The worldwide prevalence of anxiety disorders among college students is high,which negatively affects countries,schools,families,and individual students to varying degrees.This paper reviews the relevant literature re...The worldwide prevalence of anxiety disorders among college students is high,which negatively affects countries,schools,families,and individual students to varying degrees.This paper reviews the relevant literature regarding risk factors and digital interventions for anxiety disorders among college students from the perspectives of different stakeholders.Risk factors at the national and societal levels include class differences and the coronavirus disease 2019 pandemic.College-level risk factors include the indoor environment design of the college environment,peer relationships,student satisfaction with college culture,and school functional levels.Family-level risk factors include parenting style,family relationship,and parental level of education.Individual-level risk factors include biological factors,lifestyle,and personality.Among the intervention options for college students'anxiety disorders,in addition to traditional cognitive behavioral therapy,mindfulness-based interventions,psychological counseling,and group counseling,digital mental health interventions are increasingly popular due to their low cost,positive effect,and convenient diagnostics and treatment.To better apply digital intervention to the prevention and treatment of college students'anxiety,this paper suggests that the different stakeholders form a synergy among themselves.The nation and society should provide necessary policy guarantees,financial support,and moral and ethical supervision for the prevention and treatment of college students'anxiety disorders.Colleges should actively participate in the screening and intervention of college students'anxiety disorders.Families should increase their awareness of college students'anxiety disorders and take the initiative to study and understand various digital intervention methods.College students with anxiety disorders should actively seek psychological assistance and actively accept and participate in digital intervention projects and services.We believe that in the future,the application of methods such as big data and artificial intelligence to improve digital interventions and provide individualized treatment plans will become the primary means of preventing and treating anxiety disorders among college students.展开更多

BACKGROUND Non-alcoholic fatty liver disease(NAFLD)management requires sustainable lifestyle modifications.This study aimed to evaluate the effectiveness of the RESET care plan,a comprehensive program that is an integ...BACKGROUND Non-alcoholic fatty liver disease(NAFLD)management requires sustainable lifestyle modifications.This study aimed to evaluate the effectiveness of the RESET care plan,a comprehensive program that is an integrated personalized diet,exercise,and cognitive behavior therapy,delivered via MyTatva’s digital health application enabled through a body composition analyzer(BCA)and smartwatch.AIM To evaluates the effectiveness of the comprehensive program delivered via My-Tatva’s digital health app enabled through internet of thing devices.METHODS This retrospective observational study analyzed deidentified data from 22 par-ticipants enrolled in the MyTatva RESET care program.Participants were divided into three groups:Group A,diet plan;Group B,diet+exercise plan;and Group C,diet+exercise+cognitive behavioral therapy plan.Participants were provided with a BCA and smartwatch for continuous monitoring of anthropometric para-meters.Statistical analysis,including one-way ANOVA and post-hoc Tukey’s Honest Significant Difference test,was conducted to compare mean changes in anthropometric parameters across the groups.INTRODUCTION Non-alcoholic fatty liver disease(NAFLD)has emerged as a global health burden,affecting approximately 1 in 4 in-dividuals worldwide.NAFLD ranges from simple steatosis(fat accumulation)to non-alcoholic steatohepatitis(NASH),and its global prevalence in the general population is estimated between 6.3%and 33%,with NASH affecting 3-5%[1,2].Obesity is a major risk factor of NAFLD,with studies showing that the likelihood of developing NAFLD increases 5-fold at a body mass index(BMI)of 30-32.5 kg/m²and up to 14-fold at BMI of 37.5-40 kg/m²compared to a BMI of 20-22.5 kg/m²[3,4].Effective NAFLD management requires both dietary and physical activity modifications.Healthy weight loss with sustained muscle mass plays a pivotal role,with a reduction of 3%-5%decreasing hepatic steatosis,5%-7%improving NASH conditions,and 10%or more needed to reverse hepatic fibrosis[5].Management also normalizes elevated liver enzymes(aspartate aminotransferase and alanine aminotransferase),enhances insulin sensitivity,and thereby reduces cardiovascular risk by improving endothelial function and increasing cardiorespiratory fitness[6].However,diet or exercise alone is often not as effective as a combined approach.Integrating both balanced dietary changes and increased physical activity yields more sustainable improvements in NAFLD and overall metabolic health[1,7].Traditional intervention methods usually involve in-person consultations,which often lack real-time and continuous patient monitoring.The recommendation of drastic changes in diet and exercise can also be overwhelming for patients,leading to low adherence rates.Many patients struggle to maintain these changes in the long-term due to a lack of con-tinuous motivational support[8,9].In recent years,the health ecosystem has witnessed a significant shift toward digital health platforms,which complement pharmacological treatments in chronic disease management,and increase scala-bility.These platforms provide continuous monitoring and personalized support,helping to bridge the gap between health care setups and patients[10].Recent digital advances enable internet of things(IoT)devices to be integrated into such management plans to track health metrics,to address the limitations of traditional methods[11,12].The MyTatva digital health application offers the RESET plan,a novel comprehensive approach for NAFLD mana-gement by integrating personalized support from nutrition,physiotherapy,and cognitive behavioral therapy(CBT)coaches.We aimed to evaluate the effectiveness of the RESET plan by analyzing the reduction in anthropometric para-meters across three different digital intervention groups.展开更多
Chinese Wine Culture influences people’s attitudes toward alcohol.The current study focuses on exploring the main features of a localized digital alcohol consumption intervention mini-program to address the difficult...Chinese Wine Culture influences people’s attitudes toward alcohol.The current study focuses on exploring the main features of a localized digital alcohol consumption intervention mini-program to address the difficulties of abstinence for native drinkers,as a promising way for long-term management of rehabilitation from alcohol use disorder.A mixed-method approach was used in this study.The self-report quantitative questionnaire recruited three groups of participants:89 drinkers,67 drinkers’relatives,and 30 medical staff.The focus group qualitative interview inspected 36 participants’perspectives on the core topics,including 21 drinkers,4 drinkers’relatives,and 11 medical staff.The results of combining the quantitative study and qualitative study indicated that the top difficulties of abstinence for native drinkers are the strong craving from the inside,the environmental influence,and the psychological health status,especially emotional states.Correspondingly,the most desired main features in an alcohol consumption digital intervention tool are the daily track of drinking conditions and craving level,periodic feedback reports that can share with others,and mood improvement training.Moreover,the top factors that influence participants’intention to use/recommend the tool are whether the tool is effective,whether the user experience is good,and whether the tool can replenish the deficiency of the current alcohol treatment.Future work needs to balance what patients want and what others around them expect,so that potential users can benefit best from the digital intervention tool in the context of Chinese culture.展开更多
Importance:Digital technology is now widely available for the interventions of autism,but its validity and feasibility remain to be proved.Objective:This study aimed to investigate the effectiveness of digital health ...Importance:Digital technology is now widely available for the interventions of autism,but its validity and feasibility remain to be proved.Objective:This study aimed to investigate the effectiveness of digital health interventions(DHIs)in improving core symptoms or intelligence quotient in patients with autism spectrum disorder(ASD).Methods:Three databases including PubMed,Cochrane,and Scopus,were searched on November 15,2022.Randomized clinical trials that enrolled patients with ASD who received DHIs and a control group without DHI treatment were included.Cochrane risk of bias tool(RoB 2)was applied to assess the risk of bias.Results:A total of 33 studies,involving 1285 participants(658[51.2%]in DHI groups and 627[48.8%]in control groups),were analyzed to investigate the differences between DHI groups and control groups.Significantly greater improvements in the overall performance of ASD were observed in the DHI groups compared to the control groups(including active,waitlist,treatment-as-usual,and no treatment)with an effect size of 1.89(Cohen’s d 95%confidence interval[CI]:1.26-2.52).Studies with treatment-as-usual,waitlist,and no treatment control demonstrated large effect sizes of Cohen’s d 3.41(95%CI:0.84-5.97),Cohen’s d 4.27(95%CI:1.95-6.59),and Cohen’s d 4.52(95%CI:2.98-6.06)respectively.In contrast,studies with active control revealed insignificant effect sizes(Cohen’s d 0.73,95%CI:0.12-1.33).Interpretation:This meta-analysis found significantly greater improvements in core symptoms or intelligence quotient in ASD patients receiving DHIs compared to those in control conditions.ASD patients may benefit from the DHIs and reduce the economic burden.展开更多
BACKGROUND The perinatal period is a challenging time of substantial emotional,physiological,social,and relational changes.Depression,anxiety,and stress symptoms are common,and co-exist in the perinatal period.Digital...BACKGROUND The perinatal period is a challenging time of substantial emotional,physiological,social,and relational changes.Depression,anxiety,and stress symptoms are common,and co-exist in the perinatal period.Digital technology continues to grow at an unprecedented pace with wide application,including psychotherapeutic intervention.A growing number of meta-analyses supported the application of digital psychotherapeutic intervention across different populations,but relatively few meta-and meta-regression analyses have concentrated on perinatal women.AIM To evaluate the effectiveness of digital psychotherapeutic intervention on improving psychological outcomes among perinatal women and identify its essential features.METHODS Randomized controlled trials(RCTs)were obtained from eight databases,including Cumulative Index to Nursing and Allied Health Literature,Cochrane Library,Embase,Scopus,PsycINFO,PubMed,Web of Science,and ProQuest Dissertation and Theses from inception up until November 24,2020.Comprehensive Meta-analysis 3.0 software was used to conduct meta-and metaregression analyses.The Cochrane risk-of-bias tool and the Grading of the Recommendation,Assessment,Development,and Evaluation system were adopted to assess the individual and overall qualities of the evidence,respectively.RESULTS A total of 25 RCTs that included 3239 women were identified.Meta-analyses revealed that intervention significantly improved depression(Hedges’s g=0.49),anxiety(g=0.25),and stress(g=0.47)symptoms compared to the control.Subgroup analyses demonstrated that a website platform with≥eight therapistguided sessions using the theoretical principle of cognitive behavioral therapy was more effective than other treatments in improving depression symptoms in postnatal women.Meta-regression analyses observed that the age of perinatal women and the type of psychotherapy also had statistically significant effects on depression symptoms.Egger’s regression asymmetry tests suggested that no publication biases occurred,but the overall quality of the evidence was very low.CONCLUSION This review suggests that digital psychotherapeutic intervention may be a potential solution to reduce psychological problems in perinatal women.Further high-quality RCTs with large sample sizes are needed.展开更多
Currently,a proportion of adolescents use alcohol,tobacco,and illicit drugs,which inevitably harms their health and academic progress.Adolescence is a peak period for substance use initiation and a critical time for p...Currently,a proportion of adolescents use alcohol,tobacco,and illicit drugs,which inevitably harms their health and academic progress.Adolescence is a peak period for substance use initiation and a critical time for preventing substance use problems.Various entities,such as families,schools,and communities,have implemented a variety of interventions to alleviate adolescent substance use problems,and schools play a unique role.To explore the types,characteristics,and effectiveness of substance use interventions in educational settings for adolescents,we conducted a scoping review and identified 32 studies after screening.We divided the 32 studies according to intervention type,including curriculum interventions focusing on cognitive-behavioral skill enhancement,exercise interventions,peer interventions and family-school cooperation,and electronic interventions.Except for the mixed results on electronic interventions,the results showed that the other interventions were beneficial to different extents in alleviating adolescent substance use problems.In addition,we analyzed and summarized the advantages and challenges of intervening in adolescent substance use in educational settings.Schools can use equipment and human resources to provide adolescents with various types of intervention measures,but they also face challenges such as stigmatization,ineffective coordination among multiple resources,and poor implementation effects.In the future,school-based intervention measures can fully utilize big data and artificial intelligence technology and collaborate with families and communities to intervene appropriately while paying attention to the comorbidity risks of substance use disorders and psychological health issues.展开更多

BACKGROUND Acceptance and commitment therapy(ACT)is a third-generation therapy that appears to be a promising psychological intervention for psychotic disorders.While several systematic reviews and meta-analyses that ...BACKGROUND Acceptance and commitment therapy(ACT)is a third-generation therapy that appears to be a promising psychological intervention for psychotic disorders.While several systematic reviews and meta-analyses that address the efficacy of ACT for psychosis have been conducted,no systematic review has specifically focused on the application of ACT to the early stages of psychosis.AIM To review the state of the art regarding the feasibility and efficacy of treating early psychosis(EP)with ACT-based interventions.METHODS First,we describe the foundations of the ACT model to provide the background required to contextualize the main objective of this review.Second,we searched the PubMed and PsycINFO databases for studies published up to January 2025 and identified eight studies that met our selection criteria.The systematic review was conducted in accordance with the PRISMA guidelines and the critical appraisal checklist provided by the Joanna Briggs Institute for randomized controlled trials(RCTs).RESULTS All studies were published after 2019.Among the reviewed studies,five were RCTs with a total combined sample of 399 nonoverlapping participants.The methodological quality was moderate for RCTs.The results showed that ACTbased treatments are feasible and improve psychotic symptoms,medication adherence,and global functioning in patients with EP.Furthermore,preliminary evidence exists for the benefits of group-based and online-delivered programs and those that combine face-to-face therapy with novel real-time digital interventions,such as“ecological momentary intervention”,to apply therapeutic con cepts to real life.CONCLUSION ACT-based treatments in the early stages of psychosis are feasible and improve symptoms,treatment adherence,and self-care skills.Although promising,these results are inconclusive.Further research is required.展开更多
Resilience-the dynamic capacity to maintain or restore mental and physical health in the face of adversity-has been linked to improved treatment adherence,reduced psychological distress,and enhanced long-term recovery...Resilience-the dynamic capacity to maintain or restore mental and physical health in the face of adversity-has been linked to improved treatment adherence,reduced psychological distress,and enhanced long-term recovery in patients confronting serious illnesses such as cancer.Psychosocial resources(e.g.,social support,peer-led groups)and neurobiological mechanisms(e.g.,Hypothalamic-Pituitary-Adrenal axis modulation,neural plasticity)interact to buffer the physiological and emotional impact of diagnosis and therapy.Interventions including cognitive-behavioural therapy,mindfulness-based stress reduction,and digitally delivered programs demonstrate moderate-to-large effect sizes(Hedges'g 0.33-1.45)in strengthening resilience and improving quality of life.However,inconsistent terminology,the absence of standardized,multidimensional assessment tools,and logistical challenges in implementation limit widespread adoption.We advocate for a unified resilience framework that integrates psychometric scales,biomarker assays,and real-time digital monitoring;the embedding of resilience-promoting strategies within multidisciplinary care pathways;and the alignment of policy support and reimbursement models to sustain these efforts.Longitudinal,multicenter trials and culturally sensitive adaptation will be essential to validate scalable models,ensuring that resilience becomes a fundamental pillar of compassionate,patient-centered care.展开更多
In this article,we discuss the recently published article by Soni et al.This study explores the effectiveness of a comprehensive digital health program,RESET care,which integrates personalized dietary plans,structured...In this article,we discuss the recently published article by Soni et al.This study explores the effectiveness of a comprehensive digital health program,RESET care,which integrates personalized dietary plans,structured exercise,and cognitive behavioral therapy delivered through a mobile app equipped with Internet of Things devices such as body composition analyzers and smartwatches.Metabolic dysfunction-associated liver disease(MASLD),a global health burden affecting approximately 25%of the population,demands sustainable lifestyle modifications as its primary management strategy.The study reports that 100%of participants in the comprehensive intervention group(diet+exercise+cognitive behavioral therapy)achieved a weight reduction≥7%(6.99±2.98 kg,7.00%±3.39%;P=0.002),a clinically significant threshold for MASLD improvement.In addition,participants showed a mean weight reduction of 6.99 kg(101.10±17.85 vs 94.11±17.38,P<0.001)and a body mass index reduction of 2.18 kg/m²(32.90±3.02 vs 30.72±3.41,P<0.001).These results underscore the potential of digital health platforms to provide scalable,evidence-based solutions for the treatment of MASLD.While these results highlight the potential of digital platforms in the scalable and personalized management of MASLD,the small study sample size and short duration of follow-up limit the generalizability of the results.Future large-scale,long-term trials are needed to confirm sustained benefits,cost-effectiveness,and broader applicability.This letter contextualizes the study within the evolving landscape of MASLD management and emphasizes the clinical implications of integrating digital technologies into standard care.展开更多
To the Editor:Subthreshold depression(SD),which often precedes major depressive disorder(MDD),is broadly defined as scoring above the cutoff point on a depression questionnaire without meeting the Diagnostic and Stati...To the Editor:Subthreshold depression(SD),which often precedes major depressive disorder(MDD),is broadly defined as scoring above the cutoff point on a depression questionnaire without meeting the Diagnostic and Statistical Manual of Mental Disorders(DSM)or International Classification of Diseases(ICD)criteria.[1]SD was estimated to have a prevalence rate ranging between 5.3%and 29.2%.[2,3]In addition,SD was linked to adverse outcomes in both physical and mental well-being,as well as poor academic,social,and mood results for children and adolescents.[4]Consequently,addressing SD with appropriate treatment was of paramount importance.Psychological interventions have shown some impact on reducing depressive symptoms in adolescents with SD but faced obstacles such as inaccessibility,high costs,and stigma of mental illness.[4]Digital(i.e.,internet-and computer-based)psychological interventions offer potential solutions to address these barriers and promote mental health care for youth.However,previous studies presented mixed results concerning the efficacy of these interventions.An evaluation of extant evidence for the efficacy of digital psychotherapy programs for youth experiencing SD will provide important evidence for primary healthcare.展开更多

文摘BACKGROUND Non-alcoholic fatty liver disease(NAFLD)management requires sustainable lifestyle modifications.This study aimed to evaluate the effectiveness of the RESET care plan,a comprehensive program that is an integrated personalized diet,exercise,and cognitive behavior therapy,delivered via MyTatva’s digital health application enabled through a body composition analyzer(BCA)and smartwatch.AIM To evaluates the effectiveness of the comprehensive program delivered via My-Tatva’s digital health app enabled through internet of thing devices.METHODS This retrospective observational study analyzed deidentified data from 22 par-ticipants enrolled in the MyTatva RESET care program.Participants were divided into three groups:Group A,diet plan;Group B,diet+exercise plan;and Group C,diet+exercise+cognitive behavioral therapy plan.Participants were provided with a BCA and smartwatch for continuous monitoring of anthropometric para-meters.Statistical analysis,including one-way ANOVA and post-hoc Tukey’s Honest Significant Difference test,was conducted to compare mean changes in anthropometric parameters across the groups.INTRODUCTION Non-alcoholic fatty liver disease(NAFLD)has emerged as a global health burden,affecting approximately 1 in 4 in-dividuals worldwide.NAFLD ranges from simple steatosis(fat accumulation)to non-alcoholic steatohepatitis(NASH),and its global prevalence in the general population is estimated between 6.3%and 33%,with NASH affecting 3-5%[1,2].Obesity is a major risk factor of NAFLD,with studies showing that the likelihood of developing NAFLD increases 5-fold at a body mass index(BMI)of 30-32.5 kg/m²and up to 14-fold at BMI of 37.5-40 kg/m²compared to a BMI of 20-22.5 kg/m²[3,4].Effective NAFLD management requires both dietary and physical activity modifications.Healthy weight loss with sustained muscle mass plays a pivotal role,with a reduction of 3%-5%decreasing hepatic steatosis,5%-7%improving NASH conditions,and 10%or more needed to reverse hepatic fibrosis[5].Management also normalizes elevated liver enzymes(aspartate aminotransferase and alanine aminotransferase),enhances insulin sensitivity,and thereby reduces cardiovascular risk by improving endothelial function and increasing cardiorespiratory fitness[6].However,diet or exercise alone is often not as effective as a combined approach.Integrating both balanced dietary changes and increased physical activity yields more sustainable improvements in NAFLD and overall metabolic health[1,7].Traditional intervention methods usually involve in-person consultations,which often lack real-time and continuous patient monitoring.The recommendation of drastic changes in diet and exercise can also be overwhelming for patients,leading to low adherence rates.Many patients struggle to maintain these changes in the long-term due to a lack of con-tinuous motivational support[8,9].In recent years,the health ecosystem has witnessed a significant shift toward digital health platforms,which complement pharmacological treatments in chronic disease management,and increase scala-bility.These platforms provide continuous monitoring and personalized support,helping to bridge the gap between health care setups and patients[10].Recent digital advances enable internet of things(IoT)devices to be integrated into such management plans to track health metrics,to address the limitations of traditional methods[11,12].The MyTatva digital health application offers the RESET plan,a novel comprehensive approach for NAFLD mana-gement by integrating personalized support from nutrition,physiotherapy,and cognitive behavioral therapy(CBT)coaches.We aimed to evaluate the effectiveness of the RESET plan by analyzing the reduction in anthropometric para-meters across three different digital intervention groups.

文摘Chinese Wine Culture influences people’s attitudes toward alcohol.The current study focuses on exploring the main features of a localized digital alcohol consumption intervention mini-program to address the difficulties of abstinence for native drinkers,as a promising way for long-term management of rehabilitation from alcohol use disorder.A mixed-method approach was used in this study.The self-report quantitative questionnaire recruited three groups of participants:89 drinkers,67 drinkers’relatives,and 30 medical staff.The focus group qualitative interview inspected 36 participants’perspectives on the core topics,including 21 drinkers,4 drinkers’relatives,and 11 medical staff.The results of combining the quantitative study and qualitative study indicated that the top difficulties of abstinence for native drinkers are the strong craving from the inside,the environmental influence,and the psychological health status,especially emotional states.Correspondingly,the most desired main features in an alcohol consumption digital intervention tool are the daily track of drinking conditions and craving level,periodic feedback reports that can share with others,and mood improvement training.Moreover,the top factors that influence participants’intention to use/recommend the tool are whether the tool is effective,whether the user experience is good,and whether the tool can replenish the deficiency of the current alcohol treatment.Future work needs to balance what patients want and what others around them expect,so that potential users can benefit best from the digital intervention tool in the context of Chinese culture.

文摘Importance:Digital technology is now widely available for the interventions of autism,but its validity and feasibility remain to be proved.Objective:This study aimed to investigate the effectiveness of digital health interventions(DHIs)in improving core symptoms or intelligence quotient in patients with autism spectrum disorder(ASD).Methods:Three databases including PubMed,Cochrane,and Scopus,were searched on November 15,2022.Randomized clinical trials that enrolled patients with ASD who received DHIs and a control group without DHI treatment were included.Cochrane risk of bias tool(RoB 2)was applied to assess the risk of bias.Results:A total of 33 studies,involving 1285 participants(658[51.2%]in DHI groups and 627[48.8%]in control groups),were analyzed to investigate the differences between DHI groups and control groups.Significantly greater improvements in the overall performance of ASD were observed in the DHI groups compared to the control groups(including active,waitlist,treatment-as-usual,and no treatment)with an effect size of 1.89(Cohen’s d 95%confidence interval[CI]:1.26-2.52).Studies with treatment-as-usual,waitlist,and no treatment control demonstrated large effect sizes of Cohen’s d 3.41(95%CI:0.84-5.97),Cohen’s d 4.27(95%CI:1.95-6.59),and Cohen’s d 4.52(95%CI:2.98-6.06)respectively.In contrast,studies with active control revealed insignificant effect sizes(Cohen’s d 0.73,95%CI:0.12-1.33).Interpretation:This meta-analysis found significantly greater improvements in core symptoms or intelligence quotient in ASD patients receiving DHIs compared to those in control conditions.ASD patients may benefit from the DHIs and reduce the economic burden.

文摘BACKGROUND The perinatal period is a challenging time of substantial emotional,physiological,social,and relational changes.Depression,anxiety,and stress symptoms are common,and co-exist in the perinatal period.Digital technology continues to grow at an unprecedented pace with wide application,including psychotherapeutic intervention.A growing number of meta-analyses supported the application of digital psychotherapeutic intervention across different populations,but relatively few meta-and meta-regression analyses have concentrated on perinatal women.AIM To evaluate the effectiveness of digital psychotherapeutic intervention on improving psychological outcomes among perinatal women and identify its essential features.METHODS Randomized controlled trials(RCTs)were obtained from eight databases,including Cumulative Index to Nursing and Allied Health Literature,Cochrane Library,Embase,Scopus,PsycINFO,PubMed,Web of Science,and ProQuest Dissertation and Theses from inception up until November 24,2020.Comprehensive Meta-analysis 3.0 software was used to conduct meta-and metaregression analyses.The Cochrane risk-of-bias tool and the Grading of the Recommendation,Assessment,Development,and Evaluation system were adopted to assess the individual and overall qualities of the evidence,respectively.RESULTS A total of 25 RCTs that included 3239 women were identified.Meta-analyses revealed that intervention significantly improved depression(Hedges’s g=0.49),anxiety(g=0.25),and stress(g=0.47)symptoms compared to the control.Subgroup analyses demonstrated that a website platform with≥eight therapistguided sessions using the theoretical principle of cognitive behavioral therapy was more effective than other treatments in improving depression symptoms in postnatal women.Meta-regression analyses observed that the age of perinatal women and the type of psychotherapy also had statistically significant effects on depression symptoms.Egger’s regression asymmetry tests suggested that no publication biases occurred,but the overall quality of the evidence was very low.CONCLUSION This review suggests that digital psychotherapeutic intervention may be a potential solution to reduce psychological problems in perinatal women.Further high-quality RCTs with large sample sizes are needed.
文摘Currently,a proportion of adolescents use alcohol,tobacco,and illicit drugs,which inevitably harms their health and academic progress.Adolescence is a peak period for substance use initiation and a critical time for preventing substance use problems.Various entities,such as families,schools,and communities,have implemented a variety of interventions to alleviate adolescent substance use problems,and schools play a unique role.To explore the types,characteristics,and effectiveness of substance use interventions in educational settings for adolescents,we conducted a scoping review and identified 32 studies after screening.We divided the 32 studies according to intervention type,including curriculum interventions focusing on cognitive-behavioral skill enhancement,exercise interventions,peer interventions and family-school cooperation,and electronic interventions.Except for the mixed results on electronic interventions,the results showed that the other interventions were beneficial to different extents in alleviating adolescent substance use problems.In addition,we analyzed and summarized the advantages and challenges of intervening in adolescent substance use in educational settings.Schools can use equipment and human resources to provide adolescents with various types of intervention measures,but they also face challenges such as stigmatization,ineffective coordination among multiple resources,and poor implementation effects.In the future,school-based intervention measures can fully utilize big data and artificial intelligence technology and collaborate with families and communities to intervene appropriately while paying attention to the comorbidity risks of substance use disorders and psychological health issues.

文摘BACKGROUND Acceptance and commitment therapy(ACT)is a third-generation therapy that appears to be a promising psychological intervention for psychotic disorders.While several systematic reviews and meta-analyses that address the efficacy of ACT for psychosis have been conducted,no systematic review has specifically focused on the application of ACT to the early stages of psychosis.AIM To review the state of the art regarding the feasibility and efficacy of treating early psychosis(EP)with ACT-based interventions.METHODS First,we describe the foundations of the ACT model to provide the background required to contextualize the main objective of this review.Second,we searched the PubMed and PsycINFO databases for studies published up to January 2025 and identified eight studies that met our selection criteria.The systematic review was conducted in accordance with the PRISMA guidelines and the critical appraisal checklist provided by the Joanna Briggs Institute for randomized controlled trials(RCTs).RESULTS All studies were published after 2019.Among the reviewed studies,five were RCTs with a total combined sample of 399 nonoverlapping participants.The methodological quality was moderate for RCTs.The results showed that ACTbased treatments are feasible and improve psychotic symptoms,medication adherence,and global functioning in patients with EP.Furthermore,preliminary evidence exists for the benefits of group-based and online-delivered programs and those that combine face-to-face therapy with novel real-time digital interventions,such as“ecological momentary intervention”,to apply therapeutic con cepts to real life.CONCLUSION ACT-based treatments in the early stages of psychosis are feasible and improve symptoms,treatment adherence,and self-care skills.Although promising,these results are inconclusive.Further research is required.
文摘Resilience-the dynamic capacity to maintain or restore mental and physical health in the face of adversity-has been linked to improved treatment adherence,reduced psychological distress,and enhanced long-term recovery in patients confronting serious illnesses such as cancer.Psychosocial resources(e.g.,social support,peer-led groups)and neurobiological mechanisms(e.g.,Hypothalamic-Pituitary-Adrenal axis modulation,neural plasticity)interact to buffer the physiological and emotional impact of diagnosis and therapy.Interventions including cognitive-behavioural therapy,mindfulness-based stress reduction,and digitally delivered programs demonstrate moderate-to-large effect sizes(Hedges'g 0.33-1.45)in strengthening resilience and improving quality of life.However,inconsistent terminology,the absence of standardized,multidimensional assessment tools,and logistical challenges in implementation limit widespread adoption.We advocate for a unified resilience framework that integrates psychometric scales,biomarker assays,and real-time digital monitoring;the embedding of resilience-promoting strategies within multidisciplinary care pathways;and the alignment of policy support and reimbursement models to sustain these efforts.Longitudinal,multicenter trials and culturally sensitive adaptation will be essential to validate scalable models,ensuring that resilience becomes a fundamental pillar of compassionate,patient-centered care.
文摘In this article,we discuss the recently published article by Soni et al.This study explores the effectiveness of a comprehensive digital health program,RESET care,which integrates personalized dietary plans,structured exercise,and cognitive behavioral therapy delivered through a mobile app equipped with Internet of Things devices such as body composition analyzers and smartwatches.Metabolic dysfunction-associated liver disease(MASLD),a global health burden affecting approximately 25%of the population,demands sustainable lifestyle modifications as its primary management strategy.The study reports that 100%of participants in the comprehensive intervention group(diet+exercise+cognitive behavioral therapy)achieved a weight reduction≥7%(6.99±2.98 kg,7.00%±3.39%;P=0.002),a clinically significant threshold for MASLD improvement.In addition,participants showed a mean weight reduction of 6.99 kg(101.10±17.85 vs 94.11±17.38,P<0.001)and a body mass index reduction of 2.18 kg/m²(32.90±3.02 vs 30.72±3.41,P<0.001).These results underscore the potential of digital health platforms to provide scalable,evidence-based solutions for the treatment of MASLD.While these results highlight the potential of digital platforms in the scalable and personalized management of MASLD,the small study sample size and short duration of follow-up limit the generalizability of the results.Future large-scale,long-term trials are needed to confirm sustained benefits,cost-effectiveness,and broader applicability.This letter contextualizes the study within the evolving landscape of MASLD management and emphasizes the clinical implications of integrating digital technologies into standard care.

文摘To the Editor:Subthreshold depression(SD),which often precedes major depressive disorder(MDD),is broadly defined as scoring above the cutoff point on a depression questionnaire without meeting the Diagnostic and Statistical Manual of Mental Disorders(DSM)or International Classification of Diseases(ICD)criteria.[1]SD was estimated to have a prevalence rate ranging between 5.3%and 29.2%.[2,3]In addition,SD was linked to adverse outcomes in both physical and mental well-being,as well as poor academic,social,and mood results for children and adolescents.[4]Consequently,addressing SD with appropriate treatment was of paramount importance.Psychological interventions have shown some impact on reducing depressive symptoms in adolescents with SD but faced obstacles such as inaccessibility,high costs,and stigma of mental illness.[4]Digital(i.e.,internet-and computer-based)psychological interventions offer potential solutions to address these barriers and promote mental health care for youth.However,previous studies presented mixed results concerning the efficacy of these interventions.An evaluation of extant evidence for the efficacy of digital psychotherapy programs for youth experiencing SD will provide important evidence for primary healthcare.
