Research and clinical applications of selective laser melting tantalum bone plates

In clinical settings,tantalum(Ta)is extensively implemented as a bone implant material.Ta is highly stable and biocompatible in vivo,being one of the metallic biomaterials having high affinity for bone tissue.However,since Ta is a refractory metal,its application as bone implant material is limited.Most recently,additive manufacturing technology has introduced a novel approach to producing Ta implants.The present study compared the microstructure,surface and mechanical characteristics,and in vitro and in vivo biological characteristics of selective laser melted Ta(SLM Ta),selective laser melted titanium alloy Ti6Al4V with Ta coating(SLM Ti6Al4V with Ta coating),and selective laser melted Ti6Al4V(SLM Ti6Al4V).Results indicate that SLM Ta possesses superior mechanical characteristics contrasted with SLM Ti6Al4V and SLM Ti6Al4V with Ta coating.Furthermore,SLM Ta has anti-inflammatory activity,excellent osseointegration performance,and osteogenic bioactivity.We fabricated an SLM porous Ta bone plate and employed it for internal fixation of ulnar and radius fractures,which has been known to promote fracture healing.Further,the SLM porous Ta bone plate could form an integrated bone plate structure with the bone tissue at the implant site.Afterward,the porous structure of the plate minimizes its elastic modulus and eliminates stress shielding,leaving no need for further surgical removal.In conclusion,the SLM porous Ta bone plate meets the performance requirements(stimulating bone regeneration,non-stress shelter,and no need for second surgery)of an ideal bone plate and may revolutionize the field of internal fixation bone plates for fractures. 展开更多

Fracture healing progress monitoring techniques attract global research attention due to the importance of selecting the timing of removing the fixation device.To this end,in this research,we present a piezoelectric-based smart internal fixation device,in which a piezoelectric sandwich structure is laminated to the surface of a bone plate.In the content,we explain the reasons for utilizing piezoelectric films,elaborate the mechanism of fracture monitoring,and introduce the mechanical parameters of the sensor.The simulation and experimental results show that the electrical output of the device is associated with the elastic modulus of the filler between the tested broken bones when the working load is maintained,indicating that the bone recovery progress could be successfully detected by the developed technique. 展开更多

Effect of Minimally Invasive Percutaneous Bone Plate Internal Fixation Combined With Jintiange Capsule in the Treatment of Humeral Shaft Fracture and Its Effect on Postoperative Trauma Index and Bone

The purpose of this study was to observe the clinical efficacy of umbilical cord in the treatment of Jinri capsule and analyze its effect on postoperative injury and bone metabolism. Methods 90 cases of SAP tree fracture were randomly divided into two groups according to 1:1. In the control group, 45 patients were treated with MIPO, and 45 patients were treated with Jintian capsule based on MIPO for 6 weeks. Organic volume content (VOC), bone mass number (TB.n), elastic bone stress (ES) and calcium (BFP) were compared according to bone volume ratio (BV/TV) before and 6 weeks after operation. Six months postoperatively, bone protective function (OPG), tartrate phosphatase (TRAP), K collagen (KTH) and shoulder recovery were observed. Results Within 6 weeks after treatment, bone quality indexes such as TV were significantly lower than those in control group (P<0.05), and bone quality indexes such as VOC and TBC were significantly lower than those in control group (P<0.05). Significantly lower than control group (P<0.05). N was significantly higher than that of control group (P<0.05). The osteogenesis rates of BGP and OPG were higher than those of the control group (P<0.05), and the osteogenesis rates of TRAP and CTX were significantly lower than those of the control group (P<0.05). Were compared between the two groups before and 6 weeks after operation. Six months after surgery, the observation group had better function recovery, and the constant Murley score was significantly higher than that of the control group. Conclusion MIPO combined with Jintiange capsule can significantly improve the function recovery effect of shoulder joint, effectively stabilize bone mass index, promote the formation of bone structure, quickly recover bone metabolism function, and finally achieve fracture healing. Bone plates play a vital role in bone fracture healing by providing the necessary mechanical fixation for fracture fragments through modulating biomechanical microenvironment adjacent to the fracture site.Good treatment effect has been achieved for fixation of bone fracture with conventional bone plates,which are made of stainless steel or titanium alloy.However,several limitations still exist with traditional bone plates including loosening and stress shielding due to significant difference in modulus between metal material and bone tissue that impairs optimal fracture healing.Additionally,due to demographic changes and non-physiological loading,the population suffering from refractory fractures,such as osteoporosis fractures and comminuted fractures,is increasing,which imposes a big challenge to traditional bone plates developed for normal bone fracture repair.Therefore,optimal fracture treatment with adequate fixation implants in terms of materials and design relevant to special conditions is desirable.In this review,the complex physiological process of bone healing is introduced,followed by reviewing the development of implant design and biomaterials for bone plates.Finally,we discuss recent development of hybrid bone plates that contains bioactive elements or factors for fracture healing enhancement as a promising direction.This includes biodegradable Mg-based alloy used for designing bone screw-plates that has been proven to be beneficial for fracture healing,an innovative development that attracts more and more attention.This paper also indicates that the tantalum bone plates with porous structure are also emerging as a new fracture internal fixation implants.The reduction of the stress shielding is verified to be useful to accelerate bone fracture healing.Potential application of biodegradable metals may also avoid a second operation for implant removal.Further developments in biometals and their design for orthopedic bone plates are expected to improve the treatment of bone fracture,especially the refractory fractures. 展开更多

Metal plates have always been the gold standard in the clinic for internal fracture fixation due to their high strength advantages.However,high elastic modulus can cause stress shielding and lead to bone embrittlement.This study used an electron beam melting method to prepare personalized porous Ti6Al4V(pTi)bone plates.Then,chemical vapor deposition(CVD)technology coats tantalum(Ta)metal on the pTi bone plates.The prepared porous Ta-coated bone plate has an elastic modulus similar to cortical bone,and no stress shielding occurred.In vitro experiments showed that compared with pTi plates,Ta coating significantly enhances the attachment and proliferation of cells on the surface of the scaffold.To better evaluate the function of the Ta-coated bone plate,animal experiments were conducted using a coat tibia fracture model.Our results showed that the Ta-coated bone plate could effectively fix the fracture.Both imaging and histological analysis showed that the Ta-coated bone plate had prominent indirect binding of callus formation.Histological results showed that new bone grew at the interface and formed good osseointegration with the host bone.Therefore,this study provides an alternative to bio-functional Ta-coated bone plates with improved osseointegration and osteogenic functions for orthopaedic applications. 展开更多

Experimental study of effect of stress-relaxation bone plate on fracture healing

To study the change of the stress shielding rate of stress-relaxation plate in vivo and its influence on fracture healing. Methods: The diaphyses of bilateral tibias in 70 New Zealand rabbits were osteotomized and fixed with stress-relaxation plates (SRP, the SRP group) and rigid plates (RP, the RP group), respectively. The fracture healing process in these 2 groups was investigated by radiography, light and polarized light microscopy and biomechanical test at 2 to 48 weeks postoperatively. Results: Early after fixation the stress shielding rate was about 70% in both groups. While in the SRP group the stress shielding rate decreased gradually as time passed, which was significantly lower than that of the RP group (P< 0.05 ) by the end of the 8th postoperative week, and stabilized at the level of about 27% at 36-48 weeks after fixation. Abundant external callus associated with the formation of cartilaginous callus could be observed in the SRP group at 2-4 weeks postoperatively. The transformation of the callus into the lamellar bone began at 8-12 weeks, the collagen gradually arranged in order, and the mechanical nature of the united bone was gradually strengthened, too. In the RP group, the external callus was scarce at the early stage of fracture healing, and the callus remodeling at the late stage of fracture healing was dominated by bone absorption. The ultimate bending strength (UBS) was only 57.95 % of that of the normal by 48 weeks. Conclusions: The decrease of the stress shielding rate of SRP in vivo was well interrelated with the time of fixation. The application of SRP could promote the callus formation and bone reconstruction thus to favor the recovery of the mechanical strength of the united bone. 展开更多

BACKGROUND The epiphyseal growth plate is an important anatomical segment localized on the ends of a long bone.Despite the abovementioned atractive reasons for alendronate’s use,few data on the effect of alendronate during epiphyseal growth exist.AIM Verify the effect of alendronate on the growth epiphyseal plate,and compare its effect with the size of the femur during the double-staining of the immunolocalization of transforming growth factor-β1(TGF-β1)and bone morphogenetic protein-2(BMP2)in endochondral ossifing in specimens that have received alendronate.METHODS Forty newborn rats were randomly divided into two groups:a control group(were given applications of 1 mg/kg physiologic saline)and a group that received Alendronate(a dose of 2.5 mg/kg).These groups were then divided into two subgroups for euthanasia in two and 12 d of life.After euthanasia,the femurs were removed,and the femoral bones were measured linearly between the apex of the greater trochanter until the lower intercondylar midlle face to verify the probable bone growth between 3 and 12 d in control and alednroanto treated rats.Posteriorly,the surgical pieces were also sent to the histopathology laboratory to produce histological slides.The obtained slides were stained with hematoxylin and eosin to measure each of the cartilage zones in endochondral development.and other slides were immunohistochemically tested for anti-TGF-β1 and BMP-2 antibodies to investigate the immunolocalization of these proteins in the epiphyseal plaque area.RESULTS On the third day,some diferences between the control group and specimens treated with alendronate were verified.Macroscopiccaly,we found similarities in size between the femoral bones when we compared the control group with the specimens that received alendronate.On the 12^th day,the bone size of the mice receiving the drug was significantly smaller than those of the control group.These results coincide with changes in the TGF-β1 and BMP-2 expression.In the specimens that received alendronate,the TGF-β1 was expressed in some sites of trabecular bone that was neoformed,peripherally to the bone marrow area.The BMP-2 was also positive in proliferative chondrocytes and hypertrofic chondrocytes.On the 12^th day,all layers of chondrocytes exhibited positivity for BMP-2 in the specimens that received alendronate.In the interface between the trabecular bone and cartilage,an area of disorganized bone deposition was evident.Neoformed bone also appeared to be different at 12 d.In the control group,BMP-2 was positive in an intense area of bone trabeculae,whereas the alendronate-treated group showed TGF-β1 positive trabeculae and a greater bone area.CONCLUSION Alendronate alters the immunolocalization of TGF-β1 and BMP-2 simultaneously,a condition that changes the usual histological aspects of the cartilage zone and impairs epiphysis growth and femur growth. 展开更多

Comminuted olecranon fracture fixation with pre-contoured plate:Comparison of composite and cadaver bones

AIM: To determine whether use of a precontoured olecranon plate provides adequate fixation to withstand supraphysiologic force in a comminuted olecranon fracture model.METHODS: Five samples of fourth generation composite bones and five samples of fresh frozen human cadaveric left ulnae were utilized for this study. The cadaveric specimens underwent dual-energy X-ray absorptiometry(DEXA) scanning to quantify the bone quality. The composite and cadaveric bones were prepared by creating a comminuted olecranon fracture and fixed with a pre-contoured olecranon plate with locking screws. Construct stiffness and failure load were measured by subjecting specimens to cantilever bending moments until failure. Fracture site motion was measured with differential variable resistance transducer spanning the fracture. Statistical analysis was performed with two-tailed Mann-Whitney-U test with Monte Carlo Exact test.RESULTS: There was a significant difference in fixation stiffness and strength between the composite bones and human cadaver bones. Failure modes differed in cadaveric and composite specimens. The load to failure for the composite bones(n = 5) and human cadaver bones(n = 5) specimens were 10.67 nm(range 9.40-11.91 nm) and 13.05 nm(range 12.59-15.38 nm) respectively. This difference was statistically significant(P ? 0.007, 97% power). Median stiffness for composite bones and human cadaver bones specimens were 5.69 nm/mm(range 4.69-6.80 nm/mm) and 7.55 nm/mm(range 6.31-7.72 nm/mm). There was a significant difference for stiffness(P ? 0.033, 79% power) between composite bones and cadaveric bones. No correlation was found between the DEXA results and stiffness. All cadaveric specimens withstood the physiologic load anticipated postoperatively. Catastrophic failure occurred in all composite specimens. All failures resulted from composite bone failure at the distal screw site and not hardware failure. There were no catastrophic fracture failures in the cadaveric specimens. Failure of 4/5 cadaveric specimens was defined when a fracture gap of 2 mm was observed, but 1/5 cadaveric specimens failed due to a failure of the triceps mechanism. All failures occurred at forces greater than that expected in postoperative period prior to healing.CONCLUSION: The pre-contoured olecranon plate provides adequate fixation to withstand physiologic force in a composite bone and cadaveric comminuted olecranon fracture model. 展开更多

The locking compression plates (LCP) are efficient tools in open reduction and internal fixation (ORIF), especially in osteoporotic bones. Two important factors of screw density and screw position can affect the functionality of the bone plate. Several studies have assessed the influence of the screw configurations on the bone-plate stiffness, but the effects of screw positions on the interfragmentary strain, εIF of LCP construct have not been investigated yet. In this study, finite element method was used to investigate the influence of screws number and position on the interfragmentary strain of LCP-femur system for a mid-shaft fracture. Results of this study showed that by insertion of screws closer to the fracture site, εIF decreases by 2nd degree polynomial function versus screw position, but by adding the screws from the ends of the plate, or by moving and placing the screws towards the fracture site, the reduction of εIF will be linear. Results of this study were compared and are in agreement with some studies in the literature, even though their scope was mostly stability of the bone-implant system, whereas our scope was focused on the interfragmentary strain. 展开更多

目的 比较可吸收接骨板与传统微型钛板内固定术治疗颌骨骨折的临床效果。方法 选取2023年6月至2024年6月榆林市第一医院口腔科、空军军医大学口腔医院颅颌面创伤正颌外科收治的90例颌骨骨折患者,采用简单随机分组法将其分为对照组和观察... 目的 比较可吸收接骨板与传统微型钛板内固定术治疗颌骨骨折的临床效果。方法 选取2023年6月至2024年6月榆林市第一医院口腔科、空军军医大学口腔医院颅颌面创伤正颌外科收治的90例颌骨骨折患者,采用简单随机分组法将其分为对照组和观察组,各45例。对照组采用传统微型钛板内固定术治疗,观察组采用可吸收接骨板内固定术治疗。比较两组的颌面部损伤程度、手术指标、临床疗效及并发症发生情况。结果 治疗后,观察组的颌面部损伤严重度评分(MFISS)评分低于对照组(P<0.05)。观察组的手术时长短于对照组(P<0.05);两组的住院时长、术中出血量比较,差异无统计学意义(P>0.05)。观察组的治疗总有效率高于对照组,并发症总发生率低于对照组(P<0.05)。结论 相较于传统微型钛板内固定术,可吸收接骨板内固定术可缩短手术时长,降低术后并发症发生风险,有助于改善颌面部的损伤状况,从而提高治疗效果。 展开更多

背景:与传统椎弓根螺钉相比,皮质骨轨迹螺钉通道全程均为皮质骨,具有更牢靠的生物力学优势,然而安全、准确地置入皮质骨轨迹螺钉对脊柱外科医师技术要求较高。理论上在3D打印个性化导板引导下能有效提高一次性置钉的准确度、简化置钉操... 背景:与传统椎弓根螺钉相比,皮质骨轨迹螺钉通道全程均为皮质骨,具有更牢靠的生物力学优势,然而安全、准确地置入皮质骨轨迹螺钉对脊柱外科医师技术要求较高。理论上在3D打印个性化导板引导下能有效提高一次性置钉的准确度、简化置钉操作、提高手术效率,但目前国内外相关报道较少。目的:探讨应用个性化3D打印导板辅助皮质骨轨迹螺钉置入治疗合并骨质疏松胸腰椎骨折的准确性与安全性。方法:选择2020年1月至2024年1月行皮质骨轨迹螺钉内固定的58例合并骨质疏松的胸腰椎(T_(11)-L_(4))骨折患者,共置入348枚螺钉。根据置钉方案分为2组,3D导板组30例行个性化3D打印导板辅助置钉,共置入螺钉180枚;对照组28例行徒手方式置钉,共计置入螺钉168枚。通过比较两组间手术相关指标、置钉情况、伤椎的复位与脊柱后凸畸形矫正情况以及术后疗效等,评估不同置钉方案的效果和适用性。结果与结论:①3D导板组在切口长度、手术时间、置钉时间、术中透视次数、术中出血量、术后引流量以及住院天数方面均显著优于对照组(P<0.05),但是两组的总费用相比差异无显著性意义(P>0.05);②3D导板组的置钉准确率高于对照组,螺钉对上关节突的侵犯率低于对照组,差异有显著性意义(P<0.05);③术后评估结果显示,两组在伤椎前缘高度百分比、Cobb角、目测类比评分以及Oswestry功能障碍指数方面均较术前有所改善,且差异有显著性意义(P<0.05);组间相比,3D导板组的Oswestry功能障碍指数更低,显示出更佳的胸腰椎功能(P<0.05),而在术后伤椎前缘高度百分比、Cobb角及目测类比评分方面,两组之间无显著性差异(P>0.05);④在围术期及随访期间,两组患者均未出现并发症;⑤提示个性化3D打印导板辅助皮质骨轨迹螺钉置入治疗合并骨质疏松的胸腰椎骨折是一种可行的方式,具有微创、高效、精准及安全等优势,可获得满意的临床效果。 展开更多

目的比较髓内钉内固定术与钢板螺钉内固定术对四肢长管骨创伤骨折的疗效。方法选取南阳市中心医院2023年2月至2024年12月的68例四肢长管骨折患者,随机分为对照组与研究组,每组各34例,对照组行钢板螺钉内固定术,研究组行髓内钉内固定术... 目的比较髓内钉内固定术与钢板螺钉内固定术对四肢长管骨创伤骨折的疗效。方法选取南阳市中心医院2023年2月至2024年12月的68例四肢长管骨折患者,随机分为对照组与研究组,每组各34例,对照组行钢板螺钉内固定术,研究组行髓内钉内固定术。比较两组围术期指标、疼痛程度、骨代谢指标、运动功能及并发症。结果两组术中出血量、手术时间比较,差异无统计学意义(P>0.05);研究组骨折愈合时间短于对照组,差异有统计学意义(P<0.05)。术后1周、术后2周,两组视觉模拟评分法(VAS)评分均低于术前1天,且研究组VAS评分均低于同时段对照组,差异有统计学意义(P<0.05)。术后3个月,两组骨钙素(BGP)、骨形态发生蛋白-2(BMP-2)、I型前胶原羧基端肽(PICP)水平均较术前1天升高,且研究组高于对照组,差异有统计学意义(P<0.05)。术后3个月,两组上肢Fugl-Meyer运动功能评定量表(FMA)评分、下肢FMA评分、FMA总分均较术前1天升高,且研究组高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论与钢板螺钉内固定术相比,髓内钉内固定术治疗四肢长管骨折愈合时间更短,术后疼痛更轻,骨代谢与运动功能恢复更快。 展开更多

目的比较3D打印模型结合增强现实(augmented reality,AR)导航系统与传统视频教学在整形外科专科医师筛骨垂直板切取操作培训中的效果。方法选取自2024年1月至2025年12月于北京大学第三医院培训的14名专科医师,随机分为AR组(7名)和视频... 目的比较3D打印模型结合增强现实(augmented reality,AR)导航系统与传统视频教学在整形外科专科医师筛骨垂直板切取操作培训中的效果。方法选取自2024年1月至2025年12月于北京大学第三医院培训的14名专科医师,随机分为AR组(7名)和视频组(7名)。AR组在3D打印患者头部模型上使用AR导航系统进行术前模拟训练,视频组仅观看手术视频学习。随后两组在无导航条件下于8个3D打印模型上独立完成筛骨垂直板切取模拟操作,记录各关键点的定位误差和操作时间,并进行统计学比较。结果AR组整体操作误差中位数(6.250 mm)显著低于视频组(7.810 mm),P<0.0001,尤其在结构复杂的模型上差异更为明显。在单个模型分析中,结构较为复杂的模型(1)和模型(4)上,AR组的误差显著低于视频组(P=0.0020和P=0.0112)。操作时间方面,AR组平均用时(32.02±21.83)s略高于视频组(21.49±11.28)s,差异无统计学意义(P=0.2235)。各关键点误差分析显示,筛骨垂直板后缘(C、E点)误差最大,AR组在该区域的误差亦低于视频组。结论结合3D打印与AR导航的培训系统能显著提升整形外科医师在筛骨垂直板切取操作中的空间定位与操作准确性,尤其适用于解剖结构复杂的案例。该系统为高风险精细手术的模拟培训提供了有效工具,具有临床应用与教学推广潜力。 展开更多

目的:探讨前路病灶清除加钛网植骨、Z形前路钢板内同定治疗多节段腰椎结核的意义及疗效。方法:总结2002—2006年期间收治的12例多节段腰椎结核患者,并采用前路病灶清除,椎体间钛网植骨、Z形前路钢板内固定进行治疗。结果:随访8-29... 目的:探讨前路病灶清除加钛网植骨、Z形前路钢板内同定治疗多节段腰椎结核的意义及疗效。方法:总结2002—2006年期间收治的12例多节段腰椎结核患者,并采用前路病灶清除,椎体间钛网植骨、Z形前路钢板内固定进行治疗。结果:随访8-29个月(平均17个月),脊髓神经功能得到不同程度的恢复,植骨融合满意,钛网无松动、移位和塌陷,无内固定失败和脊柱结核复发,后凸角度平均矫正16.5(°)。2年后随访患者临床症状好转,脊柱畸形矫正情况满意。结论:前路病灶清除加钛网植骨、Z—plate内固定治疗多节段腰椎结核的术式有术野显露清楚、病灶清除更彻底、脊柱稳定性重建可靠、植骨融合率和结核治愈率高等特点,是目前外科治疗多节段腰椎结核的可靠方法之一。 展开更多

目的:观察改良Nice结辅助接骨板内固定术治疗掌指骨斜形骨折患者的效果。方法:选取2022年1月至2025年1月该院收治的120例掌指骨斜形骨折患者进行前瞻性研究,根据随机数字表法将其分为对照组和研究组各60例。对照组采用克氏针内固定术治... 目的:观察改良Nice结辅助接骨板内固定术治疗掌指骨斜形骨折患者的效果。方法:选取2022年1月至2025年1月该院收治的120例掌指骨斜形骨折患者进行前瞻性研究,根据随机数字表法将其分为对照组和研究组各60例。对照组采用克氏针内固定术治疗,研究组采用改良Nice结辅助接骨板内固定术治疗,比较两组手术相关指标(手术时间、住院时间、术中出血量、透视次数、骨折临床愈合时间)水平,手术前后应激指标[C反应蛋白(CRP)、去甲肾上腺素(NE)、皮质醇(Cor)]水平,术后关节活动度(握力丢失度、手指关节总活动度),手术效果,以及并发症发生率。结果:两组手术时间、住院时间比较,差异均无统计学意义(P>0.05);研究组术中出血量、透视次数均少于对照组,骨折临床愈合时间短于对照组,差异有统计学意义(P<0.05);术后3 d,两组CRP、NE、Cor水平均高于术前,但研究组低于对照组,差异有统计学意义(P<0.05);术后3、6个月,研究组握力丢失度均低于对照组,手指关节总活动度均大于对照组,差异有统计学意义(P<0.05);研究组手术优良率为94.92%,高于对照组的80.70%,差异有统计学意义(P<0.05);两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:改良Nice结辅助接骨板内固定术治疗掌指骨斜形骨折患者可提高关节活动度和手术优良率,减少术中出血量和透视次数,缩短骨折临床愈合时间,以及降低应激指标水平的效果优于氏针内固定术治疗。 展开更多

Acetabular component revision in the presence of severe bone loss is difficult for reconstructing an acetabular component in the anatomical hip center. Various treatment options are available, although often the acetabular defect cannot be corrected with a single option alone. Precise assessment of the bone loss and a suitable combination of methods are needed. Here we report a case of multiple failures with impaction bone grafting reconstruction for an acetabular bone defect of American Academy of Orthopedic Surgeons classification type III. We finally reconstructed the acetabulum with three femoral head allografts and a Kerboull-type acetabular reinforcement device. The allograft was a casted, jet-type helmet-like shape. A year later the patient was able to walk without a cane and perform light agricultural work. Accurate evaluation of the acetabular bone loss and appropriate reconstruction is important. 展开更多

掌侧钢板内固定术后桡骨远端骨折患者背侧关节内骨块移位发生情况及影响因素分析

目的探讨行掌侧钢板内固定术(VPF)的桡骨远端骨折(DRF)患者术后背侧关节内骨块移位发生情况,并分析其影响因素。方法回顾性分析100例行掌侧钢板内固定术的DRF患者的临床资料,依据术后是否发生背侧关节内骨块移位分为发生组、未发生组,... 目的探讨行掌侧钢板内固定术(VPF)的桡骨远端骨折(DRF)患者术后背侧关节内骨块移位发生情况,并分析其影响因素。方法回顾性分析100例行掌侧钢板内固定术的DRF患者的临床资料,依据术后是否发生背侧关节内骨块移位分为发生组、未发生组,并通过单因素、多因素Logistic回归分析术后背侧关节内骨块移位发生的影响因素。结果100例DRF患者掌侧钢板内固定术后发生背侧关节内骨块移位的有24例(发生组),发生率为24.00%(24/100);未发生背侧关节内骨块移位的有76例(未发生组),未发生率为76.00%(76/100)。单因素分析显示,发生组骨块总移位距离(11.25±1.78)mm比未发生组的(7.46±1.05)mm长,骨块平均横向宽度(8.63±1.48)mm比未发生组的(10.60±1.92)mm短,骨块平均纵横比(1.23±0.30)、背侧骨块总体积(685.86±124.67)mm^(3)比未发生组的(1.45±0.35)、(943.97±178.96)mm^(3)小,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,骨块总移位距离较长[OR=0.095,95%CI=(0.027,0.330)]、骨块平均横向宽度较短[OR=1.832,95%CI=(1.347,2.493)]、骨块平均纵横比较小[OR=6.692,95%CI=(1.566,28.601)]以及背侧骨块总体积较小[OR=1.009,95%CI=(1.005,1.013)]是DRF患者掌侧钢板内固定术后发生背侧关节内骨块移位的独立危险因素(P<0.05)。结论DRF患者掌侧钢板内固定术后背侧关节内骨块移位发生率仍然较高,主要与骨块总移位距离较长、骨块平均横向宽度较短等因素有关,临床应采取相应措施进行干预。 展开更多

背景:PauwelsⅢ型股骨颈骨折往往会伴随不同程度的骨缺损,其骨缺损程度与内固定的稳定性是影响骨折愈合的关键因素。目的:有限元分析模拟并对比联合内侧支撑钢板固定PauwelsⅢ型股骨颈骨折伴不同程度骨缺损的稳定性,为实际临床术式提供... 背景:PauwelsⅢ型股骨颈骨折往往会伴随不同程度的骨缺损,其骨缺损程度与内固定的稳定性是影响骨折愈合的关键因素。目的:有限元分析模拟并对比联合内侧支撑钢板固定PauwelsⅢ型股骨颈骨折伴不同程度骨缺损的稳定性,为实际临床术式提供实验依据。方法:利用Mimics 21.0、Geomagic和Solidworks软件构建Pauwels角为70°且伴有4种不同程度后内侧骨缺损的PauwelsⅢ型股骨颈骨折模型,模拟空心钉联合内侧支撑钢板、股骨颈动力交叉钉系统联合内侧支撑钢板、单独股骨颈动力交叉钉系统3种内固定方法,两两组合成12组骨折骨缺损内固定模型组。在单腿站立工况3倍体质量2100 N的载荷下分析12组模型股骨、内固定、股骨头及骨折面的应力和位移变化。结果与结论:①在各个程度的骨缺损中,空心钉联合内侧支撑钢板无论在股骨整体还是内固定,其位移与应力都比股骨颈动力交叉钉系统联合内侧钢板及单独用股骨颈动力交叉钉系统固定小;②随着股骨后内侧骨缺损体积的增大,股骨与内固定的位移呈递增趋势;③提示空心钉联合内侧支撑钢板固定相较于股骨颈动力交叉钉系统联合内侧支撑钢板固定在生物力学上更具优势,表现为更小的位移峰值、更均匀的应力分布及更低的应力集中风险;骨缺损区的结构性支撑重建对于伴中、大程度骨缺损的股骨颈骨折治疗极为重要。 展开更多
