Amyotrophic lateral sclerosis is a rare neurodegenerative disease characterized by the involvement of both upper and lower motor neurons.Early bilateral limb involvement significantly affects patients'daily lives ...Amyotrophic lateral sclerosis is a rare neurodegenerative disease characterized by the involvement of both upper and lower motor neurons.Early bilateral limb involvement significantly affects patients'daily lives and may lead them to be confined to bed.However,the effect of upper and lower motor neuron impairment and other risk factors on bilateral limb involvement is unclear.To address this issue,we retrospectively collected data from 586 amyotrophic lateral sclerosis patients with limb onset diagnosed at Peking University Third Hospital between January 2020 and May 2022.A univariate analysis revealed no significant differences in the time intervals of spread in different directions between individuals with upper motor neuron-dominant amyotrophic lateral sclerosis and those with classic amyotrophic lateral sclerosis.We used causal directed acyclic graphs for risk factor determination and Cox proportional hazards models to investigate the association between the duration of bilateral limb involvement and clinical baseline characteristics in amyotrophic lateral sclerosis patients.Multiple factor analyses revealed that higher upper motor neuron scores(hazard ratio[HR]=1.05,95%confidence interval[CI]=1.01–1.09,P=0.018),onset in the left limb(HR=0.72,95%CI=0.58–0.89,P=0.002),and a horizontal pattern of progression(HR=0.46,95%CI=0.37–0.58,P<0.001)were risk factors for a shorter interval until bilateral limb involvement.The results demonstrated that a greater degree of upper motor neuron involvement might cause contralateral limb involvement to progress more quickly in limb-onset amyotrophic lateral sclerosis patients.These findings may improve the management of amyotrophic lateral sclerosis patients with limb onset and the prediction of patient prognosis.展开更多

Coloboma can affect the adnexa and/or the eyeball. We report the iconography of two cases of ocular colobomas and one case of eyelids coloboma at the IOTA CHU. Case 1: This was a 32-year-old patient who consulted for decreased visual acuity in the left eye. The clinical examination found visual acuity of 10/10 in the right eye and hand movement in the left eye. Biomicroscopic examination revealed an iris and chorioretinal coloboma encompassing the papilla. Case 2: This was a 3-year-old child. Who consulted for eyelid malformation since birth. On ophthalmological examination, there was a bilateral coloboma of the inner half of the upper eyelid, associated with an underlying symblepharon and left eye corneal dystrophy. Our course of action consisted of a release of the symblepharon followed by a blepharoplasty which allowed a good anatomical and functional restitution. Case 3: This was a 15-year-old patient who consulted for bilateral visual acuity loss measured by counting fingers at 1m in the right eye and counting fingers at 3m in the left eye. The biomicroscopic examination found a bilateral papillary coloboma associated with a bilateral progressive cataract, an old detachment of the left retina. The etiological assessment was unremarkable. Conclusion: Coloboma is a congenital condition that can affect all parts of the eye. We reported three cases of colobomas, one palpebral, one iris and chorioretinal, and finally, one papillary associated with other ocular pathologies.展开更多

BACKGROUND Bilateral internal mammary arteries(BIMAs)as the most advanced surgical option for coronary artery bypass grafting(CABG)are usually recommended for younger patients without traditional risk factors.This stu...BACKGROUND Bilateral internal mammary arteries(BIMAs)as the most advanced surgical option for coronary artery bypass grafting(CABG)are usually recommended for younger patients without traditional risk factors.This study compares outcomes in propensity score-matched patients aged over 70 years who received BIMAs versus those who received a single internal mammary artery(SIMA).METHODS From 2013 to 2024,8123 patients underwent primary CABG for multivessel coronary artery disease at our institution.BIMA grafting was performed in 1233 patients(15.17%),with in situ BIMA grafting in 290 patients(3.57%).For in situ BIMA group,the right internal mammary artery was used to revascularize the right coronary artery,while the left internal mammary artery was utilized for the left anterior descending artery.BIMA patients aged over 70 years(n=79)were compared with SIMA patients(n=79)using propensity score matching.Primary outcome was all-cause mortality at 30 days and 8 years.Secondary outcomes included length of hospital stay,incidence of postoperative major adverse cardiovascular and cerebrovascular events,sternal wound infection and the need for subsequent percutaneous revascularization.RESULTS There was no difference in immediate postoperative primary and secondary outcomes.Mean follow-up was 8.3±1.0 years with an 8-year freedom from death of 67.08%±1.1%in the BIMA group versus 58.22%±0.9%in the SIMA group(P<0.05).CONCLUSIONS BIMAs as in situ grafts can be successfully used in CABG for patients aged 70 years and older.Consequently,the refined techniques for constructing internal mammary artery grafts used in this study challenge traditionally accepted limitations regarding the use of BIMAs.展开更多

BACKGROUND Bilateral hip disorder is a common finding that can occur in approximately 42%of the population with osteoarthritis.It is estimated that 25%individuals with osteoarthritis requiring total hip replacement(TH...BACKGROUND Bilateral hip disorder is a common finding that can occur in approximately 42%of the population with osteoarthritis.It is estimated that 25%individuals with osteoarthritis requiring total hip replacement(THR)may require a bilateral replacement.This has resulted in the test of the greatest strategy to run single staged bilateral hip replacement while addressing the outcomes to achieve swift and cost-effective patient recovery.AIM To assess the outcomes and cost effectiveness of bilateral THR(B/L THR)at our tertiary care hospital.METHODS Retrospective observational cross-sectional study was undertaken from Jan 2018 to July 2023 to assess the clinical outcomes of patients who underwent single stage B/L THR.RESULTS Data of 75 patients were analysed.The mean age was 36 years.Our complication rate was 4.0%including acute coronary syndrome,intra-operative acetabular fracture and paralytic ileus.The re-admission rate was 4%.CONCLUSION The choice of sequential or bilateral hip replacement is controversial.While,our study showed that bilateral hip replacement is safe and cost effective.As surgeons,we were careful in patient selection(low American Society of Anesthesiologist score).Though more than 50%of our B/L THR patients were obese[body mass index(BMI)>25],our outcomes were equivalent to normal BMI patients with lower risk of complication as well as early ambulation.Systemic complication deep vein thrombosis and pulmonary embolism were handled prophylactively by close monitoring,use of mechanical and pharmacological agents along with anticoagulants.Patients who require THR,often require them bilaterally and single stage replacement thus offers early restoration of an individual into their activities of daily living with minimal complications.Our findings support the use of single-stage B/L THR as a viable option for bilateral hip disorders,having favourable outcomes.展开更多

Objective:Nowadays robot-assisted partial nephrectomy(RAPN)represents the standard of care for clinical T1(cT1)renal masses,providing similar oncological outcomes when compared to open or laparoscopic PN with advantag...Objective:Nowadays robot-assisted partial nephrectomy(RAPN)represents the standard of care for clinical T1(cT1)renal masses,providing similar oncological outcomes when compared to open or laparoscopic PN with advantages in terms of functional outcomes and lower perioperative comorbidity,when compared to radical nephrectomy.Methods:We performed an extensive literature review of studies regarding RAPN,its evolution,technical aspects and applications,and new technological tools using different combinations of Medical Subject Headings terms“RAPN”,“partial nephrectomy”,“robot-assisted”,“nephron-sparing surgery”,“renal cell carcinoma”,“complex renal masses”,“endophytic renal masses”,and“bilateral renal tumors”.Results:A consistent body of evidence was selected,including original articles,systematic reviews,meta-analyses,and clinical trials having RAPN as the central focus in adult patients,with all its technical nuances.We started our narrative review with a background on PN and its evolution toward the robotic era with a special spotlight on the extending indications for PN in large and highly complex renal masses.Our review continued with an overview of nephron-sparing surgery in bilateral and recurrent masses.RAPN for bilateral synchronous renal masses represents a challenging scenario with no formal recommendations provided by international guidelines and controversial management and decision-making.Additionally,we reported evidence on redo RAPN which seems to be safe and effective.A final overview of the available technological tools,and in particular on three-dimensional reconstruction was provided.Conclusion:RAPN has been established as the standard of care for cT1 renal masses with an expanding spectrum of applications in different scenarios,including large(cT2),highly complex,and bilateral renal masses,as well as the surgical treatment of local recurrences after nephron-sparing surgery with acknowledged advantages in terms of functional outcomes and perioperative risk profiles while maintaining similar oncological outcomes when compared to open or laparoscopic PN and radical treatment.展开更多

Background:Deficits in quadriceps strength of the injured leg have been observed in patients following anterior cruciate ligament(ACL)reconstructions and may contribute to ACL re-injury risk.Single-leg forward hopping...Background:Deficits in quadriceps strength of the injured leg have been observed in patients following anterior cruciate ligament(ACL)reconstructions and may contribute to ACL re-injury risk.Single-leg forward hopping is a widely used task for assessing knee function in patients following ACL reconstructions as it has been shown not to be particularly challenging to the knee.This study aimed to quantify the effect of decreased quadriceps strength induced by a fatigue protocol on hopping performance and lower limb mechanics in single-leg forward,vertical,and backward hopping.Methods:Thirty-four injury-free participants performed single-leg forward,vertical,and backward hopping on both legs pre-and post-fatigue,with 1 leg experiencing a fatigue protocol.Peak moments,power,and work of hip,knee,and ankle joints were quantified during the jumping phase.Hopping performance and bilateral asymmetries in performance were assessed.Results:Single-leg backward hopping demonstrated the greatest knee moments,power,and work compared to forward and vertical hopping,regardless of leg and fatigue.Fatigue protocol resulted in significantly less knee moments,power,and work,and decreased performance of the fatigued leg among all tasks.Bilateral symmetries in hopping performance decreased in post-fatigue,with the greatest decrease in backward hopping.Conclusion:The greater sensitivity of the backward hopping to detect quadriceps fatigue suggests it may act as a better or at least an additional metric to evaluate quadriceps strength deficits.The findings may contribute to the development of a clinically applicable and valid strength assessment to monitor the rehabilitation progress in patients following ACL reconstructions.展开更多

文摘Force feedback bilateral teleoperation represents a pivotal advancement in control technology,finding widespread application in hazardous material transportation,perilous environments,space and deep-sea exploration,and healthcare domains.This paper traces the evolutionary trajectory of force feedback bilateral teleoperation from its conceptual inception to its current complexity.It elucidates the fundamental principles underpinning interaction forces and tactile exchanges,with a specific emphasis on the crucial role of tactile devices.In this review,a quantitative analysis of force feedback bilateral teleoperation development trends from 2011 to 2024 has been conducted,utilizing published journal article data as the primary source of information.The review accentuates classical control frameworks and algorithms,while also delving into existing research advancements and prospec-tive breakthrough directions.Moreover,it explores specific practical scenarios ranging from intricate surgeries to hazardous environment exploration,underscoring the technology’s potential to revolutionize industries by augmenting human manipulation of remote systems.This underscores the pivotal role of force feedback bilateral teleoperation as a transformative human-machine interface,capable of shaping flexible control strategies and addressing technological bottlenecks.Future research endeavors in force feedback bilateral teleoperation are expected to prioritize the creation of more immersive experiences,overcoming technical hurdles,fortifying human-machine collaboration,and broadening application domains,particularly within the realms of medical intervention and hazardous environments.With the continuous progression of technology,the integration of human intelligence and robotic capabilities is expected to produce more innovations and breakthroughs in the field of automatic control.
文摘BACKGROUND Bilateral internal mammary arteries(BIMAs)as the most advanced surgical option for coronary artery bypass grafting(CABG)are usually recommended for younger patients without traditional risk factors.This study compares outcomes in propensity score-matched patients aged over 70 years who received BIMAs versus those who received a single internal mammary artery(SIMA).METHODS From 2013 to 2024,8123 patients underwent primary CABG for multivessel coronary artery disease at our institution.BIMA grafting was performed in 1233 patients(15.17%),with in situ BIMA grafting in 290 patients(3.57%).For in situ BIMA group,the right internal mammary artery was used to revascularize the right coronary artery,while the left internal mammary artery was utilized for the left anterior descending artery.BIMA patients aged over 70 years(n=79)were compared with SIMA patients(n=79)using propensity score matching.Primary outcome was all-cause mortality at 30 days and 8 years.Secondary outcomes included length of hospital stay,incidence of postoperative major adverse cardiovascular and cerebrovascular events,sternal wound infection and the need for subsequent percutaneous revascularization.RESULTS There was no difference in immediate postoperative primary and secondary outcomes.Mean follow-up was 8.3±1.0 years with an 8-year freedom from death of 67.08%±1.1%in the BIMA group versus 58.22%±0.9%in the SIMA group(P<0.05).CONCLUSIONS BIMAs as in situ grafts can be successfully used in CABG for patients aged 70 years and older.Consequently,the refined techniques for constructing internal mammary artery grafts used in this study challenge traditionally accepted limitations regarding the use of BIMAs.

文摘BACKGROUND Bilateral hip disorder is a common finding that can occur in approximately 42%of the population with osteoarthritis.It is estimated that 25%individuals with osteoarthritis requiring total hip replacement(THR)may require a bilateral replacement.This has resulted in the test of the greatest strategy to run single staged bilateral hip replacement while addressing the outcomes to achieve swift and cost-effective patient recovery.AIM To assess the outcomes and cost effectiveness of bilateral THR(B/L THR)at our tertiary care hospital.METHODS Retrospective observational cross-sectional study was undertaken from Jan 2018 to July 2023 to assess the clinical outcomes of patients who underwent single stage B/L THR.RESULTS Data of 75 patients were analysed.The mean age was 36 years.Our complication rate was 4.0%including acute coronary syndrome,intra-operative acetabular fracture and paralytic ileus.The re-admission rate was 4%.CONCLUSION The choice of sequential or bilateral hip replacement is controversial.While,our study showed that bilateral hip replacement is safe and cost effective.As surgeons,we were careful in patient selection(low American Society of Anesthesiologist score).Though more than 50%of our B/L THR patients were obese[body mass index(BMI)>25],our outcomes were equivalent to normal BMI patients with lower risk of complication as well as early ambulation.Systemic complication deep vein thrombosis and pulmonary embolism were handled prophylactively by close monitoring,use of mechanical and pharmacological agents along with anticoagulants.Patients who require THR,often require them bilaterally and single stage replacement thus offers early restoration of an individual into their activities of daily living with minimal complications.Our findings support the use of single-stage B/L THR as a viable option for bilateral hip disorders,having favourable outcomes.

文摘Objective:Nowadays robot-assisted partial nephrectomy(RAPN)represents the standard of care for clinical T1(cT1)renal masses,providing similar oncological outcomes when compared to open or laparoscopic PN with advantages in terms of functional outcomes and lower perioperative comorbidity,when compared to radical nephrectomy.Methods:We performed an extensive literature review of studies regarding RAPN,its evolution,technical aspects and applications,and new technological tools using different combinations of Medical Subject Headings terms“RAPN”,“partial nephrectomy”,“robot-assisted”,“nephron-sparing surgery”,“renal cell carcinoma”,“complex renal masses”,“endophytic renal masses”,and“bilateral renal tumors”.Results:A consistent body of evidence was selected,including original articles,systematic reviews,meta-analyses,and clinical trials having RAPN as the central focus in adult patients,with all its technical nuances.We started our narrative review with a background on PN and its evolution toward the robotic era with a special spotlight on the extending indications for PN in large and highly complex renal masses.Our review continued with an overview of nephron-sparing surgery in bilateral and recurrent masses.RAPN for bilateral synchronous renal masses represents a challenging scenario with no formal recommendations provided by international guidelines and controversial management and decision-making.Additionally,we reported evidence on redo RAPN which seems to be safe and effective.A final overview of the available technological tools,and in particular on three-dimensional reconstruction was provided.Conclusion:RAPN has been established as the standard of care for cT1 renal masses with an expanding spectrum of applications in different scenarios,including large(cT2),highly complex,and bilateral renal masses,as well as the surgical treatment of local recurrences after nephron-sparing surgery with acknowledged advantages in terms of functional outcomes and perioperative risk profiles while maintaining similar oncological outcomes when compared to open or laparoscopic PN and radical treatment.

文摘Background:Deficits in quadriceps strength of the injured leg have been observed in patients following anterior cruciate ligament(ACL)reconstructions and may contribute to ACL re-injury risk.Single-leg forward hopping is a widely used task for assessing knee function in patients following ACL reconstructions as it has been shown not to be particularly challenging to the knee.This study aimed to quantify the effect of decreased quadriceps strength induced by a fatigue protocol on hopping performance and lower limb mechanics in single-leg forward,vertical,and backward hopping.Methods:Thirty-four injury-free participants performed single-leg forward,vertical,and backward hopping on both legs pre-and post-fatigue,with 1 leg experiencing a fatigue protocol.Peak moments,power,and work of hip,knee,and ankle joints were quantified during the jumping phase.Hopping performance and bilateral asymmetries in performance were assessed.Results:Single-leg backward hopping demonstrated the greatest knee moments,power,and work compared to forward and vertical hopping,regardless of leg and fatigue.Fatigue protocol resulted in significantly less knee moments,power,and work,and decreased performance of the fatigued leg among all tasks.Bilateral symmetries in hopping performance decreased in post-fatigue,with the greatest decrease in backward hopping.Conclusion:The greater sensitivity of the backward hopping to detect quadriceps fatigue suggests it may act as a better or at least an additional metric to evaluate quadriceps strength deficits.The findings may contribute to the development of a clinically applicable and valid strength assessment to monitor the rehabilitation progress in patients following ACL reconstructions.
文摘BACKGROUND Congenital hallux varus(CHV)is a rare form of hallux varus deformity,characterized by medial deviation of the first toe at the metatarsophalangeal joint.It may be primary or secondary and presents clinically with pain and asymmetry with footwear.CASE SUMMARY We documented a case of a 6-year-old girl with bilateral CHV,accompanied by adduction of the toes in the left foot.Clinical diagnosis was made by physical examination and X-ray imaging based on Bleck’s classification.Conservative treatment did not show any noticeable improvement,so the child underwent corrective surgeries on both feet.CONCLUSION The patient’s family history is positive,which requires us to take into account the importance of checking for a family history with any complaint of CHV,and both feet must be evaluated to confirm whether the deformity is unilateral or bilateral.
