Methods: A total of 52 cases with slow transit constipation in conformity with the inclusion criteria were selected and divided into a control group and an observation group according to their visit order and random digital table, 26 cases in each group. Patients in the control group received routine nursing guide. Besides the same routine nursing guide, patients in the observation group received moxibustion at the abdominal acupoints, once every day. The course of the treatment was 4 weeks in the two groups, and the 3-month follow-up was given after the course was finished, for comparing the clinical symptoms, results of colon transit tests, scores of depression/anxiety scale and nursing satisfaction. Results: The total effective rate was 92.3% in the observation group and 69.2% in the control group, with a significant difference between the two groups. After the treatment and during the follow-up checks, the scores of Chinese medical symptoms in the two groups were remarkably decreased than those before the treatment(all P〈0.01); the scores of the observation group were obviously lower than those in the control group(all P〈0.01). The discharge rates of the markers in the two groups were remarkably increased than those at the same time period before the treatment; moreover, the discharge rates of the markers at various time periods were remarkably better in the observation group than those in the control group(P〈0.01). SDS and SAS scores were remarkably decreased after the treatment in the two groups(P〈0.01). In comparison of SDS score between the two groups after the treatment, the difference was statistically significant(P〈0.01). In comparison of SAS score between the two groups after the treatment, the difference was statistically significant(P〈0.05). The nursing satisfaction was 96.2% in the observation group after the treatment, obviously better than that in the control group(73.1%). The recurrence rate was 8.3% in the observation group, remarkably lower than that in the control group(33.3%). Conclusion: Moxibustion at the abdominal acupoints plus routine nursing can remarkably improve the colon transit functions and anxious and depressive emotion in patients with STC, and the therapeutic effects are remarkable. Not only the clinical satisfaction is higher, but the recurrence rate is obviously lower than that of routine nursing.展开更多

Methods: Seventy patients with simple obesity due to yang deficiency of the spleen and kidney were randomly divided into a warm needling moxibustion group and an EA group, with 35 subjects in each group. Same major acupoints were selected for the two groups, including Shuifen (CV 9), Guanyuan (CV 4), Daheng (SP 15), Shuidao (ST 28), Shousanli (LI 10), Zusanli (ST 36), Sanyinjiao (SP 6) and Taixi (KI 3). The warm needling moxibustion group received warm needling moxibustion, while the EA group received EA treatment. The interventions were performed once every other day, with 15 treatments as one course. The therapeutic efficacy, body weight and body mass index (BMI) were then observed and compared. Results: The total effective rate in the warm needling moxibustion group was 85.7% versus 77.1% in the EA group, and the between-group difference was statistically significant (P<0.05). The warm needling moxibustion was remarkably superior to the EA in weight loss and lowering BMI, both with statistical significance (P<0.05, P<0.01). At the three-month follow-up, the body weight and BMI further decreased in the warm needling moxibustion group (both P<0.05), and the levels were lower than those in the EA group (P<0.05, P<0.01). Conclusion: Warm needling moxibustion can produce reliable and consistent efficacy in the treatment of simple obesity due to yang deficiency of the spleen and kidney. Compared with EA, warm needling moxibustion shows advantage in both short-term and long-term efficacies, and thus is worth promotion in clinical practice.展开更多

文摘BACKGROUND Hypertension is a significant global health concern and serves as a critical risk factor for cardiovascular diseases, stroke, and renal failure.Traditional Chinese Medicine(TCM)has been utilized for an extended period to address hypertension, with the syndrome known as Yin deficiency and Yang hyperactivity syndrome(YDYHS)frequently observed in individuals with elevated blood pressure. This syndrome is characterized by symptoms including dizziness, tinnitus, irritability, and insomnia, which are thought to result from an imbalance between Yin,representing the cooling and nourishing aspects, and Yang, denoting the active and warming aspects of the body.Despite the prevalent application of TCM in clinical settings, the metabolic mechanisms underlying the YDYHS in the context of hypertension remain inadequately elucidated.AIM To explore the differential plasma metabolites and associated pathways in hypertensive patients with YDYHS using gas chromatography-mass spectrometry(GC-MS)to elucidate the distinctive blood metabolite pattern in this patient population.METHODS GC-MS was used to analyze plasma samples from 51 hypertensive patients with YDYHS and 20 healthy controls.Chemometric methods, including principal component analysis and partial least squares discriminant analysis, were employed to identify potential biochemical patterns.Simultaneously, the high-quality Kyoto Encyclopedia of Genes and Genomes metabolic pathways database was used to identify associated metabolic pathways.Using variable importance in projection and receiver operating characteristic curve analyses, potential biomarkers were extracted to assess their clinical utility.RESULTS Metabolomic profiling of hypertensive patients with YDYHS identified 20 potential biomarkers(4-hydroxybenzoic acid,pectin,1,2,3-trihydroxybenzene,D-ribose,3-hydroxybutyric acid, quinic acid,L-lysine,oleic acid,2-hydroxybutyric acid, linoleic acid, citric acid,alpha-tocopherol,D-glucuronic acid,glycerol,N-acetyl-L-aspartic acid, betamannosylglycerate, indolelactic acid,L-glutamic acid,D-maltose,L-aspartic acid)and four metabolic pathways(linoleic acid metabolism;alanine, aspartate and glutamate metabolism;D-glutamine and D-glutamate metabolism;arginine biosynthesis). The identified differential metabolites may serve as potential biomarkers for distinguishing hypertensive patients with YDYHS from healthy controls. The area under the curve values ranged from 0.750 to 0.866.Receiver operating characteristic curve analysis showed that these differential metabolites can effectively classify hypertensive patients with YDYHS and healthy individuals.CONCLUSION The metabolomic analysis revealed a distinct blood metabolite pattern in hypertensive patients with YDYHS compared to the healthy control group, highlighting the potential role of the identified 20 biomarkers and four metabolic pathways in these patients. These findings may serve as an important material basis for understanding the occurrence and development of the disease, providing a scientific foundation for future clinical diagnosis.

文摘Objective: To observe the clinical effects of moxibustion at abdominal acupoints for slow transit constipation(STC) due to yang deficiency of the spleen and kidney. Not only the clinical satisfaction is higher, but the recurrence rate is obviously lower than that of routine nursing.
文摘Objective To observe the clinical efficacy of long-snake moxibustion plus Western medicine for chronic heart failure due to heart-kidney yang deficiency. Methods A total of 80 patients were randomized into a control group and an observation group, with 40 cases in each group. The control group was treated with routine Western medicine, and the observation group was treated with long-snake moxibustion therapy on the basis of routine Western medicine.Left ventricular ejection fraction(LVEF),stroke volume(SV)and left ventricular end-diastolic diameter(LVEDd)were measured before and after treatment, and the heart function was assessed. The serum brain natriuretic peptide(BNP)level was measured, the 6 min walking distance was recorded, and the Minnesota living with heart failure questionnaire(MLHFQ)was assessed and scored. The serum galectin-3(Gal-3)and soluble growth stimulation expressed gene 2(sST2)protein levels were measured.Clinical efficacy was evaluated after treatment. Results The total effective rate of the observation group was higher than that of the control group, and the difference between the two groups was statistically significant(P<0.05).After treatment,LVEF and LVEDd in both groups were significantly improved(all P<0.05),and LVEF,SV and LVEDd in the observation group were superior to those in the control group(all P<0.05).The heart function classification in both groups were improved after treatment(both P<0.05),and the heart function classification in the observation group was superior to that in the control group(P<0.05).The serum BNP level in both groups decreased(both P<0.05),and the serum BNP level in the observation group was lower than that in the control group(P<0.05).The 6 min walking distance and MLHFQ scores in both groups were improved(all P<0.05),and the 6 min walking distance and MLHFQ score in the observation group were superior to those in the control group(both P<0.05).The serum Gal-3 and sST2 levels in the observation group decreased significantly(both P<0.05),and all lower than those in the control group(both P<0.05).Conclusion Long-snake moxibustion plus Western medicine has significant efficacy in treating chronic heart failure due to heart-kidney yang deficiency;it can improve heart function, reduce the serum BNP level, and improve the exercise endurance and quality of life in the patients. This may be related to the down-regulation of serum Gal-3 and sST2 levels.

文摘Kidney-yang deficiency syndrome(KYDS)is a diagnostic pattern in the traditional Chinese medicine.Studies have shown that KYDS is related to the functional disorder of hormones of the hypothalamic-pituitary-target gland axes. The standard procedure used to mimic KYDS is the injection of a high dose of exogenous glucocorticoid(hydrocortisone and corticosterone).Such a model showed symptoms such as exhaustion, body twists, cold limbs, lying crowded together, decreased rectal temperature, sexual dysfunction, decreased reaction speed, reduced spontaneous activity, hair loss, loss of appetite, and weight loss. Moreover, the model manifested an imbalance in mutual control among the hormones of the pituitary-target gland axes, including adrenocorticotrophic hormone,CORT,CRH,thyroid-stimulating hormone,triiodothyronine,thyroxine,T,E2,follicle-stimulating hormone, luteinizing hormone, and 17-OHCS.

文摘OBJECTIVE:Yang-deficiency constitution(YADC)is a common unbalanced constitution that predisposes individuals to certain diseases. However, not all people with YADC manifest develop diseases. This calls for delineation of the underlying molecular mechanisms.Previous studies suggested that the gut microbiota and gene differential expression should be considered.METHODS:In the present study,we compared profiles of gut microbiota between four healthy YADC individuals and those of five healthy balanced constitution(BC)counterparts, based on 16 S r RNA gene sequence analysis.Furthermore,YADC relevant genes identified by comparing 62 healthy YADC and 58 healthy BC individuals in total to perform intersection analysis, functional clustering and pathway enrichment analyses.RESULTS:The levels of harmful gut microbiota(Prevotellaceae,LDA score>4.0,P=0.0141)and beneficial gut microbiota(Ruminococcaceae,LDA score>4.0,P=0.0025,Faecalibacterium,LDA score>4.0,P=0.0484)were both elevated in healthy YADC individuals. Also,we found that the specific metabolic pathway with 2,6-Dichloro-p-hydroquinone 1,2-Dioxygenase(Pcp A)as the core in gut microbiota and the glutathione transferase activity has been enriched by YADC relevant genes in healthy YADC individuals were both responsible for the detoxification of halogenated aromatic hydrocarbon substances.CONCLUSIONS:Both beneficial and harmful factors had been detected in healthy YADC individuals, functionally, they may have triggered homeostasis to maintain the health of individuals with YADC.The homeostasis may be maintained by beneficial and harmful factors from gut flora and genes.Future studies are expected to focus on halogenated aromatic hydrocarbons and their detoxification processes.
文摘Objective:To explore the law of birth and yang deficiency constitution based on the theory of five movements and six qi,as well as to provide a new way to better realize the prevention of yang deficiency constitution. Methods:The data of TCM quality table and luck data were collected via questionnaires.According to the data of from the TCM quality table, the respondents were divided into yang deficiency constitution group and non-yang deficiency group.According to the luck data, the distributions of gestational age were compared between the two groups. Results:A total of 175 questionnaires were collected,of which 89 were non-yang deficiency constitution and 86 were yang deficiency constitution. The proportion of Ding year(15.1%)in the yang deficiency group was significantly higher than that in non-yang deficiency group(5.6%),in which the difference was statistically significant(P<0.05).Conclusion:The possibility of yang deficiency constitution is higher.
