Controversy of hand-assisted laparoscopic colorectal surgery 被引量:24

Laparoscopically assisted colorectal procedures are time-consuming and technically demanding and hence have a long steep learning curve. In the technical demand, surgeons need to handle a long mobile organ, the colon, and have to operate on multiple abdominal quadrants, most of the time with the need to secure multiple mesenteric vessels. Therefore, a new surgical innovation called hand-assisted laparoscopic surgery (HALS) was introduced in the mid 1990s as a useful alternative to totally laparoscopic procedures. This hybrid operation allows the surgeon to introduce the non-dominant hand into the abdominal cavity through a special hand port while maintaining the pneumoperitoneum. A hand in the abdomen can restore the tactile sensation which is usually lacking in laparoscopic procedures. It also improves the eye-to-hand coordination, allows the hand to be used for blunt dissection or retraction and also permits rapid control of unexpected bleeding. All of those factors can contribute tremendously to reducing the operative time. Moreover, this procedure is also considered as a hybrid procedure that combines the advantages of both minimally invasive and conventional open surgery. Nevertheless, the exact role of HALS in colorectal surgery has not been well defined during the advanced totally laparoscopic procedures. This article reviews the current status of hand-assisted laparoscopic colorectal surgery as a minimally invasive procedure in the era of laparoscopic surgery. 展开更多

Regarding the worsening of symptoms in people... <span style="font-family:Verdana;">The Japan Dementia Prevention Society conducted a questionnaire survey of dementia specialists conducted in August 2020. Regarding the worsening of symptoms in people with dementia, 8% answered that they “accept more” and 32% answered that they “recognize a minority”. In the previous research report (Open Journal of Therapy and Rehabilitation Vol.8 No.3), we examined whether hand care therapy (HCT) as one of the passive horticultural therapy was effective in preventing dementia/MCI. Start of the test in August 2018 (I: BL) to February 2020 (VII: 18M) was, the use of HCT was to be effective in preventing dementia and MCI illness and slowing progression. Due to the COVID19 pandemic, VIII (24M) was evaluated in August 2020, and IX (27M) also measured ANS during activity. At that time, they divided into the Group E who had refrained from welfare facilities for the elderly due to the influence of COVID19 and the Group F who HCT continued. In the evaluation of VIII (24M), it was found that the cognitive function of the Group E was significantly reduced compared to VII, and Group F did not show a significant decrease. ADL decreased and depressive symptoms increase by continuing a life with less going out due to the influence of COVID 19 regardless of the use of the facility. It became clear that both Group E and Group F show good changes in ANS balance in the IX. Therefore, by utilizing horticultural therapy that the power of nature and plants in addition to utilizing welfare facilities for the elderly, cognitive impairment in COVID 19</span><span style="font-family:Verdana;"><span style="font-family:Verdana;"><span style="font-family:Verdana;"><span style="font-family:Verdana;">.</span></span></span></span><span style="font-family:Verdana;"><span style="font-family:Verdana;"><span style="font-family:Verdana;"><span style="font-family:Verdana;"> It was expected to lead to prevention and prevention of negative psychology and behavior such as depressive views and depressive tendencies. Objective: To observe the effects of Xingnao Kaiqiao acupuncture(醒脑开窍针) on the motor function of upper limb and hand in the recovery period after stroke. Methods: Seventy-six cases of hemiplegia patients with ischemic stroke were divided into the treatment group and the control group(n=38 in each). Based on the Brunnstrom's stage of Xingnao Kaiqiao acupuncture combined with rehabilitation training was used in the treatment group, and the control group was given rehabilitation training. FuglMeyer Assessment of the upper extremity(FMA-UE), Action Research Arm Test(ARAT) and Simple Test for Evaluating hand Function(STEF) were adopted separately to compare scores before treatment and 8 weeks after treatment. Results: The difference was not statistically significant in the two groups of patients for comparison of FMA-UE, ARAT and STEF scores before treatment(P>0.05). The difference was statistically significant in the two groups of score comparison of FMA-UE, ARAT and STEF after treatment(P<0.05). Conclusion: The Xingnao Kaiqiao acupuncture has its unique advantages in improving recovery of motor function of upper limb and hand in recovery period after stroke. 展开更多

Although some patients have successful peripheral nerve regeneration,a poor recovery of hand function often occurs after peripheral nerve injury.It is believed that the capability of brain plasticity is crucial for the recovery of hand function.The supplementary motor area may play a key role in brain remodeling after peripheral nerve injury.In this study,we explored the activation mode of the supplementary motor area during a motor imagery task.We investigated the plasticity of the central nervous system after brachial plexus injury,using the motor imagery task.Results from functional magnetic resonance imaging showed that after brachial plexus injury,the motor imagery task for the affected limbs of the patients triggered no obvious activation of bilateral supplementary motor areas.This result indicates that it is difficult to excite the supplementary motor areas of brachial plexus injury patients during a motor imagery task,thereby impacting brain remodeling.Deactivation of the supplementary motor area is likely to be a serious problem for brachial plexus injury patients in terms of preparing,initiating and executing certain movements,which may be partly responsible for the unsatisfactory clinical recovery of hand function. 展开更多

Studies were carried out to assess the effect of distance of sanitary pits on the microbial and heavy metal levels in hand dug well water samples consumed by people living in Akwuke, Enugu South Local government area of Enugu State, using standard biochemical and spectrophotometric analysis. The well water samples were digested with aqua-regia and four heavy metals (Pb, Cd, Cu and Zn) were assayed in the well water samples at sampling distances of 3, 6 and 10 m respectively from the sanitary pits. The range of mean bacterial counts (cfu/g) for the four detected pathogenic bacteria;<em> S. aureus</em>, <em>Klebsiella pnemeoniae</em>, <em>Echerichia coli</em> and <em>Salmonella enteritidis </em>were 83 - 3730, 510 - 870, 50 - 2535 and 240 - 3420 Cfu/g at sampling distances of 3, 6 and 10 m respectively from the sanitary pits. The mean bacteria counts of the four detected pathogenic organisms in the well water sample at sampling distances of 3 and 6 m respectively from the sanitary pits were above the WHO recommended threshold limits for a safe drinking water. The mean bacterial counts of <em>Salmonella enteritidis</em> in the well water samples of sampling distance of 10 m from the sanitary pits was above the WHO recommended permissible limits. At sampling distances of 3, 6 and 10 m from the sanitary pits, the mean range of Pb, Cd, Cu and Zn in the well water samples were, 0.03 - 0.3, 0.02 - 0.05, 0.46 - 1.71 and 1.63 - 7.03 μg/g respectively. The mean levels of Pb and Cd in the well water samples at sampling distances of 3 and 6 m respectively from the sanitary pits were above their respective WHO recommended threshold limits. The mean heavy metals (Cd, Pb, Cu and Zn) in the well water samples at sampling distances of 3, 6, and 10 m respectively from the sanitary pits were statistically significant at p < 0.05. 展开更多

Aim: A standardized preparation of seeds and shells of selected sub-species of Rosa canina L, trade name Hyben Vital, reduces discomfort from osteoarthritis of the knee and hip. This study aims to investigate the impact of the same rose-hip powder (RHP) on discomfort and the consumption of rescue medication, in patients with osteoarthritis of the hand. Methods: The double blind, placebo-controlled, crossover trial included 30 patients with osteoarthritis of the dominant hand. Patients were randomly allocated to treatment with either five gram encapsulated RHP or placebo, for three months (Phase 1), after which they switched to the corresponding treatment for a further three months period (Phase 2). Before entering the study, after 3 weeks and following three months of each of the study phases, scores for pain, stiffness and general feeling of discomfort were evaluated using a 10 step categorical scale, focusing on 16 different daily activities of the hand. The consumption of rescue medication was also calculated at the beginning and at the end of each study phase. Data are based on the intention to treat. Results: At the end of Phase 1, 90% of patients in the group receiving RHP first (group A), showed a reduction in pain, as compared to 36% in the group B initially given placebo (p 0.029). In line with this observation, stiffness and the general feeling of discomfort from the disease declined during RHP treatment (p 0.032). In group A, symptom reduction was still indicated by the study subjects 3 weeks after the switch to placebo. The consumption of rescue medication such as paracetamol, codeine and tramadol also declined significantly in group A when compared to group B (p 0.013). Conclusion: The present data suggest that administration of RHP, containing seeds and shells can reduce symptoms of osteoarthritis of the hand and consumption of rescue medication. 展开更多

He underwent successful surgery after placement of an ultrasound-guided infraclavicular brachial plexus block with ropivacaine 0.5% as the main anesthetic. This case report discusses the anesthetic management of patients with HCM and AICD, different approaches available for brachial plexus blockade, and potential complications of anesthesia and surgery in this group of patients. 展开更多

We report a case of blast injury to the left hand which resulted in fractures of the fingers with exposure of bones and joints of the phalanges. We used three reverse adipofascial cross finger flaps raised at the same time from 2 fingers to reconstruct adjacent fingers of the patient. The patient recovered well postoperatively and had good range of movement of the fingers. This avoided the complications of the use of regional or distal flaps. To our knowledge, this is the first case reported in which three reverse adipofascial cross fingers flaps are raised at the same time, two of them from an injured finger, to cover three raw areas on two fingers of a patient. 展开更多

Objective To assess health equity in the Yangtze River region to improve understanding of the correlation between hand, foot, and mouth disease(HFMD) and socioeconomic factors.Methods From 2014–2016, data on HFMD incidence, population statistics, economic indicators, and meteorology from 26 cities along the Yangtze River were analyzed. A multi-city random-effects metaanalysis was performed to study the relationship between temperature and HFMD transmission, and health equity was assessed with respect to socio-economic impact.Results Over the study period, 919,458 HFMD cases were reported, with Shanghai(162,303) having the highest incidence and Tongling(5,513) having the lowest. Males were more commonly affected(male-to-female ratio, 1.49:1). The exposure-response relationship had an M-shaped curve, with two HFMD peaks occurring at 4 ℃ and 26 ℃. The relative risk had two peaks at 1.30 ℃(1.834, 95% CI:1.204–2.794) and 31.4 ℃(1.143, 95% CI: 0.901–1.451), forming an M shape, with the first peak higher than the second. The most significant impact of temperature on HFMD was observed between –2 ℃ and18.1 ℃. The concentration index(0.2463) indicated moderate concentration differences, whereas the Theil index(0.0418) showed low inequality in distribution.Conclusion The incidence of HFMD varied across cities, particularly with changes in temperature.Economically prosperous areas showed higher risks, indicating disparities. Targeted interventions in these areas are crucial for mitigating the risk of HFMD. 展开更多

Background:Healthcare-associated infections continue to pose a significant threat to patient safety,and hand hygiene practices remain an effective preventive measure.Despite global campaigns,nurse compliance often remains substandard.Previous research has focused on technical and environmental factors;however,the influence of internal psychological aspects,such as self-leadership,has been rarely studied,particularly in Indonesia.Increasing compliance depends not only on external interventions but also on empowering nurses’motivation and self-regulation.Objective:This study aims to determine the relationship between self-leadership and hand hygiene compliance among inpatient nurses.Materials and Methods:A quantitative correlational design with a total sample of 88 nurses was used.Self-leadership was measured using the Abbreviated Self-Leadership Questionnaire,and compliance was assessed through direct observation based on the World Health Organization“5 Moments for Hand Hygiene”guidelines.Data analysis was conducted descriptively and using the Chi-square test.Results:The results showed that 54.5%of nurses had high self-leadership,but only 29.5%were compliant with hand hygiene procedures.The Chi-square test showed a significant relationship between self-leadership and compliance(c2=7.453;P=0.006).Conclusion:Higher self-leadership correlates with better compliance;however,low overall compliance indicates an intention-behaviour gap.Therefore,integrating self-leadership training with organisational support is necessary to strengthen the patient safety culture. 展开更多

The rapid evolution of virtual reality(VR)and augmented reality(AR)technologies has significantly transformed human-computer interaction,with applications spanning entertainment,education,healthcare,industry,and remote collaboration.A central challenge in these immersive systems lies in enabling intuitive,efficient,and natural interactions.Hand gesture recognition offers a compelling solution by leveraging the expressiveness of human hands to facilitate seamless control without relying on traditional input devices such as controllers or keyboards,which can limit immersion.However,achieving robust gesture recognition requires overcoming challenges related to accurate hand tracking,complex environmental conditions,and minimizing system latency.This study proposes an artificial intelligence(AI)-driven framework for recognizing both static and dynamic hand gestures in VR and AR environments using skeleton-based tracking compliant with the OpenXR standard.Our approach employs a lightweight neural network architecture capable of real-time classification within approximately 1.3mswhilemaintaining average accuracy of 95%.We also introduce a novel dataset generation method to support training robust models and demonstrate consistent classification of diverse gestures across widespread commercial VR devices.This work represents one of the first studies to implement and validate dynamic hand gesture recognition in real time using standardized VR hardware,laying the groundwork for more immersive,accessible,and user-friendly interaction systems.By advancing AI-driven gesture interfaces,this research has the potential to broaden the adoption of VR and AR across diverse domains and enhance the overall user experience. 展开更多

Background:Muscular strength is a powerful marker of current health status and robust predictor of age-related disease and disability.Handgrip strength(HGS)using isometric dynamometry is a convenient,feasible,and widely used method of assessing muscular strength among people of all ages.While adult HGS norms have been published for many countries,no study has yet synthesized available data to produce international norms.The objective of this study was to generate international sex-and age-specific norms for absolute and body size-normalized HGS across the adult lifespan.Methods:Systematic searches were conducted in 6 databases/web search engines(MEDLINE,SPORTDiscus,Embase,Web of Science,CINAHL,and Google Scholar)up to December 1,2023.We included full-text peer-reviewed observational studies that reported normative HGS data for adults aged ≥20 years by sex and age.Pseudo data were generated using Monte Carlo simulation following harmonization for methodo-logical variation.Population-weighted Generalized Additive Models for Location,Scale,and Shape were used to develop sex-and age-specific norms for absolute HGS(kg)and HGS normalized by height(Ht,m)squared(i.e.,HGS/Ht^(2)in kg/m^(2)).Norms were tabulated as percentile values(5th to 95th)and visualized as smoothed percentile curves.Results:We included data from 100 unique observational studies representing 2,405,863 adults(51.9%female)aged 20 to 100+years from 69 countries and regions tested from the year 2000 onward.On average,absolute and normalized HGS values negligibly improved throughout early adulthood,peaked from age 3039 years(at 49.7 kg(males)and 29.7 kg(females)for absolute HGS or 16.3 kg/m^(2)(males)and 11.3 kg/m^(2)(females)for HGS/Ht^(2)),and declined afterwards.The age-related decline in HGS accelerated from middle to late adulthood and was slightly larger for males than for females during middle adulthood.Conclusion:This study provides the world’s largest and most geographically comprehensive international norms for adult HGS by sex and age.These norms have utility for global peer-comparisons,health screening,and surveillance. 展开更多

Objective:While albumin and the weight-standardized hand grip strength(HGS/W)serve as valuable prognostic indicators for cancer patients,their correlation with the prognosis in frail cancer patients remains inadequately explored.This study aimed to investigate the prognostic importance of the albumin level and HGS/W in cancer patients with frailty and to further investigate their combined prognostic value.Moreover,this comprehensive evaluation aimed to facilitate timely intervention and treatment for frail patients.Methods:The research enrolled 5,794 cancer patients identified with frailty from a multicenter research database.The diagnosis of frailty was based on the FRAIL scale.An Albumin-HGS/W score was constructed by combining the albumin and HGS/W values.Cox proportional hazard regression was utilized to examine the association between the albumin level and HGS/W and patient outcomes.Results:Among these patients,2,543 were females and 3,251 were males,with a median age of 60.0 years.Optimal stratification based on patient survival revealed the ideal threshold for HGS/W to be 0.48 for males and 0.39 for females,and for albumin to be 38 for both sexes.The fully adjusted model revealed that higher Albumin-HGS/W scores were correlated with a poorer patient prognosis.Notably,an Albumin-HGS/W score of 2 was associated with a higher risk of mortality compared with a score of 0 in the total population(HR:1.813,95%CI:1.580-2.080,P<0.001).Conclusions:Low albumin or HGS/W values are associated with low survival in cancer patients with frailty.Elevated Albumin-HGS/W scores were linked to decreased survival rates in cancer patients with frailty. 展开更多

Puffy Hand Syndrome with Pneumonia and Spontaneous Pneumothorax in an Intravenous Drug User: A Case Report

Background: Puffy hand syndrome (PHS) is a rare complication primarily associated with intravenous drug use (IVDU), characterized by chronic swelling and fibrosis of the hands due to lymphatic damage. Concurrent pulmonary complications, such as pneumonia and pneumothorax, significantly contribute to increased morbidity in this population. Case Presentation: We report the case of a 28-year-old female who injects drugs, and presents with fever, bilateral hand edema, and respiratory symptoms. Clinical evaluation revealed erythema and edema of both hands, elevated inflammatory markers, and a left lower lobe infiltration that progressed to pneumothorax. A diagnosis of PHS and left lower lobe pneumonia complicated by pneumothorax was established. Management and Outcomes: The patient was treated with broad-spectrum antibiotics, including ceftriaxone, levofloxacin, dexamethasone, and oxygen supplementation, as well as antipyretics. She demonstrated partial clinical improvement and was referred to another hospital’s thoracic surgery department for specialized care. Conclusions: This case underscores the importance of early recognition and multidisciplinary management of rare but serious complications in IVDU patients. Further research is necessary to elucidate the interplay between lymphatic dysfunction and pulmonary pathophysiology in this demographic. 展开更多
