In a post conflict environment, the abnormal movement of firearms and poverty from them may change the configuration’s epidemiological, anatomical and clinical fractures of the limbs bones as well as their care. Goal: To make an inventory on the fractures of the limb bones in Bukavu town, a town in post armed conflict. Method: This work is transversal, retrospective and descriptive on 500 fractures of the limbs bones recorded on a period of 3 months from 1st June to 31st August 2013. Twenty investigators went around different places of Bukavu town to interview and complete a research protocol ad hoc. In total, we recorded 500 subjects with 570 fractures of the limb bones. Results: The average age is 29.4 years old and 37% are male fractured. The main circumstances of occurrence of the fracture of the limbs bones are the falls (31.7%), firearms (22.2%) followed closely by road traffic accidents (21.9%). Students (38.6%) and the unemployed (29.4%) are most victims’ fractures of limb bones. The forearm bones (31.7%) and leg bones (27.2%) are most concerned, 52.2% fractured turned to modern medicine against 41.8% for traditional medicine. Conclusion: The fractures of the limbs bones in Bukavu mainly concern young male subjects. They occur with the waning of a fall and preferentially affect the bones of the forearm. Good governance would reduce the incidence of fractures of the limbs bones and their assessment.展开更多

We recorded 127 (65.1%) cases of wound infection and 143 (73.3%) cases of limb shortening as the most common complications. Lower limb injures are frequently observed in passenger car traffic accidents.Previous studies of the injuries focus on long bone fractures by using either cadaver component tests or simulations of the long bone kinem...Lower limb injures are frequently observed in passenger car traffic accidents.Previous studies of the injuries focus on long bone fractures by using either cadaver component tests or simulations of the long bone kinematics,which lack in-depth study on the fractures in stress analysis.This paper aims to investigate lower limb impact biomechanics in real-world car to pedestrian accidents and to predict fractures of long bones in term of stress parameter for femur,tibia,and fibula.For the above purposes,a 3D finite element(FE) model of human body lower limb(HBM-LL) is developed based on human anatomy.The model consists of the pelvis,femur,tibia,fibula,patella,foot bones,primary tendons,knee joint capsule,meniscus,and ligaments.The FE model is validated by comparing the results from a lateral impact between simulations and tests with cadaver lower limb specimens.Two real-world accidents are selected from an in-depth accident database with detailed information about the accident scene,car impact speed,damage to the car,and pedestrian injuries.Multi-body system(MBS) models are used to reconstruct the kinematics of the pedestrians in the two accidents and the impact conditions are calculated for initial impact velocity and orientations of the car and pedestrian during the collision.The FE model is used to perform injury reconstructions and predict the fractures by using physical parameters,such as von Mises stress of long bones.The calculated failure level of the long bones is correlated with the injury outcomes observed from the two accident cases.The reconstruction result shows that the HBM-LL FE model has acceptable biofidelity and can be applied to predict the risk of long bone fractures.This study provides an efficient methodology to investigate the long bone fracture suffered from vehicle traffic collisions.展开更多

In 6 patients, aged 19 to 54,with the limb-length discrepancy being at least 3 to 4. 5 cm, the tibia was lengthened by 3 cm in 3 cases, 4 to 4. 5 cm in 3 cases. The consolidation and maturation of the bone were accomplished within 3 to 6 months. The authors believe that this procedure is simple, less invasive and involves no bone-graft.展开更多
BACKGROUND Severe open tibia fractures are challenging to treat with a lack of published clear management strategies.Our aim was to provide an overview of the largest singlecenter experience in the literature,with min...BACKGROUND Severe open tibia fractures are challenging to treat with a lack of published clear management strategies.Our aim was to provide an overview of the largest singlecenter experience in the literature,with minimum 1-year follow-up,of adult type 3 open tibial shaft fractures at Cambridge University Hospitals(a United Kingdom major trauma center).We sought to define patient characteristics and our main outcome measures were infection,union and re-fracture.AIM To retrospectively define patient and injury characteristics,present our surgical methods and analyze our outcomes–namely infection,union and re-fracture rates.METHODS Consecutive series of 74 patients with 75 open tibial fractures treated between 2014 and 2020(26 classified as Gustilo-Anderson 3A,47 were 3B and two were 3C).Nine patients underwent intramedullary nailing(IMN),61 underwent Taylor spatial frame(TSF)fixation and 5 were treated with Masquelet technique(IMN and subsequent bone grafting).RESULTS Mean follow-up was 16 mo(IMN)and 25 mo(TSF).We had an infection rate of 6.7%(5),non-union rate of 4%(3)and re-fracture rate of 2.7%(2).Average time to union was 22 wk for IMN and 38.6 wk for TSF.Thirty-three cases had a bone defect with a mean of 5.4 cm(2-11).Patient age,sex,diabetes,smoking status or injury severity did not have a significant effect on union time with either fixation method.Our limb salvage rate was 98.7%.CONCLUSION Grade 1 to 3A injuries can effectively be treated with reamed or unreamed IMN.Grade 3B/C injuries are best treated by circular external fixators as they provide good,reproducible outcomes and allow large bone defects to be addressed via distraction osteogenesis.展开更多

Bone is a unique tissue that is capable of repairing itself after damage.However,there are certain instances of fractures and defects that require clinical intervention for proper alignment and healing.As with any imp...Bone is a unique tissue that is capable of repairing itself after damage.However,there are certain instances of fractures and defects that require clinical intervention for proper alignment and healing.As with any implant,careful consideration of the material used to create the implants to treat these problems is needed.If the incorrect material is chosen,the implants themselves can lead to bone fractures or defects,or bone healing may not take place at all.All three classes of biomaterials-metals,ceramics,and polymers-have been used in the treatment of both bone fractures and bone defects,and each has its own unique benefits and limitations for its applications.Furthermore,composites of these different materials have also been created to try to take advantage of all the different benefits offered by each different material.This review highlights different materials that have been used for the development of internal fixators and bone graft substitutes to treat fracture and bone defects as well as their limitations and needed future research.展开更多
In vivo testing is crucial for the evaluation of orthopedic implant efficacy and safety.However,the translation and reproducibility of preclinical animal experiments are not always satisfactory,and reporting quality i...In vivo testing is crucial for the evaluation of orthopedic implant efficacy and safety.However,the translation and reproducibility of preclinical animal experiments are not always satisfactory,and reporting quality is among the essential factors that ensure appropriate delivery of information.In this study,we assessed the reporting quality of in vivo investigations that examined the use of degradable metal materials in fracture or bone defect repair.We employed scientific databases,such as PubMed,EMBASE,Web of Science,Cochrane Library,CNKI,WanFang,VIP and Sinomed to screen for in vivo investigations on fracture or bone defect repair using degradable metal materials,and extracted both epidemiological and main characteristics of eligible studies,and assessed their reporting quality using the ARRIVE guidelines 2.0.Overall,263 publications were selected,including 275 animal experiments.The overall coincidence rate of Essential 10(22 sub-items)and Recommended Set(16 sub-items)were 42.0%and 41.5%,respectively.Based on our analysis,the reporting quality of the published in vivo investigations examining fracture/bone defect repair with degradable metal materials was low,and there was a lack of transparent,accurate and comprehensive reporting on key elements of the experimental design and other elements that are meant to avoid bias.展开更多

文摘In vivo testing is crucial for the evaluation of orthopedic implant efficacy and safety.However,the translation and reproducibility of preclinical animal experiments are not always satisfactory,and reporting quality is among the essential factors that ensure appropriate delivery of information.In this study,we assessed the reporting quality of in vivo investigations that examined the use of degradable metal materials in fracture or bone defect repair.We employed scientific databases,such as PubMed,EMBASE,Web of Science,Cochrane Library,CNKI,WanFang,VIP and Sinomed to screen for in vivo investigations on fracture or bone defect repair using degradable metal materials,and extracted both epidemiological and main characteristics of eligible studies,and assessed their reporting quality using the ARRIVE guidelines 2.0.Overall,263 publications were selected,including 275 animal experiments.The overall coincidence rate of Essential 10(22 sub-items)and Recommended Set(16 sub-items)were 42.0%and 41.5%,respectively.Based on our analysis,the reporting quality of the published in vivo investigations examining fracture/bone defect repair with degradable metal materials was low,and there was a lack of transparent,accurate and comprehensive reporting on key elements of the experimental design and other elements that are meant to avoid bias.
