BACKGROUND Depression is a common psychological disorder often associated with genetic,neurobiological,and psychosocial factors.This condition is characterized by low mood and decreased conscious activity and can lead...BACKGROUND Depression is a common psychological disorder often associated with genetic,neurobiological,and psychosocial factors.This condition is characterized by low mood and decreased conscious activity and can lead to severe consequences such as suicidal thoughts.Respiratory failure,a disorder of pulmonary ventilation and gas exchange,is frequently complicated by depression,aggravating the condition and creating complications in treatment.Patients with both conditions tend to exhibit poor compliance with medical advice,resulting in adverse outcomes such as sleep disorders,arrhythmias,and electrolyte imbalances.Therefore,effective nursing interventions are crucial for improving patient outcomes.While routine nursing care can provide basic support,dedicated nursing,which involves specialized and personalized care by highly trained professionals,may offer additional benefits.AIM To examine the effect of dedicated nursing intervention on medical advice compliance and adverse event incidence in patients with depression complicated by respiratory failure.METHODS A total of 160 patients with depression complicated by respiratory failure admitted to the Second Affiliated Hospital of Soochow University from January to December 2024 were randomly categorized into a control group(n=80)receiving routine nursing care and observation group(n=80)receiving routine nursing combined with dedicated nursing care.The establishment of a specialized nursing team;the development of a personalized nursing plan;and the implementation of comprehensive care strategies targeting emotional support,sleep improvement,pulmonary function enhancement,and adherence to medical advice were included in the dedicated nursing intervention.Emotional state[evaluated using the Self-Rating Depression Scale(SDS)],sleep quality[assessed using the Pittsburgh Sleep Quality Index(PSQI)],hope level[measured using the Herth Hope Scale(HHS)],pulmonary function[comprising forced vital capacity(FVC)and forced expiratory volume in the first second(FEV1)],exercise endurance[6-minute walk distance test(6MWD)],compliance with medical advice,and incidence of adverse events were compared between the two groups.RESULTS No significant differences were observed in SDS,PSQI,HHS,FVC,FEV1,6MWD,or compliance with medical advice between the two groups(P>0.05)before the intervention.However,after the intervention,the control group exhibited significantly lower SDS and PSQI scores;higher HHS scores;greater improvements in FVC,FEV1,and 6MWD;and higher compliance with medical advice compared with the control group(P<0.05).In addition,the incidence of adverse events was significantly lower in the control group(P<0.05).CONCLUSION Dedicated nursing intervention significantly improves emotional state,sleep quality,hope level,pulmonary function,exercise endurance,and compliance with medical advice while reducing adverse events in patients with depression complicated by respiratory failure.Therefore,dedicated nursing appears to be an effective approach to enhance patient outcomes in this population.展开更多

BACKGROUND Total neoadjuvant therapy(TNT)has been proposed as an advancement over standard long-course chemoradiotherapy(LCCRT)for the treatment of locally advanced rectal cancer(LARC).It has been suggested that TNT e...BACKGROUND Total neoadjuvant therapy(TNT)has been proposed as an advancement over standard long-course chemoradiotherapy(LCCRT)for the treatment of locally advanced rectal cancer(LARC).It has been suggested that TNT enhances resect-ability,improves treatment compliance,increases the rate of pathological comp-lete response,and reduces the risk of systemic recurrence.However,concerns have been raised that the prolonged interval to surgery associated with TNT,particularly in regimens such as the Rectal Cancer and Preoperative Induction Therapy Followed by Dedicated Operation(RAPIDO)protocol,may exacerbate fibrosis,leading to more technically challenging resections and poorer surgical outcomes.RAPIDO vs LCCRT.METHODS A single-center,retrospective cohort study was conducted of patients with LARC treated with TNT-RAPIDO or standard LCCRT followed by surgical resection between 2014 and 2024.A total of 99 patients with LARC were analyzed,inclu-ding 29 treated with TNT-RAPIDO and 70 treated with standard LCCRT.Demo-graphics,clinicopathological characteristics and early post-operative outcomes were compared between both groups.RESULTS Both groups were comparable in terms of demographics and clinicopathological characteristics.The median interval from initiation of neoadjuvant therapy to Core Tip:Rectal cancer and preoperative induction therapy followed by dedicated operation has emerged as a total neoadjuvant therapy strategy with improved oncological and functional outcomes.The impact of total neoadjuvant therapy on operative difficulty and short-term surgical outcomes,compared with long-course chemoradiotherapy,remains an area of ongoing debate.This cohort study of 99 patients demonstrated that the Rectal Cancer and Preoperative Induction Therapy Followed by Dedicated Operation(RAPIDO)protocol does not increase surgical difficulty or compromise early surgical outcomes compared with long-course chemoradiotherapy.It may also confer a shorter total stoma duration and a lower permanent stoma rate.randomized controlled trials in the future are warranted to more accurately assess the differences between TNT-RAPIDO and LCCRT in relation to oncological outcomes.展开更多

The medal was initially exclusively awarded to living people but evolved to include both living and deceased people. The recipients were balanced among all countries. The gender of the winners showed a changing trend from being limited to only females to including both males and females.There was also a shift from emphasis on only dedication of the recipient to equal emphasis on both dedication and innovation.Conclusions: The selection criteria for recipients of the Florence Nightingale Medal evolved from consideration of those showing only dedication to equal consideration of both dedication and innovation.展开更多

In this paper,we aim to unlock the potential of intelligent reflecting surfaces(IRSs)in cognitive internet of things(loT).Considering that the secondary IoT devices send messages to the secondary access point(SAP)by s...In this paper,we aim to unlock the potential of intelligent reflecting surfaces(IRSs)in cognitive internet of things(loT).Considering that the secondary IoT devices send messages to the secondary access point(SAP)by sharing the spectrum with the primary network,the interference is introduced by the IoT devices to the primary access point(PAP)which profits from the IoT devices by pricing the interference power charged by them.A practical path loss model is adopted such that the IRSs deployed between the IoT devices and SAP serve as diffuse scatterers,but each reflected signal can be aligned with its own desired direction.Moreover,two transmission policies of the secondary network are investigated without/with a successive interference cancellation(SIC)technique.The signal-to-interference plus noise ratio(SINR)balancing is considered to overcome the nearfar effect of the IoT devices so as to allocate the resource fairly among them.We propose a Stackelberg game strategy to characterize the interaction between primary and secondary networks.For the proposed game,the Stackelberg equilibrium is analytically derived to optimally obtain the closed-form solution of the power allocation and interference pricing.Numerical results are demonstrated to validate the performance of the theoretical derivations.展开更多
The study proposes an improved Harris hawks optimization(IHHO) algorithm by integrating the standard Harris hawks optimization(HHO) algorithm and mutation-based search mechanism for developing a high-performance machi...The study proposes an improved Harris hawks optimization(IHHO) algorithm by integrating the standard Harris hawks optimization(HHO) algorithm and mutation-based search mechanism for developing a high-performance machine learning solution for predicting soil compression index. HHO is a newly introduced meta-heuristic optimization algorithm(MOA) used to solve continuous search problems.Compared to the original HHO, the proposed IHHO can evade trapping in local optima, which in turn raises the search capabilities and enhances the search mechanism relying on mutation. Subsequently, a novel meta-heuristic-based soft computing technique called ELM-IHHO was established by integrating IHHO and extreme learning machine(ELM) to estimate soil compression index. A sum of 688 consolidation test data was collected for this purpose from an ongoing dedicated freight corridor railway project. To evaluate the generalization capability of the proposed ELM-IHHO model, a detailed comparison between ELM-IHHO and other well-established MOAs, such as particle swarm optimization,genetic algorithm, and biogeography-based optimization integrated with ELM, was performed. Based on the outcomes, the ELM-IHHO model exhibits superior performance over the other MOAs in predicting soil compression index.展开更多
Recently,the importance of vehicle safety supporting system has been highlighted as autonomous driving and platooning has attracted the researchers.To ensure driving safety,each vehicle must broadcast a basic safety m...Recently,the importance of vehicle safety supporting system has been highlighted as autonomous driving and platooning has attracted the researchers.To ensure driving safety,each vehicle must broadcast a basic safety message(BSM)every 100 ms.However,stable BSM exchange is difficult because of the changing environment and limited bandwidth of vehicular wireless communication.The increasing number of vehicles on the road increases the competition to access wireless networks for BSM exchange;this increases the packet collision rate.An increased packet collision rate impairs the transmission and reception of BSM information,which can easily cause a traffic accident.We propose a solution,the vehicular safety support system(V3S),which exchanges BSMs reliably even when many vehicles are on the road.The V3S uses a clustering scheme to decrease network traffic by reducing the amount of data exchanged between a vehicle and the roadside unit(RSU).In addition,the V3S reduces the collision rate of wireless network packets by broadcasting the vehicle’s BSM in an allocated timeslot using the time division multiple access(TDMA)MAC protocol.The V3S also deals with insufficient bandwidth for dedicated short-range communications(DSRC)by changing DSRC channels according to traffic flow.In evaluating the packet error rate for stable BSM packet delivery,the V3S demonstrates an excellent packet error rate of less than 1%,compared to the 802.11p with its packet error rate of 82%.展开更多

文摘BACKGROUND Total neoadjuvant therapy(TNT)has been proposed as an advancement over standard long-course chemoradiotherapy(LCCRT)for the treatment of locally advanced rectal cancer(LARC).It has been suggested that TNT enhances resect-ability,improves treatment compliance,increases the rate of pathological comp-lete response,and reduces the risk of systemic recurrence.However,concerns have been raised that the prolonged interval to surgery associated with TNT,particularly in regimens such as the Rectal Cancer and Preoperative Induction Therapy Followed by Dedicated Operation(RAPIDO)protocol,may exacerbate fibrosis,leading to more technically challenging resections and poorer surgical outcomes.RAPIDO vs LCCRT.METHODS A single-center,retrospective cohort study was conducted of patients with LARC treated with TNT-RAPIDO or standard LCCRT followed by surgical resection between 2014 and 2024.A total of 99 patients with LARC were analyzed,inclu-ding 29 treated with TNT-RAPIDO and 70 treated with standard LCCRT.Demo-graphics,clinicopathological characteristics and early post-operative outcomes were compared between both groups.RESULTS Both groups were comparable in terms of demographics and clinicopathological characteristics.The median interval from initiation of neoadjuvant therapy to Core Tip:Rectal cancer and preoperative induction therapy followed by dedicated operation has emerged as a total neoadjuvant therapy strategy with improved oncological and functional outcomes.The impact of total neoadjuvant therapy on operative difficulty and short-term surgical outcomes,compared with long-course chemoradiotherapy,remains an area of ongoing debate.This cohort study of 99 patients demonstrated that the Rectal Cancer and Preoperative Induction Therapy Followed by Dedicated Operation(RAPIDO)protocol does not increase surgical difficulty or compromise early surgical outcomes compared with long-course chemoradiotherapy.It may also confer a shorter total stoma duration and a lower permanent stoma rate.randomized controlled trials in the future are warranted to more accurately assess the differences between TNT-RAPIDO and LCCRT in relation to oncological outcomes.
文摘This special issue of China PARTICUOLOGY is dedicated to Professors Yong Jin and Zhiqing Yu of Tsinghua University, China, to celebrate over five decades of their careers in chemical engineering research and education. Prof. Jin graduated in 1959 from Ural Polytechnic College in Svivdelovsk in the former USSR and then completed his post-graduate studies in Tianjin University in China in 1960. Thereafter, he taught at the Chinese University of Science and Technology for 13 years before joining Tsinghua University in 1973. Prof. Yu graduated in 1956 from Tianjin University in China and joined Tsinghua University thereafter. Between 1957 and 1959, Prof. Yu undertook more studies in Moscow Mendeleev Institute of Chemical Technology in the former USSR. During the 1970s, Professors Jin and Yu jointly created the Reaction Engineering Program in the Department of Chemical Engineering at Tsinghua University, which later became one of the largest fluidization research groups in the world, known as FLOTU - Fluidization Laboratory of Tsinghua University.

文摘车载网VANET是一种应用于智能交通系统的新型无线移动自组织网络(mobile ad hoc network,MANET).随着车辆以及移动ad hoc网络技术的发展,车载网已经成为一个新兴的研究领域.针对VANET中车辆行驶的特征,提出一种拓扑相对稳定的车辆分簇算法.在此算法基础上,根据专用短程通信(dedicated short range communication,DSRC)标准中控制信道(control channel,CCH)和服务信道(service channel,SCH)的分配,考虑车辆间的无线通信干扰和不同应用的QoS需求,提出一种基于分簇的多信道混合型MAC协议,簇内通信采用非竞争的TDMA机制,簇间通信采用基于竞争的CSMA?CA机制,相邻簇采用不同的服务信道.模拟实验表明,提出的MAC协议在同时满足实时应用的延迟需求和非实时应用的吞吐量方面,优于现有协议.

文摘Objective: This study aimed to explore the 100-year changes in regulations for the Florence Nightingale Medal.Methods: We used the announcements of the Florence Nightingale Medal awards as the research object to analyze-via historical and inductive methods-the regulations for receiving the Florence Nightingale Medal.Results: In the award years, the greatest number of winners per award year was 71 and the lowest was 12. The medal was initially exclusively awarded to living people but evolved to include both living and deceased people. The recipients were balanced among all countries. The gender of the winners showed a changing trend from being limited to only females to including both males and females.There was also a shift from emphasis on only dedication of the recipient to equal emphasis on both dedication and innovation.Conclusions: The selection criteria for recipients of the Florence Nightingale Medal evolved from consideration of those showing only dedication to equal consideration of both dedication and innovation.
文摘Abstract:Dedication: The research of the US Transuranium and Uranium Registries relies heavily upon postmortem autopsy findings and radiochemical analysis of tissues. The enormous debt owed to those now-deceased registrants who unselfishly voluntarily participated in the US Transuranium and Uranium Registries program through postmortem donation of their tissues and to those stillliving registrants who have volunteered to be future postmortem tissue donors is hereby acknowledged with gratitude. The scientific findings derived from postmortem analysis of these tissues have been instrumental in advancing our understanding of the actinide elements in humans and have led to refinement, validation, and confidence in safety standards for those who work with these elements as well as for the general public. To these generous and anonymous persons who made this ultimate contribution, this paper is dedicated with great thanks and admiration.

文摘In this paper,we aim to unlock the potential of intelligent reflecting surfaces(IRSs)in cognitive internet of things(loT).Considering that the secondary IoT devices send messages to the secondary access point(SAP)by sharing the spectrum with the primary network,the interference is introduced by the IoT devices to the primary access point(PAP)which profits from the IoT devices by pricing the interference power charged by them.A practical path loss model is adopted such that the IRSs deployed between the IoT devices and SAP serve as diffuse scatterers,but each reflected signal can be aligned with its own desired direction.Moreover,two transmission policies of the secondary network are investigated without/with a successive interference cancellation(SIC)technique.The signal-to-interference plus noise ratio(SINR)balancing is considered to overcome the nearfar effect of the IoT devices so as to allocate the resource fairly among them.We propose a Stackelberg game strategy to characterize the interaction between primary and secondary networks.For the proposed game,the Stackelberg equilibrium is analytically derived to optimally obtain the closed-form solution of the power allocation and interference pricing.Numerical results are demonstrated to validate the performance of the theoretical derivations.
文摘The study proposes an improved Harris hawks optimization(IHHO) algorithm by integrating the standard Harris hawks optimization(HHO) algorithm and mutation-based search mechanism for developing a high-performance machine learning solution for predicting soil compression index. HHO is a newly introduced meta-heuristic optimization algorithm(MOA) used to solve continuous search problems.Compared to the original HHO, the proposed IHHO can evade trapping in local optima, which in turn raises the search capabilities and enhances the search mechanism relying on mutation. Subsequently, a novel meta-heuristic-based soft computing technique called ELM-IHHO was established by integrating IHHO and extreme learning machine(ELM) to estimate soil compression index. A sum of 688 consolidation test data was collected for this purpose from an ongoing dedicated freight corridor railway project. To evaluate the generalization capability of the proposed ELM-IHHO model, a detailed comparison between ELM-IHHO and other well-established MOAs, such as particle swarm optimization,genetic algorithm, and biogeography-based optimization integrated with ELM, was performed. Based on the outcomes, the ELM-IHHO model exhibits superior performance over the other MOAs in predicting soil compression index.

文摘Recently,the importance of vehicle safety supporting system has been highlighted as autonomous driving and platooning has attracted the researchers.To ensure driving safety,each vehicle must broadcast a basic safety message(BSM)every 100 ms.However,stable BSM exchange is difficult because of the changing environment and limited bandwidth of vehicular wireless communication.The increasing number of vehicles on the road increases the competition to access wireless networks for BSM exchange;this increases the packet collision rate.An increased packet collision rate impairs the transmission and reception of BSM information,which can easily cause a traffic accident.We propose a solution,the vehicular safety support system(V3S),which exchanges BSMs reliably even when many vehicles are on the road.The V3S uses a clustering scheme to decrease network traffic by reducing the amount of data exchanged between a vehicle and the roadside unit(RSU).In addition,the V3S reduces the collision rate of wireless network packets by broadcasting the vehicle’s BSM in an allocated timeslot using the time division multiple access(TDMA)MAC protocol.The V3S also deals with insufficient bandwidth for dedicated short-range communications(DSRC)by changing DSRC channels according to traffic flow.In evaluating the packet error rate for stable BSM packet delivery,the V3S demonstrates an excellent packet error rate of less than 1%,compared to the 802.11p with its packet error rate of 82%.
