Background Obesity is a risk factor for developing cardiometabolic disease.Exercise training is pivotal in the treatment of obesity and is associated with reduced cardiovascular mortality.This study examined the effec...Background Obesity is a risk factor for developing cardiometabolic disease.Exercise training is pivotal in the treatment of obesity and is associated with reduced cardiovascular mortality.This study examined the effect of high-fat feeding on cardiac morphology and mitochondrial function,alongside the mitigating effects of voluntary exercise training.Methods Six-week-old male C57Bl/6 J mice commenced a high fat diet(HFD)or chow diet and were randomized to receive locked(sedentary)or unlocked(voluntary exercise training(VET))running wheels at 10 weeks of age.Mice were monitored until 30 weeks of age and euthanized for collection of tissues.Magnetic resonance imaging was performed to assess body composition,and echocardiography was performed to assess cardiac function.Results Compared with chow-fed animals,the HFD increased body weight and adiposity and decreased cardiolipins(CL)in the heart,which are required for maintaining adequate mitochondrial respiration.Importantly,VET reversed these effects and induced physiological cardiac hypertrophy.Cardiac mitochondrial respiratory chain analysis revealed decreased complexes II and IV activity following high fat feeding,while VET enhanced complex I activity,emphasizing the cardioprotective effect of exercise training in obesity.Conclusion This study uncovers mechanisms by which obesity and exercise impact cardiac mitochondrial health and suggests the mitochondria is a therapeutic target in obesity-related cardiovascular diseases.展开更多
BACKGROUND Type 2 diabetes mellitus(T2DM),a chronic metabolic disease with a high global incidence,has become a serious public health challenge.China has the largest number of T2DM patients worldwide,imposing a signif...BACKGROUND Type 2 diabetes mellitus(T2DM),a chronic metabolic disease with a high global incidence,has become a serious public health challenge.China has the largest number of T2DM patients worldwide,imposing a significant economic burden on the healthcare system.T2DM is closely associated with insulin resistance,impaired pancreatic B cell function,and disordered glucose and lipid metabolism,which can lead to various complications,reducing patients'quality of life and increasing the risk of disability and death.Thus,finding effective preventive and intervention measures is crucial.Exercise therapy,a key part of diabetes management,has gained attention in recent years,with many studies indicating its benefits for blood glucose control and other aspects in diabetic patients.AIM To assess the effectiveness of combined resistance and aerobic exercise interventions on blood glucose control and metabolic indicators in patients with T2DM and to explore their application in diabetes management.METHODS Systematic searches were conducted using PubMed,EMBASE,Cochrane Library,and Chinese databases for relevant randomized controlled trials(RCTs).The inclusion criteria were participants aged≥18 years with T2DM and the intervention involved combined resistance and aerobic exercise for≥8 weeks.The primary outcome indicators were fasting blood glucose,low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),triglycerides(TG),glycated hemoglobin A1c(HbA1c),and total cholesterol(TC)levels.Data analysis was performed using RevMan software,and the interventional effects were assessed using weighted mean differences or standardized mean differences(SMD).RESULTS Six RCTs meeting the inclusion criteria were included,with a total sample size of 366 participants.The meta-analysis results showed that combined resistance and aerobic exercise significantly improved several metabolic indicators in patients with T2DM.Specific results were as follows:(1)For fasting blood glucose,combined exercise was more effective than aerobic exercise alone[SMD=1.22;95%confidence interval(95%CI):0.70,1.74;P<0.00001];(2)LDL-C levels were significantly reduced by the combined intervention(SMD=1.45;95%CI:1.18-1.72;P<0.00001);(3)The combined intervention significantly increased HDL-C levels(SMD=1.42;95%CI:0.98-1.87;P<0.00001);(4)The combined intervention significantly reduced TG levels(SMD=1.12;95%CI:0.85-1.39;P<0.00001;(5)No statistically significant difference was observed in HbA1c between the combined and the aerobic exercise group(SMD=-0.03;95%CI:-1.09 to 1.04;P<0.00001);and(6)The combined exercise intervention group significantly reduced TC levels(SMD=2.66;95%CI:1.93-3.38;P<0.00001).The subgroup analysis results suggest that the effect of exercise interventions may be influenced by various factors,including the patient's age,baseline blood glucose levels,and exercise intensity.CONCLUSION Combined resistance and aerobic exercise intervention significantly improved fasting blood glucose,LDL-C,HDL-C,TG,and TC levels in patients with T2DM,especially in terms of blood glucose control and cardiovascular risk,demonstrating better outcomes than aerobic exercise alone.展开更多
Objectives:Aerobic exercise training can increase skeletal muscle mitochondrial content.Supine exercise training with legs above the heart potentially augments these increases.However,the impact of supine exercise tra...Objectives:Aerobic exercise training can increase skeletal muscle mitochondrial content.Supine exercise training with legs above the heart potentially augments these increases.However,the impact of supine exercise training on mitochondrial biogenesis and cardiovascular adaptations remains unclear.Methods:In this single-centred,randomized,parallel arm trial,19 recreationally active individuals underwent seven sessions of either supine with legs up(SUP;n=9,6 females)or upright with legs down(UP;n=10,7 females)aerobic training on a recumbent bike at 71±7%and 71±2%of peak work rate(WRpeak),respectively.The study aimed to test the effects of training with decreased muscle oxygenation on indices of muscle mitochondrial remodelling.Secondary outcomes included exercise performance,muscle oxygenation,and cardiovascular responses.Results:Secondary outcomes revealed significant interaction effects for time to fatigue(TTF)and WRpeak in the SUP group during supine testing,suggesting enhanced exercise tolerance and performance.No between group interaction effects were observed for upright testing.No clear effects on mitochondrial biogenesis were observed based on expression of mitochondrial protein subunits and transcriptional regulators.Acutely,HRpeak was lower during the SUP Test compared to the UP Test.No central cardiovascular adaptations were observed following training.Conclusions:Our exploratory analyses showed that supine aerobic training more effectively improves supine exercise tolerance and performance compared with upright training,despite no differences in measured proteins related to mitochondrial biogenesis.Further research is needed to elucidate the mechanisms underlying these postural-specific training effects.Registration:clinicaltrials.gov:NCT04151095.展开更多
Background:This study investigates the effects of exercise training on exerkines in patients with type 2 diabetes mellitus to determine the optimal exercise prescription.Methods:A systematic search for relevant studie...Background:This study investigates the effects of exercise training on exerkines in patients with type 2 diabetes mellitus to determine the optimal exercise prescription.Methods:A systematic search for relevant studies was performed in 3 databases.Randomized controlled trials investigating the effects of exercise training on at least one of the following exerkines were included:adiponectin,apelin,brain-derived neurotrophic factor,fetuin-A,fibroblast growth factor-21,follistatin,ghrelin,interleukin(IL)-6,IL-8,IL-10,IL-15,IL-18,leptin,myostatin,omentin,resistin,retinol-binding protein 4,tumor necrosis factor-α,and visfatin.Results:Forty randomized controlled trials were selected for data extraction(n=2160).Exercise training induces changes in adiponectin,fetuin-A,fibroblast growth factor-21,IL-6,IL-10,leptin,resistin,and tumor necrosis factor-a levels but has no significant effects on apelin,IL-18,and ghrelin compared to controls.Physical exercise training favored large and positive changes in pooled exerkines(i.e.,an overall effect size calculated from several exerkine s)(Hedge’s g=1.02,95%confidence interval(95%CI):0.76-1.28),which in turn were related to changes in glycated hemoglobin(mean difference(MD)=-0.81%,95%CI:-0.95%to-0.67%),fasting glucose(MD=-23.43 mg/dL,95%CI:-30.07 mg/dL to-16.80 mg/dL),waist circumference(MD=-3.04 cm,95%CI:-4.02 cm to-2.07 cm),and body mass(MD=-1.93 kg,95%CI:-2.00 kg to-1.86 kg).Slightly stronger effects were observed with aerobic,resistance,or high-intensity interval protocols at moderate-to vigorous-intensity and with programs longer than 24 weeks that comprise at least 3 sessions per week and more than 60 min per session.Conclusion:Exercise training represents an anti-inflammatory therapy and metabolism-improving strategy with minimal side effects for patients with type 2 diabetes mellitus.展开更多

Background:The inability of damaged neurons to regenerate and of axons to estab-lish new functional connections leads to permanent functional deficits after spinal cord injury(SCI).Although astrocyte reprogramming hol...Background:The inability of damaged neurons to regenerate and of axons to estab-lish new functional connections leads to permanent functional deficits after spinal cord injury(SCI).Although astrocyte reprogramming holds promise for neurorepair in various disease models,it is not sufficient on its own to achieve significant functional recovery.Methods:A rat SCI model was established using a spinal cord impactor.Seven days postsurgery,adeno-associated virus were injected to overexpress the transcription factors NeuroD1 and Neurogenin-2(Ngn2)in the spinal cord.The rats were then trained to walk on a weight-supported treadmill for 4 weeks,starting 14 days after modeling.The effects of these interventions on motor and sensory functions,as well as spinal cord tissue repair,were subsequently evaluated.Results:The combination of NeuroD1 and Ngn2 overexpression with weight-supported exercise training significantly improved gait compared to either inter-vention alone.The group receiving the combined intervention exhibited enhanced sensitivity in sensory assessments.Immunofluorescence analysis revealed increased colocalization of astrocytes and microtubule-associated protein 2-positive neurons in the injury area.These effects were more pronounced than those observed with spinal cord tissue repair alone.Additionally,the combined intervention significantly reduced glial scarring and the size of the injury area.Conclusion:Exercise intervention enhances the reprogramming effects of astrocytes and restores motor function,yielding better results than either intervention alone.展开更多
Background:Fat loss mainly conveys the benefits of caloric restriction for people living with type 2 diabetes.The literature is equivocal regarding whether exercise facilitates fat loss during caloric restriction.This...Background:Fat loss mainly conveys the benefits of caloric restriction for people living with type 2 diabetes.The literature is equivocal regarding whether exercise facilitates fat loss during caloric restriction.This analysis aimed to assess the dose-response effects of exercise in combination with a caloric restriction on fat mass(FM)and FM percentage(FM%)in persons with diagnosed type 2 diabetes.Methods:In this secondary analysis of a 4-armed randomized trial,82 persons living with type 2 diabetes were randomly allocated to the control group(CON)(n=21),diet control(DCON)(25%caloric restriction;n=20),diet control and exercise 3 times per week(MED)(n=20),or diet control and exercise 6 times per week(HED)(n=21)for 16 weeks.The primary analysis was the change in FM%points.Secondary analyses included fat-free mass and visceral adipose tissue(VAT)volume(cm^(3)).Results:FM%decreased compared to CON by a mean difference of-3.5%(95%confidence interval(95%CI):-5.6%to-1.4%),-6.3%(95%CI:-8.4%to-4.1%),and-8.0%(95%CI:-10.2%to-5.8%)for DCON,MED,and HED,respectively.Compared to DCON,MED,and HED decreased FM%by-2.8%(95%CI:-4.9%to-0.7%)and-4.5%(95%CI:-6.6%to-2.4%),respectively.The difference in FM%between HED and MED was-1.8%(95%CI:-3.9%to 0.4%).DCON and MED decreased fat-free mass compared to CON,whereas HED preserved fat-free mass(-0.2%;95%CI:-2.0%to 1.7%).Compared to CON,VAT volume decreased by-666.0 cm^(3)(95%CI:-912.8 cm^(3) to-385.1 cm^(3)),-1264.0 cm^(3)(95%CI:-1679.6 cm^(3) to-655.9 cm^(3)),and-1786.4 cm^(3)(95%CI:-2264.6 cm^(3) to-1321.2 cm^(3))more for DCON,MED,and HED,respectively.HED decreased VAT volume more than DCON(-1120.4 cm^(3);95%CI:-1746.6 cm^(3) to-639.4 cm^(3))while the remaining comparisons did not reveal any differences.Conclusion:All interventions were superior in reducing FM%compared to standard care.Adding exercise to a caloric restriction was superior in reducing FM%compared to a caloric restriction alone.展开更多
Objective:To explore the effect of narrative nursing combined with exercise training on health beliefs and cardiac rehabilitation in patients with acute myocardial infarction(AMI)after interventional treatment.Methods...Objective:To explore the effect of narrative nursing combined with exercise training on health beliefs and cardiac rehabilitation in patients with acute myocardial infarction(AMI)after interventional treatment.Methods:A total of 111 patients with acute myocardial infarction(AMI)who received interventional therapy in Department of Cardiology of Hefei First People’s Hospital from January 2022 to September 2023 were selected as the study subjects.They were randomly divided into a control group(n=55)and a study group(n=56)using a random table method.Both groups received routine nursing care,while the control group received exercise training on top of it.The study group received narrative nursing care on top of the control group.Both groups were intervened until the patients were discharged.The health beliefs,heart function,psychological status,and quality of life after intervention were compared between two groups using a 6-minute walk test(6MWT).Results:After intervention,the scores of the TSK-SV Heart in all dimensions(fear of movement,functional disorders,risk perception,and avoidance of movement)decreased in both groups,and the study group was lower than the control group,with a statistical significant difference(P<0.05).After intervention,the scores of the Self Rating Anxiety Scale(SAS)and Self Rating Depression Scale(SDS)in both groups decreased,and the study group was lower than the control group,with a statistical significant difference(P<0.05).After intervention,both groups showed an increase in left ventricular ejection fraction(LVEF)and left ventricular early diastolic peak flow velocity/left ventricular late diastolic peak flow velocity(E/A),with the study group being higher than the control group.The left ventricular end diastolic diameter(LVEDD)and left atrial volume index(LAVI)decreased,and the study group was lower than the control group,with statistical significant differences(P<0.05).The walking distance of the study group at 6MWT(488.8±31.4)m was greater than that of the control group(425.54±30.7)m,with statistical significant differences(P<0.05).The quality of life measurement scale(CROQ-PTCA-Post)of the study group after coronary intervention treatment had higher scores in all dimensions(physical function,satisfaction,psychosocial function,adverse reactions,symptoms,cognitive function)than the control group,with statistical significant differences(P<0.05).Conclusion:Narrative nursing combined with exercise training can enhance the health beliefs of AMI patients towards exercise training after intervention therapy,which is beneficial for cardiac rehabilitation and can improve psychological status and quality of life.展开更多

BACKGROUND Diabetes is characterized by insulin resistance as well as impaired insulin production,withβ-cell dysfunction playing a critical role in disease progression.Exercise is known to improve insulin sensitivity...BACKGROUND Diabetes is characterized by insulin resistance as well as impaired insulin production,withβ-cell dysfunction playing a critical role in disease progression.Exercise is known to improve insulin sensitivity,but its effects on pancreatic islet quality and function remain poorly understood.This work hypothesized that swimming training enhances glycemic control and insulin secretion by upregulating the insulin-like growth factor 1(IGF-1)/phosphatidylinositol 3-kinase/protein kinase B(PI3K/AKT)pathway in streptozotocin(STZ)-induced diabetic rats.AIM To investigate the effects of swimming on pancreatic islet quality and function in STZ-induced diabetic rats via the IGF-1/PI3K/AKT pathway.METHODS Twenty-six Sprague-Dawley rats were grouped into diabetic and control groups,with each group further split into exercise and sedentary subgroups.Diabetic rats were induced with STZ.The exercise groups underwent swimming training for 60 minutes/day,5 days/week,for 8 weeks.Body weight,food intake,blood glucose,insulin,lipids,and muscle glycogen were measured.Pancreatic islet morphology and the protein expression levels of IGF-1,PI3K,and AKT were analyzed.Data were analyzed using two-way repeated-measure ANOVA,followed by Tukey’s post-hoc test.RESULTS Exercise training significantly improved body weight[diabetic exercise group(D-Ex):390.66±50.14 g vs diabetic sedentary group(D-Sed):315.89±50.12 g,P<0.05],reduced blood glucose(D-Ex:12.21±4.43 mmol/L vs D-Sed:17.79±2.05 mmol/L,P<0.05),and increased insulin levels(D-Ex:53.50±15.31 pmol/L vs D-Sed:25.31±10.23 pmol/L,P<0.05)in diabetic rats.It also enhanced islet morphology,increased IGF-1 expression,and activated the PI3K/AKT pathway(P<0.05).In-vitro experiments confirmed that IGF-1 positively regulated insulin expression and inhibitedβ-cell apoptosis via the PI3K/AKT pathway.CONCLUSION Exercise training improves pancreatic islet quality and function in diabetic rats by modulating the IGF-1/PI3K/AKT pathway,highlighting its therapeutic potential for diabetes management.展开更多

Objective: This study was conducted to examine the effects of aerobic exercise alone and aerobic exercise with resistance training on the quality of life in men over the age of 55 years with type 2 diabetes mellitus. ...Objective: This study was conducted to examine the effects of aerobic exercise alone and aerobic exercise with resistance training on the quality of life in men over the age of 55 years with type 2 diabetes mellitus. Methods: A total of 54 participants were divided into the following three groups so that there were no significant differences in blood chemistry or physical ability indexes among the three groups: control, aerobic exercise, and aerobic exercise with resistance training. The latter two groups exercised for 24 weeks, while the control group performed no exercise. Blood chemistry levels and measures of physical ability in each group members were examined one day before and one day after the exercise regimens. Results: Compared with those before the study, blood glucose, glycated hemoglobin, triglycerides, cholesterol, and low-density lipoprotein levels as well as vital capacity, reaction time, sit-and-reach ability, and balancing while standing on one leg with closed eyes were significantly improved in the aerobic exercise only group(P < 0.05). All these measures as well as high-density lipoprotein levels and grip, back, and leg strength were significantly improved in the combined aerobic and resistance training group(P < 0.05). By contrast, no significant differences before and after the experiment were found in any measure for the control group(P > 0.05). Conclusion: Although both aerobic exercise and aerobic exercise combined with resistance training for 24 weeks effectively improved the quality of life in patients with type 2 diabetes, the effect of the combined training was better than that of aerobic exercise alone. These results suggest that resistance training may be safely added to the rehabilitation training regimen of patients with type 2 diabetes mellitus.展开更多
Type 1 diabetes mellitus(T1DM)is an autoimmune condition whose prevalence is prominent in children and adolescents,resulting in insulin deficiency with a potential for long-term complications induced by glucotoxicity....Type 1 diabetes mellitus(T1DM)is an autoimmune condition whose prevalence is prominent in children and adolescents,resulting in insulin deficiency with a potential for long-term complications induced by glucotoxicity.As an autoimmune disease where the body's immune system attack insulin-producing beta cells in the pancreas,leading up to complete or near-complete inability to control the blood glucose levels independently.The lack of glycemic control calls for lifelong exogenous insulin administration in conjunction with careful monitoring to control blood sugar levels and prevent acute and chronic health issues complications.Regular physical activity,notably resistance exercise(RE),may be beneficial in the glycemic management of this population,enhancement of muscle strength,and general health for the growing,development and maturation in children.The evidence depicting its benefits and safeguard for RE in pediatric T1DM patients remains underexamined.This mini-review seeks to synthesize qualitatively the current evidence on RE regarding its global effects on the T1DM in children.A search for peer-reviewed papers is carried out through primary databases,centering on publications that examined the physiological,metabolic,and psychosocial consequences of RE in children with T1DM.Emerging evidence indicates that RE is one potential method of safe and efficacious intervention to improve glycemic management,physical capacity,and quality of life.However,there is still some reluctance to this type of training in the pediatric population.The available research has not only refuted the belief that strength training was contraindicated in the pediatric population but also recommends its systematic practice to enjoy its benefits on the three spheres of health.Nevertheless,methodological differences and small population studies pose challenges to drawing firm conclusions.The review underscores other areas,including the need for standardizing protocols for including patients such as follow-ups and greater considerations for psychosocial effects of RE in this population.This minireview underlines the importance of RE in a global approach to pediatric diabetes care by providing practical insight for both clinicians and researchers.展开更多
The aim of this present study was to investigate the effects of training on exercise tolerance of patients with coronary heart disease after percutaneous coronary intervention.Fifty-seven cases of coronary heart disea...The aim of this present study was to investigate the effects of training on exercise tolerance of patients with coronary heart disease after percutaneous coronary intervention.Fifty-seven cases of coronary heart disease after percutaneous coronary intervention were divided randomly into the rehabilitation training group(26 cases) and control group(31 cases).Patients in the rehabilitation training group received rehabilitation training at different stages and exercise intensities 3 d after percutaneous coronary intervention for 3 months.The heart rate,blood pressure,ECG changes in treadmill exercise test,and the frequency of anginal episodes were observed.The results showed that NST and ΣST of ECG and the frequency of anginal episodes were significantly reduced in the rehabilitation training group.In addition,exercise tolerance was improved and the total exercise time was lengthened in these patients.Moreover,ST segment depression time and emergence time of angina with exercise were also lengthened compared with controls(P 〈 0.05,or 0.01).However,the heart rate and blood pressure before and after exercise of the two groups were similar.The study indicated that rehabilitation training could significantly relieve angina,amend ischemic features of ECG,and improve exercise tolerance of coronary heart disease patients after percutaneous coronary intervention.展开更多

The markers related to neurotoxicity were measured by enzyme-linked immunosorbent assay method. 8 weeks of treadmill exercise significantly increased brain-derived neurotrophic factor level in the hippocampus (P = 0.04) and plasma level of total antioxidant capacity of rats exposed to lead acetate (P 〈 0.001), and significantly decreased plasma level of malondialdehyde (P 〈 0.001). Acute exercise only decreased the hippocampal malondialdehyde level (P = 0.09) and increased brain-derived neurotrophic factor level in the hippocampus (P = 0.66). Acute exercise also enhanced the total antioxidant capacity in rats exposed to lead acetate, insignificantly (P = 0.99), These findings suggest that chronic treadmill exercise can significantly decrease neurotoxicity and alleviate oxidative stress in rats exposed to lead acetate. Background Heat shock proteins(HSPs)are key molecular chaperones that help maintain protein homeostasis by stabilizing or removing damaged proteins during cellular stress.Aging weakens these stress–response systems,d...Background Heat shock proteins(HSPs)are key molecular chaperones that help maintain protein homeostasis by stabilizing or removing damaged proteins during cellular stress.Aging weakens these stress–response systems,disrupting proteostasis and increasing vulnerability to sarcopenia.High-intensity training(HIT)can counteract these declines by activating protective pathways such as the HSP response.HSPs are highly responsive to stress,examining their regulation during aging is important,as altered HSP activity is linked to the progressive loss of muscle mass.Methods This study investigated the abundance and phosphorylation of HSPs in skeletal muscle from healthy,active young and older adults(n=7 per group),assessed at baseline and again in the older group following 12 weeks of HIT.Using calibrated Western blotting on both whole-muscle homogenates and pooled single muscle fibres,we quantified HSP content and phosphorylation to determine how aging and exercise influence stress–responsive protein regulation at both the tissue and cellular levels.Results In whole muscle homogenates,HSPs(HSP72,HSP27,andαB-crystallin)did not differ between young and older adults,while higher phosphorylation of small HSPs(sHSPs):phospho-HSP27 at Serine15(pHSP27 Ser15)and phospho-αB-crystallin at Serine59(pαB-crystallin Ser59)(∼1.8-fold and∼2.9-fold,respectively)were found in muscle from older adults,indicating higher cellular stress associated with aging.A 12-week HIT intervention in older adults reduced homogenate pHSP27 Ser15 and pαB-crystallin Ser59 abundances to similar levels found in young adults.Total HSPs typically displayed a distinct fiber-type profile in both age groups,with more in type I compared to type II fibers,distinguished by the presence of myosin heavy chain I(MHCI)or MHCII.Phosphorylation at pHSP27 Ser15 and pαB-crystallin Ser59 was not different between type I and type II fibers.The HIT in older adults decreased total and phosphorylated sHSPs in both type I and type II fibers but increased HSP72 in type I fibers.Conclusion HIT has the potential to mitigate age-related cellular stress and modulate protein expression patterns in aging skeletal muscle and,perhaps,has the potential to delay age-related muscle decline,thereby improving muscle health in older adults.展开更多

Regular exercise can prevent or delay type 2 diabetes in those of high-risk populations. Although there are great benefits of regular exercise, unfortunately majority of people with type 2 diabetes are not active. <strong>Objectives:</strong> The main goal of this study was to assess patients adherence to regular exercise among type 2 diabetes Mellitus (T2DM) attending Ribat University Hospital Diabetic Clinic (RUHDC) Khartoum state, Sudan. <strong>Methods:</strong> Analytical cross sectional hospital based study was carried out among 351 patients from August 2012 to February 2013. Structured questionnaire and patients cards were used to collect data. <strong>Results:</strong> Out of 351 patients, female were 65.8% and male 34.2%. Patients older than 60 years 48.4%, illiterate 21.0%, only 44.2% had basic educational level, house wife 56.1% and retired 18.5%. Majority had irregular exercise program (80.1%), 12.1% specified lack of time whilst 87.9% did not specify any other reasons as barrier for regular exercise “no reason”. Daily activity was the main type of physical activity (59.8%). Although insignificant, irregular exercise was higher among female patients, their age above 60 years, patients had low education levels and those had financial problems. <strong>Conclusion:</strong> Regular exercise was low in this study. Lack of time, female, patients older than 60 years, had low education levels, and had financial problems, which were the main barriers of regular exercise. Family support, tailored patients’ education and reduction of patients’ use of technology and modern living can improve the patients’ activity.</span> </div>展开更多
Background Stable angina pectoris is a common subtype of coronary heart disease.Patients suffer from chest tightness,chest pain and crushing pain under the inducement of fatigue and emotional agitation.This study aims...Background Stable angina pectoris is a common subtype of coronary heart disease.Patients suffer from chest tightness,chest pain and crushing pain under the inducement of fatigue and emotional agitation.This study aims to investigate the effect of exercise training guidance based on action research on exercise endurance and readmission rate of patients with stable angina pectoris.Methods A retrospective study was conducted on 60 patients with stable angina pectoris due to coronary heart disease admitted to our hospital from February 2020 to November 2023.Patients were divided into a control group of 29 cases(receiving conventional exercise training guidance)and a guidance group of 31 cases(receiving action research-based exercise training guidance).Both groups received continuous training for one month.A comparison was made between the exercise endurance indicators[6-minute walk test distance(6MWT),peak oxygen consumption(peak VO2),anaerobic threshold(AT),exercise duration(ED)],quality of life,and readmission rate within 6 months after intervention in both groups.Results After 1 month of intervention,6MWT,peak VO2,AT and ED were increased in both groups,and those in the guidance group were higher than those in the conventional group(P<0.05).The score of quality of life(The 36-item shot-form health status survey,SF-36)in both groups was increased,and the guidance group scored higher than the conventional group(P<0.05).The readmission rate of patients in the guidance group was 6.45%,which was lower than that in the conventional group(27.59%)(P<0.05).Conclusions The application of action research-based exercise training guidance in patients with stable angina pectoris due to coronary heart disease has significant effects,improving exercise endurance,significantly improving quality of life,and reducing readmission rate,thus having practical value.[S Chin J Cardiol 2024;25(3):162-168]展开更多

This is true even for people with various co...Aerobic capacity, which is expressed as peak oxygen consumption (VO2peak), is well-known to be an independent predictor of all-cause mortality and cardiovascular prognosis. This is true even for people with various coronary risk factors and cardiovascular diseases. Although exercise training is the best method to improve VO2peak, the guidelines of most academic societies recommend 150 or 75 min of moderate- or vigorous- intensity physical activities, respectively, every week to gain health benefits. For general health and primary and secondary cardiovascular prevention, high-intensity interval training (HIIT) has been recognized as an efficient exercise protocol with short exercise sessions. Given the availability of the numerous HIIT protocols, which can be classified into aerobic HIIT and anaerobic HIIT [usually called sprint interval training (SIT)], professionals in health-related fields, including primary physicians and cardiologists, may find it confusing when trying to select an appropriate protocol for their patients. This review describes the classifications of aerobic HIIT and SIT, and their differences in terms of effects, target subjects, adaptability, working mechanisms, and safety. Understanding the HIIT protocols and adopting the correct type for each subject would lead to better improvements in VO2peak with higher adherence and less risk.展开更多
AIM:To evaluate the long-term effect of aerobic and/or anaerobic exercise on glycemic control in youths with type 1 diabetes.METHODS:Literature review was performed in spring and summer 2014 using Pub Med/MEDLINE,Goog...AIM:To evaluate the long-term effect of aerobic and/or anaerobic exercise on glycemic control in youths with type 1 diabetes.METHODS:Literature review was performed in spring and summer 2014 using Pub Med/MEDLINE,Google Scholar,Scopus,and Science Direct with the following terms:aerobic,anaerobic,high-intensity,resistance,exercise/training,combined with glycemic/metabolic control,glycated haemoglobin A1c(Hb A1c) and type 1diabetes.Only peer-reviewed articles in English were included published in the last 15 years.It was selected from 1999 to 2014.Glycemic control was measured with Hb A1 c.Studies with an intervention lasting at least 12 wk were included if the Hb A1 c was measured before and after the intervention.RESULTS:A total of nine articles were found,and they were published between the years of 2002-2011.The sample size was 401 diabetic youths(166 males and 235 females) with an age range of 10-19 years except one study,in which the age range was 13-30 years.Study participants were from Australia,Tunisia,Lithuania,Taiwan,Turkey,Brazilia,Belgium,Egypt and France.Four studies were aerobic-based,four were combined aerobic and anaerobic programs,and one compared aerobic exercise to anaerobic one.Available studies had insufficient evidence that any type of exercise or combined training would clearly improve the glycemic control in type 1 diabetic youth.Only three(two aerobic-based and one combined) studies could provide a significant positive change in glycemic control.CONCLUSION:The regular physical exercise has several other valuable physiological and health benefits that justify the inclusion of exercise in pediatric diabetes treatment and care.展开更多

文摘Background Obesity is a risk factor for developing cardiometabolic disease.Exercise training is pivotal in the treatment of obesity and is associated with reduced cardiovascular mortality.This study examined the effect of high-fat feeding on cardiac morphology and mitochondrial function,alongside the mitigating effects of voluntary exercise training.Methods Six-week-old male C57Bl/6 J mice commenced a high fat diet(HFD)or chow diet and were randomized to receive locked(sedentary)or unlocked(voluntary exercise training(VET))running wheels at 10 weeks of age.Mice were monitored until 30 weeks of age and euthanized for collection of tissues.Magnetic resonance imaging was performed to assess body composition,and echocardiography was performed to assess cardiac function.Results Compared with chow-fed animals,the HFD increased body weight and adiposity and decreased cardiolipins(CL)in the heart,which are required for maintaining adequate mitochondrial respiration.Importantly,VET reversed these effects and induced physiological cardiac hypertrophy.Cardiac mitochondrial respiratory chain analysis revealed decreased complexes II and IV activity following high fat feeding,while VET enhanced complex I activity,emphasizing the cardioprotective effect of exercise training in obesity.Conclusion This study uncovers mechanisms by which obesity and exercise impact cardiac mitochondrial health and suggests the mitochondria is a therapeutic target in obesity-related cardiovascular diseases.

文摘BACKGROUND Type 2 diabetes mellitus(T2DM),a chronic metabolic disease with a high global incidence,has become a serious public health challenge.China has the largest number of T2DM patients worldwide,imposing a significant economic burden on the healthcare system.T2DM is closely associated with insulin resistance,impaired pancreatic B cell function,and disordered glucose and lipid metabolism,which can lead to various complications,reducing patients'quality of life and increasing the risk of disability and death.Thus,finding effective preventive and intervention measures is crucial.Exercise therapy,a key part of diabetes management,has gained attention in recent years,with many studies indicating its benefits for blood glucose control and other aspects in diabetic patients.AIM To assess the effectiveness of combined resistance and aerobic exercise interventions on blood glucose control and metabolic indicators in patients with T2DM and to explore their application in diabetes management.METHODS Systematic searches were conducted using PubMed,EMBASE,Cochrane Library,and Chinese databases for relevant randomized controlled trials(RCTs).The inclusion criteria were participants aged≥18 years with T2DM and the intervention involved combined resistance and aerobic exercise for≥8 weeks.The primary outcome indicators were fasting blood glucose,low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),triglycerides(TG),glycated hemoglobin A1c(HbA1c),and total cholesterol(TC)levels.Data analysis was performed using RevMan software,and the interventional effects were assessed using weighted mean differences or standardized mean differences(SMD).RESULTS Six RCTs meeting the inclusion criteria were included,with a total sample size of 366 participants.The meta-analysis results showed that combined resistance and aerobic exercise significantly improved several metabolic indicators in patients with T2DM.Specific results were as follows:(1)For fasting blood glucose,combined exercise was more effective than aerobic exercise alone[SMD=1.22;95%confidence interval(95%CI):0.70,1.74;P<0.00001];(2)LDL-C levels were significantly reduced by the combined intervention(SMD=1.45;95%CI:1.18-1.72;P<0.00001);(3)The combined intervention significantly increased HDL-C levels(SMD=1.42;95%CI:0.98-1.87;P<0.00001);(4)The combined intervention significantly reduced TG levels(SMD=1.12;95%CI:0.85-1.39;P<0.00001;(5)No statistically significant difference was observed in HbA1c between the combined and the aerobic exercise group(SMD=-0.03;95%CI:-1.09 to 1.04;P<0.00001);and(6)The combined exercise intervention group significantly reduced TC levels(SMD=2.66;95%CI:1.93-3.38;P<0.00001).The subgroup analysis results suggest that the effect of exercise interventions may be influenced by various factors,including the patient's age,baseline blood glucose levels,and exercise intensity.CONCLUSION Combined resistance and aerobic exercise intervention significantly improved fasting blood glucose,LDL-C,HDL-C,TG,and TC levels in patients with T2DM,especially in terms of blood glucose control and cardiovascular risk,demonstrating better outcomes than aerobic exercise alone.

文摘Objectives:Aerobic exercise training can increase skeletal muscle mitochondrial content.Supine exercise training with legs above the heart potentially augments these increases.However,the impact of supine exercise training on mitochondrial biogenesis and cardiovascular adaptations remains unclear.Methods:In this single-centred,randomized,parallel arm trial,19 recreationally active individuals underwent seven sessions of either supine with legs up(SUP;n=9,6 females)or upright with legs down(UP;n=10,7 females)aerobic training on a recumbent bike at 71±7%and 71±2%of peak work rate(WRpeak),respectively.The study aimed to test the effects of training with decreased muscle oxygenation on indices of muscle mitochondrial remodelling.Secondary outcomes included exercise performance,muscle oxygenation,and cardiovascular responses.Results:Secondary outcomes revealed significant interaction effects for time to fatigue(TTF)and WRpeak in the SUP group during supine testing,suggesting enhanced exercise tolerance and performance.No between group interaction effects were observed for upright testing.No clear effects on mitochondrial biogenesis were observed based on expression of mitochondrial protein subunits and transcriptional regulators.Acutely,HRpeak was lower during the SUP Test compared to the UP Test.No central cardiovascular adaptations were observed following training.Conclusions:Our exploratory analyses showed that supine aerobic training more effectively improves supine exercise tolerance and performance compared with upright training,despite no differences in measured proteins related to mitochondrial biogenesis.Further research is needed to elucidate the mechanisms underlying these postural-specific training effects.Registration:clinicaltrials.gov:NCT04151095.
文摘Background:This study investigates the effects of exercise training on exerkines in patients with type 2 diabetes mellitus to determine the optimal exercise prescription.Methods:A systematic search for relevant studies was performed in 3 databases.Randomized controlled trials investigating the effects of exercise training on at least one of the following exerkines were included:adiponectin,apelin,brain-derived neurotrophic factor,fetuin-A,fibroblast growth factor-21,follistatin,ghrelin,interleukin(IL)-6,IL-8,IL-10,IL-15,IL-18,leptin,myostatin,omentin,resistin,retinol-binding protein 4,tumor necrosis factor-α,and visfatin.Results:Forty randomized controlled trials were selected for data extraction(n=2160).Exercise training induces changes in adiponectin,fetuin-A,fibroblast growth factor-21,IL-6,IL-10,leptin,resistin,and tumor necrosis factor-a levels but has no significant effects on apelin,IL-18,and ghrelin compared to controls.Physical exercise training favored large and positive changes in pooled exerkines(i.e.,an overall effect size calculated from several exerkine s)(Hedge’s g=1.02,95%confidence interval(95%CI):0.76-1.28),which in turn were related to changes in glycated hemoglobin(mean difference(MD)=-0.81%,95%CI:-0.95%to-0.67%),fasting glucose(MD=-23.43 mg/dL,95%CI:-30.07 mg/dL to-16.80 mg/dL),waist circumference(MD=-3.04 cm,95%CI:-4.02 cm to-2.07 cm),and body mass(MD=-1.93 kg,95%CI:-2.00 kg to-1.86 kg).Slightly stronger effects were observed with aerobic,resistance,or high-intensity interval protocols at moderate-to vigorous-intensity and with programs longer than 24 weeks that comprise at least 3 sessions per week and more than 60 min per session.Conclusion:Exercise training represents an anti-inflammatory therapy and metabolism-improving strategy with minimal side effects for patients with type 2 diabetes mellitus.
文摘Type 2 diabetes is characterized by a decreased ability of insulin to facilitate glucose uptake into insulin sensitive tissue,i.e.,skeletal muscle.The mechanism behind this is at the moment unresolved.It has been suggested that increased amount of lipids inside the skeletal muscle(intramuscular triglyceride,diacylglycerol and ceramides)will impair insulin action in skeletal muscle,but data are not consistent in the human literature.It has also been hypothesized that the impaired insulin sensitivity is due to a dysfunction in the mitochondria resulting in an impaired ability to oxidize lipids,but the majority of the literature is not supporting this hypothesis.Recently it has been suggested that the production of reactive oxygen species play an essential role in skeletal muscle insulin sensitivity.It is well accepted that physical activity(endurance,strength and high intensity training)improves insulin sensitivity in healthy humans and in patients with type 2 diabetes.Whether patients with type 2 diabetes have the same beneficial effects(same improvement)as control subjects,when it comes to regular physical activity in regard to mitochondrial function,is not established in the literature.This review will focus only on the effect of physical activity on skeletal muscle(mitochondrial function)in patients with type 2 diabetes.
文摘Background:The inability of damaged neurons to regenerate and of axons to estab-lish new functional connections leads to permanent functional deficits after spinal cord injury(SCI).Although astrocyte reprogramming holds promise for neurorepair in various disease models,it is not sufficient on its own to achieve significant functional recovery.Methods:A rat SCI model was established using a spinal cord impactor.Seven days postsurgery,adeno-associated virus were injected to overexpress the transcription factors NeuroD1 and Neurogenin-2(Ngn2)in the spinal cord.The rats were then trained to walk on a weight-supported treadmill for 4 weeks,starting 14 days after modeling.The effects of these interventions on motor and sensory functions,as well as spinal cord tissue repair,were subsequently evaluated.Results:The combination of NeuroD1 and Ngn2 overexpression with weight-supported exercise training significantly improved gait compared to either inter-vention alone.The group receiving the combined intervention exhibited enhanced sensitivity in sensory assessments.Immunofluorescence analysis revealed increased colocalization of astrocytes and microtubule-associated protein 2-positive neurons in the injury area.These effects were more pronounced than those observed with spinal cord tissue repair alone.Additionally,the combined intervention significantly reduced glial scarring and the size of the injury area.Conclusion:Exercise intervention enhances the reprogramming effects of astrocytes and restores motor function,yielding better results than either intervention alone.

文摘Background:Fat loss mainly conveys the benefits of caloric restriction for people living with type 2 diabetes.The literature is equivocal regarding whether exercise facilitates fat loss during caloric restriction.This analysis aimed to assess the dose-response effects of exercise in combination with a caloric restriction on fat mass(FM)and FM percentage(FM%)in persons with diagnosed type 2 diabetes.Methods:In this secondary analysis of a 4-armed randomized trial,82 persons living with type 2 diabetes were randomly allocated to the control group(CON)(n=21),diet control(DCON)(25%caloric restriction;n=20),diet control and exercise 3 times per week(MED)(n=20),or diet control and exercise 6 times per week(HED)(n=21)for 16 weeks.The primary analysis was the change in FM%points.Secondary analyses included fat-free mass and visceral adipose tissue(VAT)volume(cm^(3)).Results:FM%decreased compared to CON by a mean difference of-3.5%(95%confidence interval(95%CI):-5.6%to-1.4%),-6.3%(95%CI:-8.4%to-4.1%),and-8.0%(95%CI:-10.2%to-5.8%)for DCON,MED,and HED,respectively.Compared to DCON,MED,and HED decreased FM%by-2.8%(95%CI:-4.9%to-0.7%)and-4.5%(95%CI:-6.6%to-2.4%),respectively.The difference in FM%between HED and MED was-1.8%(95%CI:-3.9%to 0.4%).DCON and MED decreased fat-free mass compared to CON,whereas HED preserved fat-free mass(-0.2%;95%CI:-2.0%to 1.7%).Compared to CON,VAT volume decreased by-666.0 cm^(3)(95%CI:-912.8 cm^(3) to-385.1 cm^(3)),-1264.0 cm^(3)(95%CI:-1679.6 cm^(3) to-655.9 cm^(3)),and-1786.4 cm^(3)(95%CI:-2264.6 cm^(3) to-1321.2 cm^(3))more for DCON,MED,and HED,respectively.HED decreased VAT volume more than DCON(-1120.4 cm^(3);95%CI:-1746.6 cm^(3) to-639.4 cm^(3))while the remaining comparisons did not reveal any differences.Conclusion:All interventions were superior in reducing FM%compared to standard care.Adding exercise to a caloric restriction was superior in reducing FM%compared to a caloric restriction alone.
文摘Objective:To explore the effect of narrative nursing combined with exercise training on health beliefs and cardiac rehabilitation in patients with acute myocardial infarction(AMI)after interventional treatment.Methods:A total of 111 patients with acute myocardial infarction(AMI)who received interventional therapy in Department of Cardiology of Hefei First People’s Hospital from January 2022 to September 2023 were selected as the study subjects.They were randomly divided into a control group(n=55)and a study group(n=56)using a random table method.Both groups received routine nursing care,while the control group received exercise training on top of it.The study group received narrative nursing care on top of the control group.Both groups were intervened until the patients were discharged.The health beliefs,heart function,psychological status,and quality of life after intervention were compared between two groups using a 6-minute walk test(6MWT).Results:After intervention,the scores of the TSK-SV Heart in all dimensions(fear of movement,functional disorders,risk perception,and avoidance of movement)decreased in both groups,and the study group was lower than the control group,with a statistical significant difference(P<0.05).After intervention,the scores of the Self Rating Anxiety Scale(SAS)and Self Rating Depression Scale(SDS)in both groups decreased,and the study group was lower than the control group,with a statistical significant difference(P<0.05).After intervention,both groups showed an increase in left ventricular ejection fraction(LVEF)and left ventricular early diastolic peak flow velocity/left ventricular late diastolic peak flow velocity(E/A),with the study group being higher than the control group.The left ventricular end diastolic diameter(LVEDD)and left atrial volume index(LAVI)decreased,and the study group was lower than the control group,with statistical significant differences(P<0.05).The walking distance of the study group at 6MWT(488.8±31.4)m was greater than that of the control group(425.54±30.7)m,with statistical significant differences(P<0.05).The quality of life measurement scale(CROQ-PTCA-Post)of the study group after coronary intervention treatment had higher scores in all dimensions(physical function,satisfaction,psychosocial function,adverse reactions,symptoms,cognitive function)than the control group,with statistical significant differences(P<0.05).Conclusion:Narrative nursing combined with exercise training can enhance the health beliefs of AMI patients towards exercise training after intervention therapy,which is beneficial for cardiac rehabilitation and can improve psychological status and quality of life.
文摘Objective:To investigate the positive eff ect of mindfulness training and exercise therapy on functional recovery and quality of life in elderly patients with fractures of the lower back.Methods:Collection of medical records of patients with lumbar vertebral fractures in Suzhou Municipal Hospital(North District),60 patients underwent percutaneous vertebroplasty(PVP/PKP),they were divided into control and experimental groups according to a random number table,30 people per group;In addition to traditional care method,Also intervened with behavioural awareness training and exercise therapy,By comparing the Oswestry dysfunction index score(ODI),visual simulation score(VAS),ability of daily living score(ADL),quality of life score(SF-36),To evaluate the eff ectiveness of the intervention.Results:Before the intervention,the ODI,VAS,ADL,and SF-36 scores were basically the same,with no significant diff erence(P>0.05).After treatment,the ODI and VAS scores of the experimental group and the control group were signifi cantly lower,and the experimental group was significantly lower than the control group(P<0.05);the ADL scores of the experimental group were signifi cantly higher than those of the control group(P<0.05);their SF-36 quality of life scores in all dimensions were signifi cantly higher(P<0.05).Conclusion:Through the rehabilitation treatment method combining mindfulness behavior training and exercise intervention,it can eff ectively promote the functional recovery of osteoporotic lumbar fractures in the elderly,thus reducing their pain and improving their quality of life.
文摘BACKGROUND Diabetes is characterized by insulin resistance as well as impaired insulin production,withβ-cell dysfunction playing a critical role in disease progression.Exercise is known to improve insulin sensitivity,but its effects on pancreatic islet quality and function remain poorly understood.This work hypothesized that swimming training enhances glycemic control and insulin secretion by upregulating the insulin-like growth factor 1(IGF-1)/phosphatidylinositol 3-kinase/protein kinase B(PI3K/AKT)pathway in streptozotocin(STZ)-induced diabetic rats.AIM To investigate the effects of swimming on pancreatic islet quality and function in STZ-induced diabetic rats via the IGF-1/PI3K/AKT pathway.METHODS Twenty-six Sprague-Dawley rats were grouped into diabetic and control groups,with each group further split into exercise and sedentary subgroups.Diabetic rats were induced with STZ.The exercise groups underwent swimming training for 60 minutes/day,5 days/week,for 8 weeks.Body weight,food intake,blood glucose,insulin,lipids,and muscle glycogen were measured.Pancreatic islet morphology and the protein expression levels of IGF-1,PI3K,and AKT were analyzed.Data were analyzed using two-way repeated-measure ANOVA,followed by Tukey’s post-hoc test.RESULTS Exercise training significantly improved body weight[diabetic exercise group(D-Ex):390.66±50.14 g vs diabetic sedentary group(D-Sed):315.89±50.12 g,P<0.05],reduced blood glucose(D-Ex:12.21±4.43 mmol/L vs D-Sed:17.79±2.05 mmol/L,P<0.05),and increased insulin levels(D-Ex:53.50±15.31 pmol/L vs D-Sed:25.31±10.23 pmol/L,P<0.05)in diabetic rats.It also enhanced islet morphology,increased IGF-1 expression,and activated the PI3K/AKT pathway(P<0.05).In-vitro experiments confirmed that IGF-1 positively regulated insulin expression and inhibitedβ-cell apoptosis via the PI3K/AKT pathway.CONCLUSION Exercise training improves pancreatic islet quality and function in diabetic rats by modulating the IGF-1/PI3K/AKT pathway,highlighting its therapeutic potential for diabetes management.

文摘Objective: This study was conducted to examine the effects of aerobic exercise alone and aerobic exercise with resistance training on the quality of life in men over the age of 55 years with type 2 diabetes mellitus. Methods: A total of 54 participants were divided into the following three groups so that there were no significant differences in blood chemistry or physical ability indexes among the three groups: control, aerobic exercise, and aerobic exercise with resistance training. The latter two groups exercised for 24 weeks, while the control group performed no exercise. Blood chemistry levels and measures of physical ability in each group members were examined one day before and one day after the exercise regimens. Results: Compared with those before the study, blood glucose, glycated hemoglobin, triglycerides, cholesterol, and low-density lipoprotein levels as well as vital capacity, reaction time, sit-and-reach ability, and balancing while standing on one leg with closed eyes were significantly improved in the aerobic exercise only group(P < 0.05). All these measures as well as high-density lipoprotein levels and grip, back, and leg strength were significantly improved in the combined aerobic and resistance training group(P < 0.05). By contrast, no significant differences before and after the experiment were found in any measure for the control group(P > 0.05). Conclusion: Although both aerobic exercise and aerobic exercise combined with resistance training for 24 weeks effectively improved the quality of life in patients with type 2 diabetes, the effect of the combined training was better than that of aerobic exercise alone. These results suggest that resistance training may be safely added to the rehabilitation training regimen of patients with type 2 diabetes mellitus.
文摘Type 1 diabetes mellitus(T1DM)is an autoimmune condition whose prevalence is prominent in children and adolescents,resulting in insulin deficiency with a potential for long-term complications induced by glucotoxicity.As an autoimmune disease where the body's immune system attack insulin-producing beta cells in the pancreas,leading up to complete or near-complete inability to control the blood glucose levels independently.The lack of glycemic control calls for lifelong exogenous insulin administration in conjunction with careful monitoring to control blood sugar levels and prevent acute and chronic health issues complications.Regular physical activity,notably resistance exercise(RE),may be beneficial in the glycemic management of this population,enhancement of muscle strength,and general health for the growing,development and maturation in children.The evidence depicting its benefits and safeguard for RE in pediatric T1DM patients remains underexamined.This mini-review seeks to synthesize qualitatively the current evidence on RE regarding its global effects on the T1DM in children.A search for peer-reviewed papers is carried out through primary databases,centering on publications that examined the physiological,metabolic,and psychosocial consequences of RE in children with T1DM.Emerging evidence indicates that RE is one potential method of safe and efficacious intervention to improve glycemic management,physical capacity,and quality of life.However,there is still some reluctance to this type of training in the pediatric population.The available research has not only refuted the belief that strength training was contraindicated in the pediatric population but also recommends its systematic practice to enjoy its benefits on the three spheres of health.Nevertheless,methodological differences and small population studies pose challenges to drawing firm conclusions.The review underscores other areas,including the need for standardizing protocols for including patients such as follow-ups and greater considerations for psychosocial effects of RE in this population.This minireview underlines the importance of RE in a global approach to pediatric diabetes care by providing practical insight for both clinicians and researchers.
文摘The aim of this present study was to investigate the effects of training on exercise tolerance of patients with coronary heart disease after percutaneous coronary intervention.Fifty-seven cases of coronary heart disease after percutaneous coronary intervention were divided randomly into the rehabilitation training group(26 cases) and control group(31 cases).Patients in the rehabilitation training group received rehabilitation training at different stages and exercise intensities 3 d after percutaneous coronary intervention for 3 months.The heart rate,blood pressure,ECG changes in treadmill exercise test,and the frequency of anginal episodes were observed.The results showed that NST and ΣST of ECG and the frequency of anginal episodes were significantly reduced in the rehabilitation training group.In addition,exercise tolerance was improved and the total exercise time was lengthened in these patients.Moreover,ST segment depression time and emergence time of angina with exercise were also lengthened compared with controls(P 〈 0.05,or 0.01).However,the heart rate and blood pressure before and after exercise of the two groups were similar.The study indicated that rehabilitation training could significantly relieve angina,amend ischemic features of ECG,and improve exercise tolerance of coronary heart disease patients after percutaneous coronary intervention.
文摘After intraperitoneal injection of 20 mg/kg lead acetate, rats received 8 weeks of treadmill exercise (15-22 m/min, 25-64 minutes) and/or treadmill exercise at 1.6 km/h until exhaustion. The markers related to neurotoxicity were measured by enzyme-linked immunosorbent assay method. 8 weeks of treadmill exercise significantly increased brain-derived neurotrophic factor level in the hippocampus (P = 0.04) and plasma level of total antioxidant capacity of rats exposed to lead acetate (P 〈 0.001), and significantly decreased plasma level of malondialdehyde (P 〈 0.001). Acute exercise only decreased the hippocampal malondialdehyde level (P = 0.09) and increased brain-derived neurotrophic factor level in the hippocampus (P = 0.66). Acute exercise also enhanced the total antioxidant capacity in rats exposed to lead acetate, insignificantly (P = 0.99), These findings suggest that chronic treadmill exercise can significantly decrease neurotoxicity and alleviate oxidative stress in rats exposed to lead acetate. However, acute endurance exercise was not associated with these beneficial effects.
文摘Background Heat shock proteins(HSPs)are key molecular chaperones that help maintain protein homeostasis by stabilizing or removing damaged proteins during cellular stress.Aging weakens these stress–response systems,disrupting proteostasis and increasing vulnerability to sarcopenia.High-intensity training(HIT)can counteract these declines by activating protective pathways such as the HSP response.HSPs are highly responsive to stress,examining their regulation during aging is important,as altered HSP activity is linked to the progressive loss of muscle mass.Methods This study investigated the abundance and phosphorylation of HSPs in skeletal muscle from healthy,active young and older adults(n=7 per group),assessed at baseline and again in the older group following 12 weeks of HIT.Using calibrated Western blotting on both whole-muscle homogenates and pooled single muscle fibres,we quantified HSP content and phosphorylation to determine how aging and exercise influence stress–responsive protein regulation at both the tissue and cellular levels.Results In whole muscle homogenates,HSPs(HSP72,HSP27,andαB-crystallin)did not differ between young and older adults,while higher phosphorylation of small HSPs(sHSPs):phospho-HSP27 at Serine15(pHSP27 Ser15)and phospho-αB-crystallin at Serine59(pαB-crystallin Ser59)(∼1.8-fold and∼2.9-fold,respectively)were found in muscle from older adults,indicating higher cellular stress associated with aging.A 12-week HIT intervention in older adults reduced homogenate pHSP27 Ser15 and pαB-crystallin Ser59 abundances to similar levels found in young adults.Total HSPs typically displayed a distinct fiber-type profile in both age groups,with more in type I compared to type II fibers,distinguished by the presence of myosin heavy chain I(MHCI)or MHCII.Phosphorylation at pHSP27 Ser15 and pαB-crystallin Ser59 was not different between type I and type II fibers.The HIT in older adults decreased total and phosphorylated sHSPs in both type I and type II fibers but increased HSP72 in type I fibers.Conclusion HIT has the potential to mitigate age-related cellular stress and modulate protein expression patterns in aging skeletal muscle and,perhaps,has the potential to delay age-related muscle decline,thereby improving muscle health in older adults.
文摘<div style="text-align:justify;"> <span style="font-family:Verdana;"><strong>Background:</strong> Exercise is important component of diabetes management. Regular exercise improves blood glucose, reduces hazard of cardiovascular diseases, decreases weight, increases insulin sensitivity and strengthens patients’ general health. Regular exercise can prevent or delay type 2 diabetes in those of high-risk populations. Although there are great benefits of regular exercise, unfortunately majority of people with type 2 diabetes are not active. <strong>Objectives:</strong> The main goal of this study was to assess patients adherence to regular exercise among type 2 diabetes Mellitus (T2DM) attending Ribat University Hospital Diabetic Clinic (RUHDC) Khartoum state, Sudan. <strong>Methods:</strong> Analytical cross sectional hospital based study was carried out among 351 patients from August 2012 to February 2013. Structured questionnaire and patients cards were used to collect data. <strong>Results:</strong> Out of 351 patients, female were 65.8% and male 34.2%. Patients older than 60 years 48.4%, illiterate 21.0%, only 44.2% had basic educational level, house wife 56.1% and retired 18.5%. Majority had irregular exercise program (80.1%), 12.1% specified lack of time whilst 87.9% did not specify any other reasons as barrier for regular exercise “no reason”. Daily activity was the main type of physical activity (59.8%). Although insignificant, irregular exercise was higher among female patients, their age above 60 years, patients had low education levels and those had financial problems. <strong>Conclusion:</strong> Regular exercise was low in this study. Lack of time, female, patients older than 60 years, had low education levels, and had financial problems, which were the main barriers of regular exercise. Family support, tailored patients’ education and reduction of patients’ use of technology and modern living can improve the patients’ activity.</span> </div>
文摘Background Stable angina pectoris is a common subtype of coronary heart disease.Patients suffer from chest tightness,chest pain and crushing pain under the inducement of fatigue and emotional agitation.This study aims to investigate the effect of exercise training guidance based on action research on exercise endurance and readmission rate of patients with stable angina pectoris.Methods A retrospective study was conducted on 60 patients with stable angina pectoris due to coronary heart disease admitted to our hospital from February 2020 to November 2023.Patients were divided into a control group of 29 cases(receiving conventional exercise training guidance)and a guidance group of 31 cases(receiving action research-based exercise training guidance).Both groups received continuous training for one month.A comparison was made between the exercise endurance indicators[6-minute walk test distance(6MWT),peak oxygen consumption(peak VO2),anaerobic threshold(AT),exercise duration(ED)],quality of life,and readmission rate within 6 months after intervention in both groups.Results After 1 month of intervention,6MWT,peak VO2,AT and ED were increased in both groups,and those in the guidance group were higher than those in the conventional group(P<0.05).The score of quality of life(The 36-item shot-form health status survey,SF-36)in both groups was increased,and the guidance group scored higher than the conventional group(P<0.05).The readmission rate of patients in the guidance group was 6.45%,which was lower than that in the conventional group(27.59%)(P<0.05).Conclusions The application of action research-based exercise training guidance in patients with stable angina pectoris due to coronary heart disease has significant effects,improving exercise endurance,significantly improving quality of life,and reducing readmission rate,thus having practical value.[S Chin J Cardiol 2024;25(3):162-168]
文摘Aerobic capacity, which is expressed as peak oxygen consumption (VO2peak), is well-known to be an independent predictor of all-cause mortality and cardiovascular prognosis. This is true even for people with various coronary risk factors and cardiovascular diseases. Although exercise training is the best method to improve VO2peak, the guidelines of most academic societies recommend 150 or 75 min of moderate- or vigorous- intensity physical activities, respectively, every week to gain health benefits. For general health and primary and secondary cardiovascular prevention, high-intensity interval training (HIIT) has been recognized as an efficient exercise protocol with short exercise sessions. Given the availability of the numerous HIIT protocols, which can be classified into aerobic HIIT and anaerobic HIIT [usually called sprint interval training (SIT)], professionals in health-related fields, including primary physicians and cardiologists, may find it confusing when trying to select an appropriate protocol for their patients. This review describes the classifications of aerobic HIIT and SIT, and their differences in terms of effects, target subjects, adaptability, working mechanisms, and safety. Understanding the HIIT protocols and adopting the correct type for each subject would lead to better improvements in VO2peak with higher adherence and less risk.
文摘AIM:To evaluate the long-term effect of aerobic and/or anaerobic exercise on glycemic control in youths with type 1 diabetes.METHODS:Literature review was performed in spring and summer 2014 using Pub Med/MEDLINE,Google Scholar,Scopus,and Science Direct with the following terms:aerobic,anaerobic,high-intensity,resistance,exercise/training,combined with glycemic/metabolic control,glycated haemoglobin A1c(Hb A1c) and type 1diabetes.Only peer-reviewed articles in English were included published in the last 15 years.It was selected from 1999 to 2014.Glycemic control was measured with Hb A1 c.Studies with an intervention lasting at least 12 wk were included if the Hb A1 c was measured before and after the intervention.RESULTS:A total of nine articles were found,and they were published between the years of 2002-2011.The sample size was 401 diabetic youths(166 males and 235 females) with an age range of 10-19 years except one study,in which the age range was 13-30 years.Study participants were from Australia,Tunisia,Lithuania,Taiwan,Turkey,Brazilia,Belgium,Egypt and France.Four studies were aerobic-based,four were combined aerobic and anaerobic programs,and one compared aerobic exercise to anaerobic one.Available studies had insufficient evidence that any type of exercise or combined training would clearly improve the glycemic control in type 1 diabetic youth.Only three(two aerobic-based and one combined) studies could provide a significant positive change in glycemic control.CONCLUSION:The regular physical exercise has several other valuable physiological and health benefits that justify the inclusion of exercise in pediatric diabetes treatment and care.
