Molecular hydrogen therapy in musculoskeletal conditions:An evidence-based review and critical analysis

Molecular hydrogen(H2)demonstrates selective antioxidant and anti-inflammatory properties with therapeutic potential across musculoskeletal conditions including osteoarthritis,rheumatoid arthritis,exercise-induced muscle damage,chronic pain syndromes,tendinopathies,and muscle atrophy.This review critically evaluates preclinical and clinical evidence for H2 therapy and identifies research gaps.A comprehensive search of PubMed,EMBASE,and Cochrane Library(up to April 2025)yielded 45 eligible studies:25 preclinical and 20 clinical trials.Preclinical models consistently showed reductions in reactive oxygen species,inflammatory cytokines,and improved cell viability.Clinical trials reported symptomatic relief in osteoarthritis,decreased Disease Activity Score 28 in rheumatoid arthritis,and accelerated clearance of muscle damage markers.Delivery methods varied-hydrogen-rich water,gas inhalation,and saline infusion-hindering direct comparison.Mechanistic biomarkers were inconsistently reported,limiting understanding of target engagement.Common limitations included small sample sizes,short durations,and protocol heterogeneity.Despite these constraints,findings suggest H2 may serve as a promising adjunctive therapy via antioxidant,anti-inflammatory,and cytoprotective mechanisms.Future research should prioritize standardized delivery protocols,robust mechanistic endpoints,and longer-term randomized trials to validate clinical efficacy and optimize therapeutic strategies. 展开更多

LitAutoScreener:Development and Validation of an Automated Literature Screening Tool in Evidence-Based Medicine Driven by Large Language Models

Background:The traditional manual literature screening approach is limited by its time-consuming nature and high labor costs.A pressing issue is how to leverage large language models to enhance the efficiency and quality of evidence-based evaluations of drug efficacy and safety.Methods:This study utilized a manually curated reference literature database—comprising vaccine,hypoglycemic agent,and antidepressant evaluation studies—previously developed by our team through conventional systematic review methods.This validated database served as the gold standard for the development and optimization of LitAutoScreener.Following the PICOS(Population,Intervention,Comparison,Outcomes,Study Design)principles,a chain-of-thought reasoning approach with few-shot learning prompts was implemented to develop the screening algorithm.We subsequently evaluated the performance of LitAutoScreener using 2 independent validation cohorts,assessing both classification accuracy and processing efficiency.Results:For respiratory syncytial virus vaccine safety validation title-abstract screening,our tools based on GPT(GPT-4o),Kimi(moonshot-v1-128k),and DeepSeek(deepseek-chat 2.5)demonstrated high accuracy in inclusion/exclusion decisions(99.38%,98.94%,and 98.85%,respectively).Recall rates were 100.00%,99.13%,and 98.26%,with statistically significant performance differences(χ^(2)=5.99,P=0.048),where GPT outperformed the other models.Exclusion reason concordance rates were 98.85%,94.79%,and 96.47%(χ^(2)=30.22,P<0.001).In full-text screening,all models maintained perfect recall(100.00%),with accuracies of 100.00%(GPT),100.00%(Kimi),and 99.45%(DeepSeek).Processing times averaged 1 to 5 s per article for title–abstract screening and 60 s for full-text processing(including PDF preprocessing).Conclusions:LitAutoScreener offers a new approach for efficient literature screening in drug intervention studies,achieving high accuracy and significantly improving screening efficiency. 展开更多

Objective:To summarize evidence on the prevention and management of intradialytic hypotension in maintenance hemodialysis patients,providing reference for clinical practice.Method:Chinese and English databases,guideline websites,and professional society websites were systematically searched for literature on intradialytic hypotension guidance,including clinical decisions,guidelines,evidence summaries,systematic reviews,and expert consensuses,from database inception to October 1,2024.Evidence was extracted after literature quality evaluation.Results:A total of 11 publications were included:2 clinical decisions,7 guidelines,1 systematic review,and 1 expert consensus.38 pieces of evidence were summarized across 4 themes:pre-dialysis assessment and prevention,monitoring and management during dialysis,medication use,and patient self-management.Conclusion:The best evidence for prevention and management of intradialytic hypotension in maintenance hemodialysis patients is scientific and comprehensive.Healthcare professionals are advised to apply this evidence judiciously in conjunction with clinical realities to ensure patient safety. 展开更多

Objective:To explore the obstructive factors in the behavior of medical staff during the implementation of respiratory rehabilitation process,and to provide a basis for the rehabilitation management intervention of COPD.Methods:A descriptive nature research method was adopted.An interview outline was formulated based on the theoretical domain framework.From October to December 2024,15 medical staff from the respiratory department of a tertiary hospital in Shaanxi Province were selected for semi-structured interviews.The interview data were analyzed using the Colaizzi 7-step analysis method.Result:The analysis of this study found that the obstructive factors for medical staff to implement respiratory rehabilitation include five theoretical domains.The problems are respectively the lack of knowledge about respiratory rehabilitation and insufficient training intensity,the insufficient self-recognition of implementing respiratory rehabilitation,the low awareness rate of patients and the low cooperation degree,the insufficient provision of instruments and facilities,the lack of rehabilitation training venues and respiratory rehabilitation clinics,and the lack of scientific and standardized respiratory rehabilitation management processes.Conclusion:There are many obstructive factors affecting the implementation of respiratory rehabilitation by medical staff.Clinical managers should take corresponding measures,continuously improve the rehabilitation management strategies for COPD,and promote the clinical application of the best evidence for respiratory rehabilitation. 展开更多

Complex evidence theory is a generalized Dempster-Shafer evidence theory,which has the ability to express uncertain information.One of the key issues is the uncertainty measure of Complex Basic Belief Assignment(CBBA).However,the research on the uncertainty measure of complex evidence theory is still an open issue.Therefore,in this paper,first,the Fractal-based Complex Belief(FCB)entropy as a generalization of Fractal-based Belief(FB)entropy,which has superiority in uncertainty measurement of CBBA,is proposed.Second,on the basis of FCB entropy,we propose Fractal-based Supremum Complex Belief(FSCB)entropy and Fractal-based Infimum Complex Belief(FICB)entropy,with FSCB entropy as the upper bound and FICB entropy as the lower bound.They are collectively called the proposed FCB entropy.Furthermore,we analyze the properties,physical interpretation and numerical examples to prove the rationality of the proposed method.Finally,a practical information fusion application is proposed to prove that the proposed FCB entropy can reasonably measure the uncertainty of CBBA.The results show that,the proposed FCB entropy can handle the uncertainty measure of CBBA,which can be a reasonable way for uncertainty measure in complex evidence theory. 展开更多

Our department established risk assessment criteria for medical adhesives in neonates by applying the best evidence in the management program for the reduction of medical adhesive-associated skin injuries in neonates, in terms of the use and removal of adhesives. Methods: A systematic search and quality assessment of topics related to medical adhesive-related skin injury in neonates was conducted to summarize the best evidence and to conduct a quality review in the neonatal unit. Results: After 2 rounds of review, medical and nursing staff in the neonatal unit had a 98% compliance rate for the knowledge of neonatal medical adhesive-related skin injury and a satisfactory compliance rate for the other 9 indicators;after the application of the evidence, the incidence of neonatal medical adhesive-related skin injury was significantly lower than that before the application of the evidence, and the differences were statistically significant (P Conclusion: The application of the best evidence-based management program in neonatal medical adhesive-associated skin injury can reduce the incidence of neonatal medical adhesive-associated skin injury, reduce neonatal infections, and improve the integrity of the protective skin barrier in neonates. 展开更多

Methods: Use the KTA knowledge translation model to guide evidence-based practice in nutrition management, and compare the nutritional status, cardiac function status, quality of life, and quality review indicators of chronic heart failure patients before and after the application of evidence. Results: After the application of evidence, the nutritional status indicators (MNA-SF score, albumin, hemoglobin) of two groups of heart failure patients significantly increased compared to before the application of evidence, with statistically significant differences (p Conclusion: The KTA knowledge translation model provides methodological guidance for the implementation of evidence-based practice for heart failure patients. D-S evidence theory provides a good approach to fuse uncertain inlbrmation. In this article, we introduce seismic multi-attribute fusion based on D-S evidence theory to predict the coalbed methane (CBM) concentrated areas. First, we choose seismic attributes that are most sensitive to CBM content changes with the guidance of CBM content measured at well sites. Then the selected seismic attributes are fused using D-S evidence theory and the fusion results are used to predict CBM-enriched area. The application shows that the predicted CBM content and the measured values are basically consistent. The results indicate that using D-S evidence theory in seismic multi-attribute fusion to predict CBM-enriched areas is feasible. 展开更多

To address the issue of incorrect fusion results caused by conflicting evidence due to inaccurate evidence and incomplete recognition frameworks in radar airborne target tactical intention recognition,a spatiotemporal evidence fusion algorithm is proposed.To resolve the conflict evidence fusion problem caused by inaccurate evidence,the algorithm performs discounting of evidence from both spatial and temporal dimensions.Spatial discounting is influenced by both inter-evidence inconsistency and intra-evidence inconsistency,while temporal discounting is determined by time intervals and information entropy.For the problem of conflicting evidence fusion due to an incomplete recognition framework,an open recognition architecture based on dynamic composite focal elements is proposed.This approach allocates some conflicting information to temporary composite focal elements,avoiding excessive basic probability assignment(BPA)of the empty set after fusion,which can lead to deviations from the actual fusion results.Simulation experiments comparing various methods indicate that the proposed method can effectively improve target intention recognition accuracy and demonstrates good stability. 展开更多

Background:Immune-mediated inflammatory disease(IMID)and cancer share underlying mechanisms.We aimed to comprehensively evaluate the associations between IMIDs and cancers from global,population and genetic perspectives.Methods:A triangulation framework was employed to assess the association between IMIDs and cancers,using the Global Burden of Disease Study(2012-2021)to analyse six IMIDs and 33 cancers.The UK Biobank(UKBB)prospective cohort was subsequently used to validate these associations,with hazard ratios(HRs)and 95%confidence intervals(CIs)estimated by Cox proportional hazards models.Causal inference based on genetic instruments was performed in the FinnGen and UKBB to assess the potential causal effects between IMIDs and cancers.Results:IMIDs were positively associated with the occurrence of cancers from a global perspective.Moreover,170 specific IMID-cancer pairs revealed statistically significant associations.A total of 20 pairs of specific IMID-cancer associations were further confirmed in the UKBB cohort.Among these,the five most pronounced associations included atopic dermatitis with Hodgkin lymphoma(HR=12.56,95%CI:1.76-89.59),with ovarian cancer(HR=5.65,95%CI:1.41-22.65)and with non-Hodgkin lymphoma(HR=5.11,95%CI:1.91-13.63);rheumatoid arthritis with Hodgkin lymphoma(HR=3.85,95%CI:1.11-13.32);and psoriasis with Hodgkin lymphoma(HR=3.43,95%CI:1.69-6.96).Additionally,a positive causal association between rheumatoid arthritis and Hodgkin lymphoma(inverse variance weighted OR=1.31,95%CI:1.10-1.57)was observed.Conclusions:This study provides comprehensive evidence of the relationships between IMIDs and cancers from global,population and genetic perspectives and identifies 20 pairs of specific IMID-cancer associations,thereby contributing to advancements in cancer prevention and control. 展开更多

Community-pioneered Nature-based Solutions(NbS)have become the main strategies in climate adaptation,although the evidence of their effectiveness and the governing conditions is still fragmented across hazards,ecosystems,and disciplines.The current review is a synthesis of the worldwide empirical research based on the concept of community-led NbS,meaning those interventions where communities have significant decision-making power and responsibility concerning the design,stewardship,sharing of benefits,and learning.On a taxonomy that differentiates between proximal ecosystem functionality and hazard modulation and distal human vulnerability reduction,and procedural,distributional,and recognition justice,we systematize the evidence-based findings according to hazardecosystem-intervention type(coastal storms and sea-level rise,flooding,drought and water insecurity,urban heat,and emerging compound risks)and we compare the outcomes.The results are reported to have the co-benefits of biodiversity gain,livelihood diversification,and better well-being,though they can be neutralized by elite capture,exclusion,tenure insecurity,as well as,in cities,green gentrification and displacement.The analysis of governance indicates repeating bundles related to longer-lasting and fairer results:hedge rights and tenure,community-enforceable and legitimizing representation institutions,financing institutions with longer horizons of maintenance and active adaptation,protection,and grievance,ethical supervision,and data governance.Our findings conclude that to scale community-led NbS,we need to switch the targets of areas to the target of governance quality and design of evaluation that would connect a change in the ecosystem to lived risk reduction and distributional change. 展开更多

Objective:To systematically summarize and evaluate the evidence on discharge preparation services for patients undergoing total knee arthroplasty,providing an evidence-based foundation for developing scientific and standardized discharge preparation intervention programs in clinical practice.Methods:Following the“5S”evidence model,literature such as guidelines,expert consensuses,evidence summaries and randomized controlled trials related to discharge preparation services for total knee arthroplasty patients were retrieved from relevant websites and databases,both domestic and international,from database inception to August 31,2025.Two researchers independently screened the literature,conducted quality appraisals,and extracted and synthesized the evidence.Results:A total of 15 articles were included,comprising 3 guidelines,4 expert consensuses,3 evidence summaries,3 systematic reviews and 2 randomized controlled trials.Ultimately,23 pieces of evidence were summarized across five aspects.Conclusion:This study synthesizes the evidence on discharge preparation services for patients undergoing total knee arthroplasty.It is recommended that healthcare professionals apply this evidence in clinical practice,considering specific circumstances and patient needs. 展开更多

Securing restricted zones such as airports,research facilities,and military bases requires robust and reliable access control mechanisms to prevent unauthorized entry and safeguard critical assets.Face recognition has emerged as a key biometric approach for this purpose;however,existing systems are often sensitive to variations in illumination,occlusion,and pose,which degrade their performance in real-world conditions.To address these challenges,this paper proposes a novel hybrid face recognition method that integrates complementary feature descriptors such as Fuzzy-Gabor 2D Fisher Linear Discriminant(FG-2DFLD),Generalized 2D Linear Discriminant Analysis(G2DLDA),andModular-Local Binary Patterns(Modular-LBP)with Dempster–Shafer(DS)evidence theory for decision fusion.The proposed framework extracts global,structural,and local texture features,models them using Gaussian distributions to estimate belief factors,and fuses these belief factors through DS theory to explicitly handle uncertainty and conflict among descriptors.Experimental validation was performed on two widely used benchmark datasets,ORL and Cropped Yale B,achieving recognition rates exceeding 98%,which outperform traditional methods as well as recent deep learning-based approaches.Furthermore,the method demonstrated strong robustness under noisy conditions,maintaining accuracies above 96%with salt-and-pepper and Gaussian noise.These results highlight the effectiveness of the proposed integration strategy in enhancing accuracy,reliability,and resilience compared to single-descriptor and conventional fusion methods.Given its high performance and efficiency,the proposed method shows strong potential for deployment in real-world restricted-zone applications such as smart parking systems,secure facility access,and other high-security domains. 展开更多

Reversible data hiding(RDH)enables secret data embedding while preserving complete cover image recovery,making it crucial for applications requiring image integrity.The pixel value ordering(PVO)technique used in multi-stego images provides good image quality but often results in low embedding capability.To address these challenges,this paper proposes a high-capacity RDH scheme based on PVO that generates three stego images from a single cover image.The cover image is partitioned into non-overlapping blocks with pixels sorted in ascending order.Four secret bits are embedded into each block’s maximum pixel value,while three additional bits are embedded into the second-largest value when the pixel difference exceeds a predefined threshold.A similar embedding strategy is also applied to the minimum side of the block,including the second-smallest pixel value.This design enables each block to embed up to 14 bits of secret data.Experimental results demonstrate that the proposed method achieves significantly higher embedding capacity and improved visual quality compared to existing triple-stego RDH approaches,advancing the field of reversible steganography. 展开更多

Traveling wave(TW)fault location technology has been widely used in transmission systems due to its high accuracy and simplicity.Recently,there has been growing interest in applying this technology to medium voltage(MV)distribution lines.However,current practices in its deployment,signal measurement,and threshold setting are usually from the application experiences in transmission lines,despite significant differences in fault-induced wave characteristics between transmission and distribution systems.To address these issues,this paper investigates the feasibility and applicability of TW fault technology in MV overhead distribution lines through characteristic analysis of fault-induced TWs.The propagation characteristics of aerial mode and zero mode TWs on overhead distribution lines are studied.Furthermore,it evaluates the influence of critical distri-bution network components including distribution transformers,multi-branch configurations,and busbar structures on wave propagation characteristics.Deployment strategies for traveling wave fault location(TWFL)devices is proposed to address the unique challenges of distribution networks,while the fault location method is also improved.Field test results demonstrate the effectiveness of the proposed methodology,showing improved fault detection accuracy and system reliability in distri-bution network applications.This research provides practical implementation suggestions for TWFL technology in distribution networks. 展开更多
