This study addresses the critical need for timely GDM detection in high-risk p...Background: Gestational Diabetes Mellitus (GDM) poses significant risks to both mothers and fetuses, with an escalating global prevalence. This study addresses the critical need for timely GDM detection in high-risk pregnancies. By comparing the efficacy of the standard 28-week oral glucose tolerance test with an early 20-week screening, the research aims to enhance preventive interventions and minimise complications, contributing valuable insights for optimal GDM management in high-risk populations. Methodology: Conducted at Teaching Hospital Peradeniya, Sri Lanka, this prospective cohort study investigated early GDM diagnosis using a 20-week OGTT in high-risk pregnancies with negative booking screens. The research involved 385 singleton pregnancies, assessing risk factors like GDM history, family history of diabetes, macrosomia, BMI > 30 kg/m2, polycystic ovary syndrome, and advanced maternal age. The study included evaluating GDM incidence at 20 and 28 weeks, analysing risk factor associations, and determining the efficacy of early OGTT compared to routine testing. The data analysis aimed to establish the significance of a 20-week OGTT, identify the main contributory risk factors, and propose an optimal timing for GDM screening in high-risk pregnancies. Results: In the study involving 385 high-risk pregnant women, the incidence of gestational diabetes mellitus (GDM) was 7.27% at 20 weeks, 10.91% at 28 weeks, and 81.82% without GDM. Significant associations were found between GDM at 20 weeks, a history of GDM (78.57%), and a family history of diabetes (28.57%) (p = 0.011, 0.010 respectively). Notably, the McNemar test revealed no significant association between GDM cases at 20 and 28 weeks. Discussion and Conclusion: This study emphasises early diagnosis of GDM and evaluates outcomes of screening at 20 weeks in high-risk pregnancies. Effective GDM management mitigates short-term complications but raises concern about long-term impacts on offspring. Limited evidence prompts a call for further research to determine the optimal intervention window. Risk factors for early GDM include family history and prior GDM. Recommendations include refining screening protocols and conducting additional randomised trials. The study’s strengths lie in its comprehensive analysis, but limitations include its single-cohort nature. Future research should focus on personalised screening approaches and improve gestational age assessments. Overall, this study contributes to the ongoing discourse on early GDM management, highlighting the need for tailored prenatal care.展开更多
A largely unexplored application of“Big Data”in urban contexts is using human mobility data to study temporal heterogeneity in intercity travel networks.Hence,this paper explores China’s intercity travel patterns a...A largely unexplored application of“Big Data”in urban contexts is using human mobility data to study temporal heterogeneity in intercity travel networks.Hence,this paper explores China’s intercity travel patterns and their dynamics,with a comparison between weekdays and holidays,to contribute to our understanding of these phenomena.Using passenger travel data inferred from Tencent Location Big Data during weekdays(April 11-15,2016)and National Golden Week(October 1-7,2016),we compare the spatial patterns of Chinese intercity travel on weekdays and during Golden Week.The results show that the average daily intercity travel during Golden Week is significantly higher than that during weekdays,but the travel distance and degree of network clustering are significantly lower.This indicates temporal heterogeneity in mapping the intercity travel network.On weekdays,the three major cities of Beijing,Shanghai,and Guangzhou take prominent core positions,while cities that are tourism destinations or transportation hubs are more attractive during Golden Week.The reasons behind these findings can be explained by geographical proximity,administrative division(proximity of cultural and policy systems),travel distance,and travel purposes.展开更多

In this study,we administered a modified schedule of weekly intravenous Bortezomib at 1.6 mg/m 2 with dexamethasone(BD) and compared it to the standard 1.3 mg/m 2 twice-weekly BD regimen in Chinese patients with newly...In this study,we administered a modified schedule of weekly intravenous Bortezomib at 1.6 mg/m 2 with dexamethasone(BD) and compared it to the standard 1.3 mg/m 2 twice-weekly BD regimen in Chinese patients with newly diagnosed multiple myeloma(MM).We assessed the difference in efficacy,safety profile and survival between the once-weekly and twice-weekly cohorts(13 vs.24 patients).The over response rate was similar with both arms of the study,being 77% in the once-weekly schedule and 74.9% in the twice-weekly schedule(P=0.690).The median overall survival was not reached in either schedule.Also,the median progression-free survival and duration of response of the once-weekly schedule did not significantly differ from those of the twice-weekly schedule(8 months vs.10 months,P=0.545 and 6 months vs.7 months,P=0.467 respectively).Peripheral sensory neuropathy and grade 3/4 hematologic toxic effects were more frequently reported in the twice-weekly schedule than the once-weekly schedule,but there was no statistically significant difference.This preliminary experience in Chinese patients with newly diagnosed MM indicated that once-weekly infusion of Bortezomib plus dexamethasone may improve safety without affecting outcome.展开更多

Methods： Scheduled Use versus on demand Reg...Aim： To examine the preference for two dosing regimens of 20 mg of tadalafil, on demand or three times per week, in men affected with erectile dysfunction （ED） in Italy. Methods： Scheduled Use versus on demand Regimen Evaluation （SURE） is a multicenter, crossover and open-label study, involving 94 urology centers in Italy. Patients aged 18 years or older affected with ED for at least 3 months were enrolled and randomized to 20 mg of tadalafil treatment on demand or three times per week for 5-6 weeks. After a 1-week washout, patients were crossed over to the alternate regimen for 5-6 weeks. A treatment preference question was used to determine the preferred treatment regimen. International Index of Erectile Function （IIEF） and Sexual Encounter Profile （SEP） questionnaire were used as efficacy measures. Results： A total of 1 058 men （mean age 54.8 years）, were randomized to treatment. Overall, 59.1% of patients preferred the on-demand regimen and 41.9% preferred the three times per week dosing. Both regimens were efficacious and well tolerated. Although a statistically higher improvement of the IIEF erectile function （IIEF-EF） domain score and the SEP questionnaire was reported for the three times per week compared to the ondemand treatment regimen, this difference was numerically minimal and lacking in clinical significance. Conclusion： Tadalafil is effective and well tolerated whether used on demand or three times per week. Patients should be given the option to choose the best treatment regimen according to personal needs and preferences.展开更多
AIM:To investigate variations in the incidence of peptic ulcers(PUs) in Taiwan by day of the week within age subgroups.METHODS:Ambulatory care data were retrieved from the Longitudinal Health Insurance Database 2000.T...AIM:To investigate variations in the incidence of peptic ulcers(PUs) in Taiwan by day of the week within age subgroups.METHODS:Ambulatory care data were retrieved from the Longitudinal Health Insurance Database 2000.There were 7204 subjects≥18 years-old with an emergency room admission claim for the treatment of PUs,resulting in a total of 9234 emergency room visits for PUs between 2009 and 2011.Data was divided into the seven days of the week and an additional variable for holidays.One-way analysis of variance was used to examine associations among the daily mean number of PU emergency room admissions and holidays/weekends/weekdays.RESULTS:One-way analysis of variance showed that there was a significant difference in emergency room admissions for PUs by the day of the week(P<0.001),with admission more likely to occur on Sundays or holidays than weekdays within the total and working populations.The weekday patterns of admission were similar for the patients aged 18-64 years and≥65years of age.Holidays,followed by Sundays,had higher PU admissions than the mean daily PU emergency room admissions.Furthermore,inclusion of only those treated for PUs with hemorrhage or perforation,Sundays and holidays had higher mean emergency room admissions than other days.Inclusion of patients who diagnosed with Helicobacter pylori infection,only holidays had higher mean emergency room admissions than other days.Inclusion of patients who had been prescribed non-steroidal anti-inflammatory drugs(NSAIDs)for over30 d,Sundays and holidays had higher mean PU ER admissions than other non-holiday weekdays.CONCLUSION:There is a higher incidence of emergency room admission for PUs on weekends than on weekdays for the total and working populations.展开更多
<strong>Introduction: </strong><span style="font-family:""><span style="font-family:Verdana;">Epithelial Ovarian Carcinoma (EOC) comprises the vast majority (almost 90%...<strong>Introduction: </strong><span style="font-family:""><span style="font-family:Verdana;">Epithelial Ovarian Carcinoma (EOC) comprises the vast majority (almost 90%) of ovarian carcinomas. Chemotherapy is the main treatment in ovarian cancers. The standard of care in the chemotherapeutic is the combination of a platinum (carboplatin or cisplatin) and a taxane (paclitaxel or docetaxel). Studies were done to determine whether this combination to be given weekly or every 3 weeks. </span><b><span style="font-family:Verdana;">Patient and Method: </span></b><span style="font-family:Verdana;">Inclusion criteria: 1) Female patients between the ages of 17 - 78 years. 2) Baseline hematological, renal and liver laboratory profiles were within accepted ranges. 3) Performance status of the patients was 0-II. 4) Patients were pathologically proven ovarian cancer. 5) A follow-up period for at least 6 months was required. Exclusion criteria: 1) Patients who had double malignancy were excluded. 2) Performance status more than II. 3) Other comorbidity. </span><b><span style="font-family:Verdana;">Results: </span></b><span style="font-family:Verdana;">We reviewed 69 female patients with EOC, with 60% received every three weeks regimen. Mean age was 53.22 years. At a median follow up of 45.9 months, there was no significant different between the two protocols in terms of mean PFS, 62.35 months (95% CI: 50.08 - 74.63 months) for the three-weekly cohort, and 69.25 months (95% CI: 55.24 - 83.26 months) for weekly protocol (p = 0.613). The three weekly regimen patients had a higher incidence of hospital admission (40% vs 18.5% for the weekly protocol patients), but it didn’t reach a statistical significance (p = 0.063). The three weekly protocol had a significantly higher incidence of causing a neutropenic fever (p = 0.003). </span><b><span style="font-family:Verdana;">Conclusion: </span></b><span style="font-family:Verdana;">In our cohort of Egyptian women with EOC, no significant difference in PFS was found when compared the weekly Carboplatin/paclitaxel when compared to the classic three weeks, although the weekly protocol may be causing less febrile neutropenia and fewer hospital admissions.</span></span>展开更多

Results： A total of 1 058 men （mean age 54.8 years）, were randomized to treatment. Overall, 59.1% of patients preferred the on-demand regimen and 41.9% preferred the three times per week dosing. Both regimens were efficacious and well tolerated. Although a statistically higher improvement of the IIEF erectile function （IIEF-EF） domain score and the SEP questionnaire was reported for the three times per week compared to the ondemand treatment regimen, this difference was numerically minimal and lacking in clinical significance. Conclusion： Tadalafil is effective and well tolerated whether used on demand or three times per week. Patients should be given the option to choose the best treatment regimen according to personal needs and preferences.
文摘AIM:To investigate variations in the incidence of peptic ulcers(PUs) in Taiwan by day of the week within age subgroups.METHODS:Ambulatory care data were retrieved from the Longitudinal Health Insurance Database 2000.There were 7204 subjects≥18 years-old with an emergency room admission claim for the treatment of PUs,resulting in a total of 9234 emergency room visits for PUs between 2009 and 2011.Data was divided into the seven days of the week and an additional variable for holidays.One-way analysis of variance was used to examine associations among the daily mean number of PU emergency room admissions and holidays/weekends/weekdays.RESULTS:One-way analysis of variance showed that there was a significant difference in emergency room admissions for PUs by the day of the week(P<0.001),with admission more likely to occur on Sundays or holidays than weekdays within the total and working populations.The weekday patterns of admission were similar for the patients aged 18-64 years and≥65years of age.Holidays,followed by Sundays,had higher PU admissions than the mean daily PU emergency room admissions.Furthermore,inclusion of only those treated for PUs with hemorrhage or perforation,Sundays and holidays had higher mean emergency room admissions than other days.Inclusion of patients who diagnosed with Helicobacter pylori infection,only holidays had higher mean emergency room admissions than other days.Inclusion of patients who had been prescribed non-steroidal anti-inflammatory drugs(NSAIDs)for over30 d,Sundays and holidays had higher mean PU ER admissions than other non-holiday weekdays.CONCLUSION:There is a higher incidence of emergency room admission for PUs on weekends than on weekdays for the total and working populations.
文摘<strong>Introduction: </strong><span style="font-family:""><span style="font-family:Verdana;">Epithelial Ovarian Carcinoma (EOC) comprises the vast majority (almost 90%) of ovarian carcinomas. Chemotherapy is the main treatment in ovarian cancers. The standard of care in the chemotherapeutic is the combination of a platinum (carboplatin or cisplatin) and a taxane (paclitaxel or docetaxel). Studies were done to determine whether this combination to be given weekly or every 3 weeks. </span><b><span style="font-family:Verdana;">Patient and Method: </span></b><span style="font-family:Verdana;">Inclusion criteria: 1) Female patients between the ages of 17 - 78 years. 2) Baseline hematological, renal and liver laboratory profiles were within accepted ranges. 3) Performance status of the patients was 0-II. 4) Patients were pathologically proven ovarian cancer. 5) A follow-up period for at least 6 months was required. Exclusion criteria: 1) Patients who had double malignancy were excluded. 2) Performance status more than II. 3) Other comorbidity. </span><b><span style="font-family:Verdana;">Results: </span></b><span style="font-family:Verdana;">We reviewed 69 female patients with EOC, with 60% received every three weeks regimen. Mean age was 53.22 years. At a median follow up of 45.9 months, there was no significant different between the two protocols in terms of mean PFS, 62.35 months (95% CI: 50.08 - 74.63 months) for the three-weekly cohort, and 69.25 months (95% CI: 55.24 - 83.26 months) for weekly protocol (p = 0.613). The three weekly regimen patients had a higher incidence of hospital admission (40% vs 18.5% for the weekly protocol patients), but it didn’t reach a statistical significance (p = 0.063). The three weekly protocol had a significantly higher incidence of causing a neutropenic fever (p = 0.003). </span><b><span style="font-family:Verdana;">Conclusion: </span></b><span style="font-family:Verdana;">In our cohort of Egyptian women with EOC, no significant difference in PFS was found when compared the weekly Carboplatin/paclitaxel when compared to the classic three weeks, although the weekly protocol may be causing less febrile neutropenia and fewer hospital admissions.</span></span>
