Methods: Since September 1998, Endovascular Graft Exclusion for thoracic aortic dissection has been performed on 10 patients. Graft was constructed from self-expanding Z-stents covered with a woven Dacron polyester fabric graft. Ged dimensions were determined ftom spiral computed tomographic scans. All operations were performed under DSA guidance. Results: There was one early death resulting from endoleaks. Proedures in the other 9 patients succeeded. No complications such as myocardial infarction, lung failure, kidney failureand paralysis that commonly occurred ther conventional operations were obsered. Immediate thrombosis in false lumen was achieved in 6 patients, and late thrombosis occurred in 3 patients. Mean follow-up duration was 3 months, the aneurysmal diameter was decreased obviously. Conclusion: These early results support the hypothesis that Endovascular Graft Exclusion may be a safe and durable treatment for selected patients with theracic aortic dissection. Endoleak may allow continued aneurysmal expansion and rupture. Further follow-up is necessary to evaluate the true long-term effectiveness of this procedure.展开更多

Methods We studied 887 patients with 1 045 non-in-stent restenosis lesions in 977 native arteries undergoing DES implantation with IVUS imaging, and compared the dissected stent end to the non-dissected stent end. Results Eighty-two dissections were detected; 51.2% (42/82) involved the proximal and 48.8% (40/82) the distal stent edge. When compared to the non-dissected stent end, residual plaque area [(8.0±4.3) mm2 vs (5.2±3.0) mm2, P【0.01], plaque burden [(52±12)% vs (36±15)%, P【0.01], plaque eccentricity (8.4±5.5 vs 4.0±3.4, P【0.01), and stent edge symmetry (1.17±0.11 vs 1.14±0.08, P=0.02) were larger; plaque burden≥50% was more frequent (62% vs 17%, P【0.01) and calcium deposits (52.5% vs 35.6%, P=0.03) more common; and the lumen/stent area (0.86±0.16 vs 1.02±0.18, P【0.01) was smaller in the stent dissected end. Independent predictors of stent edge dissection were residual plaque eccentricity (OR=1.3, P【0.01) and residual plaque burden≥50% (OR=7.3, P【0.01). Intramural hematomas occurred in 34.1% (28/82) of dissections.Independent predictors of intramural hematomas were plaque eccentricity (OR=1.4, P=0.005), plaque burden≥50% (OR=7.1, P=0.02), and mean lumen diameter to stent diameter ratio (OR=0.37, P=0.04).Concluslon IVUS identified edge dissections after 9.4% of DES implantations. Residual plaque eccentricity and significant plaque burden predicted coronary stent edge dissections. Dissections in less diseased reference segments with an arc of normal vessel wall (greater plaque eccentricity) more often evolved into an intramural hematoma.展开更多

Accordingly, precise handling of the dissected lymph nodes is important for histopathological work-up. The authors have developed a lymph node plastic tray shaping the abdomen and pelvis in which the dissected lymph nodes are placed in its determined location. This can be applied for any urologic cancer surgery. The research was designed to test the usage of a new histological tray. The objective was to assess how helpful it was for the surgical team and in the pathological process. The newly developed lymph node tray has been applied in 150 urological cancer surgeries and its efficacy and outcome have been evaluated in all these cases by involved doctors and assistants. This new tray simplifies lymph node removing and identification (staging), making it safer and quicker in any uro-oncological surgery. It facilitates the work of the pathologist and the flow of reliable information along the surgeon—pathologist-oncologist team. With usage of the tray, lymph node dissections are more structured by methodical means compared to any of the present methods.展开更多

The incidence and outcome of edge dissections after coronary stent implantation were reportedly different using different imaging techniques. We used optical coherence tomography (OCT) to assess the incidence, morphological findings and related factors of edge dissections after drug-eluting stent (DES) implantation. Methods Totally 42 patients with 43 de novo lesions in 43 native arteries undergoing DES implantation with OCT imaging were enrolled in this study. Results Nine edge dissections were detected in 43 arteries after DES implantation. There were four morphological patterns of stent edge dissections indentified in this study: (1) superficial intimal tears (n=3), (2) subintimal dissections (n=4), (3) split of media (n=1), (4) disruption of the fibrotic cap of plaque (n=1). Stent edge expansion and stent expansion were both higher in the group with dissections than those in the group without dissections (1.682±0.425 vs. 1.229±0.285, P=0.0290; 1.507±0.445 vs. 1.174±0.265, P=0.0072). Conclusions The incidence of stent edge dissections detected by OCT was 21%. Stent edge dissection is related with stent edge expansion and stent expansion.展开更多

This study aimed to evaluate the immediate and long-term outcomes of hybrid procedures for TAAA in high-risk patients. Methods Between September 1998 and May 2012, 32 high-risk TAAA patients (five females, median age 61.5 years) underwent hybrid procedures at a single institution. Simultaneous approach and staged approach were performed on the basis of patients' conditions. Follow-up computed tomography angiography (CTA) was routinely performed before discharge and at 6, 12 months and annually thereafter. Results Procedural success was achieved in all cases. The median hospital stay was (21.5+2.3) days, and the median procedure time was (420+31) minutes. Blood loss averaged (2100_+261) ml. A total of 124 visceral artery bypasses was performed. Two patients (6.3%) died within 30 days. One patient exhibited complete paraplegia (3.1%). The visceral graft patency was 96.1% at 3 years. All-cause survival rates were 93.8%, 87.5%, 81.3% and 53.1% at 1, 2, 3 and 5 years, respectively. No patient died due to aortic events. The freedom rates from aortic events were 96.9%, 93.6%, 87.5%, 68.8% at 1,2, 3 and 5 years, respectively. Conclusions The results of visceral hybrid repair for high-risk patients with complex TAAAs are encouraging. However, the procedure is still a significant physiological insult to patients. Until branched and fenestrated endovascular repair become more common, hybrid procedure will continue to have a role in high-risk patients.展开更多

However, i...Transradial access (TRA) has emerged as the preferred vascular access route forcoronary angiography and percutaneous coronary interventions due to itssuperior safety profile compared to transfemoral access. However, its widespreadadoption raises concerns regarding structural alterations in the radial artery, which may impact long-term vascular health and future procedural feasibility.TRA is associated with histopathologic changes in the arterial wall, such asintimal injury and hyperplasia, medial remodeling and adventitial inflammation, collectively contributing to radial artery remodeling. Moreover, TRA can inducechanges in radial artery lumen diameter driven by an inflammatory response dueto arterial puncture and mechanical friction during the procedure. Nonetheless, amore clinically significant consequence is radial artery occlusion, which is influencedby various procedural and patient-related factors. Strategies to minimizeremodeling include meticulous pre-procedural ultrasound assessment to ensureappropriate sheath-to-artery size matching, periprocedural pharmacologicalinterventions and implementation of patent hemostasis techniques. This reviewsynthesizes current knowledge regarding the mechanisms, clinical implications, and preventive strategies related to radial artery remodeling following TRA. Further research is needed toelucidate the long-term consequences of radial artery remodeling and to refine preventive strategies for preservingradial artery patency and its suitability for future interventions.展开更多

文摘Objective: To assess the operation indications, preoperative evaluation, technique essential and clinical prospective of Endovascular Graft Exclusion for thoracic aortic dissection. Further follow-up is necessary to evaluate the true long-term effectiveness of this procedure.
文摘Non-A non-B aortic dissection(AAD)is an infrequently documented condition, comprising of only a small proportion of all AADs.The unique anatomy of the aortic arch and the failure of the existing classifications to adequately define individuals with non-A non-B AAD,have led to an ongoing controversy around the topic. It seems that the clinical progression of acute non-A non-B AAD diverges from the typical type A and B dissections, frequently leading to serious complications and thus mandating early intervention.Currently, the available treatment methods in the surgical armamentarium are conventional open, endovascular techniques and combined hybrid methods. The optimum approach is tailored in every individual case and may be determined by the dissection’s location, extent, the aortic diameter, the associated complications and the patient’s status. The management of non-A non-B dissections still remains challenging and a unanimous consensus defining the gold standard treatment has yet to be reached. In an attempt to provide further insight into this perplexing entity,we performed a minireview of the literature, aiming to elucidate the epidemiology, clinical course and the optimal treatment modality.
文摘Objective We used intravascular ultrasound (IVUS) to assess incidence, predictors, morphology, and angiographic findings of edge dissections and intramural hematomas after drug-eluting stent (DES) implantation. Dissections in less diseased reference segments with an arc of normal vessel wall (greater plaque eccentricity) more often evolved into an intramural hematoma.
文摘In any urologic cancer surgery, lymph node dissection and its processing play a significant role in staging and management of the patients. With usage of the tray, lymph node dissections are more structured by methodical means compared to any of the present methods.
文摘We value the interest in our work(Schipani et al)1 expressed by Zhang et al2 in their recent letter to the editor. We welcome the oppor-tunity to respond and offer the following clarifications.Traditionally, spontaneous and trau-matic cervical artery dissections(CeAD)have been classified separately. However, despite conflicting results from observational cohorts,3 there is currently no convincing evidence supporting significant differences in the underlying pathophysiology between the two types. Therefore,we believe this distinc-tion to be arbitrary.
文摘We recently read with great interest the article‘Sex differences in the epidemiology of spontaneous and traumatic cervical artery dissections’.1 The authors conducted a retrospective analysis of 144 patients with cervical artery dissection(CeAD),revealing important sex-related differences that offer valuable insights for clinicians.Despite the quality of the work,we believe the study has several limitations that should be addressed.
文摘Background Recent studies show cervical artery dissection(CeAD)is equally common between sexes, and that the incidence of CeAD has risen at a greater rate in females than males. In this population-based study,we identify sex differences in patients diagnosed with spontaneous and traumatic CeAD.Methods 144 patients with spontaneous or traumatic CeAD were studied for sex differences in medical comorbidities, presenting symptoms and outcomes. Results Females were more likely to carry a diagnosis of migraine, while males were more likely to have hyperlipidaemia.Females were more likely to present with neck pain, males with stroke.Females were significantly more likely to develop recurrent dissections in the study period. Conclusions These findings underscore the importance of understanding CeAD through the lens of sex differences and may explain the significant rise in the diagnosis of CeAD in females. These findings support the importance of considering sex-specific risk factors and medical comorbidities with sex predilection in the diagnosis and management of CeAD.Furthermore,it emphasises the importance of female patients understanding risk factors and presenting signs that should prompt evaluation for CeAD.
文摘Background Edge dissections after coronary stent implantation are associated with increased short-term risk of major adverse cardiovascular events. Stent edge dissection is related with stent edge expansion and stent expansion.

文摘Early-stage gastric cancer;Extent of lymph node dissection;D1 dissection;D2 dissection;Recurrence rate;Survival rateObjective:To investigate the impact of different lymph node dissection scopes on postoperative recurrence and survival rates in patients with early gastric cancer, providing evidence-based support for optimizing clinical surgical plans. Methods:A retrospective analysis was conducted on the clinical data of 100 patients with early gastric cancer who underwent surgical treatment at our hospital from October 2021 to October 2023.Patients were divided into Group D1(n=50)and Group D2(n=50)based on the extent of lymph node dissection.Group D1 underwent limited lymph node dissection(dissection of the first station of lymph nodes around the stomach),while Group D2 underwent standard lymph node dissection(dissection of the first and second stations of lymph nodes around the stomach).Surgical-related indicators, the incidence of postoperative complications, the 2-year recurrence rate, and the 2-year survival rate were compared between the two groups of patients. Results:The operative time, intraoperative blood loss, postoperative hospital stay, and the number of lymph nodes dissected were significantly higher in the D2 group than in the D1 group(all P<0.001).The overall incidence of postoperative complications was higher in the D1 group than in the D2 group, but the difference was not statistically significant(χ^(2)=0.884,P=0.766).After a 2-year follow-up,the recurrence rate was significantly higher in the D1 group than in the D2 group(χ^(2)=4.000,P=0.046).The 2-year survival rate was significantly lower in the D1 group than in the D2 group(χ^(2)=5.005,P=0.025).A total of 100 patients with early-stage gastric cancer were grouped according to the depth of invasion, degree of differentiation, and lymph node metastasis status, and the recurrence rates of different subgroups were compared. The results showed that the recurrence rate was higher in patients with T1b stage than in those with T1a stage(χ^(2)=5.005,P=0.025),higher in poorly differentiated patients than in moderately and well-differentiated patients(χ^(2)=4.155,P=0.042),and higher in patients with lymph node metastasis than in those without lymph node metastasis(χ^(2)=4.512,P=0.034).Conclusion:Compared with D1 limited lymph node dissection,D2 standard lymph node dissection can significantly reduce the postoperative recurrence rate and improve the 2-year survival rate in patients with early-stage gastric cancer without significantly increasing the risk of postoperative complications.Although the surgical trauma is slightly greater, the overall prognosis is better, making it a preferred surgical treatment option for patients with early-stage gastric cancer.

文摘Debris flows have increased in frequency within the arid Daheba Basin on the northeastern Tibetan Plateau, but their sediment sources remain poorly quantified.Using high-resolution UAV-derived DEMs from 51 small catchments, this study evaluates the relative contributions of landslide-derived and channel-derived sediment in controlling debris-flow fan magnitude, and quantifies sediment supply during the 2023 rainy season using DEM differencing.A total of 766 landslides occurred predominantly on slopes of 40°-50°and southeast-southwest aspects, generating 36.17×10^(4)m^(3)of material.Gully heads exhibit exceptionally lower landscape dissection thresholds compared with loess and Quaternary regions in China, indicating high susceptibility to failure under intensified runoff. The results show that Landslide area-volume scaling exponent(b)varies with hillslope geometry(K_(u)):b>1.3 for K_(u)<8 and generally b<1.3 for K_(u)>8,indicating more complete scar evacuation upslope and partial erosion downslope.Despite the abundance of landslides, their contribution to debris flow fan magnitude is minor(<25%),with channel debris dominating(>75%).DEM differencing of a small catchment before and after the 2023 rainy season further reveals that sediment supply originates primarily from the main channel(60.6%)and tributaries(23.3%),with smaller contributions from channel banks(6.8%)and channel heads(9.2%).Tributaries exhibit the greatest mean erosion depth(4.2 m),exceeding that of the main channel(3.8 m).These findings demonstrate that debris-flow material supply in the Daheba Basin is transport-limited and controlled mainly by fluvial entrainment rather than slope failures.Climatic warming and wetting may enhance slope instability, but sediment mobilization is dominantly governed by runoff-driven channel erosion. This study underscores the importance of prioritizing channel sediment dynamics in debris flow hazards assessments for arid regions of the Tibetan plateau.

文摘BACKGROUND Inappropriate selection of patients with early gastric cancer(EGC)for endoscopic submucosal dissection(ESD)may lead to non-curative resection, necessitating additional gastrectomy.Conversely, inappropriate selection for gastrectomy may result in overtreatment, adversely affecting patients’quality of life.Few have systematically evaluated the concordance between therapeutic indications under current Japanese guidelines and pathological criteria in EGC.To minimize noncurative resection risks while sparing unnecessary surgery for low-risk patients’,we specifically assess the suitability of Japanese guidelines in non-Japanese populations. This work aims to optimize clinical practice by refining endoscopic treatment criteria for adoption beyond Japan.AIM To evaluate EGC clinical decision accuracy by comparing therapeutic indication with postoperative pathological criteria and analyzing factors influencing discrepancies.METHODS A retrospective analysis was conducted on 796 EGC cases diagnosed at Peking University Third Hospital between January 2010 and December 2022.Cases were categorized into three groups:Same-estimated(preoperative therapeutic indication with postoperative pathological criteria matched),underestimated(preoperative ESD indication but postoperative surgical criteria),and overestimated(preoperative surgical indication but postoperative ESD criteria). The rate of discrepancy and associated risk factors were assessed.RESULTS The accuracy rates of preoperative evaluation for ESD and gastrectomy indications were 73.0%(321/430)and 76.0%(278/366),respectively.The overall discrepancy rate was 25.6%(204/796).Multivariate analysis identified tumor location in the upper-third stomach(odds ratio=2.158,95%confidence interval:1.373-3.390,P=0.001)was significantly associated with a higher likelihood of being underestimated and undifferentiated histologic type on preoperative biopsy(odds ratio=2.005,95%confidence interval:1.036-3.879,P=0.039)was more likely to be overestimated.Significant differences were observed in tumor diameter(P<0.001),depth of infiltration(P<0.001),ulcerative findings(P<0.001),and histologic type(P<0.001)between preoperative and postoperative evaluations.CONCLUSION The accuracy of preoperative EGC indications is 74.4%.Upper-third stomach and undifferentiated histology are primary discrepancy predictors.Upper-third tumors are prone to underestimation, while undifferentiated tumors are prone to overestimation.

文摘Objectives:PSMA PET/CT(Prostate-Specific MembraneAntigen Positron Emission Tomography/Computed Tomography)offers improved accuracy in detecting lymph node invasion(LNI)in prostate cancer(PC)patients, potentially reducing the need for extended pelvic lymph node dissection(ePLND).This study aims to evaluate a patient-tailored care pathway in which ePLND is performed only in patients with unfavorable intermediate-or high-risk PC who are deemed at risk for LNI based on PSMA PET/CT findings. Methods:In this interventional cohort study,81 patients were managed according to the new care pathway.ePLND was omitted in cases of negative PSMA PET/CT findings(N0M0),while those with positive PSMA PET/CT findings(N1M0)underwent ePLND.A comparator group of 81 patients was selected from a prospectively generated database for comparison. Results:The intervention group experienced a 75% reduction in the number of ePLNDs performed compared to the comparator group(p<0.001).ePLND-related complications were significantly lower in the intervention group(p=0.008).No significant difference was observed in 3-year biochemical-recurrence free survival(BRFS)between the two groups(p=0.958).Conclusion:Omitting ePLND in patients with negative PSMA PET/CT findings(N0M0)leads to a substantial reduction in the number of ePLNDs performed, resulting in a decrease in morbidity, without compromising early oncological outcomes.
文摘Objective:Open retroperitoneal lymph node dissection(RPLND)is the gold-standard surgical approach for the management of metastatic testicular cancer, but robotic RPLND is becoming increasingly popular. There is limited research directly comparing open and robotic RPLND.The objective of this systematic review is to identify all the literature with direct comparisons between the open and robotic techniques for RPLND and to compare the two techniques. The primary outcome was peri-operative outcomes, and the secondary outcomes included oncological outcomes and patient demographics. Methods:This systematic review was prospectively registered and was conducted in accordance with the PRISMA statement. The PubMed,Embase and MEDLINE databases were searched for relevant publication from January 2006 to August 2024.Results:Eight studies, totaling 3995 patients, are included in this systematic review, with 3521 patients who underwent open RPLND and 474 who underwent robotic RPLND.For open RPLND,the mean operative duration, blood loss and length of stay were 267.8 min,475 mL and 7.3 d,respectively. For robotic RPLND,the mean operative duration, blood loss and length of stay were 334.5 min,94.6 mL and 3.7 d,respectively.Teratoma was the most common RPLND specimen pathology from both open and robotic surgeries. For open RPLND,the specimens have 13–23 nodes(26–32 mm),whereas the robotic RPLND specimens have 13–28 nodes(18–20 mm). Conclusion:This systematic review suggests that the benefitsof robotic RPLND may be associated with reduced blood loss, shorter hospitalisation and an overall lower risk of minor and major complications while maintaining oncological safety.
文摘Objective To investigate the clinical and radiologic features of patients with spontaneous cerebral arterial dissections(CADs)presenting with ischemic stroke and to explore the effect of gender and age on those features. Methods Patients admitted to our stroke center diagnosed as ischemic stroke secondary to CADs from August 2008to April 2015 were prospectively registered.
文摘Objective:Patients are increasingly turning to the Internet as a source of healthcare information.Given that neck dissection is a common procedure within the field of Otolaryngology-Head and Neck Surgery, the aim of this study was to evaluate the quality and readability of online patient education materials on neck dissection. Methods:A Google search was performed using the term"neck dissection."The first 10 pages of a Google search using the term"neck dissection"were analyzed. The DISCERN instrument was used to assess quality of information.Readability was calculated using the Flesch-Reading Ease,Flesch-Kincaid Grade Level,Gunning-Fog Index,Coleman-Liau Index, and Simple Measure of Gobbledygook Index.Results:Thirty-one online patient education materials were included.Fifty-five percent(n=17)of results originated from academic institutions or hospitals. The mean Flesch-Reading Ease score was 61.2±11.9.Fifty-two percent(n=16)of patient education materials had Flesch-Reading Ease scores above the recommended score of 65.The average reading grade level was 10.5±2.1.The average total DISCERN score was 43.6±10.1.Only 26%of patient education materials(PEMs)had DISCERN scores corresponding to a"good quality"rating. There was a significant positive correlation between DISCERN scores and both Flesch-Reading Ease scores and average reading grade level. Conclusions:The majority of patient education materials were written above the recommended sixth-grade reading level and the quality of online information pertaining to neck dissections was found to be suboptimal. This research highlights the need for patient education materials regarding neck dissection that are high quality and easily understandable by patients.
文摘Background Hybrid procedures including debranching of visceral and renal arteries followed by endovascular exclusion of the thoracoabdominal aortic aneurysm (TAAA) have recently been proposed as a less invasive alternative to conventional TAAA surgery. Until branched and fenestrated endovascular repair become more common, hybrid procedure will continue to have a role in high-risk patients.

文摘Transradial access (TRA) has emerged as the preferred vascular access route forcoronary angiography and percutaneous coronary interventions due to itssuperior safety profile compared to transfemoral access. Further research is needed toelucidate the long-term consequences of radial artery remodeling and to refine preventive strategies for preservingradial artery patency and its suitability for future interventions.

文摘BACKGROUND Esophageal stricture ranks among the most significant complications following endoscopic submucosal dissection(ESD).Excessive fibrotic repair is a typical pathological feature leading to stenosis after ESD.AIM To examine the effectiveness and underlying mechanism of Kangfuxin solution(KFX)in mitigating excessive fibrotic repair of the esophagus post-ESD.METHODS Pigs received KFX at 0.74 mL/kg/d for 21 days after esophageal full circumferential ESD.Endoscopic examinations occurred on days 7 and 21 post-ESD.In vitro, recombinant transforming growth factor(TGF)-β1(5 ng/mL)induced a fibrotic microenvironment in primary esophageal fibroblasts(pEsF).After 24 hours of KFX treatment(at 1.5%,1%,and 0.5%),expression ofα-smooth muscle actin-2(ACTA2),fibronectin(FN),and type collagen I was assessed.Profibrotic signaling was analyzed, including TGF-β1,Smad2/3,and phosphor-smad2/3(p-Smad2/3).RESULTS Compared to the Control group, the groups treated with KFX and prednisolone exhibited reduced esophageal stenosis, lower weight loss rates, and improved food tolerance 21 d after ESD.After treatment,Masson staining revealed thinner and less dense collagen fibers in the submucosal layer.Additionally, the expression of fibrotic effector molecules was notably inhibited.Mechanistically,KFX downregulated the transduction levels of fibrotic functional molecules such as TGF-β1,Smad2/3,and p-Smad2/3.In vitro,pEsF exposed to TGF-β1-induced fibrotic microenvironment displayed increased fibrotic activity, which was reversed by KFX treatment, leading to reduced activation of ACTA2,FN,and collagen I.The 1.5%KFX treatment group showed decreased expression of p-Smad 2/3 in TGF-β1-activated pEsF.CONCLUSION KFX showed promise as a therapeutic option for post-full circumferential esophageal ESD strictures, potentially by suppressing fibroblast fibrotic activity through modulation of the TGF-β1/Smads signaling pathway.
