Objective:To investigate the effectiveness and feasibility of using a central line bundle(CLB)guideline with a standard checklist in the prevention of peripherally inserted central catheter(PICC)-related infections(CRIs)in very low-birth-weight infants(VLBWIs).Methods:Fifty-seven VLBWIs who underwent PICC insertion at a hospital in Qingdao,China,between November 2012 and June 2013,were monitored with the CLB guideline and a standard checklist.Fifty-three VLBWIs who underwent PICC insertion were monitored by standard hospital procedures.The incidence of CRIs was compared between the two groups.Results:The incidence of infection significantly decreased from 10.0%catheter days in the control group to 2.2%catheter days in the study group(p<0.05).The indwelling catheter time significantly increased in the study group compared to the control group(31.9±15.0days vs.24.8±7.4 days,respectively,p<0.05).Colonization infections also decreased from 6.9% catheter days in the control group to 2.2%catheter days in the study group(p<0.05).The incidence of catheter-related bloodstream infections decreased from 3.1%catheter days in the control group to 0%catheter days in the study group.Conclusion:The use of a CLB guideline with a standard checklist could be an effective and feasible protocol for preventing CRIs and prolonging indwelling catheter timein VLBWIs. 展开更多

BACKGROUND The central venous line is an essential component in monitoring and managing critically ill patients.However,it poses patients with increased risks of severe infections with a higher probability of morbidity and mortality.AIM To define the trends of the rates of central line-associated bloodstream infections(CLABSI)over four years,its predicted risk factors,aetiology,and the antimicrobial susceptibility of the isolated pathogens.METHODS The study was a prospective case-control study,performed according to the guidelines of the Center for Disease Control surveillance methodology for CLABSI in patients admitted to the adult intensive care unit(ICU)and auditing the implementation of its prevention bundle.RESULTS Thirty-four CLABSI identified over the study period,giving an average CLABSI rate of 3.2/1000 central line days.The infection's time trend displayed significant reductions over time concomitantly with the CLABSI prevention bundle's reinforcement from 4.7/1000 central line days at the beginning of 2016 to 1.4/1000 central line days by 2018.The most frequently identified pathogens causing CLABSI in our ICU were gram-negative organisms(59%).The most common offending organisms were Acinetobacter,Enterococcus,and Staphylococcus epidermidis,each of them accounted for 5 cases(15%).Multidrug-resistant organisms contributed to 56%of CLABSI.Its rate was higher when using femoral access and longer hospitalisation duration,especially in the ICU.Insertion of the central line in the non-ICU setting was another identified risk factor.CONCLUSION Implementing the prevention bundles reduced CLABSI significantly in our ICU.Implementing the CLABSI prevention bundle is crucial to maintain a substantial reduction in the CLABSI rate in the ICU setting. 展开更多

BACKGROUND Pre-transplant nutrition is a key driver of outcomes following liver transplantation in children.Patients with biliary atresia(BA) may have difficulty achieving satisfactory weight gain with enteral nutrition alone,and parenteral nutrition(PN) may be indicated.While PN has been shown to improve anthropometric parameters of children with BA listed for liver transplantation,less is known about the risks,particularly infectious,associated with this therapy among this specific group of patients.AIM To describe the incidence,microbiology,and risk factors of central line-associated bloodstream infection(CLABSI) among children with BA listed for liver transplantation.METHODS Retrospective review of children aged ≤ 2-years of age with BA who were listed for primary liver transplantation at Texas Children's Hospital from 2008 through2015(n = 96).Patients with a central line for administration of PN(n = 63) were identified and details of each CLABSI event were abstracted.We compared the group of patients who experienced CLABSI to the group who did not,to determine whether demographic,clinical,or laboratory factors correlated with development of CLABSI.RESULTS Nineteen of 63 patients(30%,95%CI:19,43) experienced 29 episodes of CLABSI during 4800 line days(6.04 CLABSI per 1000 line days).CLABSI was predominantly associated with Gram-negative organisms(14/29 episodes,48%)including Klebsiella spp.,Enterobacter spp.,and Escherichia coli.The sole polymicrobial infection grew Enterobacter cloacae and Klebsiella pneumoniae.Grampositive organisms(all Staphylococcus spp.) and fungus(all Candida spp.)comprised 9/29(31%) and 6/29(21%) episodes,respectively.No demographic,clinical,or laboratory factors were significantly associated with an increased risk for the first CLABSI event in Cox proportional hazards regression analysis CONCLUSION There is substantial risk for CLABSI among children with BA listed for liver transplantation.No clinical,demographic,or laboratory factor we tested emerged as an independent predictor of CLABSI.While our data did not show an impact of CLABSI on the short-term clinical outcome,it would seem prudent to implement CLABSI reduction strategies in this population to the extent that each CLABSI event represents potentially preventable hospitalization,unnecessary healthcare dollar expenditures,and may exact an opportunity cost,in terms of missed allograft offers. 展开更多

目的建立标准化的中心静脉导管相关血流感染(CLABSI)监测流程,实施全流程质量管理以提升监测质量为有效防控CLABSI提供依据。方法通过多学科协作,梳理CLABSI监测全流程,包括血标本培养、置管信息获取及感染诊断存在的问题;针对性开展质... 目的建立标准化的中心静脉导管相关血流感染(CLABSI)监测流程,实施全流程质量管理以提升监测质量为有效防控CLABSI提供依据。方法通过多学科协作,梳理CLABSI监测全流程,包括血标本培养、置管信息获取及感染诊断存在的问题;针对性开展质量管理干预,干预措施包括统一监测标准、优化置管医嘱、规范血培养采集;比较干预前后系统抓取的置管信息与填报数据的一致性和血培养污染情况的变化,并评价质量管理干预的效果。结果建立了统一的CLABSI监测流程及中心静脉导管置管医嘱方案。2024年中心静脉置管日数229439 d,发生CLABSI 60例次,CLABSI发病率为0.26‰。CLABSI集中发生在血液科、重症监护病房。检出病原菌65株,凝固酶阴性葡萄球菌是CLABSI主要病原菌,占49.23%(32株)。干预后,信息系统按医嘱抓取的置管日数与临床手工填报数据的一致性显著提升,由干预前的61.72%升至干预后的78.18%(P<0.05)。干预前后血培养污染率均维持在较低水平。结论CLABSI监测的全流程质量管理能够显著提升监测效率和效果。 展开更多

Objective:To evaluate the complications of central venous catheterization(CVC).Methods:A prospective,observational study was conducted at a tertiary care center in India from December 2018 to September 2020.Critically ill patients(aged≥18 years)in the intensive care unit undergoing CVC procedures were included in the study.Baseline demographics and detailed medical history were recorded.Chest X-rays and electrocardiography were performed on all the patients.Complications associated with CVC were recorded.Results:A total of 100 patients with the indication for central venous catheter insertion were included.The majority(81%)of the patients were inserted with CVC at the right internal jugular vein.Complications such as arterial puncture(2%),hematoma(4%),blood clot formation(4%),catheter kinking(3%),thoracic injury(1%),thrombophlebitis(6%),sepsis(9%)and nerve injury(1%)were reported.Conclusions:Though central venous access is preferred in management of critically ill patients,it has its risks.However,early recognition and prompt management of complications may reduce mortality and morbidity.Physicians and intensive care unit intensivists should be vigilant for central venous catheter-related complications.Suitable site selection,operator experience,and proper catheter maintenance are associated with optimal outcomes. 展开更多

The prevention and control of catheter line-associated bloodstream infections (CLABSI) have become the key content of medical care and have become the core improvement goal of nursing quality control. However, the prevention and control status of CLABSI in hospitals of different grades in Guangxi is not clear. In this study, we aim to investigate central venous catheter (CVC) placement and disinfectant use in second and third-level hospitals in Guangxi. This survey was conducted on the second and third-level hospitals in Guangxi, China from 13th April 2021 to 19th April 2021. The results show that a total of 283 questionnaires were collected, including 206 secondary hospitals and 77 tertiary hospitals. In terms of the CVC, tertiary hospitals were able to place CVC entirely under the guidance of B-ultrasound, which was 24 (31.6%) and secondary hospitals were 26 (20.6%). In secondary hospitals, Most CVC placements were performed in operating rooms 94 (74.6%) and 65 (85.5%) on the third level hospital, but 32.5% of secondary hospitals and 48.7% of tertiary hospitals were selected at the bedside of patients in general wards, and 27.8% of the second-level hospital, 43.4% of third-level hospitals was done in general ward treatment rooms, only 61.9% of secondary hospitals and 64.5% of tertiary hospitals could fully achieve the maximum sterile barrier. In terms of skin disinfectants, only 36.0% of tertiary hospitals and 16.4% of second-level CVC-operators chose > 0.5% chlorhexidine alcohol. In conclusion, the prevention and control of catheter line-associated bloodstream infections (CLABSI) in Guangxi are not ideal. The prevention and control department should increase training, implement guidelines and standardize management to reduce the incidence of CLABSI. 展开更多

AIM To establish how neonatal units in England and Wales currently confirm longline tip position, immediately after insertion of a longline.METHODS We conducted a telephone survey of 170 neonatal units(37 special care baby units, 81 local neonatal units and 52 neonatal intensive care units) across England and Wales over the period from January to May 2016. Data was collected on specifically designed proformas. We gathered information on the following: Unit Level designation; whether the unit used longlines and specific type used? Modality used to confirm longline tip position? Whether guide wires were routinely removed and contrast injected to determine longline position? The responders were primarily senior nurses.RESULTS We had 100% response rate. Out of the total neonatal units surveyed(170) in England and Wales, 141 units(83%) used longlines. Fifty-five out of 81 local neonatal units(68%) using longlines, used ones that came with guide wires; a similar percentage of neonatal intensive care units, i.e., 31 out of 52 units(60%) did the same. All of those units used radiography, plain X-rays, to establish longline tip position. Out of 55 local neonatal units usinglonglines with guide wires, 42(76%) were not removing wire to use contrast while this figure was 58%(18 out of 31 units) for neonatal intensive care units. Overall, only 49 out of 141 units(35%) of the units using longlines were using contrast. However it was interesting to note that use of contrast increased as one moved from special care baby units(25%, 2 out of 8 units) to local neonatal units(28%, 23 out of 81 units) and neonatal intensive care units level(46%, 24 out of 52 units) designation. CONCLUSION Neonatal units in England and Wales are overwhelmingly relying on plain radiographs to assess longline tip position immediately after insertion. Despite evidence of its usefulness, and in the absence of perhaps more accurate methods of assessing longline tip position in a reliable and consistent way, i.e., ultrasonography, contrast is only used in a third of units. 展开更多

Glacier reconstruction of La Covacha Massif in Sierra de Gredos(central Spain) during the Last Glacial Maximum

A paleoglacier reconstruction during the Last Glacial Maximum(LGM) is presented for a high mountain area of La Covacha massif in Sierra de Gredos(Iberian Central System) in central Spain. The moraines that, according with the previous literature, had formed during the global LGM, were mapped through photo interpretation of digital aerial photographs and 3D images and detailed field surveys. The topography of the paleoglaciers was estimated using a simple steady-state model that assumed a perfectly plastic ice rheology, reconstructing the theoretical ice profiles and obtaining the ice thickness of the glaciers during the LGM. The reconstruction of the glaciers was carried out using automated and semi-automated physically-based models, obtaining more realistic results with semi-automated models. According to our study, the paleoglaciers in the study area covered an area of 34.79 km^2 during the global LGM, with a maximum ice thickness of 366 meters in La Vega gorge, a total volume of 34.25 × 108 m^3 and a mean paleoELA of 1932 meters. Most of the ice(~82%) was in paleoglaciers facing north, and the rest was in paleoglaciers with other orientations. This emphasizes the importance of orientation in relation to glacier dynamics and ice accumulation. The results obtained in the calculation of paleoELAs during the global LGM in Gredos are average compared to the Iberian mountains of the Northwest where the values were much lower, and with respect to those of the Southwest, much higher. This demonstrates the importance of the exposure of Mediterranean mountains to the humidity sources coming from the NW during the global LGM, as is the case at present. 展开更多

ICU重症患者中心静脉导管相关血流感染的卫生经济学分析 被引量:1

【目的】探讨ICU重症患者,进行中心静脉置管(CVC)后发生导管相关血流感染(CLABSI)的增量成本,以及院感防控措施的主要成本,通过增量成本和院感防控措施成本的价值对比,从医疗质量和经济效益角度提高医务人员对预防CLABSI的院感防控措施... 【目的】探讨ICU重症患者,进行中心静脉置管(CVC)后发生导管相关血流感染(CLABSI)的增量成本,以及院感防控措施的主要成本,通过增量成本和院感防控措施成本的价值对比,从医疗质量和经济效益角度提高医务人员对预防CLABSI的院感防控措施的重视程度,促进防控措施的落实。【方法】本研究采用整群抽样方法,选取2021年1月至2023年12月期间,在某三甲中医院ICU进行CVC置管的重症患者,CLABSI组65例和非CLABSI组61例,共126例,通过医院医疗病历系统回顾性地收集患者资料,梳理研究对象的疾病类型、性别、年龄、住院时长、转归、住院各项费用等。分析比较不同手卫生方法、不同环境清洁消毒方法的成本。【结果】CLABSI组和非CLABSI组患者的住院时长(Z=-5.35,P<0.05)和住院总费用(Z=-6.79,P<0.05)均存在显著性差异;不同住院时长的患者住院总费用存在显著性差异(H=43.01,P<0.05);住院时长>60 d的患者住院总费用中位数远高于其他3组的患者;男性组(Z=-4.85,P<0.05)住院总费用中位数的差异大于女性组(Z=-3.98,P<0.05);61~80岁组(Z=-5.79,P<0.05)的差异大于其他两组。【结论】CLABSI的发生显著增加了ICU患者的住院时间和住院费用。虽然不同手卫生方式、不同环境清洁消毒方案在成本上存在差异,但与CLABSI发生后产生的增量成本相比,这些成本是可以接受的。因此,医疗机构应重视手卫生和环境清洁消毒等院感防控措施的投入,制定切实、合理、可行的实施方案,确保院感防控措施落实,以避免医院感染发生、促进医疗质量提升、有效控制患者的住院时间和住院费用,力求保障患者安全。 展开更多

目的 基于Joinpoint回归模型分析12年重症监护病房(ICU)医院感染监测情况,并评估医疗保健相关感染(HAI)发病率变化趋势及相关因素。方法 选取某三级甲等医院2012年1月—2023年12月ICU患者,进行前瞻性HAI监测,并分析HAI发病率变化趋势及... 目的 基于Joinpoint回归模型分析12年重症监护病房(ICU)医院感染监测情况,并评估医疗保健相关感染(HAI)发病率变化趋势及相关因素。方法 选取某三级甲等医院2012年1月—2023年12月ICU患者,进行前瞻性HAI监测,并分析HAI发病率变化趋势及与手卫生用品消耗量、医院感染管理控制措施的相关性。结果 2012—2023年纳入ICU监测患者6 929例,发生HAI 543例,655例次,HAI发病率为7.84%,例次发病率为9.45%,平均病情严重程度为3.62,调整HAI发病率为2.17%。呼吸机相关肺炎(VAP)、导尿管相关尿路感染(CAUTI)、中央导管相关血流感染(CLABSI)日发病率分别为6.19‰、3.45‰、1.23‰。手卫生用品消耗量为122.98 mL/床日,手卫生依从率为90.63%,手卫生正确率为90.46%。2012—2023年HAI发病率(51.29%VS 4.39%)、例次发病率(72.41%VS 4.94%)、调整发病率(15.98%VS 1.04%)、VAP日发病率(22.50‰VS 4.33‰)、CAUTI日发病率(14.23‰VS 1.64‰)、CLABSI日发病率(10.60‰VS 0.20‰)均明显下降(均P<0.05),手卫生用品消耗量(75.16 mL/床日VS 147.35 mL/床日)、手卫生正确率(85.00%VS 90.28%)均明显增加(P<0.05)。共检出病原体1 946株,金黄色葡萄球菌占比有所增加(1.30%VS 9.57%),真菌占比有所降低(11.04%VS 1.74%);手卫生用品床日消耗量与HAI发病率、HAI例次发病率及CAUTI、CLABSI日发病率呈负相关(均P<0.05);医院感染实时监测系统及医院感染管理纳入绩效考核能降低HAI相关发病率(P<0.05)。结论 HAI相关发病率呈下降趋势,医务人员手卫生管理、医院感染实时监测系统上线及医院感染管理纳入绩效考核等科学、综合的医院感染防控管理措施可降低HAI相关发病率,促进医疗质量的提升。 展开更多

目的探讨中心静脉导管相关血流感染(CLABSI)的发病风险及窗口期,分析应用多学科(MDT)管理方法降低CLABSI的效果,为CLABSI防控策略的制定提供依据。方法选择2019年1月—2022年12月,某三级甲等医院重症监护病房(ICU)所有中心静脉导管(CVC... 目的探讨中心静脉导管相关血流感染(CLABSI)的发病风险及窗口期,分析应用多学科(MDT)管理方法降低CLABSI的效果,为CLABSI防控策略的制定提供依据。方法选择2019年1月—2022年12月,某三级甲等医院重症监护病房(ICU)所有中心静脉导管(CVC)置管的住院患者。使用2个阶段的单中心前后对照真实世界研究,第1阶段(2019年1月—2020年12月)为基线对照阶段,第2阶段(2021年1月—2022年12月)为干预阶段,项目组运用MDT管理方法持续强化组织管理,完善工作流程,使用独立研发的“CLABSI吹哨系统”前瞻性监控CLABSI发病风险,并在CLABSI集束化防控策略中根据风险实际情况增加了CVC置管后7 d内医护间交互核查(cross-checklist)的机制。结果基线期CLABSI发病率为9.16%,日发病率为10.75‰;干预期CLABSI发病率为5.99%,日发病率为6.48‰;干预前后CLABSI发病率、日发病率比较,差异均有统计学意义(均P<0.001)。CVC置管后的第7~9天是CLABSI的急性发病期,急性期后发病风险趋于平缓,项目组在置管后的7 d内通过干预,成功降低了CLABSI发病风险(χ^(2)=19.130,P<0.001)。结论CVC置管后第7~9天是降低CLABSI的机会窗口,把握住这个时期,可以有效降低CLABSI发病风险。 展开更多

目的:构建ICU中心静脉导管相关血流感染(Central Line-associated Bloodstream Infection,CLABSI)防控平台,并探讨其应用效果,以期为ICU CLABSI防控提供参考。方法:以“以用户为中心”理念及人机界面理论为指导,对ICU CLABSI相关因素及... 目的:构建ICU中心静脉导管相关血流感染(Central Line-associated Bloodstream Infection,CLABSI)防控平台,并探讨其应用效果,以期为ICU CLABSI防控提供参考。方法:以“以用户为中心”理念及人机界面理论为指导,对ICU CLABSI相关因素及实验室指标进行系统文献检索,并将其纳入平台监测内容,构建ICU CLABSI防控平台。平台于2023年7月正式应用于全国22家不同级别医院。便利选取各医院101名ICU护士,对平台可用性进行评价,同时比较平台应用前后ICU CLABSI发生率、医护人员操作行为依从率、护士满意度的差异。结果:ICU护士对平台的可用性评价得分为(70.0±16.9)分,提示平台可用性良好。平台应用前后,ICU CLABSI患者数由10例(1.5%)下降至0例,ICU CLABSI患者数季度变化百分比为-44.3%,差异均有统计学意义(均P<0.05),医护人员操作行为依从率、护士满意度均提升,差异有统计学意义(均P<0.05)。结论:ICU CLABSI防控平台可用性良好,有利于降低患者CLABSI发生风险,规范医护人员操作行为,提高护士满意度,具有一定的临床价值。 展开更多

目的 以用户体验为中心,了解监护室中心静脉导管相关血流感染防控数据管理平台的可用性,为其推广奠定基础。方法 2024年9-11月,采用目的抽样法选择上海、山东、四川、南通等地共7所三级医院的11名重症监护室(intensive care unit, ICU)... 目的 以用户体验为中心,了解监护室中心静脉导管相关血流感染防控数据管理平台的可用性,为其推广奠定基础。方法 2024年9-11月,采用目的抽样法选择上海、山东、四川、南通等地共7所三级医院的11名重症监护室(intensive care unit, ICU)临床护士和6名ICU护士长参加平台用户测试并进行使用体验访谈。2024年11月,采用便利抽样法选择164名使用过平台的ICU护士填写研究后系统可用性问卷(post-study system usability questionnaire, PSSUQ),以评估用户对平台的满意度。结果 17名平台用户完成了7项测试任务并接受访谈后反馈平台使用的整体感觉较好,各项功能完善,该平台的应用对于临床护理及医疗决策有重要的意义;同时也对平台的不足之处提出了意见。接受问卷调查的164名ICU护士PSSUQ得分为(2.02±1.12)分。结论 平台整体可用性良好,值得推广,未来需要进一步优化。 展开更多

重度烧伤患者发生中心静脉导管相关性血流感染的危险因素分析

目的分析重度烧伤患者深静脉置管后发生中心静脉导管相关性血流感染(CLABSI)的相关危险因素。方法选取2020年1月至2023年1月中国人民解放军联勤保障部队第九六〇医院收治的66例进行深静脉置管的重度烧伤患者作为研究对象,收集患者性别... 目的分析重度烧伤患者深静脉置管后发生中心静脉导管相关性血流感染(CLABSI)的相关危险因素。方法选取2020年1月至2023年1月中国人民解放军联勤保障部队第九六〇医院收治的66例进行深静脉置管的重度烧伤患者作为研究对象,收集患者性别、年龄、烧伤总面积、有无吸入性损伤、有无营养不良、是否使用烧伤治疗床、导管留置部位、导管留置时间、置管部位有无创面以及感染情况等相关资料,并根据是否发生CLABSI将其分为发生组和未发生组,分析重度烧伤患者深静脉置管后发生CLABSI的相关危险因素。结果66例进行深静脉置管的重度烧伤患者中17例(25.76%)发生CLABSI,设为发生组;49例(74.24%)未发生CLABSI,设为未发生组。发生组有营养不良、导管留置部位为股静脉、置管部位有创面的患者明显多于未发生组(χ2=8.848、16.903、5.126,P=0.003、P<0.001、P=0.024)。多因素Logistic回归分析结果显示,有营养不良、导管留置部位为股静脉是重度烧伤患者深静脉置管后发生CLABSI的独立危险性因素(95%CI为1.233~23.741、0.015~0.435,P=0.025、0.003)。结论营养状态、导管留置部位与重度烧伤患者深静脉置管后CLABSI的发生密切相关。 展开更多
