AIM:To investigate the efficacy and mechanisms of indirect intense pulsed light(IPL)irradiation on meibomian gland dysfunction(MGD).METHODS:A total of 60 MGD patients was included in this prospective randomized contro...AIM:To investigate the efficacy and mechanisms of indirect intense pulsed light(IPL)irradiation on meibomian gland dysfunction(MGD).METHODS:A total of 60 MGD patients was included in this prospective randomized controlled trial.Patients were randomly assigned 1:1 into two groups(3-mm group and 10-mm group)in which IPL was applied at distances from the lower eyelid margin of 3 and 10 mm,respectively.Both groups received three times treatment with 3-week interval.Meibomian gland yield secretion score(MGYSS),standard patient evaluation of eye dryness(SPEED)questionnaire,tear break-up time(TBUT),corneal fluorescein staining(CFS),and in vivo confocal microscopy were performed at baseline and after every treatment.RESULTS:After three IPL treatments,both groups had significant improvement in MGYSS(both P<0.05).The noninferiority test showed that improvement in 10-mm group was not inferior to that in 3-mm group(P<0.001).In both groups,temporal regions of both upper and lower eyelids showed significant improvement in MGYSS.Scores of SPEED questionnaire in both groups declined significantly(both P<0.001)and changes of SPEED had no difference between two groups(P=0.57).Density of central corneal subepithelial nerves and TBUTs showed no statistically significant changes.The 3-mm group had improvement on corneal fluorescein staining(P=0.048)and meibomian gland morphology(acini wall thickness P=0.003,hyperreflective points P=0.024)while the 10-mm group had not.CONCLUSION:The efficacy of IPL indirect irradiation in improving meibomian gland secretion and alleviating dry eye symptoms remains unchanged with increase in treatment distance.IPL may primarily act on the functional improvement of the meibomian glands and corneal nerves.展开更多

Mild hepatic dysfunction in dengue haemorrhagic fever is usual.However,its presentation as acute liver failure(ALF) is unusual.We report a patient with dengue shock syndrome who presented with acute liver failure and hepatic encephalopathy in a recent outbreak of dengue fever in Delhi,India.展开更多
We present the case of an 86-year-old woman referred to us from Orthopaedics Division after surgical treatment for femur fracture without personal or family history of ischemic cardiopathy. During hospitalization the ...We present the case of an 86-year-old woman referred to us from Orthopaedics Division after surgical treatment for femur fracture without personal or family history of ischemic cardiopathy. During hospitalization the patient had stomach-ache and the electrocardiogram demonstrated ST-segment elevation in anterior leads. The coronarography showed haziness, but no critical stenosis;ventricolography revealed apical ballooning of the left ventricle with severe systolic dysfunction. She was treated with dopamine in renal dose, beta-blockers and warfarin. When she was in fairly good condition, she started physiotherapy for endoprosthesis for the femur fracture. Clinicians should consider takotsubo cardiomyopathy in the differential diagnosis of patients presenting with chest pain, especially in women with a recent history of physical stress.展开更多

Polycystic ovary syndrome(PCOS)is the predominant cause of subfertility in reproductive-aged women;however,its pathophysiology remains unknown.Neurotensin(NTS)is a member of the gut–brain peptide family and is involv...Polycystic ovary syndrome(PCOS)is the predominant cause of subfertility in reproductive-aged women;however,its pathophysiology remains unknown.Neurotensin(NTS)is a member of the gut–brain peptide family and is involved in ovulation;its relationship with PCOS is unclear.Here,we found that NTS expression in ovarian granulosa cells and follicular fluids was markedly decreased in patients with PCOS.In the in vitro culture of cumulus–oocyte complexes,the neurotensin receptor 1(NTSR1)antagonist SR48692 blocked cumulus expansion and oocyte meiotic maturation by inhibiting metabolic cooperation and damaging the mitochondrial structure in oocytes and surrounding cumulus cells.Furthermore,the ERK1/2-early growth response 1 pathway was found to be a key downstream mediator of NTS/NTSR1 in the ovulatory process.Animal studies showed that in vivo injection of SR48692 in mice reduced ovulation efficiency and contributed to irregular estrus cycles and polycystic ovary morphology.By contrast,NTS partially ameliorated the ovarian abnormalities in mice with dehydroepiandrosterone-induced PCOS.Our findings highlighted the critical role of NTS reduction and consequent abnormal NTSR1 signaling in the ovulatory dysfunction of PCOS,suggesting a potential strategy for PCOS treatment.展开更多
The dysfunction of the lysosome and autophagy-lysosome system serves as a driving force for neurodegenerative diseases,metabolic disorders,inflammatory conditions,and other related diseases,closely influencing their o...The dysfunction of the lysosome and autophagy-lysosome system serves as a driving force for neurodegenerative diseases,metabolic disorders,inflammatory conditions,and other related diseases,closely influencing their onset and progression.Therefore,restoring the function of the lysosome or autophagy-lysosome system has become an increasingly crucial therapeutic strategy in disease management.In this review,we will introduce the lysosomal biogenesis,structure,and function,as well as the biological process of the autophagy-lysosome system.Various diseases closely associated with lysosomal/autophagic dysfunction are also reviewed,emphasizing the significance of targeting the function of the lysosome or autophagy-lysosome system in disease treatment.Finally,we focus on engineered nanomaterials that have the capabilities to restore the function of the lysosome or autophagy-lysosome system,and summarize different strategies and methods for achieving this goal.This review aims to elucidate the latest progress in the field of nanomedicine for lysosomal/autophagic defect-related diseases and inspire the development of innovative and clinically valuable nanomedicines.展开更多
