Introduction:Having a primary care usual source of care(USC)is associated with better population health outcomes.However,the percent of adults in the United States(US)with a usual primary care provider is declining.We...Introduction:Having a primary care usual source of care(USC)is associated with better population health outcomes.However,the percent of adults in the United States(US)with a usual primary care provider is declining.We sought to identify factors associated with establishing a USC at an urgent care clinic or emergency department as opposed to primary care.Methods:We analyzed data from 57,152 participants in the All of Us study who reported having a USC.We used the Andersen Behavioral Model of Health Services Use framework and multivariable logistic regression to examine associations among predisposing,enabling,and need factors,according to the source of usual care.Results:An urgent care clinic,minute clinic,or emergency department was the source of usual care for 6.3%of our sample.The odds of seeking care at this type of facility increased with younger age,lower educational attainment,and better health status.Black and Hispanic individuals,as well as those who reported experiencing discrimination in medical settings or that their provider was of a different race and ethnicity,were also less likely to have a primary care USC.Financial concerns,being anxious about seeing a provider,and the inability to take time off from work also increased the likelihood of having a non‐primary care USC.Conclusions:Improving the rates of having a primary care USC among younger and healthy adults may be achievable through policies that can improve access to convenient,affordable primary care.Efforts to improve diversity among primary care providers and reduce discrimination experienced by patients may also improve the USC rates for racial and ethnic minority groups.展开更多
Quantum dynamical maps are defined and studied for quantum statistical physics based on Orlicz spaces.This complements earlier work[26]where we made a strong case for the assertion that statistical physics of regular ...Quantum dynamical maps are defined and studied for quantum statistical physics based on Orlicz spaces.This complements earlier work[26]where we made a strong case for the assertion that statistical physics of regular systems should properly be based on the pair of Orlicz spaces(Lcosh-1,L log(L+1)),since this framework gives a better description of regular observables,and also allows for a well-defined entropy function.In the present paper we"complete"the picture by addressing the issue of the dynamics of such a system,as described by a Markov semigroup corresponding to some Dirichlet form(see[4,13,14]).Specifically,we show that even in the most general non-commutative contexts,completely positive Markov maps satisfying a natural Det ailed Balance condition canonically admit an action on a large class of quantum Orlicz spaces.This is achieved by the development of a new interpolation strategy for extending the action of such maps to the appropriate intermediate spaces of the pair(L∞,L1).As a consequence,we obtain that completely positive quantum Markov dynamics naturally extends to the context proposed in[26].展开更多

The products were easily isolated by extraction; and the catalyst system was readily recycled at least thrice without significant loss of efficiency. African swine fever(ASF)is an acute,highly contagious and deadly viral disease in swine that jeopardizes the worldwide pig industry.Unfortunately,there are no authoritative vaccine and antiviral drug available for ASF...African swine fever(ASF)is an acute,highly contagious and deadly viral disease in swine that jeopardizes the worldwide pig industry.Unfortunately,there are no authoritative vaccine and antiviral drug available for ASF control.African swine fever virus(ASFV)is the etiological agent of ASF.Among the ASFV proteins,p72 is the most abundant component in the virions and thus a potential target for anti-ASFV drug design.Here,we con-structed a luciferase reporter system driven by the promoter of p72,which is transcribed by the co-transfected ASFV RNA polymerase complex.Using this system,we screened over 3200 natural product compounds and obtained three potent candidates against ASFV.We further evaluated the anti-ASFV effects and proved that among the three candidates,ailanthone(AIL)inhibits the replication of ASFV at the nanomolar concentration(IC_(50)=15 nmol/L).Our in vitro experiments indicated that the antiviral effect of AIL is associated with its inhibition of the HSP90-p23 cochaperone.Finally,we showed the antiviral activity of AIL on Zika virus and hepatitis B virus(HBV),which supports that AIL is a potential broad-spectrum antiviral agent.展开更多
