腰椎间盘突出症患者疼痛区域相关腧穴特异性特征分析

目的:评估腰椎间盘突出症患者疼痛区域相关腧穴体表温度、浅层组织压痛阈特征与深层肌肉紧张程度,并分析三者之间的相关性,进一步探索腰椎间盘突出症患者疼痛区域腧穴特异性特征。方法:纳入80例腰椎间盘突出症患者为观察组,34例无腰椎... 目的:评估腰椎间盘突出症患者疼痛区域相关腧穴体表温度、浅层组织压痛阈特征与深层肌肉紧张程度,并分析三者之间的相关性,进一步探索腰椎间盘突出症患者疼痛区域腧穴特异性特征。方法:纳入80例腰椎间盘突出症患者为观察组,34例无腰椎间盘突出症健康受试者为对照组。应用红外热成像技术检测受试者命门、腰阳关及双侧肾俞、大肠俞及委中穴的表皮温度,压力痛阈测试技术评估腧穴压力痛觉阈值,剪切波弹性成像技术检测受试者多裂肌杨氏模量值。通过统计分析揭示两组差异性特征指标。结果:与对照组比较,观察组患者命门、腰阳关等腧穴表皮温度显著升高(P<0.05),压痛阈值显著下降(P<0.01);与健侧比较,观察组患者患侧肾俞、大肠俞穴表皮温度显著升高(P<0.05),委中穴表皮温度显著降低(P<0.01),肾俞穴、大肠俞、委中穴压痛阈值显著降低(P<0.01),激活态多裂肌杨氏模量值显著升高(P<0.01)。观察组患者双侧腧穴表皮温度差值、压痛阈值差值、激活态多裂肌杨氏模量值差值均显著大于对照组(P<0.05,P<0.01),且三者之间呈显著正相关关系(P<0.05)。结论:腰椎间盘突出症患者腧穴体表温度、压力痛阈及多裂肌紧张程度在疾病状态下均出现特异性变化,且具有相关性,能较为客观反映人体生理与病理状态。 展开更多

Two 3D models of lumbar intervertebral discrespectively with a real human nucleus and with the nucleus removed were developed and validatedusing published experimental and clinical data. Then the models with a stainless steel nucleusprosthesis implanted and with polymer nucleus prostheses of various properties implanted were usedfor the 3D finite-element biomechanical analysis. All the above simulation and analysis were carriedout for the L4/L5 disc under a human worst--daily compression load of 2000 N. The results show thatthe polymer materials with Young's modulus of elasticity E = 0.1-100 MPa and Poisson's ratio v=0.35-0.5 are suitable to produce artificial nucleus prosthesis in view of biomechanicalconsideration. 展开更多
