Brain-derived neurotrophic factor is a crucial neurotrophic factor that plays a significant role in brain health. Although the vast majority of meta-analyses have confirmed that exercise interventions can increase brain-derived neurotrophic factor levels in children and adolescents, the effects of specific types of exercise on brain-derived neurotrophic factor levels are still controversial. To address this issue, we used meta-analytic methods to quantitatively evaluate, analyze, and integrate relevant studies. Our goals were to formulate general conclusions regarding the use of exercise interventions, explore the physiological mechanisms by which exercise improves brain health and cognitive ability in children and adolescents, and provide a reliable foundation for follow-up research. We used the Pub Med, Web of Science, Science Direct, Springer, Wiley Online Library, Weipu, Wanfang, and China National Knowledge Infrastructure databases to search for randomized controlled trials examining the influences of exercise interventions on brain-derived neurotrophic factor levels in children and adolescents. The extracted data were analyzed using Review Manager 5.3. According to the inclusion criteria, we assessed randomized controlled trials in which the samples were mainly children and adolescents, and the outcome indicators were measured before and after the intervention. We excluded animal experiments, studies that lacked a control group, and those that did not report quantitative results. The mean difference(MD;before versus after intervention) was used to evaluate the effect of exercise on brain-derived neurotrophic factor levels in children and adolescents. Overall, 531 participants(60 children and 471 adolescents, 10.9–16.1 years) were included from 13 randomized controlled trials. Heterogeneity was evaluated using the Q statistic and I^(2) test provided by Review Manager software. The meta-analysis showed that there was no heterogeneity among the studies(P = 0.67, I^(2) = 0.00%). The combined effect of the interventions was significant(MD = 2.88, 95% CI: 1.53–4.22, P < 0.0001), indicating that the brain-derived neurotrophic factor levels of the children and adolescents in the exercise group were significantly higher than those in the control group. In conclusion, different types of exercise interventions significantly increased brain-derived neurotrophic factor levels in children and adolescents. However, because of the small sample size of this meta-analysis, more high-quality research is needed to verify our conclusions. This metaanalysis was registered at PROSPERO(registration ID: CRD42023439408). 展开更多

The endpoint carbon content in the converter is critical for the quality of steel products, and accurately predicting this parameter is an effective way to reduce alloy consumption and improve smelting efficiency. However, most scholars currently focus on modifying methods to enhance model accuracy, while overlooking the extent to which input parameters influence accuracy. To address this issue,in this study,a prediction model for the endpoint carbon content in the converter was developed using factor analysis(FA)and support vector machine(SVM)optimized by improved particle swarm optimization(IPSO).Analysis of the factors influencing the endpoint carbon content during the converter smelting process led to the identification of 21 input parameters.Subsequently,FA was used to reduce the dimensionality of the data and applied to the prediction model. The results demonstrate that the performance of the FA-IPSO-SVM model surpasses several existing methods, such as twin support vector regression and support vector machine. The model achieves hit rates of 89.59%,96.21%,and 98.74%within error ranges of±0.01%,±0.015%,and±0.02%,respectively.Finally,based on the prediction results obtained by sequentially removing input parameters, the parameters were classified into high influence(5%-7%),medium influence(2%-5%),and low influence(0-2%)categories according to their varying degrees of impact on prediction accuracy. This classi-fication provides a reference for selecting input parameters in future prediction models for endpoint carbon content. 展开更多

BACKGROUND Bile leakage is a common complication following laparoscopic common bile duct exploration(LCBDE)with primary duct closure(PDC).Identifying and analyzing the risk factors associated with bile leakage is crucial for improving surgical outcomes.AIM To explore the value analysis of common risk factors for bile leakage after LCBDE and PDC,with a focus on strict adherence to indications.METHODS Clinical data of 106 cases undergoing LCBDE+PDC in the Hepatobiliary and Pancreatic Surgery Department(Division 1)of Chuzhou First People’s Hospital from April 2019 to March 2024 were collected.Retrospective and multiple factor regression analysis were conducted on common risk factors for bile leakage. The change in surgical time was analyzed using the cumulative summation(CUSUM)method, and the minimum number of cases required to complete the learning curve for PDC was obtained based on the proposed fitting curve by identifying the CUSUM maximum value.RESULTS Multifactor logistic regression analysis showed that fibrinous inflammation and direct bilirubin/indirect bilirubin were significant independent high-risk factors for postoperative bile leakage(P<0.05).The time to drain removal and length of hospital stay in cases without bile leakage were significantly shorter than in cases with bile leakage(P<0.05),with statistical significance. The CUSUM method indicated that a minimum of 51 cases were required for the surgeon to complete the learning curve(P=0.023).CONCLUSION With a good assessment of duodenal papilla sphincter function, unobstructed bile-pancreatic duct convergence, exact stone clearance, and sufficient surgical experience to complete the learning curve,PDC remains the preferred method for bile duct closure and is worthy of clinical promotion. 展开更多

Restoration of phase aberrations is crucial for addressing atmospheric turbulence in light propagation.Traditional restoration algorithms based on Zernike polynomials(ZPs)often encounter challenges related to high computational complexity and insufficient capture of high-frequency phase aberration components,so we proposed a Principal-Component-Analysis-based method for representing phase aberrations. This paper discusses the factors influencing the accuracy of restoration, mainly including the sample space size and the sampling interval of D/r_(0),on the basis of characterizing phase aberrations by Principal Components(PCs).The experimental results show that a larger D/r_(0)sampling interval can ensure the generalization ability and robustness of the principal components in the case of a limited amount of original data, which can help to achieve high-precision deployment of the model in practical applications quickly. In the environment with relatively strong turbulence in the test set of D/r_(0)=24,the use of 34 terms of PCs can improve the corrected Strehl ratio(SR)from 0.007 to 0.1585,while the Strehl ratio of the light spot after restoration using 34 terms of ZPs is only 0.0215,demonstrating almost no correction effect. The results indicate that PCs can serve as a better alternative in representing and restoring the characteristics of atmospheric turbulence induced phase aberrations. These findings pave the way to use PCs of phase aberrations with fewer terms than traditional ZPs to achieve data dimensionality reduction, and offer a reference to accelerate and stabilize the model and deep learning based adaptive optics correction. 展开更多

Objective:This study aimed to investigate the prevalence and determinants of metabolic syndrome(MetS)among the population in Gannan(Southern Jiangxi),and to develop a nomogram for MetS prediction. Methods:In 2022,a multi-stage stratified random sampling method was used to select permanent residents aged 35 years and above in southern Jiangxi as study participants.MetS was defined according to the International Diabetes Federation(IDF)criteria.Participants'demographics, medical history, blood biochemistry data, and anthropometric variables were collected to screen for significant variables for the prediction model of MetS.Multivariable logistic regression was employed to explore the factors associated with MetS.Subsequently, the data were divided into a training set and a validation set,and a nomogram was developed to create the predictive model for MetS.The training set was utilized for nomogram model construction and preliminary validation, while the validation set was used for internal validation. The performance of the nomogram was assessed based on receiver operating characteristic curve(ROC),calibration curves, and decision curve analysis(DCA).Results:A total of 1581 participants were enrolled in the study, revealing a prevalence of MetS of 27.39%(95%CI:25.19%-29.59%).The age-standardized prevalence was calculated to be 27.81%.Nine variables were identified as influencing factors for MetS:age, residence, occupation, history of hyperlipidemia, history of hyperuricemia, hip circumference, glycated hemoglobin A1c(HbA1c),resting heart rate(RHR),and body mass index(BMI).The participants were randomly divided into a training set(n=1107,70%)and a validation set(n=474,30%).The nomogram was validated through preliminary validation area under curve(AUC:0.844)and internal validation(AUC:0.825).Calibration plots demonstrated good agreement in the training sets. Conclusion:The prevalence of MetS is notably high in Ganzhou,Jiangxi. The nomogram, which is based on age, residence, occupation, history of hyperlipidemia, history of hyperuricemia, hip circumference,HbA1c,RHR and BMI variables, exhibits strong predictive efficacy and can be utilized to assess the risk of MetS in middle-aged and elderly populations. 展开更多

BACKGROUND Anxiety complicates recovery in esophagectomy patients undergoing esophagectomy, while preoperative depressive symptoms may further delay surgeryassociated malnutrition resolution, highlighting a critical need for psychological intervention.AIM To explore the correlation of anxiety and depression with mindfulness levels in patients undergoing esophagectomy for esophageal cancer and analyze associated risk factors.METHODS A total of 120 patients with esophageal cancer, all of whom underwent esophagectomy in The Affiliated Yongchuan Hospital of Chongqing Medical University between September 2022 and September 2024,were enrolled. The hospital anxiety and depression scale(HADS)was utilized to evaluate the anxiety(HADS-anxiety subscale,HADS-A)and depression(HADS-depression subscale,HADS-D)levels of these patients, and the Mindful Attention Awareness Scale(MAAS)was employed to assess their mindfulness levels. The relationships among anxiety, depression, and mindfulness levels were analyzed, and both univariate and multivariate analyses were conducted to identify the risk factors for anxiety and depression in these patients.RESULTS Data revealed that the mean HADS-A score of patients undergoing esophagectomy was(10.43±4.24)points,and 40.00%,35.00%,and 10.83%of the patients had mild, moderate, and severe anxiety, respectively. The mean HADS-D score https://www.wjgnet.com 2 September 19,2025 Volume 15 Issue 9 was(9.20±4.66)points,and 37.50%,23.33%,and 9.17%of the patients had mild, moderate, and severe depression, respectively.Patients with negative or mild anxiety(or depression)exhibited significantly higher MAAS scores than those with moderate or severe anxiety(or depression). The HADS-A and HADS-D scores showed a strong and negative correlation with the MAAS score.Univariate and multivariate analyses demonstrated that sex(P=0.006),age(P=0.017),per capita monthly household income(P=0.005),and educational level(P=0.004)were all independently associated with the exacerbation of anxiety in patients undergoing esophagectomy.Age(P=0.026),living status(P=0.005),per capita monthly household income(P=0.024),educational level(P=0.002),and postoperative complications(P<0.001)were all independently related to the aggravation of depression in such patients.CONCLUSION This study indicated a significant negative correlation among anxiety, depression, and mindfulness levels in patients undergoing esophagectomy.Sex,age, per capita monthly household income, and educational level were risk factors for anxiety in patients undergoing esophagectomy, whereas age, living status, per capita monthly household income, educational level, and postoperative complications were risk factors for depression. 展开更多

Objective:To analyze the related factors affecting the professional identity of nursing students before and after clinical practice, hoping to provide reference for the career development and education of nursing students. Methods:A total of 393 undergraduate nursing students of grade 2020 from the Department of Nursing of a university in Shanxi Province were selected. The professional choice motivation, educational attainment expectation, future career planning, professional practice, and employment prospect expectation of nursing students before clinical practice, and the willingness to upgrade educational attainment, professional experience, career development expectation, and employment planning of nursing students after clinical practice were investigated.Through statistical analysis of the survey data, the related influencing factors affecting the professional identity of nursing students before and after clinical practice were explored. Results:The observation of general data showed that the proportion of female students in the survey sample was higher than that of male students, and the proportion of those whose family residence was in rural areas was slightly higher than that in urban areas. The analysis of students’own situation showed that nursing students chose the nursing major because of the high expected employment rate, but they were generally anxious about the employment prospect before the internship, showing a contradictory mentality. The analysis of students’professional ability showed that nursing students believed that their professional ability was mainly reflected in the vocational adaptability. The analysis of the social environment showed that the educational attainment expectations of nursing students were mainly undergraduate and master’s degrees, and they preferred to engage in clinical nursing work rather than nursing research. The analysis on the selection of internship hospitals shows that nursing students’choice of internship hospitals is mainly influenced by the duration of the internship, and they consider the correlation between the selection of internship hospitals and future employment decisions to be low.The analysis on the surrounding environment such as family and school shows that the professional identity of nursing students is mainly influenced by the opinions of school teachers and parents, but the occupations of most of the family members of nursing students are not related to medicine. Conclusion:Targeted intervention strategies should be formulated based on the relevant influencing factors that affect the professional identity of nursing students before and after clinical internship,to enhance the professional identity and employment confidence of nursing students, and promote the healthy and sustainable development of nursing education and the medical service industry. 展开更多
