The wavelength-tunable and switchable narrow bandwidth mode-locking operation is demonstrated in an all fiber laser based on semiconductor-saturable absorber mirror (SESAM). Two narrow-band fiber Bragg gratings centered at 1029.9nm and 1032nm respectively with a polarization controller inserted between them are used to realize the wavelength switchable between 1029.9nm and 1032nm. The laser delivers different pulse widths of 7.5ps for 1030nm and 20ps for 1032nm. The maximum output power for both could reach -6.5mW at single pulse operation. The output wavelength couM be tuned to about 0.gnm intervals ranging from 1030.2nm to 1031.1 nm and from 1032.15nm to 1033.7nm with the temperature change of the fiber Bragg grating, respectively. 展开更多

Delivering bad news regarding patient's health is an unavoidable task for physicians and medical students in their careers.This is especially difficult for beginners.Disclosing undesired medical information to patients is technically and mentally demanding.Only with proper communication skills and sufficient patient-contact practice can medical practitioners smoothly cope with this stressful mission.There are a number of common mistakes we fall into when delivering bad news,which might induce or exacerbate patients' anxiety.Sometimes we are prone to disclose medical information in a "straight-to-the-point" manner.This is often a futile attempt made on the physician's part to ease his or her own uncomfortableness when delivering bad news,but unfortunately it does no good to facilitate patients' acceptance of the actual condition.Instead of using medical jargons,physicians are obliged to use "layman's terms" that make better sense to their patients.Declaring a clinical diagnosis without corresponding explanation on the prognosis and the discussion of treatment can also bring much confusion and anxiety to patients.Another even worse situation might be that we finish the "lecture" in our own words without confirming the patient's comprehension and addressing their questions.Professionals have recommended a six-step protocol on delivering bad news to patients:"SPIKES":S-Setting up the interview;P-assessing the patient's Perception;I-obtaining the patient's Invitation;K-giving Knowledge and information to the patient;E-addressing the patient's Emotions with Empathic responses;S-Strategy and Summary. 展开更多

This study presents a comparative analysis of optimisation strategies for designing hull shapes of Autonomous Underwater Vehicles(AUVs),paying special attention to drag,lift-to-drag ratio,and delivered power.A fully integrated optimisation framework is developed accordingly,combining a single-objective Genetic Algorithm(GA)for design parameter generation,Computer-Aided Geometric Design(CAGD)for the creation of hull geometries and associated fluid domains,and a Reynolds-Averaged Navier-Stokes(RANS)solver for evaluating hydrodynamic performance metrics.This unified approach eliminates manual intervention,enabling automated determination of optimal hull configurations.Three distinct optimisation problems are addressed using the proposed methodology.First,the drag minimisation of a reference afterbody geometry(A1)at zero angle of attack is performed under constraints of fixed length and internal volume for various flow velocities spanning the range from 0.5 to 15 m/s.Second,the lift-to-drag ratio of A1 is maximised at a 6°angle of attack,maintaining constant total length and internal volume.Third,delivered power is minimised for A1 at a 0°angle of attack.The comparative analysis of results from all three optimisation cases reveals hull shapes with practical design significance.Notably,the shape optimised for minimum delivered power outperforms the other two across a range of velocities.Specifically,it achieves reductions in required power by 7.6%,7.8%,10.2%,and 13.04%at velocities of 0.5,1.0,1.5,and 2.152 m/s,respectively. 展开更多

Objective:The objective of this work is to design and fabricate a novel multifunctional nanocarrier combining thrombus-targeted imaging and ultrasound-mediated drug delivery for the theranostics of thrombotic diseases.Impact Statement:This study develops a new technology that can accurately visualize the thrombus and deliver drugs with controllable properties to diagnose and treat thrombotic diseases.Introduction:Thrombotic diseases are a serious threat to human life and health.The diagnosis and treatment of thrombotic diseases have always been a challenge.In recent years,nanomedicine has brought new ideas and new methods for the theranostics of thrombotic diseases.However,there are also many problems need to be solved,such as biosafety and stability of nanocarriers,early diagnosis,and timely treatment of thrombotic diseases,difficulty in clinical translation.Methods:The S1P@CD-PLGA-rtPA nanobubbles(NBs)were prepared by integrating sulfur hexafluoride(SF6)-loaded poly(D,L-lactide-co-glycolide)(PLGA)NBs,cyclodextrin(CD),sphingosine-1-phosphate(S1P),and recombinant tissue plasminogen activator(rtPA).Results:S1P@CD-PLGA-rtPA NBs had rapid and excellent thrombosis targeting imaging performance based on the specific interaction of S1P-S1PR1(sphingosine-1-phosphate receptor 1).Furthermore,S1P@CD-PLGA-rtPA NBs that specifically targeting to the thrombosis regions could also respond to external ultrasound to achieve accurate and efficient delivery of rtPA to enhance the thrombolysis effectiveness and efficiency.Conclusion:This study proposes a new idea and strategy of targeting thrombus in rats via the specific interaction of S1P-S1PR1.On this basis,the acoustic response properties of bubble carriers could be fully utilized by combining thrombus-specific targeted imaging and ultrasound-mediated drug delivery for effective thrombolysis,which is expected to be applied in targeted diagnosis and treatment of thrombotic diseases in the future. 展开更多

BACKGROUND: The development of a harmless and effi- cient nonviral gene delivery system that can facilitate the penetration of nucleic acids through the plasma membrane is a key to successful gene therapy. The aim of this study was to test a nonviral gene transferring vector's function of delivering DNA into liver cells to provide an important clue for gene transfer in liver gene therapy. METHODS: The complex of DNA and DNA delivering protein was injected into mice through their tail veins. Then the mice were killed and their liver tissue was sec- tioned. The gene transferring results were detected using a confocal laser scanning microscope. RESULTS: Fluorescence analysis indicated that both DNA- membrane penetrating peptide (MPP) complex and DNA- hepatocyte specific receptor binding domain ( HSRBD) - MPP complex could go into liver cells. The fluorescence value of liver cells in the DNA-HSRBD-MPP group was higher than that in the DNA-MPP group. CONCLUSIONS; MPP can successfully deliver DNA and protein into cells, and MPP with a HSRBD can specifically deliver DNA into liver cells. These have laid a foundation for further study on the nonviral liver cell gene delivering system. 展开更多

However, often size increases causing pain, and complications of fractures can severely compromise the patient.<strong> Aim:</strong> The results in a series of cases treated minimally invasive using a new device for the application of allogenic bone material appear highly promising and shall be presented. <strong>Patients and Methods:</strong> Eight consecutive patients with symptomatic Unicameral Bone Cysts (UBC) were treated by percutaneous instillation of Grafton<span style="white-space:nowrap;">?</span> DBF Putty (demineralised allogenic bone containing fibers) mixed with autologous bone marrow using the Kyphon<span style="white-space:nowrap;">?</span> Cement Delivery System (Medtronic), which allows the injection of this high viscosity paste by controlled high pressure. Five patients with Aneurysmal Bone Cysts (ABC) were treated accordingly after inactivation by Aethoxysclerol 3% and lacking bone formation. Recombinant bacteria secreting the transmembrane-truncated region of white spot syndrome virus (WSSV) envelope protein VP28 named DhpVB were constructed, using live Escherichia coli as a deliverer and a con- stitutive secretory expression plasmid as the expression vector. With the ability to deliver recombinant pro- tein products outside, DhpVB could make VP28 directly interact with the shrimp when injected into shrim- p. In order to test the protection potential, live DhpVB were injected into the marine shrimp Exopalamon carincauda Holthuis for three times and WSSV challenge was performed twice simultaneously at the last two injections of the bacteria. The results showed that DhpVB displayed statistically significant advantage over bacteria without VP28 after the second challenge with an average RI (Resistance index) of 0.67±0.08, compared with 0.41±0.09 of bacteria without VP28 (P 〈0.05). The data suggested that a quasi-immune response may exist in E. carincauda and live bacteria carrying VP28 could enhance the shrimp resistance against WSSV. 展开更多

Optimal indicator for changing the filter during the continuous renal replacement therapy in intensive care unit patients with acute kidney injury:A crossover randomized trial 被引量:3

BACKGROUND:The study aims to investigate an optimal indicator for changing the filter during the continuous renal replacement therapy(CRRT)in intensive care unit(ICU)patients with acute kidney injury(AKI).METHODS:Patients with AKI requiring CRRT in an ICU were randomly divided into two groups for crossover trial,i.e.,groups A and B.Patients in the group A were firstly treated with continuous veno-venous hemofiltration(CVVH),followed by continuous veno-venous hemodiafiltration(CVVHDF).Patients in the group B were firstly treated with CVVHDF followed by CVVH.Delivered doses of solutes with different molecular weights at the indicated time points between groups were compared.A correlation analysis between the delivered dose and pre-filter pressure(P_(PRE))and transmembrane pressure(P_(TM))was performed.Receiver operating characteristic(ROC)curves were constructed to evaluate the accuracy of P_(TM) as an indicator for filter replacement.RESULTS:A total of 50 cases were analyzed,27 in the group A and 23 in the group B.Delivered doses of different molecular-weight solutes significantly decreased before changing the filter in both modalities,compared with those at the initiation of treatment(all P<0.05).In the late stage of CRRT,the possible rebound of serum medium-molecular-weight solute concentration was observed.P_(TM) was negatively correlated with the delivered dose of medium-molecular-weight solute in both modalities.The threshold for predicting the rebound of serum concentration of medium-molecularweight solute by P_(TM) was 146.5 mm Hg(1 mm Hg=0.133 k Pa).CONCLUSIONS:The filter can be used as long as possible within the manufacturer’s safe use time limits to remove small-molecular-weight solutes.P_(TM) of 146.5 mm Hg may be an optimal indicator for changing the filter in CRRT therapies to remove medium-molecular-weight solutes. 展开更多
