Diabetes is highly prevalent among the elderly worldwide,with the highest number of diabetes cases in China.Yet,the management of diabetes remains unsatisfactory.Recent advances in digital health technologies have fac...Diabetes is highly prevalent among the elderly worldwide,with the highest number of diabetes cases in China.Yet,the management of diabetes remains unsatisfactory.Recent advances in digital health technologies have facilitated the establishment of smart wards for diabetes patients.There is a lack of smart wards tailored specifically for older diabetes patients who encounter unique challenges in glycemic control and diabetes management,including an increased vulnerability to hypoglycemia,the presence of multiple chronic diseases,and cognitive decline.In this review,studies on digital health technologies for diabetes in China and beyond were summarized to elucidate how the adoption of digital health technologies,such as real-time continuous glucose monitoring,sensor-augmented pump technology,and their integration with 5th generation networks,big data cloud storage,and hospital information systems,can address issues specifically related to elderly diabetes patients in hospital wards.Furthermore,the challenges and future directions for establishing and implementing smart wards for elderly diabetes patients are discussed,and these challenges may also be applicable to other countries worldwide,not just in China.Taken together,the smart wards may enhance clinical outcomes,address specific issues,and eventually improve patient-centered hospital care for elderly patients with diabetes.展开更多

Objective:To explore the effect of applying safety culture construction in nursing management of infectious disease areas.Methods:During the one-year period from January 2024 to December 2024,patients admitted to the ...Objective:To explore the effect of applying safety culture construction in nursing management of infectious disease areas.Methods:During the one-year period from January 2024 to December 2024,patients admitted to the infectious disease area of our hospital were randomly selected for the study.92 patients were divided into two groups using a computerized double-blind method for intervention.The control group received routine nursing management,while the observation group applied safety culture construction management.The effects of nursing management in the two groups were studied and compared.Results:Before management,the nursing management quality scores of the two groups were evaluated,showing little difference in various scores,and the difference was not statistically significant(P>0.05).Through the implementation of nursing management,the scores of the two groups were significantly improved,and the observation group had significantly higher scores compared to the control group(P<0.05).Compared to the incidence of nursing risk events,the observation group(6.52%)was significantly lower than the control group(30.43%),and there was a significant difference between the two groups(P<0.05).The observation group had significantly higher satisfaction scores for nursing skills,health education,service attitude,and professionalism compared to the control group,indicating patient satisfaction with safety culture construction(P<0.05).Conclusion:Applying safety culture construction in nursing management of infectious disease areas has a preventive effect on nursing risk events and is beneficial for improving patients’risk cognition level.展开更多

Hospitals are one of the sensitive areas of our society, for they are built as sanctuarie...Air quality is a critical factor in maintaining health and well-being, influencing both current conditions and future outcomes. Hospitals are one of the sensitive areas of our society, for they are built as sanctuaries for treatment and recovery, making the quality of paramount importance. This study investigates the impact of traffic-related emissions on indoor air quality within a Level 5 Hospital outpatient ward. Measurements were taken over five consecutive days, revealing that while CO2 levels generally remained within safe limits, there were instances where concentrations exceeded 3000 ppm, categorizing them as “Hazardous.” Notably, particulate matter (PM2.5 and PM10) levels fluctuated significantly, with peak concentrations observed during working hours correlating with increased vehicle activity. The data indicated that PM2.5 levels reached as high as 75 µg/m3, with 91.68% of recorded values exceeding the World Health Organization’s (WHO) and Environmental Protection Agency 24-hour mean threshold of 25 µg/m3. Similarly, PM10 concentrations peaked at 120 µg/m3, with 61.19% of values surpassing the WHO threshold of 50 µg/m3, both of which pose serious health risks, particularly to vulnerable populations such as pregnant women, infants, and the elderly. Additionally, the study highlighted the critical role of wind direction in pollutant dispersion, with specific patterns contributing to elevated indoor concentrations. These findings underscore the urgent need for targeted interventions and proactive air quality management strategies in healthcare facilities, including the strategic design of hospital wards away from primary emission sources and the promotion of electric vehicle use to mitigate traffic-related emissions.展开更多

BACKGROUND Restrictive practices(RPs)are defined by measures linked to physical and chemical restraints to reduce the movement or control behaviours during any emergency.Seclusion is an equal part of RPs intended to i...BACKGROUND Restrictive practices(RPs)are defined by measures linked to physical and chemical restraints to reduce the movement or control behaviours during any emergency.Seclusion is an equal part of RPs intended to isolate and reduce the sensory stimulation to safeguard the patient and those within the vicinity.Using interventions by way of virtual reality(VR)could assist with reducing the need for RPs as it could help reduce anxiety or agitation by way of placing users into realistic and immersive environments.This could also aid staff to and change current RPs.AIM To assess the feasibility and effectiveness of using a VR platform to provide reduction in RP training.METHODS A randomised controlled feasibility study,accompanied by evaluations at 1 month and 6 months,was conducted within inpatient psychiatric wards at Southern Health National Health Service Foundation Trust,United Kingdom.Virti VR scenarios were used on VR headsets to provide training on reducing RPs in 3 inpatient psychiatric wards.Outcome measures included general self-efficacy scale,generalised anxiety disorder assessment 7(GAD-7),Burnout Assessment Tool 12,the Everyday Discrimination Scale,and the Compassionate Engagement and Action Scale.RESULTS Findings revealed statistically significant differences between the VR and treatment as usual groups,in the Everyday Discrimination Scale items Q8 and Q9:P=0.023 and P=0.040 respectively,indicating higher levels of perceived discrimination in the VR group.There were no significant differences between groups in terms of general self-efficacy,generalised anxiety disorder assessment 9,and Burnout Assessment Tool 12 scores.A significant difference was observed within the VR group for compassionate engagement from others(P=0.005)over time.Most respondents recorded System Usability Scale scores above 70,with an average score of 71.79.There was a significant reduction in rates of RPs in the VR group vs treatment as usual group with a fluctuating variability observed in the VR group likely due to external factors not captured in the study.CONCLUSION Ongoing advancement of VR technology enables the possibility of creating scenarios and simulations tailored to healthcare environments that empower staff by providing more comprehensive and effective training for handling situations.展开更多

Along with the development of 5G network and Internet of Things technologies,there has been an explosion in personalized healthcare systems.When the 5G and Artificial Intelligence(Al)is introduced into diabetes manage...Along with the development of 5G network and Internet of Things technologies,there has been an explosion in personalized healthcare systems.When the 5G and Artificial Intelligence(Al)is introduced into diabetes management architecture,it can increase the efficiency of existing systems and complications of diabetes can be handled more effectively by taking advantage of 5G.In this article,we propose a 5G-based Artificial Intelligence Diabetes Management architecture(AIDM),which can help physicians and patients to manage both acute complications and chronic complications.The AIDM contains five layers:the sensing layer,the transmission layer,the storage layer,the computing layer,and the application layer.We build a test bed for the transmission and application layers.Specifically,we apply a delay-aware RA optimization based on a double-queue model to improve access efficiency in smart hospital wards in the transmission layer.In application layer,we build a prediction model using a deep forest algorithm.Results on real-world data show that our AIDM can enhance the efficiency of diabetes management and improve the screening rate of diabetes as well.展开更多

Only 10% of newborns will need birth assistance. Successful resuscitation is linked to the skills of the health worker. It is to assess their skills that this work was undertaken with the objective of evaluating the practice of neonatal resuscitation in the delivery room of the RHC maternity hospital in District V of Bamako. Methodology: This was a descriptive cross-sectional study over a four-month period. We included in the study all live newborns who had a gestational age greater than or equal to 32 weeks and who had an Apgar score at the first minute of less than 7. At each birth, we observe the health agent responsible for the care of the newborn by observing the preparation of resuscitation and compliance with the neonatal resuscitation algorithm. We have excluded all newborns who met our inclusion criteria, were reanimated outside of our collection time and had visible or diagnosed anomalies or malformations in the prenatal period, and those whose parents refused to give their consent to participate in the study. Data were collected from the survey sheet and analyzed with the Statistical Package for Social Sciences (SPSS) software version 25. Results: We observed a 24.66% frequency of neonatal resuscitation. Pregnancies were too close in 15% (less than one year). Caesarean section delivery represented 34% of the sample. They were at term in 93% of cases. All newborns were well dried (98.5%), with wet linen change only at 49.5%. Apgar was less than 3 in 7.5% of newborns at first. The resuscitation needs were for the absence of a scream in 78.5% of cases and or heart rate Conclusion: The study evaluated neonatal resuscitation practices at a maternity hospital in Bamako, Mali. It found a 24.66% resuscitation rate, with a 95.5% success rate despite technical limitations. Most steps were correctly applied, though some improvements are needed in areas like preventing hypothermia and equipment preparation.展开更多

Methods: A total of 300 nurses working in community comprehensive care wards and 360 care managers working in B City in A Prefecture were surveyed using an anonymous self-administered questionnaire. Results: The highest percentage of responses regarding necessary collaborative activities with multiple professions in supporting hospital discharge were the same for Ns and CMs. The items regarding practice with the highest percentages were “relationship as a team” for Ns, and “user-centered awareness” for CM. While these professionals were willing to share information about their patients’ lives after discharge, the percentage of those explaining their expertise was low. It is thought that collaborative activities focusing on these aspects would lead to more appropriate discharge support.展开更多

文摘This manuscript critically evaluates the randomized controlled trial(RCT)conducted by Phiri et al,which assessed the effectiveness of virtual reality(VR)training for psychiatric staff in reducing restrictive practices(RPs).Specifically,this RCT investigated the impact of VR on the handling of aggressive patients by psychiatric staff compared to traditional training methods.Despite significant reductions in perceived discrimination in the VR group,there were no major improvements in self-efficacy or anxiety levels.The system usability scale rated the VR platform highly,but it did not consistently outperform traditional training methods.Indeed,the study shows the potential for VR to reduce RPs,although fluctuations in RP rates suggest that external factors,such as staff turnover,influenced the outcomes.This manuscript evaluates the study’s methodology,results,and broader implications for mental health training.Additionally,it highlights the need for more comprehensive research to establish VR as a standard tool for psychiatric staff education,focusing on patient care outcomes and real-world applicability.Finally,this study explores future research di-rections,technological improvements,and the potential impact of policies that could enhance the integration of VR in clinical training.

文摘BACKGROUND Restrictive practices(RPs)are defined by measures linked to physical and chemical restraints to reduce the movement or control behaviours during any emergency.Seclusion is an equal part of RPs intended to isolate and reduce the sensory stimulation to safeguard the patient and those within the vicinity.Using interventions by way of virtual reality(VR)could assist with reducing the need for RPs as it could help reduce anxiety or agitation by way of placing users into realistic and immersive environments.This could also aid staff to and change current RPs.AIM To assess the feasibility and effectiveness of using a VR platform to provide reduction in RP training.METHODS A randomised controlled feasibility study,accompanied by evaluations at 1 month and 6 months,was conducted within inpatient psychiatric wards at Southern Health National Health Service Foundation Trust,United Kingdom.Virti VR scenarios were used on VR headsets to provide training on reducing RPs in 3 inpatient psychiatric wards.Outcome measures included general self-efficacy scale,generalised anxiety disorder assessment 7(GAD-7),Burnout Assessment Tool 12,the Everyday Discrimination Scale,and the Compassionate Engagement and Action Scale.RESULTS Findings revealed statistically significant differences between the VR and treatment as usual groups,in the Everyday Discrimination Scale items Q8 and Q9:P=0.023 and P=0.040 respectively,indicating higher levels of perceived discrimination in the VR group.There were no significant differences between groups in terms of general self-efficacy,generalised anxiety disorder assessment 9,and Burnout Assessment Tool 12 scores.A significant difference was observed within the VR group for compassionate engagement from others(P=0.005)over time.Most respondents recorded System Usability Scale scores above 70,with an average score of 71.79.There was a significant reduction in rates of RPs in the VR group vs treatment as usual group with a fluctuating variability observed in the VR group likely due to external factors not captured in the study.CONCLUSION Ongoing advancement of VR technology enables the possibility of creating scenarios and simulations tailored to healthcare environments that empower staff by providing more comprehensive and effective training for handling situations.

文摘The haematopoietic stem cell transplantation(HSCT)ward serves as a temporary residence for patients following their surgical procedures,necessitating adherence to rigorous aseptic standards.However,the current atmosphere within these wards frequently contributes to feelings of depression among patients.Research indicates that a restorative environment has the potential to alleviate negative emotional states in individuals.This study utilized the HSCT ward of Peking University First Hospital as a case study to examine the decorative preferences.This investigation was conducted through a questionnaire that was informed by the patients’inherent preferences and insights derived from research on restorative environments.The results indicated that the incorporation of floral decorations,particularly those resembling sunflowers,in ward corridors,communal activity areas,and walls can significantly enhance patients’sense of hope.Additionally,it is essential to improve the environmental visual experience in the nurses’lounge and demonstration rooms for medical staff.

文摘Along with the development of 5G network and Internet of Things technologies,there has been an explosion in personalized healthcare systems.When the 5G and Artificial Intelligence(Al)is introduced into diabetes management architecture,it can increase the efficiency of existing systems and complications of diabetes can be handled more effectively by taking advantage of 5G.In this article,we propose a 5G-based Artificial Intelligence Diabetes Management architecture(AIDM),which can help physicians and patients to manage both acute complications and chronic complications.The AIDM contains five layers:the sensing layer,the transmission layer,the storage layer,the computing layer,and the application layer.We build a test bed for the transmission and application layers.Specifically,we apply a delay-aware RA optimization based on a double-queue model to improve access efficiency in smart hospital wards in the transmission layer.In application layer,we build a prediction model using a deep forest algorithm.Results on real-world data show that our AIDM can enhance the efficiency of diabetes management and improve the screening rate of diabetes as well.
文摘Scalar-tensor theories of gravity are considered to be competitors to Einstein's theory of general relativity for the description of classical gravity,as they are used to build feasible models for cosmic inflation.These theories can be formulated both in the Jordan and Einstein frame,which are related by a Weyl transformation with a field transformation,known together as a frame transformation.These theories formulated in the above two frames are often considered to be equivalent from the point of view of classical theory.However,this is no longer true from the quantum field theoretical perspective.In the present article,we show that the Ward identities derived in the above two frames are not connected through the frame transformation.This shows that the quantum field theories formulated in these two frames are not equivalent to each other.Moreover,this inequivalence is also shown by comparing the effective actions derived in these two frames.

文摘This editorial examines the application of virtual reality(VR)training to mitigate restrictive practices(RPs)within psychiatric facilities.RPs include physical restraints,seclusion,and chemical restraints,used to ensure patient safety but with varying usage rates across regions.In recent years,there has been a growing focus on the adverse effects of RPs on both healthcare workers and patients,leading to calls for its reduction.Previous research has shown the efficiency of VR training in RP reduction.This editorial will analyze the limitations of VR training in prior research aimed at reducing RP,emphasizing that the essence of RPs is a medical safety issue,calling for careful differentiation of the causes of RPs,and avoiding the use of AR technology as a"new bottle"for"old wine"to improve the quality and reproducibility of future research in this field.

文摘Through the collection and systematic analysis of documents related to window views of hospitals,it is found that natural window views had a significant impact on patients’health benefits.The research focused on three aspects:“shortening length of stay”,“pain reduction”and“improvement of recovery rate”,mainly covering three types of patients:“heart patients”,“postoperative patients”and“patients of rehabilitation centers”.Based on the above analysis,summary and sorting,new directions and perspectives of hospital environment design and research under the concept of comprehensive health of people’s physiological,psychological and social adaptation will be obtained to provide references for the research and practice of health architecture,rehabilitation architecture,biophile design and other fields.
文摘Birth is the transition from fetal life to ectopic life. This transition is usually smooth. Only 10% of newborns will need birth assistance. Successful resuscitation is linked to the skills of the health worker. It is to assess their skills that this work was undertaken with the objective of evaluating the practice of neonatal resuscitation in the delivery room of the RHC maternity hospital in District V of Bamako. Methodology: This was a descriptive cross-sectional study over a four-month period. We included in the study all live newborns who had a gestational age greater than or equal to 32 weeks and who had an Apgar score at the first minute of less than 7. At each birth, we observe the health agent responsible for the care of the newborn by observing the preparation of resuscitation and compliance with the neonatal resuscitation algorithm. We have excluded all newborns who met our inclusion criteria, were reanimated outside of our collection time and had visible or diagnosed anomalies or malformations in the prenatal period, and those whose parents refused to give their consent to participate in the study. Data were collected from the survey sheet and analyzed with the Statistical Package for Social Sciences (SPSS) software version 25. Results: We observed a 24.66% frequency of neonatal resuscitation. Pregnancies were too close in 15% (less than one year). Caesarean section delivery represented 34% of the sample. They were at term in 93% of cases. All newborns were well dried (98.5%), with wet linen change only at 49.5%. Apgar was less than 3 in 7.5% of newborns at first. The resuscitation needs were for the absence of a scream in 78.5% of cases and or heart rate Conclusion: The study evaluated neonatal resuscitation practices at a maternity hospital in Bamako, Mali. It found a 24.66% resuscitation rate, with a 95.5% success rate despite technical limitations. Most steps were correctly applied, though some improvements are needed in areas like preventing hypothermia and equipment preparation.
文摘Objective: The purpose of this study was to clarify the collaborative activities and mutual recognition between community comprehensive care unit nurses (Ns) and care managers (CM) in supporting the discharge of the elderly from the hospital. Methods: A total of 300 nurses working in community comprehensive care wards and 360 care managers working in B City in A Prefecture were surveyed using an anonymous self-administered questionnaire. Results: The highest percentage of responses regarding necessary collaborative activities with multiple professions in supporting hospital discharge were the same for Ns and CMs. The items regarding practice with the highest percentages were “relationship as a team” for Ns, and “user-centered awareness” for CM. While these professionals were willing to share information about their patients’ lives after discharge, the percentage of those explaining their expertise was low. It is thought that collaborative activities focusing on these aspects would lead to more appropriate discharge support.
