OBJECTIVE:To utilize the Traditional Chinese Medicine constitution(TCMC)as a complementary and alternative approach for early disease detection and treatment, with a focus on Yin and Yang deficiency constitutions, which serve as key references for disease prevention and management.METHODS:The dataset containing the data of Yin and Yang deficiency constitution was identified through the Gene Expression Omnibus database. This database was used for differential expression genes(DEGs)analysis and weighted gene co-expression network analysis(WGCNA),and the characteristic genes were then obtained in the dataset using a machine learning method. The hub genes of Yin and Yang deficiency constitution were obtained after analysis using the above three methods, and the hub genes were enriched and analyzed.Subsequently, the hub genes of Yin and Yang deficiency constitution were validated using external datasets.Receiver operating characteristic(ROC)analysis was used on each hub genes of the two groups to further understand their diagnostic performance. The mi RNA–lnc RNA-gene network was used to further analyze the hub genes.Immunoinfiltration and gene set enrichment analysis were performed on the shared hub genes.RESULTS:The GSE87474 dataset was used for DEGs analysis and WGCNA.Using machine learning analyses,we identified 15 and 14 hub genes for Yin and Yang deficiency constitutions, respectively. The results of enrichment analyses showed that Yin deficiency constitution was associated with interleukin-17 signaling pathway, whereas Yang deficiency constitution was associated with glycosaminoglycan biosynthesis–keratan sulfate. The validation dataset GSE56116 showed statistically significant data for s-adenosylmethionine sensor upstream of MTORC1(SAMTOR,also named C7orf60),cofilin 2(CFL2),cytohesin 1 interacting protein(CYTIP),G protein-coupled receptor 183(GPR183),hippocampus abundant transcript 1(HIAT1),kelch like family member 15(KLHL15),mitogen-activated protein kinase 6(MAPK6),and prostaglandin-endoperoxide synthase 2(PTGS2)in Yin deficiency and fucosyltransferase 8(FUT8),TATA-box binding protein associated factor,RNA polymerase I subunit D(TAF1D),zinc finger protein 24(ZNF24),MAPK6,and leptin receptor overlapping transcript like 1(LEPROTL1)in Yang deficiency. The ROC results indicated that these genes have diagnostic value.MAPK6 is a shared hub gene for Yin and Yang deficiencies.CONCLUSIONS:This study identified C7orf60,CFL2,CYTIP,GPR183,HIAT1,KLHL15,MAPK6,and PTGS2 in Yin deficiency and FUT8,TAF1D,ZNF24,MAPK6,and LEPROTL1 in Yang deficiency as potential biomarkers, providing insights into their pathogenesis. This theory not only guides the diagnostic approach in TCM but also extends its influence to various scientific research fields. 展开更多

OBJECTIVE:To explore the effect of Traditional Chinese Medicine(TCM)constitution on cognitive impairment(CI)in patients with cerebral small vessel disease(CSVD)and its underlying neuroimaging mechanism and to provide countermeasures for health management of CSVD patients.METHODS:A total of 241 CSVD patients were recruited from the Department of Neurology. All subjects underwent head magnetic resonance imaging(MRI),Chinese Medicine Questionnaire(CCMQ)and cognitive function examination. The CSVD patients were divided according to the Montreal Cognitive Assessment(MoCA)score into a normal cognitive group(73 cases)and a CI group(168 cases).Logistic regression was used to analyse the risk constitution of CSVD-CI and to construct a risk prediction model.3DT1 MRI images and FreeSurfer 6.0 software(Athinoula A.Martinos,Boston,MA,USA)were used to further explore the involvement of hippocampal subregion volume in patients with at-risk constitution and its correlation with cognitive function.RESULTS:Logistic regression analysis showed that Yang-deficiency constitution(YADC)(P=0.020),older age(P=0.008)and hypertension(P=0.011)were risk factors for CSVD-CI but that balanced constitution(P=0.003)and education(P<0.001)were protective factors.A receiver operating characteristic curve(ROC)was drawn, and the area under the curve was 0.820.Further comparison of overall hippocampal and 12 hippocampal subregion volumes between YADC and non-YADC patients revealed decreased total volume of the left and right hippocampus, bilateral subiculum, presubiculum, molecular layer and right fimbria in the YADC group(P<0.05/13,13 is the number of hypothesis tests). Moreover,in the YADC group, the cognitive function of CSVD patients correlated positively with the overall volume of the left hippocampus(r=0.304,P<0.05)and the molecular layer volume of the left hippocampus(r=0.288,P<0.05).CONCLUSION:CSVD patients with YADC are more prone towards developing CI,and asymmetric atrophy of the hippocampus might be the underlying neuroimaging mechanism. In particular, the volume of the left whole hippocampus and the left hippocampal molecular layer correlated significantly with general cognitive function. 展开更多

BACKGROUND Hypertension is a significant global health concern and serves as a critical risk factor for cardiovascular diseases, stroke, and renal failure.Traditional Chinese Medicine(TCM)has been utilized for an extended period to address hypertension, with the syndrome known as Yin deficiency and Yang hyperactivity syndrome(YDYHS)frequently observed in individuals with elevated blood pressure. This syndrome is characterized by symptoms including dizziness, tinnitus, irritability, and insomnia, which are thought to result from an imbalance between Yin,representing the cooling and nourishing aspects, and Yang, denoting the active and warming aspects of the body.Despite the prevalent application of TCM in clinical settings, the metabolic mechanisms underlying the YDYHS in the context of hypertension remain inadequately elucidated.AIM To explore the differential plasma metabolites and associated pathways in hypertensive patients with YDYHS using gas chromatography-mass spectrometry(GC-MS)to elucidate the distinctive blood metabolite pattern in this patient population.METHODS GC-MS was used to analyze plasma samples from 51 hypertensive patients with YDYHS and 20 healthy controls.Chemometric methods, including principal component analysis and partial least squares discriminant analysis, were employed to identify potential biochemical patterns.Simultaneously, the high-quality Kyoto Encyclopedia of Genes and Genomes metabolic pathways database was used to identify associated metabolic pathways.Using variable importance in projection and receiver operating characteristic curve analyses, potential biomarkers were extracted to assess their clinical utility.RESULTS Metabolomic profiling of hypertensive patients with YDYHS identified 20 potential biomarkers(4-hydroxybenzoic acid,pectin,1,2,3-trihydroxybenzene,D-ribose,3-hydroxybutyric acid, quinic acid,L-lysine,oleic acid,2-hydroxybutyric acid, linoleic acid, citric acid,alpha-tocopherol,D-glucuronic acid,glycerol,N-acetyl-L-aspartic acid, betamannosylglycerate, indolelactic acid,L-glutamic acid,D-maltose,L-aspartic acid)and four metabolic pathways(linoleic acid metabolism;alanine, aspartate and glutamate metabolism;D-glutamine and D-glutamate metabolism;arginine biosynthesis). The identified differential metabolites may serve as potential biomarkers for distinguishing hypertensive patients with YDYHS from healthy controls. The area under the curve values ranged from 0.750 to 0.866.Receiver operating characteristic curve analysis showed that these differential metabolites can effectively classify hypertensive patients with YDYHS and healthy individuals.CONCLUSION The metabolomic analysis revealed a distinct blood metabolite pattern in hypertensive patients with YDYHS compared to the healthy control group, highlighting the potential role of the identified 20 biomarkers and four metabolic pathways in these patients. These findings may serve as an important material basis for understanding the occurrence and development of the disease, providing a scientific foundation for future clinical diagnosis. 展开更多

Objective: To observe the clinical effects of moxibustion at abdominal acupoints for slow transit constipation(STC) due to yang deficiency of the spleen and kidney. Methods: A total of 52 cases with slow transit constipation in conformity with the inclusion criteria were selected and divided into a control group and an observation group according to their visit order and random digital table, 26 cases in each group. Patients in the control group received routine nursing guide. Besides the same routine nursing guide, patients in the observation group received moxibustion at the abdominal acupoints, once every day. The course of the treatment was 4 weeks in the two groups, and the 3-month follow-up was given after the course was finished, for comparing the clinical symptoms, results of colon transit tests, scores of depression/anxiety scale and nursing satisfaction. Results: The total effective rate was 92.3% in the observation group and 69.2% in the control group, with a significant difference between the two groups. After the treatment and during the follow-up checks, the scores of Chinese medical symptoms in the two groups were remarkably decreased than those before the treatment(all P〈0.01); the scores of the observation group were obviously lower than those in the control group(all P〈0.01). The discharge rates of the markers in the two groups were remarkably increased than those at the same time period before the treatment; moreover, the discharge rates of the markers at various time periods were remarkably better in the observation group than those in the control group(P〈0.01). SDS and SAS scores were remarkably decreased after the treatment in the two groups(P〈0.01). In comparison of SDS score between the two groups after the treatment, the difference was statistically significant(P〈0.01). In comparison of SAS score between the two groups after the treatment, the difference was statistically significant(P〈0.05). The nursing satisfaction was 96.2% in the observation group after the treatment, obviously better than that in the control group(73.1%). The recurrence rate was 8.3% in the observation group, remarkably lower than that in the control group(33.3%). Conclusion: Moxibustion at the abdominal acupoints plus routine nursing can remarkably improve the colon transit functions and anxious and depressive emotion in patients with STC, and the therapeutic effects are remarkable. Not only the clinical satisfaction is higher, but the recurrence rate is obviously lower than that of routine nursing. 展开更多

Objective: To compare the therapeutic efficacy between warm needling moxibustion and electroacupuncture (EA) in the treatment of simple obesity due to yang deficiency of the spleen and kidney. Methods: Seventy patients with simple obesity due to yang deficiency of the spleen and kidney were randomly divided into a warm needling moxibustion group and an EA group, with 35 subjects in each group. Same major acupoints were selected for the two groups, including Shuifen (CV 9), Guanyuan (CV 4), Daheng (SP 15), Shuidao (ST 28), Shousanli (LI 10), Zusanli (ST 36), Sanyinjiao (SP 6) and Taixi (KI 3). The warm needling moxibustion group received warm needling moxibustion, while the EA group received EA treatment. The interventions were performed once every other day, with 15 treatments as one course. The therapeutic efficacy, body weight and body mass index (BMI) were then observed and compared. Results: The total effective rate in the warm needling moxibustion group was 85.7% versus 77.1% in the EA group, and the between-group difference was statistically significant (P<0.05). The warm needling moxibustion was remarkably superior to the EA in weight loss and lowering BMI, both with statistical significance (P<0.05, P<0.01). At the three-month follow-up, the body weight and BMI further decreased in the warm needling moxibustion group (both P<0.05), and the levels were lower than those in the EA group (P<0.05, P<0.01). Conclusion: Warm needling moxibustion can produce reliable and consistent efficacy in the treatment of simple obesity due to yang deficiency of the spleen and kidney. Compared with EA, warm needling moxibustion shows advantage in both short-term and long-term efficacies, and thus is worth promotion in clinical practice. 展开更多

Objective To observe the clinical efficacy of acupuncture and moxibustion for functional dyspepsia(FD)due to Yang deficiency of the spleen and stomach. Methods A total of 90 patients meeting the inclusion criteria of FD due to Yang deficiency of the spleen and stomach were randomized into an acupuncture-moxibustion group,an electroacupuncture(EA)group, and a mosapride group by the random number table method, with 30 cases in each group. The mosapride group was treated with mosapride citrate dispersible tablets. The EA group was treated with EA treatment. The acupuncture-moxibustion group was treated with additional moxibustion therapy based on the treatment in the EA group. All three groups took 10 d as one treatment course and 2 courses in total, with a 2-day interval between two courses. The traditional Chinese medicine(TCM)symptoms score, functional digestive disorder quality of life questionnaire(FDDQL)score,oral-colon transit time(OCTT),and serum glucagon-like peptide-1(GLP-1)level was compared before and after treatment among the three groups. And the clinical efficacy was evaluated. Results The total effective rate in the acupuncture-moxibustion group was 96.7%,which was higher than 86.7%in the EA group and 73.3%in the mosapride group, and the total effective rate in the EA group was higher than that in the mosapride group;the inter-group differences were all statistically significant(P<0.05).After treatment, the TCM symptom score in the three groups was lower than that before treatment(P<0.05),and the FDDQL score was higher than that before treatment(P<0.05).The change after treatment in the TCM symptom score in the acupuncture-moxibustion group and the EA group was more significant than that in the mosapride group, and the change in the acupuncture-moxibustion group was more significant than that in the EA group;the inter-group differences were statistically significant(P<0.05).The change after treatment in the FDDQL score in the acupuncture-moxibustion group and the EA group was greater than that in the mosapride group, and the change in the acupuncture-moxibustion group was larger than that in the EA group;the inter-group differences were all statistically significant(P<0.05).After treatment, the OCTT in all three groups was lower than that before treatment(P<0.05),and the serum GLP-1 level was higher than that before treatment(P<0.05);the change after treatment in the OCTT in the acupuncture-moxibustion group and the EA group was more significant than that in the mosapride group, and the change in the acupuncture-moxibustion group was more significant than that in the EA group;the inter-group differences were statistically significant(P<0.05).The change after treatment in the serum GLP-1 level in the acupuncture-moxibustion group and the EA group was greater than that in the mosapride group, and the change in the acupuncture-moxibustion group was larger than that in the EA group;the inter-group differences were statistically significant(P<0.05).Conclusion Acupuncture and moxibustion can improve the clinical symptoms in patients with FD and enhance their quality of life, producing better effects than EA or mosapride alone.Shortening gastrointestinal movement time and increasing gastrointestinal hormone levels may be its mechanism. 展开更多

Kidney-yang deficiency syndrome(KYDS)is a diagnostic pattern in the traditional Chinese medicine.Studies have shown that KYDS is related to the functional disorder of hormones of the hypothalamic-pituitary-target gland axes. The standard procedure used to mimic KYDS is the injection of a high dose of exogenous glucocorticoid(hydrocortisone and corticosterone).Such a model showed symptoms such as exhaustion, body twists, cold limbs, lying crowded together, decreased rectal temperature, sexual dysfunction, decreased reaction speed, reduced spontaneous activity, hair loss, loss of appetite, and weight loss. Moreover, the model manifested an imbalance in mutual control among the hormones of the pituitary-target gland axes, including adrenocorticotrophic hormone,CORT,CRH,thyroid-stimulating hormone,triiodothyronine,thyroxine,T,E2,follicle-stimulating hormone, luteinizing hormone, and 17-OHCS. 展开更多

OBJECTIVE:Yang-deficiency constitution(YADC)is a common unbalanced constitution that predisposes individuals to certain diseases. However, not all people with YADC manifest develop diseases. This calls for delineation of the underlying molecular mechanisms.Previous studies suggested that the gut microbiota and gene differential expression should be considered.METHODS:In the present study,we compared profiles of gut microbiota between four healthy YADC individuals and those of five healthy balanced constitution(BC)counterparts, based on 16 S r RNA gene sequence analysis.Furthermore,YADC relevant genes identified by comparing 62 healthy YADC and 58 healthy BC individuals in total to perform intersection analysis, functional clustering and pathway enrichment analyses.RESULTS:The levels of harmful gut microbiota(Prevotellaceae,LDA score>4.0,P=0.0141)and beneficial gut microbiota(Ruminococcaceae,LDA score>4.0,P=0.0025,Faecalibacterium,LDA score>4.0,P=0.0484)were both elevated in healthy YADC individuals. Also,we found that the specific metabolic pathway with 2,6-Dichloro-p-hydroquinone 1,2-Dioxygenase(Pcp A)as the core in gut microbiota and the glutathione transferase activity has been enriched by YADC relevant genes in healthy YADC individuals were both responsible for the detoxification of halogenated aromatic hydrocarbon substances.CONCLUSIONS:Both beneficial and harmful factors had been detected in healthy YADC individuals, functionally, they may have triggered homeostasis to maintain the health of individuals with YADC.The homeostasis may be maintained by beneficial and harmful factors from gut flora and genes.Future studies are expected to focus on halogenated aromatic hydrocarbons and their detoxification processes. 展开更多
