Introduction: Standard Precautions are a set of measures applied in the care of all individuals/patients regardless of their infectious status aiming at preventing healthcare workers and patients from infections, prev...Introduction: Standard Precautions are a set of measures applied in the care of all individuals/patients regardless of their infectious status aiming at preventing healthcare workers and patients from infections, preventing environmental contamination and spread of infections to the community. Many health facilities have not implemented them to an acceptable level. The purpose of the study is to report progress of improvement in health facilities readiness to implement standard precautions for infection prevention and control based on SARA reports. Methods: We generated mean scores of all standard precautions each year and calculated their standard deviations, variances and confidence intervals. One-way ANOVA was used to determine if the mean scores were equal. Finally, the trend of improvement in health facilities readiness to implement the standard precautions was generated. Results: A total of nine standard precautions were reported in SARA reports for 2012, 2017 and 2020. The mean scores of the standard precautions were 52.22% in 2012, 64.55% in 2017 and 69.66% in 2020. The overall trend showed an increase in health facilities readiness to implement standard precautions, although the mean scores were not statistically different (p-value 0.3217). Conclusion: SARA surveys conducted in Tanzania in 2012, 2017 and 2020 have shown an overall increase in health facilities readiness to implement standard precautions. Safe final waste disposal was being done in fewest facilities while single-use or auto-disable syringes were in most facilities that were sampled in all years. SARA surveys may be a useful way to evaluate Infection Prevention and Control adherence in health facilities.展开更多

Objective:Hospital personnel,especially nurses are at risk for tuberculosis(TB) infection and the intervention for reducing the risk should be established.Methods:To compare the TB infection control practices and stan...Objective:Hospital personnel,especially nurses are at risk for tuberculosis(TB) infection and the intervention for reducing the risk should be established.Methods:To compare the TB infection control practices and standard precaution in 154 registered nurses and auxiliary nurses working in risk wards.Additionally,microbial air quality in the studied wards was investigated before and after implementation of an intervention including twoday training program on TB infection control and standard precaution practices and managing the ward environments. Results:Post-intervention,the percentage of studied nurses who always practiced increased in every item of TB infection control practice(6 items) and standard precaution(9 items) when compared with the preintervention period.Both mean scores were significantly higher than those before intervention(5.0±1.0 vs 4. 4±1.1,and 7.6±1.3 vs 6.7±1.5,P【0.001,respectively).As well as,bacterial and fungal counts in air samples collected from every studied ward decreased after implementing the intervention,for example, mean of bacterial counts in OPD decreased from 387.8±249.5 cfu/m^3 to 194.4±134.3 cfu/m^3.Additionally, there was no air sample with high level of bacterial and fungal counts,whereas,3.7 -22.2%of air samples collected before intervention had high level of bacterial counts(】500 cfu/m^3).Conclusion:Two-day training program and management of the ward environment could improve the scores of TB infection control practices and the standard precaution among studied nurses and reduce the microbial counts in air samples collected from the studied wards.展开更多
AIM:To compare the eradication rates for Helicobacter pylori(H.pylori) and ulcer recurrence of standard triple therapy(STT) and levofloxacin based therapy(LBT).METHODS:Seventy-four patients with perforated duodenal ul...AIM:To compare the eradication rates for Helicobacter pylori(H.pylori) and ulcer recurrence of standard triple therapy(STT) and levofloxacin based therapy(LBT).METHODS:Seventy-four patients with perforated duodenal ulcer treated with simple closure and found to be H.pylori infected on 3 mo follow up were randomized to receive either the STT group comprising of amoxicillin 1 g bid,clarithromycin 500 mg bid and omeprazole 20 mg bid or the LBT group comprising of amoxicillin 1 g bid,levofloxacin 500 mg bid and omeprazole 20 mg bid for 10 d each.The H.pylori eradication rates,side effects,compliance and the recurrence of ulcer were assessed in the two groups at 3 mo follow up.RESULTS:Thirty-four patients in the STT group and 32 patients in the levofloxacin group presented at 3 mo follow up.H.pylori eradication rates were similar with STT and the LBT groups on intention-to-treat(ITT) analysis(69% vs 80%,P = 0.425) and(79% vs 87%,P = 0.513) by per-protocol(PP) analysis respectively.Ulcer recurrence in the STT and LBT groups on ITT analysis was(20% vs 14%,P = 0.551) and(9% vs 6%,P = 1.00) by PP analysis.Compliance and side effects were also comparable between the groups.A complete course of STT costs Indian Rupees(INR) 1060.00,while LBT costs only INR 360.00.CONCLUSION:H.pylori eradication rates and the rate of ulcer recurrence were similar between the STT and LBT.The LBT is a more economical option compared to STT.展开更多
Objective: To assess the safety and efficacy of herbal formulation rich in standardized fenugreek seed extract(IND-2) add-on therapy in type 2 diabetes mellitus(T2DM) patients who were on insulin treatment in prospect...Objective: To assess the safety and efficacy of herbal formulation rich in standardized fenugreek seed extract(IND-2) add-on therapy in type 2 diabetes mellitus(T2DM) patients who were on insulin treatment in prospective, single arm, open-label, uncontrolled, multicentre trial.Methods: T2DM patients(n=30) with aged 18-80 years who were stabilized on insulin treatment with fasting blood sugar(FBS) level between 100-140 mg/dL received IND-2 capsules(700 mg, thrice a day) for 16 weeks.The primary endpoints were an assessment of FBS at week 2, 4, 6, 8, 12 and 16.Secondary end-points include post-prandial blood sugar level, glycosylated Hb(HbA1c), reduction in the dose of insulin and number of hypoglycemic attacks, and improvement in lipid profile at various weeks.Safety and adverse events(AEs) were also assessed during the study.Results: Study was completed in twenty T2DM patients, and there was no significant reduction in FBS and post-prandial blood sugar level after addon therapy of IND-2.However, add-on therapy of IND-2 significantly reduced(P<0.01) the HbA1c values, requirements of insulin and hypoglycemic events as compared with baseline.Total cholesterol, high-density lipoproteins-cholesterol, and low-density lipoproteincholesterol levels were significantly increased(P<0.01) after IND-2 add-on therapy.Body weight and safety outcomes did not differ significantly in IND-2 add-on therapy group at week 16.Additionally, add-on therapy of IND-2 did not produce any serious adverse events.Conclusions: The results of present investigation suggest that add-on therapy of IND-2 with insulin in T2DM patients improves glycaemic control through a decrease in levels of HbA1c and number of insulin doses needed per day without an increase in body weight and risk of hypoglycemia.Thus, IND-2 may provide a safe and well-tolerated add-on therapy option for the management of T2DM.展开更多

This is attributed to the diurnal warming ...Sea surface temperature SST obtained from the initial version of the Korea Operational Oceanographic System(KOOS) SST satellite have low accuracy during summer and daytime. This is attributed to the diurnal warming effect. Error estimation of SST data must be carried out to use the real-time forecasting numerical model of the KOOS. This study suggests two quality control methods for the KOOS SST system. To minimize the diurnal warming effect, SSTs of areas where wind speed is higher than 5 m/s were used. Depending on the wind threshold value, KOOS SST data for August 2014 were reduced by 0.15°C. Errors in SST data are considered to be a combination of random, sampling, and bias errors. To estimate bias error, the standard deviation of bias between KOOS SSTs and climatology SSTs were used. KOOS SST data yielded an analysis error standard deviation value similar to OSTIA and NOAA NCDC(OISST) data. The KOOS SST shows lower random and sampling errors with increasing number of observations using six satellite datasets. In further studies, the proposed quality control methods for the KOOS SST system will be applied through more long-term case studies and comparisons with other SST systems.展开更多

This system can be operated under the transmission rate ranging from 128kb/s to 2Mb/s. With these strategies for the videoconference system, the high quality image is obtained. 文摘This paper describes the relation and difference of "emission up to standards" and "total quantity control" ofmain atmospheric pollutants, as well as their legal status. It points out that the total quantity control is a good approachto solve a problem that environmental quality do not yet reach the requirements in an area where emission concentrationhas came up to standards, or to solve a problem that the interregional transportation of pollutants (e. g. acid rain) arises.And further, it put forward five proposals for the total quantity control.[

文摘Objective:Hospital personnel,especially nurses are at risk for tuberculosis(TB) infection and the intervention for reducing the risk should be established.Methods:To compare the TB infection control practices and standard precaution in 154 registered nurses and auxiliary nurses working in risk wards.Additionally,microbial air quality in the studied wards was investigated before and after implementation of an intervention including twoday training program on TB infection control and standard precaution practices and managing the ward environments. Results:Post-intervention,the percentage of studied nurses who always practiced increased in every item of TB infection control practice(6 items) and standard precaution(9 items) when compared with the preintervention period.Both mean scores were significantly higher than those before intervention(5.0±1.0 vs 4. 4±1.1,and 7.6±1.3 vs 6.7±1.5,P【0.001,respectively).As well as,bacterial and fungal counts in air samples collected from every studied ward decreased after implementing the intervention,for example, mean of bacterial counts in OPD decreased from 387.8±249.5 cfu/m^3 to 194.4±134.3 cfu/m^3.Additionally, there was no air sample with high level of bacterial and fungal counts,whereas,3.7 -22.2%of air samples collected before intervention had high level of bacterial counts(】500 cfu/m^3).Conclusion:Two-day training program and management of the ward environment could improve the scores of TB infection control practices and the standard precaution among studied nurses and reduce the microbial counts in air samples collected from the studied wards.
文摘AIM:To compare the eradication rates for Helicobacter pylori(H.pylori) and ulcer recurrence of standard triple therapy(STT) and levofloxacin based therapy(LBT).METHODS:Seventy-four patients with perforated duodenal ulcer treated with simple closure and found to be H.pylori infected on 3 mo follow up were randomized to receive either the STT group comprising of amoxicillin 1 g bid,clarithromycin 500 mg bid and omeprazole 20 mg bid or the LBT group comprising of amoxicillin 1 g bid,levofloxacin 500 mg bid and omeprazole 20 mg bid for 10 d each.The H.pylori eradication rates,side effects,compliance and the recurrence of ulcer were assessed in the two groups at 3 mo follow up.RESULTS:Thirty-four patients in the STT group and 32 patients in the levofloxacin group presented at 3 mo follow up.H.pylori eradication rates were similar with STT and the LBT groups on intention-to-treat(ITT) analysis(69% vs 80%,P = 0.425) and(79% vs 87%,P = 0.513) by per-protocol(PP) analysis respectively.Ulcer recurrence in the STT and LBT groups on ITT analysis was(20% vs 14%,P = 0.551) and(9% vs 6%,P = 1.00) by PP analysis.Compliance and side effects were also comparable between the groups.A complete course of STT costs Indian Rupees(INR) 1060.00,while LBT costs only INR 360.00.CONCLUSION:H.pylori eradication rates and the rate of ulcer recurrence were similar between the STT and LBT.The LBT is a more economical option compared to STT.
文摘Objective: To assess the safety and efficacy of herbal formulation rich in standardized fenugreek seed extract(IND-2) add-on therapy in type 2 diabetes mellitus(T2DM) patients who were on insulin treatment in prospective, single arm, open-label, uncontrolled, multicentre trial.Methods: T2DM patients(n=30) with aged 18-80 years who were stabilized on insulin treatment with fasting blood sugar(FBS) level between 100-140 mg/dL received IND-2 capsules(700 mg, thrice a day) for 16 weeks.The primary endpoints were an assessment of FBS at week 2, 4, 6, 8, 12 and 16.Secondary end-points include post-prandial blood sugar level, glycosylated Hb(HbA1c), reduction in the dose of insulin and number of hypoglycemic attacks, and improvement in lipid profile at various weeks.Safety and adverse events(AEs) were also assessed during the study.Results: Study was completed in twenty T2DM patients, and there was no significant reduction in FBS and post-prandial blood sugar level after addon therapy of IND-2.However, add-on therapy of IND-2 significantly reduced(P<0.01) the HbA1c values, requirements of insulin and hypoglycemic events as compared with baseline.Total cholesterol, high-density lipoproteins-cholesterol, and low-density lipoproteincholesterol levels were significantly increased(P<0.01) after IND-2 add-on therapy.Body weight and safety outcomes did not differ significantly in IND-2 add-on therapy group at week 16.Additionally, add-on therapy of IND-2 did not produce any serious adverse events.Conclusions: The results of present investigation suggest that add-on therapy of IND-2 with insulin in T2DM patients improves glycaemic control through a decrease in levels of HbA1c and number of insulin doses needed per day without an increase in body weight and risk of hypoglycemia.Thus, IND-2 may provide a safe and well-tolerated add-on therapy option for the management of T2DM.
文摘Now the surface settlement induced by shallow buried tunnel to under-traverse highway,mostly adopt the empirical value,3 cm,acquired from Beijing or Shenzhen metro,which is regarded as the controlling standard.But to more project,tunnel under-traverse highway,we must decide the controlling standard flexibly,which should base on the pavement service function and the pavement condition.The thesis,based on the pavement evenness and running comfort ability in the view of completing the pavement service function,supposes the longitudinal pavement settlement cross section as a quadratic parabola,and gets the maximal settlement value in the range of longitudinal pavement influence when the tunnel is under-traverse highway perpendicularly.Then the controlling standard is decided.As Wenxiang Tunnel under-traverse Lianhuo Freeway on Zhengzhou-Xi'an Passenger Dedicated Railway Line for an example,the settlement controlling standard is acquired,and we hope it has an direction and reference value to similar tunnel construction.

文摘[Objectives]The paper was to establish the quality standard of Shenqi Qiangjing granules and to conduct routine inspection of granular dosage forms.[Methods]The particle size,moisture,loss on drying,dissolubility,content uniformity and microbial limit of Shenqi Qiangjing granules were detected.Radix Astragali,Herba Epimedii,Radix Dipsaci and Fructus Schisandrae Chinensis were qualitatively identified by thin layer chromatography(TLC).[Results]The particle size,moisture,loss on drying,dissolubility,content uniformity and microbial limit of Shenqi Qiangjing granules were all in line with the relevant regulations of Pharmacopoeia of the People's Republic of China(2020 edition).Radix Astragali,Herba Epimedii,Radix Dipsaci and Fructus Schisandrae Chinensis had clear fluorescent spots in TLC assay,and no such spots were found in negative control.[Conclusions]The method has the advantages of simple operation,accurate and reliable results,strong specificity and good repeatability,and can effectively control the quality of Shenqi Qiangjing granules.

文摘Sea surface temperature SST obtained from the initial version of the Korea Operational Oceanographic System(KOOS) SST satellite have low accuracy during summer and daytime. This is attributed to the diurnal warming effect. Error estimation of SST data must be carried out to use the real-time forecasting numerical model of the KOOS. This study suggests two quality control methods for the KOOS SST system. To minimize the diurnal warming effect, SSTs of areas where wind speed is higher than 5 m/s were used. Depending on the wind threshold value, KOOS SST data for August 2014 were reduced by 0.15°C. Errors in SST data are considered to be a combination of random, sampling, and bias errors. To estimate bias error, the standard deviation of bias between KOOS SSTs and climatology SSTs were used. KOOS SST data yielded an analysis error standard deviation value similar to OSTIA and NOAA NCDC(OISST) data. The KOOS SST shows lower random and sampling errors with increasing number of observations using six satellite datasets. In further studies, the proposed quality control methods for the KOOS SST system will be applied through more long-term case studies and comparisons with other SST systems.

文摘There had been no standardized rules for citing ethical Kampo products used in clinical trials in journal articles. Although the name of a Kampo manufacturer was described in 77.9% of research articles, the name and ratios of crude drug components of Kampo formulas were not described in 77.5% of these papers. Considering the importance of proper characterization of interventions in the Consolidated Standards of Reporting Trials (CONSORT) checklist, we hereby propose the use of the Standards of Reporting Kampo Products (STORK) website, http://mpdb.nibiohn.go.jp/stork, as a reference for Kampo products. This will provide an official source on the internet for verified information on individual Kampo formulations for citation purposes in clinical research articles.

文摘The design and realization of a videoconference system based on international recommendation are introduced in this paper, and the hardware implementation of video codec based on ITU-T H. 261 is briefly discussed. Furthermore, the buffer control method and the adaptive control strategy for quantization are proposed, which are adaptive and robust. This system can be operated under the transmission rate ranging from 128kb/s to 2Mb/s. With these strategies for the videoconference system, the high quality image is obtained. The time delay of the system is about half a second.
