BACKGROUND Due to the increasing rate of thyroid nodules diagnosis,and the desire to avoid the unsightly cervical scar,remote thyroidectomies were invented and are increasingly performed.Transoral endoscopic thyroidec...BACKGROUND Due to the increasing rate of thyroid nodules diagnosis,and the desire to avoid the unsightly cervical scar,remote thyroidectomies were invented and are increasingly performed.Transoral endoscopic thyroidectomy vestibular approach and trans-areolar approaches(TAA)are the two most commonly used remote approaches.No previous meta-analysis has compared postoperative infections and swallowing difficulties among the two procedures.AIM To compared the same among patients undergoing lobectomy for unilateral thyroid carcinoma/benign thyroid nodule.METHODS We searched PubMed MEDLINE,Google Scholar,and Cochrane Library from the date of the first published article up to August 2025.The term used were transoral thyroidectomy vestibular approach,trans areolar thyroidectomy,scarless thyroidectomy,remote thyroidectomy,infections,postoperative,inflammation,dysphagia,and swallowing difficulties.We identified 130 studies,of them,30 full texts were screened and only six studies were included in the final meta-analysis.RESULTS Postoperative infections were not different between the two approaches,odd ratio=1.33,95%confidence interval:0.50-3.53,theχ2 was 1.92 and the P-value for overall effect of 0.57.Similarly,transient swallowing difficulty was not different between the two forms of surgery,with odd ratio=0.91,95%confidence interval:0.35-2.40;theχ2 was 1.32,and the P-value for overall effect of 0.85.CONCLUSION No significant statistical differences were evident between trans-oral endoscopic Mirghani H.Infections and swallowing difficulty in scarless thyroidectomy WJCC https://www.wjgnet.com 2 January 6,2026 Volume 14 Issue 1 thyroidectomy vestibular approach and trans-areolar approach regarding postoperative infection and transient swallowing difficulties.Further longer randomized trials are needed.展开更多
Objective:To investigate subjective and objective listening abilities in noise in young adults,older adults with normal hearing,and adults with listening difficulties(LiD).Methods:This study examined 20 young adults,2...Objective:To investigate subjective and objective listening abilities in noise in young adults,older adults with normal hearing,and adults with listening difficulties(LiD).Methods:This study examined 20 young adults,20 older adults with normal hearing,and 20 adults with LiD.All participants underwent pure-tone audiometry and the Hearing in Noise Test-Japanese version(HINT-J)and completed the Speech,Spatial and Qualities of Hearing Scale(SSQ).Results:All groups had normal hearing thresholds(125-8,000 Hz).Older adults with normal hearing performed worse on the HINT-J than young adults,while adults with LiD reported greater subjective LiD on the SSQ.No significant correlations were found between HINTJ and SSQ scores with any group.Significant differences were found between groups in terms of both the HINT-J(χ^(2)(2)=17.9,p<0.01)and SSQ(χ^(2)(2)=38.7,p<0.01).Conclusion:Despite normal audiometric thresholds,older adults with normal hearing and adults with LiD experience distinct LiD in noisy environments,highlighting the need for comprehensive assessment beyond traditional audiometry.Future research should focus on developing more sensitive diagnostic tools.展开更多
Objectives:One of the most notable challenges in endoscopic procedures is maintaining correct orientation.Mental rotation exercise(MRE)has been suggested as a potential aid for improving orientation.However,there is a...Objectives:One of the most notable challenges in endoscopic procedures is maintaining correct orientation.Mental rotation exercise(MRE)has been suggested as a potential aid for improving orientation.However,there is a lack of research on designing MREs with varying difficultylevels for training purposes.Furthermore,few studies provide solid evidence linking MRE difficultylevels with cognitive load measurements.This study aims to address this gap by investigating the correlation between the MRE difficultylevels and participants’cognitive load,as measured by pupil dilation.Method:We recruited 33 participants to perform MREs on a computer equipped with a screen-mounted eye-tracker.The test consisted of 15 MREs,with the first10 relatively easy(traditional cube)and the next 5 more complex(invented molecule).The participants’eye movements during MREs were recorded.The participants’MRE scores and pupil dilation were obtained and compared between two MRE difficultylevels.Results:The participants who performed traditional cube MREs achieved significantlybetter MRE scores(0.77±0.11 vs.0.58±0.03,p<0.001)and lower pupil dilation(0.27±0.04 pixels vs.0.47±0.09 pixels,p<0.001)than did those who performed the invented molecule MREs.Moreover,there were significant negative correlations(r=0.62,p=0.015)between pupil dilation and MRE scores.Conclusions:The results revealed a significantnegative correlation between MRE scores and pupil dilation.The more challenging MRE questions led to worse MRE scores but increased pupil dilation.The MRE difficultylevels can be evaluated not only by the degrees or dimensions with which the objects were rotated but also by the participants’MRE scores and pupil dilation.The results of this study provide a basis for training orientation skills in endoscopy using MREs.By incorporating MREs with varying difficultylevels,customized training programs can be developed to enhance camera navigation in endoscopic and laparoscopic procedures.展开更多
This study aimed to determine the reliability,validity and measurement invariance of scores from the Difficulties in Emotion Regulation Scale-8 in Chinese context.A total of 1114 Chinese adolescents were participants ...This study aimed to determine the reliability,validity and measurement invariance of scores from the Difficulties in Emotion Regulation Scale-8 in Chinese context.A total of 1114 Chinese adolescents were participants in three phases:N=424 for the initial DERS-8 measure completion;N=586 the DERS-8,General Anxiety Disorder Scale,Depression Scale and Emotion Regulation Scale completion,with an interval of one month.Then an additional 104 adolescents also completed DERS-8,General Anxiety Disorder Scale,Depression Scale and Emotion Regulation Scale.Both exploratory and confirmatory factor analyses confirmed the one-factor model of the scale,and the fitness indicators wereχ^(2)/df=4.05,RMSEA=0.07,CFI=0.98,and TLI=0.97.Each item of the DERS-8 had good discrimination.The internal consistency reliability coefficient,split-half reliability coefficient and test-retest reliability coefficient of the scale scores were 0.90,0.87 and 0.66,respectively.The findings suggest the Chinese version of the DERS-8 is a reliable measure of difficulty of emotion regulation in Chinese adolescents.展开更多
This study investigated the differences between children with and without learning difficulties(LD)regarding academic performance(AP)and sleep quality(SLQ).A total of 1,501 children were evaluated,including 810 with L...This study investigated the differences between children with and without learning difficulties(LD)regarding academic performance(AP)and sleep quality(SLQ).A total of 1,501 children were evaluated,including 810 with LD and 691 without LD.The results indicated that children with LD scored significantly lower in AP and had poorer sleep quality,with greater variability in SLQ patterns.Correlation analysis revealed a significant negative association between sleep quality and academic performance(r=-0.646;p<0.01),indicating that poorer sleep quality is related to lower academic performance.These findings underscore the importance of interventions focused on promoting sleep hygiene and consistent routines to mitigate the negative impacts of learning difficulties.Study limitations include the absence of mediating variables,such as screen time and mental health,suggesting the need for future research exploring longitudinal interventions and associated factors.展开更多

The purpose of this research is to analyze the causal mechanisms of learning difficulties of middle school students and use them to propose strategies to help them.This research is particularly valuable for its focus ...The purpose of this research is to analyze the causal mechanisms of learning difficulties of middle school students and use them to propose strategies to help them.This research is particularly valuable for its focus on middle school students.Research on this critical transition period is often lacking compared to primary and high school.Therefore,this research establishes a structured equation model and analyzes the data from the survey using the partial least squares method.The data were obtained from a 13,900 Wenzhou City,China students’questionnaire.The research found that learning strategies were the most significant influences on learning effectiveness,followed by learning motivation and learning relationships.Meanwhile,learning relationships had a significant impact on learning pressure.Therefore,this research proposes targeted support strategies.It aims to enhance learning motivation(Set achievable learning goals for each student with learning difficulties based on their actual situation),optimize learning strategies(Encourage students with learning difficulties to learn self-regulatory strategies such as goal setting,time management,and self-reflection),and improve learning relationships(Establish a good social network to promote positive interaction between students with learning difficulties and their peers).At the same time,it reduces students’learning pressure.Ultimately,the learning effectiveness of students with learning difficulties is improved.展开更多

文摘目的探讨脂代谢指标与噪声下言语识别困难之间的因果关系,并评估其在听力损失早期阶段的作用。方法基于孟德尔随机化(Mendelian randomization,MR)设计,利用英国生物银行和全球脂质遗传学联盟两个大型全基因组关联分析数据库的数据,评估高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇、甘油三酯等脂代谢指标与噪声下言语识别困难之间的因果关联。采用逆方差加权方法作为主要分析方法,并通过多变量MR进一步验证脂代谢指标的独立贡献。结果两样本MR分析表明,HDL-C与噪声下言语识别困难具有显著的保护性因果关系(英国生物银行数据集:OR=0.993,95%CI:0.987~0.999,P=0.014),这一关联在多变量分析中保持稳健(OR=0.992,95%CI:0.984~1.000,P=0.045)。相比之下,其他脂代谢指标未显示出显著的独立因果效应。此外,全球脂质遗传学联盟数据集中HDL-C的保护作用虽然显示出类似趋势,但未达到统计学显著性(OR=0.994,95%CI:0.987~1.000,P=0.054),多变量MR结果(OR=0.992,95%CI:0.983~1.000,P=0.064)。结论HDL-C在噪声下言语识别困难中具有保护作用,HDL-C水平的改善可能通过抑制炎症、抗氧化和改善微循环等机制对听力健康产生积极影响。
