Methods Fifty athletes with FAI wer...Objective To observe the efficacy differences between electroacupuncture (EA) and physiotherapy (PT) on the proprioception of athletes with functional ankle instability (FAI). Methods Fifty athletes with FAI were randomly divided into an EA group and a PT group. The EA group was treated with EA at Jiexi (解溪 ST 41), Kunlun (昆仑 BL 60), Qjuxu (丘墟 GB 40) and Ashi points, and the PT group was treated with low frequency electrical stimulation and infrared radiation at medial malleolus and lateral malleolus, thrice each week for consecutive 8 weeks. The joint position sense: active (JPSA), joint position sense: passive (JPSP) and Kinaesthesia (KT) were assessed at the ankle by use of Biodex System isokinetic dynamometer to test the proprioception before and after the treatment. Results In the EA group, JPSA of 11.09°±3.1° and JPSP of 9.67°±2.8° before the treatment were reduced to 9.14°±4.0° and 6.89°±3.3° respectively after the treatment, and there were statistically significant differences in comparison between pre- and post-treatment (all P〈0.05). Comparison between the EA group and the PT group, there were significant differences in JPSA and JPSP (all P〈0.05), no significant difference in KT (P〉0.05). There were no significant differences in the indices of JPSA, JPSP and KT in the PT group after the treatment than those before the treatment (all P〉0.05). Conclusion EA can effectively improve the proprioception of athletes with FAI and achieve a better efficacy as compared with the conventional physiotherapy.展开更多
BACKGROUND Laparoscopic surgery has emerged as the preferred treatment for acute appendicitis,a common acute abdominal condition.Despite surgical advances,postoperative gastrointestinal dysfunction and delayed recover...BACKGROUND Laparoscopic surgery has emerged as the preferred treatment for acute appendicitis,a common acute abdominal condition.Despite surgical advances,postoperative gastrointestinal dysfunction and delayed recovery continue to negatively impact rehabilitation outcomes and patient quality of life.Laser acupuncture represents a promising emerging complementary therapy with potential benefits for promoting functional recovery.AIM To evaluate the clinical efficacy of laser acupuncture in enhancing gastrointestinal function recovery following laparoscopic appendectomy and to investigate its potential applications in postoperative rehabilitation.METHODS This retrospective controlled clinical study selected 120 acute appendicitis patients who underwent laparoscopic appendectomy from January 2022 to December 2023 at a tertiary hospital.Patients were randomly divided into two groups:Routine treatment group(n=60)and laser acupuncture intervention group(n=60).There were no significant statistical differences in baseline characteristics(age,gender,body mass index,appendicitis severity)between the two groups.The routine treatment group received standard postoperative care,while the laser acupuncture intervention group received additional laser acupuncture treatment on top of standard care.Key observation indicators included time to first exhaust,time to first bowel movement,time to gastrointestinal function recovery,and postoperative complication rates.RESULTS Patients in the laser acupuncture intervention group showed significantly faster gastrointestinal function recovery compared to the routine treatment group.Specifically,time to first exhaust was significantly shortened(intervention group:12.5±2.3 hours vs control group:18.7±3.1 hours,P<0.05),time to first bowel movement was earlier(intervention group:36.4±4.6 hours vs control group:48.2±5.7 hours,P<0.05),postoperative complication rate was significantly reduced(intervention group:8.3%vs control group:20.0%,P<0.05),postoperative pain score was significantly lower(intervention group:3.2±1.1 vs control group:4.7±1.5,P<0.05),and intestinal motility recovery was faster and more effective.CONCLUSION Laser acupuncture,as an auxiliary treatment method,can significantly promote gastrointestinal function recovery in patients after laparoscopic appendectomy,reduce complications,and improve postoperative comfort.This technique has advantages such as minimal invasiveness,rapid recovery,and few side effects,making it worthy of further clinical promotion and application.展开更多
BACKGROUND Psychological comorbidities,such as anxiety and depression,in patients with chronic ankle instability(CAI)may impede ankle function improvement,although the precise nature of this association warrants furth...BACKGROUND Psychological comorbidities,such as anxiety and depression,in patients with chronic ankle instability(CAI)may impede ankle function improvement,although the precise nature of this association warrants further investigation.AIM To analyze the correlation of anxiety and depression with ankle function in patients with CAI and discussing the risk factors.METHODS This study included 116 patients with CAI,who were admitted to our hospital from July 2022 to July 2024.Anxiety and depression states of patients were assessed with the self-rating anxiety scale(SAS)and self-rating depression scale(SDS),respectively,and their ankle joint function was assessed with the anklehindfoot function score of the American Orthopedic Foot and Ankle Society.Further,the ankle function of patients with CAI with different anxiety and depression states was discussed.Furthermore,the Pearson correlation coefficient was used to analyze the correlation of anxiety and depression with ankle joint function in such patients.Univariate and multivariate analyses were conducted to investigate the factors affecting ankle joint function in patients with CAI.RESULTS Among the 116 patients with CAI,97,13,5,and 1 cases demonstrated none,mild,moderate,and severe anxiety,whereas 95,15,6,and 0 cases showed none,mild,moderate,and severe depression,respectively.The average ankle joint function score was 74.82±6.93 points.The ankle joint function in patients with CAI presented a significant downward tendency as the degree of anxiety and depression increased.Correlation analysis revealed that both the SAS and SDS scores of patients with CAI were significantly negatively correlated with the ankle joint function score.Univariate and multivariate analyses indicated that the risk factors affecting patients’ankle joint function included early functional rehabilitation,visual analog scale,and SDS.CONCLUSION A substantial number of patients with CAI suffer from anxiety and depression,and these negative emotions,to a certain extent,harm the smooth rehabilitation of ankle joint function.展开更多

BACKGROUND This study seeks to systematically assess the effects of warm acupuncture combined with acupoint massage on the recovery of gastrointestinal function following gastric cancer surgery.AIM To evaluate the imp...BACKGROUND This study seeks to systematically assess the effects of warm acupuncture combined with acupoint massage on the recovery of gastrointestinal function following gastric cancer surgery.AIM To evaluate the impact of warm acupuncture combined with acupoint massage on postoperative gastrointestinal function recovery after gastric cancer surgery,based on a systematic review and meta-analysis.METHODS A comprehensive search was conducted across multiple databases,including PubMed,Cochrane Library,EMBASE,Web of Science,CNKI,Wanfang,and VIP,for relevant studies published up to January 2025.Meta-analyses were carried out using RevMan 5.4,with results presented as standardized mean difference(SMD)or odds ratio with 95%confidence interval(CI).Study heterogeneity was evaluated using the I2 statistic,and sensitivity analyses were performed to assess the stability of the findings.RESULTS A total of 8 randomized controlled trials involving 694 patients were included.The meta-analysis showed that warm acupuncture combined with acupoint massage significantly improved postoperative gastrointestinal function by reducing the time to first flatus(SMD=-2.14,95%CI:-3.14 to-1.14,P<0.0001),time to first bowel movement(SMD=-2.43,95%CI:-3.52 to-1.34,P<0.0001),time to bowel sounds recovery(SMD=-3.15,95%CI:-4.50 to-1.80,P<0.00001),and time to initiate nasogastric or jejunal feeding(SMD=-1.31,95%CI:-2.44 to-0.18,P=0.02).CONCLUSION The combination of warm acupuncture and acupoint massage markedly enhances gastrointestinal recovery following surgery.展开更多
OBJECTIVE:To investigate the effects of acupuncture combined with upper limb rehabilitation robot on neural remodeling and functional recovery in post-stroke patients.METHODS:There were 50 stroke patients were randoml...OBJECTIVE:To investigate the effects of acupuncture combined with upper limb rehabilitation robot on neural remodeling and functional recovery in post-stroke patients.METHODS:There were 50 stroke patients were randomly divided into an experimental group(acupuncture combined with upper limb rehabilitation robot assisted training)and a control group(upper limb rehabilitation robot assisted training).Various assessments were conducted to compare the effects of the two treatments on neural remodeling and functional recovery.Functional near-infrared spectroscopy technology was used to assess the effects of different treatments on neural plasticity and their impact on upper limb function and activities of daily living.RESULTS:The experimental group showed significantly higher concentrations of oxygenated hemoglobin and total hemoglobin in specific brain regions compared to the control group(P<0.05).Additionally,the experimental group had significantly lower concentrations of deoxygenated hemoglobin(P<0.05).After treatment,both groups showed improvements in various measures,but the experimental group had significantly greater improvements(P<0.05).CONCLUSION:Acupuncture combined with upper limb rehabilitation robot can effectively improve upper limb function and neural remodeling in stroke patients.This study supports the integration of Traditional Chinese and Western Medicine in improving limb dysfunction poststroke.展开更多
The recovery of motor function in spastic hemiplegia following stroke is closely associated with cortical reorganisation.This study aimed to investigate the effects of warm acupuncture combined with rehabilitation tra...The recovery of motor function in spastic hemiplegia following stroke is closely associated with cortical reorganisation.This study aimed to investigate the effects of warm acupuncture combined with rehabilitation training on cortical activity and upper limb function using functional near-infrared spectroscopy.Ninety patients were randomly assigned to a warm acupuncture group,a rehabilitation training group,or a combined intervention group.After four weeks of intervention,clinical scores and changes in oxyhaemoglobin concentration in the affected motor cortex were assessed.Results demonstrated that the combined intervention group showed significantly greater improvement in Fugl-Meyer scores,reduced muscle tone,and enhanced activation in the affected primary motor cortex compared to the single intervention groups.Furthermore,functional improvement was significantly correlated with cortical activation changes.These findings indicate a synergistic effect between warm acupuncture and rehabilitation training,potentially mediated through promoting functional reorganisation of the affected cerebral cortex.This provides neuroimaging evidence supporting integrated Chinese and Western medicine rehabilitation protocols.展开更多
BACKGROUND Gastrointestinal(GI)dysfunction is common after laparoscopic radical gastrectomy for gastric cancer and affects rehabilitation.While conventional treatments can alleviate symptoms to a certain extent,they o...BACKGROUND Gastrointestinal(GI)dysfunction is common after laparoscopic radical gastrectomy for gastric cancer and affects rehabilitation.While conventional treatments can alleviate symptoms to a certain extent,they often fail to fully address the issue of insufficient GI motility.The GI motility therapeutic apparatus promotes dynamic recovery by simulating GI electric waves,whereas acupuncture regulates zang-fu qi movement,both offering effective interventions.However,there are few clinical studies investigating the combined use of GI motility therapy and acupuncture to promote GI function recovery in patients after GI laparoscopic radical surgery.AIM To evaluate the effects of combining GI motility therapy devices with acupuncture on GI function in patients undergoing radical laparoscopic surgery.METHODS This retrospective study included 196 patients who underwent radical GI endoscopic surgery at the Shanghai Yangzhi Rehabilitation Hospital(Shanghai Sunshine Rehabilitation Center),School of Medicine,Tongji University,from June 2022 to May 2024.Patients were classified into a normal group(conventional treatment,n=96)and an integrated group(conventional+GI motility therapy device+acupuncture,n=100).The effects on GI function,hormone levels preand post-treatment,GI symptoms,immune function,adverse reactions,and patient satisfaction in both groups were assessed.RESULTS Compared with the normal group,the integrated group demonstrated significantly better overall effectiveness(93.00%vs 84.3%;P<0.05)and shorter durations for first exhaust,feeding,defecation,and hospital stay(P<0.05).Post-treatment,the integrated group had lower gastrin and GI symptom rating scale scores and higher motilin,vasoactive intestinal peptide,and immune marker(CD3+,CD4+,CD4+/CD8+,and natural killer cells)levels(P<0.05).The integrated group,compared to the normal group,also reported fewer adverse reactions(5.00%vs 14.58%)and higher patient satisfaction(97.00%vs 84.38%),both statistically significant(P<0.05).CONCLUSION The combination of a GI motility therapy device and acupuncture promotes GI function recovery after radical gastrectomy,regulates GI hormones and immune function,and is safe and effective.展开更多

A total of 20 low-back and leg pain patients with lumbar disc herniation were treated with balanced acupuncture or body acupuncture. Central mechanisms of vaded acupunctures were compared using resting-state functional MRI. Patients from both groups received functional MRI before and after acupuncture. Functional connectivity in brain regions that were strongly associated with the bilatera amygdala was analyzed utilizing AFNI software. Visual analogue scale scores were greater in the balanced acupuncture group compared with the body acupuncture group. Function of the endogenous pain regulation network was enhanced in patients in the balanced acupuncture group, but was not changed in the body acupuncture group. This result indicates that the analgesic effects of body acupuncture do not work through the central nervous system. These data suggest that balanced acupuncture exerts analgesic effects on low-back and leg pain patients with lumbar disc herniation by regulating the function of the endogenous pain regulation network.展开更多

However, the central anal- gesic mechanism remains poorly understood. Studies have demonstrated that single stimulus of the verum acupuncture in healthy subjects can induce significant connectivity or activity changes in pain- related central networks compared with sham acupuncture. However, these findings are not indicative of the central analgesic mechanism of acupuncture at Shaoyang acupoints. Thus, we recruited 100 migraine sufferers and randomly assigned them into five groups: Shaoyang uncommon acupoint, Shaoyang common acupoint, Yangming uncommon acupoint, non-acupoint control, and blank control groups. Subjects were subjected to evaluation of curative effects and functional MRI prior to and after 10 and 20 acupuncture treatments. All subjects were diagnosed by physicians and enrolled following clinical physical examination. Subjects were observed during 1-4 weeks after inclusion. At the fifth week, the first clinical evaluation and resting functional MRI were conducted. The Shaoyang uncom- mon acupoint, Shaoyang common acupoint, Yangming uncommon acupoint, and non-acupoint control grousp then were treated with acupuncture, five times per week, 20 times in total over 4 weeks. The second and third clinical evaluations and resting functional MRI screenings were conducted following 10 and 20 acupuncture treatments. The blank control group was observed during the 5 to 8 week pe- riod, followed by clinical evaluation and resting functional MRI. The aim of this study was to examine changes in brain functional activity and central networks in subjects with migraine undergoing acu- puncture at Shaoyang uncommon acupoints. Objective To observe the clinical efficacy of acupuncture and moxibustion in treatment of functional amenorrhea by establishing artificial cycle.Method One hundred and twelve eligible patients with functional amenorrh...Objective To observe the clinical efficacy of acupuncture and moxibustion in treatment of functional amenorrhea by establishing artificial cycle.Method One hundred and twelve eligible patients with functional amenorrhea were divided into an acupuncture-moxibustion group(64 cases) and a western medication group(48 cases) by adopting random method.According to the pattern differentiation of amenorrhea of traditional Chinese medicine,the patients in the acupuncture-moxibustion group were classified into two types:amenorrhea due to blood depletion and amenorrhea due to stagnation of blood.For patients with amenorrhea due to blood depletion,Guanyuan(关元 CV4),Zhongji(中极CV 3),GuiTlai(归来ST 29),Geshu(膈俞 BL 17),Ganshu(肝俞BL 18),Pishu(脾俞 BL 20) and Shenshu(肾俞 BL 23) were selected.At first,BL 17,BL 18,BL 20 and BL 23 were needled;after deqi,reinforcing method was applied,and the needles were not retained.Then,warmingneedle moxibustion was applied at CV 4,CV 3 and ST 29,and the needles were retained for 30 min.For patients with amenorrhea due to stagnation of blood,CV 3,Qihai(气海CV 6),Xuehai(血海 SP 10),Sanyinjiao(三阴交 SP 6)and Xingjian(行间 LR 2) were selected,reducing method was applied,and the needles were retained for 30 min.The treatment was performed once a day,and treatment for 15 consecutive days were the 1st cycle.The 2nd and3 rd cycles started from the 5th day of menstruation(for the patients still not menstruating,the 2nd and 3rd cycles started from the 29 th and 57 th days since the 1st day of treatment),and the treatment lasted for 15 days.Three cycles were needed.In the western medicine group,estrogen-progestogen was taken orally for 21 days(one cycle) to establish a artificial cycle.The levels of FSH and LH were tested and compared on the 3rd day of menstruation before treatment and after the 3rd cycle of treatment,and the adverse effects were analyzed statistically.Follow-up visit was conducted for the patients after treatment for three cycles,and the recurrence rate was calculated.Result The levels of FSH and LH of patients with functional amenorrhea were enhanced significantly through establishing artificial cycle by acupuncture and moxibustion.There was no significant difference between the acupuncture-moxibustion group and the western medication group in effective rate(P〉0.05);but the adverse effect rate(1.49%) and recurrence rate(25.0%) of the acupuncture-moxibustion group were lower than that of the western medication group(with the adverse effect rate of 4.14%,and recurrence rate of 69.8%)(both P〈0.05).Conclusion The levels of FSH and LH of patients with functional amenorrhea can be enhanced significantly through establishing artificial cycle by acupuncture and moxibustion.The efficacy of acupuncture and moxibustion in treatment of functional amenorrhea by establishing artificial cycle is equivalent to that of oral administration of estrogen-progestogen in treatment of functional amenorrhea by establishing artificial cycle,but the adverse effect rate and recurrence rate of acupuncture and moxibustion group are lower.展开更多
Objective To compare the influences of acupuncture combined with rehabilitative training and simple rehabilitative training on neural functional recovery of stroke patients during recovery phase.Methods Sixty-seven st...Objective To compare the influences of acupuncture combined with rehabilitative training and simple rehabilitative training on neural functional recovery of stroke patients during recovery phase.Methods Sixty-seven stroke patients during recovery phase were randomly divided into an acupuncture combined with rehabilitative training group(group A) and a simple rehabilitative training group(group B) through random number table method,with 34 cases and 33 cases respectively;33 cases and 31 cases were respectively completed actually other than cases dropped out during the observation.Rehabilitation methods such as motion control and functional exercise training were primarily used for rehabilitative training,once daily for 45 min at a time,and 6 times a week;Baihui(百会 GV 20) and Fengchi(风池 GB 20) on the head and relevant acupoints on limbs were selected for acupuncture,once every other day,and 3 times a week.Both groups were treated for 8 weeks,National Institnte of Health Stroke Scale(NIHSS) was used to assess neurologic impairment,Fugl-Meyer scale was used to assess motor function,Barthel was used to assess activity of daily living,and comprehensive assessment was finally made to patients.Results NIH score,Fugl-Meyer scale scores of upper and lower limbs,Barthel score,and comprehensive assessment of group A after treatment were respectively:12.78±2.99,37.55±2.09 and25.67±3.93,63.76±4.65,11.33±1.98,superior to 16.06±4.57,28.71±4.07 and20.41±2.98,53.8214.35,15.18±1.72 of group B.Indices of both groups were significantly improved compared with those before treatment(P〈0.01,P〈0.05),but improvement of indices of group A was more obvious that that of group B after treatment subject to comparison(all P〈0.05).Conclusions Acupuncture combined with rehabilitative training has definite therapeutic effect on recovery of various functions such as neural functional recovery of stroke patients during recovery phase,obviously superior to simple rehabilitative training.展开更多
Objective To explore clinical efficacy and safety of acupuncture in treatment of functional constipation.Methods One hundred and twenty cases were randomly divided into an observation group and a control group,60 case...Objective To explore clinical efficacy and safety of acupuncture in treatment of functional constipation.Methods One hundred and twenty cases were randomly divided into an observation group and a control group,60 cases in each group.The patients in the observation group were treated with acupuncture at Tiānshū(天枢 ST 25),Shàngjùxū(上巨虚 ST 37),Zúsānlǐ(足三里 ST 36),Dàchángshū(大肠俞 BL 25),etc.,and the control group was treated with oral administration of Macrogol 4000 and Mosapride.Defecation frequency,stool property,constipation symptom score,accompanying symptom score,gastrointestinal transit time and adverse reaction were observed before treatment,at the end of treatment and 4 weeks after treatment in the two groups.Results Compared with those before treatment,defecation frequency was increased significantly at the end of treatment in the two groups,while stool property,constipation symptom score,accompanying syndrome score and gastrointestinal transit time all were decreased significantly(all P0.01),with no significant difference in these indexes between the two groups at the end of treatment (all P0.05).Compared with the end of treatment,the above-mentioned indexes did not significantly recur in the observation group 4 weeks after the treatment (all P0.05),but the therapeutic effects were unstable with decreased to some extent in the control group (all P0.05).No serious adverse reaction was found in the two groups.Conclusion In the patients with functional constipation,acupuncture can significantly increase defecation frequency,change stool property,alleviate constipation and accompanying symptoms,and shorten gastrointestinal transit time with better safety and tolerance,and the therapeutic effect is more stable than that of Macrogol 4000 and Mosapride.展开更多

ST 25), 6), Shemgjfuxu (上巨虚 ST 37), etc. (2)Zhongliao (中髎 BL 33), Xiadiao (下髎 BL 34), Dachangshu (大肠俞 BL 25), etc. Electroacupuncture was applied in combination at Zhongliao (中髎 BL 33), Xialiao (下髎 BL 34), Tianshu(天枢 ST 25) and Shamgjuxu (上巨虚 ST 37), once a day, 10 treatments made one session. By constipation diary of patients, frequency of going to stool, straining severity, time of once bowl evacuation, a sense of incomplete bowel emptying, stool quality, a sense of defecation and the Patient Assessment of Constipation Quality of Life questionnaire (PAC-QOL), as well as the changes before and after treatment were observed. Results After treatment, the apparent improvements were achieved in frequency of going to stool, straining severity, time of once bowl evacuation, a sense of incomplete bowel emptying, a sense of defecation and score of PAC-QOL as compared with those before treatment (all P〈0.01). The total effective rate was 67.7% (61/70). The acupuncture effieacies were various on CFC of different dynamic mechanisms. The efficacy on slow transit constipation (STC) was superior to that caused by spastic pelvic floor syndrome (SPFS-C) (P〈0.05), the efficacy on constipation caused by irritable bowel syndrome (IBS-C) was superior to that on either SPFS-C or constipation caused by relaxant pelvic floor syndrome (RPFS-C) (both P〈0.05). Of 52 eases in effective follow-up, 1 month after treatment, 3 eases were cured, 6 eases markedly effective, 23 eases effective, and 20 cases failed; 3 months after treatment, 3 eases were cured, 5 eases markedly effective, 16 cases effective and 28 cases failed. Conclusion Acupuncture has definite efficacy on CFC with definite etiology, loci and diagnostic classification. But, the efficacies are different due to various dynamic mechanisms. A further optimized treatment program is required to improve the efficacy on SPFS-C and RPFS-C.展开更多

In ischemic stroke subjects, functional MRI showed enhanced activation in Broadmann areas 5, 6, 7, 18, 19, 24, 32, the hypothalamic inferior lobe, the mamiilary body, and the ventral posterolateral nucleus of the left hemisphere, and Broadmann areas 4, 6, 7, 18, 19 and 32 of the right hemisphere, but attenuated activation of Broadmann area 13, the hypothalamic inferior lobe, the posterior lobe of the tonsil of cerebellum, and the culmen of the anterior lobe of hypophysis, in the left hemisphere and Broadmann area 13 in the right hemisphere. In ischemic stroke subjects, a number of deactivated brain areas were enhanced, including Broadmann areas 6, 11,20, 22, 37, and 47, the culmen of the anterior lobe of hypophysis, alae lingulae cerebella, and the posterior lobe of the tonsil of cerebellum of the left hemisphere, and Broadmann areas 8, 37, 45 and 47, the culmen of the anterior lobe of hypophysis, pars tuberalis adenohypophyseos, inferior border of lentiform nucleus, lateral globus pallidus, inferior temporal gyrus, and the parahippocampal gyrus of the right hemisphere. These subjects also exhibited attenuation of a number of deactivated brain areas, including Broadmann area 7. These data suggest that acupuncture at Waiguan specifically alters brain function in regions associated with sensation, vision, and motion in ischemic stroke patients. By contrast, in normal individuals, acupuncture at Waiguan generally activates brain areas associated with insomnia and other functions.展开更多
BACKGROUND Functional dyspepsia(FD)is a common digestive disease with limited therapeutic options.According to evidence-based clinical practice,acupuncture or electroacupuncture(EA)seems to be a promising therapy for ...BACKGROUND Functional dyspepsia(FD)is a common digestive disease with limited therapeutic options.According to evidence-based clinical practice,acupuncture or electroacupuncture(EA)seems to be a promising therapy for patients with FD.However,there is still a lack of systematic reviews that have analyzed current clinical trials for a better understanding of mechanisms involved in the ameliorating effect of acupuncture and EA on FD.AIM To evaluate the results and qualities of existing clinical evidence for researching the underlying mechanisms of acupuncture/EA in treating FD.METHODS A systematic search of the literature was performed to identify randomized controlled trials in which research on the mechanism of acupuncture or EA was conducted in FD patients.Databases searched included PubMed,EMBASE,Cochrane Library,and Web of Science.Data extraction and quality assessment were completed by two investigators independently and the results of quality evaluation were exported through Review Manager V5.3.RESULTS Eight studies were included in this review with a total of 17 items for detecting techniques for mechanistic research.Positive effects of acupuncture and EA were observed in regulating gastric motility,gastric accommodation,mental status,gastrointestinal hormones,and central and autonomic functions while improving dyspeptic symptoms and quality of life.CONCLUSION The key findings of this systematic review support the potential of acupuncture and EA in altering the heterogeneous pathophysiology in patients with FD.However,high-quality studies with well-planned designs are necessary to provide more credible evidence.展开更多
Objective:To review systematically the therapeutic effect and safety of acupuncture or electroacupuncture(EA)on functional dyspepsia(FD).Methods:Using computer,the randomized controlled trials(RCTs)of FD treated with ...Objective:To review systematically the therapeutic effect and safety of acupuncture or electroacupuncture(EA)on functional dyspepsia(FD).Methods:Using computer,the randomized controlled trials(RCTs)of FD treated with acupuncture or EA were retrieved from the databases,i.e.Cochrane Library,PubMed,EMbase and Web of science,from the date of database establishment to June 1,2020.Using the assessment tool of risk of bias in Cochrane Handbook for systematic reviews of interventions,5.1.0,the quality of the included RCTs were evaluated in methodology.Meta-analysis was performed by RevMan 5.3 software.Results:A total of 10 articles were included,with 1202 cases involved.In this study,the effectiveness of EA or acupuncture on FD was analyzed in comparison with the treatment with sham-acupuncture,sham-EA or western medication.The results indicated that compared with sham-acupuncture or shamEA,acupuncture or EA greatly relieved the clinical symptoms of FD(SMD=-3.03,95%Cl[-3.56,-2.50],P<0.00001)and obtained a higher clinical response rate(OR=5.09,95%CI[3.30,7.86],P<0.00001),but the difference was not significant statistically in gastric electrical activity(P>0.05).Compared with gastric prokinetics,acupuncture or EA more effectively reduced the symptom score(MD=-3.03,95%CI[-3.56,-2.50],P=0.02),but the differences were not significant in the aspects of the improvement of clinical effective rate(OR=1.33,95%CI[0.70,2.52],P=0.39),the promotion of gastric half-emptying(MD=0.23,95%Cl[-1.94,2.40],12=0%,P=0.83),the promotion of motilin secretion(SMD=-0.06,95%CI[-0.23,0.11],I^(2)=0%,P=0.50)and the increase of gastric electrical activity(MD=-0.01,95%Cl[-0.03,0.01],I^(2)=0%,P=0.41).The three RCT studies with adverse events reported indicated that the difference in the incidence of the adverse events was not significant statistically in comparison of acupuncture with sham-acupuncture or gastric prokinetic agent(P<0.05).Conclusion:Regarding the improvement in the symptoms of FD,the effect of EA or acupuncture is superior to the sham-acupuncture group or the western medication group.Simultaneously,the clinical response rate in the treatment with EA or acupuncture is higher than the sham-acupuncture group.During the treatment with EA,some adverse reactions happened in treatment of FD with acupuncture or western medication.Even though there are the limitations in quality and quantity of the included trials,EA and acupuncture still deserve to be promoted in treatment of FD.However,more studies of RCTs with largescale sample,high quality and multiple centers are required for a further verification of the therapeutic effect.展开更多
Objective:This study is conducted to investigate the effects of occupational therapy(OT) with or without combined acupuncture for upper limb pain and hand functions among children with spastic hemiplegic cerebral pals...Objective:This study is conducted to investigate the effects of occupational therapy(OT) with or without combined acupuncture for upper limb pain and hand functions among children with spastic hemiplegic cerebral palsy.Methods:A total of 90 patients who met the inclusion criteria were enrolled in a three-arm randomized,placebo-controlled trial,and were allocated to mono-occupational therapy(OT) group(patients=30),OT combined with real-acupuncture(ORA) group(patients=30),and OT combined with placeboacupuncture(OPA) group(patients=30),respectively.In addition to receiving the conventional OT program as the basic treatment for total 8 weeks,patients in ORA and OPA group also received corresponding acupuncture treatment 5 days per week for the first two weeks,3 days per week for the next two weeks,2 days per week for another two weeks,and 1 day per week for the last two weeks.Non-communicating children’s pain checklist-revised scale(NCCPC-R) was used for assessing the upper limb pain among patients.Squeeze dynamometry,modified Ashworth scale(MAS),manual muscle testing(MMT),JebsenTaylor hand function test(JTHFT) and box and block test(BBT) were used for assessing various aspects of upper limb and hand functions among patients.Meanwhile,adverse effects were monitored and recorded.Results:After 8-week treatment,NCCPC-R global scores witnessed a dramatic decline in ORA group(P<0.05),but not in either OPA(P> 0.05) or OT group(P> 0.05).Dramatic increases were identified in both squeeze dynamometry scores and MMT scores in all three groups(all P <0.05).Furthermore,the increases of ORA group were more significant than those of another two groups(both P <0.05).There were no significant changes in MAS global scores in each group(all P> 0.05).The BBT global scores of all three groups significantly increased(all P <0.05) after treatment.Among them,the increase of the ORA group was more than that of the other two groups(P <0.05).The 8-week treatment also had a significant impact on several sub-tests of JTFHT in each group.No significant adverse event was reported.Conclusion:ORA is a potential and promising alternative therapy for mitigating upper limb pain as well as improving both upper limb and hand functions among children with spastic hemiplegic cerebral palsy.展开更多

文摘Objective To observe the efficacy differences between electroacupuncture (EA) and physiotherapy (PT) on the proprioception of athletes with functional ankle instability (FAI). Methods Fifty athletes with FAI were randomly divided into an EA group and a PT group. The EA group was treated with EA at Jiexi (解溪 ST 41), Kunlun (昆仑 BL 60), Qjuxu (丘墟 GB 40) and Ashi points, and the PT group was treated with low frequency electrical stimulation and infrared radiation at medial malleolus and lateral malleolus, thrice each week for consecutive 8 weeks. The joint position sense: active (JPSA), joint position sense: passive (JPSP) and Kinaesthesia (KT) were assessed at the ankle by use of Biodex System isokinetic dynamometer to test the proprioception before and after the treatment. Results In the EA group, JPSA of 11.09°±3.1° and JPSP of 9.67°±2.8° before the treatment were reduced to 9.14°±4.0° and 6.89°±3.3° respectively after the treatment, and there were statistically significant differences in comparison between pre- and post-treatment (all P〈0.05). Comparison between the EA group and the PT group, there were significant differences in JPSA and JPSP (all P〈0.05), no significant difference in KT (P〉0.05). There were no significant differences in the indices of JPSA, JPSP and KT in the PT group after the treatment than those before the treatment (all P〉0.05). Conclusion EA can effectively improve the proprioception of athletes with FAI and achieve a better efficacy as compared with the conventional physiotherapy.
文摘BACKGROUND Laparoscopic surgery has emerged as the preferred treatment for acute appendicitis,a common acute abdominal condition.Despite surgical advances,postoperative gastrointestinal dysfunction and delayed recovery continue to negatively impact rehabilitation outcomes and patient quality of life.Laser acupuncture represents a promising emerging complementary therapy with potential benefits for promoting functional recovery.AIM To evaluate the clinical efficacy of laser acupuncture in enhancing gastrointestinal function recovery following laparoscopic appendectomy and to investigate its potential applications in postoperative rehabilitation.METHODS This retrospective controlled clinical study selected 120 acute appendicitis patients who underwent laparoscopic appendectomy from January 2022 to December 2023 at a tertiary hospital.Patients were randomly divided into two groups:Routine treatment group(n=60)and laser acupuncture intervention group(n=60).There were no significant statistical differences in baseline characteristics(age,gender,body mass index,appendicitis severity)between the two groups.The routine treatment group received standard postoperative care,while the laser acupuncture intervention group received additional laser acupuncture treatment on top of standard care.Key observation indicators included time to first exhaust,time to first bowel movement,time to gastrointestinal function recovery,and postoperative complication rates.RESULTS Patients in the laser acupuncture intervention group showed significantly faster gastrointestinal function recovery compared to the routine treatment group.Specifically,time to first exhaust was significantly shortened(intervention group:12.5±2.3 hours vs control group:18.7±3.1 hours,P<0.05),time to first bowel movement was earlier(intervention group:36.4±4.6 hours vs control group:48.2±5.7 hours,P<0.05),postoperative complication rate was significantly reduced(intervention group:8.3%vs control group:20.0%,P<0.05),postoperative pain score was significantly lower(intervention group:3.2±1.1 vs control group:4.7±1.5,P<0.05),and intestinal motility recovery was faster and more effective.CONCLUSION Laser acupuncture,as an auxiliary treatment method,can significantly promote gastrointestinal function recovery in patients after laparoscopic appendectomy,reduce complications,and improve postoperative comfort.This technique has advantages such as minimal invasiveness,rapid recovery,and few side effects,making it worthy of further clinical promotion and application.
文摘BACKGROUND Psychological comorbidities,such as anxiety and depression,in patients with chronic ankle instability(CAI)may impede ankle function improvement,although the precise nature of this association warrants further investigation.AIM To analyze the correlation of anxiety and depression with ankle function in patients with CAI and discussing the risk factors.METHODS This study included 116 patients with CAI,who were admitted to our hospital from July 2022 to July 2024.Anxiety and depression states of patients were assessed with the self-rating anxiety scale(SAS)and self-rating depression scale(SDS),respectively,and their ankle joint function was assessed with the anklehindfoot function score of the American Orthopedic Foot and Ankle Society.Further,the ankle function of patients with CAI with different anxiety and depression states was discussed.Furthermore,the Pearson correlation coefficient was used to analyze the correlation of anxiety and depression with ankle joint function in such patients.Univariate and multivariate analyses were conducted to investigate the factors affecting ankle joint function in patients with CAI.RESULTS Among the 116 patients with CAI,97,13,5,and 1 cases demonstrated none,mild,moderate,and severe anxiety,whereas 95,15,6,and 0 cases showed none,mild,moderate,and severe depression,respectively.The average ankle joint function score was 74.82±6.93 points.The ankle joint function in patients with CAI presented a significant downward tendency as the degree of anxiety and depression increased.Correlation analysis revealed that both the SAS and SDS scores of patients with CAI were significantly negatively correlated with the ankle joint function score.Univariate and multivariate analyses indicated that the risk factors affecting patients’ankle joint function included early functional rehabilitation,visual analog scale,and SDS.CONCLUSION A substantial number of patients with CAI suffer from anxiety and depression,and these negative emotions,to a certain extent,harm the smooth rehabilitation of ankle joint function.

文摘Pain-induced emotions are the negative moods caused by pain,such as depression and anxiety.Acupunc-ture can effectively relieve pain-induced emotions,and its mechanism is closely related to the regulation of neuroplasticity.Neuroplasticity is composed of two types,functional neuroplasticity and structural neuroplasticity.(1)Acupuncture improves functional neuroplasticity by inhibiting the activation of mi-croglia and astrocytes,regulating the expression of neurotransmitters and receptors,modulating cellular signal transduction pathways,and optimizing synaptic transmission efficiency.(2)Acupuncture improves structural neuroplasticity by modulating neuronal synaptic plasticity,inhibiting neuronal apoptosis,and up-regulating the expression of the BDNF/TrKB/CREB signaling pathway.Additionally,acupuncture up-regulates the expression of brain-derived neurotrophic factors to improve both the functional and struc-tural neuroplasticity,thus relieves pain-induced emotions.The above discovery provides an approach to the mechanism research of acupuncture for pain-induced emotions.
文摘BACKGROUND This study seeks to systematically assess the effects of warm acupuncture combined with acupoint massage on the recovery of gastrointestinal function following gastric cancer surgery.AIM To evaluate the impact of warm acupuncture combined with acupoint massage on postoperative gastrointestinal function recovery after gastric cancer surgery,based on a systematic review and meta-analysis.METHODS A comprehensive search was conducted across multiple databases,including PubMed,Cochrane Library,EMBASE,Web of Science,CNKI,Wanfang,and VIP,for relevant studies published up to January 2025.Meta-analyses were carried out using RevMan 5.4,with results presented as standardized mean difference(SMD)or odds ratio with 95%confidence interval(CI).Study heterogeneity was evaluated using the I2 statistic,and sensitivity analyses were performed to assess the stability of the findings.RESULTS A total of 8 randomized controlled trials involving 694 patients were included.The meta-analysis showed that warm acupuncture combined with acupoint massage significantly improved postoperative gastrointestinal function by reducing the time to first flatus(SMD=-2.14,95%CI:-3.14 to-1.14,P<0.0001),time to first bowel movement(SMD=-2.43,95%CI:-3.52 to-1.34,P<0.0001),time to bowel sounds recovery(SMD=-3.15,95%CI:-4.50 to-1.80,P<0.00001),and time to initiate nasogastric or jejunal feeding(SMD=-1.31,95%CI:-2.44 to-0.18,P=0.02).CONCLUSION The combination of warm acupuncture and acupoint massage markedly enhances gastrointestinal recovery following surgery.

文摘OBJECTIVE:To investigate the effects of acupuncture combined with upper limb rehabilitation robot on neural remodeling and functional recovery in post-stroke patients.METHODS:There were 50 stroke patients were randomly divided into an experimental group(acupuncture combined with upper limb rehabilitation robot assisted training)and a control group(upper limb rehabilitation robot assisted training).Various assessments were conducted to compare the effects of the two treatments on neural remodeling and functional recovery.Functional near-infrared spectroscopy technology was used to assess the effects of different treatments on neural plasticity and their impact on upper limb function and activities of daily living.RESULTS:The experimental group showed significantly higher concentrations of oxygenated hemoglobin and total hemoglobin in specific brain regions compared to the control group(P<0.05).Additionally,the experimental group had significantly lower concentrations of deoxygenated hemoglobin(P<0.05).After treatment,both groups showed improvements in various measures,but the experimental group had significantly greater improvements(P<0.05).CONCLUSION:Acupuncture combined with upper limb rehabilitation robot can effectively improve upper limb function and neural remodeling in stroke patients.This study supports the integration of Traditional Chinese and Western Medicine in improving limb dysfunction poststroke.

文摘The recovery of motor function in spastic hemiplegia following stroke is closely associated with cortical reorganisation.This study aimed to investigate the effects of warm acupuncture combined with rehabilitation training on cortical activity and upper limb function using functional near-infrared spectroscopy.Ninety patients were randomly assigned to a warm acupuncture group,a rehabilitation training group,or a combined intervention group.After four weeks of intervention,clinical scores and changes in oxyhaemoglobin concentration in the affected motor cortex were assessed.Results demonstrated that the combined intervention group showed significantly greater improvement in Fugl-Meyer scores,reduced muscle tone,and enhanced activation in the affected primary motor cortex compared to the single intervention groups.Furthermore,functional improvement was significantly correlated with cortical activation changes.These findings indicate a synergistic effect between warm acupuncture and rehabilitation training,potentially mediated through promoting functional reorganisation of the affected cerebral cortex.This provides neuroimaging evidence supporting integrated Chinese and Western medicine rehabilitation protocols.

文摘BACKGROUND Gastrointestinal(GI)dysfunction is common after laparoscopic radical gastrectomy for gastric cancer and affects rehabilitation.While conventional treatments can alleviate symptoms to a certain extent,they often fail to fully address the issue of insufficient GI motility.The GI motility therapeutic apparatus promotes dynamic recovery by simulating GI electric waves,whereas acupuncture regulates zang-fu qi movement,both offering effective interventions.However,there are few clinical studies investigating the combined use of GI motility therapy and acupuncture to promote GI function recovery in patients after GI laparoscopic radical surgery.AIM To evaluate the effects of combining GI motility therapy devices with acupuncture on GI function in patients undergoing radical laparoscopic surgery.METHODS This retrospective study included 196 patients who underwent radical GI endoscopic surgery at the Shanghai Yangzhi Rehabilitation Hospital(Shanghai Sunshine Rehabilitation Center),School of Medicine,Tongji University,from June 2022 to May 2024.Patients were classified into a normal group(conventional treatment,n=96)and an integrated group(conventional+GI motility therapy device+acupuncture,n=100).The effects on GI function,hormone levels preand post-treatment,GI symptoms,immune function,adverse reactions,and patient satisfaction in both groups were assessed.RESULTS Compared with the normal group,the integrated group demonstrated significantly better overall effectiveness(93.00%vs 84.3%;P<0.05)and shorter durations for first exhaust,feeding,defecation,and hospital stay(P<0.05).Post-treatment,the integrated group had lower gastrin and GI symptom rating scale scores and higher motilin,vasoactive intestinal peptide,and immune marker(CD3+,CD4+,CD4+/CD8+,and natural killer cells)levels(P<0.05).The integrated group,compared to the normal group,also reported fewer adverse reactions(5.00%vs 14.58%)and higher patient satisfaction(97.00%vs 84.38%),both statistically significant(P<0.05).CONCLUSION The combination of a GI motility therapy device and acupuncture promotes GI function recovery after radical gastrectomy,regulates GI hormones and immune function,and is safe and effective.

文摘Balanced acupuncture, a single-acupoint balance therapy, regulates the balance of the cerebral center, and is characterized by exerting quick effects and a short treatment course. A total of 20 low-back and leg pain patients with lumbar disc herniation were treated with balanced acupuncture or body acupuncture. Central mechanisms of vaded acupunctures were compared using resting-state functional MRI. Patients from both groups received functional MRI before and after acupuncture. Functional connectivity in brain regions that were strongly associated with the bilatera amygdala was analyzed utilizing AFNI software. Visual analogue scale scores were greater in the balanced acupuncture group compared with the body acupuncture group. Function of the endogenous pain regulation network was enhanced in patients in the balanced acupuncture group, but was not changed in the body acupuncture group. This result indicates that the analgesic effects of body acupuncture do not work through the central nervous system. These data suggest that balanced acupuncture exerts analgesic effects on low-back and leg pain patients with lumbar disc herniation by regulating the function of the endogenous pain regulation network.

Studies have demonstrated that single stimulus of the verum acupuncture in healthy subjects can induce significant connectivity or activity changes in pain- related central networks compared with sham acupuncture. However, these findings are not indicative of the central analgesic mechanism of acupuncture at Shaoyang acupoints. Thus, we recruited 100 migraine sufferers and randomly assigned them into five groups: Shaoyang uncommon acupoint, Shaoyang common acupoint, Yangming uncommon acupoint, non-acupoint control, and blank control groups. Subjects were subjected to evaluation of curative effects and functional MRI prior to and after 10 and 20 acupuncture treatments. All subjects were diagnosed by physicians and enrolled following clinical physical examination. Subjects were observed during 1-4 weeks after inclusion. At the fifth week, the first clinical evaluation and resting functional MRI were conducted. The Shaoyang uncom- mon acupoint, Shaoyang common acupoint, Yangming uncommon acupoint, and non-acupoint control grousp then were treated with acupuncture, five times per week, 20 times in total over 4 weeks. The second and third clinical evaluations and resting functional MRI screenings were conducted following 10 and 20 acupuncture treatments. The blank control group was observed during the 5 to 8 week pe- riod, followed by clinical evaluation and resting functional MRI. The aim of this study was to examine changes in brain functional activity and central networks in subjects with migraine undergoing acu- puncture at Shaoyang uncommon acupoints. 文摘Objective To observe the clinical efficacy of acupuncture and moxibustion in treatment of functional amenorrhea by establishing artificial cycle.Method One hundred and twelve eligible patients with functional amenorrhea were divided into an acupuncture-moxibustion group(64 cases) and a western medication group(48 cases) by adopting random method.According to the pattern differentiation of amenorrhea of traditional Chinese medicine,the patients in the acupuncture-moxibustion group were classified into two types:amenorrhea due to blood depletion and amenorrhea due to stagnation of blood.For patients with amenorrhea due to blood depletion,Guanyuan(关元 CV4),Zhongji(中极CV 3),GuiTlai(归来ST 29),Geshu(膈俞 BL 17),Ganshu(肝俞BL 18),Pishu(脾俞 BL 20) and Shenshu(肾俞 BL 23) were selected.At first,BL 17,BL 18,BL 20 and BL 23 were needled;after deqi,reinforcing method was applied,and the needles were not retained.Then,warmingneedle moxibustion was applied at CV 4,CV 3 and ST 29,and the needles were retained for 30 min.For patients with amenorrhea due to stagnation of blood,CV 3,Qihai(气海CV 6),Xuehai(血海 SP 10),Sanyinjiao(三阴交 SP 6)and Xingjian(行间 LR 2) were selected,reducing method was applied,and the needles were retained for 30 min.The treatment was performed once a day,and treatment for 15 consecutive days were the 1st cycle.The 2nd and3 rd cycles started from the 5th day of menstruation(for the patients still not menstruating,the 2nd and 3rd cycles started from the 29 th and 57 th days since the 1st day of treatment),and the treatment lasted for 15 days.Three cycles were needed.In the western medicine group,estrogen-progestogen was taken orally for 21 days(one cycle) to establish a artificial cycle.The levels of FSH and LH were tested and compared on the 3rd day of menstruation before treatment and after the 3rd cycle of treatment,and the adverse effects were analyzed statistically.Follow-up visit was conducted for the patients after treatment for three cycles,and the recurrence rate was calculated.Result The levels of FSH and LH of patients with functional amenorrhea were enhanced significantly through establishing artificial cycle by acupuncture and moxibustion.There was no significant difference between the acupuncture-moxibustion group and the western medication group in effective rate(P〉0.05);but the adverse effect rate(1.49%) and recurrence rate(25.0%) of the acupuncture-moxibustion group were lower than that of the western medication group(with the adverse effect rate of 4.14%,and recurrence rate of 69.8%)(both P〈0.05).Conclusion The levels of FSH and LH of patients with functional amenorrhea can be enhanced significantly through establishing artificial cycle by acupuncture and moxibustion.The efficacy of acupuncture and moxibustion in treatment of functional amenorrhea by establishing artificial cycle is equivalent to that of oral administration of estrogen-progestogen in treatment of functional amenorrhea by establishing artificial cycle,but the adverse effect rate and recurrence rate of acupuncture and moxibustion group are lower.
文摘Objective To compare the influences of acupuncture combined with rehabilitative training and simple rehabilitative training on neural functional recovery of stroke patients during recovery phase.Methods Sixty-seven stroke patients during recovery phase were randomly divided into an acupuncture combined with rehabilitative training group(group A) and a simple rehabilitative training group(group B) through random number table method,with 34 cases and 33 cases respectively;33 cases and 31 cases were respectively completed actually other than cases dropped out during the observation.Rehabilitation methods such as motion control and functional exercise training were primarily used for rehabilitative training,once daily for 45 min at a time,and 6 times a week;Baihui(百会 GV 20) and Fengchi(风池 GB 20) on the head and relevant acupoints on limbs were selected for acupuncture,once every other day,and 3 times a week.Both groups were treated for 8 weeks,National Institnte of Health Stroke Scale(NIHSS) was used to assess neurologic impairment,Fugl-Meyer scale was used to assess motor function,Barthel was used to assess activity of daily living,and comprehensive assessment was finally made to patients.Results NIH score,Fugl-Meyer scale scores of upper and lower limbs,Barthel score,and comprehensive assessment of group A after treatment were respectively:12.78±2.99,37.55±2.09 and25.67±3.93,63.76±4.65,11.33±1.98,superior to 16.06±4.57,28.71±4.07 and20.41±2.98,53.8214.35,15.18±1.72 of group B.Indices of both groups were significantly improved compared with those before treatment(P〈0.01,P〈0.05),but improvement of indices of group A was more obvious that that of group B after treatment subject to comparison(all P〈0.05).Conclusions Acupuncture combined with rehabilitative training has definite therapeutic effect on recovery of various functions such as neural functional recovery of stroke patients during recovery phase,obviously superior to simple rehabilitative training.
文摘Objective To explore clinical efficacy and safety of acupuncture in treatment of functional constipation.Methods One hundred and twenty cases were randomly divided into an observation group and a control group,60 cases in each group.The patients in the observation group were treated with acupuncture at Tiānshū(天枢 ST 25),Shàngjùxū(上巨虚 ST 37),Zúsānlǐ(足三里 ST 36),Dàchángshū(大肠俞 BL 25),etc.,and the control group was treated with oral administration of Macrogol 4000 and Mosapride.Defecation frequency,stool property,constipation symptom score,accompanying symptom score,gastrointestinal transit time and adverse reaction were observed before treatment,at the end of treatment and 4 weeks after treatment in the two groups.Results Compared with those before treatment,defecation frequency was increased significantly at the end of treatment in the two groups,while stool property,constipation symptom score,accompanying syndrome score and gastrointestinal transit time all were decreased significantly(all P0.01),with no significant difference in these indexes between the two groups at the end of treatment (all P0.05).Compared with the end of treatment,the above-mentioned indexes did not significantly recur in the observation group 4 weeks after the treatment (all P0.05),but the therapeutic effects were unstable with decreased to some extent in the control group (all P0.05).No serious adverse reaction was found in the two groups.Conclusion In the patients with functional constipation,acupuncture can significantly increase defecation frequency,change stool property,alleviate constipation and accompanying symptoms,and shorten gastrointestinal transit time with better safety and tolerance,and the therapeutic effect is more stable than that of Macrogol 4000 and Mosapride.

文摘Methods Two Qihai (气海 CV Objective To discuss the efficacy of acupuncture for groups of points were applied alternatively to 90 cases by chronic functional constipation acupuncture: (1) Tianshhu (CFC). ST 25), 6), Shemgjfuxu (上巨虚 ST 37), etc. (2)Zhongliao (中髎 BL 33), Xiadiao (下髎 BL 34), Dachangshu (大肠俞 BL 25), etc. Electroacupuncture was applied in combination at Zhongliao (中髎 BL 33), Xialiao (下髎 BL 34), Tianshu(天枢 ST 25) and Shamgjuxu (上巨虚 ST 37), once a day, 10 treatments made one session. By constipation diary of patients, frequency of going to stool, straining severity, time of once bowl evacuation, a sense of incomplete bowel emptying, stool quality, a sense of defecation and the Patient Assessment of Constipation Quality of Life questionnaire (PAC-QOL), as well as the changes before and after treatment were observed. Results After treatment, the apparent improvements were achieved in frequency of going to stool, straining severity, time of once bowl evacuation, a sense of incomplete bowel emptying, a sense of defecation and score of PAC-QOL as compared with those before treatment (all P〈0.01). The total effective rate was 67.7% (61/70). The acupuncture effieacies were various on CFC of different dynamic mechanisms. The efficacy on slow transit constipation (STC) was superior to that caused by spastic pelvic floor syndrome (SPFS-C) (P〈0.05), the efficacy on constipation caused by irritable bowel syndrome (IBS-C) was superior to that on either SPFS-C or constipation caused by relaxant pelvic floor syndrome (RPFS-C) (both P〈0.05). Of 52 eases in effective follow-up, 1 month after treatment, 3 eases were cured, 6 eases markedly effective, 23 eases effective, and 20 cases failed; 3 months after treatment, 3 eases were cured, 5 eases markedly effective, 16 cases effective and 28 cases failed. Conclusion Acupuncture has definite efficacy on CFC with definite etiology, loci and diagnostic classification. But, the efficacies are different due to various dynamic mechanisms. A further optimized treatment program is required to improve the efficacy on SPFS-C and RPFS-C.

文摘In the present study, 10 patients with ischemic stroke in the left hemisphere and six healthy controls were subjected to acupuncture at right Waiguan (TE5). In ischemic stroke subjects, functional MRI showed enhanced activation in Broadmann areas 5, 6, 7, 18, 19, 24, 32, the hypothalamic inferior lobe, the mamiilary body, and the ventral posterolateral nucleus of the left hemisphere, and Broadmann areas 4, 6, 7, 18, 19 and 32 of the right hemisphere, but attenuated activation of Broadmann area 13, the hypothalamic inferior lobe, the posterior lobe of the tonsil of cerebellum, and the culmen of the anterior lobe of hypophysis, in the left hemisphere and Broadmann area 13 in the right hemisphere. In ischemic stroke subjects, a number of deactivated brain areas were enhanced, including Broadmann areas 6, 11,20, 22, 37, and 47, the culmen of the anterior lobe of hypophysis, alae lingulae cerebella, and the posterior lobe of the tonsil of cerebellum of the left hemisphere, and Broadmann areas 8, 37, 45 and 47, the culmen of the anterior lobe of hypophysis, pars tuberalis adenohypophyseos, inferior border of lentiform nucleus, lateral globus pallidus, inferior temporal gyrus, and the parahippocampal gyrus of the right hemisphere. These subjects also exhibited attenuation of a number of deactivated brain areas, including Broadmann area 7. These data suggest that acupuncture at Waiguan specifically alters brain function in regions associated with sensation, vision, and motion in ischemic stroke patients. By contrast, in normal individuals, acupuncture at Waiguan generally activates brain areas associated with insomnia and other functions.

文摘BACKGROUND Functional dyspepsia(FD)is a common digestive disease with limited therapeutic options.According to evidence-based clinical practice,acupuncture or electroacupuncture(EA)seems to be a promising therapy for patients with FD.However,there is still a lack of systematic reviews that have analyzed current clinical trials for a better understanding of mechanisms involved in the ameliorating effect of acupuncture and EA on FD.AIM To evaluate the results and qualities of existing clinical evidence for researching the underlying mechanisms of acupuncture/EA in treating FD.METHODS A systematic search of the literature was performed to identify randomized controlled trials in which research on the mechanism of acupuncture or EA was conducted in FD patients.Databases searched included PubMed,EMBASE,Cochrane Library,and Web of Science.Data extraction and quality assessment were completed by two investigators independently and the results of quality evaluation were exported through Review Manager V5.3.RESULTS Eight studies were included in this review with a total of 17 items for detecting techniques for mechanistic research.Positive effects of acupuncture and EA were observed in regulating gastric motility,gastric accommodation,mental status,gastrointestinal hormones,and central and autonomic functions while improving dyspeptic symptoms and quality of life.CONCLUSION The key findings of this systematic review support the potential of acupuncture and EA in altering the heterogeneous pathophysiology in patients with FD.However,high-quality studies with well-planned designs are necessary to provide more credible evidence.
文摘Objective:To review systematically the therapeutic effect and safety of acupuncture or electroacupuncture(EA)on functional dyspepsia(FD).Methods:Using computer,the randomized controlled trials(RCTs)of FD treated with acupuncture or EA were retrieved from the databases,i.e.Cochrane Library,PubMed,EMbase and Web of science,from the date of database establishment to June 1,2020.Using the assessment tool of risk of bias in Cochrane Handbook for systematic reviews of interventions,5.1.0,the quality of the included RCTs were evaluated in methodology.Meta-analysis was performed by RevMan 5.3 software.Results:A total of 10 articles were included,with 1202 cases involved.In this study,the effectiveness of EA or acupuncture on FD was analyzed in comparison with the treatment with sham-acupuncture,sham-EA or western medication.The results indicated that compared with sham-acupuncture or shamEA,acupuncture or EA greatly relieved the clinical symptoms of FD(SMD=-3.03,95%Cl[-3.56,-2.50],P<0.00001)and obtained a higher clinical response rate(OR=5.09,95%CI[3.30,7.86],P<0.00001),but the difference was not significant statistically in gastric electrical activity(P>0.05).Compared with gastric prokinetics,acupuncture or EA more effectively reduced the symptom score(MD=-3.03,95%CI[-3.56,-2.50],P=0.02),but the differences were not significant in the aspects of the improvement of clinical effective rate(OR=1.33,95%CI[0.70,2.52],P=0.39),the promotion of gastric half-emptying(MD=0.23,95%Cl[-1.94,2.40],12=0%,P=0.83),the promotion of motilin secretion(SMD=-0.06,95%CI[-0.23,0.11],I^(2)=0%,P=0.50)and the increase of gastric electrical activity(MD=-0.01,95%Cl[-0.03,0.01],I^(2)=0%,P=0.41).The three RCT studies with adverse events reported indicated that the difference in the incidence of the adverse events was not significant statistically in comparison of acupuncture with sham-acupuncture or gastric prokinetic agent(P<0.05).Conclusion:Regarding the improvement in the symptoms of FD,the effect of EA or acupuncture is superior to the sham-acupuncture group or the western medication group.Simultaneously,the clinical response rate in the treatment with EA or acupuncture is higher than the sham-acupuncture group.During the treatment with EA,some adverse reactions happened in treatment of FD with acupuncture or western medication.Even though there are the limitations in quality and quantity of the included trials,EA and acupuncture still deserve to be promoted in treatment of FD.However,more studies of RCTs with largescale sample,high quality and multiple centers are required for a further verification of the therapeutic effect.

文摘Objective:This study is conducted to investigate the effects of occupational therapy(OT) with or without combined acupuncture for upper limb pain and hand functions among children with spastic hemiplegic cerebral palsy.Methods:A total of 90 patients who met the inclusion criteria were enrolled in a three-arm randomized,placebo-controlled trial,and were allocated to mono-occupational therapy(OT) group(patients=30),OT combined with real-acupuncture(ORA) group(patients=30),and OT combined with placeboacupuncture(OPA) group(patients=30),respectively.In addition to receiving the conventional OT program as the basic treatment for total 8 weeks,patients in ORA and OPA group also received corresponding acupuncture treatment 5 days per week for the first two weeks,3 days per week for the next two weeks,2 days per week for another two weeks,and 1 day per week for the last two weeks.Non-communicating children’s pain checklist-revised scale(NCCPC-R) was used for assessing the upper limb pain among patients.Squeeze dynamometry,modified Ashworth scale(MAS),manual muscle testing(MMT),JebsenTaylor hand function test(JTHFT) and box and block test(BBT) were used for assessing various aspects of upper limb and hand functions among patients.Meanwhile,adverse effects were monitored and recorded.Results:After 8-week treatment,NCCPC-R global scores witnessed a dramatic decline in ORA group(P<0.05),but not in either OPA(P> 0.05) or OT group(P> 0.05).Dramatic increases were identified in both squeeze dynamometry scores and MMT scores in all three groups(all P <0.05).Furthermore,the increases of ORA group were more significant than those of another two groups(both P <0.05).There were no significant changes in MAS global scores in each group(all P> 0.05).The BBT global scores of all three groups significantly increased(all P <0.05) after treatment.Among them,the increase of the ORA group was more than that of the other two groups(P <0.05).The 8-week treatment also had a significant impact on several sub-tests of JTFHT in each group.No significant adverse event was reported.Conclusion:ORA is a potential and promising alternative therapy for mitigating upper limb pain as well as improving both upper limb and hand functions among children with spastic hemiplegic cerebral palsy.

文摘Few studies have examined the effects of different stimuli at a single acupoint using functional magnetic resonance imaging.The present study applied acupuncture at the Neiguan(PC 6),Waiguan(SJ 5),Zhigou(SJ 6) and Yanglingquan(GB 34) acupoints in healthy volunteers.fMRI was used to examine the activation of brain areas in response to different types of acupuncture(cutaneous or routine acupuncture) at each acupoint.There were no significant differences in the distribution of activation in the regions of interest between cutaneous and routine acupuncture at the Neiguan,Waiguan,and Zhigou acupoints,but some differences were observed between the two methods of acupuncture at the Yanglingquan acupoint.There were no significant differences in the intensity of induced activation between cutaneous and routine acupuncture at the Neiguan,Zhigou and Yanglingquan acupoints,but the activation intensity in the right cerebellum induced by routine acupuncture at the Waiguan acupoint was greater than that induced by cutaneous acupuncture.Results confirmed that cutaneous and routine acupuncture at the Neiguan,Waiguan,Zhigou and Yanglingquan acupoints activated different functional brain areas,and caused activation of different intensities in some areas.
文摘Wrist-ankle acupuncture is performed at the specific points of the wrist or ankle to treat diseases inyolving corresponding parts of the body. It is a new therapeutic method enlightened by the theory of channels and collaterals and the theory of nerves, and advanced through clinical trials. It, therefore, exhibits not only the advantages of traditional acupuncture, but also its unique characteristics, yielding satisfactory results especially in pain of various origins. A report is presented below.
