The pathogenesis of orthopedic diseases is intimately linked to blood stasis,frequently arising from damage to primary and secondary blood channels.This disruption can lead to“blood leaving the meridians”or Qi stagn...The pathogenesis of orthopedic diseases is intimately linked to blood stasis,frequently arising from damage to primary and secondary blood channels.This disruption can lead to“blood leaving the meridians”or Qi stagnation,resulting in blood stasis syndrome.Taohong Siwu Decoction(THSWD)is a renowned classical Chinese medicinal formula extensively used to promote blood circulation and mitigate blood stasis.Clinical studies have demonstrated its significant therapeutic effects on various orthopedic conditions,particularly its anti-inflammatory and analgesic properties,as well as its efficacy in preventing deep vein thrombosis post-surgery.Despite these findings,research on THSWD remains fragmented,and its interdisciplinary impact is limited.This review aims to provide a comprehensive evaluation of the efficacy and pharmacological mechanisms of THSWD in treating common orthopedic diseases.Additionally,we employ bibliometric analysis to explore research trends and hotspots related to THSWD.We hope this review will enhance the recognition and application of THSWD in orthopedic treatments and guide future research into its pharmacological mechanisms.展开更多
OBJECTIVE:To investigate the effect of correspondence of prescriptions and the traditional Chinese medicine(TCM) syndrome of ischemic stroke.METHODS:Based on standard syndrome diagnosis and a series of effective formu...OBJECTIVE:To investigate the effect of correspondence of prescriptions and the traditional Chinese medicine(TCM) syndrome of ischemic stroke.METHODS:Based on standard syndrome diagnosis and a series of effective formulas,a randomized,double-blind,parallel-controlled clinical trial on correspondence of prescriptions and the TCM syndrome of ischemic stroke was conducted.The formulas were dynamically changed according to the patient's TCM syndrome.RESULTS:The treatment was composed of a series of formulas changed daily according to the TCM syndrome.Treatments which were composed of a series of formulas had similar short-term(1 month after onset) effects in improving mild and moderate neurological deficits compared with treatments composed of only one formula.However,patients treated with a series of formulas showed improved subjective symptoms in the short-term and a decreased risk of stroke recurrence within three months after onset.CONCLUSION:Complete(series of formulas) and incomplete(only one formula) correspondence of prescriptions and TCM syndrome have similar effects on the central clinical manifestation of ischemic stroke,such as neurological deficit and activities in daily living.However,they have significant differences regarding individual dysfunctions,such as subjective symptoms.展开更多
Objective:The paper is to comprehensively summarize and analyze the basic situation and methodological quality of clinical randomized controlled trials(RCTs)of traditional Chinese patent medicines and traditional Chin...Objective:The paper is to comprehensively summarize and analyze the basic situation and methodological quality of clinical randomized controlled trials(RCTs)of traditional Chinese patent medicines and traditional Chinese classic famous prescriptions published in 2022,to provide evidence and reasonable suggestions for the advancement of clinical research and the formulation of policies and guidelines.Methods:The Evidence Database System of clinical evidence-based evaluation of traditional Chinese medicine was searched,and data from China National Knowledge Infrastructure(CNKI),PubMed,and other databases were supplemented.The search duration was from January 1,2022,to December 31,2022.RCTs of traditional Chinese patent medicines and traditional Chinese classic famous prescriptions were included as the source of clinical evidence,and published information,sample size,intervention,control measures,treatment course,methodological quality,and key link report were analyzed and evaluated.Results:A total of 1,464 RCTs of traditional Chinese patent medicines were included,which comprised 667 types of traditional Chinese patent medicines;“traditional Chinese patent medicines+Western medicine vs.Western medicine”was the most widely used intervention and control setting,involving 417 RCTs(28.48%).A total of 245 RCTs of traditional Chinese classic famous prescriptions were included,comprising 55 types of traditional Chinese classic famous prescriptions.“Decoction+conventional treatment vs.conventional treatment”was the most widely used intervention and control setting,with 87 RCTs(35.51%).Published RCTs on traditional Chinese patent medicines and traditional Chinese classic famous prescriptions were limited by the study design and implementation.Most“allocation concealment”and“blinding of patients and personnel”were rated as medium to high risk.There are insufficient reports on key research links such as experimental registration and ethical approval.Conclusions:The number of RCTs on traditional Chinese patent medicines has decreased in 2022,but there has been a slight improvement in the research quality and impact.There are relatively few studies on traditional Chinese classic famous prescriptions.Measures must be taken to improve clinical trial design,implementation,and reporting.Methodological experts should be invited to provide professional technical guidance on the trial design.In the research implementation process,attention should be paid to quality control,particularly the standardization of the randomized execution.展开更多

Pinelliae Rhizoma(PR),known as Banxia in Chinese,Hange in Japanese,and Banha in Korean,is a renowned herbal medicine in East Asia derived from the dry tuber of Pinellia ternata(Thunb.)Breit.(PT).It is extensively util...Pinelliae Rhizoma(PR),known as Banxia in Chinese,Hange in Japanese,and Banha in Korean,is a renowned herbal medicine in East Asia derived from the dry tuber of Pinellia ternata(Thunb.)Breit.(PT).It is extensively utilized in dispensing granules,classical prescriptions,and herbal formulas to treat various conditions,including cough,infection,phlegm,nausea,asthma,and inflammation.Despite numerous studies on PR and its classical prescriptions over recent decades,a comprehensive synthesis of available evidence regarding its multifunctional roles and therapeutic potential is lacking.This review aims to address this gap by examining emerging evidence from metabonomics,preclinical studies,and clinical trials,while exploring potential trends and prospects for future research.A systematic literature search was conducted across six electronic databases,including Pub Med,Web of Science,Scopus,Science Direct,Wanfang,and China National Knowledge Infrastructure,to identify relevant articles on PR published until March 2023.PR contains 107 compounds with diverse pharmacological activities,including anti-inflammatory,immune regulatory,anti-viral,anti-cancer,anti-asthma,antitussive and expectorant,antioxidant,anti-obesity,anti-atherosclerosis,antimicrobial,emetic and anti-emetic,anti-convulsant and anti-epileptic,sedative and hypnotic,learning and memory enhancement,and anti-depressant effects.Metabonomic studies suggest that raw PR may exhibit cardiotoxicity and pregnancy toxicity while showing no apparent hepatorenal toxicity.However,limited pharmacokinetic investigations on PR constrain its clinical translation.Furthermore,clinical safety data on PR is scarce,with only four clinical trials assessing its positive effects in pediatric epilepsy,nausea and vomiting,soft tissue injury,and chronic sinus tract.This review aims to enhance understanding of PR and provide valuable information and recommendations for further research and development of herbal medicine.展开更多
The official release of the“Drug Registration Management Measures”in January 2020 marked a new milestone in the classification management of traditional Chinese medicine(TCM)registration,ushering in a new era for th...The official release of the“Drug Registration Management Measures”in January 2020 marked a new milestone in the classification management of traditional Chinese medicine(TCM)registration,ushering in a new era for the registration and application of innovative TCM drugs.This article provides a detailed analysis of the approval status of innovative TCM drugs,improved new drugs,and ancient classic prescription compound preparations from 2020 to 2024,revealing the significant achievements made in the inheritance and innovation of TCM in China.The study found that the number of New Drug Applications(NDAs)and Investigational New Drug Applications(INDs)have increased annually,with a particularly notable surge in the submission of applications for classic prescription compound preparations.The approval rates for innovative TCM drugs and improved new drugs have significantly increased,along with the streamlining of review and approval process,leading to shorter development cycles and reduced costs.Key findings include:(1)Enterprises demonstrate strong enthusiasm for the research and development of innovative and improved TCM drugs,actively engaging in various types of drug R&D and submissions;(2)Classic prescription compound preparations have become a focal point for enterprises due to their shorter development cycles and lower costs;(3)Improvement to the“three-combined”evidence system has accelerated the R&D process by incorporating real-world data.Through this study,we aim to provide valuable insights for TCM R&D enterprises and policymakers,promoting the sustained prosperity and development of the TCM sector.展开更多

Objective:This study aimed to examine the mechanism of classic ancient prescription of Chinese medicine Yinchenhao decoction in treating jaundice based on network pharmacology.Method:An oral bioavailability of≥30%,a ...Objective:This study aimed to examine the mechanism of classic ancient prescription of Chinese medicine Yinchenhao decoction in treating jaundice based on network pharmacology.Method:An oral bioavailability of≥30%,a drug likeness of≥0.18,and literature studies were used to screen for Yinchen(Artemisiae scopariae herba),Zhizi(Gardeniae fructus),Dahuang(Rhei radix et rhizome)in the Chinese Medicine System Pharmacology Database and Analysis Platform.The active ingredient was introduced into the PubChem database to collect drug component targets and import into the Uniprot database for gene standardization.The target gene of Yinchen(Artemisiae scopariae herba)was screened via Human Gene Database(GeneCards).Then,use the Cytoscape 3.7.2 software was used for network visualization analysis,and the R3.6.1 software was used for gene ontology functional and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analyses.Results:We collected a total of 47 active constituents of classic ancient prescription of Chinese medicine Yinchenhao decoction,of which 17 were related to jaundice;189,9 targets of jaundice were screened,of which 41 were interdigitated with the targets of classic ancient prescription of Chinese medicine Yinchenhao decoction.Gene ontology functional enrichment analysis revealed 111 biological processes,14 cellular components,and 28 molecular functions,and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis showed 34 Kyoto Encyclopedia of Genes and Genomes pathways including hepatocellular carcinoma,PI3K-Akt signaling pathway,HIF-1 signaling pathway,prolactin signaling pathway,and non-alcoholic fatty liver disease.Conclusion:Based on the network pharmacology,the analysis of jaundice and classic ancient prescription of Chinese medicine Yinchenhao decoction provides a novel idea and direction for the study of classic ancient prescription of Chinese medicine Yinchenhao decoction in the treatment of jaundice.展开更多

Chronic obstructive pulmonary disease(COPD)is a heterogeneous disease with a complex pathogenesis,which causes the lack of precise and personalized prevention,control,and treatment approaches in clinical practice.Trad...Chronic obstructive pulmonary disease(COPD)is a heterogeneous disease with a complex pathogenesis,which causes the lack of precise and personalized prevention,control,and treatment approaches in clinical practice.Traditional Chinese medicine(TCM)with multiple components acting on multiple targets plays a role in the prevention and treatment of COPD.After evaluating the treatment of cough,dyspnea,and lung distension based on syndrome differentiation in TCM,we summarized the commonly used TCMpreparations for treating COPD.These preparations are mainly composed of medicines for clearing interior heat and releasing exterior,tonifying,activating blood and resolving stasis,resolving phlegm,relieving cough and breathlessness,and regulating Qi movement.Because all of them contain a variety of active ingredients such as flavonoids,terpenoids,and phenols,we analyzed the mechanisms of the classic prescriptions alone or in combination with othermedicines or therapeuticmethods.These mechanisms involve counteracting inflammation,oxidative stress,and fibrosis,inhibiting apoptosis,alleviating airway remodeling,and enhancing immunity.Meantime,we summarized the existing problems in the treatment of COPD by TCM.This review provides a scientific basis for the research and treatment of COPD in the future.展开更多

文摘Gegen Qinlian Decoction is one of the commonly used classical prescriptions,which consists of fourherbs:Gegen(Puerariae Lobatae Radix),Huangqin(Scutellariae Radix),Huanglian(Coptidis Rhizoma),and Zhigancao Glycyrrhizae Radix et Rhizoma Praeparata cum Melle.The clinical application experience of Gegen Qinlian Decoction is as follows:first,according to Zhongjing Zhang's original text,heat distressing the large intestine with unresolved exterior syndrome is theoriginal meaningof Gegen Qinlian Decoction syndrome.In clinical practice,bacillary dysentery,gastrointestinal cold,etc.,belong to simultaneous exterior and interior disease,which are completely consistent with the original meaning of this prescription;second,the clinical digestive system diseases with exuberant heat of the large intestine as the core manifestation cannot be treated merely based on the exterior syndrome;third,due to the interior-exterior relationship between the lung and the large intestine and between the meridians,the use of Gegen(Puerariae Lobatae Radix),Huangqin(Scutellariae Radix),and Huanglian(Coptidis Rhizoma)can make the source of body clear,and the waste qi can be released and thepore can be easily opened,so this prescription can also be used for exogenous fever;fourth,with multidimensional comprehensive understanding of the syndrome,pathogenesis,symptoms and pharmacology,Gegen Qinlian Decoction has also been further applied to the treatment of hypertension,diabetes,etc.
文摘Objective:This study aimed to examine the mechanism of classic ancient prescription of Chinese medicine Yinchenhao decoction in treating jaundice based on network pharmacology.Method:An oral bioavailability of≥30%,a drug likeness of≥0.18,and literature studies were used to screen for Yinchen(Artemisiae scopariae herba),Zhizi(Gardeniae fructus),Dahuang(Rhei radix et rhizome)in the Chinese Medicine System Pharmacology Database and Analysis Platform.The active ingredient was introduced into the PubChem database to collect drug component targets and import into the Uniprot database for gene standardization.The target gene of Yinchen(Artemisiae scopariae herba)was screened via Human Gene Database(GeneCards).Then,use the Cytoscape 3.7.2 software was used for network visualization analysis,and the R3.6.1 software was used for gene ontology functional and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analyses.Results:We collected a total of 47 active constituents of classic ancient prescription of Chinese medicine Yinchenhao decoction,of which 17 were related to jaundice;189,9 targets of jaundice were screened,of which 41 were interdigitated with the targets of classic ancient prescription of Chinese medicine Yinchenhao decoction.Gene ontology functional enrichment analysis revealed 111 biological processes,14 cellular components,and 28 molecular functions,and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis showed 34 Kyoto Encyclopedia of Genes and Genomes pathways including hepatocellular carcinoma,PI3K-Akt signaling pathway,HIF-1 signaling pathway,prolactin signaling pathway,and non-alcoholic fatty liver disease.Conclusion:Based on the network pharmacology,the analysis of jaundice and classic ancient prescription of Chinese medicine Yinchenhao decoction provides a novel idea and direction for the study of classic ancient prescription of Chinese medicine Yinchenhao decoction in the treatment of jaundice.
文摘Gastroesophageal reflux disease (GERD) is a high-incidence digestive system disease. Western medicine mainly uses drugs such as proton pump inhibitors to inhibit gastric acid secretion, but some patients are accompanied by symptoms such as non-acid reflux and gas reflux, which cannot effectively treat the disease. It is necessary to actively explore other treatment schemes. Traditional Chinese medicine (TCM) has a long history of research on gastroesophageal reflux disease, which emphasizes the treatment based on syndrome differentiation as a whole. Through the treatment of various and multi-component TCM prescriptions, the patient’s body condition can be adjusted, and the treatment effect on gastroesophageal reflux disease is reliable, which has obvious therapeutic advantages. To further clarify the treatment of gastroesophageal reflux disease, this study reviewed and analyzed the research progress of the treatment of liver disease with modified prescriptions, and the report is as follows.

文摘Chronic obstructive pulmonary disease(COPD)is a heterogeneous disease with a complex pathogenesis,which causes the lack of precise and personalized prevention,control,and treatment approaches in clinical practice.Traditional Chinese medicine(TCM)with multiple components acting on multiple targets plays a role in the prevention and treatment of COPD.After evaluating the treatment of cough,dyspnea,and lung distension based on syndrome differentiation in TCM,we summarized the commonly used TCMpreparations for treating COPD.These preparations are mainly composed of medicines for clearing interior heat and releasing exterior,tonifying,activating blood and resolving stasis,resolving phlegm,relieving cough and breathlessness,and regulating Qi movement.Because all of them contain a variety of active ingredients such as flavonoids,terpenoids,and phenols,we analyzed the mechanisms of the classic prescriptions alone or in combination with othermedicines or therapeuticmethods.These mechanisms involve counteracting inflammation,oxidative stress,and fibrosis,inhibiting apoptosis,alleviating airway remodeling,and enhancing immunity.Meantime,we summarized the existing problems in the treatment of COPD by TCM.This review provides a scientific basis for the research and treatment of COPD in the future.
