Diagnostic dilemma of avoidant/restrictive food intake disorder after bariatric surgery:A case report and review of literature

BACKGROUND Bariatric surgery is an effective treatment for severe obesity but is associated with an increased risk for development of eating disorders.Indeed,numerous maladaptive eating behaviors and eating disorders have been described following bariatric surgery.However,the differentiation of pathologic eating patterns from expected dietary changes following bariatric surgery can sometimes be difficult to discern.CASE SUMMARY A female in her early 40s presented for medical stabilization of severe protein calorie malnutrition after losing 52.3 kg over the last six months after Roux-en-Y gastric bypass,with subsequent development of cyclic nausea and vomiting.Fear of these aversive physical symptoms led to further restriction of nutritional intake and weight loss.The patient was diagnosed with avoidant/restrictive food intake disorder,which has not been previously reported after bariatric surgery.CONCLUSION Improvement in the diagnostic nomenclature for feeding and eating disorders is warranted for patients who have undergone bariatric surgery. 展开更多

The clinical outcomes of adolescents with avoidant/restrictive food intake disorder(ARFID)remain unclear.Furthermore,no report has compared the characteristics of ARFID and restricting-type anorexia nervosa(R-AN)in elementary-school students on total parenteral nutrition(TPN).This study retrospectively reviewed inpatients diagnosed with ARFID or R-AN between 2005 and 2019.Patients with ARFID(two boys and seven girls)and R-AN(13 girls)were hospitalized because of rapid physical deterioration,and nutrition therapy was continued without withdrawal.The ARFID group exhibited significantly lower body weights at admission than the R-AN group and gained an average of 6.5 kg during hospitalization;furthermore,the monthly weight gain during hospitalization was significantly higher,and no relapse was observed.Early physical improvement in ARFID resulted in good recovery.In conclusion,TPN can be easily introduced to patients with ARFID,in whom aversive eating is a concern,and is a suitable treatment for ARFID. 展开更多

Background We reported a case of cervical invasive vagus nerve stimulation(iVNS)treatment for avoidant/restrictive food intake disorder(ARFID)in a patient with severe anxiety and depression.This patient was even given a critical illness notice during his hospitalization and all treatment efforts were failed.Objective We aimed to verfiy the effectiveness of iVNS in a patient with ARFID.Methods We first attempted to perform cervical iVNS in this case and then observed the changes in clinical scores.We also analyzed the alterations in brain magnetic resonance imaging characteristics before and after iVNS using multi-modal neuroimagings.Results After 18 days of iVNS(from 1 to 19 July 2023),the patient's clinical symptoms improved significantly and he rapidly gained 5 kg in weight.The brain functional characteristics of this patient tended toward those of the normal group.Functional connectivities of the medial of orbitalis prefrontal cortex returned to the normal range after iVNS.Conclusion This is a precedent for performing cervical iVNS in an ARFID patient.Brain neural activity can be modulated through iVNS.The observed improvements in clinical scores and positive changes in brain function validated the effectiveness of iVNS.This case study provides evidence that this intervention technique could be used to reduce the burden on more similar ARFID patients. 展开更多

This manuscript critically evaluates the randomized controlled trial(RCT)conducted by Phiri et al,which assessed the effectiveness of virtual reality(VR)training for psychiatric staff in reducing restrictive practices(RPs).Specifically,this RCT investigated the impact of VR on the handling of aggressive patients by psychiatric staff compared to traditional training methods.Despite significant reductions in perceived discrimination in the VR group,there were no major improvements in self-efficacy or anxiety levels.The system usability scale rated the VR platform highly,but it did not consistently outperform traditional training methods.Indeed,the study shows the potential for VR to reduce RPs,although fluctuations in RP rates suggest that external factors,such as staff turnover,influenced the outcomes.This manuscript evaluates the study’s methodology,results,and broader implications for mental health training.Additionally,it highlights the need for more comprehensive research to establish VR as a standard tool for psychiatric staff education,focusing on patient care outcomes and real-world applicability.Finally,this study explores future research di-rections,technological improvements,and the potential impact of policies that could enhance the integration of VR in clinical training. 展开更多

This paper presents an intelligent patrol and security robot integrating 2D LiDAR and RGB-D vision sensors to achieve semantic simultaneous localization and mapping(SLAM),real-time object recognition,and dynamic obstacle avoidance.The system employs the YOLOv7 deep-learning framework for semantic detection and SLAM for localization and mapping,fusing geometric and visual data to build a high-fidelity 2D semantic map.This map enables the robot to identify and project object information for improved situational awareness.Experimental results show that object recognition reached 95.4%mAP@0.5.Semantic completeness increased from 68.7%(single view)to 94.1%(multi-view)with an average position error of 3.1 cm.During navigation,the robot achieved 98.0%reliability,avoided moving obstacles in 90.0%of encounters,and replanned paths in 0.42 s on average.The integration of LiDAR-based SLAMwith deep-learning–driven semantic perception establishes a robust foundation for intelligent,adaptive,and safe robotic navigation in dynamic environments. 展开更多

DFCOA:Distributed Formation Control and Obstacle Avoidance for Multi-UGV Systems

Researchers are increasingly focused on enabling groups of multiple unmanned vehicles to operate cohesively in complex,real-world environments,where coordinated formation control and obstacle avoidance are essential for executing sophisticated collective tasks.This paper presents a Distributed Formation Control and Obstacle Avoidance(DFCOA)framework for multi-unmanned ground vehicles(UGV).DFCOA integrates a virtual leader structure for global guidance,an improved A^(*)path planning algorithm with an advanced cost function for efficient path planning,and a repulsive-force-based improved vector field histogram star(VFH^(*))technique for collision avoidance.The virtual leader generates a reference trajectory while enabling distributed execution;the improved A^(*)algorithm reduces planning time and number of nodes to determine the shortest path from the starting position to the goal;and the improved VFH^(*)uses 2D LiDAR data with inter-agent repulsive force to simultaneously avoid collision with obstacles and maintain safe inter-vehicle distances.The formation stability of the proposed DFCOA reaches 95.8%and 94.6%in two scenarios,with root mean square(RMS)centroid errors of 0.9516 and 1.0008 m,respectively.Velocity tracking is precise(velocity centroid error RMS of 0.2699 and 0.1700 m/s),and linear velocities closely match the desired 0.3 m/s.Safety metrics showed average collision risks of 0.7773 and 0.5143,with minimum inter-vehicle distances of 0.4702 and 0.8763 m,confirming collision-free navigation of four UGVs.DFCOA outperforms conventional methods in formation stability,path efficiency,and scalability,proving its suitability for decentralized multi-UGV applications. 展开更多

To address the critical challenge of risk perception and assessment for autonomous vehicles in dynamic interactive envi-ronments,this study proposes a semi-supervised spatiotemporal interaction risk cognition network with attention mecha-nism(SS-SIRCN),inspired by the behavioral adaptation patterns of biological groups under external threats.First,by thoroughly analyzing the dynamic interaction characteristics exhibited by typical biological collectives when exposed to risk,the study reveals the underlying patterns of trajectory changes influenced by external danger.Then,an attention-based spatiotemporal risk cognition network is designed to establish a mapping between driving behavior features and potential driving risks.Finally,a semi-supervised learning framework is employed to enable risk assessment for autono-mous vehicles using only a small amount of labeled data.Experimental results on real-world vehicle trajectory datasets demonstrate that the proposed method achieves a risk prediction accuracy of 90.76%,outperforming other baseline models in performance. 展开更多

Legged robots have considerable potential for traversing unstructured situations;nonetheless,their inflexible frameworks often constrain adaptability and obstacle negotiation.The study article presents a revolutionary Soft Tri-Legged Robot(STLR)that improves movement and obstacle-avoidance skills by using a bio-inspired pneumatic artificial muscle(Bubble Artificial Muscles)and a bio-inspired tactile sensor(TacTip).The STLR is activated by BAMs,which are flexible,pneu-matic-driven actuators that provide fine control over forward,backward,and steering movements.Obstacle identification and avoidance are facilitated by the TacTip sensor,which delivers tactile input for traversing unstructured terrains.We delineate the mechanical features of the BAMs,assess the functionality of the robot's legs,and elaborate on the incorpora-tion of the tactile sensing system.Experimental results demonstrate that the STLR can effectively achieve multi-directional flexible movement and obstacle avoidance through a cross-modal perception-actuation mechanism.This study highlights the promise of soft robotics for search and rescue,medical aid,and autonomous exploration,while delineating difficulties and opportunities for future improvements in functionality and efficiency. 展开更多

Deep reinforcement learning-based adaptive collision avoidance method for UAV in joint operational airspace

As joint operations have become a key trend in modern military development,unmanned aerial vehicles(UAVs)play an increasingly important role in enhancing the intelligence and responsiveness of combat systems.However,the heterogeneity of aircraft,partial observability,and dynamic uncertainty in operational airspace pose significant challenges to autonomous collision avoidance using traditional methods.To address these issues,this paper proposes an adaptive collision avoidance approach for UAVs based on deep reinforcement learning.First,a unified uncertainty model incorporating dynamic wind fields is constructed to capture the complexity of joint operational environments.Then,to effectively handle the heterogeneity between manned and unmanned aircraft and the limitations of dynamic observations,a sector-based partial observation mechanism is designed.A Dynamic Threat Prioritization Assessment algorithm is also proposed to evaluate potential collision threats from multiple dimensions,including time to closest approach,minimum separation distance,and aircraft type.Furthermore,a Hierarchical Prioritized Experience Replay(HPER)mechanism is introduced,which classifies experience samples into high,medium,and low priority levels to preferentially sample critical experiences,thereby improving learning efficiency and accelerating policy convergence.Simulation results show that the proposed HPER-D3QN algorithm outperforms existing methods in terms of learning speed,environmental adaptability,and robustness,significantly enhancing collision avoidance performance and convergence rate.Finally,transfer experiments on a high-fidelity battlefield airspace simulation platform validate the proposed method's deployment potential and practical applicability in complex,real-world joint operational scenarios. 展开更多

Previous studies have confirmed that both honeybee and Drosophila are capable of learning and memory. This study aimed to investigate whether the house fly (Aldrichina grahami), with strong instincts to adapt their living environment, have the learning ability to associate odor stimulus to avoid electric shock in free flying state using a device developed by the authors. The result showed the learning ability ofA. grahami at the electric shock voltages of 5 V, 25 V and 45 V AC. When 60 V was used, the flies were frequently injured. Our results indicate that A. grahami is a good model to study the neural mechanism of learning and memory. The paradigm in this study has some advantages that can be used in future studies of free insects. 展开更多

Objective To identify the pathogenic variant responsible for restrictive cardiomyopathy (RCM) in aChinese family.Methods Next generation sequencing was used for detecting the mutation and results verified bysequencing. We used restriction enzyme digestion to test the mutation in the family members and 200 unrelatednormal subjects without any cardiac inherited diseases when the mutation was identified.Results Five individuals died from cardiac diseases, two of whom suffered from sudden cardiacdeath. Two individuals have suffered from chronic cardiac disorders. Mutation analysis revealed a novelmissense mutation in exon 7 of troponin I type 3 (TNNI3), resulting in substitution of serine (S) withproline (P) at amino acid position 150, which cosegregated with the disease in the family, which is predictedto be probably damaging using PolyPhen-2. The mutation was not detected in the 200 unrelated subjectswe tested.Conclusion Using next generation sequencing, which has very recently been shown to be successfulin identifying novel causative mutations of rare Mendelian disorders, we found a novel mutation of TNNI3 in aChinese family with RCM. 展开更多

The restrictive ecological factors, including causing ecological factors, such as COD/SO 4 2- ratio and sulfate loading rate (Ns), and following ecological factors, such as pH value, oxidation reduction potential (ORP) and alkalinity (ALK) have significant effect on the ability and stability of acidogenic de sulfate bio reactor. Continuous flow and batch test were carried out to investigate the quantification and control of COD/SO 4 2- ratio, Ns, pH value, ORP and ALK in acidogenic de sulfate bioreactor supplied with molasses wastewater as sole organic carbon source and sodium sulfate as electron donor. It was demonstrated that In order to maintain high sulfate removal rate (SRR) of 80% to 90%, the restrictive factors should meet all the requirement as follows: k COD/ SO 4 2-  ratio≥2.0, Ns≤7.5 kg (m 3·d) -1 ,pH=5.7~6.2,ORP=-320~-420 mV and  ALK= 1 500~2 000 mg/L. 展开更多

Out of the three major categories of cardiomyopathies (hypertrophic, dilated and restrictive), restrictive cardiomyopathy (RCM) is less common and also the least studied. However, the prognosis for RCM is poor as some patients dying in their childhood. The molecular mechanisms behind the disease development and progression are not very clear and the treatment of RCM is very difficult and often ineffective. In this article, we reviewed the recent progress in RCM research from the clinical studies and the translational studies done on diseased transgenic animal models. This will help for a better understanding of the mechanisms underlying the etiology and development of RCM and for the design of better treatments for the disease. 展开更多

In order to identify the optimum period of spring water-restrictive irrigation for winter wheat(Triticum aestivum L.)in the Haihe Plain,China and elucidate its effects on flag leaf senescence and yield formation,field experiments were conducted at the Xinji Experimental Station of Hebei Agricultural University from 2016 to 2019 by using different irrigation regimes in spring,including the conventional regime involving two irrigation periods(control(CK),the 3-leaf unfolding stage and the anthesis stage)and a series of single,restrictive irrigation regimes(SRI)comprising irrigation at the 3-leaf unfolding stage(3 LI),4 LI,5 LI,and 6 LI.There are five major findings:(1)The senescence(determined by the green leaf area,GLA)in the 4 LI treatment occurred moderately earlier than that in CK,showed no significant difference with that in 5 LI and 6 LI,and occurred significantly later than that in 3 LI.(2)Compared with other SRI treatments,the GLA value and photosynthetic rate in 4 LI were 14.82 and 20.1%higher,respectively.Microstructural analysis of flag leaf also revealed that the mesophyll cells and chloroplasts were irregularly arranged under drought stress in 3 LI and 6 LI;however,drought stress had minimal negative effects on the microstructure in 4 LI and 5 LI.(3)Postponed irrigation in spring could significantly increase superoxide dismutase(SOD)and catalase(CAT)activities in the early stage of grain filling;however,these activities would subsequently decrease.Among the four SRI treatments,the overall enzyme activities were the highest in 4 LI,and the combined malondialdehyde(MDA)content in flag leaves in 4 LI and 5 LI was 14.5%lower on average than that in 3 LI and 6 LI.(4)The soluble sugar(SS)and proline(Pro)contents in 4 LI were the highest among the four SRI treatments;however,they were lower than those in CK.The abscisic acid(ABA)hormone content in 4 LI and 5 LI was lower than that in 3 LI and 6 LI,respectively,suggesting a smaller drought stress effect in 4 LI and 5 LI.(5)In two growing seasons,there was a larger number of spikes per unit area in 4 LI(i.e.,13.4%higher than that in 5 LI and 6 LI)and the 1000-grain weight in 4 LI was the highest among the four SRI treatments(i.e.,6.0%higher than that in the other three SRI treatments).Therefore,a single restrictive irrigation regime at the 4-leaf unfolding stage is recommended to be effective in slowing down the senescence process of flag leaves and achieving high yield. 展开更多

The compare of experimental results and computer simulation was also discussed. BACKGROUND Restrictive practices(RPs)are defined by measures linked to physical and chemical restraints to reduce the movement or control behaviours during any emergency.Seclusion is an equal part of RPs intended to isolate and reduce the sensory stimulation to safeguard the patient and those within the vicinity.Using interventions by way of virtual reality(VR)could assist with reducing the need for RPs as it could help reduce anxiety or agitation by way of placing users into realistic and immersive environments.This could also aid staff to and change current RPs.AIM To assess the feasibility and effectiveness of using a VR platform to provide reduction in RP training.METHODS A randomised controlled feasibility study,accompanied by evaluations at 1 month and 6 months,was conducted within inpatient psychiatric wards at Southern Health National Health Service Foundation Trust,United Kingdom.Virti VR scenarios were used on VR headsets to provide training on reducing RPs in 3 inpatient psychiatric wards.Outcome measures included general self-efficacy scale,generalised anxiety disorder assessment 7(GAD-7),Burnout Assessment Tool 12,the Everyday Discrimination Scale,and the Compassionate Engagement and Action Scale.RESULTS Findings revealed statistically significant differences between the VR and treatment as usual groups,in the Everyday Discrimination Scale items Q8 and Q9:P=0.023 and P=0.040 respectively,indicating higher levels of perceived discrimination in the VR group.There were no significant differences between groups in terms of general self-efficacy,generalised anxiety disorder assessment 9,and Burnout Assessment Tool 12 scores.A significant difference was observed within the VR group for compassionate engagement from others(P=0.005)over time.Most respondents recorded System Usability Scale scores above 70,with an average score of 71.79.There was a significant reduction in rates of RPs in the VR group vs treatment as usual group with a fluctuating variability observed in the VR group likely due to external factors not captured in the study.CONCLUSION Ongoing advancement of VR technology enables the possibility of creating scenarios and simulations tailored to healthcare environments that empower staff by providing more comprehensive and effective training for handling situations. 展开更多

Network virtualization(NV) is widely considered as a key component of the future network and promises to allow multiple virtual networks(VNs) with different protocols to coexist on a shared substrate network(SN). One main challenge in NV is virtual network embedding(VNE). VNE is a NPhard problem. Previous VNE algorithms in the literature are mostly heuristic, while the remaining algorithms are exact. Heuristic algorithms aim to find a feasible embedding of each VN, not optimal or sub-optimal, in polynomial time. Though presenting the optimal or sub-optimal embedding per VN, exact algorithms are too time-consuming in smallscaled networks, not to mention moderately sized networks. To make a trade-off between the heuristic and the exact, this paper presents an effective algorithm, labeled as VNE-RSOT(Restrictive Selection and Optimization Theory), to solve the VNE problem. The VNERSOT can embed virtual nodes and links per VN simultaneously. The restrictive selection contributes to selecting candidate substrate nodes and paths and largely cuts down on the number of integer variables, used in the following optimization theory approach. The VNE-RSOT fights to minimize substrate resource consumption and accommodates more VNs. To highlight the efficiency of VNERSOT, a simulation against typical and stateof-art heuristic algorithms and a pure exact algorithm is made. Numerical results reveal that virtual network request(VNR) acceptance ratio of VNE-RSOT is, at least, 10% higher than the best-behaved heuristic. Other metrics, such as the execution time, are also plotted to emphasize and highlight the efficiency of VNE-RSOT. 展开更多
