Background:The COVID-19 pandemic profoundly impacted medical education and caused psychological distress and worry about clinical learning for those who received clinical medical education during this period.Although ...Background:The COVID-19 pandemic profoundly impacted medical education and caused psychological distress and worry about clinical learning for those who received clinical medical education during this period.Although the acute phase of COVID-19 has now ended,understanding its impact is essential for strengthening medical system preparedness.Therefore,the present study examined the association between changes in medical education and worry about clinical learning,considering the mediating role of psychological distress among participants who received clinical training.Methods:A cross-sectional online survey was conducted with 542 post-graduate year(PGY)doctors and allied health students from the 2020 and 2021 cohorts in Taiwan.The survey assessed changes in clinical training modes,direct patient care,learning stress,psychological distress,and worry about clinical learning.Results:Among the participants,76.0%experienced changes in clinical training mode,and 84.3%had direct patient care experience.Direct patient care/contact was significantly correlated with lower levels of worry about clinical learning.Learning stress was positively correlated with increased psychological distress and worry about clinical learning.Psychological distress partially mediated the relationship between learning stress and worry about clinical learning.Conclusion:The findings highlight the importance of direct contact with patients because it was associated with lower levels of worry about clinical learning.Moreover,increased learning stress was associated with psychological distress and worry about clinical learning.Therefore,effective support systems are essential to mitigate these impacts and enhance clinical training during national health crises.The findings provide valuable insights for improving medical education and support systems in future health emergencies.However,the present study used a cross-sectional study design.Therefore,the mediation findings should be interpreted with caution(i.e.,no causal relationships can be determined based on cross-sectional data).展开更多

The present study examined the factor structure and psychometric properties of th...The Penn State Worry Questionnaire(PSWQ) is a measure of worry phenomena and has been demonstrated valid in cross-cultural populations. The present study examined the factor structure and psychometric properties of the Chinese version of PSWQ(Ch-PSWQ) in a Chinese college sample(n=1243). Exploratory factor analysis of the Ch-PSWQ revealed a two-factor solution(engagement of worry and absence of worry). Confirmatory factor analysis and model comparison supported that the model of one factor with method effect provided the best fit to the data. The Ch-PSWQ and its factors evidenced good internal consistency and both convergent and discriminate validity. The present study supports the opinion that the second factor of PSWQ not only contains the component of evaluating pathological worry,but also might represent other traits.展开更多

BACKGROUND The mental status of gynecologic patients has an important influence on their quality of life(QOL).Although high-quality QOL studies into breast cancer patients have been performed internationally,few QOL s...BACKGROUND The mental status of gynecologic patients has an important influence on their quality of life(QOL).Although high-quality QOL studies into breast cancer patients have been performed internationally,few QOL studies have examined patients with gynecological cancer.AIM To investigate the brief cancer-related worry inventory(BCWI)could evaluate the mental status of gynecological patients.METHODS Between July 2018 and December 2018,19 consecutive gynecological cancer patients were prospectively recruited for this study.The BCWI is a 15-item selfreported questionnaire that assesses cancer-related worries on a numeric scale(0–100).RESULTS The high BCWI group was significantly younger than the low BCWI group.Regarding social status,the absence of a spouse and children was significantly more common in the high BCWI group than in the low BCWI group.The operation time was longer in the worsening BCWI group than in the stable BCWI group(305.3±140.5 min vs 171.1±97.2 min;P=0.026).CONCLUSION Being young,having no family,and a long operation time were found to be risk factors for increased anxiety.Therefore,gynecological cancer patients should be assessed using the BCWI,and specific perioperative mental care should be considered for highly anxious patients.展开更多

The connection between the two is discussed in three lessons that Soren Kierkegaard provides in his religious discourses What We Learn from the Lilies in the Field and from the Birds of the Air. The importance of the connection between faith and well-being, especially within Kierkegaard's authorship, derives from his definition of a human being as a synthesis of the infinite and the finite, of the temporal and the eternal, of freedom and necessity. It is important to note that this synthesis is not yet a self, not yet an authentic human being. In order to become a self a deviation is needed, a split within this elementarysynthesis. Only upon achieving such split a new synthesis is reconstructed, the synthesis that amounts to an authentic selfhood. The believer who has stopped worrying over his/her mundane concerns, by shifting its existential gravity towards God, will then have to reconstruct a new synthesis. This new synthesis will resemble the fundamental one in almost every aspect. But the difference would lie in choosing God, in the believer's hidden inwardness. Method: A qualitative content analysis with an inductive approach has been used to analyze fourteen interviews that served as the data base for this study. The content analysis resulted in the development of five categories which evolved into one primary theme. Findings: The five categories identified were Worry and preoccupation;Distance;managing their feelings;Mourning what is lost;Guarded happiness and expectations. These categories were compiled into a main theme, “Worry consumes a lot of energy, but on the other side lies happiness”. This theme focused on whether the women could feel any happiness about being pregnant again despite their concerns with the previous miscarriage. Conclusions: The emotional states of the women when they get pregnant again are typically characterized by anxiety, worry and concerns about their current pregnancy. The women have a tendency to distance themselves emotionally from their pregnancy but also strive to find the joy of being pregnant again. During the new pregnancy they find themselves in need of support from their family and friends as well as in need of support from the healthcare system.展开更多
