Hypothyroidism—A Question towards Quality and Patient Centricity

The metabolic and endocrine disorders are either increasing across the globe or may have been observed with more focus in recent years. It is not uncommon now to find out neither patients of endocrine and metabolic disorders nor a physician of the associated diseases. There were 42 million people who had thyroid disorders including hypothyroidism according to the survey of endocrine society of India. Hypothyroidism is said when less activity of thyroid glands is observed then the normal. So, to evaluate the quality standard, empowerment and patient centricity in Pakistan with present prevalence of hypothyroidism in different age groups, gender along with the prescribed medication against pre diagnosed disease and associated co-morbidities, the study is designed with a questionnaire development including both qualitative and quantitative variables. Study was conducted on various OPD’s, profit and not for profit secondary and tertiary care hospitals and consultant clinics during July 2013 to Jan 2014 and it was given consideration to include people from all areas of life with minimization of any unforeseen biasness and comprehensive data. After full filling the inclusion criteria, 102 patients were selected as study participants. After completion and reviewing the questionnaire, data were analyzed by 21.00 SPSS version. Chi square/ fisher test, spearmen’s rank correlation were applied for categorical quantitative variable. Paired t test before and after were applied for qualitative variable. Highly significant reliability of items in the questionnaire was found along with inter item correlation (P < 0.05). Out of 102 patients of the study, 80 patients fell into the inclusion criteria, while 22 were excluded. A total of 80 patients were registered in the incorporate duration. Among the total samples 36 (42.9%) were males and 44 (52.4%) were females. Mean age males and females included in the study were 34.89 ± 17.73 and 33.36 ± 13.73 years respectively. The systolic and diastolic blood pressure (120/80 mg Hg) was observed in 18 (40.9%) females’ and 13 (36.1%) males’ patients. Quality, patient centricity and empowerment were the poor areas which need additional consultations and counseling for crutches free environment. 展开更多

The airline industry is operating in a highly competitive and saturated market and is considered to be one of the most dynamic service industries in the world.Maintaining a high level of passenger satisfaction and incorporating customer centricity into an airline’s strategy can be the key competitive advantage.Despite the growing interest of scholars and managers in customer centricity,research on this topic in the airline industry has just started.However,it is key to understand how a firm’s intentions to become customer-centric finally translate into customer perceptions.For this purpose,the authors developed a perceived customer centricity framework which is tailored to the environment of the passenger airline industry.A systematic literature review was conducted to review and analyze key characteristics and measures of perceived customer centricity in the airline industry.The authors identified 15 distinct key characteristics(measures)in literature and conclude that airlines need to focus on various aspects to truly be customer-centric.Investing in sustainable practices,employee training,modern in-flight experiences,competitive pricing,technological infrastructure,and positive brand associations all contribute to customer centricity.Understanding how these elements are interconnected allows airlines to customize their efforts,meeting current and future passenger needs,and ultimately creating a standout customer-centric culture in the competitive airline industry.By formulating concrete measures this paper establishes a foundation for future perceived customer centricity research,such as surveys,and presents alternative measures to the widely adopted net promoter score(NPS). 展开更多

This study addresses a significant gap in current research by developing and validating a comprehensive index for measuring perceived customer centricity in the passenger airline industry.The index was derived using a rigorous scale development methodology based on three surveys with a total of 963 participants.The process involved construct definition,item generation,and item validation through exploratory and confirmatory factor analyses.The final measurement index comprises 19 items across four dimensions(Customer Care&Experience,Communication&Product,Sustainability,and Value)and demonstrates strong discriminant and convergent validity,providing a reliable tool for assessing customer perceptions.Therefore,this research introduces the first validated measure of perceived customer centricity specifically designed for the airline industry and offers practical insights for airline managers,enabling them to identify specific areas where customer perceptions fall short and to make targeted improvements.Additionally,the index serves as an alternative to the industry-wide adopted Net Promoter Score(NPS).It extends current literature by providing an industry-specific scale and offers a practical instrument for enhancing customer-centric strategies in real-world applications,thereby fostering long-term,mutually beneficial relationships.Additionally,it provides empirical evidence of the multidimen-sional nature of customer centricity and offers a validated tool that can be applied in future research to explore customer perceptions and their impact on airline performance,thus satisfying both practical and academic needs. 展开更多

The dissemination of information across various locations is an ubiquitous occurrence,however,prevalent methodologies for multi-source identification frequently overlook the fact that sources may initiate dissemination at distinct initial moments.Although there are many research results of multi-source identification,the challenge of locating sources with varying initiation times using a limited subset of observational nodes remains unresolved.In this study,we provide the backward spread tree theorem and source centrality theorem,and develop a backward spread centrality algorithm to identify all the information sources that trigger the spread at different start times.The proposed algorithm does not require prior knowledge of the number of sources,however,it can estimate both the initial spread moment and the spread duration.The core concept of this algorithm involves inferring suspected sources through source centrality theorem and locating the source from the suspected sources with linear programming.Extensive experiments from synthetic and real network simulation corroborate the superiority of our method in terms of both efficacy and efficiency.Furthermore,we find that our method maintains robustness irrespective of the number of sources and the average degree of network.Compared with classical and state-of-the art source identification methods,our method generally improves the AUROC value by 0.1 to 0.2. 展开更多

Two new freshwater centric diatom species were described from the southwest of China based on light and scanning electron microscopy.Pliocaenicus weixiense sp.nov.has a transversely undulate valve face,marginal fultoportulae situated at the end of the hyaline strips,central fultoportulae forming a ring positioned in a circular pattern,each with 3-4(usually 3)satellite pores,and a single rimoportula located at the base of the inner distal end of the costa.Tertiarius aspera sp.nov.has an uneven valve face with small granules that may be present on the marginal area and valve mantle,marginal fultoportulae situated on each costa close to the margin of the valve,6-7 central fultoportulae forming a ring near the valve center each with two satellite pores,and one rimoportula located on the valve face/mantle junction in the middle of costa on the internal valve.T.aspera sp.nov.is the first reported extant species in the genus.This study played an important role in studying the taxonomic status and ecological information of the genus,and the establishment of the two new taxa will help demonstrate the unique nature of freshwater diatom flora in the southwest of China. 展开更多

The coupling of data and digital innovation opens the way for new business in the financial services sector,where customers are placed at the centre of decisions and data can help to develop customer knowledge.To carry out our research,we adopted a multi-case study approach to explore how a data strategy is developed in the retail banking industry,together with its relationship with customer value,paying particular attention to the heterogeneity between traditional banks and financial technology companies(FinTechs).Two main points emerged from the study.Firstly,there are three possible approaches to Open Finance,which are mainly defined by their different corporate cultures,organisational configurations,technological architecture and data value.Secondly,it is not enough to be a FinTech to be best placed to exploit the market,as some traditional banks share the FinTechs’approach to Open Finance.Designing new tailored products,customising their prices and offering them over the right channels through targeted communication are all data-driven initiatives that stem from cross-or up-selling potential,core to the retail banking industry for turning a customer into a cash flow,thus enabling value to be created for customers.Our findings additionally revealed that there is a form of external information asymmetry between the customer and the bank,and that there is also an internal asymmetry between bank departments,as their visibility on information about the same customer may differ. 展开更多

Collaborative unmanned systems have emerged to meet our society’s wide-ranging grand challenges,with their advantages including high performance,efficiency,flexibility,and inherent resilience.Increasing levels of group/team autonomy have also been achieved due to the embodiment of artificial intelligence(AI).However,the current networked unmanned systems are primarily designed for and applicable to a narrow range of domain-specific missions,and do not have sufficient human-level intel-ligence and human needs fulfillment for the challenging missions in our lives.We propose in this paper a vision of human-centric networked unmanned systems:Unmanned Intelligent Cluster(UnIC).Within this vision,distributed unmanned systems and humans are connected via knowledge sharing and social awareness to achieve collaborative cognition.This paper details UnIC’s concept,sources of intelligence,and layered architecture,and reviews enabling technologies for achieving this vision.In addition to the technological aspects,the social acceptance issues are highlighted. 展开更多

Ethernet fundamental and its data transmission model are introduced in brief and end-to-end network latency was analyzed in this paper. On the premise of not considering transmission quality and transmission cost, latency was the function of the rest of network resource parameter (NRP). The relation between the number of nodes and that of end-to-end links was presented. In ethernet architecture, the algorithm to determine the link with the smallest latency is a polynomial issue when the number of network nodes is limited, so it can be solved by way of polynomial equations. Latency measuring is the key issue to determine the link with the smallest network latency. 3-node brigade (regiment) level network centric warfare (NCW) demonstration platform was studied and the latency between the detectors and weapon control stations was taken as an example. The algorithm of end-to-end network latency and link information in NCW was presented. The algorithm program based on Server/Client architecture was developed. The data transmission optimal link is one whose end-to-end latency is the smallest. This paper solves the key issue to determine the link whose end-to-end latency is the smallest in ethernet architecture. The study can be widely applied to determine the optimal link which is in the complex network environment of multiple service provision points. 展开更多

AIM:To investigate the usefulness of endoscopic ultrasound-guided fine needle aspiration(EUS-FNA) in the differentiation of autoimmune pancreatitis(AIP).METHODS:We retrospectively reviewed 47 of 56 AIP patients who underwent EUS-FNA and met the Asian diagnostic criteria.On 47 EUS-FNA specimens,we evaluated the presence of adequate material and characteristic features of lymphoplasmacytic sclerosing pancreatitis(LPSP) and idiopathic duct-centric pancreatitis(IDCP) mentioned in the International Consensus Diagnostic Criteria and examined if these findings make a contribution to the differential diagnosis of type 1 and type 2 AIP.A disposable 22-gauge needle was used for EUS-FNA.RESULTS:Adequate specimens including pancreatic tissue for differentiating AIP from cancer were obtained from 43 of 47 patients who underwent EUSFNA.EUS-FNA was performed from the pancreatic head in 21 cases,which is known to be technically difficult when performed by core biopsy;there was no significant difference in the results compared with pancreatic body-tail.Nine of 47 patients met level 1 findings of LPSP and 5 patients met level 2 findings of LPSP.No one met level 1 findings of IDCP,but 3 patients met level 2 findings of IDCP.Of 10 seronegative cases,2 cases were diagnosed with "definitive type 1 AIP",and 3 cases were diagnosed with "probable type 2 AIP" when considering both the level 2 histological findings and response to steroids.CONCLUSION:EUS-FNA is useful in the differentiation of type 1 and type 2 AIP,particularly in seronegative cases. 展开更多

In this paper, based on the analysis of content distribution, status of content store, and pending interest, we propose a novel caching algorithm with which the resources of content store and pending interest table can be efficiently used. Due to the high intermittent links, it has become a critical challenge to deliver high quality video streaming in vehicular networks. In this paper, we reform the Information Centric Networking (ICN) concept for multimedia delivery in urban vehicular networks. By leveraging the 1CN perspective, we highlight that vehicular peers can obtain multimedia chunks via the vehicle-to-cloud (V2C) approach to improve the delivery quality. Based on this, we propose a lightweight multipath selection strategy to guide the network system to adaptively adjust the forwarding means. Extensive simulations show that the proposed solution can optimize the utilization of network paths, lighten network loads as well as avoid wasting resources. 展开更多
