Aim: To identify and define prostate and seminal vesicle abnormalities in patients with chronic male accessory gland infection (MAGI) who failed to respond to antibacterial treatment. Methods: We selected 67 consecutive patients with MAGI and persistently elevated bacteriospermia (≥ 10^6 colony forming units [CFU]/mL) after three antibiotic courses. Fourteen infertile patients with initial chronic microbial (≥ 10^6 CFU/mL) MAGI who responded to antibacterial treatment (〈 10^3 CFU/mL) served as a control group. All patients and controls underwent transrectal ultrasonography (TRUS) scans and semen analysis. Patients with low seminal plasma volume (〈 1.5 mL) underwent both preejaculatory and post-ejaculatory TRUS examination. Results: TRUS revealed multiple abnormalities indicative of: (i) bilaterally extended prostato-vesiculitis (group A: 52 cases, 77.6%) (nine of these patients also had micro-emphysematous prostate abscess); and (ii) prostato-vesiculitis with unilateral or bilateral sub-obstruction of the ejaculatory ducts (group B: 15 cases, 22.4%). Mean sperm concentration, total sperm number, ejaculate volume and pH value were significantly higher in group A than in group B. In addition, sperm forward motility and the percentage of normal forms were significantly worse than in controls, whereas leukocyte concentration was significantly higher in group A. Group B patients had all sperm parameters, but their pH values, significantly different from those of controls. Conclusion: Although antibiotic therapy is considered suitable when microbial MAGI is suspected, it is impossible to account for a poor response to antibiotics merely on the basis of conventional criteria (clinical history, physical and ejaculate signs). Thus, TRUS may be helpful in the follow-up of these patients. 展开更多

Objective:To determine the prevalence of bacteriospermia,the bacterial load,and the potential factors associated with bacterial contamination in boar semen collected by local smallholder artificial insemination operators.Methods:Fifteen individual raw semen samples were collected from locally available artificial insemination boars owned by different smallholder boar operators within the 5th district of Leyte,Philippines and were subjected to standard bacteriological culture and identification,including a survey of potentially associated factors.Prevalence and bacterial count were determined accordingly,while boar characteristics and collection practices were clustered following agglomerative hierarchical clustering technique.Results:One hundred percent contamination with a bacterial count of(2.01±0.38)×10^(3) CFU/mL was observed.At least 73.33%of the samples were positive for Bacillus spp.,while other identified isolates included Enterobacter spp.,Staphylococcus spp.,E.coli,Pseudomonas spp.,Citrobacter spp.,and Klebsiella spp.Conclusions:Despite the high prevalence of bacteriospermia,the bacterial count is low.Nevertheless,on-farm practices on boar health and management,semen collection,and sanitation as well as the enhancement of basic protocols to control contamination should be conscientiously considered in smallholder artificial insemination operation. 展开更多

Tracing of Antibiotic-Resistant Bacteria Isolated from Semen of Iraqi Males with Primary Infertility 被引量:1

Objectives: The main objective of this study was to investigate the bacterial infections of seminal fluid in males with primary infertility. The patients were referred to and/or visiting the Urology Department in Salah Al-Deen Teaching Hospital. Methods: The present study was carried out from May 2017 to June 2018 and the number of the patients group was 60. The patients aged between 20 to 50 years. Semen was collected from infertile men of a couple that female failed to become pregnant after one year of regular and unprotected intercourse of marriage and submitted for seminal fluid analysis for the bacteriological analyses. The control group consisted of 50 fertile male who were selected randomly from Tikrit City during the period of this study. Semen culture was done for all the patients and the control group. Results: There were 30 patients who suffered from bacteriospermia and 16 of them (53.3%) were infected with gram positive organisms, while only 14 (46.7%) patients were infected with gram negative organisms. The most common organism was Staphylococcus aureus which was isolated from bacteriospermia. Escherichia coli, Enterococcus faecalis, Klebsiella pneumoniae, S. saprophyticus, Proteus mirabilis and Niesseria gonorrhoae were also isolated and the frequencies of isolation were 23.3% (7), 13.3% (4), 10.0% (3), 10.0% (3), 6.6% (2), and 6.6% (2) respectively. The present study showed many antibiotics were not active against gram positive bacteria isolated e.g. the resistance frequenciy to rifampcin, vancomycin and clindamycin was 48%, 44% and 40% respectively. Conclusions: The gram positive organisms were more prevalent than gram negative organisms in patients with bacteriospermia and the isolationfrequencies of two groups were 16 (53.3%) and 14 (46.7%) respectively. The most effective antimicrobial agents were amikacin, ciprofloxacin and levofloxacin and the least effective one was ceftazidime, whereas moxifloxacin, norfloxacin, gentamicin, azithromycin, moxifloxacin, norfloxacin, cefotaxime, and ceftriaxone were effective at different levels. 展开更多

The semen quality of 479 men in infertile marriages, who attended the infertility clinic of OOUTH, Sagamu between May 2004 and April 2009 were analyzed. The semen samples were physically examined, microscopically analyzed and aerobically cultured. The result showed that out of 51.5% oligospermic cases recorded, 39.8% could be classified severe. Cryptozoospermia were diagnosed in 72 (15%) cases. Bacteriospermia and Necrozoospermia were found in 149 and 6 cases respectively. The predominant organism isolated was Staphylococcus aureus (50.32%) while Neisseria gonorrhoea was isolated in only 2 cases. Highest infection of 62.4% was noticed among the group with 1% - 19% motility. From the studied population, 8.8% were azoospermic. The commonest morphological defects were pin and ragged heads (48%). It is therefore obvious that the assessment of semen quality remains one of the important steps in infertility interventions. 展开更多
