Backgroundand Aim:Using simulation in learning medical interventions is a common practice.Simulation techniques provided a safe environment for trainees and students to acquire clinical skills and knowledge to practic...Backgroundand Aim:Using simulation in learning medical interventions is a common practice.Simulation techniques provided a safe environment for trainees and students to acquire clinical skills and knowledge to practice in a real clinical environment.The aim of this review is to assess the use of health simulation in the field of complementary medicine education.Methods:Three medical databases were searched from inception to 15 November 2021.The inclusion criteria were clinical trials that used simulation in the field of complementary medicine training and in English language and the full text can be retrieved.Results:After searching,495 articles were retrieved,235 articles were excluded for duplication and 249 articles were excluded after screening of titles/abstracts and applying inclusion and exclusion criteria.Finally,11 articles were included in this review.These clinical trials had 719 participants and reported positive impact of using simulation in complementary medicine education.Conclusion:Simulation wasused for training of some complementary medicine therapies such as chiropractic,acupuncture,massage therapy,cupping therapy and osteopathy.Using simulation in complementary medicine education is recommended to ensure a safe environment for training.Conducting high quality clinical trials to evaluate the use of simulation in complementary medicine education is highly recommended.展开更多
Background:The use of complementary and alternative medicine(CAM)is common among individuals with underlying chronic illnesses.Objective:This systematic review and meta-analysis investigated the global prevalence and ...Background:The use of complementary and alternative medicine(CAM)is common among individuals with underlying chronic illnesses.Objective:This systematic review and meta-analysis investigated the global prevalence and patterns of CAM use among individuals with chronic kidney disease(CKD).Search strategy:PubMed,Embase,and Cumulative Index to Nursing and Allied Health Literature Plus were searched from inception until 26th February 2024.Inclusion criteria:Original articles reporting the use of at least one type of CAM among individuals aged above 18 years old and at all stages of CKD or undergoing any form of kidney replacement therapy.Data extraction and analysis:Two independent reviewers performed the literature screening.The data were extracted from the included studies by one reviewer and cross-checked by another.Discrepancies were resolved by discussion and consensus among two reviewers.Primary information included prevalence of CAM use,types of CAM used,reasons for CAM use,factors associated with CAM use,and disclosure to healthcare providers.Meta-analyses were performed to determine the pooled prevalence of CAM use and non-disclosure of CAM using a random effect model.Results:Forty-one studies were included in this systematic review and meta-analysis.The pooled prevalence of CAM use was 43%(95%confidence interval:34%,51%),I2=99.46%.The reasons for CAM use included treatment of underlying comorbidities,complications or symptoms,maintenance of general health,and treatment of CKD.Nutritional approaches were the most common CAM modality,with 412different herbal and dietary supplements reported;psychological and physical approaches included massage therapy,relaxation techniques,and mind–body practices;and other complementary health approaches such as homeopathy,traditional Chinese medicine,and Ayurvedic medicine were also frequently reported.Factors associated with CAM use included sociodemographic characteristics such as older age,female gender,or higher income;disease or therapy factors such as not having diabetes,relying on hemodialysis,or poor adherence to medication;and patient or internal factors such as positive attitude towards CAM and perceived safety of CAM.About 66%(61%,72%)of CAM users did not disclose the use of CAM to their healthcare providers.Conclusion:CAM use is prevalent among individuals with CKD,and healthcare providers should communicate openly and effectively to emphasize the rational use of CAM to avoid potential harm.展开更多

Objective:A collaborative team is necessary to help patients achieve their healthcare goals using complementary medicine.At present,healthcare professionals do not feel sufficiently qualified to provide this service.T...Objective:A collaborative team is necessary to help patients achieve their healthcare goals using complementary medicine.At present,healthcare professionals do not feel sufficiently qualified to provide this service.This study sought to identify competencies and teaching methods for interprofessional training on complementary and integrative medicine at medical schools.Methods:Sixty-five German-speaking experts with various professional backgrounds were invited to take part in a three-round Delphi study.In the first round,predefined competencies were assessed on a seven-point Likert scale,and participants were invited to propose additional competencies that would be evaluated in the subsequent rounds.The competencies were ranked based on the participant assessments and were assigned to four relevance groups.In the second and third rounds,suitable teaching methods were identified using free-text fields and multiple-choice questions.In a final workshop,participants synthesized the outcomes of the previous sessions and derived key competencies that would be a benefit to undergraduate interprofessional training in complementary and integrative medicine at medical schools.Results:The three rounds plus final worksop were attended by 50,40,36 and 11 experts.The competencies that these experts determined to be highly relevant to teaching complementary and integrative medicine emphasized,in particular,the respectful treatment of patients and the importance of taking a medical history.From these highly relevant competencies,three key targets were agreed upon in the final workshop:students are able to 1)classify and assess complementary medical terms and methods;2)work collaboratively and integrate patients into the interprofessional team;3)involve patients and their relatives respectfully and empathetically in all healthcare processes.To achieve these competency goals,the following teaching methods were highlighted:students discuss therapy options based on authentic patient cases with each other and practice empathic patient communication incorporating complementary medicine.Further,the theoretical background of complementary medicines could be provided as online-training,to use the class sessions for hands-on exercises and interprofessional exchange and discussion.Conclusion:Despite the heterogeneous panel of experts,a consensus was reached on the competency orientation and teaching approaches.The results can promote the implementation of interprofessional training for complementary medicine in undergraduate education.展开更多

Iran has various ethnic populations, and the usage of TM/CAM by the different ethnic groups has not been researched. The aim of this study is to understand the use of TM/CAM modalities in pregnant women of different ethnic groups in southern Iran.Methods: This study was conducted in primary health care centers in Khuzestan Province, Iran, from May to August, 2016. A semi-structured valid questionnaire was used to gather information in pregnant women(n = 170).Results: About 46.5% of women in this study used TM/CAM during previous pregnancies. The majority(67%) of study participants were Arab. The Arab population showed increased use of TM/CAM during pregnancy(P = 0.014). Women with complications in former pregnancies had significantly higher incidence rate of TM/CAM use(65.8%) compared to the other group(39.6%)(P = 0.001).Conclusion: Understanding TM/CAM use and the associated factors in different ethnic groups of pregnant women will benefit the evaluation of the proper health policies and mother-infant health programs. This study contributes to the growing literature on TM/CAM, ethnic diversity and pregnant women.展开更多

Many people are looking for ...Objective: Some epidemic diseases such as coronavirus disease 2019(COVID-19) have caused many physical, psychological, and social challenges, despite the existence of treatment strategies. Many people are looking for complementary and alternative medicine(CAM) to prevent such diseases. The present study was performed to determine how some types of CAM were being used during the COVID-19 epidemic in Iran.Methods: The present study had a cross-sectional descriptive correlational design. All Iranian residents above 17 years old were eligible to participate in the study. A total of 782 participants completed a demographic information questionnaire, a questionnaire about their use of CAMs and a questionnaire about their satisfaction with the CAMs they used. Web-based sampling was conducted from 20 April 2020 to 20 August 2020.Results: Of the participants, 84% used at least one type of CAM during the COVID-19 outbreak. The most used CAMs were dietary supplements(61.3%), prayer(57.9%), and herbal medicines(48.8%). The majority of the participants(50%–66%) have used CAMs to prevent the transmission of COVID-19 or to reduce anxiety caused by the COVID-19 pandemic. CAM use was associated with sex, having children, place of residence, COVID-19 status, and source of gathering information about CAM(P < 0.05). All 32 participants who had been infected with COVID-19 used at least one type of CAM for treatment or alleviation of the disease symptoms.Conclusion: During the COVID-19 outbreak, some types of CAM, particularly nutritional supplements,medicinal herbs, and prayer, were commonly used to prevent COVID-19 and reduce pandemic-related anxiety.展开更多
Objective:To evaluate the prevalence and types of complementary and alternative medicine(CAM)modalities among patients with cancer in Karachi,Pakistan.Methods:This descriptive cross-sectional study was conducted from ...Objective:To evaluate the prevalence and types of complementary and alternative medicine(CAM)modalities among patients with cancer in Karachi,Pakistan.Methods:This descriptive cross-sectional study was conducted from March 2021 to December 2021.Five hundred patients with cancer were invited to participate in the study.Electronic databases,namely,Google scholar,Publons,EMBASE,PubMed,Chinese National Knowledge Infrastructure Database,and ResearchGate was used for questionnaire designed.The self-administered survey included questions on demographic characteristics,education level,socio-economic conditions and information about CAM therapies,prevalence,effectiveness,and common CAM modalities.Statistical analysis was conducted using SPSS software version 22.Results:Out of the 500 invited patients,433(86.6%)successfully completed and returned the questionnaires.In contrast to patients who were with younger,highly educated,professionally active,higher income,and had advanced cancer,time since diagnosis,type of treatment,cancer types and family history are significantly associated with CAM use.The results showed that 59.8%of the participants were acquainted with complementary and/or alternative medicine and considered safe owing to its natural ingredients.The prevalence of CAM usage among cancer patients was 40.9%and the most widely used CAM modality was herbal medicine(27.7%)and dietary supplements(28.8%).Patients used CAM as a complementary therapy to improve the morphological parameter(28.2%),strengthen the immune system(6.8%),and to decrease the side effects of conventional treatment(18.1%).Most of the respondents get the information regarding CAM therapy from the electronic media(43.2%)and the family members(48%)rather than healthcare personnel.Conclusions:Participants used CAM modalities along with the conventional health care practices.Further multicentre studies should be conducted to provide information regarding the usage of CAM therapies and their eventual benefits in patients with cancer.展开更多

However, research on integrating traditional and complementary medicines(T&CM) into conventional health care is lacking. T& CM approaches, may provide a cost effective option given the increase in chronic diseases with evidence of effectiveness of T& CM being key if such approaches are to be offered as part of integrative patient care, in most countries, an integrated healthcare approach is unavailable to patients. Evidence based clinical guidelines on T& CM should influence care provision. However evidence that is available may not even be cited. Poor use of evidence may possibly be a reflection of lack of knowledge and/or biases of guideline developers but additionally stakeholders vary in their perceived need for evidence to implement health policy change. Journal editors should focus on publishing high quality manuscripts, with clear rationales, design and reporting frameworks. Researchers should consider carrying out careful feasibility studies prior to conducting trials taking into account the UK Medical Research Council's framework on complex interventions. Examples of feasibility studies highlighting their importance for trial design are given in this paper. If preparation for more definitive studies is conducted this will in turn improve the quality of research being designed, executed and published and subsequently encourage the inclusion of evidence by guideline developers. Robust evidence is necessary in order to inform governments' health policies providing appropriate T&CM interventions which can ultimately help to provide best patient care.展开更多

OBJECTIVE:Complementary and alternative medicine(CAM)use has an increasing trend around the world.Despite the wild application of CAM in patients with coagulation disorders,its efficacy is still questioned by many ...OBJECTIVE:Complementary and alternative medicine(CAM)use has an increasing trend around the world.Despite the wild application of CAM in patients with coagulation disorders,its efficacy is still questioned by many studies.The aim of this study was to determine the frequency and types of CAM usage,and the factors affecting CAM use among patients with bleeding disorders.METHODS:This cross-sectional study investigated the usage of CAM with a standard validated questionnaire in coagulopathic patients who were referred to Dastgheib Hospital,an educational therapeutic center affiliated to the Shiraz University of Medical Sciences in Shiraz,Southern Iran.RESULTS:Between December 2015 and May 2016,75 patients were included in this survey.Severe hemophilia A and rare bleeding disorders were the most frequent among our participants.Overall,nearly half of the studied population(49.3%)used at least one CAM product or practices.The most common products were multivitamin(29.3%),followed by folic acid(21.3%)and chamomile(12%).32%of the patients resorted to faith healing.The main reasons of using CAM were reported to be increased general health,correction of anemia and thrombocytopenia and control of bleeding tendency.CONCLUSION:CAM is being used frequently in patients with coagulation disorders like many other chronic illnesses all around the world.Due to emerging concern regarding the safety and possible interaction with the conventional medicine,it is necessary to develop basic data about the hematologic effects of these methods in order to use them more safely.展开更多

W...Use of complementary and alternative medicine (CAM) in children with cancer is common and probably increasing. However data concerning use of CAM in siblings of children with an oncologic disease are missing so far. We conducted a population-based survey over a one-year period with 233 participants. Of the 209 respondents (90% response rate) 20% reported CAM use from the time on when the sibling was diagnosed to have cancer, compared to a prevalence rate of 48% before cancer diagnosis. The most prevalent therapies were homeopathy, anthroposophic medicine and Bach flowers. The main reasons for use were to strengthen the immune system and to achieve physical stabilization. Socio-demographic factors associated with CAM use were higher parental education and higher family income. A majority of CAM users would recommend them. 66% of the users informed a physician about CAM use. No side effects were reported.Conclusions: There is a significant decrease in CAM use in siblings of pediatric cancer patients after ascertainment of cancer in the affected sibling. Being out of the focus seems to be the main reason for non-use. Nevertheless the 20% CAM users rate CAM as very effective and would recommend CAM use to other parents.展开更多
Irritable bowel syndrome(IBS)is a common gastrointestinal disorder with a high incidence in the general population.The diagnosis of IBS is mainly based on exclusion of other intestinal conditions through the absence o...Irritable bowel syndrome(IBS)is a common gastrointestinal disorder with a high incidence in the general population.The diagnosis of IBS is mainly based on exclusion of other intestinal conditions through the absence of inflammatory markers and specific antigens.The current pharmacological treatment approaches available focus on reducing symptom severity while often limiting quality of life because of significant side effects.This has led to an effectiveness gap for IBS patients that seek further relief to increase their quality of life.Complementary and alternative medicines(CAM)have been associated with a higher degree of symptom management and quality of life in IBS patients.Over the past decade,a number of important clinical trials have shown that specific herbal therapies(peppermint oil and Iberogast?),hypnotherapy,cognitive behavior therapy,acupuncture,and yoga present with improved treatment outcomes in IBS patients.We propose an integrative approach to treating the diverse symptoms of IBS by combining the benefits of and need for pharmacotherapy with known CAM therapies to provide IBS patients with the best treatment outcome achievable.Initial steps in this direction are already being considered with an increasing number of practitioners recommending CAM therapies to their patients if pharmacotherapy alone does not alleviate symptoms sufficiently.展开更多

However, to date, little is known about why Indonesian T2DM patie...Objective: The prevalence of complementary and alternative medicine(CAM) usage among patients with type 2 diabetes mellitus(T2DM) in Indonesia is high. However, to date, little is known about why Indonesian T2DM patients choose CAM therapies, how their knowledge, attitude and practice(KAP) of CAM affects their choices, or how demographics correlate with patient choices. Therefore, this study aimed to investigate the KAP and predictors of CAM usage in T2DM patients in Indonesia.Methods: This was an observational, cross-sectional study. Patients were interviewed using a questionnaire. Chi-square tests or Fisher’s exact tests were used to compare demographic and clinical data, as well as KAP assessments, between T2DM patients who use and do not use CAM. Multivariate logistic regression analyses were used to investigate predictors of CAM usage.Results: A total of 628 T2DM patients were enrolled in the study. CAM therapies were used by 341 patients(54.3%). The most common therapies were herbs and spiritual healing, used by 100.0% and 68.3% of CAM-using patients, respectively. CAM therapies were frequently recommended by family members(91.5%), and CAM users had significantly more knowledge and more positive attitudes toward CAM therapies than nonusers. Among users, 66% said they would not follow their healthcare providers’ instructions to not use CAM therapies, and 69.5% said they would not disclose their plan to use CAM therapies with their healthcare provider. Neither demographic nor clinical characteristics were associated with CAM use. The factors that best predicted the use of CAM therapies were their availability and low cost(odds ratio [OR] = 4.59;95% confidence interval [CI]: 3.01–7.01), the belief that CAM therapies were safe(OR = 2.04;95% CI: 1.40–2.95), the belief that CAM therapies could help with diabetes control(OR = 1.75;95% CI: 1.15–2.66), and the belief that CAM therapies could help maintain physical health(OR = 1.68;95% CI: 1.13–2.49).Conclusion: CAM therapy users were more knowledgeable and had more positive attitudes toward CAM,but most of them chose not to disclose their CAM use to their healthcare providers. CAM use in Indonesia was associated with its accessibility, affordability, safety and effectivity, but not with any demographic or clinical characteristics. This study provided new evidence and insights for nurses and physicians in Indonesia that will help to design educational programs about the safety and efficacy of CAM therapies.展开更多

It is important to research the cultural context and meaning of CATM, including its definitions and examples, among different communities of psychology because CATM's use is dependent on how it is understood by the members. The aim of this pilot study is to provide an interpretation of the Indonesian psychology community's understanding of CATM through a qualitative approach. METHODS: Online interviews with open-ended questions and purposive sampling were used. Participants were dominantly psychologists or lecturers in clinical psychology area. Ten males and 12 females with an average age of 28.0 + 2.5 years voluntarily participated in this study. Interviews were audio-recorded, transcribed verbatim, and reviewed and analysed by the two authors to ensure accuracy of interpretation. RESULTS: It was found that there was no single meaning of CATM among the Indonesian community of psychology. Participants were not familiar enough with the terms and tended to use them with overlap. It can be suggested that "complementary medicine" and "alternative medicine" or "complementary- alternative medicine" combined provides more suitable terminology for use among Indonesian psychology community when communicating with other health care professionals. CONCLUSION: The understanding of the terms and examples of CATM were diverse and were often used interchangeably in the projects/interviews. It was also found that Indonesian psychologists required more education regarding CATM. In addition, future studies with more participants from various aspects of the psychology community should be conducted to capture a more representative sample.展开更多
Purpose:The main goal of this study is to discover the scientific evolution of Cancer-Related Symptoms in Complementary and Alternative Medicine research area,analyzing the articles indexed in the Web of Science datab...Purpose:The main goal of this study is to discover the scientific evolution of Cancer-Related Symptoms in Complementary and Alternative Medicine research area,analyzing the articles indexed in the Web of Science database from 1980 to 2013.Design/Methodology/Approach:A co-word science mapping analysis is performed under a longitudinal framework(1980 to 2013).The documental corpus is divided into two subperiods,1980–2008 and 2009–2013.Thus,the performance and impact rates,and conceptual evolution of the research field are shown.Findings:According to the results,the co-word analysis allows us to identify 12 main thematic areas in this emerging research field:anxiety,survivors and palliative care,meditation,treatment,symptoms and cancer types,postmenopause,cancer pain,low back pain,herbal medicine,children,depression and insomnia,inflammation mediators,and lymphedema.The different research lines are identified according to the main thematic areas,centered fundamentally on anxiety and suffering prevention.The scientific community can use this information to identify where the interest is focused and make decisions in different ways.Research limitation:Several limitations can be addressed:1)some of the Complementary and Alternative Medicine therapies may not have been included;2)only the documents indexed in Web of Science are analyzed;and 3)the thematic areas detected could change if another dataset was considered.Practical implications:The results obtained in the present study could be considered as an evidence-based framework in which future studies could be built.Originality/value:Currently,there are no studies that show the thematic evolution of this research area.展开更多

Objective:Patients with gynecological cancer commonly use complementary and alternative medicine(CAM)methods to cope with the disease.However,despite the existence of treatment strategies,the effect of fear and anxiet...Objective:Patients with gynecological cancer commonly use complementary and alternative medicine(CAM)methods to cope with the disease.However,despite the existence of treatment strategies,the effect of fear and anxiety caused by coronavirus disease 2019(COVID-19)pandemic on attitudes about CAM use is unclear.This study was carried out to investigate the effect of fear and anxiety experienced by patients with gynecological cancer during the COVID-19 pandemic on their attitudes towards the use of CAM.Methods:This is a cross-sectional and descriptive study that included 177 women with gynecological cancer;participants were recruited from a social networking site for cancer patients in Turkey between June and December 2021.Data were collected using an online survey that included the Personal Information Form,the Fear of COVID-19 Scale,the Coronavirus Anxiety Scale,and the Attitude Towards Holistic Complementary and Alternative Medicine Scale.Descriptive statistics,t-test,Pearson’s correlation test,and simple linear and multiple regression analyses were performed to analyze the data.Results:During the pandemic,55.4%of the participants reported using CAM methods,but only 22.6%were using CAM before the pandemic.The participants who used CAM during the pandemic also scored higher on the fear of COVID-19 and coronavirus anxiety scales(20.69±5.37 and 13.09±6.29,respectively)compared to the participants who did not use CAM(9.29±2.72 and 6.35±2.06,respectively).Fear of COVID-19 accounted for 52%of the CAM attitude score,while coronavirus anxiety accounted for 15%of the CAM attitude score.Fear of COVID-19 and coronavirus anxiety together accounted for 53%of CAM attitude.Conclusion:Gynecological cancer patients with high levels of COVID-19-related anxiety and fear during the pandemic used CAM more.Given that the psychological effects of the pandemic will continue in the coming years,CAM use should be evaluated as a coping strategy,especially due to the COVID-19-related anxiety and fear experienced by patients with gynecological cancer.While the rational and effective CAM methods should be supported,strategies should be developed to prevent misuse of CAM and its interference in prescribed medical treatments.展开更多

文摘Backgroundand Aim:Using simulation in learning medical interventions is a common practice.Simulation techniques provided a safe environment for trainees and students to acquire clinical skills and knowledge to practice in a real clinical environment.The aim of this review is to assess the use of health simulation in the field of complementary medicine education.Methods:Three medical databases were searched from inception to 15 November 2021.The inclusion criteria were clinical trials that used simulation in the field of complementary medicine training and in English language and the full text can be retrieved.Results:After searching,495 articles were retrieved,235 articles were excluded for duplication and 249 articles were excluded after screening of titles/abstracts and applying inclusion and exclusion criteria.Finally,11 articles were included in this review.These clinical trials had 719 participants and reported positive impact of using simulation in complementary medicine education.Conclusion:Simulation wasused for training of some complementary medicine therapies such as chiropractic,acupuncture,massage therapy,cupping therapy and osteopathy.Using simulation in complementary medicine education is recommended to ensure a safe environment for training.Conducting high quality clinical trials to evaluate the use of simulation in complementary medicine education is highly recommended.
文摘Background:The use of complementary and alternative medicine(CAM)is common among individuals with underlying chronic illnesses.Objective:This systematic review and meta-analysis investigated the global prevalence and patterns of CAM use among individuals with chronic kidney disease(CKD).Search strategy:PubMed,Embase,and Cumulative Index to Nursing and Allied Health Literature Plus were searched from inception until 26th February 2024.Inclusion criteria:Original articles reporting the use of at least one type of CAM among individuals aged above 18 years old and at all stages of CKD or undergoing any form of kidney replacement therapy.Data extraction and analysis:Two independent reviewers performed the literature screening.The data were extracted from the included studies by one reviewer and cross-checked by another.Discrepancies were resolved by discussion and consensus among two reviewers.Primary information included prevalence of CAM use,types of CAM used,reasons for CAM use,factors associated with CAM use,and disclosure to healthcare providers.Meta-analyses were performed to determine the pooled prevalence of CAM use and non-disclosure of CAM using a random effect model.Results:Forty-one studies were included in this systematic review and meta-analysis.The pooled prevalence of CAM use was 43%(95%confidence interval:34%,51%),I2=99.46%.The reasons for CAM use included treatment of underlying comorbidities,complications or symptoms,maintenance of general health,and treatment of CKD.Nutritional approaches were the most common CAM modality,with 412different herbal and dietary supplements reported;psychological and physical approaches included massage therapy,relaxation techniques,and mind–body practices;and other complementary health approaches such as homeopathy,traditional Chinese medicine,and Ayurvedic medicine were also frequently reported.Factors associated with CAM use included sociodemographic characteristics such as older age,female gender,or higher income;disease or therapy factors such as not having diabetes,relying on hemodialysis,or poor adherence to medication;and patient or internal factors such as positive attitude towards CAM and perceived safety of CAM.About 66%(61%,72%)of CAM users did not disclose the use of CAM to their healthcare providers.Conclusion:CAM use is prevalent among individuals with CKD,and healthcare providers should communicate openly and effectively to emphasize the rational use of CAM to avoid potential harm.

文摘Objective:A collaborative team is necessary to help patients achieve their healthcare goals using complementary medicine.At present,healthcare professionals do not feel sufficiently qualified to provide this service.This study sought to identify competencies and teaching methods for interprofessional training on complementary and integrative medicine at medical schools.Methods:Sixty-five German-speaking experts with various professional backgrounds were invited to take part in a three-round Delphi study.In the first round,predefined competencies were assessed on a seven-point Likert scale,and participants were invited to propose additional competencies that would be evaluated in the subsequent rounds.The competencies were ranked based on the participant assessments and were assigned to four relevance groups.In the second and third rounds,suitable teaching methods were identified using free-text fields and multiple-choice questions.In a final workshop,participants synthesized the outcomes of the previous sessions and derived key competencies that would be a benefit to undergraduate interprofessional training in complementary and integrative medicine at medical schools.Results:The three rounds plus final worksop were attended by 50,40,36 and 11 experts.The competencies that these experts determined to be highly relevant to teaching complementary and integrative medicine emphasized,in particular,the respectful treatment of patients and the importance of taking a medical history.From these highly relevant competencies,three key targets were agreed upon in the final workshop:students are able to 1)classify and assess complementary medical terms and methods;2)work collaboratively and integrate patients into the interprofessional team;3)involve patients and their relatives respectfully and empathetically in all healthcare processes.To achieve these competency goals,the following teaching methods were highlighted:students discuss therapy options based on authentic patient cases with each other and practice empathic patient communication incorporating complementary medicine.Further,the theoretical background of complementary medicines could be provided as online-training,to use the class sessions for hands-on exercises and interprofessional exchange and discussion.Conclusion:Despite the heterogeneous panel of experts,a consensus was reached on the competency orientation and teaching approaches.The results can promote the implementation of interprofessional training for complementary medicine in undergraduate education.
文摘Objective: The use of traditional medicine and complementary alternative medicine(TM/CAM) derived from herbal remedies or natural supplements is increasing worldwide, but there are limited data on the use of TM/CAM during pregnancy. Iran has various ethnic populations, and the usage of TM/CAM by the different ethnic groups has not been researched. The aim of this study is to understand the use of TM/CAM modalities in pregnant women of different ethnic groups in southern Iran.Methods: This study was conducted in primary health care centers in Khuzestan Province, Iran, from May to August, 2016. A semi-structured valid questionnaire was used to gather information in pregnant women(n = 170).Results: About 46.5% of women in this study used TM/CAM during previous pregnancies. The majority(67%) of study participants were Arab. The Arab population showed increased use of TM/CAM during pregnancy(P = 0.014). Women with complications in former pregnancies had significantly higher incidence rate of TM/CAM use(65.8%) compared to the other group(39.6%)(P = 0.001).Conclusion: Understanding TM/CAM use and the associated factors in different ethnic groups of pregnant women will benefit the evaluation of the proper health policies and mother-infant health programs. This study contributes to the growing literature on TM/CAM, ethnic diversity and pregnant women.

文摘Objective: Some epidemic diseases such as coronavirus disease 2019(COVID-19) have caused many physical, psychological, and social challenges, despite the existence of treatment strategies. Many people are looking for complementary and alternative medicine(CAM) to prevent such diseases. The present study was performed to determine how some types of CAM were being used during the COVID-19 epidemic in Iran.Methods: The present study had a cross-sectional descriptive correlational design. All Iranian residents above 17 years old were eligible to participate in the study. A total of 782 participants completed a demographic information questionnaire, a questionnaire about their use of CAMs and a questionnaire about their satisfaction with the CAMs they used. Web-based sampling was conducted from 20 April 2020 to 20 August 2020.Results: Of the participants, 84% used at least one type of CAM during the COVID-19 outbreak. The most used CAMs were dietary supplements(61.3%), prayer(57.9%), and herbal medicines(48.8%). The majority of the participants(50%–66%) have used CAMs to prevent the transmission of COVID-19 or to reduce anxiety caused by the COVID-19 pandemic. CAM use was associated with sex, having children, place of residence, COVID-19 status, and source of gathering information about CAM(P < 0.05). All 32 participants who had been infected with COVID-19 used at least one type of CAM for treatment or alleviation of the disease symptoms.Conclusion: During the COVID-19 outbreak, some types of CAM, particularly nutritional supplements,medicinal herbs, and prayer, were commonly used to prevent COVID-19 and reduce pandemic-related anxiety.
文摘Objective:To evaluate the prevalence and types of complementary and alternative medicine(CAM)modalities among patients with cancer in Karachi,Pakistan.Methods:This descriptive cross-sectional study was conducted from March 2021 to December 2021.Five hundred patients with cancer were invited to participate in the study.Electronic databases,namely,Google scholar,Publons,EMBASE,PubMed,Chinese National Knowledge Infrastructure Database,and ResearchGate was used for questionnaire designed.The self-administered survey included questions on demographic characteristics,education level,socio-economic conditions and information about CAM therapies,prevalence,effectiveness,and common CAM modalities.Statistical analysis was conducted using SPSS software version 22.Results:Out of the 500 invited patients,433(86.6%)successfully completed and returned the questionnaires.In contrast to patients who were with younger,highly educated,professionally active,higher income,and had advanced cancer,time since diagnosis,type of treatment,cancer types and family history are significantly associated with CAM use.The results showed that 59.8%of the participants were acquainted with complementary and/or alternative medicine and considered safe owing to its natural ingredients.The prevalence of CAM usage among cancer patients was 40.9%and the most widely used CAM modality was herbal medicine(27.7%)and dietary supplements(28.8%).Patients used CAM as a complementary therapy to improve the morphological parameter(28.2%),strengthen the immune system(6.8%),and to decrease the side effects of conventional treatment(18.1%).Most of the respondents get the information regarding CAM therapy from the electronic media(43.2%)and the family members(48%)rather than healthcare personnel.Conclusions:Participants used CAM modalities along with the conventional health care practices.Further multicentre studies should be conducted to provide information regarding the usage of CAM therapies and their eventual benefits in patients with cancer.
文摘Policy makers should consider improving access to treatments normally considered outside of healthcare in the west, particularly given the recent WHO strategy. However, research on integrating traditional and complementary medicines(T&CM) into conventional health care is lacking. T& CM approaches, may provide a cost effective option given the increase in chronic diseases with evidence of effectiveness of T& CM being key if such approaches are to be offered as part of integrative patient care, in most countries, an integrated healthcare approach is unavailable to patients. Evidence based clinical guidelines on T& CM should influence care provision. However evidence that is available may not even be cited. Poor use of evidence may possibly be a reflection of lack of knowledge and/or biases of guideline developers but additionally stakeholders vary in their perceived need for evidence to implement health policy change. Journal editors should focus on publishing high quality manuscripts, with clear rationales, design and reporting frameworks. Researchers should consider carrying out careful feasibility studies prior to conducting trials taking into account the UK Medical Research Council's framework on complex interventions. Examples of feasibility studies highlighting their importance for trial design are given in this paper. If preparation for more definitive studies is conducted this will in turn improve the quality of research being designed, executed and published and subsequently encourage the inclusion of evidence by guideline developers. Robust evidence is necessary in order to inform governments' health policies providing appropriate T&CM interventions which can ultimately help to provide best patient care.

文摘OBJECTIVE:Complementary and alternative medicine(CAM)use has an increasing trend around the world.Despite the wild application of CAM in patients with coagulation disorders,its efficacy is still questioned by many studies.The aim of this study was to determine the frequency and types of CAM usage,and the factors affecting CAM use among patients with bleeding disorders.METHODS:This cross-sectional study investigated the usage of CAM with a standard validated questionnaire in coagulopathic patients who were referred to Dastgheib Hospital,an educational therapeutic center affiliated to the Shiraz University of Medical Sciences in Shiraz,Southern Iran.RESULTS:Between December 2015 and May 2016,75 patients were included in this survey.Severe hemophilia A and rare bleeding disorders were the most frequent among our participants.Overall,nearly half of the studied population(49.3%)used at least one CAM product or practices.The most common products were multivitamin(29.3%),followed by folic acid(21.3%)and chamomile(12%).32%of the patients resorted to faith healing.The main reasons of using CAM were reported to be increased general health,correction of anemia and thrombocytopenia and control of bleeding tendency.CONCLUSION:CAM is being used frequently in patients with coagulation disorders like many other chronic illnesses all around the world.Due to emerging concern regarding the safety and possible interaction with the conventional medicine,it is necessary to develop basic data about the hematologic effects of these methods in order to use them more safely.

文摘Use of complementary and alternative medicine (CAM) in children with cancer is common and probably increasing. However data concerning use of CAM in siblings of children with an oncologic disease are missing so far. We conducted a population-based survey over a one-year period with 233 participants. Of the 209 respondents (90% response rate) 20% reported CAM use from the time on when the sibling was diagnosed to have cancer, compared to a prevalence rate of 48% before cancer diagnosis. The most prevalent therapies were homeopathy, anthroposophic medicine and Bach flowers. The main reasons for use were to strengthen the immune system and to achieve physical stabilization. Socio-demographic factors associated with CAM use were higher parental education and higher family income. A majority of CAM users would recommend them. 66% of the users informed a physician about CAM use. No side effects were reported.Conclusions: There is a significant decrease in CAM use in siblings of pediatric cancer patients after ascertainment of cancer in the affected sibling. Being out of the focus seems to be the main reason for non-use. Nevertheless the 20% CAM users rate CAM as very effective and would recommend CAM use to other parents.
文摘Irritable bowel syndrome(IBS)is a common gastrointestinal disorder with a high incidence in the general population.The diagnosis of IBS is mainly based on exclusion of other intestinal conditions through the absence of inflammatory markers and specific antigens.The current pharmacological treatment approaches available focus on reducing symptom severity while often limiting quality of life because of significant side effects.This has led to an effectiveness gap for IBS patients that seek further relief to increase their quality of life.Complementary and alternative medicines(CAM)have been associated with a higher degree of symptom management and quality of life in IBS patients.Over the past decade,a number of important clinical trials have shown that specific herbal therapies(peppermint oil and Iberogast?),hypnotherapy,cognitive behavior therapy,acupuncture,and yoga present with improved treatment outcomes in IBS patients.We propose an integrative approach to treating the diverse symptoms of IBS by combining the benefits of and need for pharmacotherapy with known CAM therapies to provide IBS patients with the best treatment outcome achievable.Initial steps in this direction are already being considered with an increasing number of practitioners recommending CAM therapies to their patients if pharmacotherapy alone does not alleviate symptoms sufficiently.

文摘Objective: The prevalence of complementary and alternative medicine(CAM) usage among patients with type 2 diabetes mellitus(T2DM) in Indonesia is high. However, to date, little is known about why Indonesian T2DM patients choose CAM therapies, how their knowledge, attitude and practice(KAP) of CAM affects their choices, or how demographics correlate with patient choices. Therefore, this study aimed to investigate the KAP and predictors of CAM usage in T2DM patients in Indonesia.Methods: This was an observational, cross-sectional study. Patients were interviewed using a questionnaire. Chi-square tests or Fisher’s exact tests were used to compare demographic and clinical data, as well as KAP assessments, between T2DM patients who use and do not use CAM. Multivariate logistic regression analyses were used to investigate predictors of CAM usage.Results: A total of 628 T2DM patients were enrolled in the study. CAM therapies were used by 341 patients(54.3%). The most common therapies were herbs and spiritual healing, used by 100.0% and 68.3% of CAM-using patients, respectively. CAM therapies were frequently recommended by family members(91.5%), and CAM users had significantly more knowledge and more positive attitudes toward CAM therapies than nonusers. Among users, 66% said they would not follow their healthcare providers’ instructions to not use CAM therapies, and 69.5% said they would not disclose their plan to use CAM therapies with their healthcare provider. Neither demographic nor clinical characteristics were associated with CAM use. The factors that best predicted the use of CAM therapies were their availability and low cost(odds ratio [OR] = 4.59;95% confidence interval [CI]: 3.01–7.01), the belief that CAM therapies were safe(OR = 2.04;95% CI: 1.40–2.95), the belief that CAM therapies could help with diabetes control(OR = 1.75;95% CI: 1.15–2.66), and the belief that CAM therapies could help maintain physical health(OR = 1.68;95% CI: 1.13–2.49).Conclusion: CAM therapy users were more knowledgeable and had more positive attitudes toward CAM,but most of them chose not to disclose their CAM use to their healthcare providers. CAM use in Indonesia was associated with its accessibility, affordability, safety and effectivity, but not with any demographic or clinical characteristics. This study provided new evidence and insights for nurses and physicians in Indonesia that will help to design educational programs about the safety and efficacy of CAM therapies.

文摘OBJECTIVE: Complementary, alternative and traditional medicine (CATM) is a new field, as well as a promising area of study and practice in psychology. It is important to research the cultural context and meaning of CATM, including its definitions and examples, among different communities of psychology because CATM's use is dependent on how it is understood by the members. The aim of this pilot study is to provide an interpretation of the Indonesian psychology community's understanding of CATM through a qualitative approach. METHODS: Online interviews with open-ended questions and purposive sampling were used. Participants were dominantly psychologists or lecturers in clinical psychology area. Ten males and 12 females with an average age of 28.0 + 2.5 years voluntarily participated in this study. Interviews were audio-recorded, transcribed verbatim, and reviewed and analysed by the two authors to ensure accuracy of interpretation. RESULTS: It was found that there was no single meaning of CATM among the Indonesian community of psychology. Participants were not familiar enough with the terms and tended to use them with overlap. It can be suggested that "complementary medicine" and "alternative medicine" or "complementary- alternative medicine" combined provides more suitable terminology for use among Indonesian psychology community when communicating with other health care professionals. CONCLUSION: The understanding of the terms and examples of CATM were diverse and were often used interchangeably in the projects/interviews. It was also found that Indonesian psychologists required more education regarding CATM. In addition, future studies with more participants from various aspects of the psychology community should be conducted to capture a more representative sample.

文摘Purpose:The main goal of this study is to discover the scientific evolution of Cancer-Related Symptoms in Complementary and Alternative Medicine research area,analyzing the articles indexed in the Web of Science database from 1980 to 2013.Design/Methodology/Approach:A co-word science mapping analysis is performed under a longitudinal framework(1980 to 2013).The documental corpus is divided into two subperiods,1980–2008 and 2009–2013.Thus,the performance and impact rates,and conceptual evolution of the research field are shown.Findings:According to the results,the co-word analysis allows us to identify 12 main thematic areas in this emerging research field:anxiety,survivors and palliative care,meditation,treatment,symptoms and cancer types,postmenopause,cancer pain,low back pain,herbal medicine,children,depression and insomnia,inflammation mediators,and lymphedema.The different research lines are identified according to the main thematic areas,centered fundamentally on anxiety and suffering prevention.The scientific community can use this information to identify where the interest is focused and make decisions in different ways.Research limitation:Several limitations can be addressed:1)some of the Complementary and Alternative Medicine therapies may not have been included;2)only the documents indexed in Web of Science are analyzed;and 3)the thematic areas detected could change if another dataset was considered.Practical implications:The results obtained in the present study could be considered as an evidence-based framework in which future studies could be built.Originality/value:Currently,there are no studies that show the thematic evolution of this research area.
文摘Besides conventional medicine,many patients with cancer seek complementary and alternative medicine(CAM)as an additional treatment option.Since the early 1970s,the use of CAM in cancer treatment has expanded worldwide.CAM,as a tempting option,was used by patients with cancer mainly due to easy accessibility.Patients with cancer used CAM to achieve better quality of life or to find a cure.As physicians are mainly unaware of CAM use by patients,doctor-patient communication about CAM use should be brought to a higher level.To identify circumstances in which CAM are preferred,further investigations are needed especially in biologically based therapies.Clinical-based evidence for mind-body therapies have been established,so this type of CAM can be recommended for patients with cancer during chemotherapy.Future studies are necessary to fill the gaps so that CAM users,as well as medical experts,are in position to clearly determine all the benefits and disadvantages of the mentioned therapy.
文摘Objective:Patients with gynecological cancer commonly use complementary and alternative medicine(CAM)methods to cope with the disease.However,despite the existence of treatment strategies,the effect of fear and anxiety caused by coronavirus disease 2019(COVID-19)pandemic on attitudes about CAM use is unclear.This study was carried out to investigate the effect of fear and anxiety experienced by patients with gynecological cancer during the COVID-19 pandemic on their attitudes towards the use of CAM.Methods:This is a cross-sectional and descriptive study that included 177 women with gynecological cancer;participants were recruited from a social networking site for cancer patients in Turkey between June and December 2021.Data were collected using an online survey that included the Personal Information Form,the Fear of COVID-19 Scale,the Coronavirus Anxiety Scale,and the Attitude Towards Holistic Complementary and Alternative Medicine Scale.Descriptive statistics,t-test,Pearson’s correlation test,and simple linear and multiple regression analyses were performed to analyze the data.Results:During the pandemic,55.4%of the participants reported using CAM methods,but only 22.6%were using CAM before the pandemic.The participants who used CAM during the pandemic also scored higher on the fear of COVID-19 and coronavirus anxiety scales(20.69±5.37 and 13.09±6.29,respectively)compared to the participants who did not use CAM(9.29±2.72 and 6.35±2.06,respectively).Fear of COVID-19 accounted for 52%of the CAM attitude score,while coronavirus anxiety accounted for 15%of the CAM attitude score.Fear of COVID-19 and coronavirus anxiety together accounted for 53%of CAM attitude.Conclusion:Gynecological cancer patients with high levels of COVID-19-related anxiety and fear during the pandemic used CAM more.Given that the psychological effects of the pandemic will continue in the coming years,CAM use should be evaluated as a coping strategy,especially due to the COVID-19-related anxiety and fear experienced by patients with gynecological cancer.While the rational and effective CAM methods should be supported,strategies should be developed to prevent misuse of CAM and its interference in prescribed medical treatments.
