目的基于“阴平阳秘”理论研究昼夜节律紊乱对多囊卵巢综合征(PCOS)模型大鼠性激素及卵巢形态的影响及作用机制。方法SD大鼠采用脱氢表雄酮(DHEA)注射法构建PCOS模型,观察大鼠一般情况,造模第12天开始阴道涂片观察动情周期,造模结束后,E... 目的基于“阴平阳秘”理论研究昼夜节律紊乱对多囊卵巢综合征(PCOS)模型大鼠性激素及卵巢形态的影响及作用机制。方法SD大鼠采用脱氢表雄酮(DHEA)注射法构建PCOS模型,观察大鼠一般情况,造模第12天开始阴道涂片观察动情周期,造模结束后,ELISA检测血清促黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、总睾酮(T)水平。成功制备PCOS模型大鼠30只,随机分为PCOS组(正常光照),PCOS+PL组(长光照),PCOS+CL组(持续光照),各10只;同时设置10只大鼠为对照组(正常光照)。光照处理结束后,所有大鼠均断颈取血并收集卵巢组织,检测各组大鼠卵巢相对质量,同时采用HE染色观察卵巢组织形态并进行有腔卵泡和囊状卵泡计数。透射电镜观察卵巢组织超微结构变化。蛋白质印迹法检测骨形态发生蛋白(BMP)、Smad4、Smad1/5/9蛋白表达水平的变化。结果PCOS组大鼠表现无规则动情周期,表明PCOS大鼠模型制备成功。与PCOS组相比,PCOS+PL组和PCOS+CL组大鼠体重、卵巢系数降低,卵巢组织形态加重且有腔卵泡和囊状卵泡数量增加,血清中LH、E2和T激素水平显著增加,而FSH激素水平显著降低,BMP6、Smad4、Smad1/5/9蛋白表达下调,光照时间越长PCOS的症状越严重。结论昼夜节律紊乱会加重PCOS模型大鼠性激素水平异常、卵巢囊性改变,可通过下调BMP/Smad信号通路进而加速PCOS的发展。 展开更多

Six new lanthanide complexes:[Ln(3,4-DEOBA)3(4,4'-DM-2,2'-bipy)]2·2C_(2)H_(5)OH,[Ln=Dy(1),Eu(2),Tb(3),Sm(4),Ho(5),Gd(6);3,4-DEOBA-=3,4-diethoxybenzoate,4,4'-DM-2,2'-bipy=4,4'-dimethyl-2,2'-bipyridine]were successfully synthesized by the volatilization of the solution at room temperature.The crystal structures of six complexes were determined by single-crystal X-ray diffraction technology.The results showed that the complexes all have a binuclear structure,and the structures contain free ethanol molecules.Moreover,the coordination number of the central metal of each structural unit is eight.Adjacent structural units interact with each other through hydrogen bonds and further expand to form 1D chain-like and 2D planar structures.After conducting a systematic study on the luminescence properties of complexes 1-4,their emission and excitation spectra were obtained.Experimental results indicated that the fluorescence lifetimes of complexes 2 and 3 were 0.807 and 0.845 ms,respectively.The emission spectral data of complexes 1-4 were imported into the CIE chromaticity coordinate system,and their corre sponding luminescent regions cover the yellow light,red light,green light,and orange-red light bands,respectively.Within the temperature range of 299.15-1300 K,the thermal decomposition processes of the six complexes were comprehensively analyzed by using TG-DSC/FTIR/MS technology.The hypothesis of the gradual loss of ligand groups during the decomposition process was verified by detecting the escaped gas,3D infrared spectroscopy,and ion fragment information detected by mass spectrometry.The specific decomposition path is as follows:firstly,free ethanol molecules and neutral ligands are removed,and finally,acidic ligands are released;the final product is the corresponding metal oxide.CCDC:2430420,1;2430422,2;2430419,3;2430424,4;2430421,5;2430423,6. 展开更多

Changes in border-associated macrophages after stroke: Single-cell sequencing analysis 被引量:1

Border-associated macrophages are located at the interface between the brain and the periphery, including the perivascular spaces, choroid plexus, and meninges. Until recently, the functions of border-associated macrophages have been poorly understood and largely overlooked. However, a recent study reported that border-associated macrophages participate in stroke-induced inflammation, although many details and the underlying mechanisms remain unclear. In this study, we performed a comprehensive single-cell analysis of mouse border-associated macrophages using sequencing data obtained from the Gene Expression Omnibus(GEO) database(GSE174574 and GSE225948). Differentially expressed genes were identified, and enrichment analysis was performed to identify the transcription profile of border-associated macrophages. CellChat analysis was conducted to determine the cell communication network of border-associated macrophages. Transcription factors were predicted using the ‘pySCENIC' tool. We found that, in response to hypoxia, borderassociated macrophages underwent dynamic transcriptional changes and participated in the regulation of inflammatory-related pathways. Notably, the tumor necrosis factor pathway was activated by border-associated macrophages following ischemic stroke. The pySCENIC analysis indicated that the activity of signal transducer and activator of transcription 3(Stat3) was obviously upregulated in stroke, suggesting that Stat3 inhibition may be a promising strategy for treating border-associated macrophages-induced neuroinflammation. Finally, we constructed an animal model to investigate the effects of border-associated macrophages depletion following a stroke. Treatment with liposomes containing clodronate significantly reduced infarct volume in the animals and improved neurological scores compared with untreated animals. Taken together, our results demonstrate comprehensive changes in border-associated macrophages following a stroke, providing a theoretical basis for targeting border-associated macrophages-induced neuroinflammation in stroke treatment. 展开更多

Comprehensive pan-cancer analysis of the receptor-interacting protein kinase family expression,genomic alterations,and functional implications

Background:Receptor-interacting protein kinases(RIPKs)regulate cell death,inflammation,and immune responses,yet their roles in cancer are not fully understood.This study investigates the expression,genomic alterations,and functional implications of RIPK family members across various cancers.Methods:We collected multi-omics data from The Cancer Genome Atlas and other public databases,including gene expression,copy number variation(CNV),mutation,methylation,tumor mutation burden(TMB),and microsatellite instability(MSI).Differential expression and survival analyses were performed using DESeq2 and Cox proportional hazards models.CNV and mutation data were analyzed with GISTIC2 and Mutect2,and methylation data with the ChAMP package.Correlations with TMB and MSI were assessed using Pearson coefficients,and gene set enrichment analysis was conducted with the MSigDB Hallmark gene sets.Results:RIPK family members show significant differential expression in various cancers,with RIPK1 and RIPK4 frequently altered.Survival analysis reveals heterogeneous impacts on overall survival.CNV and mutation analyses identify high alteration frequencies for RIPK2 and RIPK7,affecting gene expression.RIPK1 and RIPK7 are hypermethylated in several cancers,inversely correlating with RIPK3 expression.RIPK1,RIPK2,RIPK5,RIPK6,and RIPK7 correlate positively with TMB,while RIPK3 shows negative correlations in some cancers.MSI analysis indicates associations with DNA mismatch repair.G ene set enrichment analysis highlights immune-related pathway enrichment for RIPK1,RIPK2,RIPK3,and RIPK6,and cell proliferation and DNA repair pathways for RIPK4 and RIPK5.RIPK family members showed heterogeneous alterations across cancers:for example,RIPK7 was mutated in up to~15%of u terine c orpus e ndometrial c arcinoma and l ung s quamous c ell c arcinoma cases,and RIPK1 and RIPK7 exhibited frequent promoter hypermethylation in multiple tumor types.Several genes displayed context-dependent associations with overall survival and with TMB/MSI.Conclusion:This pan-cancer analysis of the RIPK family reveals their diverse roles and potential as biomarkers and therapeutic targets.The findings emphasize the importance of RIPK genes in tumorigenesis and suggest context-dependent functions across cancer types.Further studies are needed to explore their mechanisms in cancer development and clinical applications. 展开更多

The sulfation and decomposition process has proven effective in selectively extracting lithium from lepidolite.It is essential to clarify the thermochemical behavior and kinetic parameters of decomposition reactions.Accordingly,comprehensive kinetic study by employing thermalgravimetric analysis at various heating rates was presented in this paper.Two main weight loss regions were observed during heating.The initial region corresponded to the dehydration of crystal water,whereas the subsequent region with overlapping peaks involved complex decomposition reactions.The overlapping peaks were separated into two individual reaction peaks and the activation energy of each peak was calculated using isoconversional kinetics methods.The activation energy of peak 1 exhibited a continual increase as the reaction conversion progressed,while that of peak 2 steadily decreased.The optimal kinetic models,identified as belonging to the random nucleation and subsequent growth category,provided valuable insights into the mechanism of the decomposition reactions.Furthermore,the adjustment factor was introduced to reconstruct the kinetic mechanism models,and the reconstructed models described the kinetic mechanism model more accurately for the decomposition reactions.This study enhanced the understanding of the thermochemical behavior and kinetic parameters of the lepidolite sulfation product decomposition reactions,further providing theoretical basis for promoting the selective extraction of lithium. 展开更多

Global research status in the treatment of glaucoma:a systematic bibliometric analysis

AIM:To present an overview of the research on global glaucoma treatment in the last decade in terms of publication year,journals,countries/regions,organizations,references,and keywords,to investigate the current research international trends and hot topics in this area.METHODS:Bibliometric analysis was conducted on 9128 articles in the Web of Science Core Collection(WoSCC;Clarivate)database.Quantitative and qualitative analysis was employed using VOSviewer(v1.6.18),Pajek(v1.0.0.0),and CiteSpace(v6.1.R2)software.RESULTS:The 9128 papers relating to glaucoma treatment were published from April 2013 to April 2023,of which 7482 articles(82%)were original research articles and 1464(18%)were review articles.The United States(2867)and Johns Hopkins University(166)were the most productive country and institution,respectively,but the University College London had the highest h-index(54).The Journal of Glaucoma was the most productive and Ophthalmology had the highest h-index compared with other journals.The Keywords of interest included treatment surgery,cyclophotocoagulation,minimally invasive glaucoma surgery(MIGS),trabeculectomy,baerveldt,epidemiology,medication adherence,nanoparticle,optical coherence tomography(OCT),gene therapy,and artificial intelligence(AI).Glaucoma surgery appeared as a current research hotspot through the analysis of keywords.CONCLUSION:This study provides insights into the research trends and potential research hotspots in the treatment of glaucoma.This will help researchers to evaluate research policies and to promote international cooperation. 展开更多

This paper investigates the reliability of internal marine combustion engines using an integrated approach that combines Fault Tree Analysis(FTA)and Bayesian Networks(BN).FTA provides a structured,top-down method for identifying critical failure modes and their root causes,while BN introduces flexibility in probabilistic reasoning,enabling dynamic updates based on new evidence.This dual methodology overcomes the limitations of static FTA models,offering a comprehensive framework for system reliability analysis.Critical failures,including External Leakage(ELU),Failure to Start(FTS),and Overheating(OHE),were identified as key risks.By incorporating redundancy into high-risk components such as pumps and batteries,the likelihood of these failures was significantly reduced.For instance,redundant pumps reduced the probability of ELU by 31.88%,while additional batteries decreased the occurrence of FTS by 36.45%.The results underscore the practical benefits of combining FTA and BN for enhancing system reliability,particularly in maritime applications where operational safety and efficiency are critical.This research provides valuable insights for maintenance planning and highlights the importance of redundancy in critical systems,especially as the industry transitions toward more autonomous vessels. 展开更多

To ensure the safe transportation of radioactive materials,numerous countries have established specific standards.For the transfer of fissile materials,it is imperative that the material within the packaging remains in a subcritical state during routine,normal,and accidental transport conditions.In the event of an accident,the rods within the storage tank may become rearranged,introducing uncertainty that must be accounted for to ensure that criticality analysis results are conservative.Historically,this uncertainty was addressed overly conservatively due to limited research on non-uniform arrangement scenarios,which proved unsuitable for criticality safety analysis of spent fuel packages.This paper introduced three distinct methods to non-uniformly rearrange fuel rods—Uniform Arrangement by Blocks,Layer-by-Layer Determination,and Birdcage Deformation—and meticulously evaluates the influences of rod rearrangement on the effective multiplication factor of neutrons,k eff,utilizing the Monte Carlo method.Ultimately,this study presents a holistic method capable of encompassing the entire spectrum of potential effects stemming from the rearrangement of fuel rods during rods mispositioning accident.By augmenting the safety margin,this approach proves to be adeptly suited for the criticality safety analysis of nuclear fuel transport containers. 展开更多

Heavy metal risks and policy analysis on using industrial waste salts for making value-added snow-melting agents

Industrial waste salts are commonly used to make value-added snow-melting agents to ensure traffic safety in northern China during winter and spring after snowfall.However,heavy metals in industrial waste salts may pose certain environmental risks.Snow-melting agents and snow samples were collected and analyzed from highways,arterial roads,footbridges,and other locations in Beijing after the snowstorm in December 2023.It was found that the main component of snow-melting agents was sodium chloride with high concentrations of Cu,Mn,and Zn,which are not regulated in the current policies,despite the recent promotion of environmentally friendly snow-melting agents.The Pb,Zn and Cr contents of some snow samples exceeded the limitation value of surface water quality standards,potentially affecting the soil and water environment near roadsides,although the snow-melting agents comply with relevant standards,which indicates the policy gap in the management of recycled industrial salts.We reviewed and analyzed the relevant standards for snow-melting agents and industrial waste salts proposed nationally and internationally over the past 30 years.Through comparative analysis,we proposed relevant policy recommendations to the existing quality standards of snow-melting agents and the management regulations of industrial waste salts,and the formulation of corresponding usage strategies,aimed at reducing the potential environmental release of heavy metals from the use of snow-melting agents,thereby promoting more sustainable green urban development and environmentally sound waste management. 展开更多

Background:Epidemiological studies have confirmed that longer exposure to insecticides like cypermethrin(CYP)significantly increases the risk of male reproductive toxicity.Crocus sativus L.has been recognized due to its therapeutic properties,but its exact role and molecular mechanisms in treatment of reproductive dysfunction remain unclear.Methods:During this study,36 rats were randomly divided into six groups(n=6):control,CYP-induced(60 mg/kg),standard(leuprolide 3 mg/kg)and three treatment groups receiving aqueous,ethanolic,and oil extracts(50 mg/kg or 20 mL/kg)for post-toxicity induction.Results:The finding represented that exposure of CYP significantly increased oxidative stress,disrupted testicular architecture,and markedly reduced testosterone levels(P<0.05).Importantly,Crocus sativus L.treatment alleviated these changes by increasing the expression of Nrf2(nuclear factor erythroid 2-related factor 2),restoring the activity of antioxidant enzymes,and enhancing testicular histomorphology.Surprisingly,molecular docking established a high binding affinity of Crocus sativus L.phytoconstituents such as gallic acid,cinnamic acid and quercetin to the Nrf2-Keap1 complex.It is worth noting that,Crocus sativus L.exhibited a high level of protection against reproductive toxicity caused by CYP in male rats,which was mediated by the activation of Nrf2 pathway,reduction of oxidative damage,and favorable ADMET characteristics.Conclusion:Notably,this research provides a more valid,safe,and effective method of developing new drugs for reproductive disorders,however,further investigation is needed to support the research findings and implement it in clinical practice. 展开更多

目的探讨助孕十三针结合补肾活血调冲汤对PCOS不孕症患者促排卵效果及妊娠的影响。方法选取2022年11月至2024年6月期间于本院生殖中心就诊的60例PCOS不孕症患者作为研究对象,采用随机数字表法,将患者分为两组,对照组(30例,枸橼酸氯米芬... 目的探讨助孕十三针结合补肾活血调冲汤对PCOS不孕症患者促排卵效果及妊娠的影响。方法选取2022年11月至2024年6月期间于本院生殖中心就诊的60例PCOS不孕症患者作为研究对象,采用随机数字表法,将患者分为两组,对照组(30例,枸橼酸氯米芬治疗)与治疗组(30例,枸橼酸氯米芬+助孕十三针+补肾活血调冲汤治疗)。比较两组的血清性激素水平[黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E2)、催乳素(PRL)、睾酮(T)和孕酮(P)]、血糖水平[空腹血糖(FPG)、餐后2 h血糖(2 hPG)]及胰岛素指标[空腹胰岛素(FINS)和餐后2 h胰岛素(2 hINS)]、子宫情况[子宫动脉阻力指数(resistance index,RI)、搏动指数(pulsatility index,PI)、收缩期与舒张期流速比(systolic/diastolic ratio,S/D)]、中医证候评分标准、治疗后妊娠率。结果治疗前,两组血清性激素水平、血糖及胰岛素水平、子宫情况、中医证候评分比较无统计学意义(P>0.05)。治疗后,两组P水平、PI数值比较无统计学差异(P>0.05);相较于对照组,治疗组LH、FSH、E2、PRL和T水平,FPG、2 hPG、FINS和2 hINS、子宫内膜厚度、子宫内膜容积、RI和S/D、中医证候评分明显降低(P<0.05);治疗组妊娠率明显高于对照组(P<0.05)。结论助孕十三针结合补肾活血调冲汤联合枸橼酸氯米芬治疗PCOS不孕症,能够有效调节患者血清性激素水平,改善糖代谢及胰岛素抵抗,优化子宫内膜容受性,并显著缓解中医临床症状,提高患者的妊娠率。 展开更多

Fibrinogen to be a laboratory screening biomarker for polycystic ovary syndrome(PCOS) patients:a Meta-analysis

Objective To assess the fibrinogen levels in polycystic ovary syndrome (PCOS) patients and to fgure out the potential of fibrinogen to be an early phase laboratory biomarker for PCOS diagnosis. Methods A systemic literature research was performed from databases of Medline via OVID, PubMed, Web of Knowledge and the Cochrane library databases, etc. Fifteen studies including 694 PCOS patients and 548 normal controls were selected after quality assessment. Data of fibrinogen levels, age and body weight were extracted and analyzed systemically. Results Fibrinogen increases in PCOS patients with regard to data poolings of overall studies (mean difference=21.51, 95%CI=3.78-39.25, P=0.02), National Institutes of Health (NIH)-based studies (mean difference=8. 50, 95%CI=0.91-16. 09, P= 0. 03) and Rotterdam-based studies (mean difference=45.61, 95%CI=12.18-79.03, P=0.007) respectively. PCOS patients are younger (mean difference= -2.15, 95%CI= -3.65 - -0. 64, P=0. 005) and with heavier body weight (mean difference=0.92, 95%CI=0.59- 1.25, P〈0.001) than normal controls. Conclusions Increasing in PCOS patients, fibrinogen might be used as a diagnosis biomarker for PCOS, especially in the population with younger age and heavier body weight, which indicates that fibrinogen might be used as an early phase laboratory screening biomarker in clinical practice. 展开更多

多囊卵巢综合征(PCOS)是一种常见的妇科内分泌和生殖功能障碍性疾病,主要临床表现为排卵障碍、胰岛素抵抗、高雄激素血症、肥胖等,该病的发生发展与细胞凋亡、自噬、氧化应激、炎症反应等生物调控过程密切相关。自噬作为一种维持细胞稳... 多囊卵巢综合征(PCOS)是一种常见的妇科内分泌和生殖功能障碍性疾病,主要临床表现为排卵障碍、胰岛素抵抗、高雄激素血症、肥胖等,该病的发生发展与细胞凋亡、自噬、氧化应激、炎症反应等生物调控过程密切相关。自噬作为一种维持细胞稳态的清除机制,在维持卵母细胞生长、发育和成熟过程中发挥着关键作用,探究疾病发生发展过程中的自噬机制,有助于设计通过调控自噬干预PCOS的治疗方法。现有研究表明,自噬在PCOS的发病过程中起着重要作用,能够多层次、多途径、多靶点影响疾病进展。中医药通过靶向自噬信号通路、调控因子及非编码单链RNA分子的表达,来调控PCOS患者卵巢颗粒细胞自噬或子宫内膜自噬,从而缓解炎症、调节PCOS代谢障碍、平衡激素水平,有效改善PCOS胰岛素抵抗、高雄激素血症、排卵障碍等病理状态。该文章对治疗PCOS的中药复方和中药提取物及涉及的主要自噬通路和调控因子进行归纳总结,以期从中医药调控自噬的角度为今后PCOS的治疗提供相关参考和建议。 展开更多

PCOS患者发生高同型半胱氨酸血症的危险因素分析

目的分析多囊卵巢综合征(PCOS)患者发生高同型半胱氨酸血症(HHCY)的危险因素。方法回顾性分析2018年9月至2023年6月在兰州大学第二医院生殖医学科门诊就诊的387例PCOS患者的临床资料,根据血清同型半胱氨酸(HCY)水平是否≥15 mmol/L分为... 目的分析多囊卵巢综合征(PCOS)患者发生高同型半胱氨酸血症(HHCY)的危险因素。方法回顾性分析2018年9月至2023年6月在兰州大学第二医院生殖医学科门诊就诊的387例PCOS患者的临床资料,根据血清同型半胱氨酸(HCY)水平是否≥15 mmol/L分为两组:病例组(HHCY组,n=108)和对照组(NHHCY组,n=279),比较两组间的外周血激素水平及生化指标,单因素及多因素Logistic分析PCOS患者罹患HHCY的危险因素,采用Pearson统计分析HCY水平与血清激素、生化指标间的相关性。结果与对照组比较,病例组年龄较小,且血中总胆红素(TBil)、直接胆红素(DBil)、间接胆红素(IBil)、尿素(Urea)及肌酐(Cr)水平均显著高于对照组(P<0.05)。多因素Logistic回归分析显示,Urea[OR=1.204,95%CI(1.010,1.435)]是PCOS患者发生HHCY的独立危险因素(P<0.05)。相关性分析显示,PCOS患者血清T(r=0.119,P=0.023)、TBil(r=0.130,P=0.011)、IBil(r=0.103,P=0.042)、DBil(r=0.113,P=0.026)和Urea(r=0.113,P=0.026)与HCY呈显著正相关,年龄(r=-0.108,P=0.035)与HCY呈显著负相关。结论血清高尿素水平与PCOS患者发生HHCY存在关联,胆红素代谢对PCOS患者发生HHCY的风险发挥着潜在作用。 展开更多

目的:通过检测多囊卵巢综合征(PCOS)患者血清中血管生成素样蛋白2(ANGPTL2)及巨噬细胞炎症蛋白-1α(MIP-1α)的表达水平,探讨PCOS与慢性低度炎症的关系。方法:选取2022年7月至10月就诊于河北大学附属医院生殖医学科的PCOS患者120例和正... 目的:通过检测多囊卵巢综合征(PCOS)患者血清中血管生成素样蛋白2(ANGPTL2)及巨噬细胞炎症蛋白-1α(MIP-1α)的表达水平,探讨PCOS与慢性低度炎症的关系。方法:选取2022年7月至10月就诊于河北大学附属医院生殖医学科的PCOS患者120例和正常女性40例。酶联免疫法(ELISA)检测血清ANGPTL2、MIP-1α表达水平,化学发光法检测血清中卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)、雌二醇(E 2)、孕激素(P)、泌乳素(PRL)水平;对所有研究对象进行胰岛素释放实验(IRT)及葡萄糖耐量实验(OGTT)。按T、身体质量指数(BMI)及胰岛素抵抗指数(HOMA-IR)将PCOS患者分成8个亚组:PCOS非高T非肥胖非IR、PCOS-IR肥胖非高T、PCOS-IR非高T非肥胖、PCOS-肥胖非高T非IR、PCOS-肥胖高T非IR、PCOS-IR高T非肥胖、PCOS高T非肥胖非IR以及PCOS-IR高T肥胖。结果:正常对照组与PCOS非高T非肥胖非IR组比较,血清ANGPTL2、MIP-1a水平明显偏低(P<0.05)。PCOS-IR非高T非肥胖组与PCOS非高T非肥胖非IR组比较,ANGPTL2、MIP-1a水平明显升高(P<0.05)。PCOS肥胖非高T非IR组与PCOS非高T非肥胖非IR组比较,ANGPTL2、MIP-1a水平亦明显升高(P<0.05)。回归分析结果显示,所有研究对象中ANGPTL2及MIP-1α均与HOMA-IR、BMI、T及LH/FSH均表现为正相关(P<0.05)。PCOS组中,ANGPTL2及MIP-1α与BMI、HOMA-IR呈正相关(P<0.05)。结论:慢性低度炎症在PCOS发病机制中起着重要作用,ANGPTL2、MIP-1α可作为诊断PCOS及评估其严重程度的新的生物标志物,为治疗PCOS提供了新方向。 展开更多

PCOS患者外周血AMH水平与基础窦卵泡数量的相关性分析

目的探讨多囊卵巢综合征(PCOS)患者外周血抗苗勒管激素(AMH)水平与基础窦卵泡数量(AFC)的相关性。方法回顾性分析2020年1月至2024年5月于我院就诊的PCOS不孕症患者及作为对照的单纯输卵管因素不孕症患者的临床资料,通过倾向评分匹配法... 目的探讨多囊卵巢综合征(PCOS)患者外周血抗苗勒管激素(AMH)水平与基础窦卵泡数量(AFC)的相关性。方法回顾性分析2020年1月至2024年5月于我院就诊的PCOS不孕症患者及作为对照的单纯输卵管因素不孕症患者的临床资料,通过倾向评分匹配法平衡组间混杂因素的差异,共获得1192例患者作为研究对象,其中PCOS组和对照组各596例。比较两组患者外周血AMH及基础性激素水平,分析外周血AMH与性激素相关性。进一步采用单因素分析外周血AMH与AFC的关系,并结合阈值、交互作用分析,研究PCOS患者外周血AMH水平与AFC之间关系及效应值变化。结果PCOS组外周血AMH、黄体生成素(LH)、睾酮(T)水平均显著高于对照组(P<0.05)。相关性分析可知,PCOS患者外周血AMH水平与LH(r=0.740)、T(r=0.728)水平呈正相关(P<0.05);调整性激素水平等其他混杂因素,单因素分析结果显示,PCOS组患者AMH、AFC及体质量指数(BMI)均显著增加(P<0.05)。交互作用分析结果显示,PCOS患者外周血AMH与BMI对AFC存在协同效应(P<0.05)。阈值效应分析显示,随着AMH增加,AFC呈上升趋势;在对照组中,当AMH≥3.9 ng/ml时,AFC显著增加(P<0.05);而AMH<3.9 ng/ml时,AMH与AFC未显示明显的相关性;在PCOS组,AMH水平与AFC呈线性正相关。随着BMI增加,AFC呈上升趋势。在对照组中,当BMI≥23.5 kg/m^(2)时,AFC明显增加(P<0.05);而BMI<23.5 kg/m^(2)时,BMI与AFC未显示明显的相关性。在PCOS组,BMI与AFC呈线性正相关。结论外周血AMH水平与AFC呈正相关,尤其是PCOS人群,随着AMH水平增加,AFC增多;而在非PCOS人群中,当AMH低于一定水平时(3.9 ng/ml),AFC与外周血AMH水平并无显著相关性,当AMH高于一定水平时(3.9 ng/ml),随着AMH水平增加,AFC增多。 展开更多
