Safety, feasibility, and efficacy of surgical intervention for Urolithiasis in patients with chronic kidney disease: A systematic review

BACKGROUND Urolithiasisposes challenges in patients with chronic kidney disease(CKD),necessitating careful consideration of surgical interventions due to potential complications.AIM To assess the safety,feasibility,and efficacy of surgical interventions for urolithiasis in CKD patients.METHODS Systematic review adhering to PRISMA guidelines.Comprehensive searches of PubMed,Scopus,Cochrane Library,Web of Science,and Embase were conducted for studies published from January 2014 to June 2024.Studies involving adult patients(≥18 years)with CKD undergoing surgical interventions for urolithiasis,including randomized controlled trials,cohort studies,case-control studies,and observational studies.Studies involving pediatric patients,those not specifically addressing CKD patients,review articles,commentaries,and editorials.Despite an extensive search,only six studies met the strict inclusion criteria,reflecting the limited available data on this topic.This limitation has been acknowledged and discussed.RESULTS A total of 6 studies met the inclusion criteria,encompassing a diverse range of surgical interventions such as percutaneous nephrolithotomy(PCNL),ureteroscopy(URS),and extracorporeal shock wave lithotripsy(ESWL).Perioperative and postoperative complications varied across studies,with bleeding,infection,and acute kidney injury being the most common.The risk of complications was higher in patients with advanced CKD.Technical success rates were generally high,but feasibility was influenced by patientspecific factors such as CKD stage and comorbidities.Modifications to standard surgical techniques were often necessary.Stone-free rates and recurrence rates varied,with PCNL generally achieving higher stone-free rates compared to URS and ESWL.Long-term outcomes on renal function were inconsistent,highlighting the need for individualized treatment plans.CONCLUSION Surgical interventions for urolithiasis in CKD patients are associated with significant risks but can be effective in achieving stone clearance and symptom relief.The safety,feasibility,and efficacy of these interventions depend on patient-specific factors,necessitating a tailored approach.Further high-quality studies are needed to develop standardized guidelines and improve clinical outcomes in this complex patient population. 展开更多

Around the world,there has been a notable shift toward the use of renewable energy technology due to the growing demand for energy and the ongoing depletion of conventional resources,such as fossil fuels.Following this worldwide trend,Brunei’s government has initiated several strategic programs aimed at encouraging the establishment of energy from renewable sources in the nation’s energy mix.These initiatives are designed not only to support environmental sustainability but also to make energy from renewable sources increasingly competitive in comparison to more conventional energy sources like gas and oil,which have historically dominated Brunei’s energy market.The optimization of a hybrid energy system that combines diesel generators,solar photovoltaic(PV)panels,and the national power grid is the focus of this study.The objective is to identify the most cost-effective and environmentally sustainable configuration that can reliably meet local energy demands.During optimization,several configuration was tried and tested,including only grid,PV and Grid and PV-generator.HOMER(Hybrid Optimization of Multiple Energy Resources)software,a popular simulation tool that makes it possible to simulate and analyze hybrid energy systems,is utilized in the optimization process.Inside the HOMER Pro optimization,various system configuration is taken into account for the optimization.While simulating,it takes into account different combinations of components such as solar panels,wind turbines and batteries.Later on,it is being ranked by different factors such as net present cost(NPC),Cost of Energy(COE),etc.A comprehensive techno-economic research is carried out to evaluate various system configurations,considering key performance indicators such as total energy generation cost,operational expenditure,and greenhouse gas emissions.The results provide valuable insights into how renewable-based hybrid systems can reduce environmental impact while maintaining economic viability,supporting Brunei’s broader goals of energy diversification and sustainability.The study also emphasizes how such hybrid systems could be scaled for off-grid and rural populations in Brunei,where a dependable electricity supply is still a problem.Furthermore,sensitivity analyses were performed to evaluate the effects of variations in solar irradiation,load demand,and fuel prices on the overall system performance.Policymakers and energy planners can use these insights to help them make data-driven decisions about future investments in infrastructure for renewable energy. 展开更多

Feasibility of Micro-Hydro Power for Rural Electrification in Bangladesh:A Case Study from the Chittagong Hill Tracts

Bangladesh has achieved notable progress in expanding electricity access nationwide.Nonetheless,remote and topographically challenging regions such as the Chittagong Hill Tracts(CHT)continue to face coverage gaps due to grid extension difficulties.This research investigates the technical feasibility of micro-hydro power(MHP)systems as viable off-grid solutions for rural electrification in CHT.Field surveys conducted across various sites assessed available head and flow rates using GPS-based elevation measurements and portable flowmeters.Seasonal fluctuations were factored into the analysis to ensure year-round operational viability.The study involved estimating power output,selecting appropriate turbine types based on head-flow data,and proposing preliminary plant configurations.Results identify multiple locations with adequate head(2.5 to 10.4 m)and flow rates(0.10 to 0.35 m3/s),capable of generating between 1.5 and 16.5 kW,sufficient for essential rural applications.Based on site-specific head and discharge characteristics,Kaplan and Francis turbines were identified as the most suitable configurations,offering high efficiency for the medium-flow,low-to-medium head environments typical of the studied regions.Despite inherent technical potential,challenges such as seasonal variability,infrastructure complexities,and policy deficiencies remain.This investigation addresses a critical knowledge gap in local renewable energy planning.It offers a data-driven foundation for pilot projects and community-scale electrification initiatives in Bangladesh’s remote mountainous areas. 展开更多

Objective:Precision medicine approaches emphasize the importance of reliable prognostic tools for guiding individualized therapy decisions.In this study,we evaluated the clinical feasibility of the single patient classifier(SPC)test,a new clinical-grade prognostic assay,in stageⅡ-Ⅲgastric cancer patients.Methods:A prospective multicenter study was conducted,involving 237 patients who underwent gastrectomy between September 2019 and August 2020 across nine hospitals.The SPC test was employed to stratify patients into risk groups,and its feasibility and performance were evaluated.The primary endpoint was the proportion of the cases in which the test results were timely delivered before selecting postoperative treatment.Furthermore,3-year disease-free survivals of risk groups were analyzed.Results:The SPC test met the primary endpoint criteria.The 99.5%of SPC tests were timely delivered to hospitals before the postoperative treatment started.In a clinical setting,the median time from the specimen transfer to laboratory to the result delivery to hospital was 4 d.Furthermore,3-year disease-free survivals were significantly different between risk groups classified with SPC tests.Conclusions:This study highlights the SPC test's feasibility in offering crucial information timely delivered for making informed decisions regarding postoperative treatment strategies.It also provides evidence to support the implementation of a future prospective clinical trial aimed at evaluating the clinical utility of the SPC test in guiding personalized treatment decisions for gastric cancer patients. 展开更多

BACKGROUND There is still some room for optimizing ambulatory pediatric surgical procedures,and the preoperative and postoperative management quality for pediatric patients needs to be improved.AIM To discuss the safety and feasibility of the enhanced recovery after surgery(ERAS)-based management model for ambulatory pediatric surgical procedures.METHODS We selected 320 pediatric patients undergoing ambulatory surgery from June 2023 to January 2024 at The First People’s Hospital of Liangshan Yi Autonomous Prefecture.Of these,220 received ERAS-based management(research group)and 100 received routine management(control group).General information,postoperative ambulation activities,surgical outcomes(operation time,postoperative gastro-intestinal ventilation time,and hospital stay),postoperative pain visual analogue scale,postoperative complications(incision infection,abdominal distension,fever,nausea,and vomiting),and family satisfaction were compared.RESULTS The general information of the research group(sex,age,disease type,single parent,family history,etc.)was comparable to that of the control group(P>0.05),but the rate of postoperative(2 h,4 h,and 6 h after surgery)ambulation activities was statistically higher(P<0.01),and operation time,postoperative gastrointestinal ventilation time,and hospital stay were markedly shorter(P<0.05).The research group had lower visual analogue scale scores(P<0.01)at 12 h and 24 h after surgery and a lower incidence of total postoperative complications than the control group(P=0.001).The research group had higher family satisfaction than the control group(P=0.007).CONCLUSION The ERAS-based management model was safe and feasible in ambulatory pediatric surgical procedures and worthy of clinical promotion. 展开更多

Introduction: Seasonal Malaria Chemoprevention (SMC) aims at preventing malaria in children during the high transmission season. Introduction: Seasonal Malaria Chemoprevention (SMC) aims at preventing malaria in children during the high transmission season. It has been recommended by the WHO since 2013 for children from the age of 3-59 months. However, despite the impact of this intervention, a peak in the prevalence and incidence of malaria is observed in children from the age of 5-9 years. The aim of this study is to determine from the current literature the feasibility, impact and cost-effectiveness of extending SMC to five cycles and to older children. Methods: A litterature search of PubMed/Medline, NCBI and Google scholar identified 1333 articles. After reading the titles and abstracts by two authors, 24 articles were selected and submitted for full reading. Random control studies on the extension of SMC, malaria, feasibility of SMC, impact of SMC and cost-effectiveness of SMC were selected. A total of 16 articles were included for the qualitative synthesis after excluding 8 studies. Results: Following the summary of the evidence, we conclude that the extension is feasible but will be confronted with the unavailability of older children. The intervention period coincides with field work. SMC is effective in reducing the prevalence and incidence of malaria and the parasite density in children. The financial cost of administering SMC is lower than that of treating a child suffering from malaria. Conclusion: After analysing the information, it was found that the majority of the African population supports the extension of the SMC to the number of cycles and the age group in order to alleviate the high mortality and morbidity rates among children due to malaria. 展开更多

Objective:The aim is to develop and verify the feasibility of an adolescent coping model in seasonal flood-prone areas.This coping model supports mental health nurses'practical application to adolescents in flood-prone areas.Methods:We developed the adolescent coping model based on the processes established in three integrated theories:self-care management for vulnerable populations,coping approaches,and coping with disaster.We conducted semi-structured interviews with 15 participants to explore their perspectives and experiences with flood disasters.We used purposeful sampling for maximum diversity.Data were collected through face-to-face,in-depth interviews.Data were transcribed verbatim,followed by qualitative content analysis.The coping model of adolescents in flood-prone areas was analyzed qualitatively and inductively from the viewpoint of acceptability.Results:The following five categories were identified:(1)coping process post-disaster;(2)coping approach;(3)community power;(4)vulnerability;and(5)self-potential.These domains formed a model for coping model adolescents in seasonal flood-prone areas.Accordingly,our findings showed that the model could be practical for mental health community nursing.Conclusions:Mental health nurses can use the coping model for disaster mental health nursing in adolescents in flood-prone areas.The application of this model will help adolescents increase self-efficacy. 展开更多

碳达峰、碳中和已成为中国核心的能源发展战略,着力发展以可再生能源为主体、广泛接纳分布式电源的新型电力系统是实现这一战略的重要途径。与发展新型电力系统相适应的电网规划方法应充分考虑发电碳排放,保障零碳、低碳电能传输,避免... 碳达峰、碳中和已成为中国核心的能源发展战略,着力发展以可再生能源为主体、广泛接纳分布式电源的新型电力系统是实现这一战略的重要途径。与发展新型电力系统相适应的电网规划方法应充分考虑发电碳排放,保障零碳、低碳电能传输,避免由输电阻塞引起的弃光弃风;同时,在规划中充分挖掘分布式电源高占比配电系统的协同作用,助力可再生能源的高效消纳。为此,文中提出面向低碳目标的输配电系统协同电网规划模型,将碳减排纳入电网规划优化目标。针对分布式电源高占比配电系统,采用可行区域投影的方法将其等效表示为可集成到电网规划模型中的一组约束。一方面,这组等效约束极大地降低了规划模型的复杂度,避免因考虑配电系统导致的模型求解困难;另一方面,等效约束不仅能有效地描绘配电系统内部的物理限制,还能在电网规划模型中准确反映配电系统的经济和运行特性,实现分布式电源高占比配电系统的高效协同。算例结果验证了等效约束在描绘配电系统内部的物理限制与反映配电系统经济和运行特性中的有效性。此外,结果还表明,扩展输电线路将新能源送出能够缓解本地线路阻塞,帮助新能源消纳,减少碳排放。 展开更多

大规模风电并网导致电力系统惯量和一次调频响应资源减少,大扰动下系统频率安全问题突出。为应对风电不确定性和系统惯量降低的挑战,提出计及风电频率支撑能力和运行风险的鲁棒机组组合(unitcommitment,UC)模型。首先,通过系统发生有功... 大规模风电并网导致电力系统惯量和一次调频响应资源减少,大扰动下系统频率安全问题突出。为应对风电不确定性和系统惯量降低的挑战,提出计及风电频率支撑能力和运行风险的鲁棒机组组合(unitcommitment,UC)模型。首先,通过系统发生有功扰动后频率偏差动力学摆动方程建立频率安全的运行约束模型,并嵌入到UC问题中。其次,考虑到风电出力不确定性,提出风电出力鲁棒可行域定义以表征系统接纳风电的安全运行范围,并基于此提出系统运行风险模型。最后,基于两阶段鲁棒优化理论提出计及风电频率支撑能力和运行风险的UC鲁棒优化模型,并采用列和约束生成(column and constraint generation,C&CG)算法求解该模型。在IEEE9和IEEE118节点测试系统进行仿真分析,结果验证了所提模型的有效性。 展开更多

CO_(2)驱油对提高采收率、减少温室气体排放有重要意义,是国家实现“双碳”目标有效手段,其中地震监测技术是关键。文中分析总结了国内外CO_(2)驱油地震监测技术的研究现状和进展,主要包括时移地震可行性分析、一致性处理技术和综合解释等,并重点论述了CO_(2)驱地震监测技术在高89区块的应用。可行性分析是研究区块开展时移地震监测的重要前提,满足油藏地质条件、岩石物理条件和地震条件才能有效地进行时移地震监测。要实现油藏动态监测,基础地震与监测地震(时移地震)的一致性处理也非常重要,需开展时差、振幅、频率、相位等要素的匹配滤波。时移地震综合解释有助于准确预测CO_(2)驱波及范围,叠前主要借助于AVO属性分析方法;叠后基于基础地震与监测地震资料的差值分析仍是主要方法,频率域信息如分频处理、速度频散、低频伴影等也值得使用;基于深度学习的波及范围预测方法方兴未艾,但其运算效率和泛化能力仍有待进一步提高。最后展望了时移地震技术在提高监测精度、开发监测方法、拓展应用市场等方面的发展潜力。 展开更多

随着各城市电动出租车群体不断扩大,其规模化的无序充电行为不仅会加剧电网负荷曲线的波动,威胁电网的稳定和安全,也会降低偏远充电站的设备利用率。为此,基于“车-站-网”信息协同建立了电动出租车有序充电引导系统,该系统在考虑用户... 随着各城市电动出租车群体不断扩大,其规模化的无序充电行为不仅会加剧电网负荷曲线的波动,威胁电网的稳定和安全,也会降低偏远充电站的设备利用率。为此,基于“车-站-网”信息协同建立了电动出租车有序充电引导系统,该系统在考虑用户参与调度可行性的基础上提出了一种电动出租车时空双层充电优化策略。在时间层模型中,以电网负荷曲线峰谷差、标准差最小以及用户充电成本最小建立多目标优化模型。该模型提前优化电动出租车的充电时间负荷,为各用户预约充电时间窗口;在空间层模型中,以各充电站平均利用率均衡及用户充电时间成本最小为目标对电动出租车充电空间负荷进行实时调度,为用户分配最优充电站。最后,通过算例仿真验证了所提优化策略的有效性,能够兼顾配电网、充电站运营商、电动出租车用户三方的利益。 展开更多
