Common gastrointestinal diseases such as radiation enteritis(RE),acute pancreatitis,inflammatory bowel diseases(IBD)and drug-induced hepatotoxicity share pathophysiological mechanisms at the molecular level,mostly inv...Common gastrointestinal diseases such as radiation enteritis(RE),acute pancreatitis,inflammatory bowel diseases(IBD)and drug-induced hepatotoxicity share pathophysiological mechanisms at the molecular level,mostly involving the activation of many pathways of the immune response,ultimately leading to tissue injury.Increased oxidative stress,inflammatory cytokine release,inflammatory cell infiltration and activation and the up-regulation of inflammatory transcription factors participate in the pathophysiology of these complex entities.Treatment varies in each specific disease,but at least in the cases of RE and IBD immunosuppressors are effective.However,full therapeutic responses are not always achieved.The pathophysiology of ischemiareperfusion(IR)injury shares many of these mechanisms.Brief and repetitive periods of ischemia in an organ or limb have been shown to protect against subsequent major IR injury in distant organs,a phenomenon called remote ischemic preconditioning(RIP).This procedure has been shown to protect the gut,pancreas and liver by modulating many of the same inflammatory mechanisms.Since RIP is safe and tolerable,and has shown to be effective in some recent clinical trials,I suggest that RIP could be used as a physiologicallyrelevant adjunct treatment for non-ischemic gastrointestinal inflammatory conditions.展开更多

With the discrete equation regarded as an atomic lattice with a three-atom potential, two accurate artificial boundary conditions are first derived here. Reflection co- efficient and numerical tests illustrate the capability of the proposed methods. In particular, the time history treatment gives an exact boundary condition, yet with sensitivity to nu- merical implementations. We establish the weighted type (p, p) estimates for 1 〈 p 〈 ∞ and the weighted weak type (1, 1) estimate for the oscillation and variation operators of singular integrals with kernels satisfying certain HSrmander type conditions, which contain the Riesz transforms, singular integrals with more general homogeneous kernels satisfying the Lipschitz conditions and the classical Dini's conditions as model examples. Meanwhile, we also obtain the weighted LP-boundeness for such operators associated to the family of commutators generated by the singular integrals above with BMO(Rd)-functions.展开更多

The SOS relaxation method is used to compute approximate invariants and approximate rank- ing functions with floating point coefficients. Then Gauss- Newton refinement and rational vector recovery are applied to approximate polynomials to obtain candidate polynomials with rational coefficients, which exactly satisfy the conditions of invariants and ranking functions. 文摘目的探究六味地黄汤加减联合达格列净用于糖尿病肾病(Diabetic nephropathy,DN)的疗效。方法80例DN患者依据随机原则分成两组,每组40例。对照组予达格列净片治疗,研究组予达格列净片联合六味地黄汤加减治疗,均持续治疗8周。对比两组血糖水平、肾功能、血清炎症因子水平及安全性。结果治疗后,两组空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)水平与血清肌酐(Scr)、血尿素氮(BUN)、尿白蛋白排泄率(UAER)、降钙素原(PCT)、超敏C反应蛋白(hs-CRP)水平均较治疗前下降,且研究组较对照组低,差异有统计学意义(P<0.05);研究组严重不良反应总发生率5.00%(2/40)低于对照组的10.00%(4/40),差异无统计学意义(P>0.05)。结论联合运用六味地黄汤与达格列净能更好地调节DN患者的血糖水平,促进肾功能改善,减轻炎症反应,且安全性高。
