesmus in 9(30%) and 5(17%) patients,respectively.We found no correlation between the endoscopic or histological findings and the symptoms such as mucous discharge,bleeding,abdominal pain and tenesmus in both time points.Diarrhea was detected in 9 patients at the second examination;this number correlated with the severity of DC(0%,0%,66.7%,33.3% vs 0%,71.4%,23.8%,4.8%,P = 0.001) and the symptom-related QoL(r =-0.791,P < 0.001).CONCLUSION:The severity of DC is related to diarrhea after an ileostomy reversal and may adversely affect QoL.展开更多
AIM:To investigate the effect of plasmapheresis via the portal vein for"small-for-size"syndrome(SFSS)aided by extracorporeal continuous portal diversion(ECPD).METHODS:Extensive or total hepatectomy in the pi...AIM:To investigate the effect of plasmapheresis via the portal vein for"small-for-size"syndrome(SFSS)aided by extracorporeal continuous portal diversion(ECPD).METHODS:Extensive or total hepatectomy in the pig is usually adopted as a postoperative liver failure(PLF)or SFSS model.In this study,animals which underwent85%-90%hepatectomy were randomized into either the Systemic group(n=7)or the Portal group(n=7).In the Systemic group,all pigs received temporal plasmapheresis(PP)via the extracorporeal catheter circuit(systemic to systemic circulation)from 24 to 30 h posthepatectomy(PH);in the Portal group,all pigs received ECPD to divert partial portal vein flow(PVF)to the systemic circulation after hepatectomy,then converted to temporal PP from 24 to 30 h PH,and subsequently converted to ECPD again until 48 h PH.In the Portal group,the PVF was preserved at 3.0-3.3 times that of the baseline value,similar to that following 70%hepatectomy,which was regarded as the optimal PVF to the hypertrophic liver remnant.At 48 h PH,all pigs were re-opened and the portal vein pressure(PVP),PVF,and HAF(hepatic artery flow)were measured,and then diversion of the portal venous flow was terminated.After1 h the PVP,PVF,and HAF were re-measured.The portal hemodynamic changes,liver injury,liver regeneration and bacterial/lipopolysaccharide(LPS)translocation were evaluated in the two groups.RESULTS:The PVP in the Portal group was significantly lower than that in the Systemic group during the time period from 2 to 49 h PH(P<0.05).Serum alanine aminotransferase(ALT),total bilirubin(TB)and ammonia were significantly reduced in the Portal group compared with the Systemic group from 24 to 48 h PH(P<0.05).The Portal group may have attenuated sinusoidal endothelial injury and decreased the level of HA compared with the Systemic group.In the Systemic group,there was significant sinusoidal dilation,hydropic changes in hepatocytes and hemorrhage into the hepatic parenchyma,and the sinusoidal endothelial lining was partially destroyed and detached into the sinusoidal space.CD31immunostaining revealed significant destruction of the endothelial lining.In the Portal group,there was no intraparenchymal hemorrhage and the sinusoidal endothelial cells and hepatocytes were well preserved.CD31immunostaining was mild which indicated less destruction of the endothelial lining.HA was significantly decreased in the Portal group compared with the Systemic group from 2 to 48 h PH.The rate of liver remnant regeneration was elevated,while apoptosis was attenuated in the Portal group compared with the Systemic group.Thymidine kinase activity was much higher in the Portal group than in the Systemic group at 48 h PH.The PCNA index was significantly increased and the apoptotic index was significantly decreased in the Portal group compared with the Systemic group.Bacterial translocation and endotoxin,as well as the inflammatory response,were significantly attenuated in the Portal group compared with the Systemic group.LPS,tumor necrosis factor-and interleukin-6 levels were all significantly decreased in the Portal group compared with the Systemic group from 24 to48 h PH,while bacterial DNA level was significantly decreased from 2 to 48 h PH.CONCLUSION:PP plus ECPD via the portal vein can attenuate toxic load and hyperperfusion injury,and should be undertaken instead of PP via the systemic circulation in SFSS or PLF.展开更多

New drainage tunnels were considered. Furthermore, lining structures and grouting pressures were modified during the excavation of tunnels. The construction scheme was updated dynamically based on the complex geological conditions. For instances, the diversion tunnels were first excavated by drilling and blasting method at the first stage of construction, and then by the combination method of tunnel boring machine (TBM) and drilling and blasting, and finally by drilling and blasting method. Through optimized scheme and updated construction scheme, the excavation of diversion tunnel #1 was successfully completed in June, 2011. This paper summarizes the key issues in rock mechanics associated with the construction of the long and deep diversion tunnels at Jinping II hydropower station. The experiences of design and construction obtained from this project could provide reference to similar projects.展开更多

A key issue is the lack of a fully-fledged theory and method to clearly define the design scope, accounting and feasibility of PES criteria. An improved watershed criteria model was developed in light of research on PES practices in China, investigations on the water source area for the Middle Route Project of South-to-North Water Diversion and ecosystem services outflows theory. The basic principle of assessment is the direct and opportunity cost for ecological conservation and environmental protection in the water source area deduct nationally-financed PES and internal effect. Then the scope and the criteria methods were determined, and internal effect was put forward to define benefits brought from water source area. Finally, Shiyan City, which is the main water source area for the Project of Water Diversion, was analyzed by this model and its payment was calculated. The results showed that: (1) during 2003–2050, the total direct cost and opportunity cost would reach up to 262.70 billion and 256.33 billion Chinese Yuan (CNY, 2000 constant prices), i.e., 50.61% and 49.38% of total cost, respectively; (2) Shiyan City would gain 0.23, 0.06 and 0.03 CNY/m3 in 2014–2020, 2021–2030, and 2031–2050, respectively.展开更多

Faced with market operations management of the water diversion system, this study defined the supply chain system for the SNWD Project, considering the actual project conditions, built a decentralized decision model and a centralized decision model with strategic customer behavior (SCB) using a floating pricing mechanism (FPM), and constructed a coordination mechanism via a revenue-sharing contract. The results suggest the following: (1) owing to water shortage supplements and the excess water sale policy provided by the FPM, the optimal ordering quantity of water resources is less than that without the FPM, and the optimal profits of the whole supply chain, supplier, and external distributor are higher than they would be without the FPM; (2) wholesale pricing and supplementary wholesale pricing with SCB are higher than those without SCB, and the optimal profits of the whole supply chain, supplier, and external distributor are higher than they would be without SCB; and (3) considering SCB and introducing the FPM help increase the optimal profits of the whole supply chain, supplier, and external distributor, and improve the efficiency of water resources usage.展开更多
The south to the north project (WDP) on the saltwater intrusion in the Changjiang Estuary is studied by the improved three-dimensionai (3D) numerical model.The net unit width flux in the Changjiang Estuary as well as ...The south to the north project (WDP) on the saltwater intrusion in the Changjiang Estuary is studied by the improved three-dimensionai (3D) numerical model.The net unit width flux in the Changjiang Estuary as well as the sectional salt flux is calculated in the North Branch (NB),the South Branch (SB),the North Channel (NC),the South Channel (SC),the North Passage (NP) and the South Passage (SP),respectively.The net seaward water flux in the SB is reduced,and the net water flux spilling over from the NB to the SB is enhanced after the eastern WDP.Under the mean river discharge condition in the dry season,the net salt flux spilling over from the NB to the SB is increased by 2.09 t/s and 0.52 t/s during the spring and neap tides,respectively,due to the eastern WDP.The saltwater intrusion in the Changjiang Estuary is enhanced by the eastern WDP.Compared with that during the spring tide,the net water diversion ratio during the neap tide in the NC is smaller,and thus the enhancement of the saltwater intrusion by the eastern WDP is smaller in the NC,and larger in the NP and the SP.The tidally averaged surface salinity at the water intakes of the Dongfengxisha Reservoir,the Chenhang Reservoir and the Qingcaosha Reservoir rises both during the spring and neap tides.展开更多

We report the case of a46...In this case report, we examined the levels of cytokines expressed before and during fecal stream diversion and after intestinal continuity was restored in a patient with collagenous colitis. We report the case of a46-year-old woman with chronic, active collagenous colitis who either failed to achieve clinical remission or experienced adverse effects with the following drugs:loperamide, cholestyramine, budesonide, methotrexate and adalimumab. Due to the intractable nature of the disease and because the patient was having up to 15 watery bowel movements per day, she underwent a temporary ileostomy. Colonic biopsies were analyzed for mucosal cytokine protein levels before and during fecal stream diversion and after intestinal continuity was restored. Mucosal protein levels of interleukin(IL)-1β,IL-2, IL-6, IL-12, IL-17 A, IL-23, TNF, IFN-γ, IL-4, IL-5,IL-10 and IL-13 were all higher during active disease and decreased to non-detectable or considerably lower levels during fecal stream diversion. One month after the restoration of bowel continuity, when the patient experienced a relapse of symptoms, IL-2, IL-23 and IL-21 levels were again increased. Our results indicate that fecal stream diversion in this patient suppressed the levels of all cytokines analyzed in colonic biopsies. With the recurrence of clinical symptoms and histological changes after bowel reconstruction,the levels of primarily proinflammatory cytokines increased. Our findings support the hypothesis that a luminal factor triggers the inflammation observed in collagenous colitis.展开更多

文摘BACKGROUND Colonic anastomotic leakage(AL)remains a feared complication of colorectal surgery.Usually,a defunctioning stoma or a proximal colostomy is performed to reduce the AL rate but cannot completely prevent AL.Moreover,defunctioning colostomy is associated with high morbidity.This study assessed the feasibility of completely preventing colonic AL using total enteric flow diversion without a defunctioning stoma in a pig model of colonic AL.AIM To determine the feasibility of preventing colonic AL via total enteric flow diversion in pigs.METHODS A total of 14 pigs underwent surgery to create colon anaesthesia with severe defects for establishing the AL model.The pigs were then randomized into the control group(n=7),which received no further therapy,and a diversion group(n=7),which underwent placement of a modified ileostomy tube to divert the enteric contents from the colon externally.The general condition,serum Creactive protein level,white blood cell count,5-day incidence of colon AL,and development of abdominal abscesses were evaluated.RESULTS A modified ileostomy tube with a balloon was placed and pressurized to 20 kPa at a distance of 10-20 cm proximal to the ileocecal valve,effectively obstructing the intestine without causing injury and efficiently diverting the enteric contents.In the diversion group,no cases of peritonitis or abscess were observed.In contrast,all pigs in the control group developed either abdominal abscesses or peritonitis.CONCLUSION Instead of ileostomy or colostomy,the total enteric flow diversion technique with the placement of a modified ileostomy tube and balloon in the ileum can effectively or completely prevent colon AL.

文摘The hydrogen-enriched direct reduction shaft furnace addresses the high CO_(2) emissions associated with the blast furnace process.A discrete element method(DEM)model was introduced to explore how the structure of the diversion cone affects particle descent behavior in a hydrogen-enriched shaft furnace.The results indicated that in the absence of a diversion cone,the descending velocity near the furnace wall zone is significantly lower than that at its center,resulting in a‘V’-shaped burden flow pattern.The discharge velocity has a minor impact on the flow pattern in the shaft furnace.Upon installation of a diversion cone,burden descending velocity becomes more uniform,leading to a‘-’-shaped burden flow pattern.As the bottom of the diversion cone ascends(i.e.,the lower end of the diversion cone is progressively closer to the top),there is an increase in the volume fraction of the dead zone within the shaft furnace.This is particularly evident in the formation of a triangular dead zone at the base of the diversion cone.It is suggested that the lower cone of the bi-conical diversion cone should maintain a sufficient height.

文摘BACKGROUND Progressive familial intrahepatic cholestasis type 1(PFIC-1)is a genetic cholestatic disease causing end-stage liver disease,which needs liver transplantation(LT).Simultaneous biliary diversion(BD)was recommended to prevent allograft steatosis after transplantation,while increasing the risk of infection.Here,an attempt was made to perform BD using appendix to prevent bacterial translocation after LT.CASE SUMMARY An 11-month-old boy diagnosed with PFIC-1 received ABO compatible living donor LT due to refractory jaundice and pruritus.His mother donated her left lateral segment with a graft-to-recipient weight ratio of 2.9%.Internal BD was constructed during LT using the appendix by connecting its proximal end with the intrahepatic biliary duct and the distal end with colon.Biliary leakage was suspected on the 5th day after transplantation and exploratory laparotomy indicated biliary leakage at the cutting surface of liver.The liver function returned to normal on the 9th day post-operation and maintained normal during the 15-month follow-up.Cholangiography at 10 months after transplantation confirmed the direct secretion of bile into colon.Computerized tomography scan(4 months and 10 months)and liver biopsy(10 months)indicated no steatosis in the allograft.No complaint of recurrent diarrhea,infection or growth retardation was reported during follow-up.CONCLUSION Internal BD using appendix during LT is effective in preventing allograft steatosis and post-transplant infection in PFIC-1 recipients.

文摘The South-to-North Water Diversion Project in China aims to address the imbalanced distribution of water resources between the northern and southern regions,a critical safeguard for sustainable socio-economic and ecological development.In this study,we investigated the hydroclimatic evolution of the water source area and its driving mechanisms using the inflow runoff data at Danjiangkou Reservoir from 1954 to 2013,along with multiple gridded hydroclimatic datasets.Based on the correlations between instrumental runoff data and gridded hydroclimatic variables,we used linear regression to extend the long-term runoff record to the period of 1902-2019.Our results indicate that climate changes,dominated by regional wet-dry cycles,have significant impacts on runoff variations,while the influence of human activities remains comparatively limited.Danjiangkou Reservoir can maintain a balanced base flow,even during the operation of the water diversion project.Preliminary synoptic climatology analyses reveal that runoff variations are mainly driven by the El Niño-Southern Oscillation(ENSO)and the Pacific Walker Circulation(PWC),which affect runoff by altering large-scale ocean-to-continent water vapor processes.This study advances the field by integrating multi-source data with analytical techniques,which enhances understanding of long-term runoff changes in the Danjiangkou Reservoir and their climatic drivers,ultimately supporting sustainable water resource management.

文摘Multi-stage and multi-cluster fracturing(MMF)is a crucial technology in unconventional oil and gas development,aiming to enhance production by creating extensive fracture networks.However,achieving uniform expansion of multi-cluster hydraulic fractures(HFs)in MMF remains a significant challenge.Field practice has shown that the use of temporary plugging and diversion fracturing(TPDF)can promote the balanced expansion of multi-cluster HFs.This study conducted TPDF experiments using a true triaxial fracturing simulation system setting a horizontal well completion with multi-cluster jetting perforations to investigate the equilibrium initiation and extension of multi-cluster fractures.The influence of key parameters,including cluster spacing,fracturing fluid viscosity,differential stress,and fracturing fluid injection rate,on fracture initiation and propagation was systematically examined.The results indicate that while close-spaced multi-cluster fracturing significantly increases the number of HFs,it also leads to uneven extension of HFs in their propagation.In contrast,TPDF demonstrates effectiveness in mitigating uneven HF extension,increasing the number of HFs,and creating a larger stimulated reservoir volume,ultimately leading to improved oil and gas well productivity.Moreover,under conditions of high differential stress,the differential stress within the formation exerts a stronger guiding effect in HFs,which are more closely aligned with the minimum principal stress.Low-viscosity fluids facilitate rapid and extensive fracture propagation within the rock formation.High-volume fluid injection,on the other hand,more comprehensively fills the formation.Therefore,employing lowviscosity and high-volume fracturing is advantageous for the initiation and extension of multi-cluster HFs.
文摘In this paper,we propose a random access scheme termed sign-compute diversity slotted ALOHA(SCDSA).The SCDSA scheme combines diversity transmission with compute-and-forward.Without considering the capture effect and multiple user detection techniques,our scheme can reach a high throughput of 0.98 without feedback under finite frame size settings,where the upper bound on performance is 1.Moreover,a lower bound on throughput performance is derived,which is tight in some parameter settings and can be used to approximate theoretical performance.Simulation results validate our analysis and confirm the advantages of our proposed scheme.

文摘Tajikistan represents a core region of the biodiversity hotspot in Central Asian mountains and has exceptional vascular plant diversity.However,the species diversity of the country faces urgent conservation challenges.There has been a lack of a comprehensive and multidimensional assessment to inform strategic conservation planning.Therefore,this study integrated 4 key biodiversity indices including species richness(SR),phylogenetic diversity(PD),threatened species richness(TSR),and endemic species richness(ESR)to map species diversity distribution patterns,identify conservation gaps,and elucidate their effects of climatic factors.This study revealed that species diversity shows a clear trend of decreasing from the western region to the eastern region of Tajikistan.The central–western mountains(specifically the Gissar-Darvasian and Zeravshanian regions)emerge as irreplaceable biodiversity hotspots.However,we found a severe spatial mismatch between these priority areas and the existing protected areas(PAs).Protection coverage for all hotspots was alarmingly low,ranging from 31.00%to 38.00%.Consequently,a critical 64.80%of integrated priority areas fall outside of the current PAs,representing a major conservation gap.This study identified precipitation seasonality and isothermality as the principal drivers,collectively explaining over 50.00%of the diversity variation and suggesting high vulnerability to hydrological shifts.Furthermore,we detected significant geographic sampling bias in the public biodiversity databases,with the most critical hotspot being systematically under-sampled.This study provides a robust scientific basis for conservation action,highlighting the urgent need to strategically expand PAs in the under-protected southwestern region and to mitigate critical sampling gaps through targeted data digitization and field surveys.These measures are indispensable for securing Tajikistan’s unique biodiversity and achieving the Kunming-Montreal Global Biodiversity Framework Target 3(“30×30 Protection”).

文摘Haemosporidian parasites are widespread among birds globally,with impacts ranging from severe disease to negligible effects,particularly in host species that have coevolved with their parasites.Despite their ecological importance,the divereity and prevalence of these parasites in the Afrotropical region remain poorly understood,especially in urban environments.Our study investigated the prevalence and diversity of haemosporidian parasites in Afrotropical avian hosts,focusing on differences between urban and non-urban habitats.We screened 95birds from various species in Nigeria and investigated whether urbanization is associated with chan ges in the prevalence and richness of lineages of three haemosporidian parasites(Haemoproteus,Plasmodium,and Leucocytozoon).We found a haemosporidian prevalence of 36.8%with genus-specific differences between urban and non-urban habitats.The probability of Haemoproteus infection was higher in urban than non-urban habitats,but Plasmodium and Leucocytozoon did not differ between these habitats.Moreover,Haemoproteus lineages were exclusively found in urban habitats,while most Plasmodium lineages were restricted to non-urban habitats.Notably,we expanded the knowledge on diversity of haemosporidian lineages and avian hosts in the Afrotropics,with the first-ever record of hPYNJOC1 and pLUME2 lineages for the region,and the addition of new hosts for four Haemoproteus and two Plasmodium lineages.Our findings highlight the complexity of host-parasite relationships and the need for further research into the dynamics of haemosporidian parasites in Afrotropical avian hosts inhabiting diverse habitats.Overall,our study contributes to a better understanding of the prevalence,diversity,and distribution of haemosporidian parasites in the Afrotropics,emphasizing the importance of continued surveillance and monitoring to inform strategies for avian conservation and management.

文摘Habitat loss driven by land-use change is a major factor shaping the dynamics of urban bird community structures.However,the potential mechanisms by which the spatial configuration and composition of blue-green infrastructure,recognized as biodiversity hotspots in urban landscapes,influence urban bird beta diversity remain insufficiently understood.This study was conducted in the built-up area of Yinchuan,an internationally recognized wetland city in Northwest China.From December 2023 to June 2024,we systematically surveyed bird communities during both the breeding and wintering periods across 29 blue-green space mosaics.We quantified taxonomic,functional,and phylogenetic beta diversity,along with their turnover component and nestedness-resultant component,based on both pairwise beta diversity and multiple-site beta diversity.We further assessed the relative importance of landscape variables and spatial geographic distance in shaping beta diversity patterns and used hierarchical modeling of species communities(HMSC)to explore the responses of bird occurrence and functional traits to landscape variables.Our results revealed that species turnover was the dominant driver of taxonomic,functional,and phylogenetic beta diversity.Seasonal differences were observed in the effects of spatial geographic distance and landscape structure on beta diversity and its components,with landscape variables showing higher explanatory power than geographic isolation.In the breeding period,landscape diversity and waterbody area had positive effects on bird occurrence,whereas in the wintering period,most landscape features—except for landscape diversity—exerted neutral or negative influences.Regarding functional traits,we found that reproductive traits,flight ability,and foraging characteristics responded significantly to landscape structure,and that some small-bodied species active in aerial and canopy layers were more adaptable to habitat fragmentation.This study provides novel insights into the assembly processes and driving mechanisms of urban bird communities and offers scientific support for the notion that designing and maintaining blue-green infrastructure can contribute to urban biodiversity conservation.

文摘The Fujian oyster(Crassostrea angulata) is an economically significant shellfish species distributed mainly along the Fujian coast, Southeast China. However, its genetic diversity and structure remain unclear. The main distribution area of the C. angulata is located in Fujian, South China. In total, 420 C. angulata were collected from 14 natural habitats(populations) along the Fujian coast, and their genetic diversity and structure were analyzed in the mitochondrial COI and nuclear gene ITS2 sequences. Results reveal that all the 14 populations of C. angulata exhibited high levels of genetic diversity, with a total of 57(haplotype diversity: 0.811±0.016) and 124(haplotype diversity: 0.912±0.007) haplotypes revealed by COI and ITS2, respectively. Notably, significant intermediate level of genetic differentiations between the Ningde Zhujiang(ZJ) population(FS T by COI: 0.035–0.142, P<0.05;FS T by ITS2: 0.078–0.123, P<0.05) with other populations were observed for the first time, which is also supported by the results of molecular variance analysis(FC T by COI: 0.105, P<0.05;FC T by ITS2: 0.086, P<0.05) and the clustering of the ZJ population into distinct branches in the interpopulation genetic differentiation tree. Furthermore, the evolutionary tree and haplotype network analyses do not support the formation of a clear geographical genealogical structure among these 14 populations. In addition, the population dynamics analysis suggests that the C. angulata may have undergone expansion during the third ice age of the Pleistocene. These results provide a reference for the preservation and further genetic improvement of C. angulata.

文摘The latitudinal diversity gradient(LDG)is one of the most notable biodiversity patterns in biogeography.The metabolic theory of ecology(MTE)explains ecological patterns,including the LDG.However,little is known about whether the LDG remains stable over time as climate warming progresses and whether MTE remains applicable to clarify this pattern.In this study,forest data spanning temperate,subtropical,and tropical zones across China were used to analyze long-term changes in the LDG of tree species over 2005-2020.Based on the MTE framework,spatial scales were considered to assess temperature dependence of typical forest trees species.Our results show that species richness decreased with increasing latitude,and that temperature was the primary driver of this change.Although temperature in China has significantly increased over the past two decades,the LDG of tree species has remained stable.However,there was a decrease in species richness in tropical regions over time.With predictions of the MTE,the logarithm of typical forest tree species richness exhibited negative linear relationships with the inverse of ambient temperature,indicating temperature dependence of species richness.However,the relationship remained stable and was strongly influenced by spatial scale,intensifying as spatial scale increased.The findings emphasize the important role of temperature in shaping the LDG.The effects of spatial scale,in particular,should be considered when biodiversity management plans are developed for future climate change.
文摘Diversion colitis is characterized by inflammation of the mucosa in the defunctioned segment of the colon after colostomy or ileostomy. Similar to diversion colitis, diversion pouchitis is an inflammatory disorder occurring in the ileal pouch, resulting from the exclusion of the fecal stream and a subsequent lack of nutrients from luminal bacteria. Although the vast majority of patients with surgically-diverted gastrointestinal tracts remain asymptomatic, it has been reported that diversion colitis and pouchitis might occur in almost all patients with diversion. Surgical closure of the stoma, with reestablishment of gut continuity, is the only curative intervention available for patients with diversion disease. Pharmacologic treatments using short-chain fatty acids, mesalamine, or corticosteroids are reportedly effective for those who are not candidates for surgical reestablishment; however, there are no established assessment criteria for determining the severity of diversion colitis, and no management strategies to date. Therefore, in this mini-review, we summarize and review various recently-reported treatments for diversion disease. We are hopeful that the information summarized here will assist physicians who treat patients with diversion colitis and pouchitis, leading to better case management.

文摘AIM:To investigated the incidence of diversion colitis(DC) and impact of DC symptoms on quality of life(QoL) after ileostomy reversal in rectal cancer.METHODS:We performed a prospective study with 30 patients who underwent low anterior resection and the creation of a temporary ileostomy for the rectal cancer between January 2008 and July 2009 at the Department of Surgery,Korea University Anam Hospital.The participants totally underwent two rounds of the examinations.At first examination,endoscopies,tissue biopsies,and questionnaire survey about the symptom were performed 3-4 mo after the ileostomy creations.At second examination,endoscopies,tissue biopsies,and questionnaire survey about the symptom and QoL were performed 5-6 mo after the ileostomy reversals.Clinicopathological data were based on the histopathological reports and clinical records of the patients.RESULTS:At the first examination,all of the patients presented with inflammation,which was mild in 15(50%) patients,moderate in 11(36.7%) and severe in 4(13.3%) by endoscopy and mild in 14(46.7%) and moderate in 16(53.3%) by histology.At the second examination,only 11(36.7%) and 17(56.7%) patients had mild inflammation by endoscopy and histology,respectively.There was no significant difference in DC grade between the endoscopic and the histological findings at first or second examination.The symptoms detected on the first and second questionnaires were mucous discharge in 12(40%) and 5(17%) patients,bloody discharge in 5(17%) and 3(10%) patients,abdominal pain in 4(13%) and 2(7%) patients and tenesmus in 9(30%) and 5(17%) patients,respectively.We found no correlation between the endoscopic or histological findings and the symptoms such as mucous discharge,bleeding,abdominal pain and tenesmus in both time points.Diarrhea was detected in 9 patients at the second examination;this number correlated with the severity of DC(0%,0%,66.7%,33.3% vs 0%,71.4%,23.8%,4.8%,P = 0.001) and the symptom-related QoL(r =-0.791,P < 0.001).CONCLUSION:The severity of DC is related to diarrhea after an ileostomy reversal and may adversely affect QoL.
文摘AIM:To investigate the effect of plasmapheresis via the portal vein for"small-for-size"syndrome(SFSS)aided by extracorporeal continuous portal diversion(ECPD).METHODS:Extensive or total hepatectomy in the pig is usually adopted as a postoperative liver failure(PLF)or SFSS model.In this study,animals which underwent85%-90%hepatectomy were randomized into either the Systemic group(n=7)or the Portal group(n=7).In the Systemic group,all pigs received temporal plasmapheresis(PP)via the extracorporeal catheter circuit(systemic to systemic circulation)from 24 to 30 h posthepatectomy(PH);in the Portal group,all pigs received ECPD to divert partial portal vein flow(PVF)to the systemic circulation after hepatectomy,then converted to temporal PP from 24 to 30 h PH,and subsequently converted to ECPD again until 48 h PH.In the Portal group,the PVF was preserved at 3.0-3.3 times that of the baseline value,similar to that following 70%hepatectomy,which was regarded as the optimal PVF to the hypertrophic liver remnant.At 48 h PH,all pigs were re-opened and the portal vein pressure(PVP),PVF,and HAF(hepatic artery flow)were measured,and then diversion of the portal venous flow was terminated.After1 h the PVP,PVF,and HAF were re-measured.The portal hemodynamic changes,liver injury,liver regeneration and bacterial/lipopolysaccharide(LPS)translocation were evaluated in the two groups.RESULTS:The PVP in the Portal group was significantly lower than that in the Systemic group during the time period from 2 to 49 h PH(P<0.05).Serum alanine aminotransferase(ALT),total bilirubin(TB)and ammonia were significantly reduced in the Portal group compared with the Systemic group from 24 to 48 h PH(P<0.05).The Portal group may have attenuated sinusoidal endothelial injury and decreased the level of HA compared with the Systemic group.In the Systemic group,there was significant sinusoidal dilation,hydropic changes in hepatocytes and hemorrhage into the hepatic parenchyma,and the sinusoidal endothelial lining was partially destroyed and detached into the sinusoidal space.CD31immunostaining revealed significant destruction of the endothelial lining.In the Portal group,there was no intraparenchymal hemorrhage and the sinusoidal endothelial cells and hepatocytes were well preserved.CD31immunostaining was mild which indicated less destruction of the endothelial lining.HA was significantly decreased in the Portal group compared with the Systemic group from 2 to 48 h PH.The rate of liver remnant regeneration was elevated,while apoptosis was attenuated in the Portal group compared with the Systemic group.Thymidine kinase activity was much higher in the Portal group than in the Systemic group at 48 h PH.The PCNA index was significantly increased and the apoptotic index was significantly decreased in the Portal group compared with the Systemic group.Bacterial translocation and endotoxin,as well as the inflammatory response,were significantly attenuated in the Portal group compared with the Systemic group.LPS,tumor necrosis factor-and interleukin-6 levels were all significantly decreased in the Portal group compared with the Systemic group from 24 to48 h PH,while bacterial DNA level was significantly decreased from 2 to 48 h PH.CONCLUSION:PP plus ECPD via the portal vein can attenuate toxic load and hyperperfusion injury,and should be undertaken instead of PP via the systemic circulation in SFSS or PLF.
文摘According to site-specific environments such as high water pressures, high in-situ stresses and strong rockbursts, the design scheme of the long and deep diversion tunnels at Jinping II hydropower station was optimized to ensure construction safety. New drainage tunnels were considered. Furthermore, lining structures and grouting pressures were modified during the excavation of tunnels. The construction scheme was updated dynamically based on the complex geological conditions. For instances, the diversion tunnels were first excavated by drilling and blasting method at the first stage of construction, and then by the combination method of tunnel boring machine (TBM) and drilling and blasting, and finally by drilling and blasting method. Through optimized scheme and updated construction scheme, the excavation of diversion tunnel #1 was successfully completed in June, 2011. This paper summarizes the key issues in rock mechanics associated with the construction of the long and deep diversion tunnels at Jinping II hydropower station. The experiences of design and construction obtained from this project could provide reference to similar projects.
文摘The proposals on entanglement diversion and quantum teleportation of entangled coherent states are presented. In these proposals, the entanglement between two coherent states, |α〉 and |-α〉, with the same amplitude but a phase difference of π is utilized as a quantum channel. The processes of the entanglement diversion and the teleportation are achieved by using the 50/50 symmctric beam splitters, the phase shifters and the photodetectors with the help of classical information.

文摘Payment for ecosystem services (PES) has attracted considerable attention as an economic incentive for promoting natural resource management recently. As emphasis has been placed on using the incentive-based mechanism by the central government, rapid progress on PES research and practice has been achieved. However PES still faces many difficulties. A key issue is the lack of a fully-fledged theory and method to clearly define the design scope, accounting and feasibility of PES criteria. An improved watershed criteria model was developed in light of research on PES practices in China, investigations on the water source area for the Middle Route Project of South-to-North Water Diversion and ecosystem services outflows theory. The basic principle of assessment is the direct and opportunity cost for ecological conservation and environmental protection in the water source area deduct nationally-financed PES and internal effect. Then the scope and the criteria methods were determined, and internal effect was put forward to define benefits brought from water source area. Finally, Shiyan City, which is the main water source area for the Project of Water Diversion, was analyzed by this model and its payment was calculated. The results showed that: (1) during 2003–2050, the total direct cost and opportunity cost would reach up to 262.70 billion and 256.33 billion Chinese Yuan (CNY, 2000 constant prices), i.e., 50.61% and 49.38% of total cost, respectively; (2) Shiyan City would gain 0.23, 0.06 and 0.03 CNY/m3 in 2014–2020, 2021–2030, and 2031–2050, respectively.

文摘The South-to-North Water Diversion (SNWD) Project is a significant engineering project meant to solve water shortage problems in North China. Faced with market operations management of the water diversion system, this study defined the supply chain system for the SNWD Project, considering the actual project conditions, built a decentralized decision model and a centralized decision model with strategic customer behavior (SCB) using a floating pricing mechanism (FPM), and constructed a coordination mechanism via a revenue-sharing contract. The results suggest the following: (1) owing to water shortage supplements and the excess water sale policy provided by the FPM, the optimal ordering quantity of water resources is less than that without the FPM, and the optimal profits of the whole supply chain, supplier, and external distributor are higher than they would be without the FPM; (2) wholesale pricing and supplementary wholesale pricing with SCB are higher than those without SCB, and the optimal profits of the whole supply chain, supplier, and external distributor are higher than they would be without SCB; and (3) considering SCB and introducing the FPM help increase the optimal profits of the whole supply chain, supplier, and external distributor, and improve the efficiency of water resources usage.

文摘The south to the north project (WDP) on the saltwater intrusion in the Changjiang Estuary is studied by the improved three-dimensionai (3D) numerical model.The net unit width flux in the Changjiang Estuary as well as the sectional salt flux is calculated in the North Branch (NB),the South Branch (SB),the North Channel (NC),the South Channel (SC),the North Passage (NP) and the South Passage (SP),respectively.The net seaward water flux in the SB is reduced,and the net water flux spilling over from the NB to the SB is enhanced after the eastern WDP.Under the mean river discharge condition in the dry season,the net salt flux spilling over from the NB to the SB is increased by 2.09 t/s and 0.52 t/s during the spring and neap tides,respectively,due to the eastern WDP.The saltwater intrusion in the Changjiang Estuary is enhanced by the eastern WDP.Compared with that during the spring tide,the net water diversion ratio during the neap tide in the NC is smaller,and thus the enhancement of the saltwater intrusion by the eastern WDP is smaller in the NC,and larger in the NP and the SP.The tidally averaged surface salinity at the water intakes of the Dongfengxisha Reservoir,the Chenhang Reservoir and the Qingcaosha Reservoir rises both during the spring and neap tides.
文摘In this case report, we examined the levels of cytokines expressed before and during fecal stream diversion and after intestinal continuity was restored in a patient with collagenous colitis. We report the case of a46-year-old woman with chronic, active collagenous colitis who either failed to achieve clinical remission or experienced adverse effects with the following drugs:loperamide, cholestyramine, budesonide, methotrexate and adalimumab. Due to the intractable nature of the disease and because the patient was having up to 15 watery bowel movements per day, she underwent a temporary ileostomy. Colonic biopsies were analyzed for mucosal cytokine protein levels before and during fecal stream diversion and after intestinal continuity was restored. Mucosal protein levels of interleukin(IL)-1β,IL-2, IL-6, IL-12, IL-17 A, IL-23, TNF, IFN-γ, IL-4, IL-5,IL-10 and IL-13 were all higher during active disease and decreased to non-detectable or considerably lower levels during fecal stream diversion. One month after the restoration of bowel continuity, when the patient experienced a relapse of symptoms, IL-2, IL-23 and IL-21 levels were again increased. Our results indicate that fecal stream diversion in this patient suppressed the levels of all cytokines analyzed in colonic biopsies. With the recurrence of clinical symptoms and histological changes after bowel reconstruction,the levels of primarily proinflammatory cytokines increased. Our findings support the hypothesis that a luminal factor triggers the inflammation observed in collagenous colitis.
