Older patients are usually discharged with complex medical problems, high stress and vulnerability, and these factors place the elderly at risk for poor outcomes. Purpose: The present review summarizes a method for providing appropriate and affordable health services by nursing professionals to meet older patient's health care needs during their transitional period which is defined as a period from discharge after hospitalization for a major disorder to recovery in a home setting. Summary: Older patients with chronic diseases need seamless health care during a transitional period-a highly stressful and vulnerable period for them. Nurse professionals can conduct decent discharge planning to assist older patients with transitional problems through continuous healthcare. This review summarized the need of continuing care for older patients during the transitional period, the definition of discharge planning, the conceptual framework of discharge planning, and the professionals involved in discharge planning. It also highlighted the problems of discharge planning and follow-up intervention implementation in the mainland of China. Clinical implications: Inadequate discharge planning and follow-up were leading factors associ- ated with the readmission of discharged older patients. Further nursing-led discharge planning should be reinforced in China.展开更多

Methods: a questionnaire was sent out to 178 patients with deep venous thrombosis of lower extremity who were hospitalized in a third-class A hospital in Jiaxing, Zhejiang Province from October 2020 to March 2021. The data were collected and sorted out for data processing, and the conclusions were drawn after statistical analysis. Results: patients with deep venous thrombosis of lower limbs had diverse needs for community continuing care services. In the dimension of nursing knowledge needs, the items with the highest scores were drug knowledge (3.03±1.24 points), emergency treatment knowledge (3.18±1.25 points) and disease recurrence prevention and guidance knowledge (3.14±1.15 points). Moreover, patients with low educational level, such as illiteracy and age > 60 years old, had a higher degree of community continuing care need for lower extremity deep vein thrombosis than those in the same group (P < 0.05). In the dimension of nursing service mode, the items of disease consultation mode were mostly telephone consultation (46.0%), and the items of service personnel were mostly specialized nurses (40.8%). Conclusion: patients with lower extremity deep vein thrombosis continuity of the community nursing should be higher, mainly reflected in drug knowledge, emergency treatment and relapse prevention guide knowledge, etc., should be in view of the continuity of the setting individualized nursing care, and improve community nursing staff professional quality, hastening the development of hospital - community integration mode.展开更多

In the demonstration process, 176 patients who had undergone enterostomy were selected. During hospitalization, the patients were taught how to observe and care for stomostomy correctly, how to use dung bags correctly and how to soothe their anxiety under the professional demonstration of doctors and nurses, laying a good foundation for post-discharge nursing. After discharge, when patients encounter problems after ostomy, they can use information methods, such as wechat video, voice phone, wechat group, QQ group and other ways to seek help from doctors and experts for remote continuing care. After discharge, 134 of the 176 patients had discomfort with ostomy and postoperative complications. They chose convenient information methods to seek help. After the remote guidance of doctors, 98 patients handled ostomy problems at home according to the guidance of doctors, and their self-resolution rate reached 73.13%. Therefore, it is concluded that the remote continuing care in an information-based way can improve the ability of patients to solve postoperative problems by themselves.展开更多

BACKGROUND Children with critical acute abdominal conditions often undergo intestinal stoma surgery.AIM To explore the impact of a visual mobile terminal-based extended care model on caregiver competence for children ...BACKGROUND Children with critical acute abdominal conditions often undergo intestinal stoma surgery.AIM To explore the impact of a visual mobile terminal-based extended care model on caregiver competence for children with enterostomies.METHODS One hundred twenty children with enterostomies and their caregivers in a children's hospital in Beijing were divided into a control group and a study group.The control group(60 cases)received traditional telephone follow-up for continuity of care,while the study group(60 cases)used a visualization mobile terminal-based care model.The incidence of stoma-related complications,caregiver burden scale,and competence scores of children with stoma were compared between the two groups.RESULTS The primary caregiver burden score in the study group(37.22±3.17)was significantly lower than that in the control group(80.00±4.47),and the difference was statistically significant(P<0.05).Additionally,the caregiving ability score of the study group(172.08±3.49)was significantly higher than that of the control group(117.55±4.28;P<0.05).The total incidence of complications in the study group(11.7%,7/60)was significantly lower compared to the control group(33.3%,20/60;χ2=8.086,P=0.004).CONCLUSION The visual mobile terminal-based care model reduces caregiver burden,improves home care ability,lowers the incidence of complications and readmission rates,and supports successful second-stage reduction surgery for children with enterostomies.展开更多
BACKGROUND Despite sharing similar pathogenic factors,cancer and coronary heart disease(CHD)occur in comparable populations at similar ages and possess similar susceptibility factors.Consequently,it is increasingly co...BACKGROUND Despite sharing similar pathogenic factors,cancer and coronary heart disease(CHD)occur in comparable populations at similar ages and possess similar susceptibility factors.Consequently,it is increasingly commonplace for patients to experience the simultaneous occurrence of cancer and CHD,a trend that is steadily rising.AIM To determine the impacts of continuing care on lung cancer patients with CHD following percutaneous coronary intervention(PCI).METHODS There were 94 lung cancer patients with CHD following PCI who were randomly assigned to the intervention group(n=38)and the control group(n=41).In the intervention group,continuing care was provided,while in the control group,routine care was provided.An evaluation of cardiac and pulmonary function,medication compliance,a 6-min walk test,and patient quality of life was performed.RESULTS Differences between the two groups were significant in left ventricular ejection fraction,6-min walk test,oxygen uptake,quality of life and medication compliance(P<0.05).In comparison with the control group,the enhancement in the intervention group was more significant.The intervention group had more patients with high medication compliance than the control group,with a statistically significant difference(P<0.05).CONCLUSION After undergoing PCI,lung patients with CHD could benefit from continued care in terms of cardiac and pulmonary function,medications compliance,and quality of life.展开更多

The overall nursing effects of patients in the two groups in the implementation of self-care management ability and nursing satisfaction were comprehensively analyzed. Results: two groups of patients after ligation of esophageal variceal bleeding were compared and the value of continuous nursing care was implemented after discharge. The scores of self-management ability and nursing satisfaction in the observation group were significantly better than those in the routine group. The data of the two groups were statistically significant (P < 0.05). Conclusion: through the analysis of patients after esophageal variceal bleeding surgery, the implementation of related continuous nursing measures can effectively improve the self-management ability of patients after ligation of esophageal variceal bleeding and the clinical nursing effect of satisfaction nursing treatment. It is helpful to promote the smooth development of nursing treatment and improve the prognosis of patients with good application value.展开更多

They were randomly assigned to the reference group and the research group. The reference group was given routine nursing intervention. The study group used a geriatric care sensitivity index system to carry out continuous care interventions. The two groups of elderly diabetic patients in the disease awareness and other conditions were compared. Results: after the intervention, the scores of DK, SDSCA, SF-36 and nursing satisfaction of middle-aged and elderly diabetic patients in the study group were much higher than those in the reference group (P < 0.05). Conclusion: continued care of elderly diabetes in the geriatric care sensitivity index system can play a very positive role in improving the management behavior, awareness and quality of life of the patients themselves.展开更多
Background:The Transitional Care Model(TCM)for nursing care has yet to be implemented in China despite its success in Western countries.However,rapid social changes have demanded an upgrade in the quality of nursing c...Background:The Transitional Care Model(TCM)for nursing care has yet to be implemented in China despite its success in Western countries.However,rapid social changes have demanded an upgrade in the quality of nursing care;in 2010,the Chinese government has acknowledged the need to implement the TCM in China.Objective:This study has the following objectives:(1)perform a thorough review of the literature regarding the development and implementation of the TCM in China's Mainland within the past 5 years;(2)provide a comprehensive discussion of the current status,problems,and strategies related to the implementation of the TCM in China's Mainland;and(3)suggest strategies pertaining to the future of the TCM in China.Design:The current pertinent literature is systematically reviewed.Data sources:Systematic and manual searches in computerized databases for relevant studies regarding the TCM led to the inclusion of 26 papers in this review.Review methods:Abstracts that satisfied the inclusion criteria were reviewed independently by the two authors of this manuscript,and discrepancies were resolved through discussion.The same reviewers independently assessed the paper in its entirety for selected abstracts.Results:The present English literature reviewrevealed a paucity of updated information about the development and implementation of the TCM in China's Mainland.Nevertheless,the dramatic growth of the TCM in the past 5 years has had a vital impact within the society and in nursing development.This review also revealed numerous issues regarding the focus of the TCM.Overall implications for practiceandrecommendations for future researchare discussed.Conclusion:Despite the potential of this nursing model to have a successful and beneficial impact in China's Mainland,it remains an under-researched topic.Further research on education and training as well as premium policies for nurses under the TCM are needed.展开更多
BACKGROUND Breast cancer is among the most common malignancies worldwide.With progress in treatment methods and levels,the overall survival period has been prolonged,and the demand for quality care has increased.AIM T...BACKGROUND Breast cancer is among the most common malignancies worldwide.With progress in treatment methods and levels,the overall survival period has been prolonged,and the demand for quality care has increased.AIM To investigate the effect of individualized and continuous care intervention in patients with breast cancer.METHODS Two hundred patients with breast cancer who received systemic therapy at The First Affiliated Hospital of Hebei North University(January 2021 to July 2023)were retrospectively selected as research participants.Among them,134 received routine care intervention(routing group)and 66 received personalized and continuous care(intervention group).Self-rating anxiety scale(SAS),self-rating depression scale(SDS),and Functional Assessment of Cancer Therapy-Breast(FACT-B)scores,including limb shoulder joint activity,complication rate,and care satisfaction,were compared between both groups after care.RESULTS SAS and SDS scores were lower in the intervention group than in the routing group at one and three months after care.The total FACT-B scores and five dimensions in the intervention group were higher than those in the routing group at three months of care.The range of motion of shoulder anteflexion,posterior extension,abduction,internal rotation,and external rotation in the intervention group was higher than that in the routing group one month after care.The incidence of postoperative complications was 18.18%lower in the intervention group than in the routing group(34.33%;P<0.05).Satisfaction with care was 90.91% higher in the intervention group than in the routing group(78.36%;P<0.05).CONCLUSION Personalized and continuous care can alleviate negative emotions in patients with breast cancer,quicken rehabilitation of limb function,decrease the incidence of complications,and improve living quality and care satisfaction.展开更多

Objective:To analyze the effect of using continuity of care for elderly patients with coronary heart disease(CHD)with unstable angina pectoris(UAP)and its impact on their quality of life.Methods:100 cases of elderly p...Objective:To analyze the effect of using continuity of care for elderly patients with coronary heart disease(CHD)with unstable angina pectoris(UAP)and its impact on their quality of life.Methods:100 cases of elderly patients with CHD with UAP admitted to our hospital from March 2022 to March 2023 were selected and grouped into an observation group and a control group of 50 cases each according to the randomized number table method.The nursing effect and quality of life of the observation group(continuity nursing)and the control group(routine nursing)were compared.Results:The total effective rate of nursing care was 96.00%observation group and 80.00%for the control group,and the differences were significant(χ2=6.061,P<0.05).Patients in the observation group had fewer episodes(1.42±0.21)times/week and a shorter duration(5.46±0.39)min,which were better than the control group(t=3.465,2.973;P<0.05).The depression self-rating depression scale(SDS)score(42.16±6.64)and anxiety self-rating scale(SAS)score(32.26±7.35)in the observation group were lower and the quality of life was higher as compared to that of the control group(P<0.05).Conclusion:Continuous nursing care improved the nursing effect of elderly CHD with UAP patients,promoted the alleviation of UAP symptoms,improved patient mentality,and improved their quality of life.Hence,continuous nursing care possesses significant clinical application value.展开更多
BACKGROUND The emergency department plays a crucial role in providing acute care to patients.Nursing interventions in this setting are essential for improving the continuity of care,enhancing patients’self-care abili...BACKGROUND The emergency department plays a crucial role in providing acute care to patients.Nursing interventions in this setting are essential for improving the continuity of care,enhancing patients’self-care abilities,and reducing psychological symp-toms.AIM To evaluate the effect of nursing interventions in the emergency department on these indicators in an emergency department.METHODS A retrospective analysis was conducted on 120 patients admitted to the emergency department between January 2022 and May 2023.The patients were divided into two groups:The control group(conventional nursing intervention)and the observation group(conventional nursing intervention+emergency department nursing intervention).The two groups were compared regarding continuity of care,self-care ability,psychological symptoms,and satisfaction with care.RESULTS The emergency department nursing interventions significantly improved the continuity of care,enhanced patients’self-care abilities,and reduced psycho-logical symptoms such as anxiety and depression.CONCLUSION Nursing interventions in the emergency department positively impact continuity of care,self-care,and psychological symptoms.However,it is important to acknowledge the limitations of this study,including the small number of studies,variable methodological quality,and the heterogeneity of the study population.Future research should address these limitations and further explore the effects of different types of nursing interventions in the emergency department.Additionally,efforts should be made to enhance the application and evaluation of these interventions in clinical practice.展开更多

Methods: It is a Prospective cohort study design. Included both gender and diabetic patient age above 24 years old. The data extracted from health care specialty center (HCSC) clinics in National guard hospital at Riyadh, through HCSC data base on three phases: 1) Phase 1: the data extracted of diabetic patients from October to November 2022 including MRN, diabetic patient, age, Hb A1C, Fasting blood glucose. 2) Phase 2: the same MRN extracted from phase 1 was extracted again retrospectively for six months from April to September 2022, to compare the indices before implanting the chronic illness clinic including Hb A1C, Fasting blood glucose. 3) Phase 3: prospectively from December 2022 to September 2023. Results: Among diabetic patients aged 60 years old and above showed better control of HbA1C and FBS comparing to these patients below age of 60 years old, with significant improvement of HbA1C after implanting chronic illness clinic. Conclusion: The significant improvement in the control of diabetic patients followed in primary health care centers reinforce the evidence of the importance of continuity of care.展开更多

Research in view of the family as the center continue nursing (first division team) and conventional nursing (group B as control group), to better the nursing effect to decide, during the study period before and after the special effect on patient care, nursing according to the daily life score, movement function development and intellectual development of the score of the priority, and in-depth analysis and comparison. Results: the scores of motor function development, intelligence development and daily life in group A were also significantly improved, and the rate of complete disability in group A was low, P<0.05 indicated that the test results were significant. Conclusion: after the application of family-centered continuous nursing intervention in the rehabilitation nursing of children with cerebral palsy, the nursing effect is good, and it can reduce the rate of complete disability of children, improve the self-care ability of children, and improve the living standard of children and their families to a certain extent, so it can be widely adopted and applied.展开更多
Objective:To explore the application effect of a doctor-nurse cooperation follow-up model based on an Internet platform in the continuation of care for patients after uro-oncology surgery.Methods:A convenient sampling...Objective:To explore the application effect of a doctor-nurse cooperation follow-up model based on an Internet platform in the continuation of care for patients after uro-oncology surgery.Methods:A convenient sampling method was used to select patients with urinary system tumors who underwent surgery in the Department of Urology in Grade III A general hospital in Shanghai from May to August 2022.Patients who underwent surgery from May to June 2022 were assigned to the control group,and those who underwent surgery from July to August 2022 were assigned to the experimental group.The control group received routine post-discharge nursing health education and telephone follow-up.On the basis of routine discharge guidance,the experimental group implemented the intervention method based on the Internet platform in continuation care.The levels of self-management efficacy,satisfaction,and incidence of unplanned readmission were compared one month after discharge between the two groups.Results:One month after discharge,the self-management efficacy of the experimental group(90.15±7.92)was significantly higher than that of the control group(79.10±7.84),and the patient satisfaction score(97.83±2.32)was significantly higher than that of the control group(90.23±2.58),with statistical significance(P<0.05).Additionally,the incidence of unplanned readmissions within one month after discharge in the experimental group(1.59%)was slightly lower than that in the control group(4.84%).Conclusion:The doctor-nurse cooperation follow-up model based on the Internet platform in continuation care can significantly improve the self-management efficiency of patients after discharge and enhance patient satisfaction,providing a new approach for discharge follow-up of urological tumor patients after surgery.展开更多

This review summarized the need of continuing care for older patients during the transitional period, the definition of discharge planning, the conceptual framework of discharge planning, and the professionals involved in discharge planning. It also highlighted the problems of discharge planning and follow-up intervention implementation in the mainland of China. Clinical implications: Inadequate discharge planning and follow-up were leading factors associ- ated with the readmission of discharged older patients. Further nursing-led discharge planning should be reinforced in China.
文摘Objective: to investigate and analyze the community continuing care needs of patients with deep venous thrombosis of lower extremities, and put forward corresponding suggestions based on the survey results, so as to provide basis for improving community continuing care services of deep venous thrombosis of lower extremities. Methods: a questionnaire was sent out to 178 patients with deep venous thrombosis of lower extremity who were hospitalized in a third-class A hospital in Jiaxing, Zhejiang Province from October 2020 to March 2021. The data were collected and sorted out for data processing, and the conclusions were drawn after statistical analysis. Results: patients with deep venous thrombosis of lower limbs had diverse needs for community continuing care services. In the dimension of nursing knowledge needs, the items with the highest scores were drug knowledge (3.03±1.24 points), emergency treatment knowledge (3.18±1.25 points) and disease recurrence prevention and guidance knowledge (3.14±1.15 points). Moreover, patients with low educational level, such as illiteracy and age > 60 years old, had a higher degree of community continuing care need for lower extremity deep vein thrombosis than those in the same group (P < 0.05). In the dimension of nursing service mode, the items of disease consultation mode were mostly telephone consultation (46.0%), and the items of service personnel were mostly specialized nurses (40.8%). Conclusion: patients with lower extremity deep vein thrombosis continuity of the community nursing should be higher, mainly reflected in drug knowledge, emergency treatment and relapse prevention guide knowledge, etc., should be in view of the continuity of the setting individualized nursing care, and improve community nursing staff professional quality, hastening the development of hospital - community integration mode.
文摘The purpose of this article is to explain the important significance of telemedicine continuous care for patients after enterostomy. In the demonstration process, 176 patients who had undergone enterostomy were selected. During hospitalization, the patients were taught how to observe and care for stomostomy correctly, how to use dung bags correctly and how to soothe their anxiety under the professional demonstration of doctors and nurses, laying a good foundation for post-discharge nursing. After discharge, when patients encounter problems after ostomy, they can use information methods, such as wechat video, voice phone, wechat group, QQ group and other ways to seek help from doctors and experts for remote continuing care. After discharge, 134 of the 176 patients had discomfort with ostomy and postoperative complications. They chose convenient information methods to seek help. After the remote guidance of doctors, 98 patients handled ostomy problems at home according to the guidance of doctors, and their self-resolution rate reached 73.13%. Therefore, it is concluded that the remote continuing care in an information-based way can improve the ability of patients to solve postoperative problems by themselves.
文摘Objective:To analyze the value of continuous care for patients with hypertensive intracerebral hemorrhage(HICH).Methods:A total of 80 patients with HICH who visited our hospital from January 2024 to December 2024 were selected as samples and randomly divided into two groups.The observation group received continuous care,while the control group received routine care.The Functional Independence Measure(FIM),Symptom Checklist-90(SCL-90),and complications were compared between the two groups.Results:The FIM score of the observation group was higher than that of the control group(P<0.05).The SCL-90 score of the observation group was lower than that of the control group(P<0.05).The incidence of HICH complications in the observation group was lower than that in the control group(P<0.05).Conclusion:The application of continuous care in HICH nursing can enhance patients’independent living skills outside the hospital,optimize their psychosocial adaptation,and is safe and efficient.

文摘BACKGROUND Children with critical acute abdominal conditions often undergo intestinal stoma surgery.AIM To explore the impact of a visual mobile terminal-based extended care model on caregiver competence for children with enterostomies.METHODS One hundred twenty children with enterostomies and their caregivers in a children's hospital in Beijing were divided into a control group and a study group.The control group(60 cases)received traditional telephone follow-up for continuity of care,while the study group(60 cases)used a visualization mobile terminal-based care model.The incidence of stoma-related complications,caregiver burden scale,and competence scores of children with stoma were compared between the two groups.RESULTS The primary caregiver burden score in the study group(37.22±3.17)was significantly lower than that in the control group(80.00±4.47),and the difference was statistically significant(P<0.05).Additionally,the caregiving ability score of the study group(172.08±3.49)was significantly higher than that of the control group(117.55±4.28;P<0.05).The total incidence of complications in the study group(11.7%,7/60)was significantly lower compared to the control group(33.3%,20/60;χ2=8.086,P=0.004).CONCLUSION The visual mobile terminal-based care model reduces caregiver burden,improves home care ability,lowers the incidence of complications and readmission rates,and supports successful second-stage reduction surgery for children with enterostomies.
文摘BACKGROUND Despite sharing similar pathogenic factors,cancer and coronary heart disease(CHD)occur in comparable populations at similar ages and possess similar susceptibility factors.Consequently,it is increasingly commonplace for patients to experience the simultaneous occurrence of cancer and CHD,a trend that is steadily rising.AIM To determine the impacts of continuing care on lung cancer patients with CHD following percutaneous coronary intervention(PCI).METHODS There were 94 lung cancer patients with CHD following PCI who were randomly assigned to the intervention group(n=38)and the control group(n=41).In the intervention group,continuing care was provided,while in the control group,routine care was provided.An evaluation of cardiac and pulmonary function,medication compliance,a 6-min walk test,and patient quality of life was performed.RESULTS Differences between the two groups were significant in left ventricular ejection fraction,6-min walk test,oxygen uptake,quality of life and medication compliance(P<0.05).In comparison with the control group,the enhancement in the intervention group was more significant.The intervention group had more patients with high medication compliance than the control group,with a statistically significant difference(P<0.05).CONCLUSION After undergoing PCI,lung patients with CHD could benefit from continued care in terms of cardiac and pulmonary function,medications compliance,and quality of life.
文摘Objective: for patients after bleeding ligation, the implementation of esophageal variceal bleeding treatment patients, after the discharge of the implementation of the relevant continuous nursing management mode of application value. Methods: the clinical effect of continuous nursing care for postoperative patients after holistic treatment of esophagus was effectively improved. A total of 80 patients who were hospitalized in our hospital from January 2019 to January 2020 were selected as the research subjects and divided into the conventional group (conventional nursing), the observation group (continuous nursing) and 40 cases in each group. The overall nursing effects of patients in the two groups in the implementation of self-care management ability and nursing satisfaction were comprehensively analyzed. Results: two groups of patients after ligation of esophageal variceal bleeding were compared and the value of continuous nursing care was implemented after discharge. The scores of self-management ability and nursing satisfaction in the observation group were significantly better than those in the routine group. The data of the two groups were statistically significant (P < 0.05). Conclusion: through the analysis of patients after esophageal variceal bleeding surgery, the implementation of related continuous nursing measures can effectively improve the self-management ability of patients after ligation of esophageal variceal bleeding and the clinical nursing effect of satisfaction nursing treatment. It is helpful to promote the smooth development of nursing treatment and improve the prognosis of patients with good application value.
文摘Objective: to analyze the continuation of nursing care in the operation of ureteral calculi.Methods: a total of 69 patients were selected after ureteral calculi in our hospital, all of whom went to our hospital from July 2020 to July 2021.According to computer randomization, control group: implement general care, accounting for 49.28%;observation group: implement continuous care, accounting for 50.72%.To explore the practical role of two groups.Results: the observation group was better than the control group in all aspects (P <0.05).Conclusion: continuous nursing promotes the treatment effect of ureterolithiasis patients, so it is worth popularizing.
文摘Objective: to establish the sensitivity index system for elderly care and the application value of this index in the process of continuing care for elderly diabetes patients. Methods: a total of 64 patients with senile diabetes who were admitted to our hospital for treatment from April 2018 to October 2018 were selected as the research object of this paper. They were randomly assigned to the reference group and the research group. The reference group was given routine nursing intervention. The study group used a geriatric care sensitivity index system to carry out continuous care interventions. The two groups of elderly diabetic patients in the disease awareness and other conditions were compared. Results: after the intervention, the scores of DK, SDSCA, SF-36 and nursing satisfaction of middle-aged and elderly diabetic patients in the study group were much higher than those in the reference group (P < 0.05). Conclusion: continued care of elderly diabetes in the geriatric care sensitivity index system can play a very positive role in improving the management behavior, awareness and quality of life of the patients themselves.
文摘Background:The Transitional Care Model(TCM)for nursing care has yet to be implemented in China despite its success in Western countries.However,rapid social changes have demanded an upgrade in the quality of nursing care;in 2010,the Chinese government has acknowledged the need to implement the TCM in China.Objective:This study has the following objectives:(1)perform a thorough review of the literature regarding the development and implementation of the TCM in China's Mainland within the past 5 years;(2)provide a comprehensive discussion of the current status,problems,and strategies related to the implementation of the TCM in China's Mainland;and(3)suggest strategies pertaining to the future of the TCM in China.Design:The current pertinent literature is systematically reviewed.Data sources:Systematic and manual searches in computerized databases for relevant studies regarding the TCM led to the inclusion of 26 papers in this review.Review methods:Abstracts that satisfied the inclusion criteria were reviewed independently by the two authors of this manuscript,and discrepancies were resolved through discussion.The same reviewers independently assessed the paper in its entirety for selected abstracts.Results:The present English literature reviewrevealed a paucity of updated information about the development and implementation of the TCM in China's Mainland.Nevertheless,the dramatic growth of the TCM in the past 5 years has had a vital impact within the society and in nursing development.This review also revealed numerous issues regarding the focus of the TCM.Overall implications for practiceandrecommendations for future researchare discussed.Conclusion:Despite the potential of this nursing model to have a successful and beneficial impact in China's Mainland,it remains an under-researched topic.Further research on education and training as well as premium policies for nurses under the TCM are needed.

文摘BACKGROUND Breast cancer is among the most common malignancies worldwide.With progress in treatment methods and levels,the overall survival period has been prolonged,and the demand for quality care has increased.AIM To investigate the effect of individualized and continuous care intervention in patients with breast cancer.METHODS Two hundred patients with breast cancer who received systemic therapy at The First Affiliated Hospital of Hebei North University(January 2021 to July 2023)were retrospectively selected as research participants.Among them,134 received routine care intervention(routing group)and 66 received personalized and continuous care(intervention group).Self-rating anxiety scale(SAS),self-rating depression scale(SDS),and Functional Assessment of Cancer Therapy-Breast(FACT-B)scores,including limb shoulder joint activity,complication rate,and care satisfaction,were compared between both groups after care.RESULTS SAS and SDS scores were lower in the intervention group than in the routing group at one and three months after care.The total FACT-B scores and five dimensions in the intervention group were higher than those in the routing group at three months of care.The range of motion of shoulder anteflexion,posterior extension,abduction,internal rotation,and external rotation in the intervention group was higher than that in the routing group one month after care.The incidence of postoperative complications was 18.18%lower in the intervention group than in the routing group(34.33%;P<0.05).Satisfaction with care was 90.91% higher in the intervention group than in the routing group(78.36%;P<0.05).CONCLUSION Personalized and continuous care can alleviate negative emotions in patients with breast cancer,quicken rehabilitation of limb function,decrease the incidence of complications,and improve living quality and care satisfaction.
文摘Objective:To analyze the impact of continuous quality care on HAMA,HAMD score,and quality of life in patients with acute myocardial infarction.Methods:The 100 patients with acute myocardial infarction admitted to our hospital from January 2020 to January 2021 were selected and divided into control and observation groups according to the random number table method,with 50 patients in each group.Patients in the control group were given routine care after discharge,and patients in the observation group received continuous quality care intervention to compare the changes in HAMA,HAMD and quality of life scores before and after care between the two groups.Results:Lower HAMA,HAMD scores and higher quality of life scores compared to care in the observation group,and the difference between groups before and after care was significant(P<0.05).Conclusion:Continuous quality care can effectively reduce anxiety and depression in AMI patients and improve quality of life,which deserves clinical application.
文摘Objective:To analyze the effect of using continuity of care for elderly patients with coronary heart disease(CHD)with unstable angina pectoris(UAP)and its impact on their quality of life.Methods:100 cases of elderly patients with CHD with UAP admitted to our hospital from March 2022 to March 2023 were selected and grouped into an observation group and a control group of 50 cases each according to the randomized number table method.The nursing effect and quality of life of the observation group(continuity nursing)and the control group(routine nursing)were compared.Results:The total effective rate of nursing care was 96.00%observation group and 80.00%for the control group,and the differences were significant(χ2=6.061,P<0.05).Patients in the observation group had fewer episodes(1.42±0.21)times/week and a shorter duration(5.46±0.39)min,which were better than the control group(t=3.465,2.973;P<0.05).The depression self-rating depression scale(SDS)score(42.16±6.64)and anxiety self-rating scale(SAS)score(32.26±7.35)in the observation group were lower and the quality of life was higher as compared to that of the control group(P<0.05).Conclusion:Continuous nursing care improved the nursing effect of elderly CHD with UAP patients,promoted the alleviation of UAP symptoms,improved patient mentality,and improved their quality of life.Hence,continuous nursing care possesses significant clinical application value.

文摘BACKGROUND The emergency department plays a crucial role in providing acute care to patients.Nursing interventions in this setting are essential for improving the continuity of care,enhancing patients’self-care abilities,and reducing psychological symp-toms.AIM To evaluate the effect of nursing interventions in the emergency department on these indicators in an emergency department.METHODS A retrospective analysis was conducted on 120 patients admitted to the emergency department between January 2022 and May 2023.The patients were divided into two groups:The control group(conventional nursing intervention)and the observation group(conventional nursing intervention+emergency department nursing intervention).The two groups were compared regarding continuity of care,self-care ability,psychological symptoms,and satisfaction with care.RESULTS The emergency department nursing interventions significantly improved the continuity of care,enhanced patients’self-care abilities,and reduced psycho-logical symptoms such as anxiety and depression.CONCLUSION Nursing interventions in the emergency department positively impact continuity of care,self-care,and psychological symptoms.However,it is important to acknowledge the limitations of this study,including the small number of studies,variable methodological quality,and the heterogeneity of the study population.Future research should address these limitations and further explore the effects of different types of nursing interventions in the emergency department.Additionally,efforts should be made to enhance the application and evaluation of these interventions in clinical practice.
文摘Background: Continuity of care is a distinguishing feature of primary care. Better continuity of care program showed a significant effect in controlling diabetes and it is complications. This study explores the effect of continuity of care on control of diabetes mellites in primary health care centres. Objectives: 1) To assess the effect of Continuity of care on controlling haemoglobin (Hb A1C) and fasting blood Sugar (FBS);2) To compare the control of Diabetes by using (Hb A1C and FBS) indices on same patient before and after application of chronic illness clinic;3) To identify the relation between age and gender affecting continuity of care in diabetic patient. Methods: It is a Prospective cohort study design. Included both gender and diabetic patient age above 24 years old. The data extracted from health care specialty center (HCSC) clinics in National guard hospital at Riyadh, through HCSC data base on three phases: 1) Phase 1: the data extracted of diabetic patients from October to November 2022 including MRN, diabetic patient, age, Hb A1C, Fasting blood glucose. 2) Phase 2: the same MRN extracted from phase 1 was extracted again retrospectively for six months from April to September 2022, to compare the indices before implanting the chronic illness clinic including Hb A1C, Fasting blood glucose. 3) Phase 3: prospectively from December 2022 to September 2023. Results: Among diabetic patients aged 60 years old and above showed better control of HbA1C and FBS comparing to these patients below age of 60 years old, with significant improvement of HbA1C after implanting chronic illness clinic. Conclusion: The significant improvement in the control of diabetic patients followed in primary health care centers reinforce the evidence of the importance of continuity of care.

Methods: The Walker and Avant concept analysis approach was applied in this paper. Results: The three attributes include: seamless transfer, flexible reaction and effective interaction. The antecedents were changed needs and patient centred care. In addition, the continuity of care had a significant effect on patients’ outcome, satisfaction and costs. Conclusions: This analysis provides nurses an understanding about continuity of care in both patient’s and care provider’s perspective. Continuity of care is a tailored services provided by care givers, which is an essential element of healthcare system.
文摘Objective: in this paper, family-centered continuing care was studied, and its application in rehabilitation nursing of children with cerebral palsy was analyzed to analyze its clinical impact and value on patients. Methods: a total of 40 children with cerebral palsy hospitalized in our hospital from January 2019 to December 2020 were randomly analyzed and divided into two groups for treatment. Research in view of the family as the center continue nursing (first division team) and conventional nursing (group B as control group), to better the nursing effect to decide, during the study period before and after the special effect on patient care, nursing according to the daily life score, movement function development and intellectual development of the score of the priority, and in-depth analysis and comparison. Results: the scores of motor function development, intelligence development and daily life in group A were also significantly improved, and the rate of complete disability in group A was low, P<0.05 indicated that the test results were significant. Conclusion: after the application of family-centered continuous nursing intervention in the rehabilitation nursing of children with cerebral palsy, the nursing effect is good, and it can reduce the rate of complete disability of children, improve the self-care ability of children, and improve the living standard of children and their families to a certain extent, so it can be widely adopted and applied.

文摘Objective:To explore the application effect of a doctor-nurse cooperation follow-up model based on an Internet platform in the continuation of care for patients after uro-oncology surgery.Methods:A convenient sampling method was used to select patients with urinary system tumors who underwent surgery in the Department of Urology in Grade III A general hospital in Shanghai from May to August 2022.Patients who underwent surgery from May to June 2022 were assigned to the control group,and those who underwent surgery from July to August 2022 were assigned to the experimental group.The control group received routine post-discharge nursing health education and telephone follow-up.On the basis of routine discharge guidance,the experimental group implemented the intervention method based on the Internet platform in continuation care.The levels of self-management efficacy,satisfaction,and incidence of unplanned readmission were compared one month after discharge between the two groups.Results:One month after discharge,the self-management efficacy of the experimental group(90.15±7.92)was significantly higher than that of the control group(79.10±7.84),and the patient satisfaction score(97.83±2.32)was significantly higher than that of the control group(90.23±2.58),with statistical significance(P<0.05).Additionally,the incidence of unplanned readmissions within one month after discharge in the experimental group(1.59%)was slightly lower than that in the control group(4.84%).Conclusion:The doctor-nurse cooperation follow-up model based on the Internet platform in continuation care can significantly improve the self-management efficiency of patients after discharge and enhance patient satisfaction,providing a new approach for discharge follow-up of urological tumor patients after surgery.
