OBJECTIVE:To investigate the difference in gut microbiota between population with damp-heat constitution(DHC)and balanced constitution(BC).METHODS:A multi-centered cross-sectional casecontrol study was conducted, which included 249 participants with damp-heat constitution or balanced constitution.Baseline information of participants was collected, and stool samples were collected for gut microbiota analysis.Principal coordinate analysis, linear discriminant analysis effect size analysis, receiver operating characteristic, random forest model, and phylogenetic investigation of communities by reconstruction of unobserved states methods were used to reveal the relationship between gut microbiota and the damp-heat constitution.RESULTS:Compared to those in the BC group, the richness and diversity of the microbiota, specifically those of several short-chain fatty acid producing genera such as Barnesiella,Coprobacter, and Butyricimonas, were significantly decreased in the DHC group.Regarding biological functions, flavonoid biosynthesis, propanoate metabolism, and nucleotide sugar metabolism were suppressed, while arachidonic acid metabolism and glutathione metabolism were enriched in the DHC group. Finally,a classifier based on the microbiota was constructed to discriminate between the DHC and BC populations.CONCLUSION:The gut microbiota of the DHC population exhibits significantly reduced diversity and is closely related to inflammation, metabolic disorders, and liver steatosis, which is consistent with clinical observations, thus serving as a potential diagnostic tool for traditional Chinese medicine constitution discrimination. 展开更多

Objective:To study the relationship between qi stagnation constitution and suboptimal health status(SHS)or lifestyle. Methods:From 2012 to 2013,we conducted a cross-sectional survey of 24159 Chinese individuals aged 12-80 years. The qi stagnation constitution was assessed using the Constitution in Chinese Medicine Questionnaire.Health status was evaluated through medical records and the Subhealth Measurement Scale V1.0(SHMS V1.0).Health-promoting lifestyles were measured using the Health-Promoting Lifestyle Profile Ⅱ(HPLP-Ⅱ).Results:Of the 24159 participants,16.1%and 15.2%were classified as“always”and“sometimes”having the qi stagnation constitution, respectively. Those classified as“rarely”having the qi stagnation constitution scored higher on both the HPLP-Ⅱ and SHMS V1.0.The participants classified as“always”having the qi stagnation constitution showed a significant association with SHS or disease compared to other imbalanced constitutions. Those in the“always”category were approximately 21 times more likely to be classified as having SHS(odds ratio[OR]:21.17,95%confidence interval[CI]:15.74-28.45),whereas those in the“sometimes”category were approximately six times more likely(OR:5.89,95%CI:5.04-6.90).Accordingly,the qi stagnation constitution score was significantly associated with the diagnosis of SHS,with an area under the curve of 0.77(P<.001).A score of 18.75 yielded the highest Youden Index(0.407),with a sensitivity of 60.5%and a specificity of 80.3%.Significant associations were observed between health-promoting lifestyles and qi stagnation constitution severity in an ordinal regression analysis(P<.001).Protective factors included stress management(OR:1.59),self-actualization(OR:1.57),and exercise(OR:1.36).In contrast, poorer interpersonal relationships(OR:0.79),greater health responsibilities(OR:0.86),and poorer nutrition(OR:0.91)were associated with increased severity. Conclusion:Modulating the qi stagnation constitution through lifestyle interventions may help prevent the progression of SHS to disease, which aligns with core preventive principles in traditional Chinese medicine. 展开更多

Background:Constitutional mismatch repair deficiency(CMMRD)is a rare disorder resulting from biallelic germline pathogenic variants in mismatch repair genes. This study described the molecular profile of two metachronous brain tumors and a patient-derived xenograft(PDX)from a Brazilian child with CMMRD.Methods:After PDX development, methylation array, whole exome sequencing, and Nano String techniques were applied to describe the genetic landscape of CMMRD.Results:A 6½-year-old girl was diagnosed with Sonic Hedgehog(SHH)-activated medulloblastoma and somatic TP53-mutant.After surgery and radiochemotherapy, she remained free of disease progression. At 10 years and 3 months, she developed a diffuse pediatric-type high-grade glioma(dp HGG).The child had a family history of cancer, and subsequent investigation revealed a biallelic germline variant on MSH6(c.3556+1G>A)with the absence of protein expression in both normal and tumor tissue.A PDX model of the dp HGG was developed. The methylation profile confirmed the diagnosis of both brain tumors and PDX,refining the classification of dp HGG,Rtk1 subtype, subclass A,with an actionable alteration on Platelet-derived growth factor receptor A(PDGFRA).Exome analysis showed high tumor mutational burden, with 3019,540,and 1049 pathogenic variants in the medulloblastoma,dp HGG,and PDX,respectively.Only the medulloblastoma exhibited microsatellite instability. The CD24,CD47,and CD276 immune checkpoints had elevated messenger RNA levels, yet no programmed death ligand 1 expression was observed in CMMRD-derived tumors. Conclusion:We report an extensive molecular profile of a CMMRD patient, and the developed PDX model can be applied to explore new therapeutic approaches for CMMRD-associated brain tumors. 展开更多

The understanding of the scope of protection for the right to health should not be limited to fragmented descriptions in departmental laws, such as those focusing on rights, interests,or legal benefits.Instead,it should be analyzed holistically within the constitutional framework of fundamental rights. From the perspective of defense right function, the right to health addresses physiological harm that progresses through stages of“risk-danger-infringement,”psychological harm that targets inner distress, and minor harm arising from challenges in social adaptation,to clarify the negative defensive obligations of the state to prevent health-related harm. From the perspectives of the beneficiary right function and the objective value order function, the right to health requires a minimum level of constitutional protection and delineating the positive obligations of the state to ensure it through the fulfillment of basic obligations in healthcare and health promotion. 展开更多

In Eastern medicine,an individual’s health and longevity is believed to depend on the balance of their metabolic energies, classified as body constitution.Traditional Mongolian Medicine categorizes the body into seven constitution types based on metabolic dominance, influencing physical traits, behavioral tendencies, disease susceptibility, and adaptability to environmental factors. This study aims to analyze the principles of disease prevention based on body constitution in Eastern medicine, with a focus on Traditional Mongolian and Tibetan medical perspectives. It looks to clarify body constitution classifications, explore dietary management strategies, and highlight their significance in maintaining health and preventing diseases. This study adopts a literature review approach, incorporating source analysis, synthesis, and hermeneutic methodology to examine traditional medical theories related to body constitution.Classical texts from Traditional Tibetan medicine, including the rGyud-bZhi and Golden Subtitles, were analyzed alongside comparative research on Traditional Chinese and Korean constitutional medicine. The study integrates historical perspectives with modern biomedical insights to establish a comprehensive understanding of constitution-based health management. The findings indicate that the three principal metabolic energies—rLung,mKhris, and Bad-Kan—serve as symbolic codes representing physiological and metabolic states.By recognizing their body constitution, individuals can adopt constitutionally tailored dietary and lifestyle practices to maintain balance and prevent diseases.A comparative analysis reveals similarities and differences among different Eastern medical systems, while still demonstrating the potential for integrating constitutional medicine with modern healthcare approaches. 展开更多

BACKGROUND The Asia-Pacific Colorectal Screening(APCS)score was designed with the purpose of distinguishing individuals at high risk(HR)for colorectal advanced neoplasia(AN).Traditional Chinese medicine(TCM)constitution was also linked with colorectal cancer(CRC).AIM To integrate the APCS score with TCM constitution identification as a new algorithm to screen for CRC.METHODS A cross-sectional multicenter study was carried out in three hospitals, enrolling 1430 patients who were asymptomatic and undergoing screening colonoscopy from 2022 to 2023.Patients were considered to have average risk, moderate risk,or HR with their APCS score.Odd ratios assessed the relationship between TCM constitution and disease progression.A TCM constitution risk score was created. The sensitivity and specificity of the new algorithm were calculated to evaluate diagnostic performance in detecting advanced adenoma(AA),CRC,and AN.RESULTS Of the 1430 patients,370(25.9%)were categorized as average risk,755(52.8%)as moderate risk, and 305(21.3%)as HR.Using the combined APCS score and the TCM constitution(damp-heat,qi-deficiency,yang-deficiency,phlegm-dampness,and inherited special constitution as positive)algorithm,72.2%of patients with AA and 73.7%of patients with AN were detected.Compared with the APCS score alone, the new algorithm significantly improved the sensitivity for screening AA[72.2%,95%confidence interval(CI):64.4%-80.0%vs 49.2%,95%CI:40.5%-57.9%]and AN(73.7%,95%CI:66.4%-81.1%vs 51.1%,95%CI:42.7%-59.5%).CONCLUSION The combination of APCS and TCM constitution identification questionnaires was valuable in identifying Chinese individuals who were asymptomatic for colorectal screening prioritization. 展开更多

Accurate assessment of coal brittleness is crucial in the design of coal seam drilling and underground coal mining operations. This study proposes a method for evaluating the brittleness of gas-bearing coal based on a statistical damage constitutive model and energy evolution mechanisms.Initially, integrating the principle of effective stress and the Hoek-Brown criterion,a statistical damage constitutive model for gas-bearing coal is established and validated through triaxial compression tests under different gas pressures to verify its accuracy and applicability.Subsequently, employing energy evolution mechanism, two energy characteristic parameters(elastic energy proportion and dissipated energy proportion)are analyzed.Based on the damage stress thresholds, the damage evolution characteristics of gas bearing coal were explored. Finally,by integrating energy characteristic parameters with damage parameters,a novel brittleness index is proposed. The results demonstrate that the theoretical curves derived from the statistical damage constitutive model closely align with the test curves, accurately reflecting the stress−strain characteristics of gas-bearing coal and revealing the stress drop and softening characteristics of coal in the post-peak stage. The shape parameter and scale parameter represent the brittleness and macroscopic strength of the coal, respectively. As gas pressure increases from 1 to 5 MPa,the shape parameter and the scale parameter decrease by 22.18%and 60.45%,respectively,indicating a reduction in both brittleness and strength of the coal.Parameters such as maximum damage rate and peak elastic energy storage limit positively correlate with coal brittleness. The brittleness index effectively captures the brittleness characteristics and reveals a decrease in brittleness and an increase in sensitivity to plastic deformation under higher gas pressure conditions. 展开更多

OBJECTIVE:To explore the effect of Traditional Chinese Medicine(TCM)constitution on cognitive impairment(CI)in patients with cerebral small vessel disease(CSVD)and its underlying neuroimaging mechanism and to provide countermeasures for health management of CSVD patients.METHODS:A total of 241 CSVD patients were recruited from the Department of Neurology. All subjects underwent head magnetic resonance imaging(MRI),Chinese Medicine Questionnaire(CCMQ)and cognitive function examination. The CSVD patients were divided according to the Montreal Cognitive Assessment(MoCA)score into a normal cognitive group(73 cases)and a CI group(168 cases).Logistic regression was used to analyse the risk constitution of CSVD-CI and to construct a risk prediction model.3DT1 MRI images and FreeSurfer 6.0 software(Athinoula A.Martinos,Boston,MA,USA)were used to further explore the involvement of hippocampal subregion volume in patients with at-risk constitution and its correlation with cognitive function.RESULTS:Logistic regression analysis showed that Yang-deficiency constitution(YADC)(P=0.020),older age(P=0.008)and hypertension(P=0.011)were risk factors for CSVD-CI but that balanced constitution(P=0.003)and education(P<0.001)were protective factors.A receiver operating characteristic curve(ROC)was drawn, and the area under the curve was 0.820.Further comparison of overall hippocampal and 12 hippocampal subregion volumes between YADC and non-YADC patients revealed decreased total volume of the left and right hippocampus, bilateral subiculum, presubiculum, molecular layer and right fimbria in the YADC group(P<0.05/13,13 is the number of hypothesis tests). Moreover,in the YADC group, the cognitive function of CSVD patients correlated positively with the overall volume of the left hippocampus(r=0.304,P<0.05)and the molecular layer volume of the left hippocampus(r=0.288,P<0.05).CONCLUSION:CSVD patients with YADC are more prone towards developing CI,and asymmetric atrophy of the hippocampus might be the underlying neuroimaging mechanism. In particular, the volume of the left whole hippocampus and the left hippocampal molecular layer correlated significantly with general cognitive function. 展开更多

OBJECTIVE:To utilize the Traditional Chinese Medicine constitution(TCMC)as a complementary and alternative approach for early disease detection and treatment, with a focus on Yin and Yang deficiency constitutions, which serve as key references for disease prevention and management.METHODS:The dataset containing the data of Yin and Yang deficiency constitution was identified through the Gene Expression Omnibus database. This database was used for differential expression genes(DEGs)analysis and weighted gene co-expression network analysis(WGCNA),and the characteristic genes were then obtained in the dataset using a machine learning method. The hub genes of Yin and Yang deficiency constitution were obtained after analysis using the above three methods, and the hub genes were enriched and analyzed.Subsequently, the hub genes of Yin and Yang deficiency constitution were validated using external datasets.Receiver operating characteristic(ROC)analysis was used on each hub genes of the two groups to further understand their diagnostic performance. The mi RNA–lnc RNA-gene network was used to further analyze the hub genes.Immunoinfiltration and gene set enrichment analysis were performed on the shared hub genes.RESULTS:The GSE87474 dataset was used for DEGs analysis and WGCNA.Using machine learning analyses,we identified 15 and 14 hub genes for Yin and Yang deficiency constitutions, respectively. The results of enrichment analyses showed that Yin deficiency constitution was associated with interleukin-17 signaling pathway, whereas Yang deficiency constitution was associated with glycosaminoglycan biosynthesis–keratan sulfate. The validation dataset GSE56116 showed statistically significant data for s-adenosylmethionine sensor upstream of MTORC1(SAMTOR,also named C7orf60),cofilin 2(CFL2),cytohesin 1 interacting protein(CYTIP),G protein-coupled receptor 183(GPR183),hippocampus abundant transcript 1(HIAT1),kelch like family member 15(KLHL15),mitogen-activated protein kinase 6(MAPK6),and prostaglandin-endoperoxide synthase 2(PTGS2)in Yin deficiency and fucosyltransferase 8(FUT8),TATA-box binding protein associated factor,RNA polymerase I subunit D(TAF1D),zinc finger protein 24(ZNF24),MAPK6,and leptin receptor overlapping transcript like 1(LEPROTL1)in Yang deficiency. The ROC results indicated that these genes have diagnostic value.MAPK6 is a shared hub gene for Yin and Yang deficiencies.CONCLUSIONS:This study identified C7orf60,CFL2,CYTIP,GPR183,HIAT1,KLHL15,MAPK6,and PTGS2 in Yin deficiency and FUT8,TAF1D,ZNF24,MAPK6,and LEPROTL1 in Yang deficiency as potential biomarkers, providing insights into their pathogenesis. This theory not only guides the diagnostic approach in TCM but also extends its influence to various scientific research fields. 展开更多

The establishment of a sound science and technology ethics governance system is an inevitable requirement for national modernization.Faced with the development of human gene technology and the chaos in research activities, the ethical standards and legal positioning of human gene research activities urgently need to be clarified. The human rights ethics view has value inclusiveness and value fundamentality, and includes three levels of connotations:content dimension, relationship dimension, and obligation dimension. It should serve as the ethical standard for human gene research activities.Based on the provisions of China’s Constitution, the human rights ethics view on human gene research,as a constitutional ethics view, can elucidate different levels of rights content, such as human dignity, life and health, and research freedom. It also addresses the weighing of basic rights conflicts and the dual obligation subjects of public and private nature.Relying on the constitutional value embedding of the research ethics view to form ethical consensus, improving ethical review through framework legislation for human rights interests, and implementing ethical responsibility through the human rights-oriented interpretation of ethical legal norms are the three pathways to realizing the human rights ethics view on human gene research. 展开更多

从设议院到立宪法——晚清“Constitution”汉译与立宪思潮形成考论 被引量:4

From the perspective of culture-based source, the author considered that the ancient Chinese poetry and paintings not only embodied the harmony and unity thought of man and nature, but also were the emotional attachment of Chinese literati. As a diverse and comprehensive aesthetic art, landscape expressed material and spiritual culture doubly, the spiritual connotation of which was inextricably linked within ancient Chinese poetry and paintings. In addition, ancient Chinese poetry and paintings can be directly used in landscape composition, such as the widely used in plaques, inscriptions, couplets, poems and inscriptions, carved beams and painted pillars, painting with words and so on.Chinese literati integrated the ethereal realm of poetry and paintings into garden art, making landscape endow with poetic charm. Therefore, the landscape revealed the traces of literature and arts in aesthetic idea and taste, pushing Chinese garden to a higher aesthetic realm. This study pointed the right way to the future of modern landscape which advocated ecological and harmonious development. 展开更多
