BACKGROUND Aseptic loosening remains the leading cause of revision in primary total hip arthroplasty(pTHA).However, the literature demonstrates significant variability regarding the relative contributions of different factors.AIM To investigate the key determinants of aseptic loosening,we performed a systematic review and meta-analysis.METHODS A comprehensive search of PubMed,Web of Science,EMBASE,and the Cochrane Library was conducted, encompassing studies from database inception to January 1,2025.Meta-analyses were performed to evaluate factors associated with aseptic loosening following pTHA.Inclusion and exclusion criteria were systematically applied at each stage to ensure methodological transparency and reproducibility.Study quality was assessed using standardized categories.Pooled odds ratio(OR)with corresponding 95%confidence interval were calculated with random-or fixed-effects models to generate reliability estimates, and study heterogeneity was visualized using forest plots.Ten factors, categorized into patient-,surgeon-,and device-related domains, were reviewed and meta-analyzed.Funnel plot analysis demonstrated a relatively symmetrical distribution, suggesting minimal publication bias.RESULTS A meta-analysis of 20 studies(520789 participants)found a pooled prevalence of 1.96%.Significant risk factors for aseptic loosening after pTHA included elevated body mass index(OR=1.116,P<0.001),higher Charlson comorbidity index(OR=1.378,P<0.001),prosthesis-related factors(OR=1.497,P<0.001),and adverse lifestyles(OR=2.198,P=0.037).Protective factors were non-white race(OR=0.445,P<0.001)and favorable genetics(OR=0.723,P<0.001).Male sex increased risk(OR=1.232,P=0.016),while age and anatomy were not significant.Surgical expertise showed a slight protective effect(OR=1.048,P<0.001).A comprehensive understanding of the modifiable and non-modifiable factors contributing to aseptic loosening after pTHA requires consideration of patient-related factors, surgical expertise, and prosthesis characteristics.CONCLUSION The identification of these factors is critical for risk mitigation.High-risk patients should receive targeted counseling regarding individualized profiles.Further studies are warranted to establish clearer causal relationships and identify additional contributing factors. 展开更多

Border-associated macrophages are located at the interface between the brain and the periphery, including the perivascular spaces, choroid plexus, and meninges. Until recently, the functions of border-associated macrophages have been poorly understood and largely overlooked. However, a recent study reported that border-associated macrophages participate in stroke-induced inflammation, although many details and the underlying mechanisms remain unclear. In this study, we performed a comprehensive single-cell analysis of mouse border-associated macrophages using sequencing data obtained from the Gene Expression Omnibus(GEO) database(GSE174574 and GSE225948). Differentially expressed genes were identified, and enrichment analysis was performed to identify the transcription profile of border-associated macrophages. CellChat analysis was conducted to determine the cell communication network of border-associated macrophages. Transcription factors were predicted using the ‘pySCENIC' tool. We found that, in response to hypoxia, borderassociated macrophages underwent dynamic transcriptional changes and participated in the regulation of inflammatory-related pathways. Notably, the tumor necrosis factor pathway was activated by border-associated macrophages following ischemic stroke. The pySCENIC analysis indicated that the activity of signal transducer and activator of transcription 3(Stat3) was obviously upregulated in stroke, suggesting that Stat3 inhibition may be a promising strategy for treating border-associated macrophages-induced neuroinflammation. Finally, we constructed an animal model to investigate the effects of border-associated macrophages depletion following a stroke. Treatment with liposomes containing clodronate significantly reduced infarct volume in the animals and improved neurological scores compared with untreated animals. Taken together, our results demonstrate comprehensive changes in border-associated macrophages following a stroke, providing a theoretical basis for targeting border-associated macrophages-induced neuroinflammation in stroke treatment. 展开更多

Six new lanthanide complexes:[Ln(3,4-DEOBA)3(4,4'-DM-2,2'-bipy)]2·2C_(2)H_(5)OH,[Ln=Dy(1),Eu(2),Tb(3),Sm(4),Ho(5),Gd(6);3,4-DEOBA-=3,4-diethoxybenzoate,4,4'-DM-2,2'-bipy=4,4'-dimethyl-2,2'-bipyridine]were successfully synthesized by the volatilization of the solution at room temperature. The crystal structures of six complexes were determined by single-crystal X-ray diffraction technology. The results showed that the complexes all have a binuclear structure, and the structures contain free ethanol molecules. Moreover, the coordination number of the central metal of each structural unit is eight.Adjacent structural units interact with each other through hydrogen bonds and further expand to form 1D chain-like and 2D planar structures. After conducting a systematic study on the luminescence properties of complexes 1-4,their emission and excitation spectra were obtained.Experimental results indicated that the fluorescence lifetimes of complexes 2 and 3 were 0.807 and 0.845 ms,respectively. The emission spectral data of complexes 1-4 were imported into the CIE chromaticity coordinate system, and their corre sponding luminescent regions cover the yellow light, red light, green light, and orange-red light bands, respectively.Within the temperature range of 299.15-1300 K,the thermal decomposition processes of the six complexes were comprehensively analyzed by using TG-DSC/FTIR/MS technology. The hypothesis of the gradual loss of ligand groups during the decomposition process was verified by detecting the escaped gas,3D infrared spectroscopy, and ion fragment information detected by mass spectrometry. The specific decomposition path is as follows:firstly, free ethanol molecules and neutral ligands are removed, and finally, acidic ligands are released;the final product is the corresponding metal oxide.CCDC:2430420,1;2430422,2;2430419,3;2430424,4;2430421,5;2430423,6. 展开更多

The endpoint carbon content in the converter is critical for the quality of steel products, and accurately predicting this parameter is an effective way to reduce alloy consumption and improve smelting efficiency. However, most scholars currently focus on modifying methods to enhance model accuracy, while overlooking the extent to which input parameters influence accuracy. To address this issue,in this study,a prediction model for the endpoint carbon content in the converter was developed using factor analysis(FA)and support vector machine(SVM)optimized by improved particle swarm optimization(IPSO).Analysis of the factors influencing the endpoint carbon content during the converter smelting process led to the identification of 21 input parameters.Subsequently,FA was used to reduce the dimensionality of the data and applied to the prediction model. The results demonstrate that the performance of the FA-IPSO-SVM model surpasses several existing methods, such as twin support vector regression and support vector machine. The model achieves hit rates of 89.59%,96.21%,and 98.74%within error ranges of±0.01%,±0.015%,and±0.02%,respectively.Finally,based on the prediction results obtained by sequentially removing input parameters, the parameters were classified into high influence(5%-7%),medium influence(2%-5%),and low influence(0-2%)categories according to their varying degrees of impact on prediction accuracy. This classi-fication provides a reference for selecting input parameters in future prediction models for endpoint carbon content. 展开更多

Background:Receptor-interacting protein kinases(RIPKs)regulate cell death, inflammation, and immune responses, yet their roles in cancer are not fully understood. This study investigates the expression, genomic alterations, and functional implications of RIPK family members across various cancers. Methods:We collected multi-omics data from The Cancer Genome Atlas and other public databases, including gene expression, copy number variation(CNV),mutation, methylation, tumor mutation burden(TMB),and microsatellite instability(MSI).Differential expression and survival analyses were performed using DESeq2 and Cox proportional hazards models.CNV and mutation data were analyzed with GISTIC2 and Mutect2,and methylation data with the ChAMP package.Correlations with TMB and MSI were assessed using Pearson coefficients, and gene set enrichment analysis was conducted with the MSigDB Hallmark gene sets. Results:RIPK family members show significant differential expression in various cancers, with RIPK1 and RIPK4 frequently altered.Survival analysis reveals heterogeneous impacts on overall survival.CNV and mutation analyses identify high alteration frequencies for RIPK2 and RIPK7,affecting gene expression.RIPK1 and RIPK7 are hypermethylated in several cancers, inversely correlating with RIPK3 expression.RIPK1,RIPK2,RIPK5,RIPK6,and RIPK7 correlate positively with TMB,while RIPK3 shows negative correlations in some cancers.MSI analysis indicates associations with DNA mismatch repair.G ene set enrichment analysis highlights immune-related pathway enrichment for RIPK1,RIPK2,RIPK3,and RIPK6,and cell proliferation and DNA repair pathways for RIPK4 and RIPK5.RIPK family members showed heterogeneous alterations across cancers:for example,RIPK7 was mutated in up to~15%of u terine c orpus e ndometrial c arcinoma and l ung s quamous c ell c arcinoma cases, and RIPK1 and RIPK7 exhibited frequent promoter hypermethylation in multiple tumor types.Several genes displayed context-dependent associations with overall survival and with TMB/MSI.Conclusion:This pan-cancer analysis of the RIPK family reveals their diverse roles and potential as biomarkers and therapeutic targets. The findings emphasize the importance of RIPK genes in tumorigenesis and suggest context-dependent functions across cancer types.Further studies are needed to explore their mechanisms in cancer development and clinical applications. 展开更多

The sulfation and decomposition process has proven effective in selectively extracting lithium from lepidolite. It is essential to clarify the thermochemical behavior and kinetic parameters of decomposition reactions.Accordingly, comprehensive kinetic study by employing thermalgravimetric analysis at various heating rates was presented in this paper. Two main weight loss regions were observed during heating. The initial region corresponded to the dehydration of crystal water, whereas the subsequent region with overlapping peaks involved complex decomposition reactions. The overlapping peaks were separated into two individual reaction peaks and the activation energy of each peak was calculated using isoconversional kinetics methods. The activation energy of peak 1 exhibited a continual increase as the reaction conversion progressed, while that of peak 2 steadily decreased. The optimal kinetic models, identified as belonging to the random nucleation and subsequent growth category, provided valuable insights into the mechanism of the decomposition reactions.Furthermore, the adjustment factor was introduced to reconstruct the kinetic mechanism models, and the reconstructed models described the kinetic mechanism model more accurately for the decomposition reactions. This study enhanced the understanding of the thermochemical behavior and kinetic parameters of the lepidolite sulfation product decomposition reactions, further providing theoretical basis for promoting the selective extraction of lithium. 展开更多

This paper investigates the reliability of internal marine combustion engines using an integrated approach that combines Fault Tree Analysis(FTA)and Bayesian Networks(BN).FTA provides a structured,top-down method for identifying critical failure modes and their root causes, while BN introduces flexibility in probabilistic reasoning, enabling dynamic updates based on new evidence. This dual methodology overcomes the limitations of static FTA models, offering a comprehensive framework for system reliability analysis.Critical failures, including External Leakage(ELU),Failure to Start(FTS),and Overheating(OHE),were identified as key risks.By incorporating redundancy into high-risk components such as pumps and batteries, the likelihood of these failures was significantly reduced. For instance, redundant pumps reduced the probability of ELU by 31.88%,while additional batteries decreased the occurrence of FTS by 36.45%.The results underscore the practical benefits of combining FTA and BN for enhancing system reliability, particularly in maritime applications where operational safety and efficiency are critical. This research provides valuable insights for maintenance planning and highlights the importance of redundancy in critical systems, especially as the industry transitions toward more autonomous vessels. 展开更多

AIM:To explore the causal relationship between several possible behavioral factors and high myopia(HM)using multivariable Mendelian randomization(MVMR)approach and to find the mediators among them with mediation analysis.METHODS:The causal effects of several behavioral factors, including screen time, education time, time spent outdoors, and physical activity,on the risk of HM using univariable Mendelian randomization(MR)and MVMR analyses were first assessed.Genome-wide association study summary statistics of serum metabolites were also used in mediation analysis to determine the extent to which serum metabolites mediate the effects of behavioral factors on HM.RESULTS:MR analyses indicated that both increased time spent outdoors and a higher frequency of moderate physical activity significantly reduced the risk of HM.Further MVMR analysis confirmed that moderate physical activity independently contributed to a lower risk of HM.Additionally,MR analyses identified 13 serum metabolites significantly associated with HM,of which 12 were lipids and one was an amino acid derivative.Mediation analysis revealed that six lipid metabolites mediated the protective effects of moderate physical activity on HM,with the highest mediation proportion observed for 1-(1-enyl-palmitoyl)-GPC(p-16:0;30.83%).CONCLUSION:This study suggests that in addition to outdoor time, moderate physical activity habits may have an independent protective effect against HM and pointed to lipid metabolites as priority targets for the prevention due to low physical activity. These results emphasize the importance of physical activity and metabolic health in HM and underscore the need for further study of these complex associations. 展开更多

Brain-derived neurotrophic factor is a crucial neurotrophic factor that plays a significant role in brain health. Although the vast majority of meta-analyses have confirmed that exercise interventions can increase brain-derived neurotrophic factor levels in children and adolescents, the effects of specific types of exercise on brain-derived neurotrophic factor levels are still controversial. To address this issue, we used meta-analytic methods to quantitatively evaluate, analyze, and integrate relevant studies. Our goals were to formulate general conclusions regarding the use of exercise interventions, explore the physiological mechanisms by which exercise improves brain health and cognitive ability in children and adolescents, and provide a reliable foundation for follow-up research. We used the Pub Med, Web of Science, Science Direct, Springer, Wiley Online Library, Weipu, Wanfang, and China National Knowledge Infrastructure databases to search for randomized controlled trials examining the influences of exercise interventions on brain-derived neurotrophic factor levels in children and adolescents. The extracted data were analyzed using Review Manager 5.3. According to the inclusion criteria, we assessed randomized controlled trials in which the samples were mainly children and adolescents, and the outcome indicators were measured before and after the intervention. We excluded animal experiments, studies that lacked a control group, and those that did not report quantitative results. The mean difference(MD;before versus after intervention) was used to evaluate the effect of exercise on brain-derived neurotrophic factor levels in children and adolescents. Overall, 531 participants(60 children and 471 adolescents, 10.9–16.1 years) were included from 13 randomized controlled trials. Heterogeneity was evaluated using the Q statistic and I^(2) test provided by Review Manager software. The meta-analysis showed that there was no heterogeneity among the studies(P = 0.67, I^(2) = 0.00%). The combined effect of the interventions was significant(MD = 2.88, 95% CI: 1.53–4.22, P < 0.0001), indicating that the brain-derived neurotrophic factor levels of the children and adolescents in the exercise group were significantly higher than those in the control group. In conclusion, different types of exercise interventions significantly increased brain-derived neurotrophic factor levels in children and adolescents. However, because of the small sample size of this meta-analysis, more high-quality research is needed to verify our conclusions. This metaanalysis was registered at PROSPERO(registration ID: CRD42023439408). 展开更多

The study aims to determine the validity and reliability of the Wechsler Preschool and Primary Scale of Intelligence–Third Edition(WPPSI-III)scores in a sample of kindergarten and lower primary pupils from Khartoum State,Sudan. It also aims to examine whether test’s factor structure in this sample replicated that of the original WPPSI-III.The study sample consisted of 384 kindergarten and primary school children in Khartoum State(females=50%mean age=4.14,SD=1.37),selected using stratified random sampling across its seven localities:Khartoum,Jebel Awliya,Khartoum Bahri,East Nile,Omdurman,Ombada,Karari. For concurrent validation, the children additionally completed the Goodenough Draw-a-Man Test, and the Colored Progressive Matrices.WPPSI-III scores demonstrated high internal consistency across the subtest items.Confirmatory factor analysis indicators for total, verbal, and performance intelligence were all excellent. The scale also showed weak to strong score stability ranging from 0.25(weak)to 0.88(strong)based on the Spearman-Brown equation,0.25 to 0.75 based on the Guttman split-half method. The Cronbach’s alpha coefficient scores ranged from 0.54 to 0.93.The WPPSI-III and Goodenough Draw-a-Man Test scores concurrent validity scores were poor(0.05)to modest(0.31),and while those with the Colored Progressive Matrices test were poor(r=0.04–0.18).Thesefindings provide evidence to suggest that the WPPSI-III is appropriate for research use with kindergarten and lower primary school students in Khartoum State,Sudan. 展开更多

BACKGROUND Endoscopic submucosal dissection(ESD)is a minimally invasive, safe, and efficient treatment technique for patients diagnosed with early esophageal cancer. However, postoperative disease recurrence remains an important clinical challenge because it negatively alters patient prognosis and quality of life.As such, identification of relevant risk factors for recurrence can help optimize postoperative management strategies.AIM To assess factors that contribute to the risk for disease recurrence after ESD for early esophageal cancer.METHODS Clinical data from 210 patients diagnosed with early stage esophageal cancer, who underwent ESD at the authors’center between March 2012 and March 2025,were retrospectively collected and analyzed.Patients were categorized into 2 groups according to postoperative disease recurrence:Recurrence(n=30),and without recurrence(n=180).Disease recurrence was defined as the appearance of new tumor lesions or pathologically confirmed tumor recurrence during the postoperative follow-up period.Risk factors associated with postoperative recurrence were identified using univariate and multivariate logistic regression analyses.RESULTS During the follow-up period,30 patients experienced tumor recurrence, corresponding to a recurrence rate of 14.19%.Multivariate analysis revealed that poor differentiation was a significant potential cause of esophageal cancer recurrence[odds ratio(OR)=1.782,95%confidence interval(CI):1.154-2.196;P<0.001].Tumors infiltrating the submucosa were more likely to recur than those penetrating the lamina propria or muscularis mucosa(OR=1.573,95%CI:1.073-2.481;P<0.001).Furthermore,inability to completely resect the tumor greatly increased the likelihood of recurrence(OR=2.189,95%CI:1.193-3.125;P=0.001).Tumor diameter≥2 cm was an independent risk factor for postoperative recurrence(OR=1.981,95%CI:1.482-2.862;P=0.005).CONCLUSION Recurrence of early esophageal cancer after ESD is largely influenced by the degree of differentiation, depth of lesion invasion, complete resection status of the tumor, and tumor diameter. 展开更多

BACKGROUND Dental implants are widely used to replace missing teeth.Currently, clinicians assess osseointegration success by measuring the implant’s stability within the bone and monitoring the marginal tissue height.Diabetes, especially type 2 diabetes mellitus(T2DM),has been reported to impair implant healing, drastically reducing implant success rates.AIM To analyze the high-risk factors for inflammatory response and prognosis after dental implantation in patients with T2DM,and provide strong evidence for reducing the incidence of postimplant peri-implantitis(PI).METHODS We performed a retrospective review of 146 patients with T2DM who had dental implants placed at Tianjin Fifth Central Hospital, between September 2021 and September 2023,which was regarded as the observation group.Moreover,60 ageand gender-matched individuals with normal blood glucose levels served as the control group. The general information, postoperative periodontal indices, and levels of inflammatory factors were comprehensively analyzed and compared.Furthermore, the incidence of postimplant PI was counted, and multivariate logistic regression was used to identify the determinants of postimplant PI.RESULTS In terms of the periodontal indices, the probing depth, modified sulcus bleeding index, and marginal bone loss in the observation cohorts began to increase significantly at 6 months and 3 months, respectively, after the completion of dental implant restoration. The T2DM cases demonstrated significantly elevated counts of leukocytes, lymphocytes, and neutrophils compared to the controls at 24 hours postoperatively. Moreover, the TNF-α,IL-1β,and IL-6 concentrations started to increase significantly in the gingival crevicular fluid 3 months after the completion of dental implant restoration in both cohorts, with the observation group exhibiting higher levels than the controls at each time point.63 out of the 146 cases developed PI.Multivariate logistic regression analysis indicated that high glycosylated hemoglobin levels, smoking, daily tooth-brushing frequency of less than once, and the anterior tooth as the implant site independently contributed to postimplant PI in T2DM cases, while a tooth-brushing duration of≥3 minutes was a protective factor.CONCLUSION Patients with T2DM are at risk of developing PI following dental implantation.Clinically,it is necessary to enhance the identification of risk factors for postimplant PI,improve risk prediction, prevention, and control, and formulate targeted intervention countermeasures to reduce the occurrence of postimplant PI. 展开更多

BACKGROUND Psychological comorbidities, such as anxiety and depression,in patients with chronic ankle instability(CAI)may impede ankle function improvement, although the precise nature of this association warrants further investigation.AIM To analyze the correlation of anxiety and depression with ankle function in patients with CAI and discussing the risk factors.METHODS This study included 116 patients with CAI,who were admitted to our hospital from July 2022 to July 2024.Anxiety and depression states of patients were assessed with the self-rating anxiety scale(SAS)and self-rating depression scale(SDS),respectively, and their ankle joint function was assessed with the anklehindfoot function score of the American Orthopedic Foot and Ankle Society.Further, the ankle function of patients with CAI with different anxiety and depression states was discussed.Furthermore, the Pearson correlation coefficient was used to analyze the correlation of anxiety and depression with ankle joint function in such patients.Univariate and multivariate analyses were conducted to investigate the factors affecting ankle joint function in patients with CAI.RESULTS Among the 116 patients with CAI,97,13,5,and 1 cases demonstrated none, mild, moderate, and severe anxiety, whereas 95,15,6,and 0 cases showed none, mild, moderate, and severe depression, respectively. The average ankle joint function score was 74.82±6.93 points. The ankle joint function in patients with CAI presented a significant downward tendency as the degree of anxiety and depression increased.Correlation analysis revealed that both the SAS and SDS scores of patients with CAI were significantly negatively correlated with the ankle joint function score.Univariate and multivariate analyses indicated that the risk factors affecting patients’ankle joint function included early functional rehabilitation, visual analog scale, and SDS.CONCLUSION A substantial number of patients with CAI suffer from anxiety and depression, and these negative emotions,to a certain extent, harm the smooth rehabilitation of ankle joint function. 展开更多
