Indwelling catheter and conservative measures in the treatment of abdominal compartment syndrome in fulminant acute pancreatitis 被引量:14

AIM: To study the effect of combined indwelling catheter, hemofiltration, respiration support and traditional Chinese medicine (e.g. Dahuang) in treating abdominal compartment syndrome of fulminant acute pancreatitis. METHODS: Patients with fulminant acute pancreatitis were divided randomly into 2 groups of combined indwelling catheter celiac drainage and intra-abdominal pressure monitoring and routine conservative measures group (group 1) and control group (group 2). Routine non-operative conservative treatments including hemofiltration, respiration support, gastrointestinal TCM ablution were also applied in control group patients. Effectiveness of the two groups was observed, and APACHE Ⅱ scores were applied for analysis. RESULTS: On the second and fifth days after treatment, APACHE Ⅱ scores of group 1 and 2 patients were significantly different. Comparison of effectiveness (abdominalgia and burbulence relief time, hospitalization time) between groups 1 and 2 showed significant difference, as well as incidence rates of cysts formation. Mortality rates of groups 1 and 2 were 10.0% and 20.7%, respectively. For patients in group 1, celiac drainage quantity and intra-abdominal pressure, and hospitalization time were positively correlated (r = 0.552, 0.748, 0.923, P 〈 0.01) with APACHE Ⅱ scores. CONCLUSION: Combined indwelling catheter celiac drainage and intra-abdominal pressure monitoring, short veno-venous hemofiltration (SVVH), gastrointestinal TCM ablution, respiration support have preventive and treatment effects on abdominal compartment syndrome of fulminant acute pancreatitis. 展开更多

This is a qualitative study that aims to describe the opinion of nursing students on the use of an educational hypermedia on Peripheral Venipuncture. We interviewed 16 nursing students of a private university on second semester of 2014. The interviews allowed the identification of units of meaning and categorization by content analysis and led to four categories: influence of hypermedia on learning, educational technology as support to teaching classroom, contribution of hypermedia for professional practice and access difficulties. Hypermedia showed positive results as most scholars reported that the material helped in learning and generated autonomy and interactivity in the moment of studying. However some reported difficulties in access hypermedia related mainly to the server used. So hypermedia proved to be a valuable tool to use, since it favored the learning and aroused the interest and curiosity of students. 展开更多

Femoral sheath removal after cardiac catheterization in the Jordanian hospitals: An explorative study

Objectives: The purpose of this study was to investigate the methods of sheath removal (femostop, compressor and manual compression) among Jordanian patients post cardiac catherization. Material and Methods: A descriptive explorative design utilized to collect data from 87 patients underwent cardiac catherization. Data collected on demographics, health remarks and complication pre and post cardiac catherization and in relation to sheath removal methods and complications. Results: The majority of the patients (94.2%, n = 80) had a manual sheath removal compared to 1% (n = 1) compression and 7% (n = 6) had femostop method. Kruskal-Wallis H test revealed there were significance difference in PTT and ACT reports in relation to type of procedures (0.24, p 0.015;0.32, p = 0.005 respectively). However, there were no significant differences between methods of sheath removal methods in relation to pain level, MAP before, MAP after, and heart rate with p value > 0.05. Patient’s age had negative association with HR after the procedure (BTAU = -0.19, p = 0.02), while no significant differences found in post catherization complication (hematoma and pain level) and all other health remarks (p > 0.05) in regards to patients’ age and gender. Moreover, patients’ MAP readings were significantly different in relation to number of rest hour that patients had post cardiac catherization (p = 0.049). Conclusion: The study found that most patients and health professionals preferred manual removal methods of sheath post cardiac catherization. Cardiac health professionals, has to consider health remarks and demographic characteristics of their patients when planning care for their patients and more research needed to explore these issues. 展开更多

BACKGROUND:Catheter-related infection(CRI)of the central vein is a common cause of nosocomial infection.This study was undertaken to investigate the pathogen culturing and risk factors of CRI in emergency intensive care unit(EICU)in order to provide the beneficial reference.METHODS:From January 2008 to December 2010,a total of 1 363 patients were subjected to catheterization.In these patients,the peak CRI rate of the patients was determined by bacterial cultivation and blood bacterial cultivation.RESULTS:CRI happened in 147 of the 1 363 patients using the central venous catheter.The peak rate of CRI was 10.79%,with an incidence of 3.05 episodes per 1 000 catheter days.Of the147 patients,46.94%had gram-negative bacilli,40.14%had gram-positive cocci,and 12.92%had fungi.Unconditional logistic regression analysis suggests that multiple catheterization,femoral vein catheterization,the application of multicavity catheter,and the duration of catheterization were the independent risk factors for CRI.CONCLUSION:The risk factors for catheter-related infections should be controlled to prevent the occurrence of nosocomial infection. 展开更多

(1913) and My A ntonia (1994). As rigid social connections and concepts restrict women's autonomy and freedom, they must transform themselves and the world in order to survive. In order to analyze pioneer women's process of self-growth in the special period, the author of this paper compares and contrasts the specific situation, and analyzes the characteristics of the protagonists in the above two novels. Through analysis, the authuor concludes that pioneer women always create a nurturing and gracious atmosphere in their household. Willa Cather (1873-1947) is one of the greatest women writers in the history of American literature, whose reputation centers on her novels about celebrating pioneer immigrants on Nebraska prairie and the American southwest. For long Willa Cather's pioneer works have gained great critical acclaim and have been interpreted from different perspectives such as mythicism, romanticism, and feminism. This paper, interpreting Willa C ather' s classical pioneer works of the three periods from the 1920s to the 1930s-My A ntonia ( 1918), A Lost Lady (1923), and "Neighbor Rosicky" (1930), is devoted to making a full exploration into the evolution of her ecological thoughts from an ecofeminist perspective. In Willa Cather's three pioneer works of different creative stages, she deliberately constructs three eco-worlds as a standard of ecological harmony. In her three eco-worlds, Willa Cather not only investigates the relationship between man and nature, but also involves androcentrism, patriarchy, and other ecofeminist topics. Willa Cather's ecological views have undergone changes in her representation of three successive phrases. To interpret Willa Cather's pioneer works from the perspective of ecofeminism can provide a deeper comprehension about her works and her ecological thoughts for the world. 展开更多
