Objective To identify the novel species ‘Mycobacterium fukienense' sp. nov of Mycobacterium chelonoe/abscessus complex from tuberculosis patients in Fujian Province, China. Methods Five of 27 clinical Mycobucterium isolates (CIs) were previously identified as M. chelonoe/obscessus complex by sequencing the hsp65, rpoB, 165-235 rRNA internal transcribed spacer region (its), recA and sodA house-keeping genes commonly used to describe the molecular characteristics of Mycobocterium. Clinical Mycobecterium isolates were classified according to the gene sequence using a clustering analysis program. Sequence similarity within clusters and diversity between clusters were analyzed. Results The 5 isolates were identified with distinct sequences exhibiting 99.8% homology in the hsp65 gene. However, a complete lack of homology was observed among the sequences of the rpoB, 165-235 rRNA internal tronscribed spacer region (its), sodA, and recA genes as compared with the M. obscessus. Furthermore, no match for rpoB, sodA, and recA genes was identified among the published sequences. Conclusion The novel species, Mycobacterium fukienense, is identified from tuberculosis patients in Fujian Province, China, which does not belong to any existing subspecies of M. cheloneo/abscessus complex. 展开更多

Objective To gain greater insight into the prevalence drug resistant profiles of M. abscessus from a general hospital in Beijing, China. Methods Partial gene sequencing of 16 S, hsp65, and rpo B were used to distinguish the species of NTM isolates. All strains identified as M. abscessus were further enrolled in the drug susceptibility testing by using broth microdilution method. Results We found that M. avium complex was the most frequent NTM organism, accounting for 54.1%(33/61) of all isolates. Behind MAC, the second most common organisms were M. abscessus(22 out of 61, 36.1%). Average rates of resistance were 4.5% for AMK, 9.1% for LZD, and 13.6% for CLA, respectively. In contrast, resistance to LEV(17/22, 77.3%), IMI(9/22, 40.9%), and SMX(10/22, 45.5%) was noted in more than 40% of M. abscessus isolates. DNA sequencing revealed that all the CLA-resistant isolates harbored nucleotide substitutions in position 2058(1/3, 33.3%) or 2059(2/3, 66.7%) of 23 S r RNA. Conclusion In conclusion, our data demonstrated that M. intracellulare and M. abscessus were the most common NTM species in the general hospital of Beijing. CLA, AMK, LZD showed promising activity, where as LEV, IMI, and SMX exhibited poor activity against M. abscessus in vitro. 展开更多

Objective To determine the susceptibilities of M. tuberculosis H37Ra and M. chelonei subsp. absecessus to several frequently-used disinfectants and to evaluate the practicability of surrogating M. tuberculosis by the latter. Methods A suspension quantitative bactericidal test was set up in accordance with Chinese Technique Standard for Disinfection to evaluate the susceptibility of each mycobacteria strain to each selected disinfectant. Killing log value was used as criterion in comparing the susceptibility to disinfectants between the two strains. Results M. chelonei subsp. abscessus was more resistant to chlorine disinfectant than M. tuberculosis while the two strains were similarly resistant to iodophor disinfectant, peracetic acid, alcohol and glutaraldehyde disinfectant. Conclusion M. chelonei subsp. abscessus has the potential to surrogate M. tuberculosis in evaluating mycobactericidal efficacies of disinfectants. 展开更多

Objective Moxifloxacin(MFX)shows good in vitro activity against Mycobacterium abscessus and can be a possible antibiotic therapy to treat M.abscessus infection;however,other studies have shown a lower or no activity.We aimed to evaluate MFX activity against M.abscessus using zebrafish(ZF)model in vivo.Methods A formulation of M.abscessus labeled with CM-Dil was micro-injected into ZF.Survival curves were determined by recording dead ZF every day.ZF were lysed,and colony-forming units(CFUs)were enumerated.Bacteria dissemination and fluorescence intensity in ZF were analyzed.Inhibition rates of MFX and azithromycin(AZM,positive control)were determined and compared.Results Significantly increased survival rate was observed with different AZM concentrations.However,increasing MFX concentration did not result in a significant decrease in ZF survival curve.No significant differences in bacterial burdens by CFU loads were observed between AZM and MFX groups at various concentrations.Bacterial fluorescence intensity in ZF was significantly correlated with AZM concentration.However,with increasing MFX concentration,fluorescence intensity decreased slightly when observed under fluorescence microscope.Transferring rates at various concentrations were comparable between the MFX and AZM groups,with no significant difference.Conclusion MFX showed limited efficacy against M.abscessus in vivo using ZF model.Its activity in vivo needs to be confirmed. 展开更多

BACKGROUND The incidence of infection with Mycobacterium abscessus(M.abscessus)has increased in recent years.This increase is partly associated with invasive cosmetic procedures.CASE SUMMARY The purpose of this case summary is to increase clinicians'awareness of M.abscessus infection and reduce mycobacterial infection caused by cosmetic procedures.We report the case of a 45-year-old woman who received acetyl hexapeptide-8(argireline)injections in the forehead and temples,and erythema,nodules,and abscesses appeared at the injection sites after one week.The pus specimens were examined by microbiological culture and confirmed to be positive for M.abscessus.Clarithromycin 500 mg twice daily and moxifloxacin 400 mg once daily were administered for 5 mo and the lesions gradually subsided.CONCLUSION We report here for the first time a case of infection with M.abscessus after argireline injection.This condition is easily misdiagnosed as a common bacterial infection.Microbiological examinations are helpful for diagnosis and standardized cosmetic procedures can prevent infection with M.abscessus. 展开更多

A delayed diagnosis can cause a wide range of bone destruction and joint contracture, which highlights the importance of early recognition of osteomyelitis. BACKGROUND Mycobacterium abscessus(M.abscessus)is a rapidly growing mycobacterium and ubiquitous in the environment,which infrequently causes disease in humans.However,it can cause cutaneous or respiratory infections among immunocompromised hosts.Due to the resistance to most antibiotics,the pathogen is formidable and difficult-to-treat.CASE SUMMARY Here,we present a case of catheter-related M.abscessus infections in a patient with motor neurone disease.Catheter and peripheral blood cultures of the patient showed positive results during Gram staining and acid-fast staining.The alarm time of catheter blood culture was 10.6 h earlier than that of peripheral blood.After removal of the peripherally inserted central catheter,secretion and catheter blood culture were positive.M.abscessus was identified by matrix-assisted laser desorption ionization-time of flight mass spectrometry and 16S rDNA sequencing.CONCLUSION For catheter-related M.abscessus infection,rapid diagnosis and timely and adequate antimicrobial therapy are crucial. 展开更多

Mycobacterium abscessus,one of the most antimicrobial-resistant bacteria,is increasingly recognized as the cause of infections that are difficult to treat.Novel genetic manipulation tools are required to elucidate the biology,pathogenesis,and antibiotic resistance mechanisms of M.abscessus.In this study,we modified the method used to prepare M.abscessus electrocompetent cells to achieve efficient transformation,and then optimized the CRISPR-Cas9-assisted genome-editing tools to allow efficient genetic manipulation.Using these tools,we constructed 66 efflux pump mutants of M.abscessus and investigated their roles in drug resistance and virulence.We found that different efflux pumps play distinct roles in drug resistance and survival in Galleria mellonella larvae.Finally,we confirmed that MAB_2806-2807,involved in transportation of triacylglycerides,is vital for the drug resistance and virulence of M.abscessus.The molecular biology tools developed in this study will facilitate molecular research on M.abscessus.In addition,the study on efflux pumps might provide new targets for the development of new drugs and treatment regimens for M.abscessus infection. 展开更多

血红素氧化酶1调控自噬对脓肿分枝杆菌胞内生长的影响

目的:探讨血红素氧化酶1(HO-1)调控自噬对脓肿分枝杆菌(M.abs)胞内生长的影响。方法:M.abs标准株ATCC19977按照指定的感染复数(MOI)刺激小鼠RAW264.7巨噬细胞特定时间,Western blot检测HO-1蛋白表达水平和自噬相关蛋白Atg5、LC3Ⅱ、p62... 目的:探讨血红素氧化酶1(HO-1)调控自噬对脓肿分枝杆菌(M.abs)胞内生长的影响。方法:M.abs标准株ATCC19977按照指定的感染复数(MOI)刺激小鼠RAW264.7巨噬细胞特定时间,Western blot检测HO-1蛋白表达水平和自噬相关蛋白Atg5、LC3Ⅱ、p62蛋白表达水平。采用HO-1特异性诱导剂CoPP和酶活性抑制剂SnPP预处理巨噬细胞12 h,加入M.abs共孵育2 h,阿米卡星去除胞外菌后继续培养至指定时间,CCK-8法检测细胞活性;Western blot、LysoTracker Red检测HO-1蛋白与自噬相关蛋白的调节关系;菌落计数、ELISA分别检测细胞内细菌存活情况和TNF-α分泌水平。结果:与对照组相比,M.abs能诱导HO-1和Atg5、LC3Ⅱ、p62蛋白表达水平升高;HO-1过表达和抑制能有效调控M.abs诱导的Atg5、LC3Ⅱ和p62表达。LysoTracker Red、菌落计数和ELISA结果显示,SnPP促进细胞内溶酶体增加,显著抑制M.abs胞内繁殖并减少TNF-α分泌。结论:抑制HO-1能增强M.abs诱导的自噬流,减少M.abs胞内生长,为研发以抑制HO-1为靶点的靶向药物提供科学依据。 展开更多

目的了解河南省脓肿分枝杆菌复合群耐药情况及基因分型结果,为脓肿分枝杆菌复合群感染防控提供支持。方法收集河南省2019—2022年分离自各地市结核病定点防治机构送检标本32株脓肿分枝杆菌复合群菌菌株。使用微孔板法检测脓肿分枝杆菌... 目的了解河南省脓肿分枝杆菌复合群耐药情况及基因分型结果,为脓肿分枝杆菌复合群感染防控提供支持。方法收集河南省2019—2022年分离自各地市结核病定点防治机构送检标本32株脓肿分枝杆菌复合群菌菌株。使用微孔板法检测脓肿分枝杆菌复合群菌对12种药物的敏感性,采用熔解曲线法和基因测序检测菌株耐药相关基因突变情况。用18位点可变数目串联重复序列(variable-number tandem repeat,VNTR)方法进行基因分型,并用BioNumerics软件对分型数据进行聚类分析。结果经鉴定32株菌株包括18株脓肿分枝杆菌、13株马赛分枝杆菌和1株博莱分枝杆菌。药敏结果显示,阿米卡星、克拉霉素、替加环素、利奈唑胺和头孢西丁等药物在体外对脓肿分枝杆菌复合群有较好的抗菌效果,耐药率分别为6.25%(2/32)、15.63%(5/32)、15.63%(5/32)、25.00%(8/32)和25.00%(8/32)。菌株对甲氧苄啶/磺胺甲恶唑、环丙沙星、美罗培南、多西环素、妥布霉素、亚胺培南和莫西沙星的耐药率较高,分别为87.50%(28/32)、81.25%(26/32)、81.25%(26/32)、78.13%(25/32)、65.63%(21/32)、65.63%(21/32)和59.38%(19/32)。脓肿分枝杆菌对头孢西丁和14 d时克拉霉素的耐药率高于马赛分枝杆菌,差异有统计学意义(P<0.05)。18位点VNTR将32株脓肿分枝杆菌复合群菌株分为28个不同的基因型,包括来自3个簇的7株菌株和25个独特模式的分离株,HunterGaston判别指数为0.990。结论河南省脓肿分枝杆菌复合群主要由脓肿分枝杆菌和马赛分枝杆菌组成,阿米卡星、克拉霉素、替加环素、利奈唑胺和头孢西丁等药物在体外对河南省分离的脓肿分枝杆菌复合群菌株具有较好的抗菌活性,18位点的VNTR分型方法对脓肿分枝杆菌复合群菌株具有较好的分型能力。 展开更多

目的分析脓肿分枝杆菌肺病的CT征象,并探讨其在脓肿分枝杆菌肺病与其他分枝杆菌肺病鉴别诊断中的应用价值。方法选取非结核分枝杆菌(NTM)肺病患者259例,根据菌种鉴定结果分为脓肿分枝杆菌肺病77例(脓肿分枝杆菌组)、其他分枝杆菌肺病182... 目的分析脓肿分枝杆菌肺病的CT征象,并探讨其在脓肿分枝杆菌肺病与其他分枝杆菌肺病鉴别诊断中的应用价值。方法选取非结核分枝杆菌(NTM)肺病患者259例,根据菌种鉴定结果分为脓肿分枝杆菌肺病77例(脓肿分枝杆菌组)、其他分枝杆菌肺病182例(其他分枝杆菌组)。所有研究对象接受常规胸部CT平扫,记录CT征象。采用二元Logistic回归模型筛选脓肿分枝杆菌肺病的危险因素。采用受试者工作特征(ROC)曲线评价CT征象对脓肿分枝杆菌肺病的鉴别诊断价值。结果脓肿分枝杆菌组女性占比多于其他分枝杆菌组,年龄小于其他分枝杆菌组(P均<0.05);其他分枝杆菌组间质性改变、肺实变、单发或多发结节、球形病变及团块影、空洞特征、支气管扩张、胸膜增厚、肺气肿占比均高于脓肿分枝杆菌组(P均<0.05),支气管扩张占比低于脓肿分枝杆菌组(P<0.05),而两组出现斑片影、片状磨玻璃影、支气管管壁增厚、树芽征、小叶中心结节、肺索条影、并发叶性肺不张、肺损毁、淋巴结肿大、淋巴结钙化、胸腔积液、钙化占比比较差异均无统计学意义(P均>0.05)。二元Logistic回归分析发现,支气管扩张是脓肿分枝杆菌肺病发生的危险因素(P<0.05),而厚壁空洞则为其保护因素(P<0.05)。ROC曲线分析发现,CT征象支气管扩张、厚壁空洞单独鉴别诊断脓肿分枝杆菌肺病的曲线下面积(AUC)分别为0.559(95%CI:0.485~0.633,P<0.05)、0.292(95%CI:0.224~0.360,P>0.05),二者联合鉴别诊断脓肿分枝杆菌肺病的AUC为0.735(95%CI:0.671~0.798,P<0.05),CT征象支气管扩张、厚壁空洞联合鉴别诊断脓肿分枝杆菌肺病的效能优于二者单独(Z分别为1.50、5.30,P均<0.05)。结论支气管扩张和厚壁空洞是脓肿分枝杆菌肺病典型的CT征象;CT征象支气管扩张、厚壁空洞对脓肿分枝杆菌肺病均有一定鉴别诊断价值,二者联合鉴别诊断价值更高。 展开更多

目的:探讨蓝萼甲素调控Toll样受体4(TLR4)/核转录因子-κB(NF-κB)通路对脓肿分枝杆菌生长及生物膜形成的影响。方法:脓肿分枝杆菌菌株ATCC19977分为空白组、低剂量蓝萼甲素组、中剂量蓝萼甲素组、高剂量蓝萼甲素组、TAK-242组、高剂量... 目的:探讨蓝萼甲素调控Toll样受体4(TLR4)/核转录因子-κB(NF-κB)通路对脓肿分枝杆菌生长及生物膜形成的影响。方法:脓肿分枝杆菌菌株ATCC19977分为空白组、低剂量蓝萼甲素组、中剂量蓝萼甲素组、高剂量蓝萼甲素组、TAK-242组、高剂量蓝萼甲素+LPS组,阿尔玛蓝液体药敏法测定蓝萼甲素对ATCC19977的抑制率大于等于90%对应的最低药物浓度(MIC 90);扫描电镜观察ATCC19977生物膜形成;结晶紫染色检测ATCC19977生物膜总生物量;MTT检测ATCC19977生物膜代谢活性;qRT-PCR检测ATCC19977中MAB_2030、MAB_2028 mRNA表达;Western blot检测ATCC19977中TLR4、p-NF-κB p65蛋白。结果:蓝萼甲素对ATCC19977的MIC 90为10μmoL/L。与空白组比较,低剂量蓝萼甲素组、中剂量蓝萼甲素组、高剂量蓝萼甲素组ATCC19977数量减少,生物膜被破坏,表现为厚度变薄,且呈松散状态,ATCC19977生物膜总生物量、生物膜代谢活性、ATCC19977中MAB_2030、MAB_2028 mRNA表达及TLR4、p-NF-κB p65蛋白表达降低,且高剂量蓝萼甲素组趋势最明显,TAK-242组对应指标变化趋势与上述一致(P<0.05);与高剂量蓝萼甲素组比较,高剂量蓝萼甲素+LPS组ATCC19977数量增多,生物膜破坏程度有所减弱,ATCC19977生物膜总生物量、生物膜代谢活性、ATCC19977中MAB_2030、MAB_2028 mRNA表达及TLR4、p-NF-κB p65蛋白表达升高(P<0.05)。结论:蓝萼甲素可能通过抑制TLR4/NF-κB通路抑制ATCC19977生长及生物膜形成。 展开更多

脓肿分枝杆菌复合群(MABC)是非结核分枝杆菌(NTM)中最常见的快速生长型NTM之一,以免疫低下人群为主要易感群体,可感染肺部、关节及皮肤等部位。由于MABC在固体培养基上生长3~7 d才可见菌落且抗酸染色涂片多为阴性,导致MABC检出困难。此... 脓肿分枝杆菌复合群(MABC)是非结核分枝杆菌(NTM)中最常见的快速生长型NTM之一,以免疫低下人群为主要易感群体,可感染肺部、关节及皮肤等部位。由于MABC在固体培养基上生长3~7 d才可见菌落且抗酸染色涂片多为阴性,导致MABC检出困难。此外,MABC具有独特的细胞壁结构与代谢机制,能够抵御巨噬细胞和中性粒细胞等免疫细胞攻击。另外,MABC具有天然、多耐药等特性,对大多数抗结核药物效果有限。值得注意的是,NTM的药敏试验尚未在国内大多数实验室开展,专门针对快速生长型NTM开展的药敏试验更是鲜见,导致其治疗周期长、疗效不理想。本文报道了1例由MABC致急性化脓性腮腺炎病例的诊治、病原学检出以及药敏试验过程,旨在提高临床医师的诊疗水平和检验人员病原学的检出能力。 展开更多

目的研究消毒剂乙酰丙酸-十二烷基硫酸钠(LVA-SDS)对脓肿分枝杆菌、龟分枝杆菌、偶发分枝杆菌的杀灭作用,为常见致病性快速生长型非结核分枝杆菌(NTM)病的预防提供新方法。方法采用微量培养法和悬液定量杀菌试验,分别检测LVA-SDS的最低... 目的研究消毒剂乙酰丙酸-十二烷基硫酸钠(LVA-SDS)对脓肿分枝杆菌、龟分枝杆菌、偶发分枝杆菌的杀灭作用,为常见致病性快速生长型非结核分枝杆菌(NTM)病的预防提供新方法。方法采用微量培养法和悬液定量杀菌试验,分别检测LVA-SDS的最低抑菌浓度(MIC)和最低杀菌浓度(MBC);以1000 mg/L的含氯消毒剂(次氯酸钠)为对照,检测不同时间及不同浓度的LVA-SDS对脓肿分枝杆菌、龟分枝杆菌和偶发分枝杆菌的杀灭作用。结果LVA-SDS对脓肿分枝杆菌、龟分枝杆菌、偶发分枝杆菌作用后的MIC值分别为0.0625%LVA+0.00625%SDS、0.03125%LVA+0.003125%SDS、0.125%LVA+0.0125%SDS,MBC值分别为0.25%LVA+0.025%SDS、0.25%LVA+0.025%SDS、1%LVA+0.1%SDS。在一定范围内,随着LVA-SDS浓度的增加和时间(1~30 min)的延长,对脓肿分枝杆菌、龟分枝杆菌、偶发分枝杆菌的杀灭作用逐渐增强。18%LVA+1.8%SDS作用5 min、6%LVA+0.6%SDS作用20 min对脓肿分枝杆菌的杀灭率均达100.00%,18%LVA+1.8%SDS作用10 min、10%LVA+1%SDS作用25 min对龟分枝杆菌的杀灭率均达100.00%,18%LVA+1.8%SDS作用5 min、6%LVA+0.6%SDS作用30 min对偶发分枝杆菌的杀灭率均达100.00%。与1000 mg/L的含氯消毒剂(次氯酸钠)相比,LVA-SDS对脓肿分枝杆菌、龟分枝杆菌的杀菌效果相当,对偶发分枝杆菌的杀菌效果更优。结论LVA-SDS对脓肿分枝杆菌、龟分枝杆菌、偶发分枝杆菌均有较好的杀灭作用,未来有可能用于NTM的消毒工作。 展开更多

目的了解综合性医院非结核分枝杆菌(NTM)感染患者临床分离NTM菌种类型,分析抗菌药物敏感性,为临床早期诊断和治疗提供依据。方法收集2023年1—12月就诊于复旦大学附属中山医院感染性疾病科NTM感染患者NTM临床分离株,通过基质辅助激光解... 目的了解综合性医院非结核分枝杆菌(NTM)感染患者临床分离NTM菌种类型,分析抗菌药物敏感性,为临床早期诊断和治疗提供依据。方法收集2023年1—12月就诊于复旦大学附属中山医院感染性疾病科NTM感染患者NTM临床分离株,通过基质辅助激光解析/电离飞行时间质谱进行菌种鉴定,采用微量肉汤稀释法检测抗菌药物敏感性。结果共收集NTM感染患者临床NTM分离株99株,其中快速生长分枝杆菌(RGM)30株,包括脓肿分枝杆菌28株、偶发分枝杆菌2株,缓慢生长分枝杆菌(SGM)69株,包括胞内分枝杆菌55株、鸟分枝杆菌8株、堪萨斯分枝杆菌6株。鸟-胞内分枝杆菌复合群(MAC)对一线药物克拉霉素和阿米卡星较敏感,对二线药物利奈唑胺和莫西沙星耐药率>60.00%。所有RGM对阿米卡星高度敏感(100.00%),对多西环素耐药率最高(86.67%)。脓肿分枝杆菌对多种抗菌药物高度耐药,对克拉霉素存在诱导耐药。结论NTM菌种分布以胞内分枝杆菌和脓肿分枝杆菌为主。SGM和RGM分离株耐药性存在一定差异。NTM菌种鉴定和药物敏感性试验有助于综合性医院NTM感染患者的精准诊疗。 展开更多

目的本研究旨在探讨播散型非结核分枝杆菌病(DDNTM)患者的临床特征及预后影响因素,为临床诊治该病提供参考。方法回顾性选取2021年1月-2023年12月昆明市第三人民医院确诊为DDNTM的73例患者,收集患者的性别、年龄、临床表现、实验室检查... 目的本研究旨在探讨播散型非结核分枝杆菌病(DDNTM)患者的临床特征及预后影响因素,为临床诊治该病提供参考。方法回顾性选取2021年1月-2023年12月昆明市第三人民医院确诊为DDNTM的73例患者,收集患者的性别、年龄、临床表现、实验室检查结果及治疗预后等信息。结果所有病例主要临床表现为持续发热(64.38%)、咳嗽(53.42%)和体重下降(46.58%);病原体以鸟分枝杆菌复合群和脓肿分枝杆菌为主。超过半数患者居住在农村地区;经过抗非结核分枝杆菌(NTM)治疗后,治疗有效率为65.75%(48/73),19.18%(14/73)的患者死亡。Logistic回归单因素分析结果显示,年龄60~岁(OR=14.667,P=0.009)、合并HIV感染(OR=0.800,P=0.032)、抗NTM联合激素治疗(OR=0.308,P=0.038)与未治疗(OR=4.466,P=0.047)预后更差,而性别、累及部位与CD4^(+)T细胞计数水平,对患者预后影响差异均无统计学意义(P均>0.05)。Logistic多因素分析结果显示,年龄60~岁(OR=25.863,P=0.006)、合并HIV感染(OR=0.938,P=0.032)与抗NTM联合激素治疗(OR=0.308,P=0.018)是影响DDNTM患者预后的危险因素。结论DDNTM对特定人群具有较高的发病率和死亡率,了解其临床特征和预后因素有助于优化治疗策略和预防措施,降低DDNTM感染的危害。 展开更多
