Beyond the evolution and erratic mutations like transposons within the genome, it’s as if the memory of a fossil genome with multiple symmetries persists. This recalls the “intermingling” of information characterizing the fractal universe of chaos theory. The result leads to a balanced and perfect tuning between the masses of the two strands of the huge DNA molecule that constitute our genome. We show here how codon populations forming the single-stranded DNA sequences can constitute a critical approach to the understanding of junk DNA function. Then, we suggest revisiting certain methods published in our 2009 book “Codex Biogenesis”. In fact, we demonstrate here how the universal genetic code table is a powerful analytical filter to characterize single-stranded DNA sequences constituting chromosomes and genomes. We can then show that any genomic DNA sequence is featured by three numbers, which characterize it and its 64 codon populations with correlations greater than 99%. The number “1” is common to all sequences, expressing the second law of Chargaff. The other 2 numbers are related to each specific DNA sequence case characterizing life species. For example, the entire human genome is characterized by three remarkable numbers 1, 2, and Phi = 1.618 the golden ratio. Associated with each of these three numbers, we can match three axes of symmetry, then “imagine” a kind of hyperspace formed by these codon populations. Then we revisit the value (3-Phi)/2 which is probably universal and common to both the scale of quarks and atomic levels, balancing and tuning the whole human genome codon population. Finally, we demonstrate a new kind of duality between “form and substance” overlapping the whole human genome: we will show that—simultaneously with the duality between genes and junk DNA—there is a second layer of embedded hidden structure overlapping all the DNA of the whole human genome, dividing it into a second type of duality information/redundancy involving golden ratio proportions.展开更多

Apolipoprotein E(APOE) genotypes and their corresponding protein(Apo E) isoforms may influence the biophysical properties of the cell membrane lipid bilayer. However, the role of APOE in central nervous system pathophysiology extended beyond its lipid transport function. In the present review article, we analyzed the links existing between APOE genotypes and the neurobiology of neuropsychiatric symptoms in neurodegenerative and vascular diseases. APOE genotypes(APOE ε2, APOE ε3, and APOE ε4) were implicated in common mechanisms underlying a wide spectrum of neurodegenerative diseases, including sporadic Alzheimer's disease, synucleinopathies such as Parkinson's disease and Lewy body disease, stroke, and traumatic brain injury. These shared pathways often involved neuroinflammation, abnormal protein accumulation, or responses to acute detrimental events. Across these conditions, APOE variants are believed to contribute to the modulation of inflammatory responses, the regulation of amyloid and tau pathology, as well as the clearance of proteins such as α-synuclein. The bidirectional interactions among Apo E, amyloid and mitochondrial metabolism, immunomodulatory effects, neuronal repair, and remodeling underscored the complexity of Apo E's role in neuropsychiatric symptoms associated with these conditions since from early phases of cognitive impairment such as mild cognitive impairment and mild behavioral impairment. Besides Apo E-specific isoforms' link to increased neuropsychiatric symptoms in Alzheimer's disease(depression, psychosis, aberrant motor behaviors, and anxiety, not apathy), the APOE ε4 genotype was also considered a significant genetic risk factor for Lewy body disease and its worse cognitive outcomes. Conversely, the APOE ε2 variant has been observed not to exert a protective effect equally in all neurodegenerative diseases. Specifically, in Lewy body disease, this variant may delay disease onset, paralleling its protective role in Alzheimer's disease, although its role in frontotemporal dementia is uncertain. The APOE ε4 genotype has been associated with adverse cognitive outcomes across other various neurodegenerative conditions. In Parkinson's disease, the APOE ε4 allele significantly impacted cognitive performance, increasing the risk of developing dementia, even in cases of pure synucleinopathies with minimal co-pathology from Alzheimer's disease. Similarly, in traumatic brain injury, recovery rates varied, with APOE ε4 carriers demonstrating a greater risk of poor long-term cognitive outcomes and elevated levels of neuropsychiatric symptoms. Furthermore, APOE ε4 influenced the age of onset and severity of stroke, as well as the likelihood of developing stroke-associated dementia, potentially due to its role in compromising endothelial integrity and promoting blood–brain barrier dysfunction.展开更多

Dorsal root ganglia neurons gradually lose their axonal regeneration ability during development and aging.To explore molecules that enhance axonal regeneration,we screened growth factors with differential gene express...Dorsal root ganglia neurons gradually lose their axonal regeneration ability during development and aging.To explore molecules that enhance axonal regeneration,we screened growth factors with differential gene expression patterns in the dorsal root ganglias of young adult and aged animals following sciatic nerve injury.In young adult animals,two transforming growth factor beta-related factors,activin A and angiopoietin 2,were found to be upregulated post nerve injury.Treatment of isolated dorsal root ganglia explants and cultured dorsal root ganglia neurons of neonatal and young adult rats with recombinant activin A or angiopoietin 2 protein stimulated neurite outgrowth and axonal elongation.The administration of recombinant activin A or angiopoietin 2 protein to sciatic nerve crush-injured dorsal root ganglias also supported the growth of sensory neurons and facilitated nerve regeneration in both young adult and aged rats.Using RNA sequencing,we characterized genetic changes in dorsal root ganglia neurons following recombinant activin A or angiopoietin 2 treatment,revealing the unique mechanisms of these transforming growth factor beta-related factors.Recombinant activin A elicited changes in the gene expression of cytoskeleton-related Gper1 and activated extracellular signal-regulated kinase signaling,while angiopoietin 2 increased the expression of the transcription factor gene E2f2.Our identification of activin A and angiopoietin 2 as crucial promotional factors of axonal regeneration may guide future therapeutic strategies for the treatment of nerve injury.展开更多

Quoting Kant, freedom and ethical necessities are reciprocally limited. In this contribution, a more mathematical than philosophical reflection about reciprocity as an ever-present dual property of everything was given. As a crystallographer, the author is familiar with the action of Fourier transforms and the relation between a crystal lattice and its reciprocal lattice, already pointing to the duality between particles and waves. A generalization of the reciprocity term was stimulated by results of the famous Information Relativity (IR) theory of Suleiman with its proven physical manifestation of matter-wave duality, compared to the set-theoretical E-Infinity theory developed by El Naschie, where the zero set represents the pre-quantum particle, and the pre-quantum wave is assigned to the empty set boundary surrounding the pre-particle. Expectedly, the most irrational number of the golden mean is involved in these thoughts, because this number is intimately connected with its inverse. An important role plays further Hardy’s maximum quantum entanglement probability as the fifth power of φand its connection to the dark matter. Remembering, the eleven dimensions in Witten’s M-theory may be decomposed into the Lucas number L5 = 11 = φ−5 – φ5. Objective:To observe the efficacy of LIU’s infantile Tuina(Chinese therapeutic massage)combined with Bi Yan Gao in treating allergic rhinitis(AR)in kids and the impact on serum inflammatory factors.Methods:A total of...Objective:To observe the efficacy of LIU’s infantile Tuina(Chinese therapeutic massage)combined with Bi Yan Gao in treating allergic rhinitis(AR)in kids and the impact on serum inflammatory factors.Methods:A total of 120 kids with AR were recruited and divided into a Tuina group,a Bi Yan Gao group,and a combined group using the random number table method,with 40 cases in each group.The three groups all received oral desloratadine syrup as the basic intervention.In addition,the Tuina group was treated with LIU’s infantile Tuina therapy once daily;the BI Yan Gao group received external application of Bi Yan Gao,once every morning and night;the combined group was intervened by LIU’s infantile Tuina plus Bi Yan Gao external application.The intervention lasted for 8 weeks.Before and after the intervention,the total nasal symptom score(TNSS)was compared,and the serum levels of interleukin(IL)-4,IL-6,IL-8,and immunoglobulin E(IgE)were detected.The total effective rate was also compared among the three groups.Results:After 8 weeks of treatment,the total effective rate was 87.2%in the combined group,60.5%in the Tuina group,and 57.5%in the Bi Yan Gao group,showing significant between-group differences(P<0.05).The contents of serum IL-6,IL-8,and IgE were lower in the combined group compared to the Tuina and Bi Yan Gao groups,presenting statistical significance(P<0.05).The level of IL-4 was statistically parallel across the three groups(P>0.05).Conclusion:The combined use of LIU’s infantile Tuina and Bi Yan Gao can improve nasal symptoms and lower the levels of serum inflammatory factors and IgE in kids with AR,producing a higher total effective rate compared to the separate use of the two approaches.展开更多

In recent years, several new techniques for the treatment of very low rectal cancer patients aiming to preserve the gastrointestinal continuity and to improve both the oncological as well as the functional outcomes, have been emerged. Literature suggest that when the intersphincteric resection is applied in T1-3 tumors located within 30-35 mm from the anal verge, is technically feasible, safe, with equal oncological outcomes compared to conventional surgery and acceptable quality of life. The Anterior Perineal Plan E for Ultra-low Anterior Resection technique, is not disrupting the sphincters, but carries a high complication rate, while the reports on the oncological and functional outcomes are limited. Transanal Endoscopic Micro Surgery(TEM) and Trans Anal Minimally Invasive Surgery(TAMIS) should represent the treatment of choice for T1 rectal tumors, with specific criteria according to the NCCN guidelines and favorable pathologic features. Alternatively to the standard conventional surgery, neoadjuvant chemo-radiotherapy followed by TEM or TAMIS seems promising for tumors of a local stage T1sm2-3 or T2. Background:Sepsis,a life-threatening systemic inflammatory response to infection,presents significant challenges in the intensive care unit(ICU).The condition is frequently accompanied by persistent neurodegenerative ...Background:Sepsis,a life-threatening systemic inflammatory response to infection,presents significant challenges in the intensive care unit(ICU).The condition is frequently accompanied by persistent neurodegenerative changes and muscle atrophy,especially in patients with burn wounds,severely affecting recuperation and quality of life.Traditional physical therapy and advanced sedative screening represent pivotal approaches for the rehabilitation and management of these cases.Methods:A comprehensive transcriptomic study was conducted using gene expression datasets from sepsis patients and important biological models to identify potential targets.We subsequently employed virtual molecular docking and molecular dynamics simulations to screen the FDA drug library and traditional Chinese medicine databases for compounds exhibiting high binding affinity to isocitrate dehydrogenase 2(IDH2).The functional role of this target was validated in vitro using an IDH2 deficient BV-2 microglial cell model stimulated by lipopolysaccharide.Results:Key features were identified through differential expression analysis across multiple datasets.Gene set enrichment analysis(GSEA)highlighted pathways associated with chronic neurodegenerative diseases.Stable interactions between IDH2 and cobicistat or licorice glycoside E were predicted,supporting the identification of IDH2 as a promising therapeutic target.Knockdown of IDH2 in microglial cells led to markedly suppressed production of pro-inflammatory cytokines,including TNF-αand IL-6,confirming its pivotal role in neuroinflammatory response regulation.Conclusion:These findings underscore the pivotal role of IDH2 in persistent neurodegenerative changes associated with sepsis,including burn-related cases.Targeting IDH2 may reduce long-term neurovascular complications and muscle atrophy in septic ICU patients.Our findings suggest that IDH2 is a potential therapeutic target,and the combination of cobicistat and licorice glycoside E warrants further investigation as a promising therapeutic strategy.The study also emphasizes the significance of novel neuroinflammatory and neurovascular biomarkers in understanding and managing long-term sequelae in septic survivors.展开更多
Objectives:Photodynamic therapy(PDT)is a minimally invasive method used in the treatment of various cancers and skin diseases,but it is not widely used in bone cancer,where the current therapy is often not effective a...Objectives:Photodynamic therapy(PDT)is a minimally invasive method used in the treatment of various cancers and skin diseases,but it is not widely used in bone cancer,where the current therapy is often not effective and accompanied by side effects.Alternative and more effective therapies like PDT are needed.In this in-vitro study,the effect of the photosensitizer(PS)chlorin e6(Ce6)on cancerous bone tumor cells using PDT was examined.Methods:A total of 27 tissue specimens from patients with primary bone cancers or bone metastases of different origins were genetically characterized and treated with PDT.Following a 24-h incubation,cell viability was determined,and the effect of PDT on cell migration was analyzed over 48 h.Results:We could demonstrate that the effect on proliferation of PDT in combination with the PS Ce6 was best in cells isolated from primary osteosarcoma and in bone metastases from mammary carcinomas.Besides proliferation,PDT was also effective in inhibiting the migration of these cells.A statistically significant correlation between the PDT effect and CD164 gene expression was detected,indicating that a high expression of this gene could result in a higher effectiveness of the photodynamic treatment.Conclusion:This study analyzes for the first time the effect of PDT in bone cancers and metastases and shows the potential of treating these cancer types with Ce6 PDT.展开更多
This review focuses on the roles antioxidants and prooxidants in colorectal cancer(CRC).Considerable evidence suggests that environmental factors play key roles in the incidence of sporadic CRC.If pro-oxidant factors ...This review focuses on the roles antioxidants and prooxidants in colorectal cancer(CRC).Considerable evidence suggests that environmental factors play key roles in the incidence of sporadic CRC.If pro-oxidant factors play an etiological role in CRC it is reasonable to expect causal interconnections between the wellcharacterized risk factors for CRC,oxidative stress and genotoxicity.Cigarette smoking,a high dietary consumption of n-6 polyunsaturated fatty acids and alcohol intake are all associated with increased CRC risk.These risk factors are all pro-oxidant stressors and their connections to oxidative stress,the intestinal microbiome,intestinal microfold cells,cyclooxygenase-2 and CRCare detailed in this review.While a strong case can be made for pro-oxidant stressors in causing CRC,the role of food antioxidants in preventing CRC is less certain.It is clear that not every micronutrient with antioxidant activity can prevent CRC.It is plausible,however,that the optimal food antioxidants for preventing CRC have not yet been critically evaluated.Increasing evidence suggests that RRR-gamma-tocopherol(the primary dietary form of vitamin E)or other"non-alpha-tocopherol"forms of vitamin E(e.g.,tocotrienols)might be effective.Aspirin is an antioxidant and its consumption is linked to a decreased risk of CRC.展开更多
BACKGROUND Colorectal cancer(CRC)is a leading cause of cancer-related mortality worldwide,primarily due to tumor heterogeneity and treatment resistance.The leucine-rich repeat-containing protein 19(LRRC19)has been lin...BACKGROUND Colorectal cancer(CRC)is a leading cause of cancer-related mortality worldwide,primarily due to tumor heterogeneity and treatment resistance.The leucine-rich repeat-containing protein 19(LRRC19)has been linked to immune regulation and tumor suppression,yet its specific role in CRC remains poorly understood.AIM To investigate the tumor-suppressive role of LRRC19 in CRC,focusing on cell cycle,immune microenvironment,and chemotherapy response.METHODS Bioinformatics analyses of Gene Expression Omnibus and The Cancer Genome Atlas databases identified differentially expressed genes in CRC.LRRC19 exp-ression was validated in CRC tissues and cell lines by quantitative PCR,immuno-histochemistry,and Western blotting.Functional assays,including proliferation,soft agar colony formation,flow cytometry,and xenograft models,assessed biological effects.Mechanistic studies with dual-luciferase reporter assays,molecular docking,and drug sensitivity testing explored LRRC19’s interaction with the cyclin-dependent kinase 6(CDK6)/E2F1 axis and oxaliplatin(OXA)response.Single-cell sequencing and immune infiltration analyses assessed its impact on the immune microenvironment.RESULTS LRRC19 expression was significantly downregulated in CRC and associated with poor prognosis.Overexpression of LRRC19 inhibited CRC cell proliferation,induced G0/G1 phase arrest,and suppressed tumor growth in vivo.Mechanistically,LRRC19 suppressed CDK6 transcription by downregulating E2F1,leading to cell cycle arrest.Additionally,LRRC19 promoted immune cell infiltration,particularly B cells and CD4+T cells,while decreasing immunosuppressive cells.LRRC19 also sensitized CRC cells to OXA,enhancing chemotherapy efficacy.CONCLUSION LRRC19 suppresses CRC by targeting the CDK6/E2F1 axis,modulating the immune microenvironment,and enhancing chemotherapy sensitivity,making it a promising therapeutic target for precision medicine in CRC.展开更多

The number “1” is common to all sequences, expressing the second law of Chargaff. The other 2 numbers are related to each specific DNA sequence case characterizing life species. For example, the entire human genome is characterized by three remarkable numbers 1, 2, and Phi = 1.618 the golden ratio. Associated with each of these three numbers, we can match three axes of symmetry, then “imagine” a kind of hyperspace formed by these codon populations. Then we revisit the value (3-Phi)/2 which is probably universal and common to both the scale of quarks and atomic levels, balancing and tuning the whole human genome codon population. Finally, we demonstrate a new kind of duality between “form and substance” overlapping the whole human genome: we will show that—simultaneously with the duality between genes and junk DNA—there is a second layer of embedded hidden structure overlapping all the DNA of the whole human genome, dividing it into a second type of duality information/redundancy involving golden ratio proportions.
文摘Complex genetic relationships between neurodegenerative disorders and neuropsychiatric symptoms have been shown, suggesting shared pathogenic mechanisms and emphasizing the potential for developing common therapeutic targets. Apolipoprotein E(APOE) genotypes and their corresponding protein(Apo E) isoforms may influence the biophysical properties of the cell membrane lipid bilayer. However, the role of APOE in central nervous system pathophysiology extended beyond its lipid transport function. In the present review article, we analyzed the links existing between APOE genotypes and the neurobiology of neuropsychiatric symptoms in neurodegenerative and vascular diseases. APOE genotypes(APOE ε2, APOE ε3, and APOE ε4) were implicated in common mechanisms underlying a wide spectrum of neurodegenerative diseases, including sporadic Alzheimer's disease, synucleinopathies such as Parkinson's disease and Lewy body disease, stroke, and traumatic brain injury. These shared pathways often involved neuroinflammation, abnormal protein accumulation, or responses to acute detrimental events. Across these conditions, APOE variants are believed to contribute to the modulation of inflammatory responses, the regulation of amyloid and tau pathology, as well as the clearance of proteins such as α-synuclein. The bidirectional interactions among Apo E, amyloid and mitochondrial metabolism, immunomodulatory effects, neuronal repair, and remodeling underscored the complexity of Apo E's role in neuropsychiatric symptoms associated with these conditions since from early phases of cognitive impairment such as mild cognitive impairment and mild behavioral impairment. Besides Apo E-specific isoforms' link to increased neuropsychiatric symptoms in Alzheimer's disease(depression, psychosis, aberrant motor behaviors, and anxiety, not apathy), the APOE ε4 genotype was also considered a significant genetic risk factor for Lewy body disease and its worse cognitive outcomes. Conversely, the APOE ε2 variant has been observed not to exert a protective effect equally in all neurodegenerative diseases. Specifically, in Lewy body disease, this variant may delay disease onset, paralleling its protective role in Alzheimer's disease, although its role in frontotemporal dementia is uncertain. The APOE ε4 genotype has been associated with adverse cognitive outcomes across other various neurodegenerative conditions. In Parkinson's disease, the APOE ε4 allele significantly impacted cognitive performance, increasing the risk of developing dementia, even in cases of pure synucleinopathies with minimal co-pathology from Alzheimer's disease. Similarly, in traumatic brain injury, recovery rates varied, with APOE ε4 carriers demonstrating a greater risk of poor long-term cognitive outcomes and elevated levels of neuropsychiatric symptoms. Furthermore, APOE ε4 influenced the age of onset and severity of stroke, as well as the likelihood of developing stroke-associated dementia, potentially due to its role in compromising endothelial integrity and promoting blood–brain barrier dysfunction.

文摘The well known finite elements Regge calculus is transformed to a triangulation in the hyperbolic plane using fractal Rindler wedges as tiling elements. The final result is an expanding de Sitter hyperbolic, i.e. Gauss-Bolyai-Lobachevsky universe with dark energy and ordinary energy densities in full agreement with cosmic observations and measurements. In the course of obtaining this vital result, the work addresses fundamental points connected to a host of subjects, namely Hardy’s quantum entanglement, an extension of Turing’s machine to a transfinite version, the phenomenon of measure concentration in the context of Banach-like spaces with high dimensionality as well as the pioneering work on the relation between quantum entanglement and computational efficiency.

文摘Dorsal root ganglia neurons gradually lose their axonal regeneration ability during development and aging.To explore molecules that enhance axonal regeneration,we screened growth factors with differential gene expression patterns in the dorsal root ganglias of young adult and aged animals following sciatic nerve injury.In young adult animals,two transforming growth factor beta-related factors,activin A and angiopoietin 2,were found to be upregulated post nerve injury.Treatment of isolated dorsal root ganglia explants and cultured dorsal root ganglia neurons of neonatal and young adult rats with recombinant activin A or angiopoietin 2 protein stimulated neurite outgrowth and axonal elongation.The administration of recombinant activin A or angiopoietin 2 protein to sciatic nerve crush-injured dorsal root ganglias also supported the growth of sensory neurons and facilitated nerve regeneration in both young adult and aged rats.Using RNA sequencing,we characterized genetic changes in dorsal root ganglia neurons following recombinant activin A or angiopoietin 2 treatment,revealing the unique mechanisms of these transforming growth factor beta-related factors.Recombinant activin A elicited changes in the gene expression of cytoskeleton-related Gper1 and activated extracellular signal-regulated kinase signaling,while angiopoietin 2 increased the expression of the transcription factor gene E2f2.Our identification of activin A and angiopoietin 2 as crucial promotional factors of axonal regeneration may guide future therapeutic strategies for the treatment of nerve injury.

文摘Reciprocity may be understood as relation of action and reaction in the sense of Hegel’s philosophical definition. Quoting Kant, freedom and ethical necessities are reciprocally limited. In this contribution, a more mathematical than philosophical reflection about reciprocity as an ever-present dual property of everything was given. As a crystallographer, the author is familiar with the action of Fourier transforms and the relation between a crystal lattice and its reciprocal lattice, already pointing to the duality between particles and waves. A generalization of the reciprocity term was stimulated by results of the famous Information Relativity (IR) theory of Suleiman with its proven physical manifestation of matter-wave duality, compared to the set-theoretical E-Infinity theory developed by El Naschie, where the zero set represents the pre-quantum particle, and the pre-quantum wave is assigned to the empty set boundary surrounding the pre-particle. Expectedly, the most irrational number of the golden mean is involved in these thoughts, because this number is intimately connected with its inverse. An important role plays further Hardy’s maximum quantum entanglement probability as the fifth power of φand its connection to the dark matter. Remembering, the eleven dimensions in Witten’s M-theory may be decomposed into the Lucas number L5 = 11 = φ−5 – φ5. Reciprocity is indeed omnipresent in our world as piloting waves that accompany all observable earthen and cosmic matter. As a side effect of the IR theory some fundamental constants such as the gyromagnetic factor of the electron, Sommerfeld’s fine-structure constant as well as the charge of the electron must be marginally changed caused by altered relativistic corrections. Consequences also arise for our vision about the evolution of life and consciousness.

文摘Objective:To observe the efficacy of LIU’s infantile Tuina(Chinese therapeutic massage)combined with Bi Yan Gao in treating allergic rhinitis(AR)in kids and the impact on serum inflammatory factors.Methods:A total of 120 kids with AR were recruited and divided into a Tuina group,a Bi Yan Gao group,and a combined group using the random number table method,with 40 cases in each group.The three groups all received oral desloratadine syrup as the basic intervention.In addition,the Tuina group was treated with LIU’s infantile Tuina therapy once daily;the BI Yan Gao group received external application of Bi Yan Gao,once every morning and night;the combined group was intervened by LIU’s infantile Tuina plus Bi Yan Gao external application.The intervention lasted for 8 weeks.Before and after the intervention,the total nasal symptom score(TNSS)was compared,and the serum levels of interleukin(IL)-4,IL-6,IL-8,and immunoglobulin E(IgE)were detected.The total effective rate was also compared among the three groups.Results:After 8 weeks of treatment,the total effective rate was 87.2%in the combined group,60.5%in the Tuina group,and 57.5%in the Bi Yan Gao group,showing significant between-group differences(P<0.05).The contents of serum IL-6,IL-8,and IgE were lower in the combined group compared to the Tuina and Bi Yan Gao groups,presenting statistical significance(P<0.05).The level of IL-4 was statistically parallel across the three groups(P>0.05).Conclusion:The combined use of LIU’s infantile Tuina and Bi Yan Gao can improve nasal symptoms and lower the levels of serum inflammatory factors and IgE in kids with AR,producing a higher total effective rate compared to the separate use of the two approaches.

文摘Low rectal cancer is traditionally treated by abdominoperineal resection. In recent years, several new techniques for the treatment of very low rectal cancer patients aiming to preserve the gastrointestinal continuity and to improve both the oncological as well as the functional outcomes, have been emerged. Literature suggest that when the intersphincteric resection is applied in T1-3 tumors located within 30-35 mm from the anal verge, is technically feasible, safe, with equal oncological outcomes compared to conventional surgery and acceptable quality of life. The Anterior Perineal Plan E for Ultra-low Anterior Resection technique, is not disrupting the sphincters, but carries a high complication rate, while the reports on the oncological and functional outcomes are limited. Transanal Endoscopic Micro Surgery(TEM) and Trans Anal Minimally Invasive Surgery(TAMIS) should represent the treatment of choice for T1 rectal tumors, with specific criteria according to the NCCN guidelines and favorable pathologic features. Alternatively to the standard conventional surgery, neoadjuvant chemo-radiotherapy followed by TEM or TAMIS seems promising for tumors of a local stage T1sm2-3 or T2. Transanal Total Mesorectal Excision should be performed only when a board approved protocol is available by colorectal surgeons with extensive experience in minimally invasive and transanal endoscopic surgery.

文摘Background:Sepsis,a life-threatening systemic inflammatory response to infection,presents significant challenges in the intensive care unit(ICU).The condition is frequently accompanied by persistent neurodegenerative changes and muscle atrophy,especially in patients with burn wounds,severely affecting recuperation and quality of life.Traditional physical therapy and advanced sedative screening represent pivotal approaches for the rehabilitation and management of these cases.Methods:A comprehensive transcriptomic study was conducted using gene expression datasets from sepsis patients and important biological models to identify potential targets.We subsequently employed virtual molecular docking and molecular dynamics simulations to screen the FDA drug library and traditional Chinese medicine databases for compounds exhibiting high binding affinity to isocitrate dehydrogenase 2(IDH2).The functional role of this target was validated in vitro using an IDH2 deficient BV-2 microglial cell model stimulated by lipopolysaccharide.Results:Key features were identified through differential expression analysis across multiple datasets.Gene set enrichment analysis(GSEA)highlighted pathways associated with chronic neurodegenerative diseases.Stable interactions between IDH2 and cobicistat or licorice glycoside E were predicted,supporting the identification of IDH2 as a promising therapeutic target.Knockdown of IDH2 in microglial cells led to markedly suppressed production of pro-inflammatory cytokines,including TNF-αand IL-6,confirming its pivotal role in neuroinflammatory response regulation.Conclusion:These findings underscore the pivotal role of IDH2 in persistent neurodegenerative changes associated with sepsis,including burn-related cases.Targeting IDH2 may reduce long-term neurovascular complications and muscle atrophy in septic ICU patients.Our findings suggest that IDH2 is a potential therapeutic target,and the combination of cobicistat and licorice glycoside E warrants further investigation as a promising therapeutic strategy.The study also emphasizes the significance of novel neuroinflammatory and neurovascular biomarkers in understanding and managing long-term sequelae in septic survivors.
文摘Objectives:Photodynamic therapy(PDT)is a minimally invasive method used in the treatment of various cancers and skin diseases,but it is not widely used in bone cancer,where the current therapy is often not effective and accompanied by side effects.Alternative and more effective therapies like PDT are needed.In this in-vitro study,the effect of the photosensitizer(PS)chlorin e6(Ce6)on cancerous bone tumor cells using PDT was examined.Methods:A total of 27 tissue specimens from patients with primary bone cancers or bone metastases of different origins were genetically characterized and treated with PDT.Following a 24-h incubation,cell viability was determined,and the effect of PDT on cell migration was analyzed over 48 h.Results:We could demonstrate that the effect on proliferation of PDT in combination with the PS Ce6 was best in cells isolated from primary osteosarcoma and in bone metastases from mammary carcinomas.Besides proliferation,PDT was also effective in inhibiting the migration of these cells.A statistically significant correlation between the PDT effect and CD164 gene expression was detected,indicating that a high expression of this gene could result in a higher effectiveness of the photodynamic treatment.Conclusion:This study analyzes for the first time the effect of PDT in bone cancers and metastases and shows the potential of treating these cancer types with Ce6 PDT.
文摘This review focuses on the roles antioxidants and prooxidants in colorectal cancer(CRC).Considerable evidence suggests that environmental factors play key roles in the incidence of sporadic CRC.If pro-oxidant factors play an etiological role in CRC it is reasonable to expect causal interconnections between the wellcharacterized risk factors for CRC,oxidative stress and genotoxicity.Cigarette smoking,a high dietary consumption of n-6 polyunsaturated fatty acids and alcohol intake are all associated with increased CRC risk.These risk factors are all pro-oxidant stressors and their connections to oxidative stress,the intestinal microbiome,intestinal microfold cells,cyclooxygenase-2 and CRCare detailed in this review.While a strong case can be made for pro-oxidant stressors in causing CRC,the role of food antioxidants in preventing CRC is less certain.It is clear that not every micronutrient with antioxidant activity can prevent CRC.It is plausible,however,that the optimal food antioxidants for preventing CRC have not yet been critically evaluated.Increasing evidence suggests that RRR-gamma-tocopherol(the primary dietary form of vitamin E)or other"non-alpha-tocopherol"forms of vitamin E(e.g.,tocotrienols)might be effective.Aspirin is an antioxidant and its consumption is linked to a decreased risk of CRC.

文摘BACKGROUND Colorectal cancer(CRC)is a leading cause of cancer-related mortality worldwide,primarily due to tumor heterogeneity and treatment resistance.The leucine-rich repeat-containing protein 19(LRRC19)has been linked to immune regulation and tumor suppression,yet its specific role in CRC remains poorly understood.AIM To investigate the tumor-suppressive role of LRRC19 in CRC,focusing on cell cycle,immune microenvironment,and chemotherapy response.METHODS Bioinformatics analyses of Gene Expression Omnibus and The Cancer Genome Atlas databases identified differentially expressed genes in CRC.LRRC19 exp-ression was validated in CRC tissues and cell lines by quantitative PCR,immuno-histochemistry,and Western blotting.Functional assays,including proliferation,soft agar colony formation,flow cytometry,and xenograft models,assessed biological effects.Mechanistic studies with dual-luciferase reporter assays,molecular docking,and drug sensitivity testing explored LRRC19’s interaction with the cyclin-dependent kinase 6(CDK6)/E2F1 axis and oxaliplatin(OXA)response.Single-cell sequencing and immune infiltration analyses assessed its impact on the immune microenvironment.RESULTS LRRC19 expression was significantly downregulated in CRC and associated with poor prognosis.Overexpression of LRRC19 inhibited CRC cell proliferation,induced G0/G1 phase arrest,and suppressed tumor growth in vivo.Mechanistically,LRRC19 suppressed CDK6 transcription by downregulating E2F1,leading to cell cycle arrest.Additionally,LRRC19 promoted immune cell infiltration,particularly B cells and CD4+T cells,while decreasing immunosuppressive cells.LRRC19 also sensitized CRC cells to OXA,enhancing chemotherapy efficacy.CONCLUSION LRRC19 suppresses CRC by targeting the CDK6/E2F1 axis,modulating the immune microenvironment,and enhancing chemotherapy sensitivity,making it a promising therapeutic target for precision medicine in CRC.

文摘Using Dvoretzky’s theorem in conjunction with Bohm’s picture of a quantum particle inside a guiding quantum wave akin to De Broglie-Bohm pilot wave we derive Einstein’s famous formula E = mc2 as the sum of two parts E(O) = mc2/22 of the quantum particle and E(D) = m c2 (21/22) of the quantum wave where m is the mass, c is the speed of light and E is the energy. In addition we look at the problem of black holes information in the presence of extra dimensions where it seems initially that extra dimensions would logically lead to a hyper-surface for a black hole and consequently a reduction of the corresponding information density due to the dilution effect of these additional dimensions. The present paper argues that the counterintuitive opposite of the above is what should be expected. Again this surprising result is a consequence of the same well known theorem on measure concentration due to I. Dvoretzky. We conclude that there are only two real applications of the theorem and we expect that many more applications in physics and cosmology will be found in due course.
