Quantifying the post-earthquake functional recovery of railway stations presents significant challenges.This paper first establishes a post-earthquake function calculation method for railway stations,encompassing the ...Quantifying the post-earthquake functional recovery of railway stations presents significant challenges.This paper first establishes a post-earthquake function calculation method for railway stations,encompassing the establishment of relationships between the station’s function and the damage state,function loss,and failure probability of components and professional equipment in each layer.Also,the“4 stages-6 sequences”post-earthquake repair method is present,taking into account the functional and structural characteristics of railway stations.Additionally,a novel piecewise function for the post-earthquake functional dynamic recovery of railway stations is developed.A case study is conducted on a typical railway station to demonstrate the analysis procedure.Results indicate that under fortification,rare,and extremely rare earthquake scenarios,the interlayer drift ratio(IDR)of the railway station were 1/276,1/143,and 1/52,respectively,and corresponding peak floor acceleration(PFA)were 6.31 m/s^(2),7.82 m/s^(2),and 8.57 m/s^(2),respectively.The post-earthquake function of the railway station was 93.21%,82.33%,and 64.16%of its initial function.The repair times were 6.66 days,18.65 days,and 37.42 days.The displacement-sensitive,non-structural components were identified as the most vulnerable to damage.And the first repair stage(R_(1))which was mainly used to repair structural components and non-structural transport components,accounted for the highest proportion of total repair time.展开更多
BACKGROUND The posterior cruciate ligament(PCL)is vital for regulating posterior tibial translation in relation to the femur,which is critical for knee stability.PCL tears are infrequently isolated in knee injuries;ho...BACKGROUND The posterior cruciate ligament(PCL)is vital for regulating posterior tibial translation in relation to the femur,which is critical for knee stability.PCL tears are infrequently isolated in knee injuries;however,the absence of the PCL results in abnormal knee kinematics,which may cause injuries to other ligaments.The ideal tendon source for PCL reconstruction is still a subject of debate.AIM To evaluate the results of employing the peroneus longus tendon(PLT)in PCL reconstruction.METHODS A comprehensive search was conducted to identify relevant randomized controlled trials and retrospective observational studies discussing the outcomes of using the PLT for PCL reconstruction.Studies published up to August 2024 were searched across multiple databases,including PubMed,EMBASE,Scopus,Web of Science,Cochrane Library,and Google Scholar.Full texts of the selected articles were retrieved,reviewed,and independently assessed by the investigators.Discrepancies were resolved by consensus,with any remaining disagreements being arbitrated by a third author.RESULTS This meta-analysis included five studies on PLT use for PCL reconstruction:(1)Four prospective studies with 104 patients;and(2)One retrospective study with 18 patients.Most studies followed up participants for 24 months,while one had a shorter follow-up of 18 months.Lysholm and modified cincinnati scores improved by pooled means of 32.2(95%CI:29.3-35.1,I2=0%)and 31.1(95%CI:27.98-34.22,I2=0%),respectively.Postoperative American Orthopaedic Foot and Ankle Society and Foot and Ankle Disability Index scores were 94.5(I2=61.5%)and 94.5(I2=80.09%),respectively.Single-hop and triple-hop test scores averaged 95.5(95%CI:94.5-96.5)and 92.4(95%CI:91.9-92.9)respectively.No significant differences were observed in thigh circumference at 10 cm and 20 cm between the injured and healthy sides.CONCLUSION Evidence supports PLT autografts for PCL reconstruction,improving knee function and patient outcomes.Larger randomized trials are needed to confirm efficacy and compare graft options.展开更多

Limitations regarding phosphorus(P)are widespread in ecosystems.Understanding the impacts of the wetland types on microbially mediated soil P availability and cycling is essential for the effective management of wetla...Limitations regarding phosphorus(P)are widespread in ecosystems.Understanding the impacts of the wetland types on microbially mediated soil P availability and cycling is essential for the effective management of wetlands.In this study,the Beidagang wetland,Baodi paddy field,and Dahuangpu wetland in Tianjin,China were chosen as representatives of the coastal wetland(B),constructed wetland(R),and swampy wetland(W),respectively.Sequential P extraction and metagenomics approaches were adopted to explore the soil P fraction and microbially regulated P cycle.Proteobacteria were the predominant microbes-related soil P cycle.IMPA,gph,rsbU_P,ugpQ,and glpK genes were dominant in organic P(Po)mineralization,while gcd,ppa,and ppx genes were dominant in inorganic P(Pi)solubilization.The salinity,NO_(3)^(-)-N concentration,the ratio of total carbon to total nitrogen(TC/TN),total carbon(TC),and the ratio of soil organic carbon to total P(SOC/TP)were the co-drivers of microbially mediated P cycle processes.Microbial network complexity-relate P cycle was the lowest in the coastal wetland.Salinity and NO_(3)^(-)-N exhibited a significant negative relation to the abundance of most genes-relate Pi solubilization and a remarkable positive correlation with the abundance of many genes-relate Po mineralization.These findings demonstrated that Po mineralization tended to occur in habitats with high salinity and nutrient imbalances,whereas the dissolution of Pi was prone to occur in low-salinity environments with relatively balanced soil nutrients.This study improves understanding of how salinity and soil nutrients jointly shape microbial-regulated soil P cycle in different types of wetlands.展开更多
BACKGROUND Autoimmune liver diseases,including primary biliary cholangitis(PBC),autoi-mmune hepatitis(AIH),and their overlap syndrome(OS),involve immune-mediated liver injury,with OS occurring in 1.2%-25%of PBC patien...BACKGROUND Autoimmune liver diseases,including primary biliary cholangitis(PBC),autoi-mmune hepatitis(AIH),and their overlap syndrome(OS),involve immune-mediated liver injury,with OS occurring in 1.2%-25%of PBC patients.OS carries a higher risk of cirrhosis,hepatocellular carcinoma,and reduced survival.While its pathogenesis remains unclear,gut microbiota dysbiosis and serum metabolite alterations may play key roles.This study uses 16S rRNA sequencing and liquid chromatography-mass spec-trometry(LC-MS)metabolomics to compare gut microbiota and serum metabolites among PBC,AIH,and OS patients,and explores their associations with liver function.AIM To differentiate OS from PBC and AIH based on gut microbiota,serum metabolites,and liver function.METHODS Gut microbiota profiles were analyzed using 16S rRNA sequencing,while untargeted serum metabolomics was conducted via LC-MS.Comparative analyses were performed to identify differences in microbial composition and serum metabolite levels among PBC,AIH,and OS groups.Correlation analyses and network visualization tech-niques were applied to elucidate the interactions among liver function parameters,gut microbiota,and serum metabolites in OS patients.RESULTS Compared to patients with PBC or AIH,OS patients demonstrated significantly reduced microbial diversity and richness.Notable taxonomic shifts included decreased abundances of Firmicutes,Bacteroidetes,and Actinobacteria,alongside increased levels of Proteobacteria and Verrucomicrobia.Distinct serum metabolites,such as pentadecanoic acid and aminoimidazole carboxamide ribonucleotide,were identified in OS patients.Correlation analysis revealed that aspartate aminotransferase(AST)levels were negatively associated with the bacterial genus Fusicatenibacter and the metabolite L-Tyrosine.A microbial-metabolite network diagram further confirmed a strong association between Fusicatenibacter and L-Tyrosine in OS patients.CONCLUSION OS patients show decreased gut microbiota diversity and unique serum metabolites.Multi-omics linked AST,Fusicatenibacter,and L-Tyrosine,revealing OS mechanisms and diagnostic potential.展开更多
Objective:To systematically evaluate the impact of phase I cardiac rehabilitation exercise prescriptions on cardiac function levels in patients after coronary intervention.Methods:Seven Chinese and English databases,i...Objective:To systematically evaluate the impact of phase I cardiac rehabilitation exercise prescriptions on cardiac function levels in patients after coronary intervention.Methods:Seven Chinese and English databases,including Cochrane Library,PubMed,Web of Science,ESC(European Society of Cardiology),and CNKI,as well as professional association websites,were searched using computers,with manual searching as a complement.Relevant literature published from the establishment of the database to February 2025 was retrieved.Two researchers independently screened the literature,extracted data,and evaluated the quality of the literature.Meta-analysis was performed using RevMan 5.3 software.Results:A total of 8 articles were included,involving 843 patients.Meta-analysis results showed that compared with routine nursing,phase I cardiac rehabilitation exercise prescriptions helped increase the 6-minute walking distance in patients after coronary intervention[MD=0.84,95%CI(0.57,1.10),P<0.001],increase the level of left ventricular ejection fraction in patients after coronary intervention[MD=0.67,95%CI(0.33,1.00),P<0.001],and cardiac rehabilitation combined with respiratory rehabilitation exercise could improve the level of left ventricular ejection fraction in patients after coronary intervention[MD=0.58,95%CI(0.40,0.76),P<0.001].Conclusion:The implementation of phase I cardiac rehabilitation exercise prescriptions can help improve cardiac function levels in patients after coronary intervention.However,the standardization of outcome evaluation indicators needs to be improved,and the long-term effects of rehabilitation still require verification through a large number of high-quality studies.展开更多
BACKGROUND There is no effective treatment for Alzheimer's disease(AD),and pharmacological treatment of AD in clinical settings is expensive and prolonged,resulting in a huge psychological and economic burden on t...BACKGROUND There is no effective treatment for Alzheimer's disease(AD),and pharmacological treatment of AD in clinical settings is expensive and prolonged,resulting in a huge psychological and economic burden on the patient's family and caregivers and society as a whole,AD is characterized by progressive,worsening cognitive impairment,and there are currently no drugs that can effectively reverse cognitive impairment.However,it is important to intervene early or delay cognitive impairment so that the condition can be delayed and,ultimately,the burden on patients and families can be reduced through maintenance treatment.It may be that non-pharmacological interventions such as cognitive stimulation therapy(CST)can help with cognitive dysfunction.AIM To provide a better treatment plan for AD patients and delay the deterioration of cognitive function,the effect of CST on cognitive function in AD was studied by Meta-analysis.METHODS Comprehensive search the Chinese and English databases were comprehensively searched by computer.Chinese databases:China Biomedical Literature Database(CBM),Wanfang Database,VIP Database,and China Periodicals Full-text Database(CNKI).The collection time limit is from July 21,2010 to July 21,2022 randomized controlled trials literature on the effects of CST on cognitive function in patients with AD.According to the inclusion and exclusion criteria,literature screening,data extraction,and quality evaluation were performed.Standardized mean difference(SMD)and 95%CI were used as evaluation criteria to evaluate the cognitive function of CST in AD patients.Sensitivity analysis and publication bias detection were performed on the results.Publication bias was assessed using funnel plots,and funnel plot symmetry was assessed with Eggr's test.RESULTS CST can not improve Mental State Examination Scale(MMSE)scores in AD patients.Meta-analysis of CST on MMSE scores showed that the heterogeneity was P=0.14,I2=35%.I2=35%<50%,and the Q test P>0.1,choose the random effect model to integrate statistics,get SMD=0.02,95%CI:-0.37,0.42,P>0.05.Meta-analysis of CST on AD Cognitive Functioning Assessment Scale scores showed that the heterogeneity was P=0.13,I2=36%.I2=36%<50 choose a fixed effect model to integrate statistics,get SMD=-0.01,95%CI:-0.40,0.39,P>0.05,the difference is not statistically significant.Meta-analysis of CST on the cognitive function indicators of patients showed that the heterogeneity was P=0.17,I2=31%.I2=31%<50%,the fixed effect model showed SMD=0.01,95%CI:-0.37,0.38,P>0.05,the difference was not statistically significant.CONCLUSION CST may not improve the cognitive function of AD patients,not improve the cognitive function of AD patients,not improve the ability of daily living,and not reduce mental behavior can improve the cognitive function of AD patients.展开更多

Dementias such as Alzheimer disease(AD)and mild cognitive impairment(MCI)lead to problems with memory,language,and daily activities resulting from damage to neurons in the brain.Given the irreversibility of this neuro...Dementias such as Alzheimer disease(AD)and mild cognitive impairment(MCI)lead to problems with memory,language,and daily activities resulting from damage to neurons in the brain.Given the irreversibility of this neuronal damage,it is crucial to find a biomarker to distinguish individuals with these diseases from healthy people.In this study,we construct a brain function network based on electroencephalography data to study changes in AD and MCI patients.Using a graph-theoretical approach,we examine connectivity features and explore their contributions to dementia recognition at edge,node,and network levels.We find that connectivity is reduced in AD and MCI patients compared with healthy controls.We also find that the edge-level features give the best performance when machine learning models are used to recognize dementia.The results of feature selection identify the top 50 ranked edge-level features constituting an optimal subset,which is mainly connected with the frontal nodes.A threshold analysis reveals that the performance of edge-level features is more sensitive to the threshold for the connection strength than that of node-and network-level features.In addition,edge-level features with a threshold of 0 provide the most effective dementia recognition.The K-nearest neighbors(KNN)machine learning model achieves the highest accuracy of 0.978 with the optimal subset when the threshold is 0.Visualization of edge-level features suggests that there are more long connections linking the frontal region with the occipital and parietal regions in AD and MCI patients compared with healthy controls.Our codes are publicly available at https://github.com/Debbie-85/eeg-connectivity.展开更多

Using Lyapunov functionals and LaSalle's invariance principle, we obtained the sufficient conditions for the global stability of the trivial equilibrium, the predator-extinction equilibrium, the disease-free equilibrium and the coexistence equilibrium, respectively. BACKGROUND Patients who undergo cardiac surgery often face postoperative pain and potential cognitive issues.Multimodal analgesia may address these problems.We hypothesized that multimodal analgesia can reduce opioid ...BACKGROUND Patients who undergo cardiac surgery often face postoperative pain and potential cognitive issues.Multimodal analgesia may address these problems.We hypothesized that multimodal analgesia can reduce opioid use and improve cognitive recovery.AIM To investigate the effects of multimodal analgesia on postoperative opioid consumption and cognitive recovery in patients who underwent cardiac surgery.METHODS A prospective,randomized controlled trial at General Hospital from January 2020 to April 2023 recruited a total of 150 adult patients who underwent elective cardiac surgery.The patients were randomly divided into two groups.Group A had patient-controlled intravenous analgesia with sufentanil and flurbiprofen axil.Group B had flurbiprofen axil and paravertebral nerve block.Data were analyzed with appropriate statistical methods.RESULTS Group B had lower postoperative patient-controlled intravenous analgesia drug consumption(2.21 mL/hour vs 4.26 mL/hour,P<0.001),shorter extubation time(2.32 hours vs 3.81 hours,P<0.001),and intensive care unit stay(15.32 h vs 28.63 h,P<0.001).Visual Analogue Scale pain scores were lower in group B(P<0.001).Group B had fewer postoperative complications(no respiratory depression vs 37.9%in group A,P<0.05),a lower postoperative cognitive dysfunction incidence(16.0%vs 28.0%,P<0.05),and higher Barthel Index scores(P<0.05).CONCLUSION Multimodal analgesia with paravertebral nerve block and flurbiprofen axil reduces opioid use and improves cognitive outcomes in patients who underwent cardiac surgery.展开更多
This study investigates the nonlinear dynamic properties of rotating functionally graded sandwich rectangular plates in a thermal environment.The nonlinear vibration equations for a rotating metal-ceramic functionally...This study investigates the nonlinear dynamic properties of rotating functionally graded sandwich rectangular plates in a thermal environment.The nonlinear vibration equations for a rotating metal-ceramic functionally graded sandwich rectangular plate in a thermal environment are derived using classical thin plate theory and Hamilton’s principle,considering geometric nonlinearity,temperature-dependent material properties,and power law distribution of components through the thickness.With cantilever boundary conditions,the flexural nonlinear differential equations of the rectangular sandwich plate are obtained via the Galerkin method.Since the natural vibration differential equations exhibit nonlinear characteristics,the multiscale method is employed to derive the expression for nonlinear natural frequency.An example analysis reveals how the natural frequency of a functionally graded sandwich rectangular plate varies with rotational speed and temperature.Results show that the nonlinear/linear frequency ratio increases with rotational angular velocity Ω and thickness-to-length ratio h/a,follows a cosine-like periodic pattern with the setting angle,and shows a sharp decrease followed by a rapid increase with increasing width-to-length ratio b/a.The derived analytical solutions for nonlinear frequency provide valuable insights for assessing the dynamic characteristics of functionally graded structures.展开更多

These incl...To evaluate the effects of sacubitril/valsartan and ARB on renal function in patients with and without heart failure, we conducted a comprehensive literature search using both Chinese and English databases. These included the Cochrane Library, Wanfang Database, PubMed, CNKI, Embase, and Clinical Trials. The search encompassed studies published from the inception of the databases to March 2024. Randomized controlled studies meeting the inclusion criteria were assessed for bias using the Cochrane risk of bias assessment tool. Data were analyzed using Review Manager 5.4, with relative risk (RR) as the effect indicator. Depending on the heterogeneity of the studies, either a fixed-effects model or a random-effects model was employed to combine effect sizes. Seven studies met the inclusion criteria. Compared with ARB drugs, sacubitril/valsartan showed a reduction in the deterioration of renal function (RR = 0.70, 95% CI: 0.44–1.12, P = 0.14) and acute renal function injury (RR = 0.77, 95% CI: 0.59–1.00, P = 0.05). The risk of end-stage renal disease was also lower (RR = 0.53, 95% CI: 0.30–0.96, P = 0.16). For serum creatinine levels greater than 2.5 mg/dL, the RR was 0.88 (95% CI: 0.68–1.15, P = 0.37). For a reduction in eGFR of more than 25%, the RR was 0.89 (95% CI: 0.57–1.41, P = 0.63). The incidence of serum potassium levels greater than 5.5 mmol/L was not significantly different between the groups (RR = 1.23, 95% CI: 0.86–1.75, P = 0.26), nor was the incidence of serum potassium levels greater than 6.0 mmol/L (RR = 1.06, 95% CI: 0.58–1.93, P = 0.86). However, the incidence of eGFR decreasing by more than 50% was significantly reduced (RR = 0.58, 95% CI: 0.34–0.99, P < 0.05). In conclusion, sacubitril/valsartan demonstrated a protective effect on the kidneys, effectively reducing the risk of renal deterioration and presenting a potential alternative to ARB drugs.展开更多
BACKGROUND Psychological comorbidities,such as anxiety and depression,in patients with chronic ankle instability(CAI)may impede ankle function improvement,although the precise nature of this association warrants furth...BACKGROUND Psychological comorbidities,such as anxiety and depression,in patients with chronic ankle instability(CAI)may impede ankle function improvement,although the precise nature of this association warrants further investigation.AIM To analyze the correlation of anxiety and depression with ankle function in patients with CAI and discussing the risk factors.METHODS This study included 116 patients with CAI,who were admitted to our hospital from July 2022 to July 2024.Anxiety and depression states of patients were assessed with the self-rating anxiety scale(SAS)and self-rating depression scale(SDS),respectively,and their ankle joint function was assessed with the anklehindfoot function score of the American Orthopedic Foot and Ankle Society.Further,the ankle function of patients with CAI with different anxiety and depression states was discussed.Furthermore,the Pearson correlation coefficient was used to analyze the correlation of anxiety and depression with ankle joint function in such patients.Univariate and multivariate analyses were conducted to investigate the factors affecting ankle joint function in patients with CAI.RESULTS Among the 116 patients with CAI,97,13,5,and 1 cases demonstrated none,mild,moderate,and severe anxiety,whereas 95,15,6,and 0 cases showed none,mild,moderate,and severe depression,respectively.The average ankle joint function score was 74.82±6.93 points.The ankle joint function in patients with CAI presented a significant downward tendency as the degree of anxiety and depression increased.Correlation analysis revealed that both the SAS and SDS scores of patients with CAI were significantly negatively correlated with the ankle joint function score.Univariate and multivariate analyses indicated that the risk factors affecting patients’ankle joint function included early functional rehabilitation,visual analog scale,and SDS.CONCLUSION A substantial number of patients with CAI suffer from anxiety and depression,and these negative emotions,to a certain extent,harm the smooth rehabilitation of ankle joint function.展开更多
Understanding the effects of point liquid loading on transversely isotropic poroelastic media is crucial for advancing geomechanics and biomechanics, where precise modeling of fluid-structure interactions is essential...Understanding the effects of point liquid loading on transversely isotropic poroelastic media is crucial for advancing geomechanics and biomechanics, where precise modeling of fluid-structure interactions is essential. This paper presents a comprehensive analysis of infinite transversely isotropic poroelasticity under a fluid source, based on Biot's theory, aiming to uncover new and previously unexplored insights in the literature. We begin our study by deriving a general solution for fluid-saturated, transversely isotropic poroelastic materials in terms of harmonic functions that satisfy sixth-order homogeneous partial differential equations, using potential theory and Almansi's theorem. Based on these general solutions and potential functions, we construct a Green's function for a point fluid source, introducing three new harmonic functions with undetermined constants. These constants are determined by enforcing continuity and equilibrium conditions. Substituting these into the general solution yields fundamental solutions for poroelasticity that provide crucial support for a wide range of project problems. Numerical results and comparisons with existing literature are provided to illustrate physical mechanisms through contour plots. Our observations reveal that all components tend to zero in the far field and become singular at the concentrated source. Additionally, the contours exhibit rapid changes near the point fluid source but display gradual variations at a distance from it. These findings highlight the intricate behavior of the system under point liquid loading, offering valuable insights for further research and practical applications.展开更多
BACKGROUND Sodium-glucose cotransporter 2(SGLT2)inhibitors are widely used for the tr-eatment of type 2 diabetes(T2D).AIM To evaluate the influence of SGLT2 inhibitors on homeostasis model assessment of insulin resist...BACKGROUND Sodium-glucose cotransporter 2(SGLT2)inhibitors are widely used for the tr-eatment of type 2 diabetes(T2D).AIM To evaluate the influence of SGLT2 inhibitors on homeostasis model assessment of insulin resistance(HOMA-IR)andβ-cell function(HOMA-β)in patients with T2D in a meta-analysis.METHODS Randomized controlled trials(RCTs)comparing SGLT2 inhibitors to placebo in T2D patients,with a minimum treatment duration of 12 weeks,were searched using the PubMed,EMBASE,and Cochrane Library databases.Risk of bias was assessed using the Cochrane Risk of Bias Tool,and the certainty of evidence was evaluated using the Grading of Recommendations,Assessment,Development and Evaluation(GRADE)system.Changes in HOMA-IR and HOMA-βwere the outcomes analyzed.Meta-analyses were performed using a random-effects model by incorporating the potential influences of heterogeneity.RESULTS Of 1388 articles identified,24 RCTs met the inclusion criteria.23 of the included studies were double-blind RCTs with low risk of bias.Pooled results including 2272 patients showed that SGLT2 inhibitors significantly reduced HOMA-IR compared to placebo[mean difference(MD)=-0.81,95%confidence interval(CI):-1.11 to-0.52,P<0.001;I2=82%],indicating reduced insulin resistance.Addi-tionally,meta-analysis with 2845 patients suggested that SGLT2 inhibitors significantly increased HOMA-β(MD=7.90,95%CI:5.44-10.37,P<0.001;I2=74%)compared to placebo in patients with T2D,indicating improvedβ-cell function.Based on GRADE assessment,the certainty of evidence was rated moderate for both outcomes due to heterogeneity.Subgroup analyses showed that HOMA-βincreased more substantially in non-Asian studies than in Asian studies(P for subgroup difference<0.01).Subgroup analyses according to the individual medications of SGLT2 inhibitors all showed significant improvement of HOMA-IR and HOMA-β(P all<0.05).No significant publication bias was detected(P for Egger’s test all>0.05).CONCLUSION SGLT2 inhibitors are associated with improvements in insulin resistance andβ-cell function in patients with T2D,although the certainty of evidence is moderate due to heterogeneity.展开更多

文摘Quantifying the post-earthquake functional recovery of railway stations presents significant challenges.This paper first establishes a post-earthquake function calculation method for railway stations,encompassing the establishment of relationships between the station’s function and the damage state,function loss,and failure probability of components and professional equipment in each layer.Also,the“4 stages-6 sequences”post-earthquake repair method is present,taking into account the functional and structural characteristics of railway stations.Additionally,a novel piecewise function for the post-earthquake functional dynamic recovery of railway stations is developed.A case study is conducted on a typical railway station to demonstrate the analysis procedure.Results indicate that under fortification,rare,and extremely rare earthquake scenarios,the interlayer drift ratio(IDR)of the railway station were 1/276,1/143,and 1/52,respectively,and corresponding peak floor acceleration(PFA)were 6.31 m/s^(2),7.82 m/s^(2),and 8.57 m/s^(2),respectively.The post-earthquake function of the railway station was 93.21%,82.33%,and 64.16%of its initial function.The repair times were 6.66 days,18.65 days,and 37.42 days.The displacement-sensitive,non-structural components were identified as the most vulnerable to damage.And the first repair stage(R_(1))which was mainly used to repair structural components and non-structural transport components,accounted for the highest proportion of total repair time.
文摘BACKGROUND The posterior cruciate ligament(PCL)is vital for regulating posterior tibial translation in relation to the femur,which is critical for knee stability.PCL tears are infrequently isolated in knee injuries;however,the absence of the PCL results in abnormal knee kinematics,which may cause injuries to other ligaments.The ideal tendon source for PCL reconstruction is still a subject of debate.AIM To evaluate the results of employing the peroneus longus tendon(PLT)in PCL reconstruction.METHODS A comprehensive search was conducted to identify relevant randomized controlled trials and retrospective observational studies discussing the outcomes of using the PLT for PCL reconstruction.Studies published up to August 2024 were searched across multiple databases,including PubMed,EMBASE,Scopus,Web of Science,Cochrane Library,and Google Scholar.Full texts of the selected articles were retrieved,reviewed,and independently assessed by the investigators.Discrepancies were resolved by consensus,with any remaining disagreements being arbitrated by a third author.RESULTS This meta-analysis included five studies on PLT use for PCL reconstruction:(1)Four prospective studies with 104 patients;and(2)One retrospective study with 18 patients.Most studies followed up participants for 24 months,while one had a shorter follow-up of 18 months.Lysholm and modified cincinnati scores improved by pooled means of 32.2(95%CI:29.3-35.1,I2=0%)and 31.1(95%CI:27.98-34.22,I2=0%),respectively.Postoperative American Orthopaedic Foot and Ankle Society and Foot and Ankle Disability Index scores were 94.5(I2=61.5%)and 94.5(I2=80.09%),respectively.Single-hop and triple-hop test scores averaged 95.5(95%CI:94.5-96.5)and 92.4(95%CI:91.9-92.9)respectively.No significant differences were observed in thigh circumference at 10 cm and 20 cm between the injured and healthy sides.CONCLUSION Evidence supports PLT autografts for PCL reconstruction,improving knee function and patient outcomes.Larger randomized trials are needed to confirm efficacy and compare graft options.

文摘In this study,an improved integrated radial basis function with nonuniform shape parameter is introduced.The proposed shape parameter varies in each support domain and is defined byθ=1/d_(max),where d_(max)is the maximum distance of any pair of nodes in the support domain.The proposed method is verified and shows good performance.The results are stable and accurate with any number of nodes and an arbitrary nodal distribution.Notably,the support domain should be large enough to obtain accurate results.This method is then applied for transient analysis of curved shell structures made from functionally graded materials with complex geometries.Through several numerical examples,the accuracy of the proposed approach is demonstrated and discussed.Additionally,the influence of various factors on the dynamic behavior of the structures,including the power-law index,different materials,loading conditions,and geometrical parameters of the structures,was investigated.
文摘This review explores multi-directional functionally graded(MDFG)nanostructures,focusing on their material characteristics,modeling approaches,and mechanical behavior.It starts by classifying different types of functionally graded(FG)materials such as conventional,axial,bi-directional,and tri-directional,and the material distribution models like power-law,exponential,trigonometric,polynomial functions,etc.It also discusses the application of advanced size-dependent theories like Eringen’s nonlocal elasticity,nonlocal strain gradient,modified couple stress,and consistent couple stress theories,which are essential to predict the behavior of structures at small scales.The review covers the mechanical analysis of MDFG nanostructures in nanobeams,nanopipes,nanoplates,and nanoshells and their dynamic and static responses under different loading conditions.The effect of multi-directional material gradation on stiffness,stability and vibration is discussed.Moreover,the review highlights the need for more advanced analytical,semi-analytical,and numerical methods to solve the complex vibration problems ofMDFG nanostructures.It is evident that the continued development of these methods is crucial for the design,optimization,and real-world application of MDFG nanostructures in advanced engineering fields like aerospace,biomedicine,and micro/nanoelectromechanical systems(MEMS/NEMS).This study is a reference for researchers and engineers working in the domain of MDFG nanostructures.

文摘Limitations regarding phosphorus(P)are widespread in ecosystems.Understanding the impacts of the wetland types on microbially mediated soil P availability and cycling is essential for the effective management of wetlands.In this study,the Beidagang wetland,Baodi paddy field,and Dahuangpu wetland in Tianjin,China were chosen as representatives of the coastal wetland(B),constructed wetland(R),and swampy wetland(W),respectively.Sequential P extraction and metagenomics approaches were adopted to explore the soil P fraction and microbially regulated P cycle.Proteobacteria were the predominant microbes-related soil P cycle.IMPA,gph,rsbU_P,ugpQ,and glpK genes were dominant in organic P(Po)mineralization,while gcd,ppa,and ppx genes were dominant in inorganic P(Pi)solubilization.The salinity,NO_(3)^(-)-N concentration,the ratio of total carbon to total nitrogen(TC/TN),total carbon(TC),and the ratio of soil organic carbon to total P(SOC/TP)were the co-drivers of microbially mediated P cycle processes.Microbial network complexity-relate P cycle was the lowest in the coastal wetland.Salinity and NO_(3)^(-)-N exhibited a significant negative relation to the abundance of most genes-relate Pi solubilization and a remarkable positive correlation with the abundance of many genes-relate Po mineralization.These findings demonstrated that Po mineralization tended to occur in habitats with high salinity and nutrient imbalances,whereas the dissolution of Pi was prone to occur in low-salinity environments with relatively balanced soil nutrients.This study improves understanding of how salinity and soil nutrients jointly shape microbial-regulated soil P cycle in different types of wetlands.

文摘BACKGROUND Autoimmune liver diseases,including primary biliary cholangitis(PBC),autoi-mmune hepatitis(AIH),and their overlap syndrome(OS),involve immune-mediated liver injury,with OS occurring in 1.2%-25%of PBC patients.OS carries a higher risk of cirrhosis,hepatocellular carcinoma,and reduced survival.While its pathogenesis remains unclear,gut microbiota dysbiosis and serum metabolite alterations may play key roles.This study uses 16S rRNA sequencing and liquid chromatography-mass spec-trometry(LC-MS)metabolomics to compare gut microbiota and serum metabolites among PBC,AIH,and OS patients,and explores their associations with liver function.AIM To differentiate OS from PBC and AIH based on gut microbiota,serum metabolites,and liver function.METHODS Gut microbiota profiles were analyzed using 16S rRNA sequencing,while untargeted serum metabolomics was conducted via LC-MS.Comparative analyses were performed to identify differences in microbial composition and serum metabolite levels among PBC,AIH,and OS groups.Correlation analyses and network visualization tech-niques were applied to elucidate the interactions among liver function parameters,gut microbiota,and serum metabolites in OS patients.RESULTS Compared to patients with PBC or AIH,OS patients demonstrated significantly reduced microbial diversity and richness.Notable taxonomic shifts included decreased abundances of Firmicutes,Bacteroidetes,and Actinobacteria,alongside increased levels of Proteobacteria and Verrucomicrobia.Distinct serum metabolites,such as pentadecanoic acid and aminoimidazole carboxamide ribonucleotide,were identified in OS patients.Correlation analysis revealed that aspartate aminotransferase(AST)levels were negatively associated with the bacterial genus Fusicatenibacter and the metabolite L-Tyrosine.A microbial-metabolite network diagram further confirmed a strong association between Fusicatenibacter and L-Tyrosine in OS patients.CONCLUSION OS patients show decreased gut microbiota diversity and unique serum metabolites.Multi-omics linked AST,Fusicatenibacter,and L-Tyrosine,revealing OS mechanisms and diagnostic potential.

文摘Objective:To systematically evaluate the impact of phase I cardiac rehabilitation exercise prescriptions on cardiac function levels in patients after coronary intervention.Methods:Seven Chinese and English databases,including Cochrane Library,PubMed,Web of Science,ESC(European Society of Cardiology),and CNKI,as well as professional association websites,were searched using computers,with manual searching as a complement.Relevant literature published from the establishment of the database to February 2025 was retrieved.Two researchers independently screened the literature,extracted data,and evaluated the quality of the literature.Meta-analysis was performed using RevMan 5.3 software.Results:A total of 8 articles were included,involving 843 patients.Meta-analysis results showed that compared with routine nursing,phase I cardiac rehabilitation exercise prescriptions helped increase the 6-minute walking distance in patients after coronary intervention[MD=0.84,95%CI(0.57,1.10),P<0.001],increase the level of left ventricular ejection fraction in patients after coronary intervention[MD=0.67,95%CI(0.33,1.00),P<0.001],and cardiac rehabilitation combined with respiratory rehabilitation exercise could improve the level of left ventricular ejection fraction in patients after coronary intervention[MD=0.58,95%CI(0.40,0.76),P<0.001].Conclusion:The implementation of phase I cardiac rehabilitation exercise prescriptions can help improve cardiac function levels in patients after coronary intervention.However,the standardization of outcome evaluation indicators needs to be improved,and the long-term effects of rehabilitation still require verification through a large number of high-quality studies.

文摘BACKGROUND There is no effective treatment for Alzheimer's disease(AD),and pharmacological treatment of AD in clinical settings is expensive and prolonged,resulting in a huge psychological and economic burden on the patient's family and caregivers and society as a whole,AD is characterized by progressive,worsening cognitive impairment,and there are currently no drugs that can effectively reverse cognitive impairment.However,it is important to intervene early or delay cognitive impairment so that the condition can be delayed and,ultimately,the burden on patients and families can be reduced through maintenance treatment.It may be that non-pharmacological interventions such as cognitive stimulation therapy(CST)can help with cognitive dysfunction.AIM To provide a better treatment plan for AD patients and delay the deterioration of cognitive function,the effect of CST on cognitive function in AD was studied by Meta-analysis.METHODS Comprehensive search the Chinese and English databases were comprehensively searched by computer.Chinese databases:China Biomedical Literature Database(CBM),Wanfang Database,VIP Database,and China Periodicals Full-text Database(CNKI).The collection time limit is from July 21,2010 to July 21,2022 randomized controlled trials literature on the effects of CST on cognitive function in patients with AD.According to the inclusion and exclusion criteria,literature screening,data extraction,and quality evaluation were performed.Standardized mean difference(SMD)and 95%CI were used as evaluation criteria to evaluate the cognitive function of CST in AD patients.Sensitivity analysis and publication bias detection were performed on the results.Publication bias was assessed using funnel plots,and funnel plot symmetry was assessed with Eggr's test.RESULTS CST can not improve Mental State Examination Scale(MMSE)scores in AD patients.Meta-analysis of CST on MMSE scores showed that the heterogeneity was P=0.14,I2=35%.I2=35%<50%,and the Q test P>0.1,choose the random effect model to integrate statistics,get SMD=0.02,95%CI:-0.37,0.42,P>0.05.Meta-analysis of CST on AD Cognitive Functioning Assessment Scale scores showed that the heterogeneity was P=0.13,I2=36%.I2=36%<50 choose a fixed effect model to integrate statistics,get SMD=-0.01,95%CI:-0.40,0.39,P>0.05,the difference is not statistically significant.Meta-analysis of CST on the cognitive function indicators of patients showed that the heterogeneity was P=0.17,I2=31%.I2=31%<50%,the fixed effect model showed SMD=0.01,95%CI:-0.37,0.38,P>0.05,the difference was not statistically significant.CONCLUSION CST may not improve the cognitive function of AD patients,not improve the cognitive function of AD patients,not improve the ability of daily living,and not reduce mental behavior can improve the cognitive function of AD patients.

文摘Dementias such as Alzheimer disease(AD)and mild cognitive impairment(MCI)lead to problems with memory,language,and daily activities resulting from damage to neurons in the brain.Given the irreversibility of this neuronal damage,it is crucial to find a biomarker to distinguish individuals with these diseases from healthy people.In this study,we construct a brain function network based on electroencephalography data to study changes in AD and MCI patients.Using a graph-theoretical approach,we examine connectivity features and explore their contributions to dementia recognition at edge,node,and network levels.We find that connectivity is reduced in AD and MCI patients compared with healthy controls.We also find that the edge-level features give the best performance when machine learning models are used to recognize dementia.The results of feature selection identify the top 50 ranked edge-level features constituting an optimal subset,which is mainly connected with the frontal nodes.A threshold analysis reveals that the performance of edge-level features is more sensitive to the threshold for the connection strength than that of node-and network-level features.In addition,edge-level features with a threshold of 0 provide the most effective dementia recognition.The K-nearest neighbors(KNN)machine learning model achieves the highest accuracy of 0.978 with the optimal subset when the threshold is 0.Visualization of edge-level features suggests that there are more long connections linking the frontal region with the occipital and parietal regions in AD and MCI patients compared with healthy controls.Our codes are publicly available at https://github.com/Debbie-85/eeg-connectivity.

文摘This paper examines an epidemic predator-prey model with prey dispersal and Holling type-II functional response. In this model, it is assumed that the predator population suffers a transmissible disease. By analyzing the corresponding characteristic equations, the local stability of each of feasible equilibria and the existence of Hopf bifurcations at the coexistence equilibrium is addressed. Using Lyapunov functionals and LaSalle's invariance principle, we obtained the sufficient conditions for the global stability of the trivial equilibrium, the predator-extinction equilibrium, the disease-free equilibrium and the coexistence equilibrium, respectively. The paper also includes numerical simulations to illustrate the analytical results.

文摘Triboelectric nanogenerators(TENGs)offer a selfsustaining power solution for marine regions abundant in resources but constrained by energy availability.Since their pioneering use in wave energy harvesting in 2014,nearly a decade of advancements has yielded nearly thousands of research articles in this domain.Researchers have developed various TENG device structures with diverse functionalities to facilitate their commercial deployment.Nonetheless,there is a gap in comprehensive summaries and performance evaluations of TENG structural designs.This paper delineates six innovative structural designs,focusing on enhancing internal device output and adapting to external environments:high space utilization,hybrid generator,mechanical gain,broadband response,multi-directional operation,and hybrid energy-harvesting systems.We summarize the prevailing trends in device structure design identified by the research community.Furthermore,we conduct a meticulous comparison of the electrical performance of these devices under motorized,simulated wave,and real marine conditions,while also assessing their sustainability in terms of device durability and mechanical robustness.In conclusion,the paper outlines future research avenues and discusses the obstacles encountered in the TENG field.This review aims to offer valuable perspectives for ongoing research and to advance the progress and application of TENG technology.
文摘BACKGROUND Patients who undergo cardiac surgery often face postoperative pain and potential cognitive issues.Multimodal analgesia may address these problems.We hypothesized that multimodal analgesia can reduce opioid use and improve cognitive recovery.AIM To investigate the effects of multimodal analgesia on postoperative opioid consumption and cognitive recovery in patients who underwent cardiac surgery.METHODS A prospective,randomized controlled trial at General Hospital from January 2020 to April 2023 recruited a total of 150 adult patients who underwent elective cardiac surgery.The patients were randomly divided into two groups.Group A had patient-controlled intravenous analgesia with sufentanil and flurbiprofen axil.Group B had flurbiprofen axil and paravertebral nerve block.Data were analyzed with appropriate statistical methods.RESULTS Group B had lower postoperative patient-controlled intravenous analgesia drug consumption(2.21 mL/hour vs 4.26 mL/hour,P<0.001),shorter extubation time(2.32 hours vs 3.81 hours,P<0.001),and intensive care unit stay(15.32 h vs 28.63 h,P<0.001).Visual Analogue Scale pain scores were lower in group B(P<0.001).Group B had fewer postoperative complications(no respiratory depression vs 37.9%in group A,P<0.05),a lower postoperative cognitive dysfunction incidence(16.0%vs 28.0%,P<0.05),and higher Barthel Index scores(P<0.05).CONCLUSION Multimodal analgesia with paravertebral nerve block and flurbiprofen axil reduces opioid use and improves cognitive outcomes in patients who underwent cardiac surgery.

文摘This study investigates the nonlinear dynamic properties of rotating functionally graded sandwich rectangular plates in a thermal environment.The nonlinear vibration equations for a rotating metal-ceramic functionally graded sandwich rectangular plate in a thermal environment are derived using classical thin plate theory and Hamilton’s principle,considering geometric nonlinearity,temperature-dependent material properties,and power law distribution of components through the thickness.With cantilever boundary conditions,the flexural nonlinear differential equations of the rectangular sandwich plate are obtained via the Galerkin method.Since the natural vibration differential equations exhibit nonlinear characteristics,the multiscale method is employed to derive the expression for nonlinear natural frequency.An example analysis reveals how the natural frequency of a functionally graded sandwich rectangular plate varies with rotational speed and temperature.Results show that the nonlinear/linear frequency ratio increases with rotational angular velocity Ω and thickness-to-length ratio h/a,follows a cosine-like periodic pattern with the setting angle,and shows a sharp decrease followed by a rapid increase with increasing width-to-length ratio b/a.The derived analytical solutions for nonlinear frequency provide valuable insights for assessing the dynamic characteristics of functionally graded structures.

文摘To evaluate the effects of sacubitril/valsartan and ARB on renal function in patients with and without heart failure, we conducted a comprehensive literature search using both Chinese and English databases. These included the Cochrane Library, Wanfang Database, PubMed, CNKI, Embase, and Clinical Trials. The search encompassed studies published from the inception of the databases to March 2024. Randomized controlled studies meeting the inclusion criteria were assessed for bias using the Cochrane risk of bias assessment tool. Data were analyzed using Review Manager 5.4, with relative risk (RR) as the effect indicator. Depending on the heterogeneity of the studies, either a fixed-effects model or a random-effects model was employed to combine effect sizes. Seven studies met the inclusion criteria. Compared with ARB drugs, sacubitril/valsartan showed a reduction in the deterioration of renal function (RR = 0.70, 95% CI: 0.44–1.12, P = 0.14) and acute renal function injury (RR = 0.77, 95% CI: 0.59–1.00, P = 0.05). The risk of end-stage renal disease was also lower (RR = 0.53, 95% CI: 0.30–0.96, P = 0.16). For serum creatinine levels greater than 2.5 mg/dL, the RR was 0.88 (95% CI: 0.68–1.15, P = 0.37). For a reduction in eGFR of more than 25%, the RR was 0.89 (95% CI: 0.57–1.41, P = 0.63). The incidence of serum potassium levels greater than 5.5 mmol/L was not significantly different between the groups (RR = 1.23, 95% CI: 0.86–1.75, P = 0.26), nor was the incidence of serum potassium levels greater than 6.0 mmol/L (RR = 1.06, 95% CI: 0.58–1.93, P = 0.86). However, the incidence of eGFR decreasing by more than 50% was significantly reduced (RR = 0.58, 95% CI: 0.34–0.99, P < 0.05). In conclusion, sacubitril/valsartan demonstrated a protective effect on the kidneys, effectively reducing the risk of renal deterioration and presenting a potential alternative to ARB drugs.
文摘BACKGROUND Psychological comorbidities,such as anxiety and depression,in patients with chronic ankle instability(CAI)may impede ankle function improvement,although the precise nature of this association warrants further investigation.AIM To analyze the correlation of anxiety and depression with ankle function in patients with CAI and discussing the risk factors.METHODS This study included 116 patients with CAI,who were admitted to our hospital from July 2022 to July 2024.Anxiety and depression states of patients were assessed with the self-rating anxiety scale(SAS)and self-rating depression scale(SDS),respectively,and their ankle joint function was assessed with the anklehindfoot function score of the American Orthopedic Foot and Ankle Society.Further,the ankle function of patients with CAI with different anxiety and depression states was discussed.Furthermore,the Pearson correlation coefficient was used to analyze the correlation of anxiety and depression with ankle joint function in such patients.Univariate and multivariate analyses were conducted to investigate the factors affecting ankle joint function in patients with CAI.RESULTS Among the 116 patients with CAI,97,13,5,and 1 cases demonstrated none,mild,moderate,and severe anxiety,whereas 95,15,6,and 0 cases showed none,mild,moderate,and severe depression,respectively.The average ankle joint function score was 74.82±6.93 points.The ankle joint function in patients with CAI presented a significant downward tendency as the degree of anxiety and depression increased.Correlation analysis revealed that both the SAS and SDS scores of patients with CAI were significantly negatively correlated with the ankle joint function score.Univariate and multivariate analyses indicated that the risk factors affecting patients’ankle joint function included early functional rehabilitation,visual analog scale,and SDS.CONCLUSION A substantial number of patients with CAI suffer from anxiety and depression,and these negative emotions,to a certain extent,harm the smooth rehabilitation of ankle joint function.

文摘Understanding the effects of point liquid loading on transversely isotropic poroelastic media is crucial for advancing geomechanics and biomechanics, where precise modeling of fluid-structure interactions is essential. This paper presents a comprehensive analysis of infinite transversely isotropic poroelasticity under a fluid source, based on Biot's theory, aiming to uncover new and previously unexplored insights in the literature. We begin our study by deriving a general solution for fluid-saturated, transversely isotropic poroelastic materials in terms of harmonic functions that satisfy sixth-order homogeneous partial differential equations, using potential theory and Almansi's theorem. Based on these general solutions and potential functions, we construct a Green's function for a point fluid source, introducing three new harmonic functions with undetermined constants. These constants are determined by enforcing continuity and equilibrium conditions. Substituting these into the general solution yields fundamental solutions for poroelasticity that provide crucial support for a wide range of project problems. Numerical results and comparisons with existing literature are provided to illustrate physical mechanisms through contour plots. Our observations reveal that all components tend to zero in the far field and become singular at the concentrated source. Additionally, the contours exhibit rapid changes near the point fluid source but display gradual variations at a distance from it. These findings highlight the intricate behavior of the system under point liquid loading, offering valuable insights for further research and practical applications.
文摘BACKGROUND Sodium-glucose cotransporter 2(SGLT2)inhibitors are widely used for the tr-eatment of type 2 diabetes(T2D).AIM To evaluate the influence of SGLT2 inhibitors on homeostasis model assessment of insulin resistance(HOMA-IR)andβ-cell function(HOMA-β)in patients with T2D in a meta-analysis.METHODS Randomized controlled trials(RCTs)comparing SGLT2 inhibitors to placebo in T2D patients,with a minimum treatment duration of 12 weeks,were searched using the PubMed,EMBASE,and Cochrane Library databases.Risk of bias was assessed using the Cochrane Risk of Bias Tool,and the certainty of evidence was evaluated using the Grading of Recommendations,Assessment,Development and Evaluation(GRADE)system.Changes in HOMA-IR and HOMA-βwere the outcomes analyzed.Meta-analyses were performed using a random-effects model by incorporating the potential influences of heterogeneity.RESULTS Of 1388 articles identified,24 RCTs met the inclusion criteria.23 of the included studies were double-blind RCTs with low risk of bias.Pooled results including 2272 patients showed that SGLT2 inhibitors significantly reduced HOMA-IR compared to placebo[mean difference(MD)=-0.81,95%confidence interval(CI):-1.11 to-0.52,P<0.001;I2=82%],indicating reduced insulin resistance.Addi-tionally,meta-analysis with 2845 patients suggested that SGLT2 inhibitors significantly increased HOMA-β(MD=7.90,95%CI:5.44-10.37,P<0.001;I2=74%)compared to placebo in patients with T2D,indicating improvedβ-cell function.Based on GRADE assessment,the certainty of evidence was rated moderate for both outcomes due to heterogeneity.Subgroup analyses showed that HOMA-βincreased more substantially in non-Asian studies than in Asian studies(P for subgroup difference<0.01).Subgroup analyses according to the individual medications of SGLT2 inhibitors all showed significant improvement of HOMA-IR and HOMA-β(P all<0.05).No significant publication bias was detected(P for Egger’s test all>0.05).CONCLUSION SGLT2 inhibitors are associated with improvements in insulin resistance andβ-cell function in patients with T2D,although the certainty of evidence is moderate due to heterogeneity.
