Grifola frondosa(Maitake)is traditionally valued for its health benefits,with polysaccharides being key bioactive components.This paper investigates a specific subfraction,Fraction D(GFP-D),evaluating its clinical eff...Grifola frondosa(Maitake)is traditionally valued for its health benefits,with polysaccharides being key bioactive components.This paper investigates a specific subfraction,Fraction D(GFP-D),evaluating its clinical effects and mechanisms in immune enhancement,adjunctive anti-tumor activity,and regulation of glucose/lipid metabolism.Three clinical trials were conducted.In an immune study,120 healthy volunteers(CD4+T cell count 500–1000 cells/μL)received 150 mg/day GFP-D for 8 weeks,resulting in significant increases in CD4+T cells(from 632±95 to 812±108 cells/μL,28.5%increase,within the physiological activation range),CD4+/CD8+ratio,NK cell activity,IL-2,and IFN-γ(all P<0.001 vs.placebo).An anti-tumor study with 80 advanced cancer patients(stratified by age,tumor stage,and histotype)showed that adding 1000 mg/day GFP-D to chemotherapy improved objective response rate(52.5%vs.30.0%,P=0.036,95%CI:1.02–3.87),one-year progression-free survival(55.8%vs.33.3%,P=0.022),and preserved immune parameters versus chemotherapy alone.A metabolic study in 90 type 2 diabetes patients found that 400 mg/day GFP-D for 12 weeks significantly lowered fasting glucose,HbA1c,total cholesterol,triglycerides,and LDL-C,while raising HDL-C(from 1.0±0.2 to 1.2±0.2 mmol/L,20%increase,supported by increased AMPK phosphorylation).Mechanistically,immune enhancement involves macrophage/dendritic cell activation via Dectin-1/TLR4 receptors(confirmed by increased receptor expression and downstream signaling molecules),promoting cytokine-driven T/NK cell responses.Anti-tumor effects stem from immunomodulation,direct induction of cancer cell apoptosis(via mitochondrial/caspase pathways,verified by increased Bax/Bcl-2 ratio and caspase-3 activation),and angiogenesis inhibition by downregulating VEGF.Metabolic benefits are linked to AMPK pathway activation in liver/muscle(confirmed by increased p-AMPK/AMPK ratio),enhancing glucose uptake and inhibiting gluconeogenesis/lipogenesis,alongside modulation of gut microbiota(increased Bifidobacterium and Lactobacillus abundance).All trials reported no severe adverse events related to GFP-D;liver/kidney function parameters(ALT,AST,creatinine,urea nitrogen)remained within normal ranges throughout the intervention.Collectively,GFP-D emerges as a multi-functional bioactive agent with substantial therapeutic potential.展开更多

BACKGROUND Perinatal depression affects 10%-20%of pregnant women and subsequently influences maternal health and fetal development.Concerns over the safety of antidepressants during pregnancy have prompted the explora...BACKGROUND Perinatal depression affects 10%-20%of pregnant women and subsequently influences maternal health and fetal development.Concerns over the safety of antidepressants during pregnancy have prompted the exploration of nutritional interventions as adjunct therapies.This study evaluated the impact of combining preconception and prenatal supplementation with myo-inositol,probiotics,and trace elements on mood,quality of life,and fetal development in depressed mothers.This retrospective cohort study included 314 pregnant women who were diag-nosed with mild to moderate depression,as determined by a Zung self-rating depression scale score of less than 69.The participants were divided into an intervention group(n=161)receiving myo-inositol,probiotics,and trace elements and a control group(n=153)without supplementation.Supplementation comm-enced 3 months before conception and continued through pregnancy.Psychiatric symptoms and quality of life were evaluated using the positive and negative affect schedule-now,state-trait anxiety inventory,Patient Health Ques-tionnaire-8,and World Health Organization Quality of life Assessment:Brief Version scales preconception and postpartum.Fetal development metrics were assessed via ultrasound,and neonatal outcomes were recorded.RESULTS The intervention group presented significant reductions in gestational diabetes mellitus(13.04%vs 23.53%,P=0.016)and gestational hypertension(3.73%vs 9.15%,P=0.049).Higher levels of inositol,iron,zinc,and probiotics were observed near term in the intervention group.Postpartum mood assessments indicated lower anxiety and depression scores for the intervention group,with significant improvements in the positive and negative affect schedule-now(P=0.002),trait anxiety(P=0.002),and Patient Health Questionnaire-8(P=0.018)scores.The World Health Organization Quality of life Assessment:Brief Version scores improved in the psychological(P=0.041)and environmental(P=0.009)domains postpartum.Fetal biparietal diameter and femoral length were greater in the intervention group alongside better neonatal body length and reduced neonatal unit admissions(2.48%vs 7.84%,P=0.031).CONCLUSION Combined supplementation with myo-inositol,probiotics,and trace elements from preconception through pregnancy may reduce pregnancy-related complications,enhance mood and quality of life,and improve fetal growth metrics.展开更多
BACKGROUND Demineralized bone matrix(DBM)is a commonly utilized allogenic bone graft substitute to promote osseous union.However,little is known regarding outcomes following DBM utilization in foot and ankle surgical ...BACKGROUND Demineralized bone matrix(DBM)is a commonly utilized allogenic bone graft substitute to promote osseous union.However,little is known regarding outcomes following DBM utilization in foot and ankle surgical procedures.AIM To evaluate the clinical and radiographic outcomes following DBM as a biological adjunct in foot and ankle surgical procedures.METHODS During May 2023,the PubMed,EMBASE and Cochrane library databases were systematically reviewed to identify clinical studies examining outcomes following DBM for the management of various foot and ankle pathologies.Data regarding study characteristics,patient demographics,subjective clinical outcomes,radiological outcomes,complications,and failure rates were extracted and analyzed.In addition,the level of evidence(LOE)and quality of evidence(QOE)for each individual study was also assessed.Thirteen studies were included in this review.RESULTS In total,363 patients(397 ankles and feet)received DBM as part of their surgical procedure at a weighted mean follow-up time of 20.8±9.2 months.The most common procedure performed was ankle arthrodesis in 94 patients(25.9%).Other procedures performed included hindfoot fusion,1st metatarsophalangeal joint arthrodesis,5th metatarsal intramedullary screw fixation,hallux valgus correction,osteochondral lesion of the talus repair and unicameral talar cyst resection.The osseous union rate in the ankle and hindfoot arthrodesis cohort,base of the 5th metatarsal cohort,and calcaneal fracture cohort was 85.6%,100%,and 100%,respectively.The weighted mean visual analog scale in the osteochondral lesions of the talus cohort improved from a pre-operative score of 7.6±0.1 to a post-operative score of 0.4±0.1.The overall complication rate was 27.2%,the most common of which was non-union(8.8%).There were 43 failures(10.8%)all of which warranted a further surgical procedure.CONCLUSION This current systematic review demonstrated that the utilization of DBM in foot and ankle surgical procedures led to satisfactory osseous union rates with favorable wound complication rates.Excellent outcomes were observed in patients undergoing fracture fixation augmented with DBM,with mixed evidence supporting the routine use of DBM in fusion procedures of the ankle and hindfoot.However,the low LOE together with the low QOE and significant heterogeneity between the included studies reinforces the need for randomized control trials to be conducted to identify the optimal role of DBM in the setting of foot and ankle surgical procedures.展开更多

Background Persistent somatic symptoms are a common feature in many psychiatric disorders.Moderate-to-severe cases often necessitate treatment in specialised inpatient psychiatric settings.Aims This study evaluated th...Background Persistent somatic symptoms are a common feature in many psychiatric disorders.Moderate-to-severe cases often necessitate treatment in specialised inpatient psychiatric settings.Aims This study evaluated the efficacy,safety and acceptability of resonant breathing(personalised slow breathing to maximise heart rate variability(HRV))as adjunctive treatment for hospitalised patients with psychiatric disorders presenting with persistent somatic symptoms on admission,when patients typically present with high symptom severity.Methods This multicentre,randomised,placebo-controlled cross-over study included 78 patients diagnosed with somatic symptom disorder or affective disorders with persistent somatic symptoms.Participants underwent lightguided resonant breathing and sham light therapy two times per day for 5weekdays each,starting an average of 11days after admission.The primary outcomes were changes in self-reported somatic well-being and arousal(efficacy),and adverse effects(safety).Secondary outcomes included somatic,anxiety,depression and insomnia symptoms,HRV measures and patient satisfaction(acceptability).The trial was preregistered in the German Clinical Trial Register(ID:DRKS00027323)and funded by the Austrian Research Promotion Agency(Grant ID.F0999886082).Results Resonant breathing rates were six breaths per minute in 60%of patients and higher in the remaining participants.The study found no intervention-specific effect on primary outcomes of self-reported somatic well-being and arousal.Patients reported no adverse effects and expressed high satisfaction with the breathing intervention.Resonant breathing significantly reduced secondary outcomes of self-reported anxiety(η^(2)_(p)=0.09,95%confidence interval(CI)0.01 to 0.22)and insomnia symptoms(η^(2)_(p)=0.10,95%CI 0.01 to 0.23)after 10sessions.It also significantly improved several HRV measures acutely(η^(2)_(p)range:0.23(95%CI 0.09 to 0.38)to0.51(95%CI 0.35 to 0.62)).Conclusions While resonant breathing did not improve somatic well-being or arousal,it was safe,well-tolerated and effective at reducing anxiety and insomnia symptoms during early hospitalisation.These findings support further investigation into resonant breathing for these specific indications over extended periods.展开更多

文摘Background Persistent somatic symptoms are a common feature in many psychiatric disorders.Moderate-to-severe cases often necessitate treatment in specialised inpatient psychiatric settings.Aims This study evaluated the efficacy,safety and acceptability of resonant breathing(personalised slow breathing to maximise heart rate variability(HRV))as adjunctive treatment for hospitalised patients with psychiatric disorders presenting with persistent somatic symptoms on admission,when patients typically present with high symptom severity.Methods This multicentre,randomised,placebo-controlled cross-over study included 78 patients diagnosed with somatic symptom disorder or affective disorders with persistent somatic symptoms.Participants underwent lightguided resonant breathing and sham light therapy two times per day for 5weekdays each,starting an average of 11days after admission.The primary outcomes were changes in self-reported somatic well-being and arousal(efficacy),and adverse effects(safety).Secondary outcomes included somatic,anxiety,depression and insomnia symptoms,HRV measures and patient satisfaction(acceptability).The trial was preregistered in the German Clinical Trial Register(ID:DRKS00027323)and funded by the Austrian Research Promotion Agency(Grant ID.F0999886082).Results Resonant breathing rates were six breaths per minute in 60%of patients and higher in the remaining participants.The study found no intervention-specific effect on primary outcomes of self-reported somatic well-being and arousal.Patients reported no adverse effects and expressed high satisfaction with the breathing intervention.Resonant breathing significantly reduced secondary outcomes of self-reported anxiety(η^(2)_(p)=0.09,95%confidence interval(CI)0.01 to 0.22)and insomnia symptoms(η^(2)_(p)=0.10,95%CI 0.01 to 0.23)after 10sessions.It also significantly improved several HRV measures acutely(η^(2)_(p)range:0.23(95%CI 0.09 to 0.38)to0.51(95%CI 0.35 to 0.62)).Conclusions While resonant breathing did not improve somatic well-being or arousal,it was safe,well-tolerated and effective at reducing anxiety and insomnia symptoms during early hospitalisation.These findings support further investigation into resonant breathing for these specific indications over extended periods.
