Bridging the eye care gap through technological innovation:the role of vision centres in India's primary healthcare landscape

Achieving universal eye health remains a global challenge,particularly in low-and middle-income countries where visual impairment and blindness are prevalent.While advances in tertiary eye care have improved outcomes,access to primary eye care(PEC)continues to be inadequate in rural and underserved regions.This gap necessitates innovative,scalable models that provide accessible,affordable,and comprehensive eye care.The Vision Centre(VC)model,pioneered by the Aravind Eye Care System(AECS),exemplifies a sustainable approach to delivering PEC.Designed as permanent facilities in rural communities,VCs are equipped with state-of-the-art diagnostic tools and staffed by trained allied ophthalmic personnel.The integration of teleophthalmology,electronic medical records,and artificial intelligence enhances the model’s capacity to address complex conditions like diabetic retinopathy and glaucoma.VCs have demonstrated significant impact in improving accessibility,reducing financial burdens,and increasing utilization of eye care services.In the fiscal year 2023-2024,AECS VCs recorded nearly one million outpatient visits,achieving a 25%population coverage rate and generating substantial cost savings of ₹647 million(US$7.8 million)for patients.The model's success is underpinned by community engagement,a focus on operational excellence,and a robust referral system to tertiary hospitals.This review explores the evolution,implementation,and impact of the AECS VC model,emphasizing its alignment with the Sustainable Development Goals and Universal Health Coverage.By addressing accessibility and affordability,the VC model serves as a scalable template for primary eye care delivery in resource-limited settings globally. 展开更多

基于福建省冬半年沿海和港湾岛屿自动站的逐时极大风观测资料和WRF(Weather Research and Forecast)、EC(European Centre for Medium-Range Weather Forecasts)细网格以及T639(TL639L60)三种模式预报的10 m风场资料,将模式预报... 基于福建省冬半年沿海和港湾岛屿自动站的逐时极大风观测资料和WRF(Weather Research and Forecast)、EC(European Centre for Medium-Range Weather Forecasts)细网格以及T639(TL639L60)三种模式预报的10 m风场资料,将模式预报的风向风速与观测资料进行对比检验,结果表明:福建省沿海冬半年大风的盛行风向以东北风为主,大风的时空分布极为不均,沿海风力的脉动性、跳跃性、局地性突出。从三种模式对风速风向的模拟效果来看, WRF和EC细网格的预报效果较好,有可参考性, T639可参考性不高。对于风速,模式预报结果相比实况极大风速偏小,港湾岛屿代表站风速的平均绝对误差均小于沿海代表站,预报平均误差由沿海向内陆逐渐减小,由中部向南北逐渐减小。风向相比风速的预报效果要差, WRF和EC细网格的风向预报误差在45°-50°,有一定的参考意义;港湾岛屿代表站风向的平均绝对误差大于沿海代表站,以浮标站的误差最大。当观测风速出现7级及以上风速时,若对大风进行分级检验,则较低风速的预报平均绝对误差小于较高风速;风向预报的平均绝对误差也大大降低,且误差都在45°以内,具有良好的参考性。 展开更多

1901~2013年GPCC和CRU降水资料在中国大陆的适用性评估 被引量:51

利用1901~2013年中国大陆地区的气象台站实测降水资料,对东英吉利(East Anglia)大学气候研究中心(Climatic Research Unit,CRU)和全球降水气候中心(Global Precipitation Climatology Centre,GPCC)的降水资料分别从季节、年际和年代际... 利用1901~2013年中国大陆地区的气象台站实测降水资料,对东英吉利(East Anglia)大学气候研究中心(Climatic Research Unit,CRU)和全球降水气候中心(Global Precipitation Climatology Centre,GPCC)的降水资料分别从季节、年际和年代际尺度上进行了评估。结果表明:1961~2013年CRU与GPCC降水资料均能较准确地描述中国大陆地区的降水特征,且在东部较西部地区、夏季较冬季与站点实测降水情况更为一致。将中国大陆划分为不同区域并在其季节、年际和年代际时间尺度上通过比较降水偏差绝对值的百分比、均方根误差和相关系数等统计量后发现:CRU在青藏高原和其它较大的山脉附近与站点实测降水的差别较大,且年均降水趋势在西北一带的阿尔金山脉、黄土高原、东南地区和长江下游地区,比实测降水的年均趋势小、甚至出现趋势相反的情况。此外,CRU降水的年代际变化趋势也偏小。而GPCC数据不论是降水量还是降水趋势都更接近实际情况。在1901~1961年,通过与65个长期气象观测站点的降水时间序列比较发现,CRU在110°E以西地区与站点观测的降水资料间的差别较大,而GPCC与站点观测资料的吻合较好。最后,利用1961~2013年两套降水资料和站点实测资料分别计算了标准化降水指数(SPI),简单分析了中国大陆地区的干旱变化,发现GPCC对旱涝的时空变化特征的描述比CRU更接近站点实际观测;并且CRU也没有反映出1997年夏季中国地区出现的严重干旱情况,而GPCC较为准确地反映出了这一干旱事件特征。因此,本文的研究结果认为,就中国大陆地区长时期降水资料而言,GPCC的适用性优于CRU。 展开更多

Face centred cubic(FCC) TiB ceramic powder synthesized by Ti-boronizing method was used as conductive filler to make ceramic electrically conductive adhesives(ECAs) with the polymer matrix.Electrically conductive properties of the ceramic ECAs were studied.The bulk electrical resistivity varied with the powder content of the FCC-TiB in ECAs.The FCC-TiB filled ECAs also showed the percolation behavior that usually occurred for the metal-filled ECAs,the percolation threshold was located at the content of 60%FCC-TiB.A minimum value of 0.1 Ω·cm was obtained at a content of 75%FCC-TiB.In order to check the reliability of mechanical property,tensile test was done to measure the shear strength,and the shear strength dropped with increasing the content of FCC-TiB powders.It is about 12.26 MPa at the content of 70%TiB powders.The Cu filled ECAs were also prepared for comparison.The properties of the oxidation resistance of the two ECAs were evaluated.The results show that the ceramic ECAs have excellent oxidation resistance and better stability compared with the Cu filled ECAs. 展开更多

The local bifurcations of the system are investigated by utilizing the bifurcation theory and the centre manifold theorem, and thus the conditions of the existence of pitchfork bifurcation and Hopf bifurcation are derived in detail. Numerical simulations are presented to verify the theoretical analysis, and they show some interesting dynamics, including stable periodic orbits emerging from the new fixed points generated by pitchfork bifurcation, coexistence of a stable limit cycle and a chaotic attractor, as well as chaos within quite a wide parameter region. 展开更多

Atrial fibrillation(AF)and heart failure(HF)are two cardiovascular diseases with an increasing prevalence worldwide.These conditions share common pathophysiologiesand frequently co-exit.In fact,the occurrence of either condition can‘cause’the development of the other,creating a new patient group that demands different management strategies to that if they occur in isolation.Regardless of the temproral association of the two conditions,their presence is linked with adverse cardi-ovascular outcomes,increased rate of hospitalizations,and increased economic burden on healthcare systems.The use of low-cost,easily accessible and applicable biomarkers may hasten the correct diagnosis and the effective treatment of AF and HF.Both AF and HF effect multiple physiological pathways and thus a great number of biomarkers can be measured that potentially give the clinician important diagnostic and prognostic information.These will then guide patient centred therapeutic management.The current biomarkers that offer potential for guiding therapy,focus on the physiological pathways of miRNA,myocardial stretch and injury,oxidative stress,inflammation,fibrosis,coagulation and renal impairment.Each of these has different utility in current clinincal practice. 展开更多

Electro-upcycling of plastic waste into value-added chemicals/fuels is an attractive and sustainable way for plastic waste management.Recently,electrocatalytically converting polyethylene terephthalate(PET)into formate and hydrogen has aroused great interest,while developing low-cost catalysts with high efficiency and selectivity for the central ethylene glycol(PET monomer)oxidation reaction(EGOR)remains a challenge.Herein,a high-performance nickel sulfide catalyst for plastic waste electro-upcycling is designed by a cobalt and chloride co-doping strategy.Benefiting from the interconnected ultrathin nanosheet architecture,dual dopants induced upshifting d band centre and facilitated in situ structural reconstruction,the Co and Cl co-doped Ni_(3)S_(2)(Co,Cl-NiS)outperforms the singledoped and undoped analogues for EGOR.The self-evolved sulfide@oxyhydroxide heterostructure catalyzes EG-to-formate conversion with high Faradic efficiency(>92%)and selectivity(>91%)at high current densities(>400 mA cm^(−2)).Besides producing formate,the bifunctional Co,Cl-NiS-assisted PET hydrolysate electrolyzer can achieve a high hydrogen production rate of 50.26 mmol h^(−1)in 2 M KOH,at 1.7 V.This study not only demonstrates a dual-doping strategy to engineer cost-effective bifunctional catalysts for electrochemical conversion processes,but also provides a green and sustainable way for plastic waste upcycling and simultaneous energy-saving hydrogen production. 展开更多
